Abstract

Background: A growing body of evidence suggests that non-viral hepatocellular carcinoma (HCC) might benefit less from immunotherapy. Materials and methods: We carried out a retrospective analysis of prospectively collected data from consecutive patients with non-viral advanced HCC, treated with atezolizumab plus bevacizumab, lenvatinib, or sorafenib, in 36 centers in 4 countries (Italy, Japan, Republic of Korea, and UK). The primary endpoint was overall survival (OS) with atezolizumab plus bevacizumab versus lenvatinib. Secondary endpoints were progression-free survival (PFS) with atezolizumab plus bevacizumab versus lenvatinib, and OS and PFS with atezolizumab plus bevacizumab versus sorafenib. For the primary and secondary endpoints, we carried out the analysis on the whole population first, and then we divided the cohort into two groups: non-alcoholic fatty liver disease (NAFLD)/non-alcoholic steatohepatitis (NASH) population and non-NAFLD/NASH population. Results: One hundred and ninety patients received atezolizumab plus bevacizumab, 569 patients received lenvatinib, and 210 patients received sorafenib. In the whole population, multivariate analysis showed that treatment with lenvatinib was associated with a longer OS [hazard ratio (HR) 0.65; 95% confidence interval (CI) 0.44-0.95; P = 0.0268] and PFS (HR 0.67; 95% CI 0.51-0.86; P = 0.002) compared to atezolizumab plus bevacizumab. In the NAFLD/NASH population, multivariate analysis confirmed that lenvatinib treatment was associated with a longer OS (HR 0.46; 95% CI 0.26-0.84; P = 0.0110) and PFS (HR 0.55; 95% CI 0.38-0.82; P = 0.031) compared to atezolizumab plus bevacizumab. In the subgroup of non-NAFLD/NASH patients, no difference in OS or PFS was observed between patients treated with lenvatinib and those treated with atezolizumab plus bevacizumab. All these results were confirmed following propensity score matching analysis. By comparing patients receiving atezolizumab plus bevacizumab versus sorafenib, no statistically significant difference in survival was observed. Conclusions: The present analysis conducted on a large number of advanced non-viral HCC patients showed for the first time that treatment with lenvatinib is associated with a significant survival benefit compared to atezolizumab plus bevacizumab, in particular in patients with NAFLD/NASH-related HCC.

Abstract

Aim: To evaluate the impact of antiplatelet therapy (APT)on the incidence of hepatocellular carcinoma (HCC) and mortality following its treatment. Methods: A systematic literature search was performed using PubMed and Cochrane Central Register of Controlled Trials Databases. Two HCC clinical settings were explored: (i) incidence, and (ii) death after any HCC treatment. Odds ratios (OR) and 95% confidence intervals (95%CI) were calculated to compare the pooled data between patients who received or did not receive APT. Results: A total of 20 studies were identified, of whom 15 focused on HCC incidence, including 2,685,009 patients, and five on post‐treatment death, including 3281 patients. APT was associated with an overall reduced risk of HCC incidence (OR: 0.63; 95%CI = 0.51–0.79; p < 0.001) as well as of post‐treatment mortality (OR: 0.54; 95%CI = 0.35–0.83; p = 0.006). Conclusions: Current data suggest that APT correlated with higher HCC incidence and poor overall survival following tumour treatment. [ABSTRACT FROM AUTHOR]

Abstract

Background & aims: The effectiveness of direct-acting antivirals (DAAs) against hepatitis C virus (HCV) after successful treatment of early hepatocellular carcinoma (HCC) has been studied extensively. However, the benefit in terms of overall survival (OS) remains to be conclusively demonstrated. The aim of this study was to assess the impact of DAAs on OS, HCC recurrence, and hepatic decompensation. Methods: We enrolled prospectively 163 consecutive patients with HCV-related cirrhosis and at first diagnosis of early Barcelona Clinic Liver Cancer (BCLC) 0/A HCC who had achieved a complete radiologic response after curative resection or ablation, subsequently treated with DAAs. DAA-untreated patients from ITA.LI.CA. cohort (n = 328) served as controls. After propensity-score matching, outcomes of 102 DAA-treated (DAA group) and 102 DAA-untreated patients (No DAA group) were compared. Results: In DAA group, 7/102 patients (6.9%) died, HCC recurred in 28/102 patients (27.5%) and hepatic decompensation occurred in 6/102 patients (5.9%), after a mean follow-up of 21.4 months. OS was significantly higher in DAA group compared with No DAA group (hazard ratio [HR] = 0.39; 95% confidence Interval [CI] = 0.17–0.91, p = 0.03). HCC recurrence was not significantly different between DAA and No DAA groups (HR = 0.70; 95%CI = 0.44–1.13, p = 0.15). A significant reduction in the rate of hepatic decompensation was observed in DAA group compared with No DAA group (HR = 0.32; 95%CI = 0.13-0.84, p = 0.02). In DAA group, sustained virologic response was a significant predictor of overall survival (HR 0.02, 95%CI 0.00–0.19, p < 0.001), HCC recurrence (HR 0.25, 95%CI 0.11–0.57, p < 0.001) and hepatic decompensation (HR 0.12, 95%CI 0.02-0.38, p = 0.02). Conclusions: In patients with HCV-related cirrhosis and previous successful treatment of early HCC, DAAs significantly improved OS compared with No DAA treatment.

