1. Wolbachia pipientis: primeira detecção em populações de Glycaspis brimblecombei (Hemiptera: Aphalaridae) e Psyllaephagus bliteus (Hymenoptera: Encyrtidae) no Brasil

Abstract

The sucking insect, Glycaspis brimblecombei Moore (Hemiptera: Aphalaridae), is originally from Australia and reduces the productivity of Eucalyptus crops. The parasitoid Psyllaephagus bliteus Riek (Hymenoptera: Encyrtidae) is the main agent used in the integrated management of G. brimblecombei. Endosymbionts, in insects, are important in the adaptation and protection of their hosts to the environment. The intracellular symbionts Wolbachia, induces reproductive changes such as cytoplasmic incompatibility, feminization, male death and parthenogenesis. The objective of this study was to report the first record of Wolbachia pipientis in populations of G. brimblecombei and of its parasitoid P. bliteus in the field in Brazil. Branches with adults of G. brimblecombei and P. bliteus were collected from eucalyptus trees in commercial farms in six Brazilian states and, after emergence, the insects obtained were frozen at -20 °C. Polymerase chain reaction (PCR) was performed to detect the Wolbachia endosymbiont. Wolbachia pipientis was identified in individuals of G. brimblecombei and its parasitoid P. bliteus from populations of the counties of Agudos and Mogi-Guaçu (São Paulo State), Itamarandiba (Minas Gerais State) and São Jerônimo da Serra (Paraná State) in Brazil. Resumo O inseto sugador, Glycaspis brimblecombei Moore (Hemiptera: Aphalaridae), é de origem australiana e reduz a produtividade de cultivos do gênero Eucalyptus. O parasitoide Psyllaephagus bliteus Riek (Hymenoptera: Encyrtidae) é o principal agente utilizado no manejo integrado de G. brimblecombei. Endossimbiontes, em insetos, são importantes na adaptação e proteção de seus hospedeiros ao ambiente que habitam. Wolbachia, um simbionte intracelular, induz alterações reprodutivas, como feminização, incompatibilidade citoplasmática, morte de machos e partenogênese. O objetivo foi relatar o primeiro registro de Wolbachia pipientis em populações de G. brimblecombei e de seu parasitoide P. bliteus em campo no Brasil. Ramos com adultos de G. brimblecombei e P. bliteus foram coletados em árvores de eucalipto em plantios comerciais em seis estados do Brasil e, após a emergência, os insetos obtidos foram congelados -20 °C. A reação em cadeia da polimerase (PCR) foi realizada para detectar o endossimbionte Wolbachia. Wolbachia pipientis foi identificado em indivíduos de G. brimblecombei e de seu parasitoide P. bliteus de populações de Agudos e Mogi-Guaçu (São Paulo), Itamarandiba (Minas Gerais) e São Jerônimo da Serra (Paraná), Brasil.

Abstract

In 2008, we published the first set of guidelines for standardizing research in autophagy. Since then, this topic has received increasing attention, and many scientists have entered the field. Our knowledge base and relevant new technologies have also been expanding. Thus, it is important to formulate on a regular basis updated guidelines for monitoring autophagy in different organisms. Despite numerous reviews, there continues to be confusion regarding acceptable methods to evaluate autophagy, especially in multicellular eukaryotes. Here, we present a set of guidelines for investigators to select and interpret methods to examine autophagy and related processes, and for reviewers to provide realistic and reasonable critiques of reports that are focused on these processes. These guidelines are not meant to be a dogmatic set of rules, because the appropriateness of any assay largely depends on the question being asked and the system being used. Moreover, no individual assay is perfect for every situation, calling for the use of multiple techniques to properly monitor autophagy in each experimental setting. Finally, several core components of the autophagy machinery have been implicated in distinct autophagic processes (canonical and noncanonical autophagy), implying that genetic approaches to block autophagy should rely on targeting two or more autophagy-related genes that ideally participate in distinct steps of the pathway. Along similar lines, because multiple proteins involved in autophagy also regulate other cellular pathways including apoptosis, not all of them can be used as a specific marker for bona fide autophagic responses. Here, we critically discuss current methods of assessing autophagy and the information they can, or cannot, provide. Our ultimate goal is to encourage intellectual and technical innovation in the field.

