Abstract

Abstract The use of instrumental tools for improving both the diagnostic accuracy and the prognostic soundness in patients with disorders of consciousness (DOC) plays an important role. However, the most recent international guidelines on DOC published by the American and the European Academies of Neurology and by the UK Royal College of Physicians contain heterogeneous recommendations on the implementation of these techniques in the clinical routine for both diagnosis and prognosis. With the present work, starting from the comparison of the DOC guidelines’ recommendations, we look for possible explanations behind such discrepancies considering the adopted methodologies and the reference health systems that could have affected the guidelines’ perspectives. We made a provocative argument about the need to find the most appropriate common methodology to retrieve and grade the evidence, increase the meta-analytic studies, and reduce the health policies that influence on the guidelines development that, in turn, should inform the health policies with the strongest scientific evidence.

2. Aqueous aluminium titanate paste for the liquid deposition modelling

Abstract

Advanced ceramic pastes, based on precursor of aluminium titanate (tialite, Al2TiO5), were prepared in two steps by ball milling starting from an aqueous slurry and with the addition of a thickener. Rheological behaviour of the prepared slurries and pastes with different formulations were analysed up to identify a tialite based paste with shear thinning behaviour and yield stress value suitable to be 3D printed by Liquid Deposition Modeling (LDM). In particular, printable pastes with a solid content up to 51 vol% were prepared. Shaping of aluminium titanate green bodies was realized both by LDM and conventional pressing to compare the properties of the sintered materials. Both shaping processes lead to a porous tialite with density value of 67.6 ± 0.7% for 3D printed tialite and of 75.6 ± 0.1% for pressed tialite. Furthermore, the morphology and thermogravimetric analysis of ceramic powder before and after ball milling were performed. Physico-chemical, microstructure and thermomechanical characterization of the obtained sintered material demonstrated that the LDM technique can be used to manufacture tialite starting from a suitable formulation of the ceramic paste. Moreover, the thermomechanical tests confirmed higher mechanical strength for pressed tialite; after a thermal shock treatment at 400 °C both 3D printed and pressed tialite shown a moderate decrease of mechanical properties.

Abstract

Sleep disorders are among the main comorbidities in patients with a Disorder of Consciousness (DOC). Given the key role of sleep in neural and cognitive functioning, detecting and treating sleep disorders in DOCs might be an effective therapeutic strategy to boost consciousness recovery and levels of awareness. To date, no systematic reviews have been conducted that explore the effect of sleep treatments in DOCs; thus, we systematically reviewed the existing studies on both pharmacological and non-pharmacological treatments for sleep disorders in DOCs. Among 2267 assessed articles, only 7 were included in the systematic review. The studies focused on two sleep disorder categories (sleep-related breathing disorders and circadian rhythm dysregulation) treated with both pharmacological (Modafinil and Intrathecal Baclofen) and non-pharmacological (positive airway pressure, bright light stimulation, and central thalamic deep brain stimulation) interventions. Although the limited number of studies and their heterogeneity do not allow generalized conclusions, all the studies highlighted the effectiveness of treatments on both sleep disorders and levels of awareness. For this reason, clinical and diagnostic evaluations able to detect sleep disorders in DOC patients should be adopted in the clinical routine for the purpose of intervening promptly with the most appropriate treatment.

Abstract

BackgroundGlobally nearly 20% of cardiovascular disease deaths were attributable to air pollution. Out-of-hospital cardiac arrest (OHCA) represents a major public health problem, therefore, the identification of novel OHCA triggers is of crucial relevance. The aim of the study was to evaluate the association between air pollution (short-, mid-and long-term exposure) and out-of-hospital cardiac arrest (OHCA) risk, during a 7 years-period from a highly polluted urban area with a high density of automated external defibrillators (AEDs).Methods and resultsOHCA were prospectively collected from the “Progetto Vita Database” between 01/01/2010 to 31/12/2017; day-by-day air pollution levels were extracted from the Environmental Protection Agency (ARPA) stations. Electrocardiograms of OHCA interventions were collected from the AEDs data cards. Day-by-day particulate matter (PM) 2.5 and 10, ozone (O3), carbon monoxide (CO) and nitrogen dioxide (NO2) levels were measured. A total of 880 OHCAs occurred in 748 days. A significantly increased in OHCA risk with the progressive increase in PM 2.5, PM 10, CO and NO2 levels was found. After adjustment for temperature and seasons, a 9% and 12% increase of OHCA risk for each 10 μg/m3 increase of PM 10 (p< 0.0001) and PM 2.5 (p< 0.0001) levels was found. Air pollutants levels were associated with both asystole and shockable rhythm risk while no correlation was found with pulseless electrical activity.ConclusionsShort-term and mid-term exposure to PM 2.5 and PM 10 is independently associated with the risk of OHCA due to asystole or shockable rhythm.

