We aimed to analyze the symptoms, complications, intensive care unit admissions, and mortality rates of patients with hematological malignancies infected with SARS-CoV-2 in Turkey. Materials and Methods: In this multicenter study, we included 340 adult and pediatric patients diagnosed with SARS-CoV-2 from March to November 2020. Diagnosis and status of primary disease, treatment schedules for hematological malignancies, time from last treatment, life expectancy related to the hematological disease, and comorbidities were recorded, together with data regarding symptoms, treatment, and outcome of SARS-CoV-2 infection. Results: Forty four patients were asymptomatic at diagnosis of SARS-CoV-2 infection. Among symptomatic patients, fever, cough, and dyspnea were observed in 62.6%, 48.8%, and 41.8%, respectively. Sixty-nine (20%) patients had mild SARS-CoV-2 disease, whereas moderate, severe, and critical disease was reported in 101 (29%), 71 (20%), and 55 (16%) patients, respectively. Of the entire cohort, 251 (73.8%) patients were hospitalized for SARS-CoV-2. Mortality related to SARS-CoV-2 infection was 26.5% in the entire cohort; this comprised 4.4% of those patients with mild disease, 12.4% of those with moderate disease, and 83% of those with severe or critical disease. Active hematological disease, lower life expectancy related to primary hematological disease, neutropenia at diagnosis of SARS-CoV-2, ICU admission, and first-line therapy used for coronavirus disease-2019 treatment were found to be related to higher mortality rates. Treatments with hydroxychloroquine alone or in combination with azithromycin were associated with a higher rate of mortality in comparison to favipiravir use. Conclusion: Patients with hematological malignancy infected with SARS-CoV-2 have an increased risk of severe disease and mortality.

Bu çalışmada Uludağ Üniversitesi Sağlık Uygulama ve Araştırma Merkezi (UÜ-SUAM) Acil Servisine başvuran ve dahiliye konsültasyonu yapılan hastaların epidemiyolojik özelliklerinin belirlenmesi ve konsültasyonla ilişkili sorunların irdelenmesi amaçlanmıştır. Çalışmaya 1 yıllık süreçte acil servise başvuran ve dahiliye konsültasyonu yapılan 3601 hasta alındı. Hastaların yaş ve cinsiyet özellikleri, majör başvuru semptomları, tanıları, hospitalizasyon ve mortalite oranları gibi epidemiyolojik özellikleri ile konsülte edildiği bölümler, acil serviste kalış süreleri ve konsültasyonla ilişkili sorunlar araştırıldı. Hastaların 2025'i (%56,2) erkek, 1576'sı (%43,8) kadın olup yaş ortalaması; 57±17 yıl idi. Acil servise en sık başvuru nedeni olan majör semptom; genel durum bozukluğu-oral alım azlığı-halsizlik (%20,9), en sık dahiliye konsültasyonu istenme nedeni; gastroenterolojik nedenler (%27,2) ve en sık konulan tanı ise çeşitli enfeksiyonlardı (%23,8). Hastaların %48,3'ü acil servisten taburcu edilirken %45,2'si değişik kliniklere yatırıldı, %5,1'i boş yatak yokluğu nedeniyle başka bir sağlık kuruluşuna sevk edildi ve %1,4'ü ise eksitus oldu. Hastalarda ortalama konsültasyon sonuçlanma süresi; 121 dk ve acil serviste kalış süresi; 403 dk idi. Acil serviste 8 saatten uzun süre kalan hasta sayısı 986 (%27,4) olup en sık nedeni boş yatak yokluğuydu (%74,1). Hastaların 165'inin (%4,7) çeşitli dahiliye polikliniklerinden sevk edildiği ve bunların %32,1'inin uygun olmayan sevk olduğu görüldü. Konsültasyonla ilişkili en sık gözlenen sorunlar; gereksiz konsültasyon isteği (%2,7) ve konsültasyonun sonuçlandırılmasının gecikmesiydi (%2,4). Sonuç olarak, acil servislerin işleyişinde dahiliye konsültasyonları çok önemlidir. Hızlı ve etkin bir konsültasyon işleyiş sistemi için acil servis hekimi ve konsültan hekimin işbirliği gerekli olup dahiliye polikliniklerinden acil servise uygun olmayan sevklerden, acil serviste gereksiz konsültasyon isteğinden kaçınılmalı ve konsültasyon isteği hızlı bir şekilde yanıtlanmalıdır.Anahtar kelimeler: Acil servis, konsültasyon, dahiliye. In this study, we aimed to evaluate the epidemiological characteristics of the patients who admitted to Emergency Department in Uludağ University Health Practice and Research Centre and consulted to the Internal Medicine Department and the problems related with consultation. The study included 3601 patients who admitted to the Emergency Department and consulted to the Internal Medicine Department for 1 year. The epidemiological characteristics of patients such as age and gender, major presenting symptoms, diagnosis, hospitalization and mortality rates and the consulted departments, stay length in Emergency Department and problems related with consultation were investigated. 2025 patients (56,2%) were male, 1576 patients (43.8%) were female and the mean age; 57 ± 17 year. The most common symptom for referring to the Emergency Department was deterioration of the general medical condition- poor oral intake-weakness (20,9%), the most common reason for Internal Medicine consultation was gastroenterologic reasons (27,2%) and the most common diagnosis was various enfections (23,8%). 48,3% of the patients were discharged from emergency department, 45,2% of the patients were hospitalized to different clinics, 5,1% of the patients were referred to another health center due to the absence of free beds and 1,4% of patients were died in the emergency department. The mean duration of consultations result; 121 min and the mean length of stay in the emergency department was 403 min. 986 patients stayed in the emergency department longer than 8 hours and the most common reason of this situation was absence of free beds (74,1%). 165 of the consulted patients (4,7%) were referred from different internal medicine policlinics and 32,1% of them were inappopriate referrals. The most commonly observed problems related with consultation were inappropriate consultation requests (2,7%) and were delayed the consultation responses (2,4%). In conclusion, internal medicine consultations in the emergency departments is very important. The emergency department physician and consultant physician cooperation is necessary for fast and effective consultation system and phycians should be avoided unsuitable dispatch from internal medicine outpatient clinics to the emergency departments and ıt should be avoided inappropriate consultation requests in the emergency departments and consultation requests must be responded quickly.Key words: Emergency Department, consultation, internal medicine. 67

