Pulmonary oxalosis can be fatal, and Aspergillus tubingensis is commonly resistant to azoles in Japan. We report a case of bronchopulmonary oxalosis caused by A. tubingensis in a non-neutropenic patient who was successfully treated with voriconazole monotherapy. The susceptibility of the isolates to voriconazole and the effective elimination of contagious necrotic tissue by expectoration seemed to be two major factors contributing to the patient's survival. According to the literature review, pulmonary oxalosis is associated with a high mortality rate over a short term. An exploration of detailed information about the genomic characteristics and drug susceptibility of Aspergillus isolates is important for the development of treatment strategies for this life-threatening disease.

症例は65歳の女性．乳癌術後10年目に前胸部の皮下腫瘤で再発した．胸部CT検査で右第3肋軟骨を中心に腫瘍が胸骨体に浸潤し，皮膚や胸腔内へと突出していた．化学療法を行ったが，腫瘍は急速に増大し自壊した．感染の合併により発熱し悪臭を放つようになり手術を決断した．腫瘍は8cm以上あり，ほぼ胸骨体全体が腫瘍で破壊された．手術は胸骨体切除，右第3，4，5肋骨切除，右肺部分切除を行い，欠損した皮膚軟部組織は，右内胸動静脈と左下腹壁動静脈とを血管吻合した有茎腹直筋皮弁により胸壁再建した．感染創のため異物を用いた再建は行わなかった．術後2年6ヵ月経過した現在再発を認めていない．骨性胸郭の再建を省略したが，呼吸機能は保たれ良好な結果を得たので報告する．

胸膜孤立性線維腫瘍（Solitary Fibrous Tumor of Pleura：以下SFTPと略す）の外科切除症例3例に対し胸腔鏡操作が，又巨大SFTP症例での術前の血管造影が，腫瘍切除のアプローチの選択に有用であったので報告する．症例1：75歳，女性．胸部X線およびCTで右胸膜腫瘍（3cm）を疑い，胸腔鏡手術を施行した．右中葉臓側胸膜より発生したSFTPに対し，VATS（Video-Assisted Thoracic Surgery）腫瘍摘出術を施行した．症例2：67歳．女性．胸部CTおよびMRI上で右胸膜腫瘍（5.7×4.7cm）を疑った．胸腔鏡下に，壁側胸膜に有茎性のSFTPを確認し，小開胸を設置して鏡視下に腫瘍摘出術を施行した．症例3：59歳，女性．労作性呼吸困を伴った，診断未定の胸腔内巨大腫瘍（22×11cm）に対し，血管造影後にVATSを先行した．横隔膜部に有茎性のSFTPを確認し，後側方開胸に移行し，横隔膜と肺を一部分含んだ腫瘍摘出術を施行した．

肺硬化性血管腫の多くが中年以降の女性に発症し，大部分は単発である．男性発症の多発例は非常にまれである．今回我々は，若年男性に発症した多発性肺硬化性血管腫を経験したので報告する．症例は22歳，男性．検診で右下肺野の異常陰影を指摘され，当科を受診した．胸部CTで右下葉に限局して多発する辺縁明瞭な類円形の腫瘤陰影を認めた．大きさは最大で約1cmであった．全身検索で他の病変は認めなかったが，転移性肺腫瘍の可能性を考慮し診断目的で胸腔鏡下生検を施行した．胸腔内観察時，CTで指摘された腫瘤に加え下葉胸膜表面に径数ミリの微小な結節を多数認めた．上・中葉には肉眼で観察可能な病変は存在しなかった．病理診断で多発性硬化性肺血管腫の確定診断を得た．現在，術後14ヵ月で，残存病変の明らかな増大所見はなく，経過観察中である．

我々は1994年より胸部外傷に対して積極的に胸腔鏡下手術を行ってきた．2005年6月まで34件の胸部外傷に対し胸腔鏡下手術を行った．年令は6から90歳（平均40.7歳），受傷原因は刺創21例，交通外傷11例，転落外傷2例であった．34例中2例は出血コントロール不良のため，標準開胸に切りかえた．胸腔鏡下手術を完遂できた32例の術式は，部分切除14例，止血7例，肺縫合もしくは修復術5例，審査胸腔鏡5例，気管支縫合術1例であった．鈍的外傷では（1）横隔膜破裂，血管損傷に対する審査胸腔鏡（2）持続する胸腔内気瘻（3）6時間で500ml以上の血胸，鋭的外傷では（1）刺創（2）汚染に対する胸腔洗浄，を手術適応とした．胸部外傷に対する手術療法はあくまで緊急開胸手術が主体であるが，その中に胸腔鏡下手術でも充分対応できる症例は存在する．

We study the variation of the Berman metric K(t, ζ )|dζ| 2 on the Riemann sur- face R(t) (which varies with complex parameter t in a disk B) such that π : R → B is a 2-dimensional Stein space with π −1 (t) = R(t), t ∈ B. We show that log K(t, ζ )|dζ| 2 is pluri- subharmonic in R, and apply it to give the uniformization condition for the special Stein space.

This article investigates the smoothness of rank M scaling functions and gives some numerical results. Smoothness of a scaling function ϕ is studied by introducing an exponent s p ( ϕ ), which is represented by the spectral radius of the transfer operator associated with the reduced symbol of ϕ . A direct method for estimating the spectral radius is also proposed and used to obtain quite sharp estimates for s p ( ϕ ) and for the Holder exponents. In fact, numerical experiments on s 1 ( ϕ ) for a certain class of scaling functions give estimates for their Holder exponents much better than those obtained by the Sobolev estimates, which are known so far, combined with the Sobolev imbedding theorem.