One of the first analysis items is the study of the f radiative decays, which allows us to investigate the nature of light pseudoscalar and scalar mesons. An integrated luminosity of 100 pb−1 is expected for next KLOE run, resulting in 3 × 108 f collected. A detailed simulation and full event reconstruction of both signal and background events indicates a sensitivity reach of 1 ÷ 2% on the branching ratios. Some preliminary studies on real data are also presented, showing a good agreement with MonteCarlo distributions

Abstract

We have measured the ratio R = Γ ( K L → γγ )/ Γ ( K L → π 0 π 0 π 0 ) using the KLOE detector. From a sample of ∼10 9 φ -mesons produced at DAΦNE, the Frascati φ -factory, we select ∼1.6×10 8 K L -mesons tagged by observing K S → π + π − following the reaction e + e − → φ → K L K S . From this sample we select 27,375 K L → γγ events and obtain R =(2.79±0.02 stat ±0.02 syst )×10 −3 . Using the world average value for BR( K L → π 0 π 0 π 0 ), we obtain BR( K L → γγ )=(5.89±0.07±0.08)×10 −4 where the second error is due to the uncertainty on the π 0 π 0 π 0 branching fraction.

Abstract

We have measured the branching ratio BR( φ → π 0 π 0 γ ) with the KLOE detector using a sample of ∼5×10 7 φ decays. φ mesons are produced at DA Φ NE, the Frascati φ -factory. We find BR( φ → π 0 π 0 γ )=(1.09±0.03 stat ±0.05 syst )×10 −4 . We fit the two-pion mass spectrum to models to disentangle contributions from various sources.

Abstract

We present a measurement of the branching ratio BR(KS --> pi e nu) performed using the KLOE detector. KS mesons are produced in the reaction e+ e- --> phi --> KS KL at the DAFNE collider. In a sample of about 5 million KS-tagged events we find 624 +- 30 semileptonic KS decays. Normalizing to the KS --> pi+ pi- count in the same data sample, we obtain BR(KS --> pi e nu) = (6.91 +- 0.37) 10^-4, in agreement with the Standard Model expectation., Comment: 9 pages, 5 Encapsulated Postscript figures. Abstract

The KLOE detector, mainly designed to measure the R ( ϵ′ ϵ ) parameter of CP violation in KL decay to an absolute precision of ≈ 10−4, is completed and will start data taking by spring 1999 at the e+ e− Φ-factory DAΦNE in Frascati. The primary K-meson physics and the experimental requirements at DAΦNE are summarized. The design strategies, the salient features and the main physics performances of all components of the experimental apparatus are reviewed.

The KLOE experiment has been collecting data since april 1999 at the DAΦNE collider in Frascati. A statistics of about 0.45 f b −1 has been analyzed. The latest results concerning the study of the ϕ radiative decays and of the kaon decays are illustrated in this paper.

Abstract

The KLOE electromagnetic calorimeter processes signals arriving over an interval of ≈200 ns before the trigger time. The KLOE ADC is based on two continuously analog baseline subtraction systems. A high stability monostable circuit is used at the input of the TDC operating in common start mode.

Abstract

The Data Acquisition system for the KLOE experiment, presently running at the Laboratori Nazionali di Frascati DAΦNE collider, has been designed to sustain an acquisition throughput of 50 Mbyte / s for an event rate of 10 kHz . Its two major components are the front end data readout, based on custom buses, and a complex network of computers and storage devices hosting a set of distributed processes. The end result is a seamless data transport from the readout system to the storage library, accompanied by concurrent on line calibrations and data quality control.

