Aims To establish recruitment approaches that leverage electronic health records in multicenter prediabetes/diabetes clinical trials and compare recruitment outcomes between electronic health record–supported and conventional recruitment methods. Methods Observational analysis of recruitment approaches in the vitamin D and type 2 diabetes (D2d) study, a multicenter trial in participants with prediabetes. Outcomes were adoption of electronic health record–supported recruitment approaches by sites, number of participants screened, recruitment performance (proportion screened who were randomized), and characteristics of participants from electronic health record–supported versus non–electronic health record methods. Results In total, 2423 participants were randomized: 1920 from electronic health record (mean age of 60 years, 41% women, 68% White) and 503 from non–electronic health record sources (mean age of 56.9 years, 58% women, 61% White). Electronic health record–supported recruitment was adopted by 21 of 22 sites. Electronic health record–supported recruitment was associated with more participants screened versus non–electronic health record methods (4969 vs 2166 participants screened), higher performance (38.6% vs 22.7%), and more randomizations (1918 vs 505). Participants recruited via electronic health record were older, included fewer women and minorities, and reported higher use of dietary supplements. Electronic health record–supported recruitment was incorporated in diverse clinical environments, engaging clinicians either at the individual or the healthcare system level. Conclusion Establishing electronic health record–supported recruitment approaches across a multicenter prediabetes/diabetes trial is feasible and can be adopted by diverse clinical environments.

Intelligence relies on our ability to find appropriate sequences of decisions in complex problem spaces. The efficiency of a problem solver depends on the speed of its individual decisions and the number of decisions it can explore in parallel. It remains unknown whether the primate brain can consider multiple decisions at the same time. We therefore trained monkeys to navigate through a decision tree with stochastic sensory evidence at multiple branching points and recorded neuronal activity in visual cortical areas V1 and V4. We found a first phase of decision making in which neuronal activity increased in parallel along multiple branches of the decision tree. This was followed by an integration phase where the optimal overall strategy crystallized as the result of interactions between local decisions. The results reveal how sensory evidence is integrated efficiently for hierarchical decisions and contribute to our understanding of the brain mechanisms that implement complex mental programs. Lorteije et al. found behavioral and physiological evidence that the primate brain can process multiple decisions in parallel. Besides decreasing overall processing time, parallel decisions can influence each other and, thereby, maximize the reward income.

