5. Surgical site infections after pancreaticoduodenectomy: preoperative biliary system interventions and antimicrobial prophylaxis

Abstract

Background: The rate of surgical site infection (SSI) after pancreaticoduodenectomy (PD) is high and insertion of preoperative bile duct catheterization (PBDC) predispose a high risk of SSI with multidrug resistant (MDR) microorganisms. Aim: To describe the effects of PBDC and the prophylaxis in development of SSI. Methods: We conducted a retrospective study between January 01, 2010 and December 2018 including the patients with PD and total pancreatectomy. Findings: In total 214 consecutive patients were included. The PBDC was inserted to 63 (29%) patients. The rate of intraoperative bile fluid culture positivity was higher among the patients with PBDC (84% vs. 17% respectively, p < 0.001). The SSI was detected in 52 patients (24%). In multivariate analysis, the rate of SSI was found to be higher among the patients with PBDC (OR: 2.33, 95% Cl: 1.14–4.76, p = 0.02). As the etiologic agents of SSI, Pseudomonas spp. and MDR pathogens were mainly detected in PBDC group. The resistance to ampicillin-sulbactam was significantly higher in the PBDC group (87.5% vs. 25%, p = 0.012). The similar bacterial species both in bile fluid and the surgical site were detected in 11 (21%) patients with SSI. Among 8 patients (15%), antimicrobial susceptibility was the same. Only in five out of 52 (10%) patients, the SSI pathogens was susceptible to the agent that was used for surgical prophylaxis. Conclusion: Unnecessary catheterizations should be avoided. By considering the increasing prevalence of resistant bacteria as the cause of SSI, the clinicians should closely follow-up their patients for choosing the proper antimicrobials., NA

Abstract

Marmara University, In our study was investigated the in vitro effects of Colistin(4 ?g/ml), alone with the combination of tigecycline( 0.87 ?g/ml), imipenem (30 ?g/ml) and rifampicin (7 ?g/ml) at therapeutic concentrations and malondialdehyde (MDA) level, superoxide dismutase (SOD), glutathione peroxidase (GSH-Px) and myeloperoxidase (MPO) activity on polymorphonuclear leukocyte (PMN) functions (phagocytic and intracellular killing activity) of patients with bronchiectasis. PMNLs were isolated from venous blood by Ficoll-hypaque gradient centrifugation method. Phagocytic activity and intracellular killing activity were assayed by modified Alexander’s method. MDA levels were determinated by Beuge’s method, SOD, GSH-Px and MPO activities were assayed by Sun’s, Paglia and Valentine’s and Hillegas’s, methods respectively. Abstract

Bu laboratuarlarında 2004-2006 yılları arasında yürütülmüştür. Denemede materyal olarak, Tarla Bitkileri Bölümü’nde geniş bir oğulotu populasyonundan seçilen tek bitkilerden çoğaltılan 76 hat kullanılmıştır. Çeliklerin dikimi 2004 yılında 60x40 cm bitki sıklığında yapılmıştır. İlk yıl plantasyonun kuruluş yılı olmuş, ikinci (2005) ve üçüncü (2006) yıllarda ikişer biçim yapılmış ve değerlendirilmiştir. 2005 yılında hatlara ait ortalama bitki boyu 37.73-56.02 cm, yeşil herba verimi 292.46-677.76 g/bitki, drog herba verimi 105.51-256.57 g/bitki, yeşil yaprak verimi 188.14-487.31 g/bitki, drog yaprak verimi 79.41-177.65 g/bitki, uçucu yağ oranı %0.036-0.100 arasında değerler alırken; 2006 yılında ortalama değerler sırasıyla 26.27-45.59 cm, 372.28-825.36 g/bitki, 104.46-176.42 g/bitki, 204.35-471.41 g/bitki, 48.20-114.54 g/bitki, %0.018-0.058 arasında değişim göstermiştir. Genel olarak 2005 yılında Hat-22, 2006 yılında Hat-7 ön plana çıkmıştır

Abstract

In this work, a matrix method based on Laguerre series to solve singularly perturbed second order delay parabolic convection-diffusion and reaction-diffusion type problems involving boundary and initial conditions is introduced. The approximate solution of the problem is obtained by truncated Laguerre series. Moreover convergence analysis is introduced and stability is explained. Besides, a test case is given and the error analysis is considered by the different norms in order to show the applicability of the method.

