4. Profile defects caused by inhomogeneous longitudinal strain distribution in roll forming.

- Abstract
Roll forming is a sheet metal forming operation that incrementally forms flat sheets into a desired profile geometry. The process is characterized by a high material utilization and a high output quantity. Concomitant with these advantages, profile defects such as bow and twist of the profile can occur. In the literature, an inhomogeneous longitudinal strain distribution across the profile cross-section is considered to be the cause of these defects. However, a quantitative cause and effect analysis is missing up to now. This paper presents an analytical model that shows a quantitative relationship between profile defects and the underlying longitudinal strain distributions. The model can be used to calculate the longitudinal strain distribution of a roll-formed profile across its cross-section based on given values for bow and twist or vice versa. It is compared with results from simulations and experiments and clearly reveals the cause for twist and bow in roll forming. [ABSTRACT FROM AUTHOR]

2022 Innovations in Intelligent Systems and Applications Conference, ASYU 2022 -- 7 September 2022 through 9 September 2022 -- 183936 First-trimester maternal screening is a widely used test for detecting fetal aneuploidies and neural tube defects for over two decades. Human chorionic gonadotropin hormone (beta-hCG) and pregnancy-associated plasma protein A (PAPP-A) are two serum biomarkers that are analyzed in this screening. The thyroid hormone is a critical hormone for normal pregnancy and fetal development. During the first half of pregnancy, placental and fetal development depends on the thyroid hormone levels in the mother. Therefore, thyroid abnormalities in the mother can result in unfavorable pregnancy outcomes such as intrauterine growth restriction, miscarriage, hypertensive disorders, premature birth, and an increase in the risk of low IQ in the newborn. In this study, we analyzed the first-trimester screening data collected from 410 pregnant women who were seen at Yale University Hospital Prenatal Unit; and checked for possible correlations of TSH levels with maternal characteristics, racial group PAPP-A MoM levels. © 2022 IEEE.

The early myocardial peak (Ea) velocity was significantly lower in AD patients, whereas the late diastolic (Aa) velocity and E/Ea ratio were similar between the two groups. In Alzheimer patients, stiffness index and pressure-strain elastic modulus were higher, and distensibility was significantly lower in the AD group compared to the control. Interatrial electromechanical delay was significantly longer in the AD group.Conclusion: Our findings suggest that patients with AD are more likely to have diastolic dysfunction, higher atrial conduction times, and increased arterial stiffness compared to the controls of same sex and similar age. Amaç: Laparoskopik kolesistektomi ameliyatında, intraoperatif ve postoperatif açık ameliyata geçilen hastaların incelenerek risk faktörlerinin ortaya konması amaçlanmıştır. Gereç ve Yöntem: Ekim 1993-Mart 2009 tarihleri arasında Ondokuz Mayıs Üniversitesi Tıp Fakültesi Genel Cerrahi Anabilim Dalı’nda aynı ekip tarafından benign safra yolları hastalığı tanısıyla laparoskopik kolesistektomi uygulanan 647 hasta ve açık ameliyata geçilen 35’i, retrospektif olarak incelendi. Açık ameliyata geçiş endikasyonlarımız literatür bilgileri ışığında tartışıldı. Bulgular: Laparoskopik kolesistektomi planlanarak ameliyatına başlanılan 647 hastanın 35’inde (%5.4) erken veya geç dönemde laparotomiye geçilmek zorunda kalınmıştır. 647 hastanın 482‘si kadın, 165’i erkekti. Açık ameliyat gerektiren olgulardan 17’si erkek, 18’i kadındı. Açığa dönülen erkek ve kadın hastaların yaş ortalamaları arasında anlamlı fark yoktu (p0.306). Ancak erkek hastalarda açığa geçiş oranı anlamlı olarak yüksekti (p0.009). İlk 100 hastanın 9’unda açığa dönüş olmuşken (%9), sonraki hastalarda bu oran % 4.75 bulundu (p0.137). En sık açık ameliyata geçiş nedenleri; mevcut akut kolesistite bağlı diseksiyon güçlüğü (13 olgu), geçirilmiş akut kolesistite bağlı yapışıklıklar (10 olgu), biliyer vasküler anomali (9 olgu) idi. Sonuç: Hastalarımızda en sık açığa geçiş nedenleri mevcut akut kolesistit ve geçirilmiş akut kolesistit atağına bağlı oluşan yapışıklıklar olarak bulundu. İlk 100 hastada açık ameliyata dönüş oranı daha yüksek idi. Erkek cinsiyetin açığa geçiş oranı, istatistiksel olarak anlamlı yüksek bulundu. Aim: The determination of the risk factors for the conversion of laparoscopic cholecystectomy to open surgery was aimed in this study. Material and Methods: The retrospective analysis of 647 patients operated on laparoscopic cholecystectomy was made. Thirty five patients were converted to open surgery. The indications for conversion to open surgery were discussed under the view of pertinent literature. Results: All of the 647 patients were planned to be operated on laparoscopic cholecystectomy. In 35 patients (5.4%), early or late open surgery (laparotomy) was needed.Four hundred and eigthy two patients were female, 165 were male. Seventeen patients converted to open surgery were male and 18 were female. There was no statistically significant difference in age between males and females who were converted to open surgery (p0.306). But the rate of male patients converted to open surgery were significantly greater than the rate of females (p0.009). In nine patients of first 100, open surgery was needed (9%); after first 100 patients the rate of conversion to open surgery became 4.5% (p0.137). The most frequent indications for open surgery were difficult dissection related with ongoing and previous acute cholecystitis (13 cases), adhesions related with old attack of acute cholecystitis and biliary vascular anomalies (9 cases). Conclusion: The most frequent indications for conversion to open surgery in laparoscopic cholecystectomy were existing acute cholecystitis and adhesions related with previous acute cholecystitis. The conversion rates in first 100 cases were more than that of later cases related with the experience. Male patients had a significantly increased rate of conversion to open surgery.