A total of 14.8 million people in Turkey (27.1%) use tobacco products. Nicotine dependence is the first cause of continued smoking behavior and overall failure of treatment interventions. Nicotine, an alkaloid derived from the leaves of the Nicotiana tabacum plant, is a very easily absorbed substance and is easily and rapidly absorbed by the lungs, mouth, nose, skin. Nicotine is the main cause of cigarette dependence. The nicotinic acetylcholine receptor (nAChR) is an ion channel receptor, which is activated by acetylcholine and nicotine which is the main component of tobacco dependence. To date, 17 nACh receptor subunits have been identified, including muscarinic (α1, β1, γ, δ and ε) and neuronal (α2-α10 and β2-β4). The most widely distributed nAChR subunits in the central nervous system are α4β2 (~ 90%) and α7 (~ 10%) subunits. The α4β2 subunit is the target structure of smoking cessation drugs. Although there are several experimental studies (in vivo and in vitro) that some phytochemicals (camphor, menton, menthol, carvacrol, thymoquinone, flavonoids, etc.) in some plants (avena sativa, hypericum perforatum, lavandula stoechas, lobelia inflata, nigella sativa, piper nigrum) are smoking cessation features, there are no any studies with nAChR (nicotinic acetylcholine receptor) including binding properties and energy. Our aim is to investigate how some phytochemicals (camphor, menton, menthol, carvacrol, thymoquinone, flavonoids, etc.) in some plants (avena sativa, hypericum perforatum, lavandula stoechas, lobelia inflata, nigella sativa, piper nigrum) interact with nAChR as an antagonist by using Docking program Autodock Vina. In the first part of the study, the validation of the AutoDock Vina program was performed for AChBP (acetylcholine binding protein) 1UW6 and 2BYR. As a result of calculation, RMSD values for 1UW6 and 2BYR were found 1.3 and 1.6 Å respectively. Due to RMSD value is less than 2 Å, AutoDock Vina has been determined to be suitable for the selected system. In the second step, geometry optimization of phytochemicals in lavandula stoechas, avena sativa, piper nigrum, lobelia inflata, hypericum perforatum and nigella sativa were performed by DFT/B3LYP/6-31G(d,p) and the binding of phytochemicals to the active site of AChBP was analyzed by using the AutoDock Vina program. İt was found that miquelianin has the highest antagonist activity. In the last part of the study, virtual ligand screening study was performed for 506 compounds in ZINC15 database. Our calculation results showed that the first 20 structures showing the best binding affinity would be promising candidates. As a result of the research carried out in this thesis, the obtained data will contribute to the design of new candidate antagonists which may be effective as nAChR antagonists. Akciğerler, ağız, burun, deriden kolay ve hızlı bir şekilde emilen nikotin Nicotiana tabacum bitkisinin yapraklarından elde edilen bir alkaloiddir. Sigara bağımlılığının başlıca sebebi Nikotin’dir. İyon kanal reseptörü olan nikotinik asetilkolin reseptörü (nAChR) asetilkolin ve tütün bağımlılığının ana maddesi olan nikotin tarafından aktive edilir. Bugüne kadar, muskarinik (α1, β1, γ, δ ve ε) ve nöronal (α2-α10 and β2-β4) olmak üzere 17 tane nACh reseptör altbirimi tespit edilmiştir. Merkezi sinir sisteminde en çok yayılan nAChR altbirimleri α4β2 (~90%) ve α7 (~10%) altbirimleridir. α4β2 altbirimi asetilkolin ve nikotine olan yüksek afinitesi ile karakterize edilir ve sigara bıraktırıcı ilaçların hedef yapısıdır. Bazı bitkilerin (çörekotu, karabaş otu, kara biber, lobelya, sarı kantaron ve yulaf) içindeki bazı fitokimyasalların (kamfor, menton, mentol, karvakrol, timokinon, flavonoidler v.s.) sigara bıraktırıcı özelliği olduğuna dair deneysel çalışmalar olmasına karşın, Nikotinik Asetilkolin Reseptörü (nAChR) ile bağlanma özelliklerini ve bağlanma enerjisini içeren herhangi bir çalışma bulunmamaktadır. Amacımız, bu bitkilerin (çörekotu, karabaş otu, kara biber, lobelya, sarı kantaron ve yulaf) içindeki bazı fitokimyasalların (kamfor, menton, mentol, karvakrol, timokinon, flavonoidler v.s.) nAChR antagonisti olarak etkisinin İlaç Tasarım yöntemlerinden Doking (kenetleme) programı olan Autodock Vina kullanılarak araştırılmasıdır. Çalışmanın ilk kısmında, AutoDock Vina programının validasyonu 1UW6 ve 2BYR kodlu Asetilkolin Bağlanma Proteini (AChBP) için yapılmıştır. Hesaplama sonucunda RMSD değerleri 1.3 ve 1.6 Å olarak bulunmuştur. RMSD değeri 2 Å ’dan küçük olduğundan dolayı AutoDock Vina programının seçilen sistem için uygun olduğu belirlenmiştir. İkinci aşamada, karabaş otu, çörekotu, yulaf, kara biber, lobelya ve sarı kantaron bitkilerindeki fitokimyasalların DFT/B3LYP/6-31G(d,p) temel seti ile geometri optimizasyonu yapılmıştır ve fitokimyasalların AChBP’nin aktif bölgesine bağlanması AutoDock Vina programı kullanılarak incelenmiştir. İlk kez yapılan bu çalışma sonucunda, antagonistik özelliği en yüksek bileşik mikuelianin olarak saptanmıştır. Çalışmanın son aşamasında, mikuelianin yapısından yola çıkılıp ZINC15 veri tabanı kullanılarak bulunan 506 bileşik ile sanal ligand taraması yapılmıştır. Hesaplamalar sonucunda en iyi bağlanma afinitesine sahip ilk 20 bileşiğin ümit verici aday olabileceği bulunmuştur. Bu tezde yapılan araştırmalar sonucunda elde ettiğimiz veriler, nAChR antagonisti olarak etkili olabilecek yeni aday antagonistlerinin tasarlanmasına katkıda bulunacaktır.