Abstract

We have measured the ratio R=Gamma(K_L -> gamma gamma)/ \Gamma(K_L -> 3 pi^0) using the KLOE detector. From a sample of ~ 10^9 phi-mesons produced at DAFNE, the Frascati phi-factory, we select ~ 1.6 10^8 K_L-mesons tagged by observing K_S -> pi^+ pi^- following the reaction e^+ e^- -> phi -> K_L K_S. From this sample we select 27,375 K_L -> gamma gamma events and obtain R = (2.79 \pm 0.02_{stat} \pm 0.02_{syst}) \times 10^{-3}. Using the world average value for BR(K_{L} -> 3 pi^0), we obtain BR(K_{L} -> gamma gamma) = (5.89 \pm 0.07 \pm 0.08) \times 10^{-4} where the second error is due to the uncertainty on the 3 pi^0 branching fraction.

Abstract

We report the results on the radiative decays phi --> eta'gamma, phi --> pi(0)pi(0)gamma and phi --> etapi(0)gamma obtained with the KLOE detector at the DAPhiNE phi-factory, using a sample of similar to 16 pb(-1). The value of the branching ratio for the phi --> eta'gamma decay is related to the s (s) over bar and gluon content of the eta'. A fit to the two-pseudoscalar invariant mass for the pi(0)pi(0)gamma and etapi(0)gamma final states gives the contribution of the direct terms phi --> f(0)gamma, phi --> sigmagamma and phi --> a(0)gamma, allowing us to investigate the nature of these light scalar mesons. Abstract

We present a study of the reaction e+e- --> pi+pi-pi0 at the phi peak W=M(phi)=1019.4 MeV observed with the KLOE detector at Dafne. The reaction is dominated by phi production and decay, e+e- --> phi --> pi+pi-pi0. From a fit to the Dalitz plot density distribution we obtain the rho-meson parameters for its three charge states. We also find the relative amplitudes for phi --> rhopi and phi -> pi+pi-pi0 and the cross-section for e+e- --> omega pi0 with omega --> pi+pi-.

Abstract

In the fixed energy environment of the $e^{+}e^{-}$ collider DA$\Phi$NE, KLOE can measure the cross section of the process $e^{+}e^{-} \to$ hadrons as a function of the hadronic system energy using the radiative return. At energies below 1 GeV, $e^{+}e^{-} \to \rho \to \pi^{+}\pi^{-}$ is the dominating hadronic process. Abstract

We have measured the radiative decays phi --> etagamma, phi --> eta'gamma selecting pi(+)pi(-)gammagammagamma final state in a sample of similar to 5 x 10(7) phi-mesons produced at the Frascati phi-factory DAPhiNE. We obtain Gamma(phi --> eta'gamma)/Gamma(phi --> etagamma) = (4.70 +/- 0.47 +/- 0.31) x 10(-3). From this result we derive new accurate values for the branching ratio BR(phi --> eta'gamma) = (6.10 +/- 0.61 +/- 0.43) x 10(-5) and the mixing angle of pseudoscalar mesons in the flavour basis phi(P) = (41.8(-1.6)(+1.9))degrees.

Abstract

The stringent requirements of the KLOE experiment on dimensions, efficiency, resolution and transparency of its drift chamber led to building a tracking device which can be considered innovative from many points of view. Details of the design and construction of this detector, together with preliminary results from the test of its full scale prototype, constructed and operated under a test beam at CERN, are presented and discussed.

Abstract

The KLOE detector was designed primarily For the study of CP violation in neutral kaon decays at DAPhiNE, the Frascati Phi-factory. The detector consists of a tracker and an electromagnetic calorimeter. A lead-scintillating-fiber sampling calorimeter satisfies best the requirements of the experiment. providing adequate energy resolution and superior timing accuracy. We describe in the following the construction of the calorimeter, its calibration and how the calorimeter information is used to obtain energy, point of entry and time of the arrival of photons, electrons and charged particles. With e(+)e(-) collision data at DAPhiNE for an integrated luminosity of some 2 pb(-1) we find for electromagnetic showers, an energy resolution of 5.7%/rootE(GeV) and a time resolution of 54/rootE(GeV) ps. Abstract

The design and construction of the large Drift Chamber for the KLOE experiment at the Frascati φ-factory, DAΦNE, are described. The relevant aspects of the various elements of the detector are reviewed together with a description of the track reconstruction program and of the calibration procedures. The performance of the detector based on measurements with cosmic rays and with e+e− colliding beams during DAΦNE commissioning is presented.

