Abstract

Umbilical venous catheters are frequently used in the neonatal period. The incidence of umbilical venous catheter-related thrombosis is between 1.3% and 43% in ultrasound scans. This study aimed to determine the incidence and risk of portal vein thrombosis in patients who were hospitalized in the neonatal intensive care unit and underwent umbilical venous catheter insertion.Premature infants (≤32 gestational weeks) who were hospitalized in a Level III neonatal intensive care unit and underwent umbilical vein catheter placement between 2016 and 2018, were included in the study. The demographic data, clinical risk factors for thrombosis, number of catheter days, catheter locations, times of detection of thrombosis using Doppler ultrasonography, treatment methods and durations, thrombosis follow-up and examinations were obtained retrospectively from the electronic patient files.Ninety-six patients whose complete data could be reached were enrolled in the study. The mean gestational age of the patients was found as 29±2 weeks and the mean birth weight was 1353±369 g. Portal vein thrombosis was detected in 13.5% (n=13) of the patients. Five of the cases of portal vein thrombose were complete occlusion and eight were partial occlusion. All patients with complete occlusion and six patients with partial occlusion were treated with low-molecular-weight heparin for a mean duration of 31±13.8 days. Thrombosis disappeared in 7-120 days in all patients. A thrombophilia mutation was detected in six patients with thrombosis, four of whom had the PAI-1 4G / 5G mutation.Portal vein thrombosis which has a significant place among the causes of portal hypertension in childhood, is mostly asymptomatic in the neonatal period and cannot be recognized clinically. It is important to screen and follow up patients with umbilical vein catheters using Doppler ultrasonography in terms of PVT after catheter removal to prevent long-term complications.Yenidoğan döneminde sıklıkla kullanılmakta olan göbek ven kateterlerine bağlı olarak, ultrasonografi taramalarında %1,3-43 arasında değişen oranlarda tromboz sıklığı bildirilmektedir. Bu çalışmada, yenidoğan yoğun bakım biriminde yatıp göbek ven kateteri takılan hastalarda, portal ven trombozu gelişme oranları ve risklerinin belirlenmesi amaçlanmıştır.Çalışmaya 2016–2018 yılları arasında üçüncü basamak yoğun bakım biriminde yatan ve göbek ven kateteri yerleştirilen 32 gebelik hafta ve altındaki erken doğan bebekler alındı. Hastaların demografik verileri, tromboz açısından klinik risk etmenleri, kateter kalış gün sayıları, kateter yerleşim yerleri, Doppler ultrasonografi ile tromboz saptanma zamanları, tedavi yöntem ve süreleri, tromboz izlem ve tetkikleri geriye dönük olarak hastane bilgi sistemindeki elektronik hasta dosyalarından elde edildi.Çalışmaya tüm verilerine ulaşılabilen 96 hasta alındı. Hastaların ortalama gebelik yaşları 29±2 hafta ve ortalama doğum ağırlıkları 1 353±369 gramdı. Hastaların %13,5’inde (n=13) portal ven trombozu saptandı. Portal ven trombozlarının beşi tam, sekizi kısmi tıkanma şeklindeydi. Tam tıkanma saptanan hastaların hepsine ve kısmi tromboz saptanan altı hastaya ortalama 31±13,8 gün düşük molekül ağırlıklı heparin tedavisi uygulandı, hastaların tümünde tromboz 7–120 gün arasında kayboldu. Tromboz saptanan altı hastada trombofili mutasyonu saptandı, dördü PAI-1 4G/5G mutasyonu şeklindeydi.Çocukluk döneminde portal hipertansiyon nedenlerinden olan portal ven trombozları yenidoğan döneminde çoğunlukla semptomsuz olup klinik olarak tanınamamaktadırlar. Göbek ven kateteri yerleştirilen hastaların kateter çekildikten sonra portal ven trombozu açısından Doppler ultrasonografiyle taranması ve izlemi, uzun dönem komplikasyonların önlenmesi için önemlidir.