5. Clinical parameters and cardiovascular risk factors related to heart failure with preserved ejection fraction: a comparative analysis between HFA-PEFF and H2FPEF Scores

Abstract

Background Heart failure with preserved ejection fraction (HFpEF) diagnosis remains challenging, since several mechanisms (diastolic and systolic reserve abnormalities, chronotropic incompetence, ventricular or vascular stiffening, atrial dysfunction, pulmonary hypertension, impaired vasodilation, endothelial dysfunction, energetic abnormalities and autonomic dysfunction) play different roles in HFpEF development. European Society of Cardiology HF guidelines recently suggested a stepwise non-invasive diagnostic approach consisting of three steps: the first is clinical, the second includes echocardiographic and laboratory data (natriuretic peptides), named HFA-PEEF score, and finally, in case of inconclusive findings, diastolic stress echocardiography is recommended. On the other hand, in United States, another multiparametric score, named H2FPEF, has been proposed for HFpEF diagnosis, and including, in addition to echocardiographic parameters, also clinical data; thereby more applicable in the outpatient clinical arena. Purpose Whether there is a clinical overlap between the two scores (HFA-PEEF and H2FPEF) as well as whether the addition of clinical data to the HFA-PEEF could improve its ability to identify different HFpEF phenotypes is still an open issue and these were the aims of our study. Methods HFA-PEEF and H2FPEF scores were systematically applied on 1,156 consecutive subjects with preserved ejection fraction who undergone cardiovascular evaluation at the Cardiovascular Prevention Center of Fondazione Don Gnocchi & University of Parma. All subjects underwent cardiovascular risk assessment followed by echocardiography and cardiopulmonary exercise testing; due to the outpatient (non-acute) setting of the evaluation, natriuretic peptides assay was not performed. Clinical data and cardiovascular risk factors data were compared between different groups of HFpEF risk. Results According to H2FPEF score, low risk (75%) in 197 (17%); according to HFA-PEEF score, 675 (58%) had 0 or 1 point, 253 (22%) had 2 points and 230 (20%) had 3 or 4 points (moderate-to-high risk). Patients with higher HFA-PEEF score were older (p75 years to a 4.6-fold higher risk, HTN to a 3.6-fold higher risk (p Conclusions Although HFA-PEEF score does not include clinical data, patients with older age, atrial fibrillation, hypertension, hypertensive heart, diabetes, sedentary lifestyle and chronic coronary syndrome show a higher ESC risk of HFpEF. 6. Prevalence and outcomes of peripheral artery disease in a real-world cohort of patients with acute coronary syndrome: insights from the prospective SPUM registry

Abstract

Background Despite substantial improvement in secondary preventive therapy, the burden of recurrent cardiovascular (CV) events remains high. Peripheral artery disease (PAD), is a potential marker of increased residual ischemic risk in acute coronary syndrome (ACS) patients suggesting greater net clinical benefit from intensified individualized therapy. Purpose We aimed to assess the prevalence and cardiovascular (CV) outcomes of peripheral arterial disease (PAD) in 4'787 patients with acute coronary syndromes (ACS) in the Swiss prospective multicenter SPUM study. Methods PAD was defined according to international guidelines. The composite primary endpoint was major adverse cardiac and cerebrovascular events (MACCE), i.e., stroke, myocardial infarction (MI) and death at 1 year. Adjusted Cox proportional hazards regression models were used to estimate the risk associated with PAD and outcomes. Results Out of 4'787 ACS patients, 285 patients (6.0%) had PAD. These patients were older (70.1±10.6 vs 63.3±12.4, p Conclusions Among a real-world cohort of ACS patients, the coexistence of PAD, a very-high CV risk phenotype, is associated with significantly increased rates of MACCE, but no difference in major bleeding events. These observations might help clinicians to further stratify this very-high risk population and to identify patients who may derive the greatest benefit from more intense secondary prevention therapies. Background Exercise intolerance evaluation in Heart failure with preserved ejection fraction (HFpEF) remains challenging, since several mechanisms (diastolic and systolic reserve abnormalities, low chronotropic reserve (CR), ventricular or vascular stiffening, atrial dysfunction, pulmonary hypertension, endothelial dysfunction, energetic abnormalities and autonomic dysfunction) play different roles. European Society of Cardiology HF guidelines recently suggested a stepwise non-invasive HFpEF diagnostic approach consisting of three steps: clinical, echocardiographic and laboratory data (natriuretic peptides), named HFA-PEEF Score, and finally, in case of inconclusive findings, diastolic stress echocardiography data. Cardiopulmonary exercise testing (CPET) may represent a promising further non-invasive diagnostic tool in HFpEF evaluation since allow to assess the presence of reduced functional capacity as well as to differentiate between cardiovascular, ventilatory or peripheral causes. Purpose Whether increased risk of HFpEF is associated with different and specific cardiopulmonary responses to exercise is still an open issue and this was the aim of our study. Methods 1.156 consecutive subjects with preserved ejection fraction undergoing cardiovascular evaluation at the Cardiovascular Prevention Center of Fondazione Don Gnocchi & University of Parma were enrolled. All subjects underwent cardiovascular evaluation and echocardiography, HFA-PEEF and H2FPEF Score assessment and cardiopulmonary exercise testing. Different cardiopulmonary response to exercise were compared between different groups of HFpEF risk. Results According to HFA-PEEF Score, 675 (58%) had 0 or 1 point, 253 (22%) had 2 points and 230 (20%) had 3 or 4 points (moderate-to-high risk). Patients with both higher HFA-PEEF and H2FPEF Score showed lower functional capacity, expressed as low peak V02 (p35) to a 2.4-fold higher risk (p Conclusions Different degrees HFpEF risk, estimated using both HFA-PEEF and H2FPEF score, are associated with different cardiopulmonary responses to exercise. High HFpEF risk patients show low functional capacity, cardiovascular and ventilator efficiency due to lower cardiac output at peak, despite preserved ejection fraction, associated to lower chronotropic response to exercise. 9. P56 SPONTANEOUS CORONARY ARTERY DISSECTIONS: ANALYSIS OF NON TRADITIONAL RISK FACTORS

