Background: The prevalence and significance of microalbuminuria have not been well studied in patients with different heart failure subtypes. Therefore, we aimed to investigate the frequency and prognostic value of microalbuminuria in patients hospitalized for acute heart failure (AHF) with preserved ejection fraction (HFpEF), mid-range ejection fraction (HFmrEF), and reduced ejection fraction (HFrEF). Methods: All consecutive adult patients referred to the hospital due to AHF between June 2016 and June 2019 were enrolled. Microalbuminuria is defined as urinary albumin to creatinine ratio (UACR) level in the range of 30-300 mg/g. Hospital mortality was the endpoint of this study. Results: Of the 426 AHF patients (mean age 70.64 ± 10.03 years, 53.3 % female), 50% had HFrEF, 38.3% had HFpEF, and 11.7% had HFmrEF at presentation.The prevalence of microalbuminuria was 35.2%, 28.8%, and 28.0% in HFrEF, HFpEF, and HFmrEF, respectively. A total of 19 (4.5%) patients died during the in-hospital course, and in-hospital mortality was higher in HFrEF patients (6.6%) compared to patients with HFpEF (2.5%) and HFmrEF (2.0%). Multivariate analysis showed that the presence of microalbuminuria predicted in-hospital mortality in patients with HFrEF and HFmrEF but not in HFpEF. Conclusion: Although microalbuminuria was common in all subgroups of AHF patients, it has been found to predict prognosis only in patients with HFrEF and HFmrEF.

Bu çalışma 2018 yılında Tokat Gaziosmanpaşa Üniversitesi Ziraat Fakültesi Bahçe Bitkileri bölümüne ait laboratuvarda yürütülmüştür. Tokat ili Erbaa ilçesinde Gerbera sp. yetiştiriciliği yapan üreticiden temin edilen kesme Gerbera sp. çiçekleri materyal olarak kullanılmıştır. Farklı solüsyon uygulamalarının (Kontrol (Saf su), Sakkaroz (%6) + Saf su, Sakkaroz (%4) + sirke (%6), Sakkaroz (%6) + Carvacrol (150 mg/lt), Sakkaroz (%6) + Carvacrol (200 mg/lt), Sakkaroz (%6) + Thymol (150 mg/lt), Sakkaroz (%6) + Thymol (200 mg/lt), Sakkaroz (%4) + Sodyum hipoklorit (5 mg), Aspirin (5 mg) + saf su, Gibberellik asit (100 ppm), Gibberellik asit (200 ppm)) Gerbera sp. bitkisinin Yeliz çeşidinde vazo ömrü, oransal taze ağırlık, toplam vazo solüsyon alımı ve günlük vazo solüsyon alımı parametreleri incelenmiştir. Çalışma tesadüf parselleri deneme desenine göre 3 tekerrürlü olarak ve her tekerrür de 1 kesme çiçek olacak şekilde kurulmuştur.Deneme sonunda farklı solüsyon ortamlarının gerbera çiçekleri üzerinde incelenen tüm parametreler istatistiksel olarak önemli olduğu belirlenmiştir. Gerbera çiçeklerinde en uzun vazo ömrü Sakkaroz (%6) + Thymol (200 mg/lt) uygulamasından (19.50 gün) elde edilmiştir. Oransal taze ağırlık değişimi en fazla olan Sakkaroz (%6) + Carvacrol (150 mg/lt) uygulamasında (%23.60) saptanmıştır. Toplam vazo solüsyon alımı parametresine baktığımızda en yüksek solüsyon alımı Sakkaroz (%6) + Thymol (200 mg/lt) uygulamasında (67.40 gr/dal) ölçülmüştür. Günlük vazo solüsyon alımı Sakkaroz (%6) + Thymol (150 mg/lt) kesim yapılan gerbera uygulamasında (7.60 gr/dal) belirlenmiştir. Çalışmada uygulanan farklı solüsyon ortamlarının özellikle şeker ve uçucu yağları içeren solüsyonların vazo ömrüne ve diğer parametrelere doğrudan etkisi olduğu sonucuna varılmıştır. This study was conducted in 2018 in Tokat Gaziosmanpaşa University, Faculty of Agriculture, Department of Horticulture. Gerbera cut from Gerbera flowers were used as material. Different solution applications (Control (Pure water), Sucrose (6%) + Pure water, Sucrose (4%) + Vinegar (6%), Sucrose (6%) + Carvacrol (150 mg / lt), Sucrose (6%) + Carvacrol (200 mg / lt), Sucrose (6%) + Thymol (150 mg / lt), Sucrose (6%) + Thymol (200 mg / lt), Sucrose (4%) + Sodium hypochlorite (5 mg), Aspirin (5 mg) + distillate water, Gibberellic acid (100 ppm), Gibberellic acid (200 ppm) Gerbera sp. vase life, proportional fresh weight, total vase solution intake and daily vase solution intake parameters of. The study was set up to be 1 cut flower in each parcel with 3 replications, according to randomized parcel trial design.At the end of the experiment, it was determined that all parameters examined on gerbera flowers of different solution media were statistically significant. The longest vase life in Gerbera flowers was obtained from Sucrose (6%) + Thymol (200 mg / lt) application (19.50 days). The proportional fresh weight change was highest in Sucrose (6%) + Carvacrol (150 mg / lt) application (23.60%). When the total vase solution intake parameter was examined, the highest solution intake was measured in Saccharose (6%) + Thymol (200 mg / lt) application (67.40 g / branch). Daily intake of vase solution was determined by sucrose (6%) + Thymol (150 mg / lt) cut gerbera application (7.60 g / twig). It was concluded that the different solution media used in the study had a direct effect on the vase life and other parameters of the solutions containing sugar and essential oils. 41

