Abstract

The present work represents an analysis on the mechanical characterization of fiber reinforced hollow circular beams. This research is based on an analytical background along with some extension on it. Since exact solution is available for plane structures, such reference is considered as an initial comparison tool for a new approach for studying fiber reinforced beams based on a homogenization method. The implemented method introduces an easy application for designers who want to employ fiber reinforced beams for designing frame structures. The present approach is based on a 3D beam-frame element equivalent to a 3D shell model used as a reference for the actual composite configuration. This simple approach helps to consider the anisotropy inherited in such profiles. Sample frame structures are shown in order to demonstrate the advantages and disadvantages of the present methodology alongside the positive and negative aspects of employing fiber reinforced beams in place of the more common steel beams in civil applications.

Abstract

We assessed the influence of clinically significant mitral regurgitation (MR) on clinical-echocardiographic response and outcome in heart failure (HF) patients treated with a biventricular defibrillator (cardiac resynchronization therapy defibrillator [CRT-D]). METHODS AND RESULTS: A total of 659 HF patients underwent successful implantation of CRT-D and were enrolled in a multicenter prospective registry (median follow-up of 15 months). Following baseline echocardiographic evaluation, patients were stratified into two groups according to the severity of MR: 232 patients with more than mild MR (Group MR+: grade 2, 3, and 4 MR) versus 427 patients with mild (grade 1) or no functional MR (Group MR-). On 6- and 12-month echocardiographic evaluation, MR was seen to have improved in the vast majority of MR+ patients, while it remained unchanged in most MR- patients. On 12-month follow-up evaluation, a comparable response to CRT was observed in the two groups, in terms of the extent of left ventricular reverse remodeling and combined clinical and echocardiographic response. During long-term follow-up, event-free survival did not differ between MR+ and MR- patients, even when subpopulations of patients with ischemic heart disease and with dilated cardiomyopathy were analyzed separately. On multivariate analysis, the only independent predictor of death from any cause was the lack of β-blocker use. CONCLUSIONS: This observational analysis supports the use of CRT-D in HF patients with clinically significant MR; MR had no major influence on patient outcome.

Abstract

Results: Out of 664159 women there were 22938 patients with hyperlipidaemia (3.5%) and 9312 patients with breast cancer. Out of the hyperlipidaemia patients, 530 patients developed breast cancer (2.3%) compared with 8782 patients developing breast cancer without hyperlipidaemia (1.4%). A logistic regression model accounting for time from first presentation to development of breast cancer showed that the presence of hyperlipidaemia increases the outcome of breast cancer by 1.64 times (95% C.I. 1.50-1.79). Conclusions: Whilst we appreciate numerous limitations of our methods, coupled with the main findings of the recent basic science research, our analysis further augments the case for the role of cholesterol in the development of breast cancer. Our data from a large clinical relevant sample further strengthens the argument to prospectively trial statins in the management of breast cancer.

20. Implementing innovation in Primary Care: an Italian experience

Abstract

Background Since 2000, in Italy the General Practitioner (GP) Contract with the National Health Service has been changing towards increased participation of GPs in healthcare management to improve professional collaboration with other practitioners such as nurses and social workers. In Emilia Romagna (ER), a region in northern Italy, the Primary Care System has been reorganized with the institution of “Primary Care Units” (NCP) as main centres where GPs, nurses and other practitioners work together to deliver comprehensive healthcare outside the hospital. The aim of this study is to analyse number, function and organization of NCPs in ER two years after the introduction of this new model in 2006. Methods A web-based questionnaire was administered to Primary Care coordinators in the 11 Local Health Authorities. The questionnaire explored the NCPs development in terms of GP participation, clinical organization, integration with other healthcare practitioners and chronic disease management. Results At 30 June 2008 there were 214 NCPs and 94 % of total GPs (3013 on 3215) joined them; each NCP is composed of an average number of 15 GPs. In 159 NCPs the GPs meet periodically for training courses and strategic planning. Only in 62 NCPs are clinical activities performed in a unique place, while other GPs continue to see patients in their own offices. Only two NCPs provide nurse care. Chronic disease (Diabetes-COPD-Heart Failure) management is implemented in several NCPs with different strategies. Conclusions The case of NCPs introduction in ER is certainly a leading model in Italy for primary care management but more efforts are necessary to develop further in this direction. From our analysis, we found that most NCPs are at this moment just a formal aggregation and a low percentage of them are engaged in integrated clinical activities. New strategies for implementing this model are requested.