No previous study has yet reported the effects of surgical treatment of hydatid cysts on spleen volume. Our aim was to use computed tomography images to evaluate the volume differences in the spleen in postoperative periods in patients who had undergone operations for hydatid cysts. The abdominal computed tomography scans of 20 patients who had undergone drainage of a hydatid cyst and omentopexy operation were examined retrospectively. The volume of the liver, the cyst, and the spleen were estimated using the Cavalieri principle of stereological methods. The volume fraction of the hydatid cyst to the liver was also estimated using the obtained volume data. The volumes of the spleen (± SD) were 302.0 ± 115.1 cm3 and 259.5±82.4 cm3 in the postoperative 1st week and 6 months, respectively (p0.05). The volume of spleen decreased after hydatid cyst surgery. This may be due to the decreased portal venous pressure of the liver. Additionally, the loss of immune stimulation may have resulted in reduction in splenic volume. Our results also indicated that stereological methods can be used to monitor volume changes of the liver and spleen. © 2011 OMU All rights reserved.

16. When does the risk of complication of cholecystectomy related with biliary and vascular anomalies increase?

Anomalies of gall bladder, biliary tract and their vascular anatomy are rare. They are usually diagnosed intraoperatively. Presence of anomaly increases rate of surgical complications. The risk factors related with these anomalies were examined .Six hundred ninty five patients were retrospectively analysed. The statistical analysis of the relation between anomalies of biliary tract and age, gender and complications was made. Fifty six patients (8.1 %) of the 695 were < 30 years old (Group I), 639 patients (91.9 %) were >30 years old(Group II). Male/female ratio was 488/207. The mean age of Group I is 26± 3.593, the mean age of Group II is 53.48± 12.685. 71.4 % (n=20) of the 28 patients (4.02 %) with anomaly were female, 28.6 % (n=8) were male. The mean age of patients with anomaly was 39.21±14.376 years. Nine patients in Group I, 19 patients in Group II had anomalies. Fourteen biliary, 4 biliary and vascular, 8 arterial anomalies, 2 duodenal diverticula were observed. In 5 patients (17.85 %) with anomaly and in 31 patients (4.64 %) without anomaly, conversion to open cholecystectomy was needed. Complications occured in 1 patient (1.75%) in Group I, in 19 patients (2.97 %) in Group II. No mortality was seen. The number of biliary and vascular anomalies detected during cholecystectomy was significantly higher in patients< 30 years of age. Their presence may increase complications of cholecystectomy. For the prevention of complications, preoperative diagnostic evaluation is necessary. © 2010 OMÜ Tüm Haklari Saklidir.

17. Is calcified hydatid cyst absolutely dead? A case report and review of the literature

How and when does a hydatid cyst die in humans? There is an ongoing debate in medicine about this subject. Calcification of the hydatid cyst wall is believed to indicate the death of the contents of the cyst. However, rarely, the cyst may be alive even though the wall is calcified. With this report, we presented a patient with a hydatid cyst with calcified wall. Our patient presented here was followed up for 8 years after the diagnosis and sufficient and effective treatment was not given thinking that the hydatid cyst was dead because of the calcified cyst wal, but intrabiliary rupture occurred after the follow-up. Criteria for the viability of hydatid cyst are discussed and the literature is reviewed. © 2010 OMU All rights reserved.

18. Liver abscess occuring after laparoscopic cholecystectomy in a case of biliary ascariasis: Case report

The pyogenic liver abscess usually develops secondary to the cholangitis. It may also be seen following invasive cholangiopancreatographic procedures or surgery of the liver and biliary tract. We here report a liver abscess developed eight days after laparoscopic cholecystectomy following endoscopic retrograde cholangiopancreatography and sphincterotomy in a diabetic patient with cholecystocholedocholithiasis and biliary ascariasis. The patient was admitted to the hospital with fever, chills and leukocytosis. Ultrasonography and computerized tomography revealed a right lobe liver abscess extending to the right subdiaphragmatic space and right pleural effusion. The abscess and pleural effusion were treated by percutaneaous drainage, thoracentesis and antibiotics. The laparoscopic cholecystectomy after endoscopic retrograde cholangiopancreatographic procedures, especially in complicated situations like in this case with stones and ascaris in common bile duct, may be delayed. © 2010 OMÜ Tüm Haklari Saklidir.

