Abstract

Introduction With the aim of implementing common policies of health promotion to counter the use of tobacco cigarettes and new products and e-cig at university, the UNIVERSITY SMOKE-FREE network was set up in Italy. Objective The main objectives of the network are: to update and uniform the university regulations; to investigate exposure of students and staff to active and passive smoking by means of a multicentre survey; to implement initiatives for helping students to stop smoking; to produce information and teaching materials. Material and Methods With the involvement of the university governments, a network was set up comprising 14 universities: Milan La Statale (UNIMI), Milan-Bicocca, Milan-Humanitas, Brescia, Insubria, Pavia, Bergamo, Rome Sapienza, Parma, Padua, Florence, Urbino, the Polytechnic University of Marche, and Palermo. In order to carry out the survey, a questionnaire was developed and validated1, investigating active cigarette smoking, use of new products, exposure to passive smoking, knowledge of the health effects of smoking, and knowledge of, and agreement on regulations. Results All network universities have started to review and update their smoke-free regulations. The survey protocol, developed at UNIMI, was shared among the universities for approval by their respective Ethics Committees. The survey was completed in 2021 by the University of Milan La Statale, and in May-July 2023 by the Universities of Brescia and Urbino, while it will be conducted in the other universities in 2024. To date, more than 10000 students participated in the survey (11-25% of students). First results show differences among universities and scientific and humanistic faculties, in terms of active and passive smoking, and awareness of risks of smoking2. Conclusions The establishment of the UNIVERSITY SMOKE-FREE network is an important step to counter smoking at university. 4. Scientific research and environmental education: the participatory approach of the MAPS-MI projects in schools of Milan, Italy [Ricerca scientifica ed educazione ambientale: l'approccio partecipativo dei progetti MAPS-MI nelle scuole di Milano]

Abstract

The accumulated scientific evidence regarding the effects of air pollution on health calls for urgent and effective action to protect the most vulnerable part of the population, such as children and the elderly. In this context, the participatory approach represents a viable option to empower citizens and increase their level of awareness and participation. The involvement of civil society is a potential ally in tackling the still open research challenges. The MAPS-MI 2018-2019 Mapping air pollution in the catchment area of an elementary school of Milan and MAPS-MI 2022-2023 Mobility, environment and participation in the schools of Milan projects are here presented.The main targets of the projects are primary and middle school children, their parents and teachers, with some activities extended to the entire citizenship. Starting from the survey on parents' habits and opinions about mobility and air pollution, to the air quality and personal exposure assessment, and the field-testing and development of an environmental education module, the MAPS-MI approach aims to increase knowledge and awareness about air pollution and to involve stakeholders in experimenting practices towards change. The first results suggest that adopting a participatory approach in the fields of exposure science and environmental epidemiology is a winning choice in terms of quality research, participation and community impact.

5. Minding the gap between cortisol levels measured with second-generation assays and current diagnostic thresholds for the diagnosis of adrenal insufficiency: a single-center experience

Abstract

Purpose: The current cut-offs for the diagnosis of adrenal insufficiency (AI) have been established using outdated immunoassays. We compared the cortisol concentrations measured with Roche Cortisol I (R1), the newly available Roche Cortisol II (R2), and liquid chromatography tandem mass spectrometry (LC-MS/MS), the gold standard procedure to measure steroids in patients undergoing the corticotropin (ACTH) test. Methods: We enrolled 30 patients (age 47 ± 21 years) referred to undergo the ACTH test (1 or 250 μg). Cortisol was measured at 0, 30, and 60 min after stimulation with R1, R2, and LC-MS/MS. AI was diagnosed for R1-stimulated peak cortisol concentrations < 500 nmol/L. Results: Mean cortisol concentrations measured with R2 and LC-MS/MS were comparable, while mean cortisol concentrations measured by R1 were higher than those of both R2 and LC-MS/MS (respectively, basal 411 ± 177, 287 ± 119, and 295 ± 119 nmol/L; at 30 min, 704 ± 204, 480 ± 132, and 500 ± 132 nmol/L; at 60 min, 737 ± 301, 502 ± 196, and 519 ± 201 nmol/L, p≤ 0.01 for R1 vs. both R2 and LC-MS/MS at each point). Considering the 500 nmol/L cortisol peak cut-off, AI was diagnosed in 5/30 patients using R1 and in 12/30 using R2 (+ 140%). Based on the correlation between R1 and R2, the threshold of 500 nmol/L became 351 nmol/L (12.7 μg/dL) when cortisol was measured with R2, and 368 nmol/L (13.3 μg/dL) with LC-MS/MS. Conclusions: The use of more specific cortisol assays results in lower cortisol concentrations. This could lead to misdiagnosis and overtreatment when assessing AI with the ACTH test if a different cut-off for cortisol peak is not adopted.