Abstract

Background The existing classifications of varicocoeles have poor predictive value regarding the effects of surgery on sperm count. Objective To develop a new grading system for varicocoeles, useful as an indication for surgery. Materials and methods This is a three-center prospective study which examined 173 men having clinically detectable left varicocoeles and oligo ± astheno ± terato-spermia. The patients underwent medical history collection, objective examination, duplex Doppler ultrasound scrotal examination, hormonal profiles, two semen analyses before surgery, and two semen analyses after surgery. Sperm concentration, motility, and morphology (standard semen parameters) were evaluated 6 months after surgery in function of the following preoperative variables: patient age, follicle-stimulating hormone, clinical grade of varicocoele, right and left testicular volume, extension of venous reflux, and semen parameters. The venous reflux was graded in two centers using duplex Doppler ultrasound: reflux visible only with Valsalva and continuous reflux. Spearman's rank semiquantitative analysis was used. All patients had their varicocoeles corrected according to the Colpi technique. Results The variables capable of determining an improvement in semen parameters after varicocoele correction were in order of decreasing importance: venous reflux extent assessed by scrotal duplex Doppler ultrasound examination, varicocoele clinical grade, basal semen parameters, and follicle-stimulating hormone. Male age did not influence postoperative semen quality. Only patients with continuous reflux had their standard semen parameters improved after surgery. Discussion Venous reflux extent is the most critical variable capable of predicting semen improvement after varicocoelectomy. Conclusions A correct duplex Doppler assessment of venous reflux is mandatory for predicting postoperative improvement of the sperm count.

Abstract

Summary The hypothesis that the assumption of a probiotic associated with a prebiotic (Flortec, Bracco; one sachet contains: Lactobacillus paracasei B21060 5 × 109 cells + arabinogalctan 1243 mg + oligo-fructosaccharides 700 mg + l-glutamine 500 mg) could improve the quality and quantity of spermatozoa in idiopathic oligoasthenoteratospermia (iAOT) patients to a larger extent than a control substance (starch) was tested. Forty-one patients with no chromosomal aberrations were randomized into two groups: 20 received Flortec, one sachet per day for 6 months, whereas 21 received the control substance. The following data were collected: clinical history, scrotal Doppler scans, Y microdeletion, karyotype and cystic fibrosis screens, follicle-stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E2), testosterone (T), and prolactin (PRL) levels, and two semen analyses. Both the Flortec and the starch groups underwent two semen analyses and one FSH, LH, T, E2, and PRL measurement in the blood, at the beginning of the study, and after 6 months. The comparisons were carried out using a non-parametric (Wilcoxon signed rank) test. The side effects were assessed and compared using the chi-squared test. Abstract

This paper describes a new possible approach to fatigue design of aerospace components founded on probabilistic bases compared with safe life and damage tolerance that are founded on deterministic bases. A numerical tool has been introduced and explained together with the experimental activity for its validation analyses. For a typical aerospace component, such as a lap-joint panel, an acceptable maximum risk level has been established and the maintenance program has been planned to ensure operating life without catastrophic failures. The analysis has shown that this new approach introduces several benefits in fatigue design.

Abstract

Dolichol is a polyprenol compound broadly distributed in membranes, biosynthetized by the general isoprenoid pathway from acetate via mevalonate and farnesyl pyrophosphate. Dolichol lays inside the membrane between the two leaflets of the lipid bilayer very close to the tail of phospholipid fatty acids. No definite catabolic pathways for this molecule have yet been identified. Evidence is produced that dolichol levels increase dramatically with increasing age; that anti-ageing caloric restriction retards this age-associated change; that dolichol may act as a radical scavenger of peroxidized lipids belonging to the cell membranes. In view of the polyunsaturated fatty acids (PUFA), dolichol and Vitamin E location and stechiometry, it is proposed that molecules might interact each-other to form a highly matched free-radical-transfer chain, whose malfunctioning might be involved in statin toxicity and neurodegenerative diseases.

Abstract

To address some basic questions about primary and secondary events in the process of aging in different cell and tissue types, we studied changes in the levels of biomarkers of the aging cells (dolichol) and connective tissue (pentosidine) in the heart of older (22-month-old) Lewis rats heterotopically transplanted in younger (3-month-old) syngenic recipients. Results showed that age-mismatched transplantation did not alter the age-related accumulation of dolichol and significantly reduced the accumulation of pentosidine in cardiac tissue. It is concluded that aging of heart muscle and connective tissues is controlled by two independent clocks; that accumulation of dolichol in older tissues may be a primary consequence of the process of aging, whereas the accumulation of pentosidine may be secondary, perhaps to changes in circulating cells endowed with advanced glycation end products-specific receptors; in the perspective of organ transplantation, the environment of a younger host may positively interact with the graft and rejuvenate its collagen.