Around the world 255 million people using drugs. According to frequency of use substances lifelong Turkey in the year 2011 held on drug addiction research it has been identified as 2.7%. Opioid drugs are classified as natural (morphine, heroin, codein...) , semi-synthetic (oxycodone, buprenorfine...) and synthetic (methadone, fentanyl....). Main reason of opioid dependece is interaction of opiates such as morphine with mü opioid receptor (MOR) which is G-protein receptor. It is a target structure for drugs used opioid treatment Currently agonist and antagonist drugs are used for the different steps of treatment. Because of their side effects, new drugs have been studied for. There are some experimental studies that some plant (hypericum perforatum, peganum harmala, papaver rose and nigella sativa) are useful for the treatment of dependency but its mechanism was not understood and it is not known which phytochemicals have what effects on. Our aim is to identify agonist and/or antagonist properties of 31 phytochemicals obtained from four plants which are hypericum perforatum, papaver rose, peganum harmala and nigella sativa for two targets by using AutoDock Vina program. In the first point of the study, validation of AutoDock Vina program was done for re-docking and cross-docking of agonist and antagonist ligand mu opioid receptor having pdb code 4DKL and 5C1M. According to redocking calculation results , RMSD values are 1.0 Å and 1.2 Å for respectively for 4DKL and 5C1M. Since RMSD values are smaller than 2.0 Å , it is shown that AutoDock Vina programme is suitable for chosen protein-ligand system. In the second part, geometry optimisation of 31 phytochemicals obtained from four plants was done by using DFT/B3LYP/6-31 G (d,p) basis set. Docking calculations of there compounds to active site of mu opioid receptor was studied by AutoDock Vina program. After analyzing agonist and antagonist properties of 31 phytochemicals based on their binding energies, rhoeagenine from papaver rose was found to be the best agonist. As a result of the research first time carried out in this thesis, the obtained data will contribute to the design of new medicine which may be effective as target structure of MOR. Türkiye’de ise 2011 yılında yapılan madde bağımlılığı araştırmalarına göre yaşam boyu madde kullanım sıklığı %2,7 olarak belirtilmiştir. Opioid maddeler doğal (morfin, eroin, kodein...) , yarı sentetik (oksikodon, buprenorfin...) ve sentetik (metadon,fentanil...) olarak sınıflandırılırlar. Opioid bağımlılığında ana sebep morfin gibi opiyatlar ile G-protein reseptörü olan mü opioid reseptörünün (MOR) etkileşmesidir. Bu yapı opioid tedavisinde hedef yapı olarak kullanılmaktadır. Günümüzde agonist ve antagonist ilaçlar tedavinin farklı aşamalarında kullanılmaktadır. Bu ilaçların güçlü yan etkilerinden dolayı bu konuyla ilgili yeni ilaç araştırmaları devam etmektedir. Bazı bitkiler (sarı kantaron, üzerlik, gelincik, çörek otu) için uyuşturucu bağımlılığında tedavi edici özelliği olduğunu gösteren deneysel çalışmalar olmasına karşın, mü opioid reseptörü ile bu bitkilerin içerisindeki hangi fitokimyasalın etkili olduğuna dair mekanizma bilinmemektedir. Bizim amacımız AutoDock Vina programını kullanarak sarı kantaron, gelincik, çörek otu ve üzerlik bitkisinde bulunan 31 fitokimyasalın agonist ve/veya antagonist özelliklerinin belirlenmesidir. Çalışmanın ilk aşamasında AutoDock Vina programını kullanarak normal ve çapraz doking ile validasyon hesaplamaları agonist ve antagonist özellikteki mü opioid reseptör kodları 5C1M ve 4DKL ile yapılmıştır. Normal validasyon hesaplamalarına göre RMSD değeri sırasıyla 5C1M ve 4DKL için 1.2 Å ve 1.0 Å olarak bulunmuştur. RMSD değeri 2 Å dan küçük olduğundan dolayı AutoDock Vina programının seçilen protein-ligand sistemi için uygun olduğu belirlenmiştir. İkinci aşamada, sarı kantaron, üzerlik, gelincik ve çörek otu bitkilerindeki 31 adet fitokimyasalın DFT/B3LYP/6-31G(d,p) temel seti ile geometri optimizasyonu yapılmıştır ve fitokimyasalların MOR’ün aktif bölgesine bağlanması AutoDock Vina programı kullanılarak incelenmiştir. Bağlanma enerjilerine dayanarak 31 fitokimyasalın agonist ve antagonist özelliklerinin incelenmesinden sonra agonist özelliği en yüksek bileşik rojenin olarak saptanmıştır. Bu tezde ilk kez yapılan araştırmalar sonucunda elde ettiğimiz veriler, hedef yapı MOR ile etkileşen yeni ilaç adaylarının tasarlanmasına katkıda bulunacaktır.

Kantaron ekstratları günümüzde yaygın olarak çoğu ülkede depresyon tedavisinde kullanılmaktadır. Bu etkinin kantaron bitkisi içindeki flavonoid bileşiklerinden kaynaklandığı ileri sürülmektedir. Ancak, kantaron favonoidlerinin MAO-A enzim aktivitesi çalışmaları tam seri için yapılmamıştır. Ayrıca, MAO-A enzimi ile flavonoidlerin bağlanma modları konusunda da literatürde tam seri halinde çalışma mevcut değildir.Amacımız, teorik ve deneysel yöntemler ile kantaron flavonoidlerinin tam serisi (12 bileşik) için MAO-A inhibitör aktifliği hakkında önemli bilgiler edinmektir.Çalışmanın teorik kısmında, öncelikle kantaron flavonidlerinin (12 bileşik) enerji minimizasyonu B3LYP/6-31G(d,p) temel seti ile Gaussian09 programı kullanılarak yapılmıştır. 12 kantaron flavonoidinin doking yöntemi için kararlı geometrileri oluşturulmuştur. Ayrıca, kuersetinin literatürde susuz geometrisi bulunmadığından dolayı konformasyon analizi yapılarak bu geometrisi belirlenmiştir. Sonuçlarımıza göre sin konformasyonların anti konformasyonlara göre daha kararlı olduğu bulunmuştur. Ek olarak, susuz kuersetinin geometrisinin sin1-sin2-sin1 olması gerektiği bulunmuştur.Daha sonra, AutoDock Vina programının validasyonu 2Z5X kodlu insan MAO-A enzimi için yapılmıştır. Hesaplama sonucunda RMSD değeri 0.89 A0 olarak bulunmuştur. RMSD değeri 2 A0'dan küçük olduğundan dolayı AutoDock Vina programının seçilen sistem için uygun olduğu belirlenmiştir. Üçüncü aşamada, 12 kantaron flavonoidinin tam serisinin insan MAO-A enziminin aktif bölgesine bağlanması AutoDock Vina programı kullanılarak incelenmiştir. Ayrıca, inhibasyon türü rekabetçi olarak belirlenmiştir.Çalışmalar sonucunda aktiflik sıralaması; flavon> flavonol> flavan-3-ol> flavonol glikozidleri> flavanon-3-ol olduğu bulunmuştur.