Abstract

WOS:000426572200004, PubMed ID: 28179213, Amaç: Türkiye'deki juvenil miyelomonositik lösemi (JMML) hastalarının durumunu, tanı zamanı, klinik özellikler, mutasyon çalışmaları, klinik gidiş ve tedavi stratejileri açısından ortaya koymaktır.Gereç ve Yöntemler: Ülkemizdeki pediatrik hematoloji ve onkoloji kliniklerinden veri istenerek, JMML tanısı ile takip ve tedavisi yapılan hastaların klinik ve laboratuvar bulguları geriye dönük olarak değerlendirildi.Bulgular: On sekiz merkezden, 2002-2016 tarihleri arasında JMML tanısı alan toplam 65 hasta çalışmaya dahil edildi. Ortanca tanı yaşı 17 ay idi (2-117 ay). Splenomegali tanıda %92 hastada vardı. Ortanca lökosit, monosit ve trombosit sayıları sırasıyla 32,9x109/L, 5,4x109/L ve 58,3x109/L idi. Monozomi 7, %18 hastada saptanmıştı. JMML mutasyonları 32 hastada (%49) çalışılmış olup, en sık rastlanan mutasyon PTPN11 idi. Hematopoetik kök hücre nakli (HKHN) 7 was present in 18% of patients. JMML mutational analysis was performed in 32 of 65 patients (49%) and PTPN11 was the most common mutation. Hematopoietic stem cell transplantation (HSCT) could only be performed in 28 patients (44%), the majority being after the year 2012. The most frequent reason for not performing HSCT was the inability to find a suitable donor. The median time from diagnosis to HSCT was 9 months (min-max: 2-63 months). The 5-year cumulative survival rate was 33% and median estimated survival time was 3017.4 months (95% CI: 0-64.1) for all patients. Survival time was significantly better in the HSCT group (log-rank p0.019). Older age at diagnosis (2 years), platelet count of less than 40x109/L, and PTPN11 mutation were the factors significantly associated with shorter survival time. Conclusion: Although there has recently been improvement in terms of definitive diagnosis and HSCT in JMML patients, the overall results.

Vitamin B12 eksikliği gelişmekte olan ülkelerde sıklıkla diyette alım kusuruna bağlı gelişir. Bu çalışmamızda çeşitli yakınmalarla çocuk genel polikliniğine başvuran çocuklar arasında vitamin B12 eksikliği saptanan olgular incelendi. Uludağ Üniversitesi Tıp Fakültesi genel çocuk polikliniğine 2005-2006 yılları arasında değişik nedenler ile başvuran 3980 olgunun 15'inde anamnez, klinik ve laboratuar bulgular eşliğinde vitamin B12 eksikliği düşünüldü. Olguların %80'ni düşük sosyoekonomik düzeyli ailelerdendi ve vitamin B12'den fakir beslendiği saptandı. Vitamin B12 eksikliği sosyoekonomik düzeyi düşük ülkelerde önemli bir halk sağlığı sorunudur. Erken tanı ve tedavi ile önemli ve kalıcı nörolojik komplikasyonlar engellenebilir., Vitamin B12 deficiency is usually due to nutritional problems in developing countries. In this study, clinical and laboratory findings of vitamin B12 deficiency are described in children. Over a one year period, from 2005 to 2006, vitamin B12 deficiency was diagnosed in 15 children due to the history, clinical and laboratory findings among 3980 cases who were admitted to Uludağ University faculty of Medicine Pediatric policlinic with different compliants. 80% of patients were in poor socioeconomic conditions and their animal food intake was inadequate. Vitamin B12 deficiency is an important public health problem among children in developing countries. Early recognition and treatment of vitamin B12 deficiency is important for prevention of irreversible neurologic complications.

Neonatal Alloimmün Trombositopeni NAT yenidoğan döneminde ağır trombositopeniyeneden olan nadir bir hastalıktır. Anne ve bebek arasındaki trombosit antijenuyumsuzluğu sonucunda annede fetal trombositlere karşı gelişen antikorlar, fetusageçerek intrauterin dönemden itibaren trombositopeniye yol açabilir. Klinik bulgularhafif ya da ağır olabilir. Olguların yaklaşık yarısı ilk gebelikte görülür. İzleyen gebeliklerdegörülme sıklığı artar. Olay antikorların uzaklaşması ile kendini sınırlar. Ancak, gebeliksüresince anne ve fetusun dikkatli ve yakından izlenmesi ve endikasyonu olduğutakdirde tedavinin hemen düzenlenmesi gerekir. Artmış mortalite ve morbiditesinedeniyle erken tanınması, tedavi edilmesi ve ailenin sonraki gebeliklerdeki risklerhakkında bilgilendirilmesi önemlidir, Neonatal alloimmune thrombocytopenia is a rare disease causing seriousthrombocytopenia during neotatal period. Antibodies developed against fetalthrombocytes due to fetal-maternal platelet antigen incompatibility causethrombocytopenia both in intrauterine period and afterwards. The clinical symptoms varyfrom mild to severe. Half of the cases occurs during first pregnancy. The recurrence riskincreases in the subsequent pregnancies. The event is limited when the antibodies arecleared away. However, a careful close follow-up is needed both for the mothers andfetus/babies and the treatment should be immediately started when it is indicated. Earlydiagnosis, treatment and giving information to the parents about the risks of thesubsuquent pregnancies are important issues due to increased morbidity and mortalityof the disease