19. Endoskopik Olarak Durdurulamayan Üst Gastrointestinal Sistem Kanamalarında Acil Cerrahi Tedavi Sonuçlarımız

- Abstract
Üst gastrointestinal sistem kanamaları halen acil cerrahi girişim gerektiren sorunlardan biridir. Endoskopinin teşhis ve tedavideki rolünün artmasına karşın endoskopik olarak kanama kontrolünün yapılamadığı, yaşlı ve ek hastalıkları olan hastaların acil şartlarda ameliyata alınmaları söz konusu olmaktadır. Kliniğimizde endoskopik olarak kanama kontrolü yapılamayan ve hastanede yatarken kanaması olup acil ameliyat edilen 45 hastanın genel özellikleri, cerrahi tedavi şekli, morbidite ve mortalite sonuçları incelenmiştir. Acil endoskopik incelemelerde olguların 17'sinde (%38) aktif kanayan duodenal ülser saptanmış, 15'inde (%33) masif kanama nedeniyle, 10'unda (%22) kanamanın durması nedeniyle kanama odağı bulunamamıştır. Bu 10 olguda endoskopi sonrası tekrar kanama olması ve hipotansiyon gelişmesi üzerine acil cerrahi tedavi uygulanmıştır. Üç olguda (%7) kanayan marjinal ülser saptanmıştır. Olguların %16'sında nonsteroid antiinflamatuar, %11'inde antikoagulan ilaç kullanımı öyküsü mevcuttu. Olguların %48'inde ek sistemik hastalık bulunmaktaydı. Ortalama preoperatif eritrosit süspansiyonu transfüzyon miktarı 7 ünite, ameliyat öncesi ortalama hemoglobin değeri 6.4 g/dl idi. En sık uygulanan cerrahi tedavi yöntemi bilateral trunkal vagotomi + piloroplastiydi (%38). Mortalite oranımız %18'di. Sonuç olarak acil şartlarda endoskopinin ulaşılabilir olması, endoskopistin teşhis ve endoskopik tedavideki deneyimi, hastaların yaşlı ve ek hastalıklarının mevcudiyeti ile kan transfüzyon miktarı; acil cerrahi tedavi kararının, cerrahinin şeklinin belirlenmesinde ve prognozda etkili olmaktadır.Results of Emergent Surgery for the Upper Gastrointestinal BleedingUpper gastrointestinal bleeding is still a major problem requiring emergent treatment. Although the diagnostic and therapeutic role of the endoscopy has been increased, elderly patients with endoscopically uncontrolled bleeding and additional systemic diseases, need surgical treatment. The general properties, modality of surgical treatments, morbidity and mortality of the forty-five patients who had endoscopically uncontrolled bleeding were evaluated. Emergent endoscopy determined active bleeding duodenal ulcer in 17 cases (38%). The site of bleeding couldn't be found in 15 patients (33%) because of massive bleeding, and in 10 patients (22%) there was no focus of bleeding in endoscopic examination. We emergently operated these 10 patients because of bleeding and hypotension recurrence after endoscopy. In 3 cases (%7), bleeding marginal ulcer was found. Sixteen percent of cases had history of using nonsteroid antiinflammatory drugs, 11% was using anticoagulant therapy. Forty-eight percent of the patients had additional systemic diseases. Mean value of preoperative erythrocyte transfusion was 7 units and mean preoperative hemoglobin value was 6.4 g/dl. The most frequently performed operative procedure was bilateral truncal vagotomy and pyloroplasty (38%). The mortality rate was 18%.As a result; the availability of endoscopic intervention in emergent cases, the diagnostic and therapeutic experience of the endoscopist, the age and the systemic diseases of the patients, the amount of blood transfusion are the determinants of the decision of the emergent surgery and type of surgery as well as prognosis

Purpose: Today, synthetic patches are being used in open or laparoscopic ventral hernia repairs widely. We have aimed to examine adhesive potentials of various patches in rats treated with laparoscopic ventral hernia repairs. Material and Method: Rats were divided into six different groups with 8 subjects on each group: control (group 1), polypropylene (PP) (group 2), hyaloronic acid/carboxymethylcellulose coated polypropylene (PP+H/K) (group 3), expanded polytetraflouroethylene (ePTFE) (group 4), anti-adhesive collagen coated polyester (PO+AHK) (group 5) and polyester (PO) (group 6). Only laparoscopy was practiced on the rats in control group. PP, PP+H/K, ePTFE, PO+AHK and PO patches were placed on front abdominal walls of the rats by laparoscopy method. After two weeks, the rats were sacrificed. Frontal abdominal walls were opened with a wide (U) cut, adhesion force and adhesion area were evaluated. Photos of the adhesions as a result of patches were taken in all rats. Results: Infra-abdominal adhesion was not observed in the rats of control group. Adhesion was observed in all the rats with patches. But there were differences in terms of adhesion force and area. Acute abdomen presenting with generalized abdominal rigidity is usually considered asa surgical emergency. Tetanus infection may also cause abdominal rigidity as in an acute abdomen. Latency of sign and symptoms of tetanus and/or the unawareness of the physician may lead to an unnecessary laparotomy. The nonimmunized middle aged and elderly people of lower socioeconomical status especially in the rural areas are at higher risk of tetanus. In this report a 57 years old patient who was admitted to the emergency unit with dispnea, abdominal pain and operated upon with the finding of abdominal rigidity is presented. Postoperative extubation of the patient could not be achieved and the patient was referred to an intensive care unit for respiratory support with the eventual diagnosis of tetanus. Despite all efforts the patient expired on the thirteenth day postoperatively. Tetanus should be kept in mind in the gamut of causes when a dignosis of "acute abdomen" is entertained especially in the above mentioned nsk groups.