Abstract

The etiology of spontaneous coronary dissection (SCAD) is not well defined and Non traditional risk factors (NT–RF) have assumed increasing interest, but few data are available. NT–RF include three categories: Sex–related (SR–NT–FR), Sex–predominant (SP–NT–RF) and Gender–related (GR–NT–RF). (Table 1) Aim of the Study The objective of our analysis was to evaluate the incidence of NT–RF in Parma SCAD registry population. Material and methods We reviewed 62 patients with SCAD enrolled between January 2013 through November 2021 Results Traditional risk factors were less common: hypertension was the most prevalent (39 pts, 62.9%). When considering NT–RF, 51 patients (82%) had at least one of all, with at least one SR–RF (66%) or GR–RF (64,5%). Patients with NT–RF were younger at time of SCAD (mean age 53 vs 66; p = 0.027) and they were predominantly females (48 vs 7 pts, p = 0.004) (Table 2). No differences were found among NT–RF SCAD and nNT–RF SCAD patients by fibromuscular dysplasia, peripheral arterial disease and chronic kidney disease. Patients with SCAD more often presented with non ST–segment elevation myocardial infarction (43 pts, 72.6%) vs ST–segment elevation (17 pts, 27.4%). No differences in clinical presentation and angiographic characteristics were found among NT–RF and nNT–RF patients group. MACE occurred in 17.7% of patients of the overall study population, at a median follow–up of 23 (interquartile range: 11;57) months. When comparing the incidence of cardiovascular events in the 2 study groups there was a trend toward a higher prevalence of MACE in NT–RF group without statistical significance (NT–RF SCAD 19.6% – nNT–RF SCAD 9.1%; p = 0.4). (Table 3) Conclusion SCAD is an emerging cause of myocardial infarction in young and middle–aged women without the traditional cardiovascular risk profile. Although overall survival seems good, SCAD is a potentially malignant disease which can present with ventricular arrhythmias and sudden cardiac death. Risk estimation is difficult in women, due to the scarce validity of prediction models, therefore a great effort must be made by the clinical community for the widespread diffusion and use of models incorporating NT–RF. Acknowledgement of peculiar features of this disease could help clinicians and researchers to establish targeted interventions for cardiovascular primary prevention, early diagnosis and secondary prevention in women, including rehabilitation and stress management programmes.

Abstract

Objective: Sensory deficits are important risk factors for delirium but have been investigated in single-center studies and single clinical settings. This multicenter study aims to evaluate the association between hearing and visual impairment or bi-sensory impairment (visual and hearing impairment) and delirium. Design: Cross-sectional study nested in the 2017 "Delirium Day" project. Setting and participants: Patients 65 years and older admitted to acute hospital medical wards, emergency departments, rehabilitation wards, nursing homes, and hospices in Italy. Methods: Delirium was assessed with the 4AT (a short tool for delirium assessment) and sensory deficits with a clinical evaluation. We assessed the association between delirium, hearing and visual impairment in multivariable logistic regression models, adjusting for: Model 1, we included predisposing factors for delirium (ie, dementia, weight loss and autonomy in the activities of daily living); Model 2, we added to Model 1 variables, which could be considered precipitating factors for delirium (ie, psychoactive drugs and urinary catheters). Results: A total of 3038 patients were included; delirium prevalence was 25%. Patients with delirium had a higher prevalence of hearing impairment (30.5% vs 18%; P < .001), visual impairment (24.2% vs 15.7%; P < .01) and bi-sensory impairment (16.2% vs 7.5%) compared with those without delirium. In the multivariable logistic regression analysis, the presence of bi-sensory impairment was associated with delirium in Model 1 [odds ratio (OR) 1.5, confidence interval (CI) 1.2-2.1; P = .00] and in Model 2 (OR 1.4; CI 1.1-1.9; P = .02), whereas the presence of visual and hearing impairment alone was not associated with delirium either in Model 1 (OR 0.8; CI 0.6-1.2, P = .36; OR 1.1; CI 0.8-1.4; P = .42) or in Model 2 (OR 0.8, CI 0.6-1.2, P = .27; OR 1.1, CI 0.8-1.4, P = .63). Conclusions and implications: Our findings support the importance of routine screening and specific interventions by a multidisciplinary team to implement optimal management of sensory impairments and hence prevention and the management of the patients with delirium.

