5. Biomechanical comparison of titanium alloy additively manufactured and conventionally manufactured plate-screw constructs.

Abstract

To biomechanically compare the bending stiffness, strength, and cyclic fatigue of titanium additively manufactured (AM) and conventionally manufactured (CM) limited contact plates (LCP) of equivalent dimensions using plate-screw constructs. Twenty-four 1.5/2.0-mm plate constructs (CM: n = 12; AM: n = 12) were placed under 4-point bending conditions. Data were collected during quasi-static single cycle to failure and cyclic fatigue testing until implants plastically deformed or failed. Bending stiffness, bending structural stiffness, and bending strength were determined from load–displacement curves. Fatigue life was determined as number of cycles to failure. Median test variables for each method were compared using the Wilcoxon rank sum test within each group. Fatigue data was also analysed by the Kaplan–Meier estimator of survival function. There was no evidence for a difference in bending stiffness and bending structural stiffness between AM and CM constructs. However, AM constructs exhibited greater bending strength (median 3.07 (min 3.0, max 3.4) Nm) under quasi-static 4-point bending than the CM constructs (median 2.57 (min 2.5, max 2.6) Nm, p = 0.006). Number of cycles to failure under dynamic 4-point bending was higher for the CM constructs (median 164,272 (min 73,557, max 250,000) cycles) than the AM constructs (median 18,704 (min 14,427, max 33,228) cycles; p = 0.02). Survival analysis showed that 50% of AM plates failed by 18,842 cycles, while 50% CM plates failed by 78,543 cycles. Additively manufactured titanium implants, printed to replicate a conventional titanium orthopaedic plate, were more prone to failure in a shorter fatigue period despite being stronger in single cycle to failure. Patient-specific implants made using this process may be brittle and therefore not comparable to CM orthopaedic implants. 16. Very high density C[O.sub.2] associated with ultrahigh-temperature metamorphism in the Eastern Ghats granulite belt, India

Abstract

Spinel-bearing high Mg-Al granulites of the Vizianagram area in the Eastern Ghats granulite belt show textural features clearly establishing that the association [spinel.sub.ss] + quartz + Fe-Ti oxide solid solution [+ or -] sillimanite [+ or -] porphyroblastic orthopyroxene was stable during peak metamorphic conditions. Pressure-temperature (P-T) conditions estimated from both mineralogical thermobarometry and phase-equilibrium limitations indicate that the peak metamorphism occurred under ultrahigh-T conditions (>1000[degrees]C) at 8-9 kbar pressure. Retrograde P-T conditions of 750-800[degrees]C, 6-7 kbar are deduced from the compositions of coronal garnet and orthopyroxene, which have rims of spinel against quartz, indicating significant cooling with slight lowering of pressure. Quartz associated with the ultrahigh-T assemblage at Vizianagram contains ubiquitous single-phase carbonic inclusions as isolated clusters that belong to two categories. Group I shows extremely high density (homogenization temperature: -51 [+ or -] 1.8[degrees]C; density 1.15 g/[cm.sup.3]) and group II trapped relatively lower density fluids (homogenization temperature: -18.4 [+ or -] 2.4[degrees]C; density 1.05 g/[cm.sup.3]). The isochores for group I inclusions pass through the peak metamorphic P-T conditions, whereas those for group II coincide with the P-T conditions of the formation of coronal garnet and orthopyroxene. Our study is the first report of very high density C[O.sub.2] associated with the Eastern Ghats granulite belt rocks and provides a strong case for the presence of C[O.sub.2]-rich fluids during ultrahigh-T metamorphism at lower crustal levels. 49. The Value of Early Depth of Response in Predicting Long-Term Outcome in EGFR-Mutant Lung Cancer

Abstract

Introduction: Traditionally, marked tumor shrinkage has been assumed to portend better outcome. We investigated whether depth of tumor response was associated with improved survival outcomes in advanced EGFR-mutant NCLC. Methods: Individual patient data from randomized trials (EURTAC, IPASS, ENSURE, LUX-Lung 3, and LUX-Lung 6) were used. The association of depth of response with progression-free survival (PFS) and overall survival was examined using landmark analyses. Depth of response based on radiologic assessments at 6 weeks and 12 weeks was calculated as the relative changes in the sum of the longest diameters of the target lesions from baseline. Results: Of 1081 evaluable patients at 6 weeks with no disease progression, 71.2% achieved Response Evaluation Criteria in Solid Tumors response. Using a landmark analysis, EGFR-TKI was more effective than chemotherapy (PFS hazard ratio = 0.36, p < .0001); and was associated with greater mean tumor shrinkage than chemotherapy (35.1% versus 18.5%, p < .0001). However, there was no significant difference in the relative PFS benefit between treatment groups across the entire spectrumof tumor shrinkage (p = .18 for test of interaction between treatment and continuously measured depth of response). Depth of response at 6 weeks was not associated with PFS when adjusted for treatment effect (hazard ratio = 0.96, p = .78). Similar results were obtained for 12-week landmark analysis and for OS outcome. Conclusions: The PFS advantage of EGFR-TKI over chemotherapy in advanced EGFR mutant NCLC is not explained by depth of response at 6 or 12 weeks.