Abstract

We previously identified serum creatinine values2 mg/dL (176.8 micromol/L) and pathological chest radiographs due to the presence of pleural effusions or pulmonary densifications as two early prognostic factors of life-threatening acute pancreatitis (AP). The aim of the present study was to validate their prognostic efficacy in combination.We analyzed as prognostic factors only the data obtained within 24 hours of admission in 539 cases of AP, including 163 patients (30.2%) with acute necrotizing pancreatitis (NP). Eleven patients (2%) presented with infected pancreatic necrosis, and 20 patients (3.7%) died.One hundred and nine cases (20.2%) presented pathological chest radiographs: 32 (5.9%) pulmonary densifications and 77 (14.3%) pleural effusions (18 right, 25 left, 34 bilateral). Fifty-one patients (9.5%) had serum creatinine values2 mg/dL. Pathological chest radiographs and serum creatinine values2 mg/dL were significantly associated both with mortality risk (P0.00001), with a diagnosis of NP (P0.00001), and with risk of developing infected necrosis (P0.0001). Considering positivity of either or both tests, the mortality risk sensitivity was 90% (95% confidence interval [CI] 70.7 to 98.3) with a specificity of 76% (CI 72.5 to 79.8), for the NP diagnosis the sensitivity was 60% (CI 52.5 to 67.4) and the specificity 88% (CI 85.0-91.5), whereas for the risk of infected necrosis the sensitivity was 73% (CI 42.2 to 92.6) and specificity 75% (CI 70.1 to 78.4). These data are comparable to those obtained with the Ranson or Glasgow scores at 24 hour in this patient group, with a cut-offor =3.Serum creatinine and chest radiographs are two indices capable of identifying, within 24 hour of admission, a subgroup of acute pancreatitis patients with a more severe or adverse clinical course.

Abstract

Manometry is considered the gold standard forevaluating sphincter of Oddi dysfunction. It hasrecently been demonstrated that the ultrasound (US)secretin test proposed a few years ago as a noninvasive test for the study of sphincter of Oddidysfunction yields a substantial percentage ofpathological findings in patients with acute recurrentpancreatitis. The aim of this study was to compare theresults of the US secretin test with sphincter of Oddimanometry findings in a consecutive series of patientswith recurrent acute pancreatitis. Forty-seven patientsadmitted to our gastrointestinal unit suffering from recurrent acute pancreatitis underwentultrasonographic measurement of the main pancreatic ductat baseline and for 60 min after maximal stimulationwith secretin at 1 IU/kg. According to the US secretin test findings in 35 healthy control subjects,the test results were considered to indicate pathologywhen the duct was still dilated after 20 min. Withinthree to seven days the same patients underwent perendoscopic manometry. Thirty-six patients(17 men, 19 women; mean age 41 ± 15 years) had asuccessful US secretin test and sphincter of Oddimanometry. Eleven patients (30.6%) presented normalmanometric findings. Two of these had an abnormal USsecretin test. Twenty-five patients had abnormalmanometry findings, revealing stenosis in 19 (52.7%) (17with abnormal US secretin test) and dyskinesia in six (five with an abnormal US secretin test).Compared to manometry findings, the US secretin testsensitivity and specificity for sphincter of Oddidysfunction were 88% and 82%, respectively. Inconclusion, most patients with recurrent acute pancreatitishave sphincter of Oddi dysfunction documented by both atthe US secretin test and sphincter of Oddi manometry;results of the US secretin test are reliable compared to sphincter of Oddi manometry, andtherefore the US secretin test may offer a validalternative to the more expensive and invasivemanometric procedure for assessing sphincter of Oddidysfunction in patients with recurrent acutepancreatitis.