22. Results of emergent surgery for the upper gastrointestinal bleeding

Upper gastrointestinal bleeding is still a major problem requiring emergent treatment. Although the diagnostic and therapeutic role of the endoscopy has been increased, elderly patients with endoscopically uncontrolled bleeding and additional systemic diseases, need surgical treatment. The general properties, modality of surgical treatments, morbidity and mortality of the forty-five patients who had endoscopically uncontrolled bleeding were evaluated. Emergent endoscopy determined active bleeding duodenal ulcer in 17 cases (38%). The site of bleeding couldn't be found in 15 patients (33%) because of massive bleeding, and in 10 patients (22%) there was no focus of bleeding in endoscopic examination. We emergently operated these 10 patients because of bleeding and hypotension recurrence after endoscopy. In 3 cases (%7), bleeding marginal ulcer was found. Sixteen percent of cases had history of using nonsteroid antiinflammatory drugs, 11% was using anticoagulant therapy. Forty-eight percent of the patients had additional systemic diseases. Mean value of preoperative erythrocyte transfusion was 7 units and mean preoperative hemoglobin value was 6.4 g/dl. The most frequently performed operative procedure was bilateral truncal vagotomy and pyloroplasty (38%). The mortality rate was 18%. As a result; the availability of endoscopic intervention in emergent cases, the diagnostic and therapeutic experience of the endoscopist, the age and the systemic diseases of the patients, the amount of blood transfusion are the determinants of the decision of the emergent surgery and type of surgery as well as prognosis.

- Abstract
OBJECTIVE: The effect of vitamin D and renin-angiotensin-aldosterone system blockade medications in pathophysiology of contrast induced nephropathy (CIN) is controversial. The effects of paricalcitol (active vitamin D analogue) and losartan treatments in an experimental model of CIN were investigated in this study. MATERIALS AND METHODS: Thirty-six male Wistar albino rats were examined in five treatment groups. Placebo group (Group A; n = 4) received no active medication; control group (Group B; n = 8) received only contrast media (CM); Group C (n = 8) received paricalcitol; Group D (n = 8) received losartan and Group E (n = 8) received paricalcitol plus losartan. CIN was induced by NG-nitro-L-arginine methyl ester and indomethacin before iohexol injection. Renal histopathological findings were categorized and renal immunohistochemical examinations by caspase-3 rabbit primary antibody were performed. RESULTS: Creatinine and cystatin C levels significantly increased in the treatment groups, compared to Group A. However, creatinine levels were not significantly increased in Groups C, D and E compared to Group B. Compared to Group B, a significant increase of cystatin C levels was observed in Group D (p < 0.01). In Group E, when paricalcitol treatment was added to losartan treatment, cystatin C levels were similar to Group B (p = 1.00). In histopathological and immunohistochemical examination frequency of Grade 2/3 tubular necrosis and renal caspase 3 activity scores were significantly higher in the losartan treatment group compared to the other treatment groups. The histopathological effects related to losartan treatment were found to be reversed when paricalcitol treatment was combined. CONCLUSIONS: Our findings suggest that paricalcitol treatment counteracts increased contrast induced nephropathy caused by losartan. These findings warrant further clinical studies to investigate the benefit of paricalcitol in CIN prophylaxis. [ABSTRACT FROM AUTHOR]

In this study, serum samples of 13011 cattle from 31 dairy herds located in various regions of Turkey were tested for presence of neutralising antibodies against BHVI using the serum neutralisation (SN) technique. It was found that 97% (30/31) of the herds selected had seropositive animals and the frequencies of seropositivity were between 0.5-79.5% in the surveyed herds. Furthermore, recommendations to the control and eradication programme of the BHV1 infection were presented.

The article examines the prevalence of antibodies to major bovine respiratory viruses in cows infected or uninfected with bovine immunodeficiency virus (BIV) under field conditions in Turkey. The serological test results showed the most prevalent infections were bovine adeno-virus serotype 1 (BAV-1) and BAV-3 in both the BIV-infected and BIV-negative groups, while the seroprevalance of bovine herpes-virus-1 (BHV-1) was the lowest.

Introduction: It is the responsibility of the ethical committees to ensure that the studies carried out on volunteers are carried out in accordance with ethical rules. In this study, it is aimed to analyze the deficiencies in the 2018-2021 application forms of the Istanbul Haydarpaşa Numune Training and Research Hospital Clinical Research Ethics Committee. Methods: The number of applications and revisions, the quality and type of research, area of application and acceptance rate were analyzed retrospectively in the archives of the ethics committee, in 598 files that were finalized between January 2018 and December 2020. Results: In the examined period, the total number of applications has doubled as of the end of 2020 compared to January 2018 (131, 194 and 273 for the years 2018, 2019 and 2020, respectively), and almost all of the applications are non-pharmacologic clinical trials. Among these studies, the highest number of applications are individual research projects, and more than half of them are prospective (63.06%, 66.46% and 58.30%, respectively). The number of applications for which corrections were requested is 77 (58.77%), 107 (55.15%) and 107 (39.19%) according to years. Most of the corrections requested are method related. The highest increase in the number of revisions was in the misidentification of the research type (0.78%, 6.02% and 6.94% for 2018-2020, respectively). Discussion and Conclusion: It was determined that the researchers had difficulties in the study methodology, informed consent and preparation of the scientific basis. Planning periodical Good Clinical Practices (GCP) trainings on an institutional basis under the leadership of ethics committees and that the specialty students have completed their current GCP training before starting their specialty thesis studies will ensure that the research projects to be carried out will be of higher quality in terms of ethics and science

