Bovine respiratory diseases (BRDs) have major socioeconomic impacts in the beef sector. Antimicrobials have been traditionally used to prevent the development of BRDs upon arrival in fattening units. Currently, from a “One Health and One Welfare” perspective, alternative solutions are being investigated. Preconditioning programmes that aim at reducing stress and reinforcing immune functions have been proposed to decrease BRDs incidence. In this study, we assessed the effect of a preconditioning protocol set up in nine commercial beef herds on young bulls’ behaviour, health, and performance. Preconditioned bulls (PREC) were weaned indoors 50 days before departure, and progressively fed a concentrate diet supplemented with vitamins and trace elements. They also received an anthelmintic treatment, and two doses of a trivalent vaccine against respiratory pathogens at 4-week intervals. In contrast, controls (CTRL) were kept on pasture with their dams with no vaccination or particular diets. All calves were transported together to a single sorting facility to be sorted by experimental groups, origins, and BW, forming new groups before dispatch to four fattening units. At arrival, CTRL were treated with anthelmintic drugs and received one injection of the same BRDs vaccine. No intervention was performed on PREC bulls. BRD-scores were 0.73 in PREC versus 0.07 in CTRL (P = 0.01) during the preconditioning period and 0.96 in PREC versus 0.54 in CTRL (P = 0.41) after 15 days in the fattening units. Estimates of winter (snow-covered non-growing season) CO2 fluxes across the Arctic region vary by a factor of 3.5, with considerable variation between measured and simulated fluxes. Measurements of snow properties, soil temperatures, and net ecosystem exchange (NEE) at Trail Valley Creek, NWT, Canada, allowed for the evaluation of simulated winter NEE in a tundra environment with the Community Land Model (CLM5.0). Default CLM5.0 parameterisations did not adequately simulate winter NEE in this tundra environment, with near-zero NEE (< 0.01 gCm-2d-1) simulated between November and mid-May. In contrast, measured NEE was broadly positive (indicating net CO2 release) from snow-cover onset until late April. Changes to the parameterisation of snow thermal conductivity, required to correct for a cold soil temperature bias, reduced the duration for which no NEE was simulated. Parameter sensitivity analysis revealed the critical role of the minimum soil moisture threshold of decomposition (Ψmin) in regulating winter soil respiration. The default value of this parameter (Ψmin) was too high, preventing simulation of soil respiration for the vast majority of the snow-covered season. In addition, the default rate of change of soil respiration with temperature (Q10) was too low, further contributing to poor model performance during winter. As Ψmin and Q10 had opposing effects on the magnitude of simulated winter soil respiration, larger negative values of Ψmin and larger positive values of Q10 are required to simulate wintertime NEE more adequately.

Abstract Background Young people (YP) in southern Africa are at substantial risk of HIV and sexually transmitted infections (STIs). Despite the epidemiological and biological link between STIs and HIV transmission and acquisition, infections such as Chlamydia trachomatis (CT) and Neisseria gonorrhoeae (NG) remain widely undiagnosed. Syndromic STI management is the standard of care in low- and middle-income countries (LMICs) despite a high prevalence of asymptomatic infections. We conducted an observational study to explore the acceptability, feasibility, and cost of a STI test-and-treat service for YP in Cape Town. Methods YP attending a mobile clinic (MC) and a youth centre clinic (YC) were offered STI screening. Urine testing for CT and NG using a 90-min molecular point-of-care (POC) test on the GeneXpert platform was conducted and treatment provided. Data were collated on demographics, sexual behaviour, presence of symptoms, uptake of same-day treatment, prevalence of CT/NG, and service acceptability. Results Three hundred sixty six participants were enrolled (median age 20, 83% female).57% (209/366) of participants tested positive for either CT (126/366, 34%) or NG (57/366, 16%) or co-infection (26/366, 7%). Clinical symptoms were a poor predictor of GeneXpert diagnosed CT or NG, with a sensitivity of 46.8% and 54.0% for CT and NG respectively. Although half of participants initially chose to receive same day results and treatment, only a third waited for results on the day. The majority of participants (91%) rated the service highly via a post-visit acceptability questionnaire. Conclusion Curable STIs are highly prevalent in this population. STI screening using POC testing was feasible and acceptability was high. The study provides further impetus for moving policy beyond syndromic management of STIs in South Africa.

