Background Hypercortisolism affects calcium and phosphate metabolism in dogs; however, the exact mechanisms are not completely understood. Objectives To evaluate circulating concentrations of whole parathormone (wPTH), 25‐hydroxyvitamin D (25‐(OH)D), calcitriol, and fibroblast growth factor‐23 (FGF‐23) in dogs with naturally occurring hypercortisolism (NOHC) and healthy dogs, and their association with calcium and phosphate homeostasis. Animals Twenty‐three client‐owned dogs with NOHC, and 12 client or staff‐owned healthy dogs. Methods Prospective cross‐sectional study. The circulating concentrations of total calcium, ionized calcium (iCa), phosphate, wPTH, 25‐(OH)D, calcitriol and FGF‐23, and the urinary fractional excretion of phosphate (FEP) and calcium (FECa) were compared between dogs with NOHC before treatment and healthy dogs. The objective of this study was to investigate the impact of the COVID-19 pandemic on the prevalence and persistence of white spot lesions (WSLs) among patients treated with traditional braces.A group of 434 consecutively finished cases who had pretreatment and post-treatment digital photographs taken were included in this study. Each set of photographs was evaluated by two investigators to determine prepandemic and postlockdown WSLs, in addition to changes in oral hygiene (OH) and other risk factors.There was a statistically significant difference in the development of WSLs pre-COVID-19 lockdown (PRL) versus post-COVID-19 lockdown (POL) (P 0.001). The overall prevalence for the development of WSLs increased to 52.8% POL. For patients with good pretreatment OH, the risk of developing WSLs POL (42.0%) was two times greater than the rate in PRL (20.1%). For patients whose OH worsened during treatment, 81.2% of POL patients developed WSL compared with 69.6% of PRL patients. The incidence of WSLs was statistically greater in the maxillary anterior region POL than PRL, whereas the incidence in the mandibular anterior region did not reach statistical significance. The distribution of WSLs was greater POL for each tooth irrespective of the arch.Because of the COVID-19 pandemic, the prevalence of visible WSLs increased from 29.5% to 52.8%. Patients whose hygiene worsened during orthodontic treatment are at an increased risk of developing WSLs.

An intersection of two roman glareatae roads and an underground stretch of a Roman aqueduct (rivus subterraneus) were found in the locality of Colle Oliva, during an emergency archaeological investigation (between 11/2018 and 3/2019). The aqueduct has been detected in several test trenches and some profiles have been obtained. These data shade some light about the Roman hydraulic technology and they show how the land morphology affected the shape and the path of the aqueduct.

Web-based surveys were conducted in 2009 and 2016. An invitation to participate in the study, along with a hyperlink to the survey, was emailed to the deans of all U.S. dental schools in the two years. In 2009, 47 of 58 dental school deans responded to the survey, for a response rate of 81%. In 2016, 48 of 64 dental school deans responded, for a response rate of 75%. From 2009 to 2016, the number of schools reporting dental student international experiences increased from 25 to 31. In 2016, 65% of responding schools offered dental student international experiences, an 11.5% increase over the results of the 2009 survey. Concomitantly, the number of deans reporting their students' participation in international opportunities not officially sanctioned by the school decreased from 41 to 34. These findings showed an increase in the number of dental schools providing international experiences for their students and established baseline data to assess trends in the future.

Although several recent studies have gauged the participation of US dental schools and dental students in international volunteer programs, no study of US dentists' international dental volunteer activities has been published to date. The goal of this study was to quantify that participation and explore dentists' motivation for service. A web-based survey was developed, and, in August 2018, a hyperlink to a web-based survey consisting of 12 questions was sent to 19,679 US dentists whose names and contact information were obtained from a commercially obtained email list. All respondents were volunteers, and no compensation was offered. Of the 19,679 potential respondents contacted via email, 1295 (6.6%) participated in the survey. Some respondents did not answer all the survey questions. A majority (79.2%) were 51 years or older, and 41.9% of respondents reported having participated in volunteer dental activities outside the United States. Of those dentists, 99.6% reported having had positive experiences, and 98.8% reported that they would recommend this activity to other dentists. Among the respondents, 27.4% reported having volunteered more than once, and 12.1% reported that they volunteered regularly.