- Abstract
Although guidelines strongly recommend low levels of low-density lipoprotein cholesterol (LDL-C) in patients with acute coronary syndrome (ACS), these goals are not achieved in many patients. We present expert recommendations for the in-hospital and post-discharge management of lipid-lowering therapy in ACS patients in Türkiye. A group of expert cardiologists comprising members of the Turkish Society of Cardiology and leading lipidologists who have all worked in national cardiology lipid working groups for at least 4 years, considered the optimal approach to lipid management in ACS patients in Türkiye, taking into account Turkish healthcare infrastructure and issues from real-world practice. Novel standardized algorithms covering the first year after an ACS event were developed. The main elements of the proposed approach are summarized. The in-hospital strategy includes lipid profile assessments and commencement of statin therapy, including appropriate measures for patients with possible familial hypercholesterolemia or with a history of recurrent cardiovascular events. The role of a specially-trained nurse, use of a patient follow-up card, and the provision of patient information and referrals to affiliated healthcare professionals, are covered. The post-discharge strategy covers follow-up visits to monitor the patient's progress and recommends timepoints at which modifications to treatment (based on LDL-C levels) should be made. A country-specific approach to lipid management in ACS patients in Türkiye is proposed. Further work is needed to determine the best way to implement the strategy, and to validate the recommendations and their application in daily practice.

- Abstract
The Golden Gate method is an efficient tool for seamless assembly of multiple DNA fragments, which uses Type IIS restriction endonucleases, cleaving the DNA outside of their recognition site to release DNA parts from PCR fragments or entry clones, thus allowing the design of overhangs for ligation at will. However, the construction of the entry clones requires the use of other restriction enzyme(s) or cloning techniques and different entry vectors for the individual overhangs. Here, we present a simplified Golden Gate cloning approach termed Golden EGG. It features (1) a single entry vector with a specific cloning site to host the DNA parts; (2) a unique primer design to create the restriction enzyme recognition site to release the fragments with the overhangs at will; (3) the use of a single Type IIS enzyme for the construction of both the entry and destination clones; (4) a specific temperature profile during the digestion-ligation reaction. Our user-friendly, streamlined method retains the key attributes of the Golden Gate technique, while offering the potential to generate compatible parts with any existing Golden Gate toolkit and to be accessible to a wide user base without the need for extensive acquisition of new vectors or expensive enzymes., (© 2024. Objective: To evaluate the position and dimension of the Vidian canal (VC) in Chiari type I malformation (CIM)., Materials and Methods: Radiologic views of 49 CIM (mean age: 23.58±15.62 y, sex: 23 males/26 females) and 51 healthy subjects (mean age: 42.50±20.12 y, sex: 21 males/30 females) were included in this computed tomography study., Results: In comparison with controls, the VC angle and the distance of the round foramen to VC were greater in CIM, but VC length and the distances of the superior wall of the bony sphenoidal sinus, midsagittal plane, and vomerine crest to VC were smaller in CIM. Relative to the sphenoid bone, the position of VC in CIM was determined as type 1 (59.2%) >type 2 (28.6%) >type 3 (12.2%), whereas in controls as type 1 (54.9%) >type 3 (25.5%) >type 2 (19.6%). Relative to the medial pterygoid plate, the position of VC in CIM was determined as type A (63.3%) >type B (20.4%) >type C (16.3%), while in controls as type B (43.1%) >type A (40.2%) >type C (16.7%)., Conclusion: VC size and position correlated with CIM. 38. Assessment of Lipoprotein (a) Levels in Patients with Atherosclerotic Cardiovascular Disease: Single Center Experience from Türkiye.

Objective: This study aims to evaluate the role of elevated lipoprotein (a) [Lp(a)] levels as a potential contributor to residual risk in individuals with atherosclerotic cardiovascular disease (ASCVD). Considering that approximately 90% of Lp(a) levels are genetically determined and can vary regionally, we assessed Lp(a) levels in a cohort of ASCVD patients from the Turkish population, where data is currently limited., Methods: We conducted a retrospective analysis of data and Lp(a) measurements collected from individuals diagnosed with ASCVD at a single center., Results: The analysis included Lp(a) levels of 1193 consecutive individuals. The mean Lp(a) level was 28.2 mg/dL, with a median of 16 mg/dL and an interquartile range (IQR) from the 25th to the 75th percentile, 7 mg/dL to 39 mg/dL. The highest recorded Lp(a) level was 326 mg/dL. Among the cases, 18.7% exhibited Lp(a) levels ≥ 50 mg/dL, 10.8% had levels ≥ 70 mg/dL, and 5.8% had levels ≥ 90 mg/dL. The mean levels of low-density lipoprotein cholesterol (LDL-C) and total cholesterol (TC) were 132 ± 47 mg/dL and 212 ± 54 mg/dL, respectively. Lp(a) levels were significantly higher in females compared to males. Furthermore, the proportion of females with Lp(a) levels ≥ 90 mg/dL was higher than in males (11.4% vs. 1.4%; P < 0.01). Additionally, a modest but significant correlation was observed between Lp(a) levels and TC (r = 0.075, P = 0.01) as well as LDL-C (r = 0.106, P < 0.01)., Conclusion: This study revealed that Lp(a) concentrations were higher in women and statin users among ASCVD patients and identified a weak but significant correlation between Lp(a) levels and both TC and LDL-C.

