Abstract

This preliminary study investigated Myrhaug's neuromuscular hypothesis by means of audiological and electromyographic assessment. After a clinical examination of the TMJ system, the activity of the tensor veli palatini muscles in the state of rest and during swallowing was investigated in 16 patients with the help of an EMG recording, and the tube-opening function was evaluated endoscopically. Results showed that 6.25% of the patients suffered from mild joint-dysfunction (group I), 81.25% from a moderate dysfunction (group II), and 12.5% from severe dysfunction (group III). Although five patients suffered from tinnitus and 11 from fullness in the ear, both the audio- and tympanogram performed on 15 patients displayed normal recordings. Only one patient suffered from moderate sensorineural hearing loss. In the EMG recording, 11 of the 16 subjects displayed normal EMG patterns in state of rest and during swallowing contraction. In two cases, abnormal EMG recordings were obtained due to incorrect insertion of the EMG needle. The study's results indicate that spasm in the masticatory muscles of TMJ patients neither seems to cause reflex spasm of the tensor palatini muscles nor to effect significant alterations in Eustachian tube function.

Abstract

This article presents an approach to identify abstract data types (ADT) and abstract state encapsulations (ASE, also called abstract objects) in source code. This approach groups together functions, types, and variables into ADT and ASE candidates according to the proportion of features they share. The set of features considered includes the context of these elements, the relationships to their environment, and informal information. A prototype tool has been implemented to support this approach. It has been applied to three C systems (each between 30-38 Kloc). The ADTs and ASEs identified by the approach are compared to those identified by software engineers who did not know the proposed approach. In a case study, this approach has been shown to identify, in most cases, more ADTs and ASEs than five published techniques applied on the same systems. This is important when trying to identify as many ADTs and ASEs as possible.

Abstract

A 40-year-old woman was evaluated for urinary incontinence, loss of bladder sensation and residual urine 12 months after radical abdominal hysterectomy and external pelvic radiation therapy for stage IIb cervical cancer. The patient had no history of abnormal lower urinary tract function before treatment. The urodynamic follow-up study at 12 months showed 80 ml of residual urine, low bladder compliance (detrusor pressure of 77 cmH2O at 200 ml filling) and an incompetent urethral closure mechanism. Cystoscopy showed a pale bladder mucosa with telangiectasia. Two years later the urodynamic findings were almost unchanged, and pudendal nerve terminal motor latency measured according to Snooks and Swash showed prolonged motor latency to the external urethral sphincter. Thus, this patient had combined stress overflow incontinence with pudendal neuropathy and fibrosis of the bladder wall.

Abstract

In the context of the authors' research on architectural feature recovery, abstract data type (ADT) and abstract state encapsulation (ASE, also called abstract object) have been identified as two of the smallest components which are useful to build a significant architectural overview of the system. The authors have named these the atomic components of an architecture. The paper compares five published techniques which extract ADT and ASE from source code. A prototype tool implementing each technique has been developed and applied to three medium-size systems written in C (each over 30 Kloc). The results from each approach are compared with the atomic components identified by hand by a group of software engineers. These people did not know the automatic techniques which were going to be applied to the systems.

Abstract

This study aimed to investigate the influence of anterior repair on pudendal nerve terminal motor latency. Neurourodynamic investigation (urethral profilometry, measurement of pudendal nerve terminal motor latency according to the method of Snooks and Swash) was performed before and 8 weeks after anterior repair (n = 14) and before and after additional endoscopic bladder neck suspension (n = 23). Stress incontinence and pudendal neuropathy were found preoperatively in all patients. In 17/37 (48%) of patients who were incontinent postoperatively the pudendal nerve latency was significantly prolonged.

Abstract

Previous studies have demonstrated that heparin-induced extracorporeal LDL precipitation is able to reduce total cholesterol, LDL, triglycerides and plasma fibrinogen at the same time and thus improve the hemorheologic pattern. A combination of H.E.L.P. and bezafibrate, which also has a lipid- and fibrinogen-lowering potency, was applied in patients suffering from cerebral multi-infarct dementia and disturbances of the hemorheologic situation. In view of observations regarding improved quality of life after H.E.L.P., a prospective, randomized trial was conducted to determine possible interactions between improved hemorheology and better quality of life. To obtain a low baseline of laboratory parameters, all the patients underwent a single H.E.L.P. session and were then (double-blind) stratified into 2 groups: group 1 (16 males, 5 females, age 67.9 +/- 6.8 years) received sustained-release bezafibrate 400 mg (Bezalip) per day, while group 2 served as controls, and comprised 14 males and 5 females (age 69.2 +/- 6.8 years) who received placebo for a period of 56 days. After H.E.L.P. a statistically significant reduction of the following parameters relevant to hemorheology was obtained: fibrinogen (p0.0001), whole blood viscosity (low shear rate p0.007 and high shear rate p0.005), plasma viscosity (p0.002) and red cell transit time (p0.0001). Abstract