Focused Clinical Question: Periodontal disease is related to use of tobacco, particularly cigarettes. Cigarette smoking is the leading cause of preventable death in the United States, with 20% of annual deaths attributable to smoking-related illness. How does motivating patients to quit smoking challenge periodontists and other providers to improve clinical management? Summary: Four patient cases from the author's (CED) clinical practice in behavioral medicine illustrate key points in management of two patients who were successful in quitting smoking and two who were unsuccessful quitting. Conclusion: These cases illustrate some of the characteristics of patients and factors that contribute to successful smoking cessation and provide examples and practical information for use in the dental office for helping patients with smoking cessation.

The purpose of this study was to assess concentrations of angiopoietin (Ang)-1 at various stages of gingival inflammation. Ang-1 is an anti-inflammatory mediator present in various inflammatory diseases. However, its presence during the pathogenesis of gingival inflammation has not been established in vivo.Gingiva was obtained from 110 human donors before extraction of the adjacent teeth. The tissue was grouped based on adjacent probing depth and bleeding on probing (BOP). Gingiva adjacent to aor=3-mm sulcus without BOP was classified as "normal" (N); gingiva adjacent to a 3-mm sulcus with BOP was classified as "diseased, slight" (DS); gingiva adjacent to a 4- to 6-mm sulcus featuring BOP was classified as "diseased, moderate" (DM); and gingiva adjacent to6-mm sulci was classified as "diseased, severe" (DSev). Tissues were solublized, and concentrations of interleukin (IL)-1beta and -6, tumor necrosis factor (TNF)-alpha, endothelin (ET)-1, Ang-1, vascular cell adhesion molecule (VCAM)-1, and vascular endothelial growth factor (VEGF) were assessed by enzyme-linked immunosorbent assay. Data were compared by factorial analysis of variance, the post hoc Tukey test, and the Pearson correlation test. Groups were defined as significantly different when P0.05.Gingival concentrations of IL-1beta and -6, TNF-alpha, VEGF, and ET-1 were significantly greater, and VCAM-1 and Ang-1 were significantly lower, in DSev and DM than in N and DS tissues (P0.05). In addition, gingival concentrations of IL-6, VEGF, and ET-1 were significantly greater, and VCAM-1 and Ang-1 were significantly lower, in DSev than in DM tissues (P0.05). There were significant positive correlations among sulcular depth, IL-1beta and -6, TNF-alpha, VEGF, and ET-1 and negative correlations among VCAM-1, Ang-1, sulcular depth, and the other biomarkers (P0.001).Depleted tissue concentrations of Ang-1 may allow gingival inflammation to become more severe because VEGF and ET-1 secretion become less inhibited. Thus, the tissues become edematous and more likely to develop BOP.

Background and Objective: While there is substantial information concerning the concentrations of interleukin-1 isoforms within gingival crevicular fluid, there is little information concerning their concentrations within either normal or diseased gingival tissues. Therefore, the aim of this study was to evaluate the relationship between the concentrations of gingival interleukin-1 isoforms and the adjacent sulcular depth. Material and Methods: Interdental gingival papillae were excised and grouped based on adjacent pocket depth and the presence of bleeding on probing. The presence of interleukin (IL)-23 has not been reported within inflamed gingiva, so we evaluated its concentration within gingiva from normal sites and sites of chronic periodontal disease.Gingiva was obtained prior to extraction of teeth. It was grouped based on clinical attachment loss (CAL): 0 to 2 mm (normal-slight), 3 to 4 mm (moderate), and5 mm (severe). Groups were defined as significantly different when P0.05.The gingival concentrations of IL-23, -17, -1beta, and -6 and IFN-gamma were significantly greater at moderate CAL sites than at normal-slight CAL sites. Gingival concentrations of IL-23, -1beta, -17, and -6 and TNF-alpha were significantly greater at severe CAL sites than at normal-slight CAL sites. In addition, the gingival concentrations of IL-23, -17, and -6 and TNF-alpha were significantly greater and the gingival concentrations of IL-12 and IFN-gamma were significantly lower at severe CAL sites than at moderate CAL sites. Gingival concentrations of IL-23, -17, -6, and -1beta and TNF-alpha correlated positively with CAL. The IL-23 gingival concentration correlated significantly with IL-17, -1beta, and -6 and TNF-alpha concentrations and correlated negatively with IL-12 and IFN-gamma concentrations.Our results suggested the possibility that the IL-23/IL-17 immune response was present within chronically inflamed gingiva. This is a host response that had not been reported previously in periodontal disease and may be an important factor in the chronic nature of the disease.