Çalışmanın son aşamasında, flavon grubu üyesi luteolin yapısından yola çıkarak ZINC15 veri tabanı kullanılarak 578 bileşik ile sanal ligand taraması yapılmıştır. Hypericum extracts have become widely used in the treatment of depression in many countries. It was proposed that source of effect comes from flavonoid compounds in hypericum. However, MAO-A enzyme activity studies of hypericum flavonoids has not been made for the their full series. Also, there is no study for full series of flavonoids in the literature about binding modes of flavonoids with MAO-A enzyme.Our aim is to obtain important informations about MAO-A inhibitor activity for full series of Hypericum flavanoids (12 compounds) by using theoretical and experimental methods. In the theoretical part of the study; first, energy minimization of Hypericum flavonoids (12 compounds) was done with B3LYP/6-31G(d,p) basis set by using Gaussian 09. Stable geometries of 12 hypericum flavonoids was created for the docking procedure. Furthermore, anhydrous quercetin has been determined by quercetin conformation analysis because of there is no study about geometry of anhydrous quercetin in the literature. According to our results, syn conformers were found more stable than anti conformers. In addition, geometry of anhydrous quercetin was found that should be syn1-syn2-syn1.Later, validation of AutoDock Vina program was done for human MAO-A enzyme that coded as 2Z5X. As a result of calculation, RMSD value was found 0.89 A0. AutoDock Vina was determined suitable for the selected system becaue of RMSD value is lower than 2 A0.In the third step, full series of 12 hypericum flavanoids binding to active side of human MAO-A enzyme was examined by using AutoDock Vina program. In this study that was done for the first time, luteolin, member of flavon group, was found as most active compound and its binding energy -10.8 kcal/mol. In this study stage, activity order of hypericum flavonoids; luteolin> kaempferol> quercetin> epicatechin> catechin> myricetin> rutin> hyperoside> quercitrin> miquelianin> astilbin> isoquercitrin was found.In the experimental part of the study, biological activity of 12 hypericum flavonoids with recombinant human MAO-A enzyme was done using Holt method. There is no experimental study for full series of hypericum flavonoids in the literature. There are only IC50 values for 4 compounds (quercetin, kaempferol, luteolin and epicatechin) and no study for Ki values in the literature. In the this study, first time, IC50 and Ki values was calculated full series of 12 hypericum flavonoids. Also, their inhibition type was determined as competitive. As a result, activity order was determined as flavon> flavonol> flavan-3-ol> flavonol glikozidleri> flavanon-3-ol.At the final stage of the study, visual ligand screening was done with 578 compounds found by using ZINC15 database based on structure of luteolin, member of flavon group. As the results of the calculations, the first 20 structures show the best binding were found that promising inhibitors, too.Our data obtained as a result of research carried out in this thesis, to be effective as an inhibitor of MAO-A inhibitors will contribute to the design of new candidates. 159

Pıhtılaşma, kan dolaşımındaki fibrinojenin pıhtının temeli olan ve çözünmeyen fibrin'e dönüşmesidir. Şimdiye kadar pıhtı oluşumunda rolü olan 13 faktör keşfedilmiştir. Bu faktörlerin tümü¬nün normal çalışması halinde, yaralanan doku sıvısında bulunan tromboplastin, dolaşımdaki protrombini, trombine dö-nüştürmekte, bu da fibrinojeni etkileyip, fibrin haline gelmesine yol açmaktadır. Fibrinojenin fibrine dönüşümünü katalizleyen enzim Trombindir.Kanı pıhtılaştıran Trombin bir serin proteaz enzimidir ve vücut içinde Trombin'e ne kadar ihtiyaç varsa o kadar üretilir. Damarların çeperinde pıhtı (trombus) oluşmasını ve gelişmesini inhibe eden ilaçlara antikoagülanlar denir. Damar içi pıhtı oluşumu insan hayatında önemli bir risk faktörü oluşturur.Trombin kanın pıhtılaşmasının son basamağındaki görevi nedeniyle yeni antikoagulan ilaçların geliştirilmesinde temel hedef olmaya devam etmektedir. Direk Trombin inhibitörü (DTI) olarak günümüzde kullanılan antikoagulan ilaçlar divalent: hirudin, lepirudin, bivaluridin, ve monovalent: argatroban, dabigatran'dır. Tek oral ilaç dabigatran ise havanın nemi ile hemen bozunmaktadır. Diğerlerinin dezavantajları ise oral alınamamaları ve genetik bozukluk nedeniyle antitrombin III eksikliği olan hastalarda kullanılamamasıdır. Bu nedenle günümüzde ilaç şirketlerinin çoğu argatroban temelli oral alınabilen yeni ilaç tasarımları üzerinde çalışmaktadırlar.Bu çalışmadaki amacımız Argatroban ve onun çıkış yapısı olan yapılar temel alınarak oral yoldan alınabilen ve de küçük molekül yapısı ve ağırlığına sahip olan bir direkt trombin inhibitörü araştırmaktır.Bunun için, öncelikle incelediğimiz trombin sistemi için uygun doking programı ve uygun trombin yapısını belirlemek için protein data bankasından alınan trombine ait üç-boyutlu yapı koordinatlarını içeren 1k22, 1dwc ve 1dwd kodlu yapılar kullanılarak Autodock ve Autodock Vina Programları arasında validasyon çalışması yapılmıştır. Tekli doking validasyon çalışması sonuçlarına göre en küçük RMSD değeri ve en düşük bağlanma enerjisine sahip olan 1dwc yapısı ve bu sonuçları veren program olarak da Autodock Vina programı uygun olarak görülmüştür.Tekli doking çalışmaları ışığında 1dwc yapısı olan argatroban yapısı ve TAME yapısında ortak olarak gözlemlenen bölümler ZINC data bankasında çizilerek 578 adet yapı içeren bir veri seti hazırlanmıştır. Bu veri seti ve Autodock-Vina programı kullanılarak yapılan sanal ligand taraması(SLT) hesaplama sonuçlarına göre, bu yapılardan 175 molekülün RMSD değeri 2 ?'den küçük olduğu gözlemlenmiştir. Bu yapılardan en düşük bağlanma enerjisine sahip olan 20 yapı, bize yeni inhibitör tasarlanması konusunda yapılacak çalışmalara ışık tutacaktır. Coagulation is the transformation of fibronogen to fibrin which is the basis of clot at blood circulation. Up till now 13 factors playing a role in clot formation have been discovered. In normal operation of all of these factors, the thromboplastine that is located in injured tissue translates the prothrombin which is in circulation to thrombin. This also influences the fibrinogen and leads the fibrinogen to turn into fibrin. The enzyme that catalyzes the conversion of fibrinogen to fibrin is Thrombin.Thrombin which leads to blood coagulation is a serine protease enzyme and so is produced in the amounts of the body needs. The drugs that inhibit the formation and development of clots on lining of blood vessels are called anticoagulants. Also, intravascular clot formation creates a significant risk factor in human life.Due to the task of thrombin at the last stage of blood clotting it continues to be the main target in the development of new anticoagulant drugs. Today, the anticoagulant drugs used as direct thrombin inhibitor (DTI) are hirudin, lepirudin, bivaluridin, and