26. Ankara Yöresinde Kongenital Anomalilerin Dağılımı

Abstract

Frequency of congenital malformation in Ankara region of Turkey✓Between 1985-1990, 133974 newborn babies who were born in Ankara Maternity Hospital resulting from pregnancies of 28 or more weeks were reviewed retrospectively to find out the incidence of various congenital malformations. Of the 13397 babies, 567 (4.2/1000) showed gross congenital anomalies which is a low value compared to previous results from Turkey. In the study central nervous system anomalies were the highest similar to previous report by Say et al.✓1985-1990 yıllarında Ankara Dr. Zekai Tahir Burak Kadın Hastanesinde 28 hafta veya daha uzun gebelik süresi sonunda doğan 133974 bebek, gros kongenital anomali varlığı bakımından retrospektif olarak değerlendirildi. Toplam 133974 bebeğin 567'sinde (4.2/1000) gros konjenital anomali saptandı. Bu değer ülkemizden daha önce rapor edilen değerlere göre daha düşüktü. Araştırmada santral sinir sistemi anomalileri en sık rastlanılan anomali gru¬bunu oluşturmaktaydı (4.2/1000). Bu değer ülkemizdeki eski değere uygunluk göstermekteydi.

Abstract

The increasing clinical importance of drug-resistant bacterial and fungal pathogens has lent additional urgency to microbiological research and novel antimicrobial compounds development. For this aim, a series of N-[(Benzhydryl) / (l-phenylethyl)]-2-pyrazolo[3,4-c]pyridin-l-yl acetamide derivatives (3a-m) were synthesized by reacting 2-chloro-N-(benzhydryl)acetamide (la) / 2-chloro-N-(1-phenylethyl)acetamide (lb) and pyrazolo[3,4-c]pyridine derivatives (2). The chemical structures of the compounds were elucidated by elemental analyses, IR, 1 H-NMR, FAB + -MS spectral data. Their antimicrobial activities against E. coli (ATCC 11229), S. aureus (ATCC 6538), B. cereus (ATCC 11778), P. mirabilis (ATCC 14153), P. aeruginosa (ATCC 1539), K. pneumoniae (ATCC 4352), C albicans (ATCC 26555), C guilliermondii (KUEN 998), C pseudotropicalis (kefyr) (KUEN 1014), and C krusei (ATCC 6258) were investigated. The results showed that some of the compounds have slight activity against B. cereus and S. aureus. Whereas, the same compounds did not show any significant antifungal activity against Candida species.

In this work, we develop a matrix method based on collocation points and Laguerre polynomials to obtain the numerical approximations of the onedimensional nonlinear Klein-Gordon equations. The method is applied on some numerical examples. Also, the numerical results and their exact solutions are compared with other applications. Abstract

The aim of this study was to share our experience in the prenatal diagnosis (PND) of organic acidemias (OAs) in our clinic. This study consisted of 10 cases in whom an invasive prenatal diagnostic test (IPNDT) was performed by a single physician for the PND of OAs. Median maternal age, parity, gestational week of IPNDT, prenatal test indications, OA types, method of IPNDT, IPNDT results and gestational outcomes were evaluated. Targeted mutation analysis was performed in fetal DNA for the specific mutations by using polymerase chain reaction (PCR) and direct Sanger sequencing. The diagnosis was confirmed by genetic targeted mutation analysis after birth. Median maternal age, parity and gestational week of IPNDT values were 30 (range 21-35), one (range 0-4) and 11.5 (range 11-17), respectively. Indications for IPNDT were mother being a carrier of the disease for one case (10.0%) and at least one child with OA in the family for nine cases (90.0%). Organic acidemia types investigated were maple syrup urine disease (MSUD), methylmalonic acidemia (MMA) and isovaleric acidemia (IVA) in five (50.0%), three (30.0%) and two (20.0%) patients, respectively. Chorion villus sampling (CVS) was done in seven (70.0%) patients and amniocentesis was performed in three (30.0%) patients. Eight fetuses (80.0%) were found to be healthy and two fetuses (20.0%) were found to be affected (one case with IVA and one case with MMA). The two pregnancies (20.0%) with affected fetuses were terminated. Prenatal diagnosis of OAs is critical. Appropriate prenatal counseling should be given to families with known risk factors.