Abstract Background While South Africa’s national HIV program is the largest in the world, it has yet to reach the UNAIDS 95–95-95 targets. To reach these targets, the expansion of the HIV treatment program may be accelerated through the use private sector delivery models. This study identified three innovative non-governmental primary health care models (private sector) providing HIV treatment, as well as two government primary health clinics (public sector) that served similar populations. We estimated the resources used, and costs and outcomes of HIV treatment across these models to provide inputs to inform decisions around how these services might best be provided through National Health Insurance (NHI). Methods A review of potential private sector models for HIV treatment in a primary health care setting was conducted. Models actively offering HIV treatment (i.e. in 2019) were considered for inclusion in the evaluation, subject to data availability and location. These models were augmented by government primary health clinics offering HIV services in similar locations. We conducted a cost-outcomes analysis by collecting patient-level resource usage and treatment outcomes through retrospective medical record reviews and a bottom-up micro-costing from the provider perspective (public or private payer). Patient outcomes were based on whether the patient was still in care at the end of the follow up period and viral load (VL) status, to create the following outcome categories: in care and responding (VL suppressed), in care and not responding (VL unsuppressed), in care (VL unknown) and not in care (LTFU or deceased). Data collection was conducted in 2019 and reflects services provided during the 4 years prior to that (2016–2019). Results Three hundred seventy-six patients were included across the five HIV treatment models. Across the three private sector models there were differences in the costs and outcomes of HIV treatment delivery, two of the models had results similar to the public sector primary health clinics. The nurse-led model appears to have a cost-outcome profile distinct from the others. Conclusion The results show that across the private sector models studied the costs and outcomes of HIV treatment delivery vary, yet there were models that provided costs and outcomes similar to those found with public sector delivery. Offering HIV treatment under NHI through private delivery models could therefore be an option to increase access beyond the current public sector capacity.

Past efforts to synthesize and quantify the magnitude and change in carbon dioxide (CO2) fluxes in terrestrial ecosystems across the rapidly warming Arctic–boreal zone (ABZ) have provided valuable information but were limited in their geographical and temporal coverage. Furthermore, these efforts have been based on data aggregated over varying time periods, often with only minimal site ancillary data, thus limiting their potential to be used in large-scale carbon budget assessments. To bridge these gaps, we developed a standardized monthly database of Arctic–boreal CO2 fluxes (ABCflux) that aggregates in situ measurements of terrestrial net ecosystem CO2 exchange and its derived partitioned component fluxes: gross primary productivity and ecosystem respiration. The data span from 1989 to 2020 with over 70 supporting variables that describe key site conditions (e.g., vegetation and disturbance type), micrometeorological and environmental measurements (e.g., air and soil temperatures), and flux measurement techniques. Here, we describe these variables, the spatial and temporal distribution of observations, the main strengths and limitations of the database, and the potential research opportunities it enables. In total, ABCflux includes 244 sites and 6309 monthly observations; 136 sites and 2217 monthly observations represent tundra, and 108 sites and 4092 observations represent the boreal biome. The database includes fluxes estimated with chamber (19 % of the monthly observations), snow diffusion (3 %) and eddy covariance (78 %) techniques. The largest number of observations were collected during the climatological summer (June–August; 32 %), and fewer observations were available for autumn (September–October; 25 %), winter (December–February; 18 %), and spring (March–May; 25 %). ABCflux can be used in a wide array of empirical, remote sensing and modeling studies to improve understanding of the regional and temporal variability in CO2 fluxes and to better estimate the terrestrial ABZ CO2 budget. Abstract Background The proposed National Health Insurance (NHI) system aims to re-engineer primary healthcare (PHC) in South Africa, envisioning both private sector providers and public sector clinics as independent contracting units to the NHI Fund. In 2017, 16% of the South African population had private medical insurance and predominately utilised private providers. However, it is estimated that up to 28% of the population access private PHC services, with a meaningful segment of the low-income, uninsured population paying for these services out-of-pocket. The study objective was to characterise the health seeking behaviour of low-income, patients accessing PHC services in both the public and private sectors, patient movement between sectors, and factors influencing their facility choice. Methods We conducted once-off patient interviews on a random sample of 153 patients at 7 private PHC providers (primarily providing services to the low-income mostly uninsured patient population) and their matched public PHC clinic (7 facilities). Results The majority of participants were economically active (96/153, 63%), 139/153 (91%) did not have health insurance, and 104/153 (68%) earned up to $621/month. A multiple response question found affordability (67%) and convenience (60%) were ranked as the most important reasons for choosing to usually access care at public clinics (48%); whilst convenience (71%) and quality of care (59%) were key reasons for choosing the private sector (32%). There is movement between sectors: 23/76 (30%) of those interviewed at a private facility and 8/77 (10%) of those interviewed at a public facility indicated usually accessing PHC services at a mix of private and public facilities. Results indicate cycling between the private and public sectors with different factors influencing facility choice. Conclusions It is imperative to understand the potential impact on where PHC services are accessed once affordability is mitigated through the NHI as this has implications on planning and contracting of services under the NHI.

