The odds ratios (ORs) of length of unstable period and unemployment were analyzed with a logistic regression model.Results: The study included 816 patients, of whom 707 were employed full-time (continuous employment) and 70 were unemployed (loss of employment). Univariate analysis showed that ORs were statistically significant for patients who were unstable for “almost all” of the year (OR = 10.4 [4.48– 24.28] p < 0.001), but not for “few” unstable periods (OR = 1.06 [0.56– 1.98] p = 0.849) and for “significant” unstable periods (OR = 1.65 [0.73– 3.74] p = 0.231) were not significantly different. Multivariate analysis showed that ORs were statistically “significant” for patients who were unstable for “almost all” (OR = 12.1 [4.37– 33.3] p < 0.001), but not for “few” unstable periods (OR = 1.07 [0.55– 2.07] p = 0.846) and for “significant” unstable periods (OR = 1.62 [0.66– 3.98] p = 0.290) did not differ significantly.Conclusion: Patients with bipolar disorder with a long unstable period were associated with a higher risk of unemployment.Keywords: mood disorder, loss of employment, nationwide study, real world

Details of the medical care received were investigated using a survey sheet, which included employment status. Odds ratios of mood episodes for employment status were analyzed using a logistic regression model.Results: Among patients aged 60 years or less, 2292 described their occupation. On univariate analysis, odds ratios were statistically significant for depressive episode (OR = 2.68 [1.50– 4.78] p = 0.001) and manic episode (OR = 2.64 [1.07– 6.47] p = 0.034), whereas no significant difference was noted for mixed episode (OR = 1.72 [0.69– 4.33] p = 0.246). On multivariate analysis, odds ratios were statistically significant for depressive episode (OR = 2.16 [1.13– 4.13], p = 0.020) and manic episode (OR = 3.55 [1.36– 9.25], p = 0.010), whereas no significant difference was noted for mixed episode (OR = 1.83 [0.65– 5.14] p = 0.254).Conclusion: Employment status among these patients with bipolar disorder receiving outpatient treatment was 43.5%. Compared to remission episodes, manic and depressive episodes were associated with a higher risk of unemployment.Keywords: mood disorder, continuous employment, occupational health, Japan

12. Suzaku observations of the low surface brightness cluster A76

Abstract

Context: We present results of Suzaku observations of a nearby galaxy cluster A76 at z=0.0395. This cluster is characterized by extremely low X-ray surface brightness and is hereafter referred to as the LSB cluster. Aims: To understand the nature and thermodynamic evolution of the LSB cluster by studying the physical properties of the hot intracluster medium in A76. Methods: We conducted two-pointed Suzaku observations of A76 and examined the global gas properties of the cluster by XIS spectral analysis. We also performed deprojection analysis of annular spectra and derived radial profiles of gas temperature, density and entropy out to approximately 850 kpc (~ 0.6 r_200) and 560 kpc (~0.4 r_200) in A76 East and A76 West, respectively. Results: The measured global temperature and metal abundance are approximately 3.3 keV and 0.24 solar, respectively. From the deprojection analysis, the entropy profile is found to be flat with respect to radius. The entropy within the central region (r < 0.2r_200) is exceptionally high (~400 keV cm^2). Materials and methods: This multicenter, prospective study evaluated 39 eyes (39 AMD patients) enrolled from August 2013 to August 2014 at 12 and 24 months. Only patients with initial best-corrected VA (BCVA) >0.3 logarithm of the minimum angle of resolution (20/40 Snellen) were eligible. Three consecutive monthly IVA injections were followed by 2 monthly injections for 12 months. Thereafter, patients received injections on a treat-and-extend regimen for up to 24 months. Outcome measures included BCVA and central macular thickness (CMT) at 12 and 24 months. Post hoc analysis, BCVA, and CMT were evaluated by AMD types (typical AMD [tAMD], type 1, and type 2 polypoidal choroidal vasculopathy [PCV]). Baseline characteristics and BCVA associations were evaluated with linear regression analysis and Student’s t-test. Results: Mean age was 69 years and 26 of 39 eyes were male. tAMD, type 1 and type 2 PCV occurred in 18, 12, and 9 eyes, respectively. Baseline mean BCVA was 0.097 logarithm of the minimum angle of resolution (20/25 Snellen) and showed significant improvement to 0.058 (20/22 Snellen, P=0.03) at 12 months and 0.066 (20/23) at 24 months. CMT improved significantly from 320 (99) µm (mean [SD]) to 250 (93) µm (P=0.002) at 12 months and 240 (93) µm (P=0.0005) at 24 months. BCVA and CMT were not significantly different among the three groups. Only subretinal hemorrhage (SRH) was significantly associated with improved BCVA. BCVA change from baseline was -0.12 with SRH and -0.011 without SRH (P=0.017) at 12 months. Conclusion: IVA showed good efficacy for exudative AMD with good VA at 24 months. tAMD and type 1 and 2 PCV showed similar prognosis. Baseline SRH predicted favorable long-term vision in AMD with good VA. The following 5 parameters were measured at the foveal center: CMT (distance from the inner limiting membrane [ILM] to Bruch’s membrane); central retinal thickness (CRT; distance from the ILM to the inner limit of the retinal pigment epithelium or subretinal fluid [SRF]); SRF thickness (SRFT); pigment epithelium detachment thickness (PEDT); and CCT. The correlation between the logarithm of the minimum angle of resolution (logMAR) best-corrected visual acuity (BCVA) and the 5 parameters was examined with generalized estimating equations. Results: CMT, CRT, and CCT were negatively correlated with logMAR BCVA (P=0.031, 0.023, and 0.036, respectively) when only CMT values less than the thickness that maximized visual acuity for each eye were used for the analysis. Each 100-µm reduction in CMT, CRT, or CCT improved logMAR BCVA by -0.1, -0.08, or -0.07, respectively. SRFT and PEDT were not correlated with BCVA. We investigated associations between the number of aspiration risk factors present (orientation disturbance, bedridden, chronic cerebrovascular disease, dementia, sleeping medications and gastroesophageal disease) and 30-day and 6-month mortality, and pneumonia recurrence within 30 days.Results: Patients were categorized by number of risk factors present into groups of 0–1, 2, 3, and 4 or more. Of a total of 322 patients, 93 (28.9%) had 0–1 risk factors, 112 (34.8%) had 2, 88 (27.3%) had 3, and 29 (9.0%) had 4 or more risk factors. The percentages of patients with recurrence of pneumonia were 13.0%, 33.0%, 43.2%, and 54.2% in the 0–1, 2, 3, and 4 or more risk factor groups, respectively. The percentages of patients with 30-day mortality were 2.2%, 5.4%, 11.4%, and 24.1%, and those of patients with 6-month mortality were 6.6%, 24.5%, 30.7%, and 50.0%, in the 0–1, 2, 3, and 4 or more risk factor groups, respectively.Conclusions: The number of aspiration risk factors was associated with increases in both mortality and recurrence in pneumonia patients. Therefore, in clinical practice, physicians should consider not only the presence of aspiration risks but also the number of aspiration risk factors in these patients. 20. Impact of menopausal symptoms on presenteeism in Japanese women.