Abstract

Monitoring the level of responsiveness of patients with Disorders of Consciousness (DoCs) represents an issue in all the settings where there is not the daily presence of clinicians, such as long-term and domestic settings. The involvement of patients’ informal caregivers (i.e., patients’ family) in such a monitoring process is thus fundamental. However, to date, no standardized tailored-made instruments exist that informal caregivers can use without the presence of clinicians, despite evidence illustrating the good accuracy of caregivers when expressing their opinions about the level of responsiveness of DoC patients. The present work aims to set the foundational knowledge, to create a standardized instrument that is able to assess the level of responsiveness of patients with DoCs by their informal caregivers. After selecting and modifying the items to be included in the new scale from the gold standard to diagnose DoCs (i.e., the Coma Recovery Scale-revised), and following a consensus process, we created the Social and Family Evaluation (SAFE) scale for caregivers of patients with DoCs. Although the SAFE needs a validation process, with the present work we provided its preliminary description along with insights into its clinical utility.

Abstract

Recent studies suggest that COVID-19 survivors may experience long-term health consequences: in particular, neurological and mental health symptoms might be associated with long-term negative outcomes. This study is a secondary analysis of a larger cohort study and aims to determine the extent to which neurological and mental health sequelae are associated with survivors’ disability. Participants include COVID-19 survivors, with no pre-morbid brain conditions, who were discharged from the COVID-19 Unit of the ASST Spedali Civili Hospital between February and April 2020. At an average of 3.5 months after discharge, they were submitted to a neurological examination and completed the WHO Disability Assessment Schedule (WHODAS-12), the Hospital Anxiety and Depression Score, the Pittsburgh Sleep Quality Index and the Montreal Cognitive Assessment. Multivariable regression analysis was carried out to analyze variables that explain WHODAS-12 variation. In total, 83 patients (63 males, average age 66.9, 95% CI: 64.2–69.7) were enrolled; average WHODAS-12 was 13.2 (95% CI: 9.7–16.6). Cognitive dysfunction, anxiety, fatigue, and hyposmia/hypogeusia explained 28.8% of WHODAS-12 variation. These findings underline the importance and need for longitudinal follow-up assessments after recovery from COVID-19 and suggest the need for early rehabilitation of residual symptoms to enhance patients’ functioning.

13. Reference Governor Based Solution for Satellite Attitude Control with Sloshing and Actuation Constraints

Abstract

Satellites propellant mass can reach up to 40\\% of an entire spacecraft. Then, slosh phenomenon can be catastrophic if uncontrolled, resulting in complete system/mission failure. Passive physical solutions, such as baffles, compartmentalization or bladders are commonly employed to prevent high amplitude/low frequency propellant motion [1]. However, physical suppression techniques increase mass, complexity, and cost of the overall system. For these reasons, active control system solutions, capable of both precise attitude control and fuel slosh suppression, are of great interest and remain an active field of research. Novel slosh modeling approaches, including major contribution by ONERA and CNES in the framework of the joint research program COSOR (Control of Orbital \\ Robotic Systems) [2], paved the way towards enhanced attitude control solutions such as LPV-based observers, [3], and $H_{\\infty}$-based control, [4]. While the obtained results so far were very promising, reaction wheels saturations handling remained an unresolved issue for some critical tank fluid initial conditions. Indeed, to reject the sloshing disturbance, the high control action employed often leads the reaction wheels to reach their maximum angular speed. The resulting constant angular momentum causes a sudden drop of the commanded torque to zero and any wheel deceleration would evolve in an opposite direction control action. In this scenario, for large angle variations demands, while a rigid satellite will rotate uncontrolled until tracking error sign inversion, in presence of sloshing perturbations the system will rapidly diverge. This paper proposes a static Reference Governor (RG) based solution to address the actuator saturation constraints problem while ensuring tracking performance specifications in presence of sloshing. In its classic formulation, the RG is an add-on control scheme that filters the reference signal by solving at each time step a constrained linear optimization problem (LP). Employing as baseline the satellite with the slosh model derived in [2], we first highlight the limits of the observer-based control solution developed in [3] and the need of extended robustness to constraints. The benefits of integrating a Reference Governor scheme are later deeply discussed: the range of reachable amplitudes is greatly enlarged while the system failure is prevented. Simulation results and stability analysis are therefore illustrated on different scenarii, ranging from trivial to strong saturation cases.In addition, partly inspired by [5], further investigations are in progress to extend the current Reference Governor version to tackle different types of disturbances, including flexible appendices and manipulator arm perturbations. The lack of accurate disturbance models on the one hand, and the stringent attitude control system requirements on the other, lead to consider advanced RG techniques to be reported in the final version of the paper. The intent is to prevent saturation for a diverse class of disturbances while limiting the system conservatism as much as possible.

