Bu araştırma zorunlu eğitim çağını geçmiş olan engelli bireylerin günümüzdeki durumlarının incelenmesi amacıyla yapılmıştır. Araştırma nitel araştırma yöntemlerinden durum çalışması desenine göre planlanmıştır. Araştırma gönüllük esasına göre engelli yetişkin çocuğu olan yedi aile üzerinde yürütülmüştür. Araştırmanın verileri görüşme tekniği ile toplanmış ve araştırmada yarı yapılandırılmış görüşme formu kullanılmıştır. Elde edilen veriler içerik ve betimsel analiz ile değerlendirilmiştir. Araştırma sonucunda; engelli bireylerin, engel derecelerinin orta ve ağır seviyede olduğu, geçmişte engelli bireylerin tümünün üniversiteye bağlı eğitim-rehabilitasyon merkezinden hizmet aldıkları, ailelerin çocuklarının eğitim süreçlerinde erişilebilirlik ve yemek hizmetlerinde sorun yaşadıkları, alınan eğitimin çocukların akademik, günlük yaşam ve sosyal becerilerine katkı sağladığı bulunmuştur. Ayrıca araştırmada tüm ailelerin çocuklarının geleceklerinden kaygı duydukları, bundan dolayı engelli bireylere yönelik bakım evlerinin kurulmasını istedikleri, engelli çocuklarının eğitimlerinin yaşam boyu yoğun bir şekilde devam etmesi gerekliliğine dikkat çektikleri ve çocuklarının rahatça katılabilecekleri sosyal etkinlikler düzenlenmesinin de önemli olduğunu ifade ettikleri belirlenmiştir., This research was conducted in order to examine the current situation of persons with disabilities who have passed the age of compulsory education. The research was planned according to the case study pattern, which is one of the qualitative research methods. The research was conducted on seven families with adult children with disabilities on a voluntary basis. The data of the study were collected by interview technique and semi-structured interview form was used in the research. The obtained data were evaluated by content and descriptive analysis. As a result of the research this study reviews that persons with disabilities have moderate and severe disability degrees and received services from a university-affiliated educational rehabilitation center in the past; families have problems about accessibility and food during their children's education life; and the education of their children contributes to the academic, daily life and social skills of their children. In addition, all families are concerned about the future of their children, so they demand to establish nursing homes for persons with disabilities and they point out the need for an intensive lifelong education for their children with disabilities, they also state that it is important to organize social activities that their children can participate in comfortably.

Engelli çocuğa sahip ailelerin eğitim hizmeti ve hakkına ilişkin algısı ve bilgi düzeylerinin incelenmesini amaçlayan bu araştırma nitel araştırma yöntemlerinden fenomenolojik desene göre tasarlanmıştır. Araştırmanın çalışma grubunu, amaçlı örnekleme yöntemi ile seçilen, Edirne ilinde bulunan 0-36 aylık engelli çocuğa sahip 10 aile, 36-72 aylık engelli çocuğa sahip 15 aile ve 72 aydan büyük engelli çocuğa sahip 15 aile olmak üzere toplam 40 aile oluşturmaktadır. Araştırmanın verileri, derinlemesine görüşme yöntemi ile yarı yapılandırılmış görüşme formu kullanılarak toplanmıştır. Araştırmadan elde edilen veriler betimsel analiz ve içerik analizi yapılarak yorumlanmıştır. Çalışma sonucunda ailelerin eğitim hakkı ve eğitim hizmeti ile ilgili düşük düzeyde bilgi sahibi oldukları belirlenmiştir. This research, which aims to examine the perceptions and knowledge levels of families with disabled children about education services and their rights, was designed according to a phenomenological pattern, one of the qualitative research methods. The study group of the research consists of 40 families in total, 10 families with 0-36 months old disabled children, 15 families with 36-72 months old disabled children and 15 families with disabled children older than 72 months in Edirne, selected by purposive sampling method. The data of the research were collected by using in-depth interview method and semi-structured interview form. The data obtained from the research were interpreted by making descriptive analysis and content analysis. As a result of the study, it was determined that the families had low level knowledge about the right to education and education service.