Abstract

Background and aimShift work, especially including night shifts, has been found associated with several diseases, including obesity, diabetes, cancers, and cardiovascular, mental, gastrointestinal and sleep disorders. Metabolomics (an omics-based methodology) may shed light on early biological alterations underlying these associations. We thus aimed to evaluate the effect of night shift work (NSW) on serum metabolites in a sample of hospital female nurses.MethodsWe recruited 46 nurses currently working in NSW in Milan (Italy), matched to 51 colleagues not employed in night shifts. Participants filled in a questionnaire on demographics, lifestyle habits, personal and family health history and work, and donated a blood sample. The metabolome was evaluated through a validated targeted approach measuring 188 metabolites. Only metabolites with at least 50% observations above the detection limit were considered, after standardization and log-transformation. Associations between each metabolite and NSW were assessed applying Tobit regression models and Random Forest, a machine-learning algorithm.ResultsWhen comparing current vs. never night shifters, we observed lower levels of 21 glycerophospholipids and 6 sphingolipids, and higher levels of serotonin (+171.0%, 95%CI: 49.1–392.7), aspartic acid (+155.8%, 95%CI: 40.8–364.7), and taurine (+182.1%, 95%CI: 67.6–374.9). The latter was higher in former vs. never night shifters too (+208.8%, 95%CI: 69.2–463.3). Tobit regression comparing ever (i.e., current + former) and never night shifters returned similar results. Years worked in night shifts did not seem to affect metabolite levels. The Random-Forest algorithm confirmed taurine and aspartic acid among the most important variables in discriminating current vs. never night shifters.ConclusionsThis study, although based on a small sample size, shows altered levels of some metabolites in night shift workers. If confirmed, our results may shed light on early biological alterations that might be related to adverse health effects of NSW.

15. The first steps of industrial hygiene and occupational toxicology at the Clinica del Lavoro di Milano under the guidance of Luigi Devoto

Introduction. The Clinica del Lavoro, the oldest clinic for occupational diseases in the world, was inaugurated in Milan on 10 March 1910; Luigi Devoto,its first director, was in charge until 1935. The purpose of this work is to review the activities of industrial hygiene and toxicology carried out at the work Clinica under the guidance of Devoto. Methods. Documents published by the Istituti Clinici di Perfezionamento, a group of clinics of which the Clinica del Lavoro was part, record the birth and organization of this structure and the presence of a laboratory of chemistry; documents by Devoto and other authors were also retrieved to extrapolate specific information on activities of industrial hygiene and toxicology. Results. The Clinica del Lavoro, at the time of its inauguration, included four laboratories: of chemistry, clinical physics, histopathology and bacteriology. The chemistry lab was located on the first floor above the entrance and was composed of 6 well-lit rooms, modernly equipped with work benches that could accommodate 12 people. In Devoto's view, the chemistry laboratory, supported by that of clinical physics, had to assess the toxicological properties of chemicals commonly found in the workplace and to reveal the mechanisms of induction of damage to humans. In the first 30 years of activity, the Clinica investigated various pathologies deriving from exposure to chemical agents, including saturnism, or lead intoxication, mercurialism, phosphorism, benzolism, sulfocarbonism, dust diseases. Several assays were developed and applied to measure toxicants in different biological and environmental means as evidenced by scientific publications starting from 1920. Conclusion. In Devoto’s view, industrial hygiene and toxicology were essential tools for the research and prevention of occupational diseases since the first years of activity of the Clinica del Lavoro.

Abstract

Objectives: Describe the first years of the laboratory of industrial hygiene of Milan through three figures who played a leading role: Enrico Carlo Vigliani, Nicola Zurlo and Gianmario Cavagna. Methods: Scientific literature of the period 1948-1970 was investigated, also interviewing first-hand witnesses of that period. Results: Enrico Vigliani was the first European scholar to understand the importance of a laboratory of industrial hygiene within his institution. Thanks to the support of private (Montecatini) and public (INAIL) institutions he succeeded in creating a laboratory in 1948. Nicola Zurlo, who directed this structure in the first thirty years, conducted innovative studies on chronic mercury intoxication, lead intoxication and silicosis, designing and creating instruments for capturing and analyzing atmospheric dust and protection devices. He conducted analysis of the health effects of organophosphorus insecticides and started to study the air pollution. Zurlo also provided an epistemological and methodological content to the discipline. Gianmario Cavagna, one of the first Italian toxicologists, contributed to the discovery of the origin of fevers caused by the inhalation of metal fumes and to the studies on the pathogenesis of byssinosis, hypothesizing a role of bacterial endotoxins in the genesis of this disease. Conclusions: The contributions provided by these three protagonists to industrial hygiene and occupational toxicology were relevant and made in those years the Clinica del Lavoro of Milan as a landmark, not only in Italy but also abroad.

