Abstract

We report two cases of rectal malignant melanomas. The patients were an 84-year-old male and a 66-year-old female who had blood in their stools. They were preoperatively diagnosed with poorly differentiated adenocarcinoma of the rectum. The clinical diagnosis for each was rectal carcinoma at stage IIIc according to the tumor-node-metastasis classification (6th edition), and the patients underwent abdominoperineal resection with dissection of lymph nodes. Pathological examination of the resected specimens revealed a malignant melanoma. Immunohistochemical analysis results were positive for HMB-45 and negative for cytokeratin AE1/AE3, CD45, and synaptophysin. Primary anorectal melanoma is an uncommon and aggressive disease that carries a poor prognosis. Therefore, it is necessary to provide systemic treatment. To improve prognosis, it is important to detect anorectal melanoma at an early stage.

Abstract

Primary malignant melanoma of the esophagus (PMME) is a rare disease with poor prognosis. [F]-Fluoro-deoxy-glucose positron emission tomography (FDG-PET) has been used extensively for initial tumor staging, therapeutic monitoring, and follow-up evaluation in various malignant tumors, but the significance of FDG-PET in PMME is currently unknown. Herein, we report 3 cases of PMME. In all 3 cases, histological and immunohistochemical examination of the biopsy specimens led to diagnoses of malignant melanoma. FDG-PET/computed tomography was performed in all cases, and was useful for the detection of distant metastases. However, the identification of lymph node metastases at initial diagnosis was difficult. Esophagectomy was performed in one case, while systemic chemotherapy was administered in 2 cases. All 3 patients had a poor prognosis; the mean survival time was 13.3 months. In conclusion, PET/computed tomography is useful for the detection of distant metastasis at initial diagnosis as well as the therapeutic monitoring of PMME. [ABSTRACT FROM AUTHOR]

Abstract

Objectives: Laminin-5 γ2 chain (LN-5 γ2) is an extracellular matrix protein that plays an important role in cell migration and tumor invasion. We evaluated the association of LN-5 γ2 and epidermal growth factor receptor (EGFR) expression in esophageal squamous cell carcinoma (SCC). Methods: LN-5 γ2 and EGFR expression was evaluated in 110 esophageal SCC patients by immunohistochemistry, and was confirmed using esophageal SCC cell lines by Western blot analysis. Results: LN-5 γ2 expression in the invasive front of the tumor was correlated with the depth of invasion (p = 0.0001), lymph node metastasis (p = 0.0011) and pathological stage (p = 0.0001). The strong expression of EGFR was also correlated with lymph node metastasis (p = 0.0456) and the pathological stage (p = 0.0055). In patient survival, LN-5 γ2 positivity and/or strong EGFR expression showed a significantly low survival rate as compared with those with lesser expression of LN-5 γ2 and EGFR. Immunohistochemically, LN-5 γ2 expression was significantly correlated with EGFR expression (p < 0.0001). Western blot analysis also confirmed the correlated expression of LN-5 γ2 and EGFR in SCC cell lines except 2 of the 5 cell lines. Conclusions: This study suggests that coexpression of LN-5 γ2 and EGFR is closely related to the progression and poor prognosis of esophageal SCC. Copyright © 2005 S. Abstract

Objective: To report a case of video-assisted thoracic surgery for esophagus (VATS-E) for the treatment of locally recurrent gastric cancer at the anastomotic site. Clinical Presentation and Intervention: A 59-year-old man underwent total gastrectomy for gastric cancer. Histopathologically, it was mucinous adenocarcinoma [pT1(sm), pN1, sM0, PM(-), DM(-), stage IB]. Local recurrence was detected and he received chemotherapy and radiotherapy; histopathologically, it was identified as mucinous adenocarcinoma [pT1(sm), pN0, sM0, stage IA]. As the local recurrence was uncontrollable, he underwent VATS-E. At the 1-year follow-up, he was recurrence-free. Abstract

A 64-year-old man was diagnosed with advanced gastric cancer type 3 and regional celiac trunk lymph node metastases. We performed preoperative chemotherapy with docetaxel, cisplatin, and S-1(DCS therapy). Total gastrectomy with lymph node dissection was performed after 2 courses of DCS. Pathologically, no viable cells were found in the primary lesion or in the dissected lymph nodes. The pathological response to preoperative DCS therapy was classified as grade 3. The postoperative course was uneventful; the patient is currently healthy and receives periodic medical examinations.

Abstract

We report our experience with four patients presenting with spontaneous pneumomediastinum (SPM) within a 3-month period. The patients (three male and one female, aged 15-17&emsp14;years) were hospitalised with SPM. All patients were kept under observation, successfully treated and followed up for several years, with no recurrences reported. Two patients had histories of asthma, while the other two developed SPM during sporting activities. SPM carries the possibility of being latent. For symptoms such as chest pain without evidence of pneumothorax in young people, it is necessary to always consider SPM and make the diagnosis accordingly.

Abstract

Background/aims: Heat shock proteins (HSPs) are well known as tumor rejection antigens, most notable of which is HSP70. HSP110 is classified as a member of the HSP70/DnaK superfamily. The objective of this study was to clarify the clinicopathological and prognostic significance of Heat Shock Protein 110 expression and T lymphocyte infiltration in esophageal cancer., Methodology: Immunohistochemical staining of HSP110, CD4 and CD8 were performed on surgical specimens obtained from 124 patients with esophageal cancer., Results: The expression of HSP110 correlated inversely with depth of invasion (p<0.0001), lymph node metastasis (p=0.0163), pathological stage (p<0.0001), lymphatic invasion (p=0.0104), blood vessel invasion (p=0.0027), infiltrative growth pattern (p=0.0368) and correlated positively with CD4+ T lymphocyte infiltration (p=0.0018). Reduction of HSP110 expression was significantly correlated with poor prognosis (p=0.0010)., Conclusions: The present findings suggest that HSP110 expression and T lymphocyte infiltration is a significant prognostic factor for esophageal cancer.