There are no comparisons of concentrations of interleukin (IL)-13 and -15 and gingival sulcular depth within healthy or inflamed gingiva. Study of non-oral inflammations indicated that IL-15 has similar functions to IL-2, and IL-13 has similar functions to IL-4 and -10. Thus, IL-13 and -15 likely have a function in gingival inflammation.We compared the concentrations of IL-13 and -15 within healthy or inflamed human gingiva to gingival sulcular depth and the concentration of various T-helper 1 and T-helper 2 cytokines in a group of Hispanic subjects. Gingival papillae were grouped by the depth of the adjacent gingival sulcus:or=3 mm (normal); 3 to 6 mm and6 mm (diseased). The gingival concentrations of IL-2, -4, -6, -10, -13, and -15 and interferon (IFN)-gamma were assessed by enzyme-linked immunosorbent assay.IL-2, -4, -6, -10, and -13 and IFN-gamma concentrations were higher within gingiva adjacent to 3 to 6 mm diseased compared to normal; IL-15 concentrations were significantly greater within normal gingiva than within diseased gingiva (P0.001). Within gingiva adjacent to6 mm sulci, IL-6 concentrations were significantly higher than in normal gingiva, and the concentrations of the other cytokines were similar to those in normal gingiva. Gingival IL-6 concentration correlated significantly with the adjacent gingival sulcular depth, whereas the IL-15 concentration correlated inversely with the adjacent gingival sulcular depth (P0.001).IL-13, -4, and -10 had a similar concentration pattern within normal and diseased tissue; however, the concentration pattern of IL-15 was not similar to IL-2. Relatively low IL-15 concentrations within diseased gingiva suggest that IL-15 might have anti-inflammatory properties.

Progression factors for periodontal diseases have been suggested by in vitro study of peripheral blood and gingival cells; however, those factors are not established in vivo. This investigation assessed biopsies of three groups of gingival tissues: those adjacent to a 1)or =3 mm (normal), 2) 4-6 mm, and 3)6 mm gingival sulcus, to determine changes in the gingival microenvironment coincident to the progression of periodontal disease. Superoxide dismutase (SOD) and catalase activity, and IL-12 and bcl-2 levels, were decreased at6 mm; total protein and IL-6 concentrations were increased adjacent to6 mm, as compared toor =3 and 4-6 mm, sites. Apoptotic cells were evident only within gingiva adjacent to6 mm sites. These data suggest that IL-12 is an important factor in the shift from a TH1 to TH2 cell profile and that a favorable gingival microenvironment for hyperinflammation may develop coincident to progression of periodontal diseases due to decreased bcl-2 and increased IL-6 concentrations within gingiva. These changes in the gingival microenvironment could impair apoptosis and promote enhanced release of reactive oxygen species (ROS) by phagocytes; decreased catalase and SOD activity could promote accumulation of ROS and result in additional tissue destruction.

Variants of the opioid receptors are the obvious candidates underlying addiction. The kappa opioid receptor (KOR) system seems to play a role in stress responsivity, opiate withdrawal and responses to psycho-stimulants, inhibiting mesolimbic dopamine. KOR gene polymorphisms have been reported to contribute to predisposition to voluntary alcohol-drinking behavior in experimental animals. In humans, the 36G > T single nucleotide polymorphism (SNP) on KOR gene, that was recently identified, has been found associate with substance dependence, with inconclusive findings. In the present study, 106 heroin addicts (West European, Caucasians) and 70 healthy control subjects matched for race and gender, with no history of substance use disorder, have been genotyped. The frequency of KOR 36G > T SNP was significantly higher among heroin-dependent individuals compared with control subjects (Fisher's exact = 0.044; Pearson chi(2) = 4.2734, P = 0.039; likelihood ratio chi(2) tests = 4.6156, P = 0.032). Although KOR silent polymorphisms may apparently have no consequences on mRNA transcription, post-transcriptional mechanisms, such as mRNA stability, translation efficiency, and regulability may impair the function of kappa receptors system, with increased risk for substance use disorders. In specific, the neurobiological changes induced by mu-kappa opioid imbalance could underlie vulnerable personality traits and risk behavior.