Abstract

The increasing incidence of both chronic pancreatitis and pan­ creatic cancer was the primary impetus for the organization of this meeting, an additional factor being the widespread existence of pancreatic disease within Italy itself. It is well known that the treatment of chronic and particularly of acute pancreatitis is still a matter of controversy, and it seemed useful to compare our own experience with that of other centers working in the same field. Consequently, in addition to numerous Italian specialists in the field, doctors from the Federal Republic of Germany, Denmark, Great Britain, France, and Austria were invited to attend the meeting. The symposium lasted two days and comprised eight sessions dealing with the anatomy and physiology of the pancreas and the pathophysiology, diagnosis, and treatment of pancreatic diseases. A central feature of the discussion on the surgical treatment of chronic pancreatitis was the debate between those who favor the use of drainage and those who believe resection to be the best treatment. Finally, I would like to thank Dr. Cavallini and Dr. Pederzoli very much for their help in organizing the meeting and also the local authorities for their aid. Abstract

Since the year 2000, grapevine Flavescence Dorée (FD) and Bois Noir (BN) phytoplasmas were found in grapevine growing areas of Modena and Reggio Emilia provinces (northern Italy). Epidemiological investigations carried out by the provincial Plant Protection Services throughout a five-year period (2000 to 2004), have highlighted the wide presence of these pathogens in all the grapevine growing areas. The results obtained from molecular analysis (PCR/RFLP) performed throughout the all period on symptomatic plants, have shown the striking prevalence of BN phytoplasmas. Out of 557 samples collected in Reggio Emilia province, 55,7% were positive to BN whereas 14,4% of the tested plants resulted infected by FD. As regard the 463 analyses on samples collected in Modena province, samples positive to BN phytoplasmas were 67,7% and those positive to FD were 10,8%. Moreover the 2,3% of the Reggio Emilia samples appeared positive to both phytoplasmas. A number of symptomatic grapevines were negative: 27,7% in Reggio Emilia and 21% in Modena provinces. Contemporary presence in the territory of the two phytoplasmas has been noticed in the majority of the counties around Reggio Emilia, while very few in the ones around Modena. Incidence data of yellows were related to the cultivar, training systems, and agronomic conduction of the vineyards. Spatial distribution of yellows diseased plants in the vineyards of the two provinces shows with a gradient decreasing from the edges to the inner parts, as was previously reported in other yellows infected vineyards (Credi et al., 2004). Molecular analysis performed on herbaceous plants around and inside the vineyards demonstrated the presence of BN phytoplasmas in some dicotyledons species (Botti et al., this book). Control measures adopted from 2000 against FD spreading allowed to greatly reducing its epidemic spreading in these provinces. This has not occurred for BN that has become eventually the most dangerous grapevine phytoplasma disease in the two provinces. Wide presence of important epidemiological factors such as etiological agent, insect vector (Hyalesthes obsoletus), herbaceous plants in vineyard’s ecosystem, have so far avoided to carry out an efficient control strategy, with the consequent rapid and large spreading of BN in these vineyards.

Abstract

Background and aim. Till now, no Italian Studies providing information on acute pancreatitis have been published. The aim of this study was to evaluate the epidemiological and clinical characteristics of acute pancreatitis in Italy. Materials and methods. The study involved 37 Italian centres distributed homogeneously throughout the entire national territory and prospectively collected epidemiological, anamnestic, laboratory, radiological, therapeutic (pharmacological, endoscopic and surgical) data, relevant to each individual case of acute pancreatitis consecutively observed during the period from September 1996 to June 2000. Results. One thousand two hundred and six case report forms were collected, but 201 patients (16.6%) were Subsequently eliminated From the final analysis. We therefore Studied 1005 patients, 533 (53%) males and 472 (47%) females, mean age 59.6 +/- 20 years. On the basis of the Atlanta classification of acute pancreatitis, 753 patients of the 1005 cases analysed (75%) were mild and 252 patients (25%) severe. The aetiology was biliary in 60% of the patients, related to alcohol abuse in 8.5%, while in 21% of the cases it could not be identified. Over 80% of the patients (83%) were admitted to hospital within 24h from the onset of clinical symptoms, while only 6% were admitted after 48 h. In particular, 65% of the patients were admitted to hospital within the first 12 h. Antibiotics were used in 85% of the severe and 75% of mild forms. Endoscopic, therapy was carried out in 65% of the severe cases, but only in 40% it was carried out prior to 72 h. Eighty-five patients (8.5% of the total, 34% of the severe forms) underwent surgical intervention: 20% on the first day, 38.5% within the fourth day, and the remaining (41.5% of the cases) later on for infected necrosis. The mean duration of hospitalisation for patients with mild pancreatitis was 13 +/- 8 days, while for the severe disease it was of 30 +/- 14 days. The overall mortality rate was 5%, 17% in severe and 1.5% in mild pancreatitis. Conclusions. Acute pancreatitis in Italy is more commonly a mild disease with it biliary aetiology. The treatment of the disease is not optimal and, on the basis of these data, needs to be standardised. Despite this, the overall mortality rate is low (5%). Abstract