Background Glycoprotein NMB (gpNMB) is an internalizable transmembrane protein overexpressed in approximately 20% of breast cancer (BC), including approximately 40% of TNBC. gpNMB is a poor prognostic marker in BC (Rose CCR 2010) and preclinically has been implicated in tumor invasion, metastasis, and angiogenesis. GV is a novel antibody-drug conjugate targeting the potent cytotoxin monomethylauristatin E (MMAE) to gpNMB overexpressing cancer cells. In a Phase I/II study and the Phase II "EMERGE" study, GV demonstrated promising activity with TNBC patients (pts) deriving the greatest benefit and exhibiting the highest degree of gpNMB overexpression. GV was well-tolerated with the most frequent treatment-related toxicities consisting of rash, neutropenia, and neuropathy. In subset analyses of the EMERGE trial, objective response rate (ORR) was 30% (7/23) for GV vs. 9% (1/11) for investigator's choice in tumors with gpNMB overexpression (>25% of tumor epithelium); 18% (5/28) vs. 0% (0/11) in TNBC; and 40% (4/10) vs. 0% (0/6) in gpNMB-overexpressing TNBC for GV and IC respectively, with apparent improvements in progression-free survival (PFS; hazard ratio (HR) = 0.11) and overall survival (OS; HR = 0.14). Trial design The METRIC Trial (NCT#01997333) is an international (USA, CA, Aus), two-arm phase II study. Pts are randomized 2:1 to GV (1.88 mg/kg IV q 21 days) or capecitabine, a current standard of care for this population (2,500 mg/m2 daily for d1-14, q21 days) until progression or intolerance. Crossover is not permitted. Eligibility criteria Key eligibility criteria include: >25% of tumor epithelium gpNMB+ by central immunohistochemistry (IHC) screening of archival tissue; estrogen receptor and progesterone receptor 2 toxicity. Specific aims The primary endpoint is PFS per independent, blinded central review committee according to RECIST 1.1. Secondary endpoints are ORR, duration of response, OS, safety, pharmacokinetics and pharmacodynamics. Exploratory endpoints are quality of life and/or cancer-related pain. Statistical methods and target accrual The trial has 85% power to detect a PFS HR of 0.64 with two sided α = 0.05. The hypothesized median PFS is 4.0 months for capecitabine and 6.25 months for GV. Target accrual is open for 300 pts. The study of decision-making has mainly focused on isolated decisions where choices are associated with motor actions. However, problem-solving often involves considering a hierarchy of sub-decisions. In a recent study (Lorteije et al. 2015), we reported behavioral and neuronal evidence for hierarchical decision making in a task with a small decision tree. We observed a first phase of parallel evidence integration for multiple sub-decisions, followed by a phase in which the overall strategy formed. It has been suggested that a 'flat' competition between the ultimate motor actions might also explain these results. A reanalysis of the data does not support the critical predictions of flat models. We also examined the time-course of decision making in other, related tasks and report conditions where evidence integration for successive decisions is decoupled, which excludes flat models. We conclude that the flexibility of decision-making implies that the strategies are genuinely hierarchical. Both options require you to make additional decisions, for example, which café would you go to, or what movie could you see? Many of the decisions we make in our daily lives feature a number of sub-decisions. Many of our day-to-day decisions have multiple layers of sub-decisions embedded within them that are not necessarily independent. Our opinions of the cafés in town and the movies showing at the theater may influence our decision over whom to spend the afternoon with. In 2015, researchers at the Netherlands Institute for Neuroscience performed experiments in macaques to try to work out how the brain makes these decisions. The monkeys learned to choose between two visual stimuli (decision 1). The outcome of decision 1 determined whether the animals then had to make decision 2 or decision 3. The results suggested that the monkeys initially made all three comparisons independently and in parallel, before combining the evidence to select their overall strategy. This process is referred to as hierarchical decision-making. In the original analogy, one would compare the relative merits of Jules versus Jim, café A versus café B, and a horror movie versus a comedy at the same time before deciding what to do. Other researchers have now reanalyzed the data from the original work using new computer simulations. This second analysis suggests that the results are more consistent with an alternative model of decision-making called a flat model, in which the brain compares all of the final options simultaneously (Jules + café A; Jules + café B; Jim + horror movie; Jim + comedy) before making a decision. In response to these findings, Zylberberg et al. – who conducted the work reported in 2015 – reanalyzed the original data and re-ran the simulations. Zylberberg et al. argue that the flat model provides a poor fit to the original data. A new experiment with human volunteers suggests that modifying the task by adding even more decisions can lead to the different comparisons being made one after the other (in series) rather than all at the same time (in parallel), before the decision is made. This is difficult to explain with a flat model. Zylberberg et al. argue that these findings confirm the original conclusion that the monkeys use a hierarchical strategy. Moreover, the new results expose a previously unknown limit in the number of decisions that the brain can evaluate at any one time. If this limit is exceeded, decision-making becomes serial. Future studies can build on these findings by further exploring the limits of parallel decision-making, which may help us to understand how the brain is able to make multiple decisions while keeping the future consequences in mind. Plasmonic effects have proven to be very efficient in coupling light to structures much smaller than its wavelength. Efficient coupling is particularly important for the infrared or terahertz (λ ∼ 0.3 mm) region where semiconductor structures and devices may be orders of magnitude smaller than the wavelength and this can be achieved through nanostructures that have a desired plasmonic response. We report and demonstrate a self-assembly method of embedding controllable semimetallic nanostructures in a semiconducting matrix in a ErSb/GaSb material system grown by molecular beam epitaxy. The plasmonic properties of the ErSb/GaSb are characterized and quantified by three polarization-resolved spectroscopy techniques, spanning more than 3 orders of magnitude in frequency from 100 GHz up to 300 THz. Surface plasmons cause the semimetallic nanostructures to resonate near 100 THz (3 μm wavelength), indicating the semimetal as a potential infrared plasmonic material. The highly conductive ErSb nanowires polarize electromagnetic radiation in a broad range from 0.2 up to ∼100 THz, providing a new platform for electromagnetics in the infrared and terahertz frequency ranges.