Objective: We evaluated the efficacy of fine needle aspiration cytology (FNAC) of the thyroid in a series of 5469 lesions with histological control and studied the causes of, and the possibility of reducing the limitations of the method. Methods: FNAC was always performed by a pathologist under the guidance of a clinician, using a 22-gauge needle. Generally two aspirations were carried out, and usually four slides were obtained for each nodule; they were then stained with May–Grunwald–Giemsa and with Papanicolaou. The cytological diagnoses were classified in four groups: inadequate, benign, suspicious and malignant. Results: We obtained a complete sensitivity of 93.4%, a positive predictive value of malignancy of 98.6%, and a specificity of 74.9%. At histological control, the cytological diagnosis of Hurthle cell neoplasm corresponded to a significantly higher incidence of malignant neoplasms than the diagnosis of non-Hurthle cell follicular neoplasm (32.1% versus 15.5%). There were 66 false-negative findings, the main cause of diagnostic error (24 cases) being failure to recognize the follicular variant of papillary carcinoma. The number of inadequate FNACs was low (4.2%). Conclusion: Our study confirmed the great efficacy of thyroid FNAC. A cytological diagnosis of Hurthle cell neoplasm should be considered an indicator of high risk. Awareness that failure to recognize the follicular variant of papillary carcinoma was the main problem in the interpretation of thyroid FNAC should lead to a decrease of false-negative diagnoses. The inadequate rate was very low, as it was the pathologist personally who performed the needle aspiration.

Interleukin (IL)-11 and IL-17 are cytokines that modulate the inflammatory process and have not been assessed within normal or inflamed gingival tissues. Our purpose was to compare concentrations of human IL-11 and IL-17 within healthy and diseased human gingiva to determine their possible role in the initiation or progression of periodontal diseases.Biopsies from healthy (non-hemorrhagic gingiva adjacent to aor = 3 mm gingival sulcus) and diseased gingiva (hemorrhagic gingiva adjacent to aor = 3 mm periodontal pocket) were studied. IL-11, IL-17, RANTES, and IL-6 concentrations were assessed within solubilized gingival biopsies by enzyme-linked immunosorbent assay. Data were compared by factorial analysis of variance and a post-hoc Tukey honestly significant difference (HSD) test. Regression analysis and partial correlation analysis (adjusted for sample weight) were also used to determine correlations between the variables.Interleukin-11 concentrations were highest within gingiva adjacent to 3 mm diseased pockets (P0.001), and IL-17 concentrations were highest at 4 to 5 mm sites compared to other sites (P0.001). Gingival concentrations of both cytokines were significantly lower in gingiva adjacent to aor = 6 mm pocket. RANTES concentrations were significantly greater in gingiva adjacent toor = 6 mm pockets than in tissues derived from other sites (P0.001). IL-11, IL-6, and RANTES concentrations were significantly correlated with sulcular depth.Gingival concentrations of IL-11 and IL-17 are different in diseased gingiva adjacent to 3, 4 to 5, andor = 6 mm pockets, suggesting that their concentrations change as a consequence of the progression of gingivitis to periodontitis and that both cytokines could have a significant role in this progression. These data may be useful for the design of procedures for prevention of periodontal disease.

Fine needle aspiration cytology of primary lymphoma of the thyroid: a report of 17 cases Between 1980 and 1998, 4272 thyroid surgical specimens with a preoperative fine needle aspirate were sent to our Anatomical Pathology Department. Among these cases there were 17 primary thyroid lymphomas, which constituted 0.3% of all the thyroid lesions and 2.3% of the thyroid malignancies. Seven cases were diffuse large B-cell (DLBC) lymphomas and 10 were MALT lymphomas. Of the DLBC lymphomas six were correctly diagnosed by fine needle aspiration cytology (FNAC) and one was diagnosed as positive for malignancy, and among MALT lymphomas four were diagnosed as lymphoma, four as suspicious for lymphoma, and three as Hashimoto’s thyroiditis (HT). Our data indicate that the diagnosis of primary thyroid lymphoma of high grade is easy, and immunocytochemistry (ICC) can confirm suspicious cases. The diagnosis of MALT lymphoma is more difficult; ICC can confirm suspicious cases, and false-negative results seem to be caused by sampling error, because HT usually coexists with MALT lymphoma.