14. Satellite Inertia Estimation and Observability Analysis

Abstract

The inertia matrix typically varies during the satellite lifetime due to fuel consumption, possible collisions and damages. The importance of an accurate estimation arises especially for agile satellites, where control laws directly proportional to the inertia matrix are typically adopted to respond the stringent performance constraints on the on-board attitude control system. During the CNES mission MICROCARB test campaign, it has been highlighted that when performing the FIXED TARGET pointing maneuver, even though the control loop is robust enough to deal with an inertia relative error of up to 15%, ensuring it to be below 5% can significantly reduce the stabilization period and the oscillation frequencies. The purpose of the study is twofold. Firstly, to compare two inertia estimation methods and secondly, to study the observability of the input data to use for the estimation. The first estimation method, the Instrumental Variable (IV) [1], is a variation of the Least Square (LS) method that remains consistent for a class of noise signals which makes the LS estimates biased. Exactly as the LS, the IV method is based on a prediction model structure, which is linearly parameterized. However, in the IV cost function, a matrix Z (called the instrumental variable) is added in order to achieve unbiased estimates. The second method is based on an Unscented Kalman Filter [2], a derivative-free alternative to the Extended Kalman Filter (EKF), which uses a statistical linearization technique of a nonlinear function by linear regression. To do that, a set of chosen sample points called sigma-points are propagated through the true nonlinear system and the posterior mean and covariance are captured accurately to the second order for any nonlinearity. The performances of IV and UKF methods have been compared in terms of precision and robustness and validated using data from the MICROCARB AOCS Simulator. For missions that do not integrate maneuvers particularly designed to estimate the satellite inertia, a telemetry data analysis is crucial to find the most dynamically interesting moment of the satellite life to launch the estimation. For this purpose, four observability metrics (namely the A-Optimality, the D-Optimality, the E-Optimality, and the Condition Number criteria [3] [4]), integrated with qualitative data analysis, are tested on a typical MICROCARB day. Once the best range of the dataset is identified, the estimation is launched and it results that for both the IV and the UKF methods the relative error remains within 2% on the three principal axes parameters ????????????, ???????????? and ????????????. Finally, with the intent of trying to get the most from the available data, the observability is computed on each guidance mode of the MICROCARB day and the ten best ones are combined in a fictitious maneuver to be tested as input for the estimation algorithms. As a result of the whole research, a MATLAB tool named CALIBRI to study the input data observability and consequently estimate the satellite inertia has been added to the CNES heritage. Anthaxia (Haplanthaxia) parvula n. sp. from Iran is herein described and illustrated. It is compared to its most similar species, Anthaxia (Haplanthaxia) mundula Kiesenwetter 1857. Its peculiar pronotal punctuation, and other morphological features, suggest not to include this new species in any “species-group” so far estabilished.

Abstract

ObjectivesOur aim was to investigate the durability of different initial regimens in patients starting ART with CD4+ counts 5 log10 copies/mL.MethodsThis was a retrospective study of HIV-infected patients prospectively followed in the ICONA cohort. Those who started ART with boosted protease inhibitors (bPIs), NNRTIs or integrase strand transfer inhibitors (InSTIs), with CD4+ 5 log10 copies/mL, were included. The primary endpoint was treatment failure (TF), a composite endpoint defined as virological failure (VF, first of two consecutive HIV-RNA >50 copies/mL after 6 months of treatment), discontinuation of class of the anchor drug or death. Independent associations were investigated by Poisson regression analysis in a model including age, gender, mode of HIV transmission, CDC stage, HCV and HBV co-infection, pre-treatment HIV-RNA, CD4+ count and CD4+/CD8+ ratio, ongoing opportunistic disease, fibrosis FIB-4 index, estimated glomerular filtration rate, haemoglobin, platelets, neutrophils, calendar year of ART initiation, anchor drug class (treatment group) and nucleos(t)ide backbone.ResultsA total of 1195 patients fulfilled the inclusion criteria: 696 started ART with a bPI, 315 with an InSTI and 184 with an NNRTI. During 2759 person-years of follow up, 642 patients experienced TF. Starting ART with bPIs [adjusted incidence rate ratio (aIRR) (95% CI) 1.62 (1.29–2.03) versus starting with NNRTIs; P ConclusionsIn patients starting ART with 5 log10 HIV-RNA copies/mL, the durability of regimens based on InSTIs was longer than that of NNRTI- and bPI-based regimens.

Chemical Vapour Infiltration (CVI) technique has been long used to produce carbon/carbon composites. The Pyrolytic Carbon (Py-C) matrix infiltrated by CVI could have different microstructures, i.e. Rough Laminar (RL), Smooth Laminar (SL) or Isotropic (ISO). These matrix microstructures, characterized by different properties, influence the mechanical behaviour of the obtained composites. Tailoring the process parameters, it is possible to direct the infiltration towards a specific Py-C type. However, the factors, influencing the production of a specific matrix microstructure, are numerous and interconnected, e.g. temperature, pressure, flow rates etc. Due to the complexity of the physical and chemical phenomena involved in CVI process, up to now it has not been possible to obtain a general correlation between CVI process parameters and Py–C microstructure. This study is aimed at investigating the relationship between infiltration temperature and the microstructure of obtained Py-C, for a pilot - sized CVI/CVD reactor. Fixing the other process parameters and varying only the temperature, from 1100°C to 1300°C, the Py-C infiltration was performed on fibrous preforms. Polarized light microscopy, with quantitative measurements of average extinction angle (Ae), and Raman spectroscopy were used to characterize the obtained Py-C microstructures.

