Abstract

Objectives Methods Results Conclusions Collagen membranes are extensively used for guided bone regeneration procedures, primarily for horizontal bone augmentation. More recently, it has been demonstrated that collagen membranes promote bone regeneration. Present study aimed at assessing if structural modifications of collagen membranes may enhance their osteoconductive capacity.Twenty‐four adult Wistar rats were used. Bilateral calvaria defects with a diameter of 5 mm were prepared and covered with prototypes of collagen membranes (P1 or P2). The P1 membrane (positive control) presented a lower onset temperature of protein denaturation and a higher solubility than the P2 membrane (test). The contralateral defects were left uncovered (NC). After 1 and 4 weeks, the animals were euthanized. A microcomputed tomography analysis of the harvested samples was performed within and above the bony defect. Undecalcified ground sections were subjected to light microscopy and morphometric analysis.Bone formation was observed starting from the circumferential borders of the defects in all groups at 1‐week of healing. The foci of ossification were observed at the periosteal and dura mater sites, with signs of collagen membrane mineralization. However, there was no statistically significant difference between the groups. At 4 weeks, remnants of the collagen fibers were embedded in the newly formed bone. In the P2 group, significantly more bone volume, more new bone, and marrow spaces compared to the NC group were observed. Furthermore, the P2 group showed more bone volume ectocranially then the P1 group.Bone formation subjacent to a P2 membrane was superior than subjacent to the P1 membrane and significantly better compared to the control. Modifications of the physico‐chemical properties may enhance the osteoconductive competence of collagen membranes, supporting bone formation outside the bony defects. [ABSTRACT FROM AUTHOR]

Abstract

The use of platelet‐rich fibrin (PRF) has gained tremendous popularity in recent years owing to its ability to speed wound healing postsurgery. However, to date, many clinicians are unaware of methods designed to optimize the technology. This overview article will discuss the advancements and improvements made over the years aimed at maximizing cell and growth factor concentrations. First, a general understanding explaining the differences between RPM and RCF (g‐force) is introduced. Then, the low‐speed centrifugation concept, fixed angle versus horizontal centrifugation, and methods to maximize platelet concentrations using optimized protocols will be discussed in detail. Thereafter, the importance of chemically modified PRF tubes without the addition of chemical additives, as well as regulation of temperature to induce/delay clotting, will be thoroughly described. This article is a first of its kind summarizing all recent literature on PRF designed to optimize PRF production for clinical treatment. [ABSTRACT FROM AUTHOR]

25. In VitroAssessment of Injectable Bone Marrow Aspirate Concentrates Compared to Injectable Platelet-Rich Fibrin

Abstract

BACKGROUND:: Injectable platelet-rich fibrin (iPRF), a liquid form of PRF that is prepared from peripheral blood without anticoagulants, promotes tissue wound healing and regeneration. The present study focused on iPRF-like bone marrow aspirate concentrate (iBMAC) prepared without anticoagulant, and the regenerative potential of iPRF and iBMAC was compared in vitro. METHODS:: iPRF and iBMAC were prepared from the same New Zealand white rabbits. The cytocompatibility and regenerative potential of each concentrate were evaluated using primary rabbit gingival fibroblasts and osteoblasts. RESULTS:: Both gingival fibroblasts and osteoblasts treated with each concentrate exhibited excellent cell viability. Interestingly, compared to cells treated with iPRF, cells treated with iBMAC demonstrated significantly greater migration potential. Furthermore, higher mRNA levels of transforming growth factor-β (TGF-β), vascular endothelial growth factor (VEGF), and collagen I (COL1) were observed in gingival fibroblasts treated with iBMAC than in those treated with iPRF. Compared with osteoblasts treated with iPRF, osteoblasts treated with iBMAC exhibited greater differentiation potential, as indicated by increased osteocalcin (OCN) expression and mineralization capability. CONCLUSION:: The results of the in vitrostudy suggest that, compared with iPRF, iBMAC may promote wound healing and bone regeneration more effectively. However, further preclinical and clinical studies are needed to confirm the regenerative potential of iBMAC in the body.

Abstract

The goals of the study were to investigate the effects on bone bioactivity of a titanium dioxide layer formed by hydrothermal oxidation of a titanium surface with hydrogen peroxide (H2O2) and loading with fibroblast growth factor‐2 (FGF‐2) in vitro and in vivo. Ti‐6Al‐4V discs were hydrothermally oxidized with H2O2 and then loaded with FGF‐2. After cytotoxicity testing, Ti‐6Al‐4V mini‐implants were subjected to the same treatment, and their osteogenic potential was evaluated histologically in a rat model. H2O2 hydrothermal oxidation resulted in a dense porous network structure and hydrophilic changes, which improved retention of FGF‐2. Morphologically, the cell density was higher, cell elongation was more pronounced, and the cell adhesion area was significantly higher in FGF‐2‐loaded cells than in those without FGF‐2. In a cell proliferation assay using mouse osteoblast‐like cells, absorbance tended to increase over time, especially in the FGF‐2 group after 7 and 14 days, and in a bone differentiation assay based on ALP activity, there was a significant increase in the FGF‐2 group after 14 days. In the rat model, H2O2 hydrothermal oxidation and FGF‐2 loading both resulted in more laminar bone tissue in the bone marrow around the mini‐implant. These results suggest that titanium surface functionalization by H2O2 hydrothermal oxidation and FGF‐2 may promote initial cell adhesion, proliferation, and osteodifferentiation, and enhance bone bioactivity. Abstract

Collagen biomaterials are widely used for soft tissue augmentation. Cross-linking techniques for collagen matrix (CM) achieve mechanical and volumetric stability; nevertheless, cross-linking may compromise biocompatibility. The aim of the present study was to investigate two different three-dimensional (3D) porcine-derived CMs, noncross-linked (ncl)_CM and cross-linked (cl)_CM, for their effects on macrophages (Mφ) and gingival fibroblasts. The effects of the CMs on the cell viability, proliferation, and polarization of Mφ derived from human monocyte THP-1 cells were assessed. The effects of paracrine factors from Mφ cultured on the CMs were further studied in human gingival fibroblasts (HGF-1 cells). The spongy layer of ncl_CM was partially resorbed after 1 day of culture. cl_CM maintained increased numbers of viable cells when compared with ncl_CM on day 3 for both THP-1 and HGF-1 cells. Higher mRNA levels of M1 markers, including IL-1 and IL-6, were found in Mφ cultured on cl_CM, while no significant differences were observed in M2 marker expression levels, including Arg1 and CD206, for cells cultured on both CMs when compared with those of the control. Furthermore, the conditioned medium collected from Mφ cultured on both CMs decreased cell viability. Nevertheless, neither of the CM-conditioned media influenced the mRNA levels of TGF-β, COL1a2, and PDGF-A in HGF-1 cells when compared with the control media. A comparison showed that cl_CM tended to result in more viable cells than ncl_CM, while cl_CM polarized Mφ toward an M1 phenotype, which was confirmed by the observation of increased mRNA levels of pro-inflammatory cytokines.