monovalent: argatroban, dabigatran. The single oral drug dabigatran is immediately broken down by the effect of the moisture in the air. The disadvantages of other inhibitors are that they can?t be taken by orally and due to a genetic disorder they shouldn?t be used by the patients who are lack of anti-thrombin III. For this reason, most of today?s pharmaceutical companies are working on the design of new drugs that is argatroban-based and so can be taken orally.The aim of this study is to investigate a direct thrombin inhibitor that has a small molecular structure and weight and can be taken orally on the basis of Argatroban and its basic structures.Firstly appropriate docking program and thrombin structure should be determined that is suitable for our thrombin system. To determine this, validation study was carried out with AutoDock and AutoDock Vina Programs by using 1k22, 1dwc and 1dwd coded structures that consist of the coordinates of three-dimensional structure of thrombin. According to the results of single docking validation study, 1dwc structure which has the smallest RMSD value and the lowest binding energy and AutoDock Vina Program which gives these results are found to be appropriate.In the light of the single docking studies, the parts that are commonly seen in TAME structure and Argatroban which is 1dwc structure are drawn by ZINC data bank. A data set was prepared that consists of 578 structures. Due to the calculation results of virtual ligand screening (VLS) that is obtained by using the data set and AutoDock Vina program, it is observed that the RMSD value of 175 molecules is smaller than 2 ?. 20 of these structures have the lowest binding energy and they will shed light to us in the design of new inhibitors on the future studies. 111

Sigara içimi, günümüzde dünyada önlenebilir ölüm nedenlerinden biridir. Sigara içiminin neden olduğu hastalıklar solunum problemleri, kanser , kalp ve dolaşım sistemi bozuklarıdır. Günümüzde, yıllık yaklaşık 42 milyon kişi, sigaranın neden olduğu hastalıklardan ölmektedir.Tütünde bulunan bileşiklerden sigara bağımlılığına neden olan ana madde Nikotindir. Nikotin-bağımlı kişiler, beyindeki nikotin seviyesini aynı tutmak amacıyla sigara içerler. Nikotin-bağımlı kişileri tedavi etmek için kullanılan yaygın kullanılan farmakolojik yöntemler, Nikotin verme terapisi(NRT) ve Nikotin İçermeyen terapi(NNRT) dir. NRT nin genel kullanım şekilleri nikotin sakızı, deri bantları, ağız ve burun spreyleridir. NNRT yönteminde ise antidepresan Bupropion(zyban) ve Varenicline(Champix) kullanılmaktadır. NRT tedavisi çeşitli nedenlerden dolayı başarılı değildir.Nikotin, karaciğerde bulunan Sitokrom P450 enzimlerinden CYP2A6 tarafından metabolize edilir. CYP2A6 enzimi, nikotini biyolojik inaktif kotinine çevirir. Ayrıca, bu enzim tütündeki özel prokanserojen nitrosoaminlerin aktif akciğer kanserojenlere dönüşmesini sağlar.Sigara alışkanlığından kurtulmadaki yeni strateji, karaciğerde bulunan CYP2A6 enziminin aktivitesinin durdurulmasıdır. CYP2A6 enziminin inhibe edilmesi, hem sigara içme isteğinin azalmasını hem de prokanserojen maddelerin kanserojenlere dönüşmesini engeller. Günümüzde kullanılan CYP2A6 enzimini inhibe eden klinik ilaçlar Metoksalen, triptamin ve tranilsipromindir. Fakat, bunlar çok güvenilir değildir.Yaptığımız çalışmadaki temel amacımız, bilgisayar-destekli ilaç tasarım yöntemlerinden doking(kenetleme) yöntemini kullanarak sigara alışkanlığından kurtulmak için kullanılabilecek yeni potansiyel, oldukça spesifik ve seçici CYP2A6 inhibitörleri tasarlamaktır. Bu nedenle ilk olarak çalışmamıza uygun CYP2A6 enzim kompleksini bulabilmek için X-ışını yapısı bilinen beş CYP2A6 enzim-ligand kompleksi (PDB kodları; 1z11, 2fdu, 2fdv, 2fdw, 2fdy ) için tekli doking yapılmıştır. Tekli doking sonucunda en düşük sapma ( RMS ) ve en yüksek eşleşme skoru 2fdu pdb kodlu yapıdan elde edilmiştir. Bu nedenle 2fdu yapısı görsel taramada kullanılacak yapı olarak seçilmiştir.Çalışmamızın bir sonraki aşamasında metoksalen molekülü hedef seçilmiş ve ZINC kütüphanesi kullanılarak benzer ligandlar taranmıştır. Görsel ligand taraması sonucu bulunan benzer ligandlar CYP2A6 enzimi üzerine çoklu doking yöntemi ile yerleştirilmiştir. Uygun bulunan ligandlar yapılan çalışma sonrasında ortaya çıkan RMS ve yerleşme skoru değerleri göz önüne alınarak sıralanmıştır. Bu kısımda en düşük sapma ( RMS ) ve en yüksek eşleşme skoru değeri veren ligand ZINC01064377 kodlu ligand olduğu görülmüş ve bu ligandla birlikte yine uygun ilaç adayı olabilecek kadar iyi RMS ve eşleşme skoru veren 27 tane daha ligand bulunmuştur.Çalışma sonucunda bulunan bu ligandlar, ilaç tasarımındaki bir sonraki aşama olan deneysel çalışmaya geçmemizi sağlayacak ve uygun ligandları bulmak için deneysel yolla harcadığımız zamanı ciddi biçimde azaltacaktır. Tobacco smoking is the single most preventable cause of death in the world today. The health consequences of smoking include respiratory, cardiovascular and cerebrovascular disorders and cancer. Currently, an estimated 4.2 million people die annually from tobacco-related diseases.Nicotine is the primary component of tobacco responsible for dependence on cigarette smoking. Nicotine-dependent individuals regulate their smoking to maintain nicotine levels in their brain. Nicotine replacement therapy(NRT) and non-nicotinic therapy(NNRT) are commonly used pharmacological approach for treatment nicotine dependence. Common forms of NRT include nicotine gum, transdermal patches, inhalers, lozenges and sprays but existing NRTs can fail for several reasons. On the other hand usage of NNRT is antidepressant Bupropion(zyban) and Varenicline(Champix). NRT is ineffective because of several reasons.Nicotine is metabolized into biologically inactive cotinine by the CYP2A6 enzyme in liver. This enzyme is also involved in the metabolic activation of tobacco-specific procarcinogenic nitrosoamines into active lung carcinogens.New strategies for smoking cessation are based on the inhibition of CYP2A6 activity in liver. Inhibiton of CYP2A6 would result not only decrease smoking behavior but also decrease in the activation of procarcinogens to carcinogens. Methoxsalen, tryptamine and tranylcpromine are clinically used drugs that inhibit the CYP2A6 but they are not safe.Our aim is to design new potent, more specific and selective CYP2A6 inhibitors for smoking cessation by using computer-aided drug design methods, such as docking. For this purpose, as a begining to find the suitable CYP2A6 enzyme complex we realised single docking to five CYP2A6 complexes (pdb codes; 1z11, 