Climate change in the Arctic is leading to shifts in vegetation communities, permafrost degradation and alteration of tundra surface–atmosphere energy and carbon (C) fluxes, among other changes. However, year-round C and energy flux measurements at high-latitude sites remain rare. This poses a challenge for evaluating the impacts of climate change on Arctic tundra ecosystems and for developing and evaluating process-based models, which may be used to predict regional and global energy and C feedbacks to the climate system. Our study used 14 years of seasonal eddy covariance (EC) measurements of carbon dioxide (CO2), water and energy fluxes, and winter soil chamber CO2 flux measurements at a dwarf-shrub tundra site underlain by continuous permafrost in Canada’s Southern Arctic ecozone to evaluate the incorporation of shrub plant functional types (PFTs) in the Canadian Land Surface Scheme Including Biogeochemical Cycles (CLASSIC), the land surface component of the Canadian Earth System Model. In addition to new PFTs, a modification of the efficiency with which water evaporates from the ground surface was applied. This modification addressed a high ground evaporation bias that reduced model performance when soils became very dry, limited heat flow into the ground, and reduced plant productivity through water stress effects. Compared to the grass and tree PFTs previously used by CLASSIC to represent the vegetation in Arctic permafrost-affected regions, simulations with the new shrub PFTs better capture the physical and biogeochemical impact of shrubs on the magnitude and seasonality of energy and CO2 fluxes at the dwarf-shrub tundra evaluation site. The revised model, however, tends to overestimate gross primary productivity, particularly in spring, and overestimated late-winter CO2 emissions. On average, annual net ecosystem CO2 exchange was positive for all simulations, suggesting this site was a net CO2 source of 18 ± 4 g C m−2 yr−1 using shrub PFTs, 15 ± 6 g C m−2 yr−1 using grass PFTs, and 25 ± 5 g C m−2 yr−1 using tree PFTs. These results highlight the importance of using appropriate PFTs in process-based models to simulate current and future Arctic surface–atmosphere interactions.