Abstract

OBJECTIVES The aim of the study was to analyse the prevalence of integrase resistance mutations in integrase strand transfer inhibitor (INSTI)-experienced HIV-1-infected patients and its predictors. METHODS We selected HIV-1 integrase sequences from the Antiviral Response Cohort Analysis (ARCA) database, derived from INSTI-experienced patients between 2008 and 2017. Differences in the prevalence of resistance to raltegravir (RAL), elvitegravir (EVG) and dolutegravir (DTG) were assessed by χ2 test and predictors of resistance were analysed by logistic regression. RESULTS We included 462 genotypes from INSTI-exposed individuals: 356 'INSTI-failing' patients and 106 'previously INSTI-exposed' patients (obtained a median of 42 weeks after INSTI discontinuation [interquartile range (IQR) 17-110 weeks]). Overall, at least low-level resistance (LLR) to any INSTI (Stanford 8.5 algorithm) was detected in 198 (42.9%) cases. The most frequent INSTI resistance mutation was N155H, followed by Q148H/K/R, G140A/C/S, E138A/K/T and Y143C/H/R. Y143R and E138A were more prevalent in viral subtype B versus non-B [5.2 versus 1.5%, respectively (P = 0.04), and 3.1 versus 0%, respectively (P = 0.02)]. Overall, the Q148H/K/R plus G140A/C/S and/or E138A/K/T pattern, defining an intermediate level of resistance to DTG, was detected in 70 (15%) cases. Independent predictors of at least LLR to any INSTI were current use versus past use of INSTIs, a lower genotypic sensitivity score (GSS) for contemporary antiretroviral drugs used, and having an integrase sequence obtained in calendar year 2016 as compared to 2008-2009. CONCLUSIONS The results support integrase resistance testing in INSTI-experienced patients. Emergence of INSTI resistance is facilitated by the reduced genetic barrier of the regimen as a consequence of resistance to companion drugs. However, INSTI resistance may become undetectable by standard population sequencing upon INSTI discontinuation.

Oggi il quadro appare in evoluzione, con dati di letteratura che permettono di prendere in considerazione prodotti non brevettati, relativamente economici, utilizzabili per lo sviluppo di specifiche applicazioni. Il progetto EEE-CFCC (www.eee-cfcc.it, finanziato dalla Regione Emilia-Romagna POR-FESR 2014-2020) si è focalizzato sulle attività di ricerca e sviluppo sui materiali compositi a matrice inorganica, per i settori trasporti e costruzioni, ed in particolare su quelli prodotti a partire da prepreg di basso costo.

Abstract

This scientific investigation on metal artwork is meant to expand knowledge regarding the technical skills developed by artists in sculpture manufacturing. Moreover all the gathered data support the speculation about the motivations behind the choices of certain material or a specific manufacturing method (e.g., economic or technical reasons, or both). The subject of this study is the Virtues sculptural group made at the end of the XVI century by Giambologna to decorate the Grimaldi Chapel in the church of San Francesco di Castelletto (Genoa, Italy). Six life-size statues depicting Charity, Justice, Hope, Fortitude, Faith, and Temperance (i.e., the artwork discussed in this article); seven bas-reliefs; and six winged representations of putti are what remains of the original monumental project. X-ray fluorescence, a nondestructive investigation method, was applied to determine the chemical nature of the metallic substrate and of the “artistic” and natural patinas. The achieved results allowed for distinguishing variations in the content of the major alloying elements that might correspond to a motivated will of the artist connected to technical and aesthetic aims: the production of defect-free and golden bronze artifacts.

Abstract

A multidisciplinary study of the prehistoric settlements in eastern Tunisia was initiated in 2002 in the coastal environment of the Hergla territory. Several archaeological surveys, mainly around the Sebkhet Halk el Menjel lagoon, resulted in the identification of many sites, among which the Holocene rammadiya (maghrebian open air site) of SHM-1. The anthropic stratigraphy of this site was established from several test-pits, one trench and an extensive excavation. A comparative study of the different sections and excavated surfaces revealed several major occupational horizons dated from 14C dating of the second half of the 7th millennium to the second half of the 6th millennium cal B.C. The economy of the site is still Epipalaeolithic, linked to hunting, fishing and gathering activities. The material culture is composed of a strong capsian substratum. Some evidences of innovations are recognized along the occupational phases, possibly linked to the initiation of contacts with distant communities, as materialized for instance by the presence of Pantellerian obsidians.

Abstract

Since its beginning, research about cognitive representation of our bodies has debated over multiple representations models. Furthermore, recent years have seen a rise in the study of body representation disorders and related impairments. However, why human beings manifest so many deficits is still a mystery. Considering human evolution, frontal brain regions are well known for their changes in dimensions and connections. Less known is that parietal and temporal lobes encountered similar changes. These areas, especially in the right hemisphere, are crucial for body representation. Our hypothesis is that evolution of these areas determined a more varied and widespread cross wiring between the temporal and parietal lobes, increasing their communication pathways and their reciprocal influence. As such, these connections could lead to an increased probability of interconnected body and emotional disorders in humans. The prediction of this hypothesis is that all body representation disorders have an associated emotional component and vice versa. Evidence supporting the interconnection between emotional and body representation disorders derives from psychiatric diseases such as eating disorders. This hypothesis opens up new directions to understand body representation and points towards innovative solutions for the clinical treatments of body representation/emotional impairments.