Objective: The association between the nutritional status and outcomes in pulmonary embolism is unclear. This study was aimed at examining the value of the Controlling Nutritional Status (CONUT) score in assessing malnutrition among acute pulmonary embolism patients. Subject and Methods: We retrospectively reviewed the records of adult patients with acute pulmonary embolism hospitalized through our ED. Demographic, clinical, and laboratory data on admission were recorded. Nutritional status was assessed with the CONUT score, which is calculated by the albumin, total cholesterol, and lymphocyte counts. The primary endpoint of the study was in-hospital mortality. Results: A total of 308 consecutive patients (mean age 68.2 ± 12.9 years, 53.9% female) were included, and 35 of the patients (11.4%) died during their in-hospital course. Multivariate analysis showed that a pulmonary embolism severity index >148 (OR 3.12, 95% CI: 1.65–8.81, p < 0.001), the presence of heart failure (1.25, 95% CI: 1.08–1.78, p = 0.03), and a CONUT score >4 (OR 1.39, 95% CI: 1.146–3.424, p = 0.015) were independent predictors of in-hospital mortality. Conclusion: The present study indicates that the presence of malnutrition defined by the CONUT score predicts in-hospital mortality following acute pulmonary embolism.

Electrocardiography (ECG) is an auxiliary test applied for differential diagnosis of Pulmonary Thromboembolism (PTE). However, its specificity is low, but it is more often used for differential diagnosis. Although there are some changes on ECG in PTE, ST elevation is not a finding that we expect to see. With this case report, we aimed to draw attention to the issue with the diagnosis of PTE in a patient with ST elevation in ECG. 52-year-old male patient presented to the emergency department with a complaint of convulsion after chest and back pain. During the examination in the emergency department, he had a convulsion and woke up on his own without entering a postictal phase. After that, he had a chest pain. For this reason, ECG was performed to the patient and revealed ST elevation in leads DI, AVL, V1-4. The results of the patient’s EEG and head computer tomography (CT) were normal, so coronary CT angiography was performed. And it was observed that the coronary arteries were patent but embolism was detected in his cross-sectional pulmonary arteries. Thus, he was treated with the diagnosis of PTE. By the obtained results in this case that ST elevation may be a finding of PE, although it is rare.