Tissues become hemorrhagic and edematous coincident to periodontal diseases; however, there is little information concerning the biologic mechanisms which may produce these changes. Vascular endothelial growth factor (VEGF) is a macromolecule which enhances blood vessel growth and permeability. However, there is no information concerning gingival VEGF concentrations within normal or diseased gingiva. The purpose of this study was to assess changes in gingival concentrations of VEGF during initiation and progression of periodontal diseases and compare them to changes in the number of blood vessel profiles and concentration of recognized markers of periodontal disease severity (interleukin-6[IL-6]).Normal (non-hemorrhagic gingiva adjacent to aor =3 mm gingival sulcus) and inflamed gingiva (hemorrhagic gingiva adjacent to aor =3 mm, 4 to 6 mm, or6 mm periodontal pocket) were studied. VEGF and IL-6 concentrations were assessed by ELISA and the number of blood vessels determined by histomorphometric techniques. Data were placed into one of the following groups:or =3 mm, normal;or =3 mm, diseased; 4 to 6 mm, diseased; and6 mm, diseased. These groups were compared by factorial ANOVA and Scheffe comparisons. In addition, groups were compared by simple and multiple regression and regression ANOVA to determine possible correlations between them.VEGF and IL-6 concentrations were significantly lower within normal than within diseased gingiva. The number of blood vessel profiles and mean IL-6 concentrations were highest in diseased tissues adjacent to6 mm sulci and were significantly correlated with sulcular depth (P0.001). In contrast, VEGF concentrations were highest within diseased gingiva adjacent to 4 to 6 mm periodontal pockets (P0.001) and were not correlated with sulcular depth.VEGF may be a factor in initiation and progression of gingivitis to periodontitis, possibly by promoting expansion of the vascular network coincident to progression of the inflammation.

Obiettivi: l’obiettivo principale dello studio è stata la valutazione dei ricoveri ripetuti nei soggetti con vasculopatia diabetica, rispetto ai soggetti con vasculopatia non diabetica.Materiali e Metodi: sono state selezionate le SDO del triennio 1998-2000 contenenti, in uno dei campi di diagnosi, i codici di diabete mellito, con e senza complicanze (codici ICD9 ed ICD9CM, 250.—), vasculopatia periferica (440.2 e 440.8), ulcera periferica (707.1, 707.8, 707.9), gangrena (785.4). Le SDO con diagnosi di vasculopatia diabetica sono state considerate come angiopatie diabetiche (AD); le altre sono state attribuite agli angiopatici non diabetici (AND). Le SDO con diabete non complicato e arteriopatia periferica sono state eliminate. L’intervento di amputazione è stato individuato dalla presenza del codice ICD9CM di procedura 84.—. La ripetitività degli episodi è stata valutata usando il codice fiscale come identificativo del paziente. Risultati: sono state rilevate 5814 dimissioni di AD e 9010 di AND. Le amputazioni sono state 285 (4,90%) negli AD e 485 (5,38%) negli AND. I ricoveri ripetuti sono stati 13 negli AD e 12 negli AND. Le amputazioni sono state osservate verso il 4°-5° episodio di ricovero negli AD, mentre già dal 2° episodio negli AND. Le ulcere erano presenti nel 36,05% (il 36,5% nel primo episodio di ricovero, che resta costante negli episodi successivi) degli AND e nel 10,97% degli AD (9,84% nel primo episodio, con un incremento fino al 20% del 5° episodio). Un episodio in day-hospital (DH) si osserva nel 12,32% degli AD, contro il 4,20% degli AND.Conclusioni: l’accesso degli AD all’assistenza intraospedaliera avviene più frequentemente in DH, i passi successivi sono caratterizzati dalla comparsa dell’ulcera o gangrena e infine dall’amputazione. Nel gruppo degli AD è stato osservato un eccesso di ospedalizzazione, probabilmente per ripetitività del ricovero, anche con ricorso al DH. La cattiva codifica potrebbe aver causato una sottostima della percentuale di amputati nel gruppo dei non diabetici.