Metal-insulator transition abstract Epitaxial single crystal films of praseodymium nickelate (PrNiO3) have been grown by molecular beam epitaxy on LaAlO3(001) substrates. In-situ electron diffraction and ex-situ X-ray diffraction techniques confirm an epitaxial relationship to the underlying substrate. Crystalline quality depends strongly on the substrate temperature and activated oxygen flux during growth with the highest quality films formed at 600 1C under high oxygen plasma fluxes. The metal-insulator transition for PrNiO3 films was suppressed as the crystalline quality of the layers was improved.

In spite of its massively parallel architecture [1], the human brain is fundamentally limited if required to perform two tasks at the same time [2, 3]. This limitation can be studied with the psychological refractory period (PRP) paradigm, where two stimuli that require speeded responses occur in close succession [4]. Interference generally takes the form of a delay in the time to respond to the second stimulus [5]. Previous studies suggested that sensory decisions require the accumulation of sensory evidence [6, 7] and that the PRP reflects the inability to form more than one decision at a time [4, 8]. In the present study, we used a psychophysical reverse-correlation technique [9, 10] to measure the time-course of evidence accumulation during the PRP. We found that the accumulation of evidence could occur during the PRP albeit with a reduced efficiency, which implies that multiple decision processes can occur in parallel in the human brain. In addition to the reduced efficiency of evidence accumulation, our results uncover an additional delay in the routing of the decision to motor structures during the PRP, which implies that the process of sensory decision making is separable from the preparation of a motor response [11-13]. © 2012 Elsevier Ltd. The paper reports on the thin film growth of a protoptype Mott insulator, ferrimagnetic GdTiO 3 , using shuttered molecular beam epitaxy. Substrates were (001) (LaAlO 3 ) 0.3 (Sr 2 AlTaO 6 ) 0.7 (LSAT), with and without epitaxial SrTiO 3 buffer layers, respectively. It was found that on bare LSAT, the starting monolayer was crucial for stabilizing the GdTiO 3 perovskite phase. The quality of the films was evaluated using structural, electric, optical and magnetic characterization. Structural characterization showed that the GdTiO 3 layers were free of pyrochlore impurity phases and that the lattice parameter was close to what was expected for coherently strained, stoichiometric GdTiO 3 . The room temperature film resistivity was 7 Ωcm and increased with decreasing temperature, consistent with Mott insulating characteristics. The Curie temperature was 30 K and a small coercivity was observed at 2 K, in good agreement with bulk GdTiO 3 properties reported in the literature.

In the cat, the analysis of visual motion cues has generally been attributed to the posteromedial lateral suprasylvian cortex (PMLS) (Toyama et al., 1985; Rauschecker et al., 1987; Rauschecker, 1988; Kim et al., 1997). The responses of neurons in this area are not critically dependent on inputs from the primary visual cortex (VC), as lesions of VC leave neuronal response properties in PMLS relatively unchanged (Spear & Baumann, 1979; Spear, 1988; Guido et al., 1990b). However, previous studies have used a limited range of visual stimuli. In this study, we assessed whether neurons in PMLS cortex remained direction-selective to complex motion stimuli following a lesion of VC, particularly to complex random dot kinematograms (RDKs). Unilateral aspiration of VC was performed on post-natal days 7–9. Single unit extracellular recordings were performed one year later in the ipsilateral PMLS cortex. As in previous studies, a reduction in the percentage of direction selective neurons was observed with drifting sinewave gratings. We report a previously unobserved phenomenon with sinewave gratings, in which there is a greater modulation of firing rate at the temporal frequency of the stimulus in animals with a lesion of VC, suggesting an increased segregation of ON and OFF sub-regions. A significant portion of neurons in PMLS cortex were direction selective to simple (16/18) and complex (11/16) RDKs. However, the strength of direction selectivity to both stimuli was reduced as compared to normals. The data suggest that complex motion processing is still present, albeit reduced, in PMLS cortex despite the removal of VC input. The complex RDK motion selectivity is consistent with both geniculo-cortical and extra-geniculate thalamo-cortical pathways in residual direction encoding.

