Abstract

SummaryBackgroundIf carotid artery narrowing remains asymptomatic (ie, has caused no recent stroke or other neurological symptoms), successful carotid endarterectomy (CEA) reduces stroke incidence for some years. We assessed the long-term effects of successful CEA.MethodsBetween 1993 and 2003, 3120 asymptomatic patients from 126 centres in 30 countries were allocated equally, by blinded minimised randomisation, to immediate CEA (median delay 1 month, IQR 0·3–2·5) or to indefinite deferral of any carotid procedure, and were followed up until death or for a median among survivors of 9 years (IQR 6–11). The primary outcomes were perioperative mortality and morbidity (death or stroke within 30 days) and non-perioperative stroke. Kaplan-Meier percentages and logrank p values are from intention-to-treat analyses. This study is registered, number ISRCTN26156392.Findings1560 patients were allocated immediate CEA versus 1560 allocated deferral of any carotid procedure. The proportions operated on while still asymptomatic were 89·7% versus 4·8% at 1 year (and 92·1% vs 16·5% at 5 years). Perioperative risk of stroke or death within 30 days was 3·0% (95% CI 2·4–3·9; 26 non-disabling strokes plus 34 disabling or fatal perioperative events in 1979 CEAs). Excluding perioperative events and non-stroke mortality, stroke risks (immediate vs deferred CEA) were 4·1% versus 10·0% at 5 years (gain 5·9%, 95% CI 4·0–7·8) and 10·8% versus 16·9% at 10 years (gain 6·1%, 2·7–9·4); ratio of stroke incidence rates 0·54, 95% CI 0·43–0·68, p

Abstract

Throughout the Northern Trust, two different thrombolytic agents, either reteplase or tenecteplase, are used as part of the treatment of acute ST elevation myocardial infarction. Having found no other comparative studies, this retrospective study was designed to compare the efficacy of the two drugs using rate of follow-on emergency angioplasty as the primary outcome. The study retrospectively recruited 40 patients who had received reteplase and 40 who had received tenecteplase. Of the patients who received reteplase, 5 required emergency angiography. Of those who received tenecteplase, 15 required further intervention. This was a significant difference with a ratio of 37.5%:12.5% (p=0.01; significance was assumed to be p, Sotos syndrome is a relatively common overgrowth disorder, following autosomal dominant inheritance, caused by mutations and deletions in the nuclear receptor Set domain containing protein-1, NSD1 gene. Affected individuals generally have advanced bone age, macrocephaly, characteristic facial gestalt and learning difficulties. Other features include scoliosis, seizures, cardiac defects and genitourinary anomalies. Tumours are a rare occurrence. Genotype-phenotype correlations are unclear, though those with a deletion appear to have more severe mental retardation. Full penetrance is seen, although familial Sotos syndrome is extremely rare. The low vertical transmission rate, which is not fully explained by cognitive impairment, is of great importance, particularly for mildly affected patients. Here we report a 3-generation pedigree with 7 affected individuals shown to harbour the NSD1 missense mutation c. 6115C>T. To our knowledge this is the largest Sotos family to be reported. The phenotype is extremely variable, thus highlighting the clinical heterogeneity that may occur. Detailed study of individuals with NSD1 gene abnormalities will be invaluable for further clarification of the phenotype and may lead to NSD1 gene analysis having prognostic value. Long-term follow up of these rare cases of familial Sotos syndrome should make an important contribution to the clarification of these uncertainties.

Abstract

The article describes preliminary surveillance data by the Foodborne Diseases Active Surveillance Network (FoodNet) of the Emerging Infections Program of the U.S. Centers for Disease Control and Prevention (CDC) on laboratory-confirmed infections with select enteric pathogens transmitted commonly through food for 2009 and trends in incidence since 1996. A total of 17,468 cases of such infection were identified in 2009. Pathogens discussed include Campylobacter, Listeria, Salmonella, and Shigella.

46. A digital library for education: the PEN-DOR project.

Abstract

Recent initiatives in digital library research have suggested new models for the creation and organisation of digital information and its dissemination to virtual communities. PEN-DOR (the Pennsylvania Education Network Digital Object Repository) is a digital library designed to provide access to the collective experience of teachers, students and administrators in public schools in building lesson plans and using curriculum materials. Using the WWW as a platform, PEN-DOR incorporates current research in digital libraries to provide K-12 educators with access to multimedia resources and tools to create new lesson plans and presentations, and to modify existing ones. Design problems addressed by the project include the design of a distributed, object-oriented database architecture, the description and cataloguing of multimedia objects, and issues related to usability and training for a geographically scattered user community. Two critical aspects of the organisation of this digital library are the development of a method for the persistent identification of resources, and the design of a record structure based on recent developments in metadata. Resource identification has been achieved by adopting a system-wide approach with an upgrade path to the emerging URN standards. In designing a record structure, the PEN-DOR project has elected to use the GEM (Gateway to Educational Materials) metadata standard developed as part of the GEM union catalogue project. Content for the database is solicited from project partners, government agencies and educational resources Web sites, as well as from participating teachers. Once incorporated in the repository, materials can be organised in frameworks that form the basis for lessons, tutorials and presentations. As frameworks arse developed, used, critiqued and modified, they will form a community memory of past experience. 47. Blood glucose and prognosis of acute stroke.

Abstract

To study the relation of blood glucose soon after the onset of stroke and outcome in terms of fatality and functional recovery 6 months later, two prospective studies were performed. Fasting blood glucose measured within 48 hours of onset was significantly higher in those who died than in those who survived. However, random blood glucose, mean daily blood glucose and HbA1 were not related to fatality or functional recovery. Fasting blood glucose levels soon after the stroke were significantly higher than those in the same patients measured 1 month later. Every year, Australia identifies 2-3 outbreaks associated with imported foods. To examine national authorities' obligations under the International Health Regulations (2005), we reviewed outbreaks in 2001-2007 that implicated internationally distributed foods. 50. FRICTION AND WEAR TESTING FOR COAL SLURRY FUEL DEVELOPMENT

Abstract

Friction and wear characteristics of coal slurries in transportation and atomization/firing systems are areas of concern in considering their use as substitutes for neat liquid fuels. The main wear phenomena encountered in slurry fuel usage are erosive and sliding wear of metal surfaces in contact with the coal slurry. Laboratory experiments have been devised and carried out to characterize and better understand these slurry wear mechanisms. The dependence of wear on the characteristics of the coal slurry and the properties of the wear surfaces have been studied. Coal slurries prepared from an Ohio coal desulfurized using a novel perchloroethylene extraction process have been studied. A Cameron-Flint reciprocating slider tribometer has been used to study sliding wear phenomena. Erosive wear has been investigated using an impingement type wear tester. The importance of impingement angle of coal slurry on platelet formation during erosion has been determined. Results of This angle corresponding to peak erosion is lover than for the softer 1020 steel, and can be attributed to the hardness of the steels.Wear tests on 1020 steel were carried out using coal slurries formulated from run-of-mine coal and coal freed of mineral matter and organic sulfur by the perchloroethylene coal cleaning process (Lee et al., 1989). It was observed that run-of-mine coal created two kinds of indentations on the 1020 steel - larger indentations attributable to large mineral matter particles, and smaller indents. Only smaller indents were present in the wear tests with a slurry prepared with cleaned coal, which was largely freed of mineral matter.