The supraventricular crest is a fleshy trabecula of the right ventricle that has an important function in guiding the blood flow. However, controversy persists regarding its anatomical constitution. In this study, we aimed to investigate its frequency, formation, termination, morphometry, and relationships with the septomarginal trabecula, septal papillary muscle, right atrioventricular ring, and left posterior semilunar valve of the pulmonary trunk valve. Our material consisted of 50 hearts from adult individuals of both sexes that had been preserved in 10% formalin. They were opened along the arterial cone by means of an incision starting at the pulmonary trunk and ending at the right margin. The supraventricular crest was always present. The marginal (right) extremity was formed by two to six muscle bundles that joined together (88%). On the septal (left) side, the single muscle bundle penetrated the interventricular septum directly (88%) or by means of two or three divisions (12%). It could form a septal band (52%) and could pass over the septal papillary muscle (43.5%) or just below it (34.8%). There was a relationship of muscle fibres between these two structures in 64% of cases. Dissection of the septal band demonstrated continuity with the septomarginal trabecula (46%). In 80% of cases, the crest was connected to the right atrioventricular ring and it participated in its outline directly (64%) or by means of muscle expansions (16%). Its muscle fibres bordered the left semilunar valve of the pulmonary valve in 50% of cases. Regarding morphometry, we observed that the length varied little with increasing weight of the heart (22.6%), but the height and width increased markedly with increasing weight of the heart.

28. Gemcitabine and continuous infusion of 5-fluorouracil in locally advanced and metastatic pancreatic cancer: a phase I-II study

Abstract

Gemcitabine has been recently recognized as standard treatment in advanced pancreatic cancer. To potentiate its single-agent activity we conducted a phase I-II study with the primary objective of establishing the maximum tolererated dose (MTD) of gemcitabine and continuous infusion 5-FU in patients with locally advanced or metastatic pancreatic cancer.Fifteen patients received a fired dose of 5-FU 200 mg/mq protracted infusion for six months. Gemcitabine was administered weekly for three out of four weeks for six cycles at escalating doses of 800 mg/mq to 1100 mg/mq.MTD was established at 1000 mg/mq of gem citabine. Of the 11 evaluable patients, 7 patients had stable disease, 1 had partial response and 3 had progressive disease. Of the 14 patients evaluable at follow-up, median time to progression was 5 months. Median survival was 10 months.This study confirms the good tolerability of the combination, of gemcitabine with 5-FU.

To ascertain whether oral propionyl-l-carnitine combined with intraplaque verapamil is a useful therapy for advanced or resistant Peyronie's disease.The combined drugs were assessed in two studies. In the first, 60 patients with advanced Peyronie's disease, diagnosed using accepted definitions, were randomized in two subgroups treated with verapamil intraplaque infiltration (10 mg weekly for 10 weeks) plus a 3-month administration of propionyl-l-carnitine (2 g/day), or verapamil infiltration plus oral tamoxifen (40 mg/day) for 3 months. In the second study, 15 patients with resistant Peyronie's disease (progression despite previous therapy) received verapamil plus propionyl-l-carnitine. The differences between subgroups or between the variables before and after therapy were compared using analysis of variance or the chi-squared test.In the first study, the reduction in pain was the same in both subgroups. Propionyl-l-carnitine plus verapamil significantly reduced penile curvature, plaque size, cavernosal artery end-diastolic velocity, the need for surgery and disease progression, and increased the International Index of Erectile Function score and resistivity index of the cavernosal arteries. Tamoxifen plus verapamil had none of these effects. No drug combination affected the peak systolic velocity. Patients receiving verapamil had no side-effects but those taking tamoxifen did. In the second study propionyl-l-carnitine and verapamil modified the disease patterns as in the first and no patient had side-effects.The combination of propionyl-l-carnitine and verapamil can be considered the therapy of choice for advanced and resistant Peyronie's disease.