We report on self-assembled ErSb nanowires in a GaSb matrix that show a strong polarization-sensitive THz response. The nanowires behave like a polarizer. Their orientation and shape can be engineered by the growth conditions.

Heterointerfaces in complex oxide systems open new arenas in which to test models of strongly correlated material, explore the role of dimensionality in metal-insulator-transitions (MITs) and small polaron formation. Close to the quantum critical point Mott MITs depend on band filling controlled by random disordered substitutional doping. Delta-doped Mott insulators are potentially free of random disorder and introduce a new arena in which to explore the effect of electron correlations and dimensionality. Epitaxial films of the prototypical Mott insulator GdTiO3 are delta-doped by substituting a single (GdO)+1 plane with a monolayer of charge neutral SrO to produce a two-dimensional system with high planar doping density. Unlike metallic SrTiO3 quantum wells in GdTiO3 the single SrO delta-doped layer exhibits thermally activated DC and optical conductivity that agree in a quantitative manner with predictions of small polaron transport but with an extremely high two-dimensional density of polarons, ~7 × 1014 cm−2.

Magnetostatic spin-wave modes were excited and detected in micron size ferromagnetic Co90Ta5 Zr5 rectangular tubes that wrapped around the shorted ends of coplanar waveguides. The observed modes can be assigned to surface spin waves of an infinite ferromagnetic film but with periodic boundary conditions and quantized wave vector imposed by the finite circumference of the tube.

A two-dimensional electron gas (2DEG) in SrTiO3 is created via modulation doping by interfacing undoped SrTiO3 with a wider-band-gap material, SrTi1-xZrxO3, that is doped n-type with La. All layers are grown using hybrid molecular beam epitaxy. Using magnetoresistance measurements, we show that electrons are transferred into the SrTiO3, and a 2DEG is formed. In particular, Shubnikov-de Haas oscillations are shown to depend only on the perpendicular magnetic field. The Shubnikov--de Haas effect is used to explore the conduction-band edge of high mobility SrTiO${}_{3}$ films doped with La. The results largely confirm the earlier measurements by Uwe et al. [Jpn. J. Appl. Phys. 24 (Suppl. 24-2), 335 (1985)]. The band edge dispersion differs significantly from the predictions of ab initio electronic structure theory.

Heterostructures and superlattices consisting of a prototype Mott insulator, GdTiO 3 , and the band insulator SrTiO 3 , are grown by molecular beam epitaxy and show intrinsic electronic reconstruction, approximately ½ electron per surface unit cell at each GdTiO 3 /SrTiO 3 interface. Insights into charge distribution, the influence of the electrostatic boundary conditions, and strong correlation effects will be presented.

We report on the magnetotransport properties of a prototype Mott insulator/band insulator perovskite heterojunction in magnetic fields up to 31 T and at temperatures between 360 mK and 10 K. Shubnikov-de Haas oscillations in the magnetoresistance are observed. The oscillations are two-dimensional in nature and are interpreted as arising from either a single, spin-split subband or two subbands. In either case, the electron system that gives rise to the oscillations represents only a fraction of the electrons in the space charge layer at the interface. The temperature dependence of the oscillations are used to extract an effective mass of ~ 1 me for the subband(s). The results are discussed in the context of the t2g-states that form the bottom of the conduction band of SrTiO3., The following article has been accepted by Applied Physics Letters. Magnetotransport and superconducting properties are investigated for uniformly La-doped SrTiO3 films and GdTiO3/SrTiO3 heterostructures, respectively. GdTiO3/SrTiO3 interfaces exhibit a high-density two-dimensional electron gas on the SrTiO3-side of the interface, while for the SrTiO3 films carriers are provided by the dopant atoms. Both types of samples exhibit ferromagnetism at low temperatures, as evidenced by a hysteresis in the magnetoresistance. For the uniformly doped SrTiO3 films, the Curie temperature is found to increase with doping and to coexist with superconductivity for carrier concentrations on the high-density side of the superconducting dome. The Curie temperature of the GdTiO3/SrTiO3 heterostructures scales with the thickness of the SrTiO3 quantum well. Heterostructures and superlattices consisting of a prototype Mott insulator, GdTiO3, and the band insulator SrTiO3 are grown by molecular beam epitaxy and show intrinsic electronic reconstruction, approximately 1/2 electron per surface unit cell at each GdTiO3/SrTiO3 interface. The sheet carrier densities in all structures containing more than one unit cell of SrTiO3 are independent of layer thicknesses and growth sequences, indicating that the mobile carriers are in a high concentration, two-dimensional electron gas bound to the interface. These carrier densities closely meet the electrostatic requirements for compensating the fixed charge at these polar interfaces. In a series of 33 intertrochanteric femoral fractures in patients suffering from classic rheumatoid arthritis (RA), the typical patient was about 70 years of age, osteopenic, and severely affected by longstanding polyarticular RA of joints other than the hip. Prefracture involvement of the hip joint (other than osteopenia) was apparent radiographically in only 9.1% of patients. There was a higher rate of avascular necrosis (9.7%) and nonunion (6.5%) than expected in the nonrheumatoid general population. There was also a high rate of secondary displacement. Of three patients with avascular necrosis, two had had corticosteroid treatment.