Celiac disease (CD) is a chronic inflammatory enteropathy of the small bowel resulting from a local TH1-mediated reaction to wheat gliadins and barley, rye and oat prolamins with the development of auto-antibodies to transglutaminases. As well as for other chronic inflammatory diseases, genetic background and environmental factors participate to pathogenesis. An increased traffic of CD34+ hemopoietic precursor and stem cells (HPC) has been reported in peripheral blood (PB) of subjects with allergic diseases that share in their pathogenesis immuno-mediated reactions, genetic and environmental factors. The aim of the present work was to investigate the CD34+ cell traffic and H2/H1 polarization of lymphoid T-cell lineage, in the peripheral blood of subjects with CD, by means of flow-cytometric techniques. Group A of control was of 20 healthy subjects, aged 5 to 58 years. Study population (Group B) was of twenty-eight patients, all females aged 13 to 70, receiving firstly a CD diagnosis at the SS Annunziata Hospital Digestive Physiopathology Out-standings' by means of clinical, serologic and small intestinal biopsy findings. Peripheral CD34+ HPCs were significantly increased in Group B (median value 0.16) when compared with Group A (median value 0.03) (p 0.0001) but did not correlate either with anti-transglutaminase (tTG) antibody levels (IgA: p 0.226; IgG: p 0.810) or with histological damage severity (p 0.41) that, on the contrary, was significantly related with anti-tTG IgA antibodies (p 0.027). Celiac circulating CD3+CD4+ lymphocytes expressed a chemokine-receptor pattern Th2-skewed in all but three patients investigated. Concluding, the CD34+ HPC highly increased peripheral traffic observed in celiac disease appears more related to a basic and emerging as common defect shared by chronic inflammatory diseases than to the gliadin-specific Th1 local reactions. Data are consistent with a potential NFkappaB deficiency and consequent prevalence of apoptotic versus survival programs leading to excessive cell-death; to replace lost cells a supplementary bone-marrow derived precursors supply, further to that physiologically provided by the gut stem cell "niches" that are cryptopatches, could be required.

Plasma cell gingivitis is a rare benign condition of the gingiva. It is marked by a dense infiltrate of normal plasma cells separated into aggregates by strands of collagen. It is a hypersensitivity reaction to some antigen, often flavorings or spices. The importance of this lesion is that it may cause severe gingival inflammation, discomfort, and bleeding and may mimic more serious conditions. Plaque control and conventional periodontal therapy alone will not cure this disease. The etiologic agent must be identified and the substance eliminated from use. This report outlines a case of plasma cell gingivitis which may have been brought on by the use of red peppers in cooking. J Periodontol 1991; 62:390–393.

Standard asbestos diagnostic protocol was applied to eleven relatives of asbestos exposure ex-workers of a cement factory in Bari. Nine wives and seventeen sons were involved as volunteers in this evaluation. In this group two pleura malignant mesotheliomas (not dose-dependent) two asbestosis and fifteen pleura plaques (dose-dependent) were detected. This situation shows high level of asbestos contamination at home. For all the test patients the contamination most probably occurred because workers carried asbestos substances home from work on their clothes.

Colorectal Kaposi's sarcoma, a human herpes virus-8 associated mesenchymal tumour, is exceedingly rare in human immunodeficiency virus-negative subjects and almost always reported in association with severe, refractory, inflammatory bowel disease. In this paper we report a case – the second from Italy – of a colorectal Kaposi's sarcoma in a human immunodeficiency virus-negative, heterosexual man with severe refractory ulcerative colitis. Kaposi's sarcoma developed after starting glucocorticosteroid therapy, supporting the theory that colorectal Kaposi's sarcoma associated with ulcerative colitis is iatrogenic.

Three new aromatic sesquiterpenes (1, 2, and 4), one new dimeric sesquiterpene of the cyclolaurane-type (3), one sesquiterpene alcohol of bisabolene type (8) along with three previously reported metabolites (5-7), were isolated from the organic extracts of Laurencia microcladia, collected from the Chios island in the North Aegean Sea. The structures of the new natural products, as well as their relative stereochemistries, were established by means of spectral data analyses, including 2D experiments. The cytotoxicity of the isolated metabolites was evaluated against five human tumor cell lines. © 2007 Elsevier Ltd. All rights reserved.