34. Acetyl-L-carnitine vs tamoxifen in the oral therapy of Peyronie's disease: a preliminary report

To detect whether oral acetyl-L-carnitine might be useful in the acute and early chronic phases of Peyronie's disease, compared with tamoxifen, a drug currently in use.The study included 48 patients with Peyronie's disease (15 acute and 33 initial chronic), randomized equally into two groups. The first group used tamoxifen 20 mg twice daily for 3 months and the second acetyl-L-carnitine 1 g twice daily for 3 months. The disease and stages were diagnosed and identified using a history, objective examination, pharmacologically induced erection, autophotography during erection, and basic and dynamic colour Doppler ultrasonography. Penile curvature, plaque size, pain and disease progression were assessed. The differences between the groups or between the variables before and after therapy were compared using analysis of variance or the chi-squared test.Acetyl-L-carnitine was significantly more effective than tamoxifen in reducing pain and in inhibiting disease progression. Acetyl-L-carnitine reduced penile curvature significantly, while tamoxifen did not; both drugs significantly reduced plaque size. Tamoxifen induced significantly more side-effects than acetyl-L-carnitine.These results suggest that acetyl-L-carnitine is significantly more effective and safe than tamoxifen in the therapy of acute and early chronic Peyronie's disease.

Abstract

The aim of this paper is to compare the activity of mepartricin vs placebo in male pain pelvic syndrome secondary to chronic nonbacterial prostatitis/prostatodynia.Forty-two patients have been tested (mean age: 35 years; range 29-44), these proved affected by male pain pelvic syndrome secondary to chronic nonbacterial prostatitis/prostatodynia, and were randomized into 2 groups: the 1st treated with mepartricin 40 mg/die for 60 days, the 2nd with placebo (C vitamin 500 mg/die) for 60 days. The following patterns were examined: spontaneous and rectal examination pain, diurnal and nocturnal urinary frequency and prostatic volume. Side effects in course of therapy were examined as well.Mepartricin proved significantly more active than placebo in reducing spontaneous pain, rectal examination pain, diurnal urinary frequency, nicturia and prostatic volume. No significant difference proved to emerge between placebo and mepartricin in terms of side effects.These data allow us to substain that mepartricin may be a useful and safe drug for the therapy of male pain pelvic syndrome secondary to chronic nonbacterial prostatitis/prostatodynia.

Abstract

Over the last few years, remarkable progress has been made in diagnosis, severity assessment and treatment as well as in our understanding of the pathophysiology of acute pancreatitis. New treatment modalities and new specific drugs have been introduced and this has led to practical changes in the daily bedside management of patients with acute pancreatitis. Treatment is essentially medical, both for mild and severe disease, and is aimed at reducing abdominal pain, restoring electrolyte and fluid losses, removing the aetiological factor(s), attenuating inflammation and autodigestive processes, as well as preventing local and systemic complications. Diagnostic and interventional percutaneous or endoscopic procedures are indicated mainly for patients with severe forms of the disease. Surgery is generally indicated for patients with necrosis infection or other local complications not manageable by percutaneous or endoscopic means.

Abstract

The aim of this study was to compare alcohol and smoking as risk factors in the development of chronic pancreatitis and pancreatic cancer. We considered only male subjects: (1) 630 patients with chronic pancreatitis who developed 12 pancreatic and 47 extrapancreatic cancers; (2) 69 patients with histologically well documented pancreatic cancer and no clinical history of chronic pancreatitis; and (3) 700 random controls taken from the Verona polling list and submitted to a complete medical check-up. Chronic pancreatitis subjects drink more than control subjects and more than subjects with pancreatic cancer without chronic pancreatitis (P0.001). The percentage of smokers in the group with chronic pancreatitis is significantly higher than that in the control group [odds ratio (OR) 17.3; 95% CI 12.6-23.8; P0.001] and in the group with pancreatic carcinomas but with no history of chronic pancreatitis (OR 5.3; 95% CI 3.0-9.4; P0.001). In conclusion, our study shows that: (1) the risk of chronic pancreatitis correlates both with alcohol intake and with cigarette smoking with a trend indicating that the risk increases with increased alcohol intake and cigarette consumption; (2) alcohol and smoking are statistically independent risk factors for chronic pancreatitis; and (3) the risk of pancreatic cancer correlates positively with cigarette smoking but not with drinking.