A compiler for VLIW and superscalar processors must expose sufficient instruction-level parallelism (ILP) to effectively utilize the parallel hardware. However, ILP within basic blocks is extremely limited for control-intensive programs. We have developed a set of techniques for exploiting ILP across basic block boundaries. These techniques are based on a novel structure called the superblock. The superblock enables the optimizer and scheduler to extract more ILP along the important execution paths by systematically removing constraints due to the unimportant paths. Superblock optimization and scheduling have been implemented in the IMPACT-I compiler. This implementation gives us a unique opportunity to fully understand the issues involved in incorporating these techniques into a real compiler. Superblock optimizations and scheduling are shown to be useful while taking into account a variety of architectural features.

We report optical conductivity measurements of low resistivity ${\text{LaNiO}}_{3}$ thin films grown on ${\text{LaAlO}}_{3}$, ${({\text{LaAlO}}_{3})}_{0.3}$ ${({\text{Sr}}_{2}{\text{AlTaO}}_{6})}_{0.7}$, and ${\text{DyScO}}_{3}$ substrates, providing coherent compressive or tensile strain. The optical conductivity was measured by infrared reflectance and transmittance up to 3 eV and extrapolates well to the measured dc values. An unambiguous Drude response is clearly distinguishable from broad interband absorption extending beyond 3 eV. The measured spectral weight of the Drude response is substantially less than predicted by band theory, indicating an enhancement of the transport mass compared to the band mass by a strain-dependent factor of 2.5--5, characteristic of a strongly correlated metal.

We present a method of fabricating nonimaging light concentrators from total internal reflection film. A prototype has been made and tested and found to operate in agreement with predictions of ray-tracing codes. The performance of the prototype is comparable with that of concentrators made from specular reflecting materials.

Micron scale ferromagnetic tubes placed on the ends of ferromagnetic CoTaZr spin waveguides are explored in order to enhance the excitation of Backward Volume Magnetostatic Spin Waves. The tubes produce a closed magnetic circuit about the signal line of the coplanar waveguide and are, at the same time, magnetically contiguous with the spin waveguide. This results in a 10 fold increase in spin wave amplitude. However, the tube geometry distorts the magnetic field near the spin waveguide and relatively high biasing magnetic fields are required to establish well defined spin waves. Results are in good agreement with model calculations of spin wave dispersion and up to three different modes are identified. We investigate the behavior of hole polarons in rare-earth titanates by combining optical conductivity measurements with first-principles hybrid density functional calculations. Sr-doped GdTiO3 (Gd1−xSrxTiO3) was grown by molecular beam epitaxy. We show that a feature in the optical conductivity that was previously identified with the Mott-Hubbard gap is actually associated with the excitation of a small polaron. The assignment is based on an excellent match between the experimental spectra and first-principles calculations for polaron excitation mechanisms.