The aims of the present study were to evaluate the efficacy and tolerability of ledipasvir/sofosbuvir (LDV/SOF) with or without ribavirin in the treatment of chronic hepatitis C (CHC) in patients with advanced liver disease and to analyse whether the use of LDV/SOF treatment is associated with a new occurrence of hepatocellular carcinoma (HCC) during and after LDV/SOF treatment. The Turkish Early Access Program provided LDV/SOF treatment to a total of 200 eligible CHC patients with advanced liver disease. The median follow-up period was 22 months. All patients were Caucasian, 84% were infected with genotype 1b, and 24% had a liver transplantation before treatment. The sustained virological response (SVR12) was 86.0% with ITT analysis. SVR12 was similar among patients with Child-Pugh classes A, B and C disease and transplant recipients. From baseline to SVR12, serum ALT level and MELD score were significantly improved (P < 0.001). LDV/SOF treatment was generally well tolerated. Only one patient developed a new diagnosed HCC. Seventeen of the 35 patients, who had a history of previous HCC, developed HCC recurrence during the LDV/SOF treatment or by a median follow-up of 6 months after treatment. HCC recurrence was less commonly observed in patients who received curative treatment for HCC compared with those patients who received noncurative treatment (P = 0.007). In conclusion, LDV/SOF with or without ribavirin is an effective and tolerable treatment in CHC patients with advanced liver disease. Eradication is associated with improvements in liver function and a reduced risk of developing a new occurrence of HCC.

Introduction: The effect of coronavirus disease 2019 (COVID-19) on outcomes of liver transplant recipients is not known very well. Aim: Methods The aim of the present study was to describe the early outcomes of liver transplant recipients with COVID-19.

Background/Aims: This study aimed to evaluate the real-life efficacy and tolerability of direct-acting antiviral treatments for patients with chronic hepatitis C (CHC) with/without cirrhosis in the Turkish population. Material and Methods: A total of 4,352 patients with CHC from 36 different institutions in Turkey were enrolled. They received ledipasvir (LDV) and sofosbuvir (SOF)+/- ribavirin (RBV) ombitasvir/paritaprevir/ritonavir +/- dasabuvir (PrOD)+/- RBV for 12 or 24 weeks. Sustained virologic response (SVR) rates, factors affecting SVR, safety profile, and hepatocellular cancer (HCC) occurrence were analyzed. Results: SVR12 was achieved in 92.8% of the patients (4,040/4,352) according to intention-to-treat and in 98.3% of the patients (4,040/4,108) according to per-protocol analysis. The SVR12 rates were similar between the treatment regimens (97.2%-100%) and genotypes (95.6%-100%). Background and Aims: In recent years, the neutrophil to lymphocyte ratio has been popular as a simple, inexpensive and effective marker associated with various inflammatory and neoplastic diseases. This marker has not yet been investigated in chronic hepatitis B patients. In our study with hepatitis B patients, we aimed to investigate the relationship between fibrosis levels and the neutrophil to lymphocyte ratio for determining the disease severity and making treatment decisions using HBV-DNA and liver biopsy results. Materials and Methods:486 hepatitis B patients subjected to liver biopsy between 2005 and 2010 were screened retrospectively from patient charts. Only 268 naive patients fit the criteria for this study. Naive patients were included while patients treated with antiviral drugs and with a positive anti-Delta result were excluded from the study. Results:The study consisted of 268 (age: 38±13 years) patients with liver biopsies [163 (60.8%) males (age: 38±13), 105 (39.2%) females (age: 37±14)]. Fibrosis score distribution of the patients was as follows: 0/6: 122 patients (45.5%), 1/6: 50 patients (18.7%), 2/6: 37 patients (13.8%), 3/6: 25 patients (9.3%), 4/6: 5 patients (1.9%), 5/6: 6 patients (2.2%), and 6/6: 23 patients (8.6%). There was a significant correlation between fibrosis score and histological activity index (r: +0.478 p: 0.000), while HBV-DNA levels showed no significant correlation (r: +0.035, p: 0.556). Conclusions: The neutrophil to lymphocyte ratio is predicted to be useful in deciding the disease severity and histopathological grade of liver disease in chronic hepatitis B patients who are first seen in the outpatient clinic., Giriş ve Amaç:Son yıllarda ilgi odağı haline gelen, değişik inflamatuvar ve neoplastik hastalıklar ile ilişkilendirilen basit, ucuz ve efektif bir marker olan nötrofil-lenfosit oranı öne çıkmakta olup kronik hepatit B hastalarında çalışılmamıştır. Biz çalışmamızda hepatit B tanısı ile hastalık düzeyini belirleme, tedavi kararı alma gibi nedenlerle HBV DNA bakılan ve karaciğer biyopsisi uygulanan hastaların fibrozis düzeyi ile nötrofil-lenfosit oranı arasındaki ilişkiyi araştırdık.Materyal ve Metod: 2005-2010 yılları arasında biyopsi alınmış 486 hepatit B hastasının dosyası retrospektif tarandı ve bu hastalardan çalışma kriterlerine uyan 268 naiv hasta bulundu Naiv hastalar çalışmaya alınırken, daha önceden antiviral tedavi almış olan hastalar ve anti-Delta pozitif olanlar çalışma dışı bırakıldılar.Bulgular:Biyopsi alınmış olan çalışma popülasyonu 163 (%60,8) erkek (yaş: 38±13), 105 (%39,2) kadın (yaş: 37±14) olmak üzere 268 (yaş: 38±13) kişiden oluşmaktaydı. Hastaların fibrozis skoruna göre dağılımlarına bakıldığında 0/6=122 hasta (%45,5), 1/6=50 hasta (%18,7), 2/6=37 hasta (%13,8), 3/6=25 hasta (%9,3), 4/6=5 hasta (%1,9), 5/6=6 hasta (%2,2), 6/6=23 hasta (%8,6) şeklinde dağılmaktaydı. Fibrozis skoru ile nötrofil-lenfosit oranı (r: +0,510 p: 0,000), histolojik aktivite indeksi (r: +0,478 p: 0,000) arasında anlamlı korelasyon saptanırken, HBV DNA düzeyi arasında anlamlı bir korelasyon izlenmedi (r: +0,035 p: 0,556). Fibrozis skoru 0-1 olan hastaların nötrofil-lenfosit oranı değeri 1,6±0,5 iken fibrozis skoru 2-6 olanlarda 2,4±0,8 saptandı (p=0,000). Tartışma:Kronik hepatit B hastasını poliklinikte ilk kez gördüğümüzde hastalığın evresi ve histopatolojik olarak karaciğerin durumu hakkında klinisyen olarak bir öngörüde bulunmak için nötrofil-lenfosit oranının yararlı olacağını düşünmekteyiz.

Çiriş: Programlanmış ölüm -l (PD-I), B7/CD28 süper ailesinden aktive olmuş T ve B hücrelerinde bulunan T bir transmembran proteindir. PD-1 'in ekspresyonunun inflamasyonda anlığı ve T hücre fonksiyonlarım ve sitokin üretimini azaltarak otoreaktivasyonun baskılanmasında önemli olduğu bilinmektedir. T hücre aracılı, oloimmün bir hastalık olan nomaloid artrit (RA) patogenezinde lenfosit aktivasyonunun bozulduğu ve self-toletansın azaldığı düşünülmektedir. CD4+ PD-1+ T lenfosit popülasyonu RA'lı hastaların sinovial sıvılarında gösterilmiştir. Programlanmış Hücre Ölümü l gen (PDCDl) polimorfemleri daha önce sistemik lupus eritematozusa yatkınlıkla ilişkili bulunmuştur. Bu çalışmada PDCDl polimorfizmlertain RA'ya yatkınlıkla olan ilişkisinin bir vaka-kontrol çalışmasıyla Türk toplumunda araştırılması amaçlanmıştır. Materyal ve Mctod: Bu çalışmada 130 RA'lı (E/K: 18/112; yaş ortalaması: 51,9±11,9 yıl) ve 148 sağlıklı kontrolde (E/K: 64/84, yaş ortalaması: 40,9±9,9 yıl) PDCDl genindeki tek nükleotid politnorfizmlerinden (PD-1.5, PD-1.3 ve PD-1.9) polimeraz /incir reaksiyonu- rcslriksiyon parça uzunluk polimorlranleri (P/R-RI'LP) yöntemiyle araştırıldı. Bu yöntemde polinıorfik bölgeler PZR ile çoğaltıldı ve polimorfizmlere uyan restriksiyon enzimleri ile kesildi. PD-1,5C/T içm/>TOII, PD-1.3A/G