This study compared the anti-aggressiveness effects of the atypical anti-psychotic olanzapine with that of selective serotonin reuptake inhibitors (SSRI) and benzodiazepines (BZD) among patients with heroin dependence submitted to opioid-agonists substitution treatment. Sixty-seven (67) patients who met the DSM-IV criteria for heroin dependence and showed aggressive personality traits, not affected by comorbid schizophrenia or bipolar disorder, accepted to participate in a 12-week prospective, observational trial. Patients were included into two subgroups in relationship with treatment, for the evaluation of the endpoints at week 12: group 1: substitution treatment in combination with OLA (32 patients); group 2: substitution treatment in combination with fluoxetine/paroxetine and clonazepam (35 patients). Efficacy measures were Buss Durkee Hostility Inventory (BDHI), Symptoms Check List-90 (SCL 90) anger--hostility scores, incidence rates of aggressive incidents and attacks. The rates of patients who remained in treatment at week 12 in group 1, treated with OLA, and group 2, treated with SSRI and BDZ, were not significantly different (17 = 53.1% vs 16 = 45.7%). BDHI total, direct aggressiveness, verbal aggressiveness scores, SCL 90 aggressiveness scores and aggressive incidents rates showed a significantly more consistent decrease from baseline in group 1 than in group 2 subjects, in the patients who completed the treatment (p < 0.001; p < 0.01; p < 0.05; p < 0.01; p < 0.001). Among the completers, 69.3% achieved early full substance abuse remission, while 30.7% achieved partial substance abuse remission, with no significant difference between 1 and 2 treatment subgroups. Although obtained by an observational--open clinical study, with multiple limitations, our findings suggest that OLA may be useful as an adjunctive agent in reducing aggressive/hostile behaviour in heroin addicted individuals during maintenance substitution treatment. Otherwise, atypical anti-psychotic OLA seems to be unable to improve the outcome in terms of addictive behavior and relapse risk in the addicted patients not affected by overt psychotic disorders.

Throughout July and August 2002, at the same time as a campaign on driving safety, questionnaire and breath alcohol tests was submitted to young people outside the best-frequented discotheques of the Apulia area (Italy), to describe the role of alcohol intake in the evening-time amusement and assess its influence on their attention level. 217 young people, aged between 18-30 years, accepted to fill in a questionnaire, and undergo a 'breathalizer' test and attention test, before and after having spent time in the discotheque. The score difference between entry and exit on the attention test was -1.48 points and alcohol blood level increase up to 0.14 g/l. Established the lower reference limit for the attention test to be 21 points, it was found that 9.7% had an attention score lower than the reference limit, and 61.9% of this group had a blood alcohol level higher than 0.5 g/l. A linear regression model between the difference of blood alcohol level before and after the time spent in the discotheque and attention score on exiting from it, turned out to be statistically significant. It's necessary to go on with continuous police checks and informative awareness campaigns regarding alcohol use, driving and night amusement.

An increased traffic of hematopoietic progenitor cells (HPC) between bone marrow and peripheral organs is a peculiar feature of the allergic inflammation. It has been recently reported that the sublingual form of specific immunotherapy (SLIT) is capable of reducing such an increased HPC traffic. The House Dust Mite major antigen Der p1 has been proved to up-regulate the expression of the ICAM-1 and VCAM-1 endothelial addressins, supporting the view of an inflammatory cell recruiting at the site of allergen extract administration. In the present work we have investigated, by flow-cytometric techniques, the expression of the two major integrins CD11a (LFA-1) and CD49d (VLA-4) that are the homing receptor cognate for ICAM-1 and VCAM-1 on human cord blood CD34 hematopoietic progenitor and stem cells. Even if both the investigated molecules resulted detectable on CD34+ HPC surfaces, being the system redundant, the density of the cellular expression was significantly higher for CD49d (median value: 158) than CD11a (median value: 20.5), suggesting a preferential usage of the homing axis VLA-4/VCAM-1. Results consistency with outcomes of clinical trials that relate SLIT efficacy to allergen dosage is discussed.

An increased traffic of circulating CD34+ Hemopoietic Precursors Cells (HPC) is an important feature of systemic allergic inflammation. Bacteria and bacterial products are capable of stimulating the transcription of the maturational cytokines IL12 and IFNs through the activation of Toll-Like-Receptor and the subsequent nuclear translocation of the NF-kappaB factor. In this study the probiotics differentiation/maturational effect potential on CD34+ HPC has been investigated. Fourteen consecutive subjects, 9M and 5F, aged 6-48, with clinical symptoms of asthma and /or conjunctivitis, rhinitis, urticaria, atopic dermatitis, food allergy and irritable bowel syndrome were enrolled. Allergen-specific serum IgE were found in twelve patients. Flow-cytometric measurement of peripheral blood CD34dim/bright HPC values were assessed before and after 30 days of therapy, consisting in the oral administration of one sachet a day of ENDOLAC (UCB Pharma, Turin, Italy). Each sachet contained a mixture of Lactobacillus acidophilus, L. delbrueckii and Streptococcus thermophilus for a total of 1 x 10(9) live bacteria. Circulating CD34+ cell values significantly (p0.001) reduced after the treatment. ENDOLAC, thus, may improve the efficacy of the standard treatments of allergic diseases.