The visual system of the cat is considered to be organized in both a serial and parallel manner. Studies of visual onset latencies generally suggest that parallel processing occurs throughout the dorsal stream. These studies are at odds with the proposed hierarchies of visual areas based on termination patterns of cortico-cortical projections. In previous studies, a variety of stimuli have been used to compute latencies, and this is problematic as latencies are known to depend on stimulus parameters. This could explain the discrepancy between latency and neuroanatomical based studies. Therefore, the first aim of the present study was to determine whether latencies increased along the hierarchy of visual areas when the same stimuli are used. In addition, the effect of stimulus complexity was assessed. Visual onset latencies were calculated for area 17, PMLS, AMLS, and AEV neurons. Latencies were also computed from neurons in the lateral posterior (LP)-pulvinar complex given the importance of this extrageniculate complex in cortical intercommunication. Latency distributions from all regions overlapped substantially, and no significant difference was present, regardless of the type of stimulus used. The onset latencies in the LP-pulvinar complex were comparable to those seen in cortical areas. The data suggest that the initial processing of information in the visual system is parallel, despite the presence of a neuroanatomical hierarchy. Simultaneous response onsets among cortical areas and the LP-pulvinar suggest that the latter is more than a simple relay station for information headed to cortex. The data are consistent with proposals of the LP-pulvinar as a center for the integration and distribution of information from/to multiple cortical areas.

The cortical regions surrounding the suprasylvian sulcus have previously been associated with motion processing. Of the six areas originally described by Palmer et al. [J Comp Neurol 177 (1978) 237], the posteromedial lateral suprasylvian (PMLS) cortex has attracted the greatest attention. Very little physiological information is available concerning other suprasylvian visual areas, and in particular, the anteromedial lateral suprasylvian cortex (AMLS). Based on its cortical and sub-cortical connectivity patterns, the AMLS cortex is a likely candidate for higher-order motion processing in cat visual cortex. We have investigated this possibility by studying the receptive field sensitivity of AMLS neurons to complex motion stimuli. Neurons in AMLS cortex exhibited large (mean of 354°2) and complex-like receptive fields, and most of them (74%) were classified as direction selective on the basis of their responses to sinusoidal drifting gratings. Most importantly, direction selectivity was present for complex motion stimuli. A subset of the neurons sampled (eight of 38 cells; 21%) exhibited pattern-motion selectivity in response to moving plaid patterns. The capacity of AMLS neurons to signal higher-order stimuli was further supported by their selectivity to moving complex random-dot kinematograms. Finally, 45% of 20 neurons were direction selective to a radial optic flow stimulus. Overall, these results suggest that AMLS cortex is involved in higher-order analyses of visual motion. It is possible that the AMLS cortex represents a region between PMLS and the anterior ectosylvian visual area in a functional hierarchy of areas involved in motion integration.

During general anesthesia with the laryngeal mask airway (LMA), a significant increase in cuff pressure due to diffusion of nitrous oxide through the cuff wall occurs. This descriptive clinical study was conducted in a university teaching hospital ambulatory surgical center with 100 patients (ASA physical status, I-III; age, 5 months to 76 years; weight, 5.8-146.3 kg) undergoing general anesthesia with an LMA. The airway pressure at which the LMA seal was broken (leak pressure) was determined immediately after the insertion of the LMA. The LMA leak pressure was determined by closing the circuit pop-off valve and recording with a stethoscope at the neck the pressure at which the gas was first heard to escape around the LMA. The LMA cuff pressure was determined by connecting the check valve of the LMA pilot balloon to a sphygmomanometer. The mean LMA cuff pressure increased 16 +/- 8.2 mm Hg (1- to 30-minute group), 38.11 +/- 15.87 mm Hg (31- to 60-minute group), 39.53 +/- 16.9 mm Hg (61- to 90-minute group), 42.63 +/- 20.36 mm Hg (91- to 120-minute group), and 44.25 +/- 14.03 mm Hg (120- to 350-minute group). This study demonstrated that there was a gradual increase in the cuff pressure well over a 3-hour period during nitrous oxide and oxygen anesthesia.