気管・気管支断裂損傷は, 鈍的胸部外傷の中で頻度は少ないが, 致命率は高い.われわれは, 受傷初期の胸部単純X線写真が診断に有用であった, 18歳男性のバイク外傷による右主気管支断裂損傷 (日本外傷学会, 気管, 気管支損傷分類Ib) の1例を経験した.胸部単純X線写真上, tracheobronchial stripeの描出, 進行性の縦走する気管と右主気管支周囲の透亮像, continuous diaphragm sign, 右側のNaclerio's V-signを認め, これらは縦隔気腫を示唆する所見であった.さらに, 奇静脈近傍の右主気管支陰影は不鮮明化し, その外側に血腫像を認めた.以上の所見から右主気管支断裂損傷が示唆された.手術は損傷部位を直接縫合閉鎖し, 救命した.

The concept of behavior spaces introduced by Shiba plays an important role of systematic investigation of abelian differentials on an open Riemann surface. A Shiba behavior space consists of harmonic differentials which satisfy a certain period condition and boundary behavior. In this paper, for any open Riemann surface of infinite genus we construct Shiba behavior spaces with arbitrarily prescribed period condition and with specific boundary behavior.

Background . Deep pulmonary laceration (DPL) is rare and its survival rate is low. The present study focused on the prognostic factors of DPL. Methods . The present study concerned 17 DPL patients treated in Tokai University Hospital between 1988 and 1998. The prognostic factors of DPL were compared with systolic blood pressure (SBP), PaO 2 , and the volume of intrathoracic blood loss. Characteristic findings of initial chest roentgenograms of DPL were investigated. Results . Eleven patients were saved and 6 patients died. An SBP of less than 80 mm Hg on arrival at the hospital and a blood loss of more than 1,000 mL through the chest tube within 2 hours after arrival were poor prognostic factors. Hypoxemia on arrival was not a poor prognostic factor. Chest roentgenograms showed macular infiltrative shadow with moderate lung collapse and deviation of the mediastinal shadow toward the unaffected side. Selective bronchial occlusion with a Univent prevented suffocation by intrabronchial blood. Conclusions . Two poor prognostic factors of DPL are SBP less than 80 mm Hg on arrival and blood loss of more than 1,000 mL through the chest tube within 2 hours after arrival.

It should be expected to construct behavior spaces with a certain period condition on a Riemann surface of infinite genus, because behavior spaces play a central role in a formulation of Riemann-Roch's and Abel's theorem on an open Riemann surface. We consider a sequence of behavior spaces and a certain weakly convergent space. We will systematically investigate in various points the structure of its convergent space. we treat some subjects on admissible sequences, complex conjugate behavior spaces and linear mapping from a subspace of holomorphic differential to a subspace of antiholomorphic differential.

We study conformal mapping between two doubly connected domains--- rectangles with a crossing slit. The ordered set of four vertices and the slit of the domain shall respectively correspond to those of the image domain. We will find the image rectangle and determine the modulus and the size of the crossing slit.

An 18-year-old female presented with an abnormal shadow of a 22-mm nodule in the left anterior segment of the upper lung lobe (S3) on chest X-ray and computed tomography (CT). Thoracoscopic partial lobectomy was performed, when a frozen section suggested spindle cell sarcoma of extrapulmonary origin, including malignant fibrous histiocytoma (MFH). A histologic diagnosis of the MFH was rendered postoperatively on permanent sections. As no other primary focus was detected in spite of intensive investigation, including 18F-fluorodeoxyglucose positron emission tomography - CT scans, a definitive diagnosis was made of primary pulmonary MFH. Primary pulmonary MFH is usually treated by total resection, however, in the current case we performed a partial lobectomy with clear margins and a watchful follow-up without any additional aggressive treatment. No relapse has been observed for 3 years after the surgery.

Since 1994, we have performed video-assisted thoracoscopic surgery in order to treat thoracic trauma. In general, emergency surgery is performed for trauma injuries incurred by knives. Between 1994 and 2005, we performed thoracoscopic surgery on eighteen cases of thoracic stab wounds. Among these eighteen cases, two were characterized by wounds to the internal thoracic artery, and they had to be switched over to open thoracotomy due to excessive bleeding. In conclusion, open thoracotomy should be performed in cases in which the patient is in a state of shock due to severe intrathoracic bleeding; however, in cases in which the vital signs are stable, thoracoscopic surgery may be carried out to stop intrathoracic bleeding, to repair the lung injury by suturing, as well as by performing a partial resection of the lung.

Background. Continuing to refine minimally invasive thoracoscopic surgical procedures, we have established the two-windows method. Methods. Skin incisions required by this method consist of a 2- to 3-cm skin incision posteriorly, and a 2- to 3-cm skin incision anteriorly in the fourth intercostal space, with the inferior angle of the scapula as the midpoint. We used this method to perform pulmonary lobectomies in combination with thoracoscopy and mediastinal lymph node dissection in 100 consecutive patients with lung cancer (preoperative diagnosis, stage I, T1 N0 M0). Results. The mean operative time was 2 hours 46 minutes, the mean blood loss was 68.2 mL, and the mean number of mediastinal lymph nodes dissected was 24.3. In developing this minimally invasive thoracoscopic procedure, which facilitates mediastinal lymph node dissection, we realized that it is best performed through the fourth intercostal space. Because the tracheal bifurcation can be seen directly below this level, surgical manipulation in this area can be easily performed. This enables the same extent of mediastinal lymph node dissection as that performed during a standard thoracotomy. Another advantage of this method is that a standard posterolateral thoracotomy incision can be made whenever necessary by simply connecting the two incisions. Conclusions. We believe that the two-windows method is capable of serving as the standard method for the surgical treatment of stage I lung cancer.