The subject of this study is the Virtues sculptural group made at the end of the XVI century by Giambologna to decorate the Grimaldi Chapel in the church of San Francesco di Castelletto (Genoa, Italy). Six life-size statues depicting Charity, Justice, Hope, Fortitude, Faith, and Temperance (i.e., the artwork discussed in this article); seven bas-reliefs; and six winged representations of putti are what remains of the original monumental project. Different micro-invasive analytical techniques (scanning electron microscopy and energy dispersive x-ray spectroscopy, SEM–EDS; micro-Raman Spectroscopy, μRS; gas chromatography–mass spectrometry, GC–MS) were applied to determine the chemical nature and the state of conservation of the “artistic” and natural patinas visible on the Virtues surfaces. Some patination tests were performed taking into account the collected analytical data and the indications provided by the scientific literature and the historical documents. The combination of the results obtained through the scientific investigations and the experimental work allowed for supposing the materials and the patination technique selected by the artist to adorn the sculptures and providing indications for the correct conservation of the six sculptures.

Abstract

The development of a new chromia-doped Zirconia Toughened Alumina (ZTA) material was previously reported as displaying mechanical properties suitable for implants with load bearing applications, such as orthopaedic and dental implants. This type of biomaterial is expected to be in contact with living tissues for a long period of time and its long-term toxicity must be carefully evaluated. In this study the suitability of this ZTA material as a candidate biomaterial for orthopaedic implants and dental devices was further investigated in vivo in comparison to alumina and zirconia, which are currently used in orthopaedic and dental surgery. Cylinders of the materials were implanted in vivo in white rabbits, and local and systemic tissue reactions were analyzed at different time intervals after surgery. Radiologic examinations displayed the absence of radiolucence around cylinders and no signs of implant loosening up to twelve months. No tumours developed in the animals either locally (at the site of implantation), or systemically in the peripheral organs. The results obtained suggest that this new ZTA material does not display any long term pathogenic effect in vivo. These findings extend our previous observations on the biocompatibility and the absence of any long-term carcinogenic effect in vitro of this material which displays interesting properties for biomedical applications. In conclusion, we report the in vivo characterization of a new chromia-doped ZTA material and confirm its suitability as a candidate biomaterial for orthopaedic implants and dental devices since it does not give any local nor systemic toxicity even after a long period of time after implantation.

Abstract

Exploration of the Moon is considered a high priority in the overall strategy of the European Space Agency (ESA), together with activities in Low Earth Orbit and missions to Mars. The Lunar South Polar region presents key opportunities for scientific discovery and advancement of exploration capabilities, but is also a uniquely challenging operational environment for robotic missions and for human explorers. ESA has placed accessing this environment, and the surface and subsurface material found there, as a high priority. In this context ESA is discussing with international partners, in particular with ROSCOSMOS, how these ambitions might be realised in cooperation. To support this work ESA is engaged in a range of mission level activities with ROSCOSMOS on the Luna-27/Luna-Resurs Lander mission as well as Lunar Polar Sample Return (LPSR). In addition to this ESA is undertaking, with European institutes and industries, a set of specific development activities which seek to address the challenges of operating in the environment of the Lunar South Pole. This paper will specifically discuss the ongoing work on sampling and handling icy materials, both for in-situ analysis (as planned for the Luna-27 mission) and for sample return (as a cooperation with the Luna-28 mission), considering aspects such as regolith penetration, sample extraction and preservation. The next steps shall also be presented, including the approach to requirements verification via early testing activities, component level breadboarding and full scale development model tests in laboratory environments and in the field.

Abstract

In order to improve knowledge on the origins of the universe, the perforation of the soil and the acquisition of the materials from planets and comets play a very important role. In the frame of research and development programs, different drill tool concepts have been studied, engineered and tested. Most of the developments are based on classical rotary drill approach with the implementation of appropriate sample collection mechanism; others utilises a novel concept based on frontal transversal rotary head which overcome the condition of zero speed. The flight version of the drilling tools is aimed at a larger penetration depth than the prototypes and can be designed either as a Single or Multi-Rod system. The Multi-Rod approach is a specially developed assembly technique of an appropriate number of drill rods. The drills can support direct down hole science by installing inside specific dedicated scientific instrumentation. This article provides an overview on the main features of the studied drill tools.

Abstract

Sticky fleabane, Inula viscosa, a wild perennial Asteracea widespread in the Mediterranean area, is characterized by high tolerance to water deficiency. In this study, plants of I. viscosa obtained from seedlings were grown in a controlled environment and irrigated from transplanting to flowering with salinized water at seven different NaCl concentrations (ECi = 0.5 to 52.1 dS m−1), in order to evaluate the salinity tolerance and the ion content (Na, Cl, N, P, K, Ca and Mg, as mg kg−1 of dry weight) within plant tissues (roots, basal stems, medium stems, apical stems, lateral shoots, basal and apical leaves). Data obtained showed for I. viscosa a marked tolerance to salt stress. A K/Na concentration ratio of 0.1 was in the tissues of the basal leaves, suggesting a mechanism of Na exclusion from the younger leaves and the apical stems. The results in the first growing year indicate that I. viscosa can grow and flower in saline conditions up to ECi = 52.1 dS m−1, even if it shows a marked reduction...