için Art ve PD-1.9C/T için Drdl enzimi kullanıldı. RA ve kontrol grubu arasında genotip ve alel dağılımları karşılaştırıldı. Ayrıca klinik, Saboratuvar ve radyolojik parametreler ile polimorfizmler arasındaki ilişki değerlendirildi. Bulgular: PD-1.5 polimorfızminde CC genotipinin RA grubunda kontrol grubuna göre anlamlı olarak artmış olduğu (%43 ve %30, p=0,028, OR-1,73; %95 güven aralığı: 1,05- 2,83), CT genotipinin ise anlamlı olarak daha düşük olduğu görüldü (%35 ve %56, p=0.001, 014=0,42; %95 güven aralığı 0,26-0,69). T alel pozitifliği RA grubunda anlamlı olarak daha düşük idi (%56 ve %69, p=0,03, OR=0,57; %95 güven aralığı 0,35-0,95). PD-I.3 ve PD-1.9 polimorfizmlerinde genolip dağılımları RA ve kontrol gruplarında benzerdi (sırasıyla p=0,175 ve p=0,565). Ayrıca hastalık şiddetini gösteren parametreler ile her üç polimorfizm arasında anlamlı ilişki saptanmadı. Sonuç: Sonuç olarak PD-1.5 polimorfizmi Türk toplumunda RA ile anlamlı derecede ilişkilidir ve etiyopatogenex.de rolü olabilir. PDCDl geni île ilgili polimorfizmler kontrol edilemeyen otoreaktiviteye neden olarak RA'ya yatkınlık oluşturabilir. Introduction: Programmed cell death 1 (PD-1) molecule, a member of B7/CD28 super family, is a transmembrane protein and expressed in activated T and B cells. PD-1 is a necessary negative regulator of self-reactivity and its expression is increased in inflammation. It is thought that rheumatoid arthritis (RA), a T-cell mediated autoimmune disease, may result in abnormal lymphocyte activation and decreased self-tolerance. CD4+ PD-1+ T lymphocytes were also present in the synovial fluids of patients with RA. Programmed Cell Death 1 gene (PDCD1) polymorphisms were previously identified as being a risk factor for a susceptibility to systemic lupus erythematosus. In this case-control study, we investigated the relationship of PDCD1 polymorphisms with susceptibility to RA in Turkish population. Materials and Methods: In this study we genotyped 130 Turkish patients (M/F: 18/112 and mean age: 51.9±1 1.9 years) with RA and 148 controls (M/F: 64/84 and mean age: 40.9±9.9 years) for PD-1. 5, PD-1. 3 and 1.9 genes. Single nucleotide polymorphisms in PD-1 gene (PDCD1) were investigated with PCR-RFLP (Polymerase chain reaction-Restriction fragment length polymorphism) method. In this method polymorphic regions were amplified with PCR and cut with related restriction enzymes. PvuW enzyme was used for PD-1.5C/T, Pst/ for PD-1.3A/G and Drdl for PD-1.9CAT. The genotype and allele distributions were compared in RA and control groups. The relations between clinical, laboratory and radiological parameters and polymorphisms were evaluated. Results: The genotype CC of PD-1. 5 showed a higher presence in RA compared to the control group (43% vs 30%, p= 0.028, odds ratio [OR]: 1.73, 95% confidence interval [95% CI]: 1.05-2.83), whereas CT genotype was found less frequently in RA (35% vs 56%, p-0.001, OR: 0.42, 95% CI: 0.26-0.69). The frequency of T allele was lower in patients with RA compared to the controls (56% vs 69%, p=0.028, OR: 0.57, 95% CI: 0.35-0.94). No significant difference was observed in the genotype distribution of PD-1. 3 (p=0.175) and PD- 1.9 (p=0.565) polymorphisms. Investigated 3 polymorphisms showed no association with parameters of RA disease severity. Conclusion: As a conclusion, PD-1. 5 polymorphism is significantly associated with RA in Turkey, implicating a possible role in the etiopathogenesis. PDCD1 gene associated polymorphisms may predispose to uncontrolled auto-reactivity and arthritis in RA patients. 70

