- Abstract
Background: In reconstructive surgery, loss of a microvascular free flap due to perfusion disorders, especially thrombosis, is a serious complication. In recent years, viscoelastic testing (VET) has become increasingly important in point-of-care (POC) anticoagulation monitoring. This paper describes a protocol for enhanced anticoagulation monitoring during maxillofacial flap surgery. Objective: The aim of the study will be to evaluate, in a controlled setting, the predictive value of POC devices for the type of flap perfusion disorders due to thrombosis or bleeding. VET, Platelet monitoring (PM) and standard laboratory tests (SLT) are comparatively examined. Methods/Design: This study is an investigator-initiated prospective trial in 100 patients undergoing maxillofacial surgery. Patients who undergo reconstructive surgery using microvascular-free flaps will be consecutively enrolled in the study. All patients provide blood samples for VET, PM and SLT at defined time points. The primary outcome is defined as free flap loss during the hospital stay. Statistical analyses will be performed using t-tests, including the Bonferroni adjustment for multiple comparisons. Discussion: This study will help clarify whether VET can improve individualized patient care in reconstruction surgery. A better understanding of coagulation in relation to flap perfusion disorders may allow real-time adaption of antithrombotic strategies and potentially prevent flap complications.

In experimental acute respiratory distress syndrome (ARDS), random variation of tidal volumes (VT) during volume controlled ventilation improves gas exchange and respiratory system mechanics (so-called stochastic resonance hypothesis). It is unknown whether those positive effects may be further enhanced by periodic VT fluctuation at distinct frequencies, also known as deterministic frequency resonance. We hypothesized that the positive effects of variable ventilation on lung function may be further amplified by periodic VT fluctuation at specific frequencies. In anesthetized and mechanically ventilated pigs, severe ARDS was induced by saline lung lavage and injurious VT (double-hit model). Animals were then randomly assigned to 6 h of protective ventilation with one of four VT patterns: (1) random variation of VT (WN); (2) P04, main VT frequency of 0.13 Hz; (3) P10, main VT frequency of 0.05 Hz; (4) VCV, conventional non-variable volume controlled ventilation. In groups with variable VT, the coefficient of variation was identical (30%). We assessed lung mechanics and gas exchange, and determined lung histology and inflammation. Compared to VCV, WN, P04, and P10 resulted in lower respiratory system elastance (63 ± 13 cm H2O/L vs. 50 ± 14 cm H2O/L, 48.4 ± 21 cm H2O/L, and 45.1 ± 5.9 cm H2O/L respectively, P < 0.05 all), but only P10 improved PaO2/FIO2 after 6 h of ventilation (318 ± 96 vs. 445 ± 110 mm Hg, P < 0.05). Cycle-by-cycle analysis of lung mechanics suggested intertidal recruitment/de-recruitment in P10. Lung histologic damage and inflammation did not differ among groups. In this experimental model of severe ARDS, periodic VT fluctuation at a frequency of 0.05 Hz improved oxygenation during variable ventilation, suggesting that deterministic resonance adds further benefit to variable ventilation.

Background Postoperative pulmonary complications (PPCs) increase the morbidity and mortality of surgery in obese patients. High levels of positive end-expiratory pressure (PEEP) with lung recruitment maneuvers may improve intraoperative respiratory function, but they can also compromise hemodynamics, and the effects on PPCs are uncertain. We hypothesized that intraoperative mechanical ventilation using high PEEP with periodic recruitment maneuvers, as compared with low PEEP without recruitment maneuvers, prevents PPCs in obese patients. Methods/design The PRotective Ventilation with Higher versus Lower PEEP during General Anesthesia for Surgery in OBESE Patients (PROBESE) study is a multicenter, two-arm, international randomized controlled trial. In total, 2013 obese patients with body mass index ≥35 kg/m2 scheduled for at least 2 h of surgery under general anesthesia and at intermediate to high risk for PPCs will be included. Patients are ventilated intraoperatively with a low tidal volume of 7 ml/kg (predicted body weight) and randomly assigned to PEEP of 12 cmH2O with lung recruitment maneuvers (high PEEP) or PEEP of 4 cmH2O without recruitment maneuvers (low PEEP). The occurrence of PPCs will be recorded as collapsed composite of single adverse pulmonary events and represents the primary endpoint. Discussion To our knowledge, the PROBESE trial is the first multicenter, international randomized controlled trial to compare the effects of two different levels of intraoperative PEEP during protective low tidal volume ventilation on PPCs in obese patients. The results of the PROBESE trial will support anesthesiologists in their decision to choose a certain PEEP level during general anesthesia for surgery in obese patients in an attempt to prevent PPCs. 38. Long-term variable controlled ventilation in severe acute respiratory distress syndrome u2013 an experimental randomized controlled study