30. Benzene and leukemia: from scientific evidence to regulations. A historical example

Abstract

BACKGROUND: Benzene is a highly flammable, highly volatile liquid aromatic hydrocarbon. It has been used in many industrial processes as a solvent or a starting material. At the beginning of the twentieth century, it was very widely used in the workplace, especially in printing and in the shoe manufacturing and rubber industries. Although benzene was first recognized to cause aplastic anemia, its association with leukemia has been investigated only since the 1930s. In 1963, Italy was one of the first countries in the world to adopt a law to ban benzene as a solvent in work activities. OBJECTIVES: This study analyzed the contribution of the Clinica del Lavoro in Milan, Italy, to studies of the relationship between exposure to benzene and leukemia. METHODS: Scientific literature and historical sources on benzene and leukemia in the twentieth century were reviewed, and interviews with a first-hand witness of that period were conducted. RESULTS: By 1928, several scholars had reported anecdotal cases of leukemia among workers exposed to benzene. Enrico Vigliani was the first to collect all of these cases and to try to conduct statistical analysis on these data, in order to support the association between benzene and leukemia. In the 1960s, Vigliani and Alessandra Forni showed that benzene could cause chromosome aberrations in the bone marrow that could produce leukemic clones. CONCLUSIONS: As a result of these studies and the subsequent regulations which banned benzene, exposure conditions changed in the workplace in the last few decades. The resulting low concentrations have prompted researchers to investigate new exposure biomarkers and to study any related health problems.

31. The contribution of the Clinica del Lavoro of Milan to the development of industrial hygiene and toxicology in the twentieth century

Abstract

In 1910, the founder of the Clinica del Lavoro of Milan, Luigi Devoto established a chemistry laboratory in the new institute, sustaining its importance in the study of occupational diseases. In 1948, the new director of the Clinica, Enrico C. Vigliani established the first laboratory of industrial hygiene in Italy, in the years of the economic boom. In 1960s, this laboratory, directed by Nicola Zurlo, significantly contributed to the research in the field. In 1980s and 1990s, the laboratory of the Clinica started to explore the field of environmental toxicology, studying the effects of benzene and other traffic pollutants on the general population. The analysis of history of the Clinica del Lavoro of Milan may represent a valuable tool for studying the origin and the development of industrial hygiene, occupational and environmental toxicology in the twentieth century

Abstract

Mené en 2017 dans 6 centres médico-sociaux (CMS) du canton, ce projet visait à optimiser le choix de l’outil d’évaluation gériatrique selon le profil du client d’aide et soins à domicile. Il a testé deux nouveaux outils: un outil de dépistage et d’orientation (interRAI Emergency Department Screener (iEDscreener©)) et un instrument d’évaluation gériatrique « courte » (interRAI Contact Assessment version test (iCAtest)) plus léger que le RAI-Home Care Suisse (RAI-HCSuisse) habituellement utilisé chez les clients de soins à domicile. Parmi les objectifs figuraient celui de tester l’utilisation d’un outil de dépistage par des infirmiers et infirmières analystes (IA), spécialisés dans l’analyse des demandes d’aide et de soins à domicile, ainsi que de tester l’évaluation iCAtest par des infirmiers et infirmières référents de situation (IR). Finalement, le pilote cherchait à déterminer l'articulation optimale entre l’iEDscreener, l’iCAtest et le RAI-HCSuisse.

Abstract

Background/Objectives: Epidemiological studies suggest a link between chromium (Cr) status and cardiovascular disease. Increased urinary excretion of Cr was reported in subjects with diabetes compared with non-diabetic controls and those with non-diabetic insulin resistance. Epigenetic alterations have been linked to the presence of Cr, and microRNA (miRNA) expression has been implicated in the pathogenesis of metabolic diseases and cardiovascular diseases (CVDs). We investigated the association between Cr excretion and miRNA expression in leukocytes from obese subjects. We also examined the relationship between altered miRNA expression and selected clinical parameters to further investigate mechanisms linking Cr to metabolic diseases and CVDs. Subjects/Methods: We analyzed urinary Cr in 90 Italian subjects using inductively coupled plasma-mass spectrometry. Peripheral blood miRNA levels were screened with TaqMan Low-Density Array Human MicroRNA A. Cr level-associated expression of miRNAs was detected with multivariate regression analyses, and the top 10 candidate miRNAs were selected for validation. We also used multivariate regression analyses to assess possible associations between validated miRNAs and glycated hemoglobin (A1c) and blood pressure (BP). The validated miRNAs were further investigated by functional analysis with Ingenuity Pathway Analysis software. Results: Urinary Cr levels (mean: 0.35 μg/l; s.d.=0.24) ranged from 0.05 to 1.27 μg/l. Abstract