Yapılan tespit ve değerlendirmeler, yönetimin bilgilendirilmesi ve hata ve noksanlıkların düzeltilmesi ve en aza indirilmesi için gerekli tedbirlerin alınmasının sağlanması amacıyla yönetime sunulur.ABSTRACTInternal supervision, of which importance is increased gradually according to the management mentality of today, is vital for the banking system when the benefits of system to production, economic development and financial conditions of Turkey are considered.The board of directors and senior management are responsible for establishing and maintaining an efficient internal supervision system but to ensure an efficient internal control, operations should be carried out by the bank staff at all levels. The internal control and the audit units take place in the system which are accountable to the member of the board of directors who is responsible for internal supervision and risk management. The aims of the system can be summarized as; ensuring that the activities are in compliance with laws and regulations, ensuring the reliability of financial and management information and ensuring the efficiency and effectiveness of activities.The internal supervision units have significant functions as the controls of; compliance of the activities with laws and the strategies and policies established by the board of directors, ensuring whether the operations are carried out prudently or not, safeguarding the bank assets, ensuring whether the necessary measures are taken or not to prevent potential losses that can be resulted from possible risks, ensuring whether the operations are carried out in compliance with authorizations and limits or not, the accuracy and reliability of accounting and recording system, financial tables and reports, contingency planning to avoid disruptions to activities, information systems and technologies. The reports are submitted to the senior management to inform and provide the necessary measures to be taken to minimize and correct the mistakes and defects.

Aims: To evaluate patient profile for epidemiological and clinicopathological characteristics and potential risk/prognostic factors in newly diagnosed hepatocellular carcinoma (HCC) patients across Turkey. Methods: A total of 547 patients (mean (SD) age 62.6 (10.3) years, 81.9% were males) were included in this registry study. Data on patient characteristics, etiologies of HCC, laboratory values, and tumor characteristics and stages were recorded at study enrollment. Results: HBV infection (68.2%) was the leading etiology, followed by HCV infection (17.2%), HDV infection (5.5%), alcohol (6.4%), and NAFLD (3.5%), as the major etiologies. Considering that 51.6% of the patients had >5 cm HCC, 44% were Child-Pugh B/C and 57% were BCLC B-D, it appears that a significant group of HCC patients were diagnosed at advanced stages. Of 540 patients, 271 (50.2%) were referred or applied with the diagnosis of HCC. Giriş Hepatoselüler karsinom (HCC) karaciğerin en sık görülen primer tümörüdür ve kanser nedenli ölümlerde dördüncü sıradadır. Transarteriyel kemoebolizayon (TAKE) tedavisi günümüzde ileri evre HCC tedavisinde önerilen lokal ablatif tedavilerdendir. Çalışmamızın amacı ileri evre HCC nedeni ile DEB (Drug eluting beads)-TAKE uygulanmış hastalarda tedavinin etkinliğini, güvenirliğini ve prognoza etkili faktörleri retrospektif olarak değerlendirmektir.Gereç ve YöntemKliniğimizde takipli olan ve Girişimsel Radyoloji tarafından Ocak 2011-Mart 2018 tarihleri arasında HCC nedeniyle TAKE uygulanan 133 hasta retrospektif olarak değerlendirildi. DEB-TAKE uygulanması sonrası yanıt değerlendirmesi 30 ve 90. günler arasında çekilen batın görüntülemelerinde (BT veya MRG ile) mRecist kriterlerine göre yapıldı. Tedaviye yanıt oranları, progresyonsuz ve genel sağkalım süreleri, yan etki profili ve prognostik faktörler araştırıldı. BulgularTanı anında ortalama yaş 65,4'tü ve hastaların %77,4'ü erkekti. En sık etyolojik faktör Kronik Hepatit B enfeksiyonuydu (%57,1). Hastaların 122'sinde (%91,7) siroz vardı. Barcelona evreleme sistemine göre 16'sı (%13) erken evre, 58'i (%48) intermediate, ve 48'i (%39) ileri (advanced) evreydi. 