2fdu, 2fdv, 2fdw, 2fdy ) for those which X-ışını structure has been defined. As a result of single docking the lowest deviation ( RMS ) and the highest total score has acquired from the complex has a pdb code of 2fdu. That?s why 2fdu has been chosen as the complex which will be used at virtual screening.Methoxsalen has been chosen as a next step of our study and similar molecules has been searched by using ZINC Database. Ligands, which have founded after the virtual ligand screening, replaced into the CYP2A6 enzyme by docking method. Ligands has ranked by which showed the lowest deviation ( RMS ) and the highest total score. In this part ligand , which has a code of ZINC01064377, showed the lowest RMS and the highest total score. Addition to this ligand 27 ligands more has founded acceptable RMS value and the total score.The ligands , that has found after this study, will let us to pass experimental study which is next step of drug design and will noteworthily reduce time which we will spend to find suitable ligands experimentally. 99

HIV Proteaz enzimi, HIV virüsünün yayılmasında en etkili yapıdır. Bu enzimin inhibe edilmesi sonucunda, virüsün diğer hücrelere saldırması ve yayılması durdurulmuş olacaktır. HIV Proteaz inhibitörlerinin kombine terapide kullanımına 1995 yılında başlanmıştır.Günümüzde AIDS tedavisinde kullanılan HIV Proteaz(PR) ilaçlarının (inhibitörlerin) çoğu, peptidomimetik yapıya sahiptir ve enzimin ? substrat bağlanma bölgesine? bağlanır. Ne yazık ki, bütün bu ilaçlara karşı virüsün direnç geliştirmiş ve ilaçların etkisi giderek azalmıştır. Bu nedenle yeni daha etkili peptidomimetrik inhibitörlere veya proteaz enzimindeki substrat bağlanma bölgesi dışında alternatif bölgeleri hedefleyen yeni ilaçlar tasarlanmasına ihtiyaç duyulmaktadır. Son yıllardaki araştırmalar, Proteaz enziminin ? kanat bölgesi? kısmını hedef alan ilaçların tasarlanmasına yönelmiştir. Yapılan çalışmalar, beta-laktam bileşiklerinin enzimin yeni hedef bölgesi olan ? kanat bölgesi? ile etkileştiğini göstermiştir.Bu çalışmanın ilk aşamasında X-Ray yapısı bilinen 14 HIV-Proteaz kompleksi için, 3 ayrı doking yönteminin; FlexX, Autodock (AD) ve Autodock (AD) Vina, validasyonu yapılmıştır. Literatürde, sadece HIV PR test seti için yapılmış herhangi bir doking programlarının karşılaştırılması ve validasyon çalışması bulunmamaktadır. Test seti içinde 7 çift FDA onaylı ilaç; Ritonavir, İndinavir, Nelfinavir, Darunavir, Saquinavir, Lopinavir ve Tipranavir ve bu ilaçların HIV PR enzimiyle kompleksleşmiş yapılarının X-Ray koordinatları bulunmaktadır. Her kompleks çifti enzimin doğal ve/veya mutant (aktif bölge aminoasitleri dışında) yapısını içermekte ve bunlar enzimin ? substrat bağlanma bölgesine? ile etkileşmektedir. Hesaplamalarda, HIV-Proteaz enziminin ?substrat bağlanma bölgesi?nde yer alan aminoasitlerin için ligandın 6.5 A etrafı aktif bölge (doking yapılacak bölge) olarak seçilmiştir. Uygun parametreler saptandıktan sonra, deneysel X Ray yapısı ile hesaplama sonucunda elde edilen iki yapı RMSD sonuçlarının karşılaştırılması ile yöntemin doğruluğu test edilmiştir. İncelenen üç yöntem arasında yapılan karşılaştırmalar sonucunda, HIV Proteaz enzimi için AD Vina'nın en iyi sonucu verdiği bulunmuştur.Çalışmanın ikinci kısmında, literatürde hiçbir hesapsal çalışmanın yer almadığı, yeni hedef bölge ? kanat bölgesi? için doking işlemleri yapılmıştır. Bu amaçla, literatürde deneysel aktiviteleri bilinen beta-laktam bileşiklerine, AD Vina programı ile HIV-PR enziminin etkileştiği bölgeyi araştırmak için serbest doking hesaplamaları yapılmış ve beta-laktam bileşikleri için enzimin hedef bölgesinin ?kanat bölge? olduğu bulunmuştur. Hesaplama sonucunda elde ettiğimiz değerler ile deneysel aktivite değerleri arasında çok yüksek bir uyum gözlenmiştir.Çalışmanın son aşamasında ise hem kendi sentezlediğimiz hem de ZINC veri bankası kullanılarak hazırlanan veri seti için görsel ligand taraması yapılmıştır. Yapılan hesaplamalar sonucunda, sentezlediğimiz bileşiğin yüksek aktiviteye sahip, ümit verici bir aday inhibitör olabileceği bulunmuştur.Araştırma sonucunda elde ettiğimiz veriler, hem yeni doking programlarının yazılımı ve test edilmesine hem de enzimin alternatif ? kanat bölgesi? ile etkileşecek yeni ve etkili aday inhibitörlerin tasarlanmasına ve sentezlenmesine katkıda bulunacaktır. HIV Protease enzyme has been the most important factor in virus infection. Inhibition of this enzyme will result with the end of virus attack into other cells and therefore spreading of the infection will be prevented. In order to fight with AIDS, HIV Protease inhibitors have been using in combination therapy since the year of 1995.Most of the HIV Protease (PR) drugs (inhibitors) marketed today are in peptidomimetic structure and bind to ?substrat binding region? of the enzyme. Unfortunately, the virus has somehow developed resistance against HIV PR inhibitors that are currently in use. Design of some new and more effective peptidomimetics or discovery of new drug candidates that targets regions of the enzyme different than substrate binding site therefore draws most attention of the pharmaceutical researchers. Research trends on this concept has directed to design of the inhibitors that targets the flap region of the enzyme. In addition, some studies in the literature are emphasizing the interaction between beta-lactam compunds and the flap region of the enzyme.The first part of this study consists of the validation of three different docking methods; FlexX, AutoDock (AD) and AutoDock (AD) Vina, against 14 HIV PR complexes. To our knowledge, there are no such study found out in the literature, that focus on the comparison of the docking programs and validaton of docking methods for a specific dataset of HIV PR. Our dataset of HIV PRs composes of 7 couples of FDA approved drugs; Ritonavir, İndinavir, Nelfinavir, Darunavir, Saquinavir, Lopinavir and Tipranavir respectively, and the X-Ray coordinates of these inhibitors in complex with their HIV PR enzyme structures. Each complex couple contains the wild type and/or mutant (mutation except the active site aminoacids) structures of enzymes and inhibitors that interact with the ?substrate binding region? of the enzyme. For computations, aminoacids that surrounds 6.5 Å of ligands were selected as the active site (docking region) of the HIV PR enzyme. After the appropriate parameters were determined, accuracy of method was tested by comparing the the X Ray structures with RMSD results obtained from computations. In the end of the comparisons between the three investigated method, AD Vina has been