39. Real-world experiences in patients with Ewing sarcoma treated at a specialist centre in Turkey.

Objectives: The present study evaluates the clinical outcomes of children, adolescents and adults with Ewing sarcoma and identifies the prognostic factors., Methods: Included in the study were 222 pediatric and adult patients diagnosed with Ewing sarcoma (EwS) who were followed up between 1992 and 2019, and whose data were analyzed retrospectively., Results: The median age of 131 male and 91 female patients included in the study was 13 (1-64). The median follow-up duration of the survivors was 79 months (range, 11-182 months). The 3-year EFS rate of the 222 patients was 34 % (Confidence Interval (CI) (0.158-0.242 %) and the OS rate was 54 % (CI, 0.289-0.590 %). For the non-metastatic patients, the 3-year EFS rate was 47 % and the OS was 68 %, while for the metastatic patients the 3-year EFS rate was 13 % and the OS was 30 %. Of the patient sample, 81 (36, 5 %) survived, of whom 72 were continuously free of disease while the disease persisted in nine, and three developed a secondary neoplasm (2 of whom subsequently died while one survived disease-free). Of the 129 patients who relapsed with metastases and/or local recurrence, eight survived and are disease-free, nine are alive with uncontrolled disease; five were lost to follow-up and 107 died., Conclusion: The findings of the present study suggest metastatic disease at presentation and positive margins after surgery to be of prognostic significance in EwS. Background: This study aimed to compare two-point discrimination in feet with ankle sprains and feet without ankle problems, and to determine whether there was a change in the two-point discrimination values in ankle sprains., Methods: A total of 108 people were included in the study. These subjects were aged between 18 and 40 years and visited the Medical Faculty of Yozgat Bozok University for various reasons in July and September of 2022. These people were divided into two groups: subjects with an ankle sprain and subjects with no ankle problems. Two-point discrimination values measured in millimeters were recorded for both groups using a caliper (esthesiometer) used in six regions of 216 feet. The two-point discrimination threshold values of the feet were compared statistically according to feet with ankle sprains and feet without ankle problems, and in right and left feet., Results: The study determined that the two-point discrimination threshold values measured at the first toe tip, heel, third plantar metatarsal head, medial malleolus, and lateral malleolus in subjects with an ankle sprain was higher than in subjects with no ankle problems. When comparing both feet of the subjects with an ankle sprain, the two-point discrimination threshold value in the heel of the foot with an ankle sprain was higher than in the heel of the foot without ankle problems., Conclusions: The two-point discrimination threshold value was higher in subjects with an ankle sprain than in subjects with no ankle problems. The data suggest that the two-point discrimination threshold may be higher in people with an ankle sprain. Further studies are needed to better understand the two-point discrimination threshold in ankle sprains.

41. Evaluation of YouTube Information Quality About Pes Planus.

Background: The aim of this study was to measure the quality of information about "flatfoot" and "pes planus" presented online on the social media site YouTube and to determine the trends of viewers to medical information on YouTube., Methods: "Flatfoot and pes planus" was typed into the YouTube search module. From the search results, videos with 50,000 views or more, longer than 45 seconds, and containing information about flatfoot and pes planus disease were selected. DISCERN and JAMA scoring, daily average views, number of likes, and number of comments were collected from 53 videos that met the criteria. The profession of the sharer was evaluated in terms of the information quality of the sharing and the orientation of the audience., Results: The mean number of views per day of the examined videos was 2,047. The mean video presentation time was 8 minutes 50 seconds. The mean JAMA score was 2 of 4 and the mean DISCERN score was 38.16 of 75. According to the DISCERN score according to the professions, the video quality was moderate for doctors (41.44 ± 12.99), moderate for physiotherapists (41.91 ± 9.04), poor for coaches (32.78 ± 7.87), poor for patients (34.50 ± 5.32), and weak for others (34.89 ± 14.00). According to the Spearman correlation between DISCERN score and mean daily viewing, significant relationships were found for the doctors (P = .0102) and others groups (P = .0033); however, no significant relationships were observed for the physiotherapists group (P = .1073), the flatfoot patients group (P = .5363), and the coaches group (P = .9111). There were significant relationships between like and comment counts in all groups (doctors, P = .0088; coaches, P = .0069; physiotherapists, P = .0007; others, P =.0018; and patients, P = .0066)., Conclusions: Looking at previous studies, it was observed that the quality of online health information was historically inadequate. Likewise, in our study on YouTube, we found that the quality of flatfoot and pes planus information was poor to moderate.