Abstract

At present, Europe lacks innovative companies, which grow quickly and end up big. This book examines SME growth, innovation and success, to suggest that fast-growing firms could offer a major contribution to the recovery of a European economy. Abstract

Deep drilling on Mars can provide soil samples taken from depth which are of great scientific interest for analysis by the instrumentation present on-board a mission vehicle. The key to a successful sample collection and delivery is the development of a performing and reliable drill tool with sample collection capabilities, properly integrated in a complete drilling system. Within this context, Galileo Avionica is involved in the design and development of drilling systems, including hardware prototyping and testing, suitable to operate in planetary and cometary environments. Several prototypes of drilling tools have been designed, manufactured and tested and new concepts are in the development pipeline. The testing so far performed indicate the feasibility of drill tools suitable to operate in very different types of soil and capable to reliably collect, recover and distribute samples to the possible scientific instrumentation. The preliminary design of integrated drill systems, both employing a single drill tool or multiple rod assembled during operation, shows their ability to achieve performances in line with the resources allowed by a Mars vehicle. This paper summarises the major developments being performed in this respect by Galileo Avionica with particular reference to the on going DeeDri (Deep Drill) program under ASI contract [1].

Abstract

Background and aims: After heart transplantation, the effects of folate supplementation on total homocysteine plasma levels (THcy) and heart allograft vascular disease (AVD) remain unclear. Methods: Accordingly, we prospectively analyzed 48 heart transplant receipients referred for routine follow-up from July to September 1998 (age 54±11 years, 75% male, 35±27 months from transplant). Among these patients, 17 were treated with folate supplementation for 12 months (Group F), while 31 cross-matched for age, gender, serum creatinine and time from transplant (P>0.3 vs Group F for all) did not assume folate supplementation (Group NF). This article describes the supervision experience of a group-analytic therapy for psychotic patients from 2000 to 2003 at the mental health centre of Carpi, Modena, Italy. The experience of supervision is presented here in its main aspects and as a tool to promote a so called reflective function of the couple supervisor- supervisee. The reflective function is described in its meaning of com-prehension from (the Latin word com-prehendere, to acquire knowledge and also to hold into affection), referring to an on going process of mutual exchange and understanding between supervisor and supervisee. According to our hypothesis the reflective function that develops inside the supervision process mirrors the reflective function of the therapeutic work and in this context research can play a stimulating role. In this paper we will make some considerations about the supervision and we will describe an attempt to combine classical supervision to evaluation work. Supervision is considered as a transgenerational transmission, where both supervisor and supervisee participate in an exchange that takes place between transmission of culture and innovation. Abstract

Introduction : Criteria of cART initiation after a first ADE have been modified over time based on evidence suggesting that treatment should be initiated earlier. The impact of these changes on clinical practice is unknown. Objective of this analysis was to evaluate temporal changes of time of starting cART after a first diagnosis of ADE in ART-naive patients (pts). Methods : All HIV+ enrolled in ICONA Foundation Study who presented with a diagnosis of ADE while cART-naive regardless of CD4 cell count were included. Pts were grouped according to have ADE for which additional medications that may have interactions with cART are required (Tb, atypical mycobacteriosis, non-Hodgkin lymphoma) [group A], ADE treatable only by cART (PML, isosporidiasis/cryptosporidiasis, KS, wasting syndrome) [group B] and ADE treatable with specific drugs (PCP, toxoplasmic encephalitis, CMV disease, esoph candidiasis, bacterial pneumonia, cervical cancer, cryptococcosis) [group C]. Standard survival analysis by KM was used to estimate the cumulative percentage of pts starting cART, overall and after stratification for calendar period of diagnosis (1996-2000, 2001-2008, 2009-2011) and type of ADE (groups A, B, C). Multivariable Cox regression was used to investigate association between calendar year of ADE and time to cART initiation after controlling for demographics. Summary of results : A total of 715 pts with a first ADE were observed over 1996-2011 (group A, n=187; B, n=123; C, n=405). 519 (73%) male, median age 38 (IQR:33-45), median CD4+ 64 (23-187)/mm 3 and HIV/RNA 5.25 (4.57-5.70) log 10 cps/mL, with no differences by calendar period. By 30 days from ADE, 23% (95% CI: 19-27) of those diagnosed in 1996-2000 have started cART vs. 32% (95% CI: 25-39) in 2001-2008 and 36% (28-44) after 2009 (log-rank p=0.001). Abstract

The work we propose in this paper is a Virtual Museum (VM) collecting the Flemish works of art which, during the XVII century, were part of the Genoese collections. One of the advantages of the virtual reality technology is the possibility to create a situation not physically realizable. Hence, our work gathers Flemish works of art linked by a common thread, scattered all over the world, making art at the disposal of everyone. Furthermore, the possibility of organizing contents in a hierarchical and interactive way allows us to provide a fruition of the conveyed information tailored to anyone's knowledge, trying to realize a "democratization of knowledge". The user should be provided with the possibility to customize his/her visit. In our work, we focus on two main aspects: the creation of a suitable environment; the transmission of education to the new generations. The didactic aim is obtained including a section for games, according to the serious games principle. Finally, the synergic cooperation between the figures of the art historian and the ICT engineer was required to develop a museum which is not a structure alien from its content, but a space thought to be appropriate and functional with respect to the content itself.