found out to gave the best results.In the second part of the study, docking procedures has been carried out for a new target, the flap region, that haven?t studied earlier in the literature. For this purpose, beta lactam compounds with known experimental activities were docked freely on HIV PR enzyme in order to search for possible interaction regions and it was found out that these copmounds do interact with the flap region of the enzyme. The results obtained from computations showed a very well correlation with experimental values.The final part of our study includes the virtual ligand screening of a set of beta lactam compunds into the flap region of HIV PR. For this study, beta lactams dataset was extracted from ZINC database and enriched with the compunds we have synthesized. The computations was resulted with a statement that the compund we have synthesized showed high affinity for the enzyme and could be considered as a promising candidate for HIV PR inhibition.The results obtained from this study will contribute both to the design and synthesis of some new and potent inhibitor candidates that interact with alternative flap region of the enzyme and to the development and testing of new docking tools. 110

HIV'in hücreye giriş mekanizmasında etkin olan yapılardan birisi glikoprotein 41 molekülüdür. Bu glikoprotein HIV virüsünün konak hücreye girişinde etkin rol oynamaktadır. Glikoprotein 41 molekülünün bloke edilmesi sayesinde HIV virüsünün hücre içine girişi ve hücre içinde yayılmasına engel olunabilecektir.Bugüne kadar yapılan çalışmalarda HIV girişine yönelik keşfedilmiş ve satışa sunulmuş olan tek ilacın (Fuzeon) peptid yapılı olması ve dolayısıyla büyük molekül ağırlığına sahip olması ilacın oral yolla kullanımını engellemektedir. Ayrıca büyük peptid yapılı ilacın kullanım maliyetide yüksektir. Bu sebeplerden ötürü peptid yapılı olmayan, küçük ve gp 41 molekülünü hedef alan ilaçların geliştirilmesine ihtiyaç duyulmaktadır. Son zamanlarda yapılan çalışmalar, gp 41 molekülünün hidrofobik cep bölgesinin hedef bölge olabileceğini ve küçük moleküllerle etkileşebileceğini ortaya koymuştur.Protein data banktan (PDB) alınan bilgiler ışığında gp 41 ile ilgili 170 adet yapı olduğu görülmüştür (Gochin et al., 2010). Bu yapıların hiçbiri glikoprotein 41 molekülünün tam zincirini içermemektedir. Protein veribankasında yer alan gp 41 ile ilgili X-ışını yapıları gp 41'in NHR ve CHR bölgelerinden türetilen sentetik C-peptid ve N-peptid yapılara aittir. Gp 41 ile ilgili X-ışını yapılarından henüz hiçbiri gp 41'in küçük molekülle birlikte bulunduğu kristalize kompleks yapıyı bulundurmamaktadır.Yapılan çalışmanın ilk bölümünde moleküler kenetlenme yöntemi kullanılarak potansiyel HIV inhibitörü olan A12 ligandının farklı HIV gp 41 monomer, trimer ve pentamer yapılarıyla etkileşimi Autodock vina programı ile araştırılmıştır. Bunun için protein veribankasından alınan HIV gp 41 NHR yapılarından 2zzo monomer, 2q7c trimer, 2r5d trimer, 1aik ise hem trimer hem de pentamer olarak kullanılmıştır. Bu sayede A12 ligandının monomer, trimer ve pentamer yapılara bağlanma isteği ve bu yapılarla etkileşimi incelenmiştir. Hesaplamalar sonucunda A12 ligandının gp 41 NHR pentamer yapısı ile monomer ve trimer yapıya göre daha iyi etkileşim verdiği bulunmuştur.Çalışmanın ikinci aşamasında ise ZINC veritabanından görsel ligand taraması sonucunda elde edilen yeni yapıların en uygun gp 41 yapısı ile kenetlenme çalışmaları gerçekleştirilmiştir. 210 ligandın sanal ligand taraması sonucunda tespit edilen ve yapısında pirol halkası içeren ilk 20 bileşiğin (ZINC 20030467, ZINC02301313, ZINC02936435, ZINC00636696 v.s kodlu) potansiyel HIV gp 41 inhibitörü olabileceği bulunmuştur.Yapılan hesaplamaların neticesinde elde edilen verilerin gelecekte tasarlanacak ve gp 41'in hidrofobik bölgesiyle etkileşim gösterecek yeni küçük ve potansiyel moleküllere katkı sağlaması beklenmektedir. Glycoprotein 41 molecule has an important role entering HIV to a host cell since glycoprotein 41 is one of the effective structure in entry mechanism of HIV in to the cell. As a result of the blockade of the gp 41 natural outcome of entering and spreading of HIV in to a cell can be inhibited.Present studies show that the discovered and marketed only drug (Fuzeon) is a peptide structure and it has high molecular weight, therefore prevents orally usage of the drug. Also, the usage cost of this drug is high. Due to all these, non-peptide and targeted to gp 41 molecule drug is needed. Lately investigations show that the hydrophobic pocket area of gp 41 can be a target area for small molecule, thus can interact with small molecule.According to information from protein data bank (PDB), 170 structures would be observed as related to gp 41. None of these structures contain complete chain of gp 41. The X-ray structure related to gp 41 existing in protein data bank, are belong to synthetic C-peptide and N-peptide derived from the NHR and CHR regions of gp 41. None of the X-ray structures related to gp 41 exist the crystalline complex structure containing small molecule with gp 41.The first part of the made study, by using molecular docking method, potential HIV inhibitor of A12 interaction with different gp 41 monomer, trimer and pentamer structures with Autodock vina software were investigated. Hence, HIV gp 41 NHR structures taken from protein data bank were used 2zzo as monomer, 2q7c as trimer, 2r5d as trimer, 1aik as both trimer and pentamer. Thus, the binding affinity and interaction of A12 ligand with monomer, trimer and pentamer structures were examined. The result of calculation, A12 ligand gives better interaction with gp 41 NHR pentamer than the monomer and trimer.The second part of the made study includes that, new structures which are obtained from ZINC database and virtual ligand screening result, the molecular docking study with most appropriate gp 41 structure were actualized. After virtual ligand screening of 210 ligand, which containing pyrrole ring (ZINC 20030467, ZINC02301313, ZINC02936435, ZINC00636696 etc. code) were determined as potential HIV gp 41 inhibitors.The results obtained from this study will contribute for designing in future, interaction with gp 41 hydrophobic pocket, new small and potential molecules. 100