The 21st century will bring a new era of cost containment to the arena of ambulatory surgery force practitioners of anesthesia to reevaluate practice patterns. Along with the current increased interest in cost containment and optimal use of limited resources, growing concerns about patient outcome result in controversial issues in adult and pediatric outpatient anesthesia that must be addressed in the areas of preoperative evaluation, patient preparation and selection, laboratory screening, and practical discharge criteria. The future challenge for all practitioners is to provide high-quality anesthesia care at a reduced cost.

Resonances are observed in the transmission between two coplanar waveguides coupled by ferromagnetic Co90Ta5Zr5 tubes that wrap around their shorted ends. The resonances are assigned to the magnetostatic surface waves that counter propagate along the tube perimeter. We use a model based on an infinite ferromagnetic tube, with elliptical cross section of roughly the same dimensions as the studied structure. Additional theoretical analysis of the fundamental precession mode observed in experiment is carried out. Periodic boundary conditions dictated by the tube perimeter and applied to magnetostatic surface waves quantitatively account for the experimentally observed bandwidth of excited modes, despite the contorted tubular shape. The tubular topology appears to be more important than the shape details.

In a series of 33 intertrochanteric femoral fractures in patients suffering from classic rheumatoid arthritis (RA), the typical patient was about 70 years of age, osteopenic, and severely affected by longstanding polyarticular RA of joints other than the hip. Prefracture involvement of the hip joint (other than osteopenia) was apparent radiographically in only 9.1% of patients. There was a higher rate of avascular necrosis (9.7%) and nonunion (6.5%) than expected in the nonrheumatoid general population. There was also a high rate of secondary displacement. Of three patients with avascular necrosis, two had had corticosteroid treatment.

Several methods of heat conservation and transfer have been suggested as aids to reducing heat loss during anesthesia. This study evaluated and compared the effectiveness of four warming and heat maintenance therapies. The efficacy of various heat conservation modalities was evaluated in 60 patients in five groups of 12 each, four experimental groups, and a control group. The study concluded that the Bair Hugger is more effective in maintaining normothermia during general anesthesia than is a reflective blanket, humidistat, or heated humidifier.

Electrical resistivity and magnetotransport are explored for thin (3–30 nm), epitaxial LaNiO3 films. Films were grown on three different substrates to obtain LaNiO3 films that are coherently strained, with different signs and magnitude of film strain. It is shown that d-band transport is inhibited as the layers progress from compression to tension. The Hall coefficient is “holelike.” Increasing tensile strain causes the film resistivity to increase, causing strong localization to appear below a critical thickness.

Midazolam is the first water soluble sedative widely used as an adjunct agent in general anesthesia. This 1,4-benzodiazepine derivative has been found to be a versatile agent, used as a premedicant, a hypnotic, an adjunct to intravenous conscious sedation, a long-term infused sedative agent, and more recently as an induction agent in selected situations. This report provides descriptive observations of midazolam used as an induction agent in 50 American Society of Anesthesiologists class I through III patients undergoing general anesthesia for elective or selected emergency surgeries. All inductions were preceded by 10 to 20 micrograms sufentanil. The use of sufentanil prior to the induction dose of midazolam facilitated a quicker onset of unconsciousness. Induction was smooth but gradual, and the course of anesthesia was otherwise satisfactory. Patients were alert and awake in the postanesthesia care unit. Recovery time was not prolonged due to somnolence or disorientation.

We reviewed the records and radiographs from 10 hospitals to identify 50 patients with rheumatoid arthritis (RA) who had sustained 52 femoral neck fractures. Most patients were female (88%), elderly (mean age 66 years) and had had severe polyarticular disease for a mean duration of 16 years. Over half had taken systemic corticosteroids, nearly all were severely osteopenic but few had rheumatoid changes in the hip. Of the 20 fractures treated by internal fixation 12 had complications including nonunion (5), osteonecrosis (5), infection (1), and intertrochanteric fracture (1). Only one of the nine undisplaced fractures required reoperation, but seven of the 11 displaced fractures had revision surgery. Twenty fractures were treated by primary total hip arthroplasty with only one early complication. The other 12 fractures had been treated by hemiarthroplasty (9), hip excision (1) or non-operatively (2). Our results suggest that, in elderly rheumatoid patients, severely displaced femoral neck fractures should be treated by total hip replacement.