High fibrinogen levels have been assessed in cerebrovascular disease with a direct relationship to blood rheology. Heparin-induced extracorporeal LDL precipitation (HELP) is a method that effectively reduces fibrinogen and lipoproteins at the same time, thus improving the hemorheologic pattern. We studied 45 patients with cerebral multiinfarct dementia. After computer-randomization 30 patients received a HELP treatment, while the remaining 15 patients served as controls. HELP produced an immediate and statistically significant reduction on parameters relevant to hemorheology, such as fibrinogen (P0.001), total cholesterol, low-density lipoprotein, triglycerides (P0.0001 each) and whole blood viscosity (at high and low shear rate) as well as plasma viscosity and red cell transit time (P0.01 each). The treated group showed improvement after HELP (P0.05 for each test) on the Mathew Neurological Scale, Mini Mental State Examination and Activities-of-Daily-Living Test and also relative to the untreated controls (P0.05 each). These results may be regarded as a basis for some reflections about new hemorheological interventions in cases of cerebral multiinfarct dementia.

10. [Association between fibrinogen and blood sedimentation rate in combined extracorporal and fibrinogen reducing drug therapy]

Abstract

Fibrinogen appears to play an important role in atherogenesis, whereby high levels of fibrinogen can lead to a disturbance of the haemorheologic pattern. To evaluate the association between fibrinogen and blood sedimentation rate (BSR) 40 patients suffering from cerebral multi-infarct disease underwent a single heparin-induced extracorporeal LDLfibrinogenprecipitation (HELP). Subsequently they were randomly assigned to two groups, either receiving sustained-release bezafibrate 400 mg daily (n = 21) or placebo (n = 19) over a period of eight weeks. HELP led to a statistically significant reduction of fibrinogen (p0.0001). At the end of the trial the difference between the bezafibrate group and the controls became significant (p0.05). The BSR strictly followed the course of fibrinogen, indicating a significant reduction after HELP (p0.0001 for both BSR values) and a difference between both groups at the end of the study (p0.05 and p0.03, respectively). The correlation between fibrinogen and BSR remained significant during the whole trial.

Abstract

Cerebrovascular disease (CVD) is associated with high fibrinogen levels and lipid fractions leading to an increase of both plasma and whole blood viscosity as well as raised aggregability of blood cells. One important goal in the treatment of cerebral multiinfarct dementia (MID) therefore should be to reduce fibrinogen and lipoproteins and thereby to improve the haemorheological state. The effect of heparin-induced extracorporeal LDL precipitation (H.E.L.P.), a method for safe and immediate reduction of parameters relevant to haemorheology, such as plasma fibrinogen and the lipoproteins, was investigated in 98 patients with MID. All the patients underwent two H.E.L.P. applications within 8 days. The impact of H.E.L.P. on CVD was studied by changes of laboratory data and by evaluation of clinical symptoms before and after treatment. Each H.E.L.P. session caused an immediate, safe and significant reduction of important rheological parameters such as fibrinogen (P < 0.001), whole blood viscosity at high and low shear rate, plasma viscosity and red cell transit time (P < 0.01 each). Also total cholesterol and low density lipoprotein (P < 0.0001 each), lipoprotein (a) (P < 0.003) and the triglycerides (P < 0.0001) had been reduced. The results in laboratory measurement were followed by a statistically significant improved neurologic recovery, represented in the values of the Mathew Scale, the Mini Mental State Examination and the Activities-of-Daily-Living-Test. These results can indicate the importance and influence of haemorheology on clinical symptoms in CVD.

Abstract

Objective: To study the long-term effects of vaginal dissection on the innervation of the striated urethral sphincter., Methods: Perineal nerve terminal motor latency was measured before and 8 weeks and 1 year after anterior colporrhaphy, colpoperineoplasty, and vaginal hysterectomy in 33 women, 19 of whom also underwent endoscopic suspension of the bladder neck., Results: In the entire series, the mean perineal nerve terminal motor latency was prolonged before surgery over that in normal continent women and further prolonged 8 weeks and 1 year postoperatively. In the subjects who underwent vaginal hysterectomy and anterior colporrhaphy, perineal motor latencies were not significantly prolonged at the 8-week postoperative follow-up and had almost returned to preoperative values 1 year later. However, in those who underwent additional endoscopic bladder neck suspension, perineal motor latencies were prolonged at both postoperative follow-up examinations. Perineal motor latencies were unchanged 1 year postoperatively in continent patients (N = 19) but were prolonged in incontinent patients (N = 14)., Conclusion: Vaginal dissection, especially during endoscopic bladder neck suspension, can worsen preexisting perineal neuropathy in patients with pelvic relaxation and stress incontinence.