Abstract

To value changes of endotracheal tube cuff pressure during anaesthesia with N2O, using standard tubes or Brandt-system tubes.Endotracheal cuff pressure during anaesthesia in three groups of patients has been monitored: Group 1 (n. 41): endotracheal tube with low-pressure cuff using N2O/O2 (2:1 or 1:1); Group 2 (n. 55): Brandt's double cuff-tubes using N2O/O2 (2:1 or 1:1); Group 3 (n. 20-control group): tubes with low-pressure cuff using O2/air. Values of pressure (M +/- DS: p0.05) have been compared with ANOVA, Bonferroni's method (p0.017).Brandt's double cuff-tubes (G2) succeed in avoiding uncontrolled increase of cuff-pressure during anaesthesia with N2O. Standard low-pressure tubes (G1) shown increase of cuff pressure during anaesthesia with N2O which is absent using no N2O (G3).

Abstract

Manometry is considered the gold standard for evaluating sphincter of Oddi dysfunction. It has recently been demonstrated that the ultrasound (US) secretin test proposed a few years ago as a noninvasive test for the study of sphincter of Oddi dysfunction yields a substantial percentage of pathological findings in patients with acute recurrent pancreatitis. The aim of this study was to compare the results of the US secretin test with sphincter of Oddi manometry findings in a consecutive series of patients with recurrent acute pancreatitis. Forty-seven patients admitted to our gastrointestinal unit suffering from recurrent acute pancreatitis underwent ultrasonographic measurement of the main pancreatic duct at baseline and for 60 min after maximal stimulation with secretin at 1 IU/kg. According to the US secretin test findings in 35 healthy control subjects, the test results were considered to indicate pathology when the duct was still dilated after 20 min. Within three to seven days the same patients underwent perendoscopic manometry. Thirty-six patients (17 men, 19 women; mean age 41 +/- 15 years) had a successful US secretin test and sphincter of Oddi manometry. Eleven patients (30.6%) presented normal manometric findings. Two of these had an abnormal US secretin test. Twenty-five patients had abnormal manometry findings, revealing stenosis in 19 (52.7%) (17 with abnormal US secretin test) and dyskinesia in six (five with an abnormal US secretin test). Compared to manometry findings, the US secretin test sensitivity and specificity for sphincter of Oddi dysfunction were 88% and 82%, respectively. In conclusion, most patients with recurrent acute pancreatitis have sphincter of Oddi dysfunction documented by both at the US secretin test and sphincter of Oddi manometry; results of the US secretin test are reliable compared to sphincter of Oddi manometry, and therefore the US secretin test may offer a valid alternative to the more expensive and invasive manometric procedure for assessing sphincter of Oddi dysfunction in patients with recurrent acute pancreatitis.

Editor,—We read with great interest the article by Ectors et al ( Gut 1997; 41 :263–8) in which the authors describe a particular form of chronic pancreatitis in patients without chronic alcohol intake. The authors suggest the term “non-alcoholic duct destructive chronic pancreatitis” for this type of pancreatic disease in order to stress: (1) the absence of any history of alcohol abuse; (2) the target of the chronic immunological reaction; and (3) the destructive nature of the process. Characteristically some of these cases were associated with an autoimmune disease, such as primary sclerosing cholangitis, ulcerative colitis, Crohn’s disease, or Sjogren’s syndrome. The authors conclude that the inflammatory changes in the pancreatic ducts observed in this particular form of chronic pancreatitis are suggestive of an autoimmune process. A few years ago our group postulated a new hypothesis for the pathogenesis of a particular form of chronic pancreatitis, which we defined as “primary chronic pancreatitis”.1 We suggested that the key step in the pathogenesis of this type of chronic pancreatitis may be …

Duodenal ulcer is regarded as the end result of alterations in which Helicobacter pylori (HP) plays a major role as a pathogenetic agent. The aim of this prospective study was to evaluate duodenal ulcer risk factors such as alcohol intake, smoking, use of non-steroidal anti-inflammatory drugs, age and sex in relation to HP colonization density.495 patients consecutively examined by diagnostic upper digestive endoscopy were assessed; none of them had had previous upper digestive endoscopy or peptic ulcer, recent anticulcer-antibiotic treatment, upper gastrointestinal surgery, or cancer. The HP colonization on antral bioptic specimens was graded semi-quantitatively as follows: HP--- (absence of HP); HP+--(focal presence of small amounts of HP); HP++- (intermediate situation between HP+-- and HP+++), and HP+++ (diffuse presence of large amounts of HP).Logistic regression analysis identified only male sex (odds ratio 1.88, p0.02), smoking more than 10 cigarettes/day (odds ratio 2.53, p0.01), and HP grade (HP +-- odds ratio 0.79, p = NS; HP++- odds ratio 2.42, p0.02; HP+++ odds ratio 3.66, p0.001) as independent risk factors of duodenal ulcer.The duodenal ulcer risk was found to correlate with HP density, male sex, and smoking, but not with non-steroidal anti-inflammatory drug use.