"Pas de retraite pour ma santé" est un projet de promotion du mouvement et de l'alimentation favorables à la santé dédié spécifiquement aux seniors. Ce projet mené par les Ligues de la Santé du Canton de Vaud est le fruit d'une collaboration entre le programme cantonal « Ça marche ! », Pro Senectute Vaud, le Service de gériatrie du CHUV et le Service de l'éducation physique et du sport. Le projet s'inscrit dans la politique « Vieillissement et Santé » du canton de Vaud. Il bénéficie également d'un soutien financier de Promotion Santé Suisse. Abstract

Models used in the pre-marketing evaluation do not cover all work scenarios and may over- or underestimate exposure.Uncertainties present in the extrapolation from pre-marketing to the post-marketing warrant exposure and risk assessment in real-life working conditions.Seven vineyard pesticide applicators were followed for a total of 12 work-days. A data collection sheet was developed specifically for this study. Workers' body exposure, hands, and head exposure were measured. Tebuconazole was analyzed using LC-MS/MS.Median potential and actual body exposures were 22.41 mg/kg and 0.49 mg/kg of active substance applied, respectively. The median protection factor provided by the coverall was 98% (range: 90-99%). Hand exposure was responsible for 61% of total actual exposure, and was reduced by more than 50% in workers using gloves. The German Model underestimated the exposure in one work-day, and grossly overestimated it in 3 work-days.High levels of potential body exposure were efficiently controlled by the cotton coverall. Use of personal protective devices, especially chemically-resistant gloves and head cover is the main determinant of skin protection. Field studies on pesticide exposure in real-life conditions and development of methods and tools for easier risk assessment are necessary to complement and confirm the risk assessment done in the authorization process.

47. Médicaments chez les seniors vaudois à domicile : nombre et composition

Abstract

La question de la pertinence et de la sécurité des traitements médicamenteux dans la population âgée prend toujours plus d'importance dans nos sociétés en raison de l'accroissement en nombre des personnes âgées et de l'expansion continuelle des possibilités thérapeutiques. La nécessité d'individualiser et de rationnaliser la prescription chez le sujet âgé est universellement reconnue. Par ailleurs, il est bien documenté que le fait de prendre un nombre élevé de médicaments différents accroît le risque d'effets indésirables, de défaut d'adhérence et de difficultés de gestion des traitements. La présente étude explore les traitements médicamenteux des seniors vaudois et formule des propositions visant une «polymédication rationnelle». Une enquête conduite en 2012 dans le canton de Vaud auprès de 3'133 personnes âgées de 69 ans et plus vivant en domicile privé comportait une section sur le traitement médicamenteux actuel des répondants (autodéclaration, questionnaire rempli à domicile). Les réponses à cette question sont analysées dans l'objectif de décrire la consommation de médicaments en termes quantitatifs (notamment mesurer la prévalence de la polymédication), ainsi que la relation entre polymédication et certains facteurs d'influence, et la composition des traitements.