78 (%64) hasta Child A evresindeydi. Medyan takip süresi 14 aydı (min 1 ay -maks 77 ay). Medyan progresyonsuz ve genel sağ kalım sırasıyla 4 ve 11 ay bulundu. Komplet yanıt (CR) 20 (%17), parsiyel yanıt (PR) 36 (%32), stabil hastalık (SD) 24 (%21) ve progrese hastalık (PD) 35 (%30) hastada vardı. Hastalık kontrol oranı %70 ve objektif yanıt oranı %49 bulundu. En sık görülen yan etki karaciğer enzim yüksekliği ve karın ağrısıydı. Tek ve çok değişkenli analizde tedavi sonrası AFP ≥ 400 ng/ml olması hem progresyonsuz hem de genel sağ kalım üzerinde etkili bağımsız prognostik faktör olarak bulundu. Child-Pugh sınıflaması ve tümör boyutu >7cm olması ise sadece genel sağkalıma etkili bağımsız prognostik faktörlerdi.Sonuç DEB-TAKE unrezektabl HCC tanılı hastalarda etkili ve kolay tolere edilebilen bir tedavi yöntemi olarak karşımıza çıkmaktadır. Child-Pugh sınıflaması ve tümör boyutu genel sağ kalım üzerine etkili bağımsız prognostik faktörler, tedavi sonrası AFP> 400ng/ml olması ise progresyonsuz ve genel sağ kalım üzerine etkili prognostik faktörler olarak bulundu. Prospektif çalışmalar tedavi etkinliğini ve prognostik faktörleri daha iyi değerlendirmek için aydınlatıcı olacaktır. Introduction Hepatocellular carcinoma (HCC) is the most common primary liver malignancy and fourth most common cause of cancer-related deaths in the worldwide. Transarterial chemoembolization (TAKE) therapy is currently recommended in the treatment of unresectable HCC. The purpose of our study is to assess the efficacy, safety and prognostic factors of the treatment retrospectively in patients who have undergone DEB (Drug Eluting Beat) -TAKE. Material and Method Retrospective analysis of data from 133 patients with unresecetabl HCC who treated with DEB- TAKE and followed in Marmara University Pendik Training and Research Hospital Gastroenterology Science Department between January 2011- March 2018 was performed. Control images were performed between thirty and ninety days after DEB-TAKE. We evaluated response according to mRecist criteria. Response rates, progression-free and overall survival, side effect profile and prognostic factors were investigated.Results At the time of diagnosis, the mean age was 65,4 years and 77,4% of the patients were male. Hepatitis B infection was the most common etiologic risk factor (57.1%). 122 patients had cirrhosis. According to the Barcelona staging system, 16 patients (13%) were in the early stage, 58 patients (48%) were in the intermediate stage and 48 patients (39%) were in the advanced stage. 78 (64 %) patients were Child-pugh stage A. Median follow-up duration was 14 months (min 1 month-max 77 months). Median progression-free survival and overall survivals were 4 and 11 months, respectively. There were complete response (CR) in 20 patients (17%), partial response (PR) in 36 patients (32%), stable disease (SD) in 24 patients (21%) and progressed disease (PD) in 35 (30%) patients. The disease control rate was 70% and the objective response rate was 49%. The most common side effects were liver enzyme elevation and abdominal pain. In the univariate and multivariate analysis, posttreatment AFP ≥ 400 ng/ml was found to be an independent prognostic factor on both progression-free survival and overall survival. Child-Pugh classification and tumor size >7 cm were independent prognostic factors on overall survıval.Conclusion DEB-TAKE is effective and a tolerable treatment method for unresectable HCC patients. Child-Pugh classification and tumor size> 7 cm were found to be independent prognostic factors on overall survival. Posttreatment AFP> 400ng / ml had a prognostic factor on progression-free survival and overall survival. ÖZET GİRİŞ VE AMAÇ: Hepatoselüler kanserde (HSK); D vitamini ile yaşam süresi arasındaki ilişkinin araştırılması amaçlanmıştır.GEREÇ VE YÖNTEM: Hastanemiz Gastroenteroloji polikliniğinden takipli;HSK tanılı 265 hastanın retrospektif olarak dosyaları ve bilgi işlem sistemi aracılığıyla;etyolojik ve epidemiyolojik verileri,laboratuar değerleri,tedavileri,takip ve yaşam süreleri kaydedildi. 