- Abstract
We assessed the ability of predicting mortality and total in-hospital bleeding and adverse outcomes by the Academic Research Consortium High Bleeding Risk (ARC-HBR) criteria in ST-segment elevation myocardial infarction (STEMI) patients undergoing primary percutaneous coronary intervention (pPCI). A total of 1441 STEMI patients were recruited: HBR group 354 (25%) patients and non-HBR group of 1087 (75%) patients. A total of 131 patients (9%) had a bleeding complication during hospitalization. The bleeding complications were also categorized according to other conventional bleeding scores. According to these conventional scores, all bleeding categories were associated with HBR. In univariate logistic regression analysis, female gender, diabetes mellitus, hypertension (HT) and HBR were associated with in-hospital bleeding. However, in multivariable analysis only HT (Odds Ratio [OR] 1.528, 95% CI 1.020-2.290; P = .040) and HBR (OR 1.612, 95% CI 1.075-2.428; P = .022) independently predicted total in-hospital bleeding complications. Hospital duration was longer and mortality rate was significantly higher in patients with HBR (OR 8.755, 95% CI 5.864-13.074; P < .01). 43. Which Diastolic Pressure Should Be Used to Assess Diastolic Function?

- Abstract
Background: Although high left ventricular filling pressures [left ventricular (LV) end-diastolic pressure or pulmonary capillary wedge pressure (PCWP)] are widely taken as surrogates for LV diastolic dysfunction, the actual distending pressure that governs LV diastolic stretch is transmural pressure difference (∆PTM). Clinically, preferring ∆PTM over PCWP may improve diagnostic and therapeutic decision-making. We aimed to compare the clinical implications of diastolic function characterization based on PCWP or ∆PTM., Methods: We retrospectively screened our hospital database for adult patients with a clinical diagnosis of heart failure who underwent right heart catheterization. Echocardiographic diastolic dysfunction was graded according to the current guidelines. LV end-diastolic properties were assessed with construction of complete end-diastolic pressure-volume relationship (EDPVR) curves using the single-beat method. Survival status was checked via the electronic national health-care system., Results: A total of 693 cases were identified in our database; the final study population comprised 621 cases. ∆PTM-based, but not PCWP-based, EDPVR diastolic stiffness constants were significantly predictive of advanced diastolic dysfunction. PCWP-based diastolic stiffness constants were not able to predict 5-year mortality, whereas ∆PTM-based EDPVR stiffness constants and volumes all turned out to have significant predictive power for 5-year mortality., Conclusion: Left ventricular diastolic function assessment can be improved using ∆PTM instead of PCWP. As ∆PTM ultimately linked to right-sided functions, this approach emphasizes the limitations of taking LV diastolic function as an isolated phenomenon and underlines the need for a complete hemodynamic assessment involving the right heart in therapeutic and prognostic decision-making processes.

- Abstract
The host-produced nodule specific cysteine-rich (NCR) peptides control the terminal differentiation of endosymbiotic rhizobia in the nodules of IRLC legumes. Although the Medicago truncatula genome encodes about 700 NCR peptides, only few of them have been proven to be crucial for nitrogen-fixing symbiosis. In this study, we applied the CRISPR/Cas9 gene editing technology to generate knockout mutants of NCR genes for which no genetic or functional data were previously available. We have developed a workflow to analyse the mutation and the symbiotic phenotype of individual nodules formed on Agrobacterium rhizogenes-mediated transgenic hairy roots. The selected NCR genes were successfully edited by the CRISPR/Cas9 system and nodules formed on knockout hairy roots showed wild type phenotype indicating that peptides NCR068, NCR089, NCR128 and NCR161 are not essential for symbiosis between M. truncatula Jemalong and Sinorhizobium medicae WSM419. We regenerated stable mutants edited for the NCR068 from hairy roots obtained by A. rhizogenes-mediated transformation. The analysis of the symbiotic phenotype of stable ncr068 mutants showed that peptide NCR068 is not required for symbiosis with S. meliloti strains 2011 and FSM-MA either. - Abstract
In the nodules of IRLC legumes, including Medicago truncatula, nitrogen-fixing rhizobia undergo terminal differentiation resulting in elongated and endoreduplicated bacteroids specialized for nitrogen fixation. This irreversible transition of rhizobia is mediated by host produced nodule-specific cysteine-rich (NCR) peptides, of which c. 700 are encoded in the M. truncatula genome but only few of them have been proved to be essential for nitrogen fixation. We carried out the characterization of the nodulation phenotype of three ineffective nitrogen-fixing M. truncatula mutants using confocal and electron microscopy, monitored the expression of defence and senescence-related marker genes, and analysed the bacteroid differentiation with flow cytometry. Genetic mapping combined with microarray- or transcriptome-based cloning was used to identify the impaired genes. Mtsym19 and Mtsym20 mutants are defective in the same peptide NCR-new35 and the lack of NCR343 is responsible for the ineffective symbiosis of NF-FN9363. We found that the expression of NCR-new35 is significantly lower and limited to the transition zone of the nodule compared with other crucial NCRs. The fluorescent protein-tagged version of NCR343 and NCR-new35 localized to the symbiotic compartment. 46. Development of a Radiolabeled Folate-Mediated Drug Delivery System for Effective Delivery of Docetaxel.