Abstract

Améliorer la coordination des soins est un des points cardinaux de la politique Vieillissement et Santé du canton de Vaud. Ceci se traduit notamment par une volonté d'optimiser l'accès des personnes âgées à l'information médico-sociale. Un des projets qui en découle consiste à mettre en place un guichet intégré, soit un système centralisé, qui permettrait d'être renseigné·e ou orienté·e sur les prestations existantes efficacement et correctement, quelle que soit l'entité à laquelle on s'adresse. C'est dans ce contexte que le Centre d'Observation et d'Analyse du Vieillissement (COAV) a réalisé en 2013 une enquête sur l'accès à l'information médico-sociale auprès des personnes de 65 ans et plus non-institutionnalisées du canton. Ses résultats montrent que : ? Les besoins en prestations médico-sociales sont potentiellement importants car ces dernières sont susceptibles d'intéresser directement, du fait de difficultés fonctionnelles, un peu plus d'un tiers des personnes non institutionnalisées de plus de 65 ans, et indirectement 17 % des personnes du même âge qui sont aussi des aidant·e·s. Les femmes et les personnes bénéficiant de prestations complémentaires à l'assurance vieillesse (PC), étant plus fragiles, sont particulièrement concernées. ? Si cette population a potentiellement d'importants besoins en prestations médico-sociales, les portes d'accès à l'information sur ces prestations restent dans leur ensemble encore mal connues, même si l'on observe de grandes variations selon le type de services en question (selon les prestations, 27% à 57% des personnes interrogées ont déclaré ne pas savoir où s'adresser pour trouver de l'information à leur sujet). ? Les femmes, les personnes recevant des PC, ainsi que les personnes vulnérables et dépendantes sont proportionnellement plus actives dans la recherche d'information. ? Bien que les personnes ayant récemment eu l'occasion de rechercher de l'information sur certaines prestations médico-sociales semblent mieux connaître le système, 7% d'entre elles ont qualifié cette information de plutôt indisponible, 21% d'incomplète, 33% de dispersée et 15% de contradictoire. ? Deux tiers des personnes âgées pensent que l'information médico-sociale devrait être plus accessible sur l'existence de prestations, leur coût et les possibilités d'aide financière pour pouvoir en bénéficier. ? Parmi les personnes n'ayant pas récemment recherché d'information, les habitants de la région Nord sauraient plus souvent où s'adresser pour s'informer que ceux des autres régions. RAISONS DE SANTÉ 221 ? Etre un homme, être défavorisé financièrement (présence de PC) et, au niveau du statut fonctionnel, être vulnérable plutôt que robuste, sont des facteurs de risque d'accès limité à l'information. ? Les mêmes facteurs de risque se retrouvent en conduisant les analyses par type de prestations, excepté pour l'aide relative aux démarches administratives. L'information sur cette aide est mieux connue des personnes recevant des PC parmi celles ayant récemment recherché de l'information. ? Il n'a cependant pas été possible d'identifier un profil-type de la personne à risque face à l'accès à l'information sur la base des données socio-démographiques et fonctionnelles disponibles. ? D'autre part, cette enquête a mis en évidence le fait que le médecin traitant (désigné par 77% des individus) et le CMS (64 %), ainsi que, dans une moindre mesure, la commune (35%), sont les acteurs vers lesquelles les personnes âgées du canton se dirigeraient le plus volontiers pour trouver des informations sur diverses prestations médico-sociales. ? Cependant, au vu des variations constatées en comparant certains sous-groupes, d'autres sources ne sont pas à négliger lors de la mise en place d'un guichet intégré (telles qu'Internet, les EMS, Pro Senectute, les pharmacies, les BRIO, les hôpitaux, la garde médicale, etc.). Cette enquête montre que malgré les efforts entrepris pour faire connaître les prestations médico-sociales, une proportion non négligeable de ces services est peu connue des bénéficiaires potentiels. Ainsi, il serait intéressant de réitérer une telle étude après la mise en place du guichet intégré afin de pouvoir évaluer son impact.

50. La santé des aînés : portrait de la santé et de ses déterminants sociaux en ville de Lausanne

Abstract

L'influence du vieillissement démographique se ressent dans divers secteurs de la société. Le financement des assurances sociales, l'organisation des soins et l'accueil destiné aux aînés doivent être repensés. Une récente analyse de l'Office fédéral de la statistique suggère que l'on vieillit en meilleure santé, avec une meilleure qualité de vie que par le passé. La mise en place de programmes pour un vieillissement en bonne santé s'impose en même temps qu'évolue notre système de santé. Les données actuelles décrivant la population des Lausannois de 65 ans et plus (la cohorte Lc65+) nous invitent à réfléchir à la question. Le présent document est le premier de trois rapports qui présenteront les données de la cohorte Lc65+. Il se fonde principalement sur les informations recueillies durant les années 2010 et 2011, et présentera avant tout les problèmes de santé d'une population âgée entre 68 et 77 ans. Les aspects de la santé seront mis en perspective avec des facteurs sociaux structurels, les déterminants sociaux de la santé. Le rapport se divise en deux parties. La première donne un aperçu de l'état de santé dans la population des seniors domiciliés à Lausanne; les fréquences relatives à chaque indicateur ont été calculées et les résultats présentés sous forme de graphiques ou tableaux. La seconde partie s'attache à évaluer le lien entre les variables de santé et les facteurs socio-économiques. La significativité statistique de l'association (brute, puis ajustée sur le sexe et le groupe d'âge) entre les facteurs socio-économiques et les variables de santé a été testée par des tests du" chi" d'indépendance.