85 hastanın 25 OH vitamin D değerine ulaşıldı.Hastaların yaşama ve exitus durumları https://obs.gov.tr adresinden öğrenildi. Veriler ortalama +/- standart sapma olarak belirtildi. Sağkalım analizi için Kaplan-Meier ve Cox regresyon testi kullanılmıştır.Cox regresyonda değişken seçimi Forward Likelihood Ratio yöntemi ile yapılmıştır.İkiden fazla grup karşılaştırılmasında Kruskal-Wallis testi uygulanmıştır.Analizler için SPSS 15 ve R programı kullanılmıştır. P değerinin 0.05 den küçük olması halinde veriler anlamlı kabul edilmiştir BULGULAR: HSK tanılı 265 hastadan 25-OH vitamin D değerleri olan 85 hasta çalışmaya dahil edildi. Hastaların 66'sı erkek (%77,6) 19'u kadındı (%22,4). Ortalama yaş 62 (28-79) yıldı. Ortalama takip süresi 51±6.45 (1-100) aydı.32 (%37,7) hastanın takip süresi içinde ex olduğu tespit edildi. Ortalama yaşam süresi 15.62±12.93 (1-38) aydı.25-OH vitamin D değeri 9 ng/dl altında olan hastaların mortalitesinin yüksek (hazard ratio 2.37; %95 confidence interval; p=0.047) olduğu görüldü. Ayrıca siroz evresi ve HSK evresi ile vitamin D düzeyleri arasında negatif ilişki saptandı (sırasıyla p=0.035 ve p=0.043).SONUÇ: Hepatoselüler kanserli hastalarda düşük 25 OH vitamin D düzeyleri ile sağkalım arasında anlamlı ilişki bulunmaktadır. Çalışmamızda bu ilişkinin karaciğer hastalığının evresi ile bağlantılı olabileceği sonucuna varılmıştır. D vitamininin hastalık patogenezinde ve prognozu üzerine olan etkisi daha geniş serilerde ve prospektif çalışmalarla değerlendirilmelidir. ABSTRACTBACKGROUND AND AIMS: The purpose of this research is to investigate the relationship between vitamin D and life expectancy in Hepatocellular Cancer (HCC).MATERIALS AND METHODS: Monitored in our hospital Gastroenterology Clinic, HCC diagnosed 265 patients files and through data processing system; etiological and epidemiological data, their laboratory values, treatments, follow-up and life expectancy were recordedretrospectively. 85 patients reached 25 OH vitamin D values.The life status of the patients is learned from web at https://obs.gov.tr The data is shown as mean of +/- standard deviation. For survival analysis, Kaplan-Meier ve Cox regression tests are used.In Cox regression, variable selection is made with Forward Likelihood Ratio method. In comparing more than two groups,Kruskal-Wallis test is used. For analysis, SPSS 15 andR software is used. The data is considered significant when the P value is smaller than 0.05.RESULTS: Eighty five patients with vitamin D values from 265 patients were included in the research.66 of them were males (77.6%), 19 of them were women (22.4%).The mean age was 62 (28-79) years.The mean follow-up period was 51 ± 6.45 months. 32 (37.7%) patients were died.Life expectancy was 15.62±12.93 (1-38) months.Patients with 25 OH vitamin D levels below 9 ng / dl were found to have a higher mortality (hazard ratio 2.37; %95 confidence interval; p=0.047).In addition, negative correlation was found between cirrhosis and HCC and vitamin D levels (respectively p=0.035 ve p=0.043).CONCLUSION: There is a significant relationship between low 25 OH vitamin D levels and survival in patients with hepatocellular carcinoma.In our research this relationship may be associated with stage of liver disease.The effect of vitamin D on the pathogenesis and prognosis of the disease should be evaluated in extensive series and prospective research. 98