Trombin; bir serin proteazdır ve kanın pıhtılasmasının son basamagındaki önemlirolü nedeniyle yeni antikoagulan ilaçların gelistirilmesinde temel hedef olmayadevam etmektedir. Trombin kan plazmasında iki önemli rol oynamaktadır.Trombinin antikoagulant (yavas) formundan prokoagulant (hızlı) formuna geçisitrombin çeliskisi olarak bilinir (Griffin,1995) ve çok ilgi duyulan bir konudur.Literatürde bazı X-ısını ve deneysel çalısmalar olmasına ragmen, bu çalısmalartrombinin yapısal degisikliklerini açıklamak için yeterli degildir.Son yıllarda protein yapılarının, dinamiginin ve kararlılıgının incelenmesindeMoleküler Dinamik (MD) simulasyonu sıklıkla kullanılmaktadır. Bu çalısmada MDsimulasyon hesaplamalarıyla, trombinin yavas-hızlı formlarının geçisini vebaslangıç yapısının simulasyon sonuçlarına etkisini gözlemlemek hedeflenmistir.Elde edilen sonuçlar trombinin vücut içerisindeki fonksiyonlarını kontrol edebilmekiçin yenilikçi stratejilere öncülük edebilir.Bu çalısmada trombinin farklı çözünürlük degerlerine sahip olan kristal yapıları(çözünürlük degerleri1.92 Å, 2.20 Å, 2.0 Å , 2.20 Å ve sırayla pdb kodları 1ppb,1qhr, 1qj1,1qj6 olan yapılar) için GROMACS 3.3.1 yazılımı moleküler dinamiksimulasyon hesaplamalarında kullanılmıstır. Bu çalısmanın ilk kısmındasimulasyon için gerekli baslangıç yapıları, Protein Veri Bankası'ndaki tüm trombinyapıları incelenerek seçilmisir. Burada amaç tam zincirli trombin yapılarınıbelirlemektir. Simulasyon kısmında ise ilk önce optimum kosullar belirlenmistir. Buyolla elde edilen sonuç dosyaları GROMACS yazılımı yardımıyla analiz edilmistir.Bir proteinin kararlılıgı ve konformasyonel degisimi kare ortalamasının karekökünün zamana baglı degisimi elde edilerek incelenmistir. Böylece simulasyonboyunca proteinin dengeye ulasıp ulasmadıgı belirlenebilir. Ayrıca proteinyapısındaki aminoasitlerin yapıdaki konformasyonel degisime etkisi belirlenmistir.Yapının ortalama boyutu, potansiyel enerjinin simulasyon zamanına baglıdegisimi, hidrojen bagı sayısı, çözücü ulasılabilir yüzey alanları incelenmis,trombinin yavas ve hızlı formların geçisine neden olan aminoasitler simulasyonhesaplamalarıyla gözlenmistir. Bunun yanı sıra ne türde ikincil yapılarınkonformasyonel degisimi etkiledigini anlamak için GROMACS yazılımındakiDSSP programı kullanılmıstır. Böylece trombin yapılarındaki - heliks ve -tabakaikincil yapılarının kararlılıgı belirlenmistir. Konformasyonel degisiklige neden olanaminoasitlerin düzensiz ikincil yapıda oldukları gözlenmisitr.MD simulasyon çalısmalarında çözünürlük degerleri çok önemlidir. En düsükçözünürlük degerine sahip (1.92 Å) olan 1ppb trombin yapısı en iyi sonuçlarıgöstermistir. Bu çalısmada yapının dengeye 2 ns dan sonra geldigi görülmektedir.Ayrıca 1ppb yapısı simulasyon boyunca en küçük potansiyel enerjiye sahiptir.Dahası konformasyonel degisim dört yapı içinde sadece 1ppb yapısındagözlenmistir. Bu çalısmada, 1ppb trombin yapısının vücut içerisindekikonformasyonel degisimini anlamak için moleküler dinamik simulasyonlarındakullanılabilecek en mantıklı yapı oldugu belirlenmistir. Enerji minimizasyonu için ilkönce basit bir minimizasyon yöntemi olan ani-düsüs (steepest descent).dahasonra ise daha çok zaman alan ve daha dogru sonuçlar veren karsılıklı degisim(conjugate gradient) metodları kullanılarak literatürdeki sonuçlardan dahaanlasılabilir sonuçlar elde edilmistir. Thrombin is a serine protease and remains the primary target for the developmentof novel anticoagulants because of its position at the end of the blood clottingcascade. Thrombin plays two important roles in hemostasis. The switch fromanticoagulant (slow) to procoagulant (fast) form of thrombin known as the thrombinparadox (Griffin,1995) and has a great interest. Although there are some X-rayand experimental studies in the literature, these studies aren?t sufficient to explainthe structural and conformational changes of thrombin.In recent years, Molecular Dynamics (MD) Simulation is often used for studyingthe protein structures, dynamics and stability. In this study it was aimed to observeslow fast transition and effect of starting structures on simulation studies wereaimed by MD simulation calculations. Obtained results may lead to innovativestrategies for control of thrombin functions in vivo.In this study for the crystal structures of thrombin having different resolution values(resolution values 1.92 Å, 2.20 Å, 2.0 Å , 2.20 Å and respectively pdb codes 1ppb,1qhr, 1qj1,1qj6) GROMACS 3.3.1 software package was used for moleculardynamic simulation calculations. At the first part of the work described herein, theinitial structures were selected with the examination of all thrombin structures inProtein Data Bank. Thus it was aimed to identify the full lenght chain thrombinstructures. In the simulation part firstly the optimum conditions were identified. Theobtained out.put files were analyzed by using GROMACS software. The stabilityand conformational drift of a protein is provided by root mean square deviation ofthe protein coordinates from their initial values as a function of time. In this waywhether the protein reached the equilibrium or not during the simulation can beidentified. Moreover the effect of aminoacids on conformational transition wereidentified.. An approximate measure of the average size of the structure, thepotantial energy fluctuations versus simulation time, number of hydrogen bonds,solvent accessible surface area were examined, and the residues that are linkedto the slow fast transition of thrombin structures were observed by simulationcalculations. Besides to understand which kind of secondary structures effect theconformational changes DSSP program in GROMACS software package wasused. Thus the stability of -helix and - sheet secondary structures wereidentified in thrombin structures. It was observed that the residues that lead toconformational changes are in irregular coil secondary structure.The resolution values are very important for MD simulation studies. 1ppb thrombinstructure has the smallest resolution value (1.92 Å) shows the best results. In thisstudy it was seen that 1ppb reached equilibrium after 2 ns. Also 1ppb has thelowest potential energy during the simulation. Moreover, conformational transitionwas observed only for 1ppb among these four structures. In this study, it wasshown that 1ppb is a reasonable structure that can be used in molecular dynamicssimulations to understand the conformational changes of thrombin in vivo. Forenergy minimisation with the usage of firstly steepest descent which is a basicminimisation method and then conjugate gradient which is more time-consumingand effective method we obtained more reasonable results than literature. Also weobserved that Na + binding sites of 1ppb thrombin structure remain unchangedduring the simulation for both slow and fast forms.Keywords Thrombin, conformation, Molecular dynamics simulation, GROMACS,conformational changes 134