Abstract

The quadrupole tile calorimeters of KLOE (QCAL) are two compact detectors placed close to the interaction point and surrounding the focusing quadrupoles. Their purpose is to increase the hermeticity of KLOE calorimetry. Each QCAL consists of a sampling structure of lead plates and scintillator tiles with wavelength shifter (WLS) fibers and mesh photomultiplier readout arranged in 16 azimuthal sectors. The arrangement of WLS fibers allows the measurement of the longitudinal position of the showers from time of flight. In this paper we describe the QCAL design and assembly and present preliminary results obtained with both cosmic rays and photons from KL decays. The time and energy calibration procedures are also discussed in detail.

Abstract

We have measured the branching ratio BR(phi-->pi(0)pi(0)gamma) with the KLOE detector using a sample of similar to5x10(7) phi decays. phi mesons are produced at DAPhiNE, the Frascati phi-factory. We find BR(phi-->pi(0)pi(0)gamma) = (1.09+/-0.03(stat)+/-0.05(syst)) x 10(-4). We fit the two-pion mass spectrum to models to disentangle contributions from various sources. Abstract

We have measured the ratio R-S(pi) = Gamma(K-S --> pi(+)pi(-)(gamma))/Gamma(K-S --> pi(0)pi(0)) with the KLOE detector at the DAPhiNE e(+)e(-) collider. This measurement is fully inclusive with respect to the pi(+)pi(-)gamma final state. The sample of over 106 two-pion decays of tagged K-S mesons allows a statistical error as low as similar to 0.1% to be obtained. The accuracy is limited by systematic uncertainties, which are estimated primarily from data. We find R-S(pi) = 2.236 +/- 0.003(stat) +/- 0.015(syst).

Abstract

A double-level trigger system has been developed for the KLOE experiment. Custom electronics asserts a trigger in a 2 mus decision time. The decision is based on the combined information of the electromagnetic calorimeter and the drift chamber. The entire trigger system is continuously monitored, and data flowing from the trigger system hal e allowed both an efficient online monitoring of the detector and an online luminosity measurement. Abstract

The main aim of the KLOE experiment at DA Phi NE, the Frascati phi -factory, is to study CP violation in the K-0-(K) over bar (0) system. Requirements on shower detection are very stringent. An hermetic, lead-scintillating fiber sampling calorimeter has been chosen and built. A review of the methods used to calibrate and reconstruct energy and timing is reported in this paper. Emphasis is given to the calibration procedures developed using the 2.4 pb(-1) collected in 1999. An energy resolution of 5.7% rootE/GeV is achieved together with a linearity in energy response better than 1% above 50 MeV. A time resolution of similar to 54 ps/ rootE/GeV is also measured on samples of radiative Bhabha and Phi decays. (C) 2001 Elsevier Science B.V. Ail rights reserved.

Abstract

KLOE is acquiring and analyzing hundreds of terabytes of data, stored as tens of millions of files. In order to simplify the access to these files, a URI-based mechanism has been put in place. The KID package is an implementation of that mechanism and is presented in this paper.

Abstract

The KLOE experiment's major physics goal is to measure the R( ϵ′ ϵ ) , with a precision of 10−4, via both the traditional double ratio method and the kaons' quantum interferometry. The experiment has started taking data at DAfNE, the f-factory at the Frascati National Laboratory (LNF) of INFN in Italy, beginning in April 1999. Integrated luminosity exceeding 100 pb−1 (108 f's) are expected to be delivered for end 1999/early 2000. A description of the detector's design and of its performances is presented. At the moment of writing the experiment has collected its first collisions for an integrated luminosity of 220 nb−1; the peak luminosity reached has been of 2 × 1030 cm−2sec−1.

Abstract

KLOE, a new state of the art detector recently commissioned for physics operation at DAFNE, has many innovative interesting features, especially in the DAQ and Trigger areas. Custom electronics assert a trigger in a 2 microseconds decision time and distributes it to the FEE with a 50 ps time resolution. Data are read out using 10 front-end data acquisition chains and sent to a farm of online servers through a FDDI Gigaswitch. This design of the KLOE DAQ system allows us to manage an input data rate as high as 50 Mbyte/s, and is completely scalable by extending the number of computers connected to the switch

