8. The Importance of Social Work in Healthcare for Individuals with Rheumatoid Arthritis

Abstract

People with rheumatoid arthritis (RA), often associatedwith psychosocial problems and reduced quality of life, benefitfrom the guidance of trained medical social workers. Thisstudy explores the effectiveness of psychosocial treatmentin patients with RA using a structured interview to detectpsychosocial problems for 100 patients. These individualswere offered regular sessions with a medical social worker.Three types of mixed-problems were found: mixed problemsrelated to RA, mixed problems related to the life situation, andmixed problems related to a combination of RA and the lifesituation. The RA patients who reported mixed-problems attime of their diagnosis received psychosocial treatment froma medical social worker regardless of the mixed problems theyexperienced. In addition, we found that disease-related mixedproblems seemed more treatable than other problems. Socialwork in somatic healthcare seems most successful in patientswith sickness-related social and psychosocial problems.

Abstract

Patients aged 18–65 years with newly diagnosed diabetes mellitus (n = 89) or rheumatoid arthritis (n = 100) were studied by the General Coping Questionnaire at baseline and after 24 months. In total, 34 diabetes mellitus and 46 rheumatoid arthritis patients were diagnosed with psychosocial problems. The use of negative coping strategies, such as protest, isolation, and intrusion, was associated mostly with being classified as having psychosocial problems. With the risk of experiencing a strong impact of the disease at 2 years of follow-up as outcome, intrusion was a significant and independent risk factor, while minimization was a significant and independent protective factor. Abstract

We have explored Swedish medical social workers' attitudes, beliefs, knowledge, and behavior concerning evidence-based practice (EBP) and investigated the properties of a questionnaire to measure EBP. One hundred seventy-four Swedish medical social workers within university hospital care and primary care participated in a cross-sectional survey. Our results showed positive attitudes toward EBP and the use of evidence to support clinical decision making. EBP was seen as necessary and something that needed to be implemented more often. The main barriers to implementing EBP were lack of time (78%), the perception that EBP does not take into account the limitations of the clinical practice setting (78%), and lack of knowledge about relevant research (46%).

Abstract

Objective: To compare the registering of diabetic complications and metabolic control in type 2 diabetic subjects in primary health care in 1995 and 2001., Design: Cross-sectional surveys of medical records., Setting: Two primary health care centres in Stockholm County., Subjects: Diabetic patients aged 35-64 years; in 1995, 59 men and 42 women and in 2001, 80 men and 80 women., Main Outcome Measures: Rate of notified examinations in medical records, diabetic complications and metabolic control., Results: Increased rates between 1995 and 2001 were found regarding examinations for neuropathy (46% vs 79%), albuminuria (74% vs 91%), micro-albuminuria (11% vs 59%), BMI (33% vs 66%), HbA1c (88% vs 99%), cholesterol (69% vs 92%) and LDL cholesterol (42% vs 89%) (p < 0.001). Increased rates of perceived treatment goals between 1995 and 2001 were found regarding HbA1c (< 6.5%) (28% vs 57%) and cholesterol (< 5.0 mmol/l) (23% vs 36%) (p < 0.001), and also regarding LDL cholesterol (30% vs 36%) (p < 0.01). In men, the number of subjects with any macrovascular complication decreased between 1995 and 2001 (34% vs 17%) (p < 0.01), and in women the number of subjects with coronary heart disease decreased (26% vs 6%) (p < 0.01)., Conclusion: Documented examination rates and metabolic control improved between 1995 and 2001.