Abstract

Endoscopic retrograde cholangiopancreatography (ERCP) is associated with elevated levels of pancreatic enzymes and pancreatitis. Gabexate, a protease inhibitor, has been used to prevent pancreatic damage related to ERCP.We conducted a multicenter, double-blind comparison of gabexate (1 g given by intravenous infusion starting 30 to 90 minutes before endoscopy and continuing for 12 hours afterward) with placebo (mannitol and sodium chloride, administered in the same fashion). A total of 435 adults scheduled to undergo ERCP and, when indicated, endoscopic sphincterotomy underwent randomization; 17 were excluded from the final analysis for various reasons. The remaining 418 patients (mean age, 60.4 years)--208 in the gabexate group and 210 in the placebo group--were analyzed. Acute pancreatitis was considered to be present if serum amylase or lipase levels (or both) were five times greater than the upper limits of normal in association with the onset of pancreatic pain.After the procedures, 276 patients (66 percent) had elevated pancreatic-enzyme levels; the frequency was similar in the two groups. Mean serum amylase values were higher in the placebo group than in the gabexate group through 24 hours of observation (P=0.03). Twelve patients in the gabexate group and 29 in the placebo group had abdominal pain (6 percent vs. 14 percent, P=0.009). Sixteen patients in the placebo group and five in the gabexate group had acute pancreatitis (8 percent vs. 2 percent, P=0.03). Two patients treated with gabexate and six given placebo had adverse events, all of which resolved. Two patients given placebo died of acute pancreatitis; one was excluded from the evaluation because pancreatitis was present before endoscopy. One patient in the gabexate group died, from a myocardial infarction.Prophylactic treatment with gabexate reduced pancreatic damage related to ERCP, as reflected by reductions in the extent but not the frequency of elevated enzyme levels and in the frequency of pancreatic pain and acute pancreatitis.

Abstract

This work studied the effect of different ethanol concentrations on the eicosanoid accumulation in human gastric incubates of healthy volunteers and chronic alcoholics, determining the in vitro eicosanoid release both in the basal condition and after different ethanol concentrations. The basal release of PGE2 and LTC4 in alcoholics is higher than in healthy volunteers. Various alcohol concentrations cause an increase in LTC4 and PGE2 in healthy volunteers; we observed no LTC4 increase in alcoholics and although PGE2 levels increased after 20% ethanol, they remained constant at higher ethanol concentrations.

Abstract

Despite controversial experimental and clinical findings, the use of Aprotinin (A) in the treatment of acute pancreatitis (AP) is still widespread in our country. Compared to A, Gabexate Mesilate (FOY) should have the following advantages: lack of antigenicity, low molecular weight with better cellular penetrance, wide inhibitory spectrum (against phospholipase A2 too) and good tolerability. In order to verify the efficacy of FOY versus, we decided to carry out a prospective multicenter (34 centers) randomized double blind trial in moderate-severe AP with FOY 3g/day versus A 1,500,000 U.I.K./day infused for 7 days with central venous line. Based on standard clinical, biochemical and radiological data, 199 patients suffering from AP were enrolled to the study from January 1989 to December 1990. The main admission criteria were: onset of the disease from no more than 72 hours, at least 2 Ranson positive criteria and C.T. evidence of acute pancreatic damage. Seventeen no protocol patients were excluded. The Etiology in the 182 analyzed patients (106 male, 76 female; mean age 57.6 years, range 21-91) were: biliary 102 (56%), alcohol 47 (25.8%), post-operative 13 (7.1%), unknown causes 13 (7.1%) and others 7 (3.8%). Median Ranson's Score was 3. CT examination showed 66 AP with edema, while 116 cases (63.7%) were recognized as severe forms with pancreatic necrotic involvement of 30% of the gland in 71 (61.2%), 50% in 31 (26.7%), and more than 50% in 14 (12.1%). Out of 182 patients, 91 were treated with A and 91 with FOY. The two groups were comparable regarding the main prognostic factors.(ABSTRACT TRUNCATED AT 250 WORDS)