Denizde, emniyeti en üst düzeye çıkarmak için risk faktörlerini modellemek ve risk tabanlı tasarım araçlarını kullanmak önemlidir. Emniyeti arttırmak ve müşteri taleplerini karşılamak için etkin risk modelleme teknikleri ve karar verme araçlarının geliştirilmesi ve uygulanması gerekmektedir. Bu çalışmada, mevcut bir deniz ambulans teknesi operasyon riskleri incelenmiş ve mevcut tekne risk tabanlı bir yaklaşımla yeniden tasarlanmıştır. Risk değerlendirmesi ve model tasarımında, Hata Türü ve Etki Analizi (FMEA) kullanılmış ve risk öncelik sayıları (RPNs) hesaplanmıştır. Bu çalışmadan elde edilen sonuçlar, deniz ambulans teknelerinin emniyetini arttırmaya ve potansiyel risklerin önlenmesine veya azaltılmasına katkıda bulunacaktır.

46. EFFECTS OF SEMEN CHARACTERISTICS ON IUI COMBINED WITH MILD OVARIAN STIMULATION

Abstract

To determine the influence of sperm parameters inseminated on the outcome of intrauterine insemination (IUI) in patients undergoing ovarian stimulation with clomiphen citrat (CC) or human menopausal gonadotropin (HMG) therapy, a retrospective review was performed for 2 years on data from the IUI program. 190 couples underwent a total of 268 IUI cycles in which CC or HMG was used for ovulation induction. The initial sperm concentration (mil/ml), motility (percent), preprocessing total motile sperm (TMS) count (million), fast motile sperm (percent) and postprocessing sperm concentration (mil/ml), motility (percent), TMS count, fast motile sperm (percent), sperm morphology, hypoosmotic swelling (HOS) scores, semen leuocytes, and bacteria were analyzed. 268 inseminations were followed by a pregnancy rate of 12% and couple pregnancy rate of 17%. On multivariable logistic regression analysis, total motile sperm (TMS) count, percent motility, and percent of fast motile sperm were independent prognostic factors of fertility. The impact of the preprocessing and postprocessing sperm parameters on pregnancy outcome after IUI was evaluated. There was a trend toward an increasing percent of conception with increasing TMS count, motility, and percent of fast motile sperm. The TMS count, motility and percent of fast motile sperm independently predict success with IUI. Patients with original sperm motility ≥30% had a higher cumulative pregnancy rate (74%) than patient with motility <30% (p < 0.005). Pregnancy rate increased 4 times with motility of ≥30%.

Abstract

Bulbs of Sternbergia fischeriana collected from the village Taşlica of the province of Marmaris in the first weeks of March, April and May 1998 were planted after separating them into 4 groups based on size with three replications each at the experimental farms of the Department of Field Crops, Faculty of the Agriculture, University of Ankara, Turkey during 1998-2000. Percentage of emerging plants, percentage of flowering plants, percentage of plants setting fruits, number of leaves per plant and the plant height showed a range of 60.00-96.67 %, 0-13.69 %, 0-7.50 %, 2.36-3.86 % and 12.61-17.69 cm during 1999 and 60.00-100.00 %, 33.60-96.97 %, 5.90-42.60 %, 5.00-11.33 and 15.14-19.23 cm during 2000, respectively.