Abstract Background Acupuncture is used to treat patients with diabetic peripheral neuropathy; however, the evidence is unclear. We present the design and methodology of the ACUDPN (ACUpuncture in Diabetic Peripheral Neuropathy) trial, which investigates the effectiveness of acupuncture for the treatment of diabetic peripheral neuropathy (DPN) symptoms. The aim of this study is to investigate whether acupuncture is effective for the treatment of DPN symptoms. Methods This study is a two-armed, randomized, controlled, parallel group, open-label, confirmatory, multicenter trial (8-week intervention period plus 16 weeks of follow-up). Physicians in outpatient units in Germany who specialize in acupuncture treatment will treat 110 diabetes type II patients with clinical symptoms of peripheral neuropathy in the feet and legs with signs of neuropathy according to nerve conduction testing. The patients will be randomized in a 1:1 ratio to one of the following two groups: (a) semi-standardized acupuncture plus routine care or (b) routine care alone. Acupuncture will consist of 12 treatments per patient over 8 weeks. The primary outcome will be the overall DPN-related complaints in the extremities after 8 weeks as measured by the Visual Analog Scale (VAS). Further outcome measures will include DPN-related pain, the Neuropathic Pain Symptom Inventory (NPSI), Diabetic Peripheral Neuropathic Pain Impact (DPNPI) scores, and nerve conduction parameters of the sural nerve at weeks 8, 16, and 24. Discussion The results of this trial will be available in 2021 and will help clarify whether acupuncture can be considered effective for the treatment of DPN with regard to the subdimensions of the neuropathic clinical picture. The recent mountain pine beetle (MPB) outbreak has had an impact on the carbon (C) cycling of lodgepole pine forests in British Columbia. This study examines how partial harvesting as a forest management response to MPB infestation affects the net ecosystem production (NEP) of a mixed conifer forest (MPB-09) in Interior BC. MPB-09 is a 70-year-old stand that was partially harvested in 2009 after it had been attacked by MPB. Using the eddy-covariance technique, the C dynamics of the stand were studied over two years and compared to an adjacent clear-cut (MPB-09C) over the summertime. The annual NEP at MPB-09 increased from −108 g C m−2 in 2010 to −57 g C m−2 in 2011. The increase of NEP was due to the associated increase in annual gross ecosystem photosynthesis (GEP) from 812 g C m−2 in 2010 to 954 g C m−2 in 2011, exceeding the increase in annual respiration (Re) from 920 g C m−2 to 1011 g C m−2 during the two years. During the four month period between June and September 2010, NEP at MPB-09C was −103 g C m−2, indicating high C losses in the clear-cut. MPB-09 was a C sink during the growing season of both years, increasing from 9 g C m−2 in 2010 to 47 g C m−2 in 2011. The increase of NEP in the partially harvested stand amounted to a recovery corresponding to a 26% increase in the maximum assimilation rate in the second year. This study shows that retaining the healthy residual forest can result in higher C sequestration of MPB-attacked stands compared to clear-cut harvesting.

Humic acids (HA) are known for their antiviral and UV-B protecting effects, and are considered promising as ingredients for a UV-protective lipstick which is being developed to minimise or even prevent recurrences of UV- induced herpes. In this study, the UV/Vis spectra of three natural HA and three synthetic HA-like substances are analysed to determine the appropriateness of their UV-absorbing characteristics for the product under development. The contribution of a matrix component (castor oil) to the total UV absorption of the lipstick is also assessed. The results confirm the expected high UV-B absorption of the individual test substances, but reveal considerable differences in the UV-A wavelength range. Castor oil absorbs only UV-B radiation; and when mixed with HA it enhances total absorption in the UV-B range, but reduces it in the UV-A range. This is probably due to molecular interactions between castor oil and HA. Preliminary results from cultures of human U937 cells assayed for survival 24 hours after exposure to UV-B radiation show that both HA and castor oil exert a significant concentration-dependent UV-B protective filter effect similar to that of the UV-B absorbing reference substance p-aminobenzoic acid (PABA).

A hot rolled Zircaloy-4 alloy, annealed with a final cold rolling, presenting rounded grains, was studied. Hydrogen cathodic charge with a homogenization heat treatment was used to pre-charge the specimens with different hydrogen contents. Hydrogen embrittlement susceptibility analysis was held using J integral and J-R curve results from CT specimens (compact tension specimens) tested at room temperature. As J IC values showed scatter, toughness was evaluated for deltaa = 1mm. Toughness clearly tended to decrease as hydrogen content increased abruptly for low H contents and gradually for high contents. A few specimens with high hydrogen content failed in brittle mode, or presented instability and posterior crack arrest. Fractographic observations showed that, despite the records had presented no signs of brittle fracture, certain specimens showed cleavage-like zones. More cleavage-like area percentage was present the higher the hydrogen content was.

In this study the relationship between propensity for participative management and tolerance for ambiguity on the one hand, and collective learning behaviours - a particular form of participation - was examined. Three questionnaires measuring these constructs were administered on a sample of 283 employees in a financial services institution. A second order factor analysis yielded two second order factors for propensity for participation, one for tolerance for ambiguity, and three for collective learning behaviours. The three factors for collective learning behaviours served as dependent variables during a multiple regression analysis. Two statistically significant correlations were obtained during a canonical correlation procedure. The findings confirm that there is a statistically significant relationship between propensity for participation and tolerance for ambiguity on the one hand, and collective learning behaviours. The findings and implications are discussed.