Results of an intervention of medical surveillance are reported. Medical surveillance was performed, from the Renam COR Basilicata-Puglia, in favour of 67 workers ex exposed to asbestos of the firm ex-Materit of Ferrandina that produced manufactured articles in cement-asbestos. The actual normative (art 29 DL 277/91) foresees that the competent plant physician informed the workers about the necessities to undergo to sanitary checks after the cessation of the activity, nevertheless the law doesn't establish that whom owes to make load of the medical surveillance. Cor, as Regional Reference Center, like in DPMC 308/2002, can guarantee definition of guide lines for uniform approach to regional level of all the figures involved in the management of the ex-exposed worker problem, in clinician and legal point of view and in the diffusion of the information (general practitioners, physicians operating in prevention services (SPESAL), competent plant physicians, hospital physicians and experts, labour unions).

Nuclear pleomorphism is a fundamental feature in evaluating the aggressiveness of ductal carcinoma in situ (DCIS) of the breast. In this study, pure DCIS and the in situ component (IS-comp) of invasive duct carcinoma (IDC) are compared in order to verify if these are two different entities or the same process observed at different times during its evolution. Five cases of pure DCIS and nine of IDC with extensive in situ component were selected. They were moderately and poorly differentiated. 30 nuclei for each DCIS, and 30 nuclei for both the in situ and invasive component of each IDC were studied; thus, a total of 720 nuclei were submitted to the SAM (Shape Analytical Morphometry) analysis, which enables a numerical expression not only of dimensions (area, perimeter, diameter) but also of nuclear contour irregularities and nuclear shape distortions. Univariate statistical comparisons were carried out between the nuclei of: (1) DCIS and in situ component of invasive duct carcinoma, (2) DCIS and the invasive component of infiltrating carcinoma and (3) between the in situ and invasive component of infiltrating carcinoma. Multivariate analysis was utilized to compare nuclei of DCIS with the in situ component of IDC. The in situ features of each tumor were also evaluated with the mitotic index (MI). Nuclei of pure DCIS resulted significantly larger (p0.001) and with a more regular shape (p0.001) than those of the in situ component of IDC. No differences were observed between the nuclei of the in situ and the invasive component of infiltrating carcinomas. Multivariate statistical analysis discriminated 77% of nuclei of in situ proliferation when both G2 and G3 tumors were considered, and 80% when only G3 tumors were considered. In conclusions morphological differences exist between pure DCIS and the in situ component of IDC, which may be an expression of their biological behavior; moreover, these morphological differences seem to have a better discriminating power within the same histological grade.

Histological diagnosis of laryngeal dysplasia is quite subjective. Since morphometry is highly reproducible, this method was applied to compare shape and size variations of the basal nuclei of the laryngeal epithelium in normal, laryngeal intraepithelium neoplasia (LIN) and invasive carcinoma to assess the reliability of light microscopic criteria used in grading dysplasia according to Friedman classification. Morphometrical analysis was carried out by Shape Analytical Morphometry (S.A.M) system. The logical architecture assumes that each irregular shape contains elements of two distinct logical domains: gross distortions that interest the contour and its local perturbations. These features were investigated separately by analytical procedures to acquire independent parameters both on the logical level and the numerical one. The nuclear area significantly increased from normal to carcinoma (p.001). The increasing of the nuclear area was evident also in LIN I. Nuclear distortions were present in LIN II and LIN III. The highest nuclear contour irregularities were found in LIN III. Multivariate analysis showed a difficulty in discriminating various grades of dysplasia, especially between LIN I and LIN II (31% of error). In conclusion, our results indicate that nuclear pleomorphism of the basal cells layer, using a unique evaluator, is an unsatisfying criterion to distinguish moderate dysplasia.