21. Impatto di diversi modelli organizzativi sulla qualità assistenziale in ambito di cure primarie nell´AUSL di Bologna

Abstract

OBIETTIVI: L’Accordo Integrativo Regionale 2006 per la disciplina dei rapporti con i medici di medicina generale (MMG) della Regione Emilia Romagna, afferma che gli obiettivi di innovazione nella organizzazione e qualificazione dell’assistenza ai cittadini trovano concreta applicazione nel sistema delle cure primarie e promuove l’evoluzione dei nuclei di cure primarie (NCP). Obiettivo dello studio è la valutazione, attraverso l’utilizzo di indicatori, della qualità assistenziale in relazione a diverse modalità organizzative nell’ ambito delle cure primarie nella Azienda Usl (AUSL) di Bologna. Sono previsti due livelli nei quali sviluppare l’analisi: la partecipazione del MMG a Medicine di Gruppo (MdG) ed a nuove modalità organizzative, i NCP. MATERIALI: Il periodo di analisi è stato l’anno 2008 e le basi informative sono state: archivio dei MMG, banca dati (BD) assistenza farmaceutica territoriale, BD schede di dimissione ospedaliera, BD specialistica ambulatoriale, la popolazione assistita in carico ai MMG e il flusso del “progetto diabete”. Gli indicatori, selezionati sulla base di un’analisi della letteratura, riguardano l’appropriatezza del trattamento delle patologie croniche a più alta prevalenza, i ricoveri per condizioni sensibili di trattamento ambulatoriale, l’accesso al Pronto Soccorso (PS) con codici verdi o bianchi e la vaccinazione antinfluenzale nei pazienti al di sopra dei 65 anni. Sono stati selezionati i MMG che avessero almeno 300 pazienti e considerati come appartenenti alla MdG solo quelli che vi aderivano da almeno 3 anni. L’analisi è stata effettuata utilizzando sia una regressione semplice che un modello multilevel nel quale sono stati considerati anche il Distretto di appartenenza, l’età ed il sesso del MMG. RIASSUNTO: Nell’AUSL di Bologna vi sono 628 MMG suddivisi in 6 Distretti ed in 41 NCP. I MMG con almeno 300 assistiti sono 354 (66% uomini), con un totale di 429.958 assistiti ed il 52,8% appartiene ad una MdG da almeno 3 anni. Abbiamo rilevato che all’aumentare degli assistiti aumenta anche l’aderenza ad una MdG. Dall’analisi effettuata risulta che l’appartenenza ad una MdG comporta un aumento della presa in carico di diabetici tipo II e della percentuale di pazienti sopra i 65 anni vaccinati, e una diminuzione di accessi al PS per codici verdi e bianchi. CONCLUSIONI: L’analisi effettuata ad oggi sembrerebbe dimostrare, che l’appartenenza ad una MdG influenzi positivamente la presa in carico del paziente, tuttavia l’analisi è ancora in fase di elaborazione e molti degli aspetti più cruciali della qualità assistenziale non sono ancora stati esaminati, inoltre deve ancora essere esplorato l’impatto del NCP sulla qualità assistenziale.

Abstract

Esiste un unanime consenso che il taglio cesareo (TC) nei paesi industrializzati sia una procedura soggetta a sovra-utilizzazione: la proporzione di parti cesarei ha subito infatti negli ultimi anni un costante incremento in tutti i paesi occidentali, compresa l’Italia, con un aumento dal 1999 al 2004 di 5 punti percentuali, raggiungendo uno tra i valori più elevati al mondo. La proporzione di parti cesarei sia a livello nazionale che a livello regionale continua ad essere in costante crescita, presentando grandi variabilità interregionali. Il parto cesareo risulta essere un argomento di grande interesse per la ricerca clinica e per i servizi sanitari, poiché, nonostante sia indiscutibile che in determinate circostanze cliniche rappresenti un intervento necessario per la madre e/o il neonato, è utile approfondire la grande variabilità osservata nell’utilizzo di tale procedura e valutare i rischi e le possibili complicanze che da esso possono derivare.