- Abstract
Many preclinical studies are carried out with the aim of developing new formulations for the effective delivery of taxane class drugs, one of the most important anticancer drugs used clinically today. In this study, a radiolabeled folate-mediated solid lipid magnetic nanoparticle (SLMNP) system was developed by loading superparamagnetic iron oxide nanoparticles (MNP) and docetaxel (DTX) into the solid lipid nanoparticles as a drug delivery system that will function both in cancer treatment and diagnosis. For this purpose, first, SLMNP was synthesized by the hot homogenization method, and the surface of the particles was modified with a folate derivative to carry the particles to tissues with folate receptors. The synthesized magnetic solid lipid nanoparticles were loaded with DTX, and then radiolabeling was carried out with technetium-99 m (
99m Tc-DTX-SLMNP). Structural characteristics of these nanoparticles were determined by characterization methods. According to the TEM images of MNPs, SLN, and SLMNPs, MNPs were observed between 25and 35 nm, SLNs between 400 and 500 nm, and SLMNPs between 350 and 450 nm. The drug entrapment efficiency of SLMNPs loaded with DTX was found to be 19%, and the percentage efficiency of radiolabeling was found to be 98.0 ± 2.0%. The biological behavior of this radiolabeled system was investigated in vitro and in vivo . Folate receptor-positive SKOV-3 and folate receptor-negative A549 cancer cell lines were studied. The IC50 values of DTX-SLMNP in SKOV-3 and A549 cells were 50.21 and 172.27 μM at 48 h, respectively. Gamma camera imaging studies of99m Tc-DTX-SLMNP and magnetically applied99m Tc-DTX-SLMNP compounds were performed on tumor-bearing CD-1 nude mice. The uptake in the folate receptor-positive tumor region was higher than that in the folate receptor negative tumor region. Background: In the descending arm of tricuspid annular plane systolic excursion (TAPSE), there is notch formation that corresponds to the contractile phase of the atrial strain curve. Theoretically, this notch formation stands for atrial contraction., Aims: We aim to characterize notch formation on TAPSE, predictors of its existence, and its relationship with the right ventricle and right atrial strain (RAS) parameters., Methods: Retrospectively selected 240 patients were investigated for the determinants of notch formation on TAPSE and the relationship between RAS and TAPSE. RAS was analyzed using 2D speckle tracking in a dedicated mode for atrial analysis and reported separately for the reservoir, conduit, and contractile phases., Results: 71.7% ( n = 172) of patients had notch formation on TAPSE and 70.4% (n = 169) had a normal value of right atrial contractile strain (RASct). Most patients with notch formation also had preserved RASct (95.9%; P <0.001). In multivariable analysis, RASct (odds ratio [OR], 1.45; 95% confidence interval [CI]: 1.13-1.77; P = 0.020) remained significant with notch formation. Receiver operator characteristic (ROC) analysis demonstrated that a RASct of -19% was a cut-off for the presence of notch formation. ROC area was 0.897 (95% CI 0.844-0.951; P <0.001)., Conclusions: The changes in TAPSE configuration reflect the changes in the atrial contractile phase. The descending arm of TAPSE indicates RASct as to whether it is preserved or not. Notch formation persists if RASct is above -19%. So, an easier, more applicable, and more effortless tool, TAPSE, can be used as an indicator of the atrial contractile phase by its configuration in daily routine.

- Abstract
Mapping clinical attributes from hospital information systems to standardized terminologies may allow their scientific reuse for multicenter studies. The Unified Medical Language System (UMLS) defines synonyms in different terminologies, which could be valuable for achieving semantic interoperability between different sites. Here we aim to explore the potential relevance of UMLS concepts and associated semantic relations for widely used clinical terminologies in a German university hospital. To semi-automatically examine a sample of the 200 most frequent codes from Erlangen University Hospital for three relevant terminologies, we implemented a script that queries their UMLS representation and associated mappings via a programming interface. We found that 94% of frequent diagnostic codes were available in UMLS, and that most of these codes could be mapped to other terminologies such as SNOMED CT. We observed that all examined laboratory codes were represented in UMLS, and that various translations to other languages were available for these concepts. The classification that is most widely used in German hospital for documenting clinical procedures was not originally represented in UMLS, but external mappings to SNOMED CT allowed identifying UMLS entries for 90.5% of frequent codes. Future research could extend this investigation to other code sets and terminologies, or study the potential utility of available mappings for specific applications.

50. Peritoneal dialysis for refractory heart failure: A single-center experience.

Introduction: Heart failure (HF) is a disease associated with poor quality of life. Peritoneal dialysis (PD) can be an alternative in the treatment of these patients to overcome fluid overload. The objective of this study is to observe the effects of PD in refractory HF patients., Methods: We conducted an observational study including 10 patients with refractory congestive HF. PD started solely for fluid overload. Patients' baseline parameters were compared with follow-up parameters., Results: Median age was 57.5 (44.8-64.3) years. Median left ventricular ejection fraction was 20% (18.8-31.3), and all patients had right ventricular dysfunction. Median estimated glomerular filtration rate was 51.2 (43.8-101.3) ml/min/1.73 m
2 . Two patients (20%) died during the follow-up period. New York Heart Association (NYHA) functional class decreased significantly from a median of 4 to 2,1 and 1 in the 3rd, 6th, and 12th months, respectively (p ≤ 0.01 for all from baseline). Number and length of hospitalization decreased significantly after treatment (number from a median of 3 to 0, p = 0.013; days from 50.5 to 0, p = 0.028)., Conclusion: PD significantly reduced NYHA functional class, number and days of hospitalization for HF.