- Abstract
Background and Goal of Study: Mechanical ventilation with variable tidal volumes (VT) is able to improve lung function and attenuate ventilator induced lung injury (VILI) in acute respiratory distress syndrome (ARDS). Previous investigations on variable ventilation have been limited to mild or moderate ARDS and short periods of mechanical ventilation. Therefore, we hypothesized that 24h of variable ventilation improves lung function and reduces VILI in experimental severe ARDS. Material and methods: A double hit model of lung injury, lung saline lavage followed by mechanical ventilation with high VT (~20 mL/kg), was used to induce severe ARDS in 14 anesthetized pigs. After lung injury had been established, animals were randomly assigned to variable or conventional volume controlled ventilation with the following ventilatory settings: PEEP and FIO2 titrated according to the low FiO2/PEEP table of the ARDS network, and respiratory rate adjusted to arterial pH > 7.30. During variable ventilation, mean VT was set at 6 ml/kg and randomly varied (normal distribution, coefficient of variation of 30%), while limiting mean plateau pressure (Pplat) to 30 cmH2O and max Pplat to 45 cmH2O. During conventional ventilation, VT was set at 6 ml/kg and Pplat limited to 30 cmH2O. Gas exchange, hemodynamics and lung mechanics were assessed during 24 hours. Lung aeration was determined by computed tomography (CT) following induction of ARDS and 24h thereafter. Positron emission tomography (PET) was used to assess lung regional [18F]fluorodeoxyglucose ([18F]FDG) uptake, as well as the distribution of perfusion by 68Ga labelled microspheres. Results: After 24h, the specific uptake rate of [18F]FDG increased in all lung regions while perfusion was redistributed towards ventral and cranial lung regions and a higher amount of non-aerated lung tissue was detected during both, variable and conventional, ventilation. Lung mechanics, gas exchange and hemodynamics did not differ between groups. Conclusion: Compared to conventional, variable controlled ventilation neither enhanced lung function nor reduced lung inflammation after 24h of mechanical ventilation in this model of severe ARDS.

39. Quantification of pulmonary inflammation in experimental ARDS using static vs. dynamic positron emission tomography

- Abstract
Background and Goal of Study: Positron emission tomography (PET) imaging using 18F-FDG followed by mathematical modelling of the pulmonary uptake rate of 18F-FDG (Ki) has been frequently used for assessment of pulmonary inflammation in acute respiratory distress syndrome (ARDS). However, dynamic PET covers only a 15cm cranio caudal field of view and requires 60-75min. It has been shown in patients with liver metastasis that Ki values are highly correlated with tumor-to-blood standard uptake ratios (SUR), derived from the last frame of a 60min dynamic 18F-FDG-PET scan [van den Hoff et al., EJNMMI Research 2013;3(1):77]. We investigated whether Ki values derived from dynamic 18F-FDG-PET scans correlated with SUR derived from static 18F-FDG-PET scans in experimental ARDS. Methods: Lung injury was induced by saline lavage and ventilation with high tidal volumes in 12 pigs. After injury (day1) and after 24h of mechanical ventilation (day2) 18F-FDG uptake was quantified as Ki using the Patlak analysis of dynamic PET frames 15-75min after 18F-FDG injection. Mean Ki values were calculated for 5 isogravimetic regions along the ventro-dorsal gradient. Static 18F-FDG-PET scans were obtained immediately after dynamic PET scans (77-81min after 18F-FDG injection). Mean SUR values were calculated for the same regions as used for Ki analysis, using plasma activity measurements interpolated for the timepoint of static PET scans. Correlation between Ki and SUR were investigated by means of linear regression of all regions and animals (n=60) on day1 and day2, respectively, and coefficients of determination (r2) were compared. 18F-FDG-activity in blood plasma was determined for samples taken during dynamic PET scans. Plasma activities were time-averaged and analyzed for all animals and both days (n=24). Results: SUR was linearly correlated with Ki within all subregions, both on day1 (r2=0.83) and day2 (r2=0.96). Time-averaged plasma 18F-FDG-activities exhibited an essentially invariant shape across all animals and timepoints, resulting in similar average mean-normalized plasma activities at the timepoint of the static PET scans (0.29 u00b1 0.04). Conclusion: For quantification of pulmonary inflammation in experimental ARDS, SUR determined with static PET scanning is a valuable and faster alternative to dynamic PET scanning and Patlak analysis. The relatively high SUR- Ki correlation is likely explained by low inter-study variability of the plasma input function.

Variable versus conventional lung protective mechanical ventilation during open abdominal surgery (PROVAR) u2013 A randomized controlled trialRelevance of research: Postoperative pulmonary complications substantially contribute to perioperative morbidity and mortality. Current evidence suggests that conventional lung protective ventilation (CNV) improves postoperative lung function and outcome. According to experimental studies, variable lung protective ventilation (CVV) might have the potential to further improve lung protection in the perioperative setting. We report on the results of the PROVAR trial, which is the first clinical investigation on the impact of CVV on postoperative lung function, and was completed in March 2016. Background and goal of the study: Experimental studies showed that CVV recruits lungs, improving lung function and attenuating lung injury compared to CNV. In the present study, we compared CVV vs. CNV in patients undergoing general anesthesia for open abdominal surgery. We hypothesized that CVV compared to CNV improves respiratory function in the intra- and postoperative periods. Material and Methods: In this single center, randomized controlled trial, we enrolled 50 patients planned for open abdominal surgery lasting >3 hours. Patients were randomly assigned to receive either CVV or CNV. In both groups, mean VT and PEEP were set at 8 mL/kg (predicted body weight) and 5 cmH2O, respectively. In CVV, VT varied randomly (normal distribution, coefficient of variation of 30%). The primary endpoint, which was also used for sample size calculation, was the forced vital capacity (FVC) on postoperative day 1. Secondary endpoints were oxygenation, non-aerated lung volume, distribution of ventilation, as well as pulmonary and extrapulmonary complications until postoperative day 5 (clinicaltrials.gov: #NCT01683578). Results and discussion: Intraoperatively, CVV did not improve respiratory function, nor impaired hemodynamics compared to CNV. FVC did not differ significantly between groups on postoperative day 1 (61.9u00b123.6% versus 61.5u00b122.1%, in CVV and CNV, respectively, P = 0.190) and thereafter (P = 0.161). Forced expiratory volume at the first second (FEV1) and FEV1/FVC showed a greater deterioration with time in CVV compared with CNV. Oxygenation, distribution of ventilation, atelectasis formation, as well as the incidence of postoperative pulmonary and extrapulmonary complications, were also comparable between CVV and CNV. Conclusion: In patients submitted to open abdominal surgery, CVV did not improve intra- and postoperative respiratory function compared to CNV.