24. Mortalità infantile e neonatale

Abstract

Il tasso di mortalità infantile, definito come il numero di bambini deceduti nel primo anno di vita su 1.000 nati vivi nello stesso anno, è considerato un’importante e sensibile misura di salute della popolazione. Molti fattori biologici, sociali ed economici sono associati al rischio di mortalità infantile. Tra i fattori biologici più importanti è compresa l’età materna, l’ordine di nascita, l’intervallo tra i parti successivi, il numero delle nascite, la presentazione fetale al momento del parto e la storia ostetrica della madre. Tra i fattori sociali ed economici si includono la legittimità, le condizioni abitative e il numero degli abitanti della casa, la nutrizione e l’educazione della madre, l’abitudine al fumo durante la gravidanza, l’occupazione del padre e il reddito. Molteplici fattori, tuttavia, fra cui l’evoluzione tecnologica delle modalità di assistenza al parto ma anche le modalità stesse con cui vengono raccolti i dati, possono alterare la confrontabilità del tasso di mortalità infantile come indicatore di salute. La tendenza mondiale è quella di una generale riduzione della mortalità infantile anche se alcuni paesi non industrializzati, soprattutto l’Africa Sub- Sahariana e l’Asia centrale, mostrano valori ancora superiori a 100 bambini morti nel primo anno di vita per 1.000 nati vivi. In Italia, il tasso di mortalità infantile è sceso al di sotto del valore raccomandato dal PSN 1998-2000 dell’8 per 1.000 nati vivi, risultando uno dei più bassi al mondo. Il tasso di mortalità neonatale è invece maggiormente legato a fattori biologici quali la salute della madre, la presenza di anomalie congenite e l’evoluzione del parto, oltre che a fattori legati all’assistenza al parto. Considerando i trienni 2001-2003 e 2002-2004, la mortalità infantile media nazionale si è ulteriormente ridotta di 9,8 punti percentuali, passando da 4,1 a 3,7 per 1.000 nati vivi. È interessante notare come, nei periodi considerati, tutte le componenti della mortalità infantile (neonatale e postneonatale) si siano ridotte, ed in particolar modo la mortalità neonatale. I determinanti della mortalità infantile e neonatale in Italia sono stati esplorati già dalla fine degli anni ’80 (1, 3, 10, 11) con studi ecologici e analitici. I risultati di questi studi suggeriscono come, data la riduzione eclatante dell’indicatore negli ultimi 15 anni, rimanga da rivalutare negli anni più recenti quali determinanti siano ancora rilevanti; occorre perciò progettare nuovi studi analitici per indagare le differenze tuttora presenti fra Nord e Sud. Solo così si potranno fornire agli amministratori, gestori ed operatori sanitari le indicazioni necessarie per orientare al meglio le politiche sanitarie.

25. Il parto cesareo in Emilia-Romagna: riproducibilità delle informazioni ottenute attraverso SDO e CeDAP

Abstract

La procedura di taglio cesareo (TC) oggi può essere considerata potenzialmente sovrautilizzata. Coerentemente con le finalità indicate dal Progetto-Obiettivo Nazionale Materno Infantile (D.P.C.M. 24 aprile 2000) e con il Piano Sanitario Nazionale 2006-2008 (D.P.R. 7 aprile 2006), il nuovo disegno di legge sulle “Norme per la tutela dei diritti della partoriente, la promozione del parto fisiologico e la salvaguardia della salute del neonato” pone fra gli obiettivi più importanti il monitoraggio e la riduzione della procedura di taglio cesareo. Tale monitoraggio si sta rivelando necessario poiché la proporzione di TC è in continuo aumento e la sua utilizzazione è sempre più frequentemente associata a determinanti che vanno al di là di quelli strettamente clinici (scelta materna). Ad oggi la principale fonte informativa corrente che individua e raccoglie le informazioni relative al parto è rappresentata dalla Scheda di Dimissione Ospedaliera (SDO) all’interno della quale l’evento nascita e il taglio cesareo possono essere selezionati utilizzando sia i DRG, sia i codici ICD-9-CM di diagnosi e procedure. Un’altra importante fonte informativa per lo studio delle tematiche materno-infantili è rappresentata dai Certificati di Assistenza al Parto (CeDAP). Anche nell’ambito dei CeDAP è possibile identificare il parto cesareo ed il pregresso cesareo. I risultati di questo lavoro dimostrano che, diversamente da quanto verificato in altri studi, utilizzando diverse fonti informative correnti e diversi indicatori per l’individuazione delle proporzioni di TC nelle strutture ospedaliere di una regione italiana, non si ottengono classificazioni per ranghi significativamente differenti. L’indicatore ottenuto dalla banca dati SDO utilizzando i “DRG 370-375” è sicuramente quello di più facile computo attraverso questa fonte informativa. I risultati ottenuti dall’utilizzo dei CeDAP sono comunque consistenti. Quando però è necessario distinguere i TC primari da quelli con pregresso TC, la modalità più corretta sembra essere quella da banca dati SDO attraverso il codice di diagnosi secondaria nel ricovero indice (654.2) come già precedentemente dimostrato in casistiche americane (5). Il risultato ottenuto attraverso la banca dati CeDAP è distorto dall’elevato numero di dati mancanti.

