Abstract

We explore the spatial and temporal variations in denudation rates in the northern Pamir—Tian Shan region using 10Be-derived denudation rates from modern (n = 110) and buried sediment (2.0–2.7 Ma; n = 3), and long-term exhumation rates from published apatite fission track (AFT; n = 705) and apatite (U-Th-Sm)/He (AHe; n = 211) thermochronology. We found moderate correlations between denudation rates and topographic metrics and weak correlations between denudation rates and annual rainfall, highlighting complex linkages among tectonics, climate, and surface processes that vary locally. The 10Be data show a spatial trend of decreasing modern denudation rates from west to east, suggesting that deformation and precipitation control denudation in the northern Pamir and western Tian Shan. Farther east, the denudational response of the landscape to Quaternary glaciations is more pronounced and reflected in our data. Modern 10Be denudation rates are generally higher than the long-term AFT and AHe exhumation rates across the studied area. In the Kyrgyz Tian Shan, on average, the highest 10Be denudation rates are recorded in the Terskey range, south of Lake Issyk-Kul. Here, modern denudation rates are higher than 10Be-derived paleo-denudation rates, which are comparable in magnitude with the long-term exhumation rates inferred from AFT and AHe. We propose that denudation in the region, particularly in the Terskey range, remained relatively steady during the Neogene and early Pleistocene. Denudation increased due to glacial-interglacial cycles in the Quaternary, but this occurred after the onset and intensification of the Northern Hemisphere glaciations at 2.7 Ma.

Abstract

Urine has been considered as an ideal nutrient source for microalgae cultivation thanks to its composition containing the high concentrations of nitrogen and phosphorus. Herein, the microalgae growth in urine was evaluated in a lab-scale membrane photobioreactor (MPBR) system. This work aimed to validate the influence of low biomass retention times (BRT) (10, 7, 5, 3, 2 d) on nutrient remediation and biomass productivity. It revealed that BRT of 7 d resulted in synergistically high biomass production (biomass productivity of 313 mg/L.d) and removal rates (TN of 90.5 mg/L.d and TP of 4.7 mg/L.d). Notably, the short BRT of 2–5 d was not sufficient to trigger actively growing microalgae and thus reduced biomass production rate. In addition, as operated at a low flux of 2 L/m2.h, MPBR system required no physical cleaning for 100 days of operation. The BRT-dependent biomass concentration played a pivotal role in changing the fouling rate of MPBR; however, the fouling is reversible in the MPBR system under the low flux condition.

Abstract

ObjectiveTo define the current treatment practice of Guillain-Barré syndrome (GBS).MethodsThe study was based on prospective observational data from the first 1,300 patients included in the International GBS Outcome Study. We described the treatment practice of GBS in general, and for (1) severe forms (unable to walk independently), (2) no recovery after initial treatment, (3) treatment-related fluctuations, (4) mild forms (able to walk independently), and (5) variant forms including Miller Fisher syndrome, taking patient characteristics and hospital type into account.ResultsWe excluded 88 (7%) patients because of missing data, protocol violation, or alternative diagnosis. Patients from Bangladesh (n = 189, 15%) were described separately because 83% were not treated. IV immunoglobulin (IVIg), plasma exchange (PE), or other immunotherapy was provided in 941 (92%) of the remaining 1,023 patients, including patients with severe GBS (724/743, 97%), mild GBS (126/168, 75%), Miller Fisher syndrome (53/70, 76%), and other variants (33/40, 83%). Of 235 (32%) patients who did not improve after their initial treatment, 82 (35%) received a second immune modulatory treatment. A treatment-related fluctuation was observed in 53 (5%) of 1,023 patients, of whom 36 (68%) were re-treated with IVIg or PE.ConclusionsIn current practice, patients with mild and variant forms of GBS, or with treatment-related fluctuations and treatment failures, are frequently treated, even in absence of trial data to support this choice. The variability in treatment practice can be explained in part by the lack of evidence and guidelines for effective treatment in these situations.

Abstract

Direct nanofiltration (NF) of secondary treated effluent can significantly simplify water recycling, especially for small scale operations. This study evaluated the performance of a novel flat-sheet submerged NF membrane module at low water flux to minimize membrane fouling. The separation performance of the NF system was assessed by periodically measuring the color, turbidity, pH, UV light absorbance at the wavelength of 254 nm, and total organic carbon concentrations of the NF feed water and permeate. During the 48-d test, negligible membrane fouling was found when direct NF treatment of secondary wastewater effluent was conducted at a water flux of 3 L/m2h. The trans-membrane pressure increased by less than 0.072 kPa/d. The foulant layer on the membrane surface was readily removed by simple physical cleaning, which suggests that the reversible fouling was the major fouling mechanism. Direct NF treatment achieved high and stable removal of organics (total organic carbon removal of >80 %) during the operation period. This study demonstrated the efficacy of the direct NF treatment using a submerged NF module for achieving stable operations and producing high-quality recycled water.

