Abstract

Onychomycosis is one of the most common foot conditions. Mixed onychomycosis and onychomycosis caused by non-dermatophyte moulds are increasing in incidence, especially in vulnerable populations, hence the importance of this study, which presents the prevalence of onychomycosis in a population of homeless people, comparing the findings with a sample of a well-resourced population. The total sample consisted of 70 participants, divided into two separate groups, a homeless population and a second group in which we included people attending a private clinic. The average age of the sample is [49.19 ± 28.81] with an age range of 18 to 78 years. In the homeless group, the most prevalent infectious agents were non-dermatophyte fungi, with a total of 48%, compared to 28% in the group housed. The most common site of infection in both groups was the nail of the first finger. We, therefore, conclude that there is a difference in the infecting agent in the homeless population and the population with homes.

Abstract

Introduction There is scarce real-world experience regarding direct oral anticoagulants (DOACs) perioperative management. No study before has linked bridging therapy or DOAC-free time (pre-plus postoperative time without DOAC) with outcome. The aim of this study was to investigate real-world management and outcomes. Methods RA-ACOD is a prospective, observational, multicenter registry of adult patients on DOAC treatment requiring surgery. Primary outcomes were thrombotic and hemorrhagic complications. Follow-up was immediate postoperative (24–48 hours) and 30 days. Statistics were performed using a univariate and multivariate analysis. Data are presented as odds ratios (ORs [95% confidence interval]). Results From 26 Spanish hospitals, 901 patients were analyzed (53.5% major surgeries): 322 on apixaban, 304 on rivaroxaban, 267 on dabigatran, 8 on edoxaban. Fourteen (1.6%) patients suffered a thrombotic event, related to preoperative DOAC withdrawal (OR: 1.57 [1.03–2.4]) and DOAC-free time longer than 6 days (OR: 5.42 [1.18–26]). Minor bleeding events were described in 76 (8.4%) patients, with higher incidence for dabigatran (12.7%) versus other DOACs (6.6%). Major bleeding events occurred in 17 (1.9%) patients. Bridging therapy was used in 315 (35%) patients. It was associated with minor (OR: 2.57 [1.3–5.07]) and major (OR: 4.2 [1.4–12.3]) bleeding events, without decreasing thrombotic events. Conclusion This study offers real-world data on perioperative DOAC management and outcomes in a large prospective sample size to date with a high percentage of major surgery. Short-term preprocedural DOAC interruption depending on the drug, hemorrhagic risk, and renal function, without bridging therapy and a reduced DOAC-free time, seems the safest practice.

Abstract

The prevalence of type 2 diabetes is increasing worldwide. The aim of our study was to detect people susceptible to DM among a university population aged 18 to 45 years and analyze the existence of modifiable risk factors in order to implement prevention programs, in addition to analyzing BMI data related to the variables under study. We proposed a descriptive, cross-sectional study following the recommendations of cross-sectional studies (STROBE), with a sample of 341 subjects, students enrolled at the University of Extremadura, carried out by two researchers. The research protocol was approved by the Bioethics Committee of the University of Extremadura (165/2021). The study considered the Findrisk questionnaire in Spanish, validated by the Blackboard Study, a stadiometer to measure height, a bioimpedance meter to evaluate weight and body composition parameters, and a blood pressure monitor to measure blood pressure. The results indicated that the participants had a low risk of suffering T2DM. The highest Findrisk test scores were found in those with a BMI value above 25, lower physical activity, poor dietary intake of fruits and vegetables, and increased fat mass. Our future research will be the implementation of T2DM prevention programs, acting on modifiable factors. [ABSTRACT FROM AUTHOR]

Introducción: La endocarditis infecciosa (EI) es una patología que conlleva una elevada morbimortalidady representa un problema creciente de salud pública. El tratamiento quirúrgico es necesario en más dela mitad de los casos, y su principal finalidad es eliminar totalmente el tejido infectado y reconstruir laanatomía cardiaca normal, incluyendo la reparación o la sustitución de la/s válvula/s afectada/s.Objetivo y métodos: El presente estudio se centra en describir el procedimiento más apropiado para lacirugía de reparación mitral en casos de EI, atendiendo a la literatura y a la amplia experiencia adquiridapor parte del equipo quirúrgico. De igual modo, hemos recopilado a través de una búsqueda sistemáticade la literatura los resultados que se han ido reportando sobre la cirugía reparadora en comparación conel reemplazo valvular mitral.Resultados: La técnica quirúrgica descrita requiere de un estudio minucioso del grado de afectación ydesestructuración de los distintos segmentos valvulares. Consideramos que la resección con interposi-ción de parche de pericardio y/o implante de neocuerdas de Goretex®es la técnica estándar que permiteobtener resultados reproducibles. La revisión estructurada de las evidencias científicas disponibles veri-fica la seguridad de la reparación mitral para el tratamiento de la EI, aportando beneficios significativosen comparación con el reemplazo valvular.Conclusión: Los resultados quirúrgicos evaluados sugieren que la cirugía de reparación mitral deberíaser considerada el procedimiento de elección para el tratamiento quirúrgico de la EI, siempre que seatécnicamente posible., Introduction: Infective endocarditis (IE) is a pathology with a high morbidity and mortality rate andrepresents a growing public health problem. Surgical treatment is necessary in more than half of thecases, and its main purpose is to completely remove the infected tissue and reconstruct normal cardiacanatomy, including repair or replacement of the affected valve(s).Objectives and methods: The present study focuses on describing the most appropriate procedure formitral repair surgery in cases of IE, based on the literature and the extensive experience gained by thesurgical team. Likewise, we have compiled through a systematic search of the literature the results thathave been reported on reparative surgery compared to mitral valve replacement. Results: The surgical technique described requires a thorough study of the degree of involvement anddestructuring of the different valve segments. We consider resection with pericardial patch interpositionand/or Goretex®neochord implantation to be the standard technique that allows reproducible results. Thestructured review of the available scientific evidence verifies the safety of mitral repair for the treatmentof IE, providing significant benefits compared to valve replacement.Conclusion: The evaluated surgical results suggest that mitral repair surgery should be considered theprocedure of choice for the surgical treatment of IE, whenever technically feasible.