26. Photochemical permutation of thiazoles, isothiazoles and other azoles.

Abstract

Thiazoles and isothiazoles are privileged motifs in drug and agrochemical discovery. 1,2 The synthesis of these derivatives is generally approached, designed and developed on a case-by-case basis. Sometimes, the lack of robust synthetic methods to a given target can pose significant difficulties or even thwart the preparation of specific derivatives for further study. 3,4 Here, we report a conceptually different approach whereby photochemical irradiation can be used to alter the structure of thiazoles and isothiazoles in a selective and predictable manner. Upon photoexcitation, these derivatives populate their π,π* singlet states that undergo a series of structural rearrangements leading to an overall permutation of the cyclic system and its substituents. This means that once the initial heteroaromatic scaffold has been prepared, it can then function as an entry point to access other molecules by selective structural permutation. This approach operates under mild photochemical conditions which tolerate complex scaffolds and chemically distinct functionalities. Preliminary findings also indicate the potential for extending this method to other azole systems, including benzo[d]isothiazole, indazole, pyrazole and isoxazole. This strategy establishes photochemical permutation as a powerful and convenient method for the preparation of complex and difficult-to-access derivatives from more available structural isomers., (© 2024. 27. Catalytic Asymmetric Conjugate Reduction.

Abstract

Enantioselective reduction reactions are privileged transformations for the construction of trisubstituted stereogenic centers. While these include established synthetic strategies, such as asymmetric hydrogenation, methods based on the enantioselective addition of hydridic reagents to electrophilic prochiral substrates have also gained importance. In this context, the asymmetric conjugate reduction (ACR) of α,β-unsaturated compounds has become a convenient approach for the synthesis of chiral compounds with trisubstituted stereocenters in α-, β-, or γ-position to electron-withdrawing functional groups. Because such activating groups are diverse and amenable of further derivatizations, ACRs provide a general and powerful synthetic entry towards a variety of valuable chiral building blocks. Abstract

Background: Algorithms for automatic pacing output adjustment have been implemented in pacemakers and implantable defibrillators (ICD) and recently in cardiac resynchronization therapy defibrillators (CRT-D). We assessed the impact and effectiveness of these automatic features., Method and Results: We prospectively enrolled patients successfully implanted with the following Medtronic CRT-Ds: Concerto [with automatic left ventricular (LV) output management algorithm], Consulta [automatic management of atrial, right ventricular (RV) and LV voltage], and Sentry (only manual voltage adjustments). Patients with complete device data available for at least 12 months were included in the analysis. We analysed data from 739 patients (360 Sentry, 335 Concerto, 44 Consulta). During the first 6 months, the LV pacing amplitude underwent more frequent adjustments in Concerto (63%, P< 0.001) and Consulta (64%, P= 0.047) patients than in Sentry (48%). Similarly, RV and atrial amplitude at 6 months differed from the pre-discharge value more frequently in Consulta (61 and 50%, respectively) than in Sentry patients (33 and 28%, both P< 0.01). The LV pulse amplitude for Concerto and the voltages in the three chambers of Consulta were significantly lower than the corresponding values programmed in Sentry at 6 and 12 months. The proportion of CRT-D interrogations involving manual reprogramming was 97 ± 8% for Sentry, 79 ± 20% for Concerto, and 56 ± 16% for Consulta (all P< 0.001)., Conclusions: Algorithms for the automatic management of the pacing output reduced pacing output in comparison with the standard manual management approach, with potential optimization of battery longevity. Moreover, they reduced the need to manually reprogram CRT-Ds, suggesting the possibility to simplify CRT-D management and facilitate remote monitoring.