Abstract

ObjectiveTo compare disease course in patients with Guillain-Barré syndrome (GBS) with a poor prognosis who were treated with one or with two intravenous immunoglobulin (IVIg) courses.MethodsFrom the International GBS Outcome Study, we selected patients whose modified Erasmus GBS Outcome Score at week 1 predicted a poor prognosis. We compared those treated with one IVIg course to those treated with two IVIg courses. The primary endpoint, the GBS disability scale at 4 weeks, was assessed with multivariable ordinal regression.ResultsOf 237 eligible patients, 199 patients received a single IVIg course. Twenty patients received an ‘early’ second IVIg course (1–2 weeks after start of the first IVIg course) and 18 patients a ‘late’ second IVIg course (2–4 weeks after start of IVIg). At baseline and 1 week, those receiving two IVIg courses were more disabled than those receiving one course. Compared with the one course group, the adjusted OR for a better GBS disability score at 4 weeks was 0.70 (95%CI 0.16 to 3.04) for the early group and 0.66 (95%CI 0.18 to 2.50) for the late group. The secondary endpoints were not in favour of a second IVIg course.ConclusionsThis observational study did not show better outcomes after a second IVIg course in GBS with poor prognosis. The study was limited by small numbers and baseline imbalances. Lack of improvement was likely an incentive to start a second IVIg course. A prospective randomised trial is needed to evaluate whether a second IVIg course improves outcome in GBS.

Abstract

ObjectiveTo compare the disease course in patients with mild Guillain-Barré syndrome (GBS) who were treated with intravenous immunoglobulin (IVIg) or supportive care only.MethodsWe selected patients from the prospective observational International GBS Outcome Study (IGOS) who were able to walk independently at study entry (mild GBS), treated with one IVIg course or supportive care. The primary endpoint was the GBS disability score four weeks after study entry, assessed by multivariable ordinal regression analysis.ResultsOf 188 eligible patients, 148 (79%) were treated with IVIg and 40 (21%) with supportive care. The IVIg group was more disabled at baseline. IVIg treatment was not associated with lower GBS disability scores at 4 weeks (adjusted OR (aOR) 1.62, 95% CI 0.63 to 4.13). Nearly all secondary endpoints showed no benefit from IVIg, although the time to regain full muscle strength was shorter (28 vs 56 days, p=0.03) and reported pain at 26 weeks was lower (n=26/121, 22% vs n=12/30, 40%, p=0.04) in the IVIg treated patients. In the subanalysis with persistent mild GBS in the first 2 weeks, the aOR for a lower GBS disability score at 4 weeks was 2.32 (95% CI 0.76 to 7.13). At 1 year, 40% of all patients had residual symptoms.ConclusionIn patients with mild GBS, one course of IVIg did not improve the overall disease course. The certainty of this conclusion is limited by confounding factors, selection bias and wide confidence limits. Residual symptoms were often present after one year, indicating the need for better treatments in mild GBS.

Abstract

Guillain-Barré syndrome (GBS) is an acute polyradiculoneuropathy with a highly variable clinical presentation, course, and outcome. The factors that determine the clinical variation of GBS are poorly understood which complicates the care and treatment of individual patients. The protocol of the ongoing International GBS Outcome Study (IGOS), a prospective, observational, multicenter cohort study that aims to identify the clinical and biological determinants and predictors of disease onset, subtype, course and outcome of GBS is presented here. Patients fulfilling the diagnostic criteria for GBS, regardless of age, disease severity, variant forms, or treatment, can participate if included within 2 weeks after onset of weakness. Information about demography, preceding infections, clinical features, diagnostic findings, treatment, course, and outcome is collected. In addition, cerebrospinal fluid and serial blood samples for serum and DNA is collected at standard time points. The original aim was to include at least 1,000 patients with a follow-up of 1-3 years. Data are collected via a web-based data entry system and stored anonymously. IGOS started in May 2012 and by January 2017 included more than 1,400 participants from 143 active centers in 19 countries across 5 continents. The IGOS data/biobank is available for research projects conducted by expertise groups focusing on specific topics including epidemiology, diagnostic criteria, clinimetrics, electrophysiology, antecedent events, antibodies, genetics, prognostic modeling, treatment effects, and long-term outcome of GBS. The IGOS will help to standardize the international collection of data and biosamples for future research of GBS.