42. Predicting survival in patients with hepatocellular carcinoma treated by transarterial chemoembolisation

Abstract

Transarterial chemoembolisation (TACE) is first-line treatment in unresectable hepatocellular carcinoma (HCC) and rescue treatment after failure of radical treatments in early stage HCC. Prognostic tools for HCC using time-fixed Cox models may be unreliable in patients treated with TACE because time-varying predictors interact.To explore time-dependent variables as survival predictors in patients with HCC receiving TACE as first-line or second-line treatment.Eighty four consecutive patients with HCC (mean age 68; male gender 62%; Child-Pugh class: A n=73, B n=11; Barcelona Clinic Liver Cancer class: A n=44, B n=24, C n=16) treated with TACE were enrolled. Clinical, laboratory and radiological follow-up data were collected from the time of first treatment. Time-fixed and time-dependent Cox analyses were done.Overall survival rates were 89.6% (95% CI 82.5-97.2) at 12months, 58.8% (95% CI 46.2-74.9) at 24, 35.4% (95% CI 22.3-56.1) at 36 and 17.2% (95% CI 7.0-41.7) at 48months. Performance status (P0.001), number of nodules (P0.016) and prior therapy (P=0.017) were the only variables strongly linked to survival by time-fixed Cox model. Performance status (P0.001), prior therapy (P=0.005), number of treatments (P=0.013), complete response after TACE (P=0.005) and bilirubin level (P0.001) were associated with survival using a time-dependent Cox model.Survival after TACE is influenced most by performance status, complete response and bilirubin. Compared with the time-fixed models, a time-dependent Cox model has the potential to estimate a more precise prognosis in HCC patients treated with TACE.

Abstract

Hepatocellular carcinoma (HCC) is a challenging malignancy of global importance, it is associated with a high rate of mortality and its prevalence in the United States and in Western Europe is increasing. Cirrhosis is the strongest and the most common known risk factor for hepatocellular carcinoma, particularly cirrhosis related to hepatitis C virus (HCV) and hepatitis B virus (HBV) infections. The stage of cancer dictates the therapeutic choice, making early detection a primary objective. Early diagnosis of hepatocellular carcinoma is feasible because HCC develops in the background of well-known, readily identifiable and potentially avoidable environmental risk factors. Many observational studies have reported that HCC is diagnosed at an earlier stage in patients who received surveillance. Current guidelines advocate the use of abdominal ultrasound (US) at 6-12 months frequency to screen for HCC in high-risk patients. The use of AFP alone is strongly discouraged, and its use in addition to US is controversial. Patients with abnormal screening tests require additional investigation. Although the optimal methods of screening and the cost-effectiveness of surveillance for HCC remain to be established, systematic screening still offers the best hope for early diagnosis, treatment eligibility, and improved survival.

Abstract

A major problem in assessing the likelihood of survival of patients with hepatocellular carcinoma (HCC) arises from a lack of models capable of predicting outcome accurately.To compare the ability of the Italian score (CLIP), the French classification (GRETCH) and the Barcelona (BCLC) staging system in predicting survival in patients with HCC.We included 406 consecutive patients with cirrhosis and HCC. Seventy-eight per cent of patients had hepatitis C. Independent predictors of survival were identified using the Cox model.One-hundred and seventy-eight patients were treated, while 228 were untreated. The observed mortality was 60.1% in treated patients and 84.9% in untreated patients. Among treated patients, albumin, bilirubin and performance status were the only independent variables significantly associated with survival. Mortality was independently predicted by bilirubin, alpha-fetoprotein and portal vein thrombosis in untreated patients. CLIP achieved the best discriminative capacity in the entire HCC cohort and in the advanced untreatable cases, while BCLC was the ablest in predicting survival in treated patients.Overall predictive ability of BCLC, CLIP and GRETCH staging systems was not satisfactory, and was not uniform for treated patients and untreated patients. None of the scoring systems provided confident prediction of survival in individual patients.

AIM: Radio-frequency thermal ablation (RFTA) may prolong the survival of patients with small hepatocellular carcinoma (HCC) associated with cirrhosis. The aim of this study was to evaluate efficacy and safety of RFTA. METHODS: We performed the Kaplan-Meier analysis to estimate the survival rate in 69 consecutive patients with HCC (mean age 66+/-6.5 years; 44/25 male/female; 56 Child-Pugh class A and 13 Child-Pugh class B) treated by RFTA. Abstract

We have measured the ratio Rsπ= Γ(KS→π+ π- (γ))/Γ(KS→π0 π 0) with the KLOE detector at the DAFNE e+e- collider. This measurement is fully inclusive with respect to the pi+pi-gamma final state. The sample of over 10^6 two-pion decays of tagged Ks mesons allows a statistical error as low as 0.1% to be obtained. The accuracy is limited by systematic uncertainties, which are estimated primarily from data. We find R=2.236 +/- 0.003(stat) +/- 0.015(syst).