Abstract

Aims: Uncontrolled ventricular rate (VR) during atrial fibrillation (AF) may cause clinical deterioration in heart failure (HF) patients who need continuous biventricular pacing to achieve cardiac resynchronization therapy (CRT). We aimed at evaluating the association between AF, uncontrolled VR, and sub-optimal CRT, defined as low biventricular pacing percentage (BIVP%)., Methods and Results: All 1404 patients had HF, New York Heart Association (NYHA) ≥II, left ventricular ejection fraction (LVEF) ≤35%, and QRS ≥120 ms, and received an implantable CRT defibrillator (CRT-D). Occurrence of AF, VR during AF and lifetime BIVP% were estimated from device data. Ventricular rate during AF was defined as uncontrolled in patients with mean VR>80 bpm and maximum VR>110 bpm. Over a median follow-up of 18 months, AF was detected in 443 of 1404 patients (32%). In this sub-group of AF patients, VR during AF was uncontrolled in 150 of 443 patients (34%). Multivariate Cox regression analysis showed that age [hazard ratio (HR) = 1.03, 95% confidence interval (CI) = 1.00-1.06, P= 0.028], and uncontrolled VR [HR = 1.69 (CI = 1.01-2.83), P= 0.046] were the only independent predictors of clinical outcome, assessed by HF hospitalizations and death. The median lifetime BIVP% was 95% (25-75 percentile range 91-99%). Biventricular pacing percentage was significantly and inversely correlated to VR, decreasing by 7% for each 10 bpm increase in VR. Sub-optimal CRT, defined as BIVP% <95%, was predicted by the occurrence of persistent or permanent AF [odds ratio (OR) = 3.77, CI = 2.44-5.82, P< 0.001], and uncontrolled VR [OR = 2.25, CI = 1.35-3.73, P= 0.002]., Conclusion: Uncontrolled VR occurs in one-third of CRT-D patients, who experience AF, and is associated with HF hospitalizations and death and with sub-optimal CRT (lifetime BIVP%<95%).

Abstract

Background: Increased plasma levels of amino-terminal fraction of brain natriuretic peptide (NT-proBNP) and alterations of diastolic filling as described by Doppler transmitral flow pattern are well-known markers of decompensated heart failure (HF). Recently, some implantable defibrillators have allowed monitoring of intrathoracic impedance, which is related to lung water content, potentially indicating HF deterioration. The aim of this study was to assess the correlation between intrathoracic impedance and NT-proBNP and echo-Doppler transmitral flow indexes., Methods: Data were collected from 111 HF patients, in six Italian centers. All patients were on optimal medical therapy. Device diagnostics, echographic data, NT-proBNP determination, and clinical status as assessed by the Heart Failure Score (HFS) were registered at baseline, at bimonthly visits, and at unscheduled examinations due to HF decompensation or device alerts., Results: Over a median follow-up of 413 days, 955 examinations were performed. Intrathoracic impedance was significantly correlated with NT-proBNP (P = 0.013) and with mitral E-wave deceleration time (DtE) (P = 0.017), but not with HFS. At the time of confirmed alert events, NT-proBNP was significantly higher than during confirmed nonalert event examinations; DtE did not differ, whereas impedance was significantly lower., Conclusion: A decrease in intrathoracic impedance is inversely correlated with NT-proBNP and directly correlated with DtE. Intrathoracic impedance monitoring therefore has the physiologic basis for being a useful tool to identify early HF decompensation., (©2010, The Authors. Abstract

Background: Some implantable cardioverter defibrillators (ICD) are able to monitor intrathoracic impedance to detect pulmonary fluid overload. This is achieved by measuring impedance between the ICD case and the right ventricular (RV) lead. We hypothesized that the measured impedance would rise with improvement in left ventricular (LV) volumes during cardiac resynchronization therapy (CRT), and that such impedance changes would be more apparent when measured with an alternative pacing vector., Methods: We analyzed echocardiographic and impedance data from heart failure patients implanted with a CRT-ICD capable of intrathoracic impedance measurement for fluid accumulation diagnosis, and LV pacing impedance recording for lead integrity monitoring., Results: In 127 out of 170 patients that received de novo CRT implantation, the LV end-systolic volume (LVESV) decreased at 6-month follow-up (LVESV at 6 month-LVESV at baseline <0: group A). For the remaining 43 patients (group B) the change was > or = 0. Despite comparable values at baseline (P = 0.262), the impedances of groups A and B gradually diverged soon after the implant, resulting in significant difference between the two groups at the 6-month visit (P = 0.001). The changes in LV dimensions produced larger differences between groups in the impedance measured between the LV and the RV leads (P < 0.001). The regression analysis demonstrated an inverse correlation between paired changes of volume and intrathoracic impedance. Higher correlation coefficient was obtained using the LV-to-RV measurement vector (r =-0.635, P < 0.001)., Conclusions: The changes in ICD-measured impedance seem associated with the LV volume changes induced by CRT. Specifically, the LV-to-RV impedance estimations seem to better correlate with paired changes of ventricular volumes.