Abstract

ObjectiveTo define the current treatment practice of Guillain-Barré syndrome (GBS).MethodsThe study was based on prospective observational data from the first 1,300 patients included in the International GBS Outcome Study. We described the treatment practice of GBS in general, and for (1) severe forms (unable to walk independently), (2) no recovery after initial treatment, (3) treatment-related fluctuations, (4) mild forms (able to walk independently), and (5) variant forms including Miller Fisher syndrome, taking patient characteristics and hospital type into account.ResultsWe excluded 88 (7%) patients because of missing data, protocol violation, or alternative diagnosis. Patients from Bangladesh (n = 189, 15%) were described separately because 83% were not treated. IV immunoglobulin (IVIg), plasma exchange (PE), or other immunotherapy was provided in 941 (92%) of the remaining 1,023 patients, including patients with severe GBS (724/743, 97%), mild GBS (126/168, 75%), Miller Fisher syndrome (53/70, 76%), and other variants (33/40, 83%). Of 235 (32%) patients who did not improve after their initial treatment, 82 (35%) received a second immune modulatory treatment. A treatment-related fluctuation was observed in 53 (5%) of 1,023 patients, of whom 36 (68%) were re-treated with IVIg or PE.ConclusionsIn current practice, patients with mild and variant forms of GBS, or with treatment-related fluctuations and treatment failures, are frequently treated, even in absence of trial data to support this choice. The variability in treatment practice can be explained in part by the lack of evidence and guidelines for effective treatment in these situations.

Abstract

Understanding how sediment transport and storage will delay, attenuate, and even erase the erosional signal of tectonic and climatic forcings has bearing on our ability to read and interpret the geologic record effectively. Here, we estimate sediment transit times in Australia's largest river system, the Murray-Darling basin, by measuring downstream changes in cosmogenic Al-26/Be-10/C-14 ratios in modern river sediment. Results show that the sediments have experienced multiple episodes of burial and reexposure, with cumulative lag times exceeding 1 Ma in the downstream reaches of the Murray and Darling rivers. Combined with low sediment supply rates and old sediment blanketing the landscape, we posit that sediment recycling in the Murray-Darling is an important and ongoing process that will substantially delay and alter signals of external environmental forcing transmitted from the sediment's hinterland.

Abstract

Quantitative tomography is carried out on datasets derived from tensile fracture sample of electrochemically precharged Al-Zn-Mg-Cu alloy in the underaged condition and its uncharged counterpart. It is shown that precharging which induces a transition of tensile fracture mode from ductile to brittle, results in a significant increase in micro-damage content in the regions near the fracture surfaces. Using quantitative tomography analysis based on spatial mapping of morphologically segmented micro-damage content of the datasets it is found that the precharged sample contains an inhomogenous distribution of micro-pores near grain boundaries. It is also shown that the spatial architecture of micro-pores in the dataset is not influenced by the plastic zone of the intergranular cracks lying along the grain boundaries. Contrastingly the micro-pores in the tomographic dataset of the uncharged sample are shown to be present near intermetallic particles. It is therefore rationalized that the spatial architecture of micro-pores in the datasets from uncharged sample originate from particle cracking during ductile fracture, and from the tendency for damage enhancement by the synergism of hydrogen exposure near grain boundaries and localization of deformation in the precharged sample dataset.

Abstract

The efficacy of a baffled osmotic membrane bioreactor-microfiltration (OMBR-MF) hybrid system equipped with thin film forward osmosis membrane for wastewater treatment was evaluated at laboratory scale. The novel OMBR-MF hybrid system involved baffles, that separate oxic and anoxic zones in the aerobic reactor for simultaneous nitrification and denitrification (SND), and a bioreactor comprised of thin film composite-forward osmosis (TFC-FO) and polyether sulfone-microfiltration (PES-MF) membranes. The evaluation was conducted under four different oxic-anoxic cycle patterns. Changes in flux, salinity build-up, and microbial activity (e.g., extracellular polymeric substances (EPS) were assessed. Over the course of a 34 d test, the OMBR-MF hybrid system achieved high removal of total organic carbon (TOC) (86-92%), total nitrogen (TN) (63-76%), and PO4-P (57-63%). The oxic-anoxic cycle time of 0.5-1.5 h was identified to be the best operating condition. Incorporation of MF membrane effectively alleviated salinity build-up in the reactor, allowing stable system operation.