Abstract

Previous studies have demonstrated that heparin-induced extracorporeal LDL precipitation is able to reduce total cholesterol, LDL, triglycerides and plasma fibrinogen at the same time and thus improve the hemorheologic pattern. A combination of H.E.L.P. and bezafibrate, which also has a lipid- and fibrinogen-lowering potency, was applied in patients suffering from cerebral multi-infarct dementia and disturbances of the hemorheologic situation. In view of observations regarding improved quality of life after H.E.L.P., a prospective, randomized trial was conducted to determine possible interactions between improved hemorheology and better quality of life. To obtain a low baseline of laboratory parameters, all the patients underwent a single H.E.L.P. session and were then (double-blind) stratified into 2 groups: group 1 (16 males, 5 females, age 67.9 +/- 6.8 years) received sustained-release bezafibrate 400 mg (Bezalip) per day, while group 2 served as controls, and comprised 14 males and 5 females (age 69.2 +/- 6.8 years) who received placebo for a period of 56 days. After H.E.L.P. a statistically significant reduction of the following parameters relevant to hemorheology was obtained: fibrinogen (p < 0.0001), whole blood viscosity (low shear rate p < 0.007 and high shear rate p < 0.005), plasma viscosity (p < 0.002) and red cell transit time (p < 0.0001). Abstract

High fibrinogen levels have been assessed in cerebrovascular disease with a direct relationship to blood rheology. Heparin-induced extracorporeal LDL precipitation (HELP) is a method that effectively reduces fibrinogen and lipoproteins at the same time, thus improving the hemorheologic pattern. We studied 45 patients with cerebral multiinfarct dementia. After computer-randomization 30 patients received a HELP treatment, while the remaining 15 patients served as controls. HELP produced an immediate and statistically significant reduction on parameters relevant to hemorheology, such as fibrinogen (P < 0.001), total cholesterol, low-density lipoprotein, triglycerides (P < 0.0001 each) and whole blood viscosity (at high and low shear rate) as well as plasma viscosity and red cell transit time (P < 0.01 each). The treated group showed improvement after HELP (P < 0.05 for each test) on the Mathew Neurological Scale, Mini Mental State Examination and Activities-of-Daily-Living Test and also relative to the untreated controls (P < 0.05 each). These results may be regarded as a basis for some reflections about new hemorheological interventions in cases of cerebral multiinfarct dementia.

Abstract

Background and Purpose: High fibrinogen levels have been assessed in cerebrovascular disease with a direct relation to both plasma and whole-blood viscosity, as well as cerebral blood flow. Heparin-induced extracorporeal low density lipoprotein precipitation (HELP) is a new method that safely and effectively reduces fibrinogen and plasma lipoproteins and improves blood flow properties., Methods: We studied 26 patients with acute embolic stroke and 22 with multi-infarct dementia. Each received two treatments with HELP within 8 days. Each patient had measurement of the important blood constituents and evaluation of changes in clinical signs and symptoms related to their cerebrovascular disease., Results: Each HELP treatment safely produced an immediate and significant reduction in rheological measures, including fibrinogen (P < .001), whole-blood viscosity at high and low shear rates, plasma viscosity, and red cell transit time (P < .01 each). Total cholesterol, low density lipoprotein (P < .0001 each), lipoprotein(a) (P < .003), and triglycerides (P < .0001) were also reduced. The treated group in both the acute stroke group and the multi-infarct group showed improvement relative to the untreated control subjects in Mathew scale, Mini-Mental State Examination, and activities of daily living test scores. These uniform improvements persisted at least 3 days past the second HELP treatment., Conclusions: These results support the hypothesis that the improved hemorheologic property of blood is an important factor in clinical recovery as well as basic neurological function.

Abstract

Fibrinogen appears to play an important role in atherogenesis, whereby high levels of fibrinogen can lead to a disturbance of the haemorheologic pattern. To evaluate the association between fibrinogen and blood sedimentation rate (BSR) 40 patients suffering from cerebral multi-infarct disease underwent a single heparin-induced extracorporeal LDL < fibrinogen > precipitation (HELP). Subsequently they were randomly assigned to two groups, either receiving sustained-release bezafibrate 400 mg daily (n = 21) or placebo (n = 19) over a period of eight weeks. HELP led to a statistically significant reduction of fibrinogen (p < 0.0001). At the end of the trial the difference between the bezafibrate group and the controls became significant (p < 0.05). The BSR strictly followed the course of fibrinogen, indicating a significant reduction after HELP (p < 0.0001 for both BSR values) and a difference between both groups at the end of the study (p < 0.05 and p < 0.03, respectively). The correlation between fibrinogen and BSR remained significant during the whole trial.

Abstract

Syntheses of steroid-linked N-[N'-(2-chloroethyl)-N'-nitrosocarbamoyl]-(CNC-) amino acid esters and -amides with potential antineoplastic activity are described. The esters are prepared by reaction of CNC-amino acids with steroids using N,N'-carbonyldiimidazole and N,N'-dicyclohexylcarbodiimide. The corresponding amides are prepared by reaction of 1-(CNC-amino acyloxy)-pyrrolidine-2,5-diones with 17 beta-amino-3-hydroxy-1,3,5(10)-estratriene or 17 beta-O-[4-(6-aminohexylamino)-1,4-dioxo-butyl]-estradiol. Estradiol-17 beta-hemisuccinate is esterified with N-(2-hydroxyethyl)-N'-(2-chloro-ethyl)-N'-nitrosourea (HECNU). Spectroscopic characteristics and relative binding affinities to steroid receptors are given.