Abstract

Purpose: Some implantable cardioverter-defibrillators (ICDs) are now able to monitor intrathoracic impedance. The aim of the study was to describe the use of such monitoring in clinical practice and to evaluate the clinical impact of the fluid accumulation alert feature of these ICDs., Methods and Results: Five hundred thirty-two heart failure (HF) patients implanted with these ICDs were followed up for 11 +/- 7 months. A clinical event (CE) was deemed to have occurred if it resulted in hospitalization or milder manifestations of HF deterioration. Three hundred sixty-two acute decreases in intrathoracic impedance (Z events) occurred in 230 patients. Of these episodes, 171 (47%) were associated with a CE within 2 weeks of the Z event. In another 71 (20%) Z events, drug therapy was adjusted despite the absence of overt signs of clinical deterioration. The rate of unexplained Z events was 0.25 per patient-year and 25 hospitalizations were not associated with Z events. The audible alert was disabled in a group of 102 patients (OFF group). HF hospitalizations occurred in 29 (7%) patients in the ON group and 20 (20%, P < 0.001) patients in the OFF group. The rate of combined cardiac death and HF hospitalization was lower in patients with Alert ON (log-rank test, P = 0.007)., Conclusions: The ICD reliably detected CE and yielded low rates of unexplained and undetected events. The alert capability seemed to reduce the number of HF hospitalizations by allowing timely detection and therapeutic intervention.

Abstract

Purpose: To determine the association between device-determined diagnostic indices, including intrathoracic impedance, and heart failure (HF) hospitalization., Methods: Clinical and device diagnostic data of 558 HF patients indicated for CRT-D therapy (In Sync Sentry, Medtronic Inc.) were prospectively collected from 34 centers. Device-recorded intrathoracic impedance fluid index threshold crossing event (TCE), mean activity counts, tachyarrhythmia events, night heart rate (NHR) and heart rate variability (HRV) were compared within patients with vs. without documented HF hospitalization., Results: Mean follow-up was 326 +/- 216 days. Patients hospitalized for HF had significantly higher rates of TCE, a higher percentage of days with the thoracic impedance fluid index above the programmed threshold, a higher percentage of days with low activity, with low HRV or with high NHR. Multivariate analysis showed that TCE resulted in a 36% increased probability of HF hospitalization. Both TCE duration and patient activity were also significantly associated with hospitalization. Kaplan Meier analysis indicated that patients with more TCE events were significantly more likely to be hospitalized (log rank test, p = 0.005)., Conclusions: Decreased intrathoracic impedance, low patient activity and low HRV were all independently associated with increased risk for HF hospitalization in HF patients treated with resynchronization therapy. Device-derived diagnostic data may provide valuable and reliable indices for the prognostic stratification of HF patients.

Abstract

Many fragmental classification of coronary artery anomalies (CAAs) exist, but a simple practical angiographic classification for angiographers has been never proposed. This study is aimed to suggest a simplified angiographic classification of congenital CAAs based on just a few univocal common angiographic patterns. The authors reviewed 5100 coronary angiographies in order to select CAAs patients and identify simple common angiographic features. Sixty-two patients (1.21%, female/male 20/42, mean age 65.3 +/- 10.6 years) had CAA on coronary angiography. The authors identified seven classes for seven angiographic patterns: I--hypoplasia/atresia, II--hyperdominance, III--fistula, IV--originating from wrong sinus, V--originating from other arteries, VI--splitting, and VII--tunnelling. A, P, B, R, L, PA, AO refer to anterior, posterior or passage between the aorta and pulmonary artery and to right, left, pulmonary artery and aorta. Three blind observers were be able to categorize all the CAAs according to this classification with no inter-observer differences: 3.2% were classified as class I, 8.1% as class II, 3.2% as class III, 24.2% as class IV, 22.5% as class V, 29% as class VI, and finally 9.7% as class VII. Eleven patients (17.7%) had 'A' passage, 10 (16.1%) 'P' passage and 9 (14.5%) 'B' passage. Twelve patients (19.5%) had anomalous origin from the right sinus of Valsalva, 2 (3.2%) from the left. This simplified classification was applicable to all most significant CAAs and in the authors' view it may make for a more rapid and univocal CAA angiographic description.