Abstract

Background Menopausal symptoms are common among middle-aged women. Working women with severe menopausal symptoms are more likely to experience presenteeism—a condition where employees continue to work despite feeling unwell. However, it remains unclear as to which specific symptoms women experience during the menopausal transition and postmenopausal periods that primarily contribute to presenteeism. Aims To evaluate the associations between types of menopausal symptoms and presenteeism among Japanese women. Methods A cross-sectional study of 4000 women aged 40–59 years who were currently working was conducted in Japan in September 2022. We used an online self-administered questionnaire that included items on demographic characteristics, the Menopause Rating Scale for measuring menopausal symptoms and the Work Functioning Impairment Scale for measuring presenteeism. Logistic regression analysis was performed. Results Women with severe overall menopausal symptoms had 12.18-fold (95% confidence interval [CI] 9.09–16.33, P  < 0.001) increased odds of presenteeism compared with those without symptoms. Participants with psychological symptoms also had significantly higher presenteeism (severe: odds ratio: 9.18, 95% CI 6.60–12.78, P  < 0.001). However, after controlling for psychological symptoms, there were no significant associations between somatic and urogenital symptoms and presenteeism. Conclusions The results indicate that menopausal symptoms, especially psychological symptoms, have a significant impact on presenteeism among Japanese women. Organizations need to address menopausal symptoms in the workplace, with an emphasis on reducing work-related stress for women with menopausal symptoms. [ABSTRACT FROM AUTHOR]

ObjectivesThe work system reform and the COVID-19 pandemic in Japan have prompted efforts toward telecommuting in Japan, and there has been little research regarding the stress and health effects of telecommuting. This study aimed to clarify the relationship between telecommuting and job stress among Japanese workers.Study DesignThis was a cross-sectional study.MethodsIn December 2020, during the ‘third wave’ of the COVID-19 pandemic, an Internet-based nationwide health survey of 33,087 Japanese workers (CORoNaWork study) was conducted. Data for 27,036 individuals was included as 6,051 individuals provided invalid responses. We analysed a sample of 13,468 office workers from this database. We classified participants into four groups according to telecommuting frequency and compared the subscale of the Job Content Questionnaire and subjective job stress among these groups: high-frequency, medium-frequency, and low-frequency telecommuters group and non-telecommuters group. We used a linear mixed model and ordinal logistic regression analysis.ResultsThere was a significant difference in the score of job control of the JCQ among the four groups after adjusting for multiple confounding factors. The high-frequency telecommuters group had the highest job control score. Regarding the fluctuation score of subjective job stress, the high- and medium-frequency telecommuters groups were significantly lower than those of the non-telecommuters group after adjusting for multiple confounding factors.ConclusionWe found that high-frequency telecommuting was associated with high job control. This study suggests that the widespread adoption of telecommuting as a countermeasure to the public health challenges associated with the COVID-19 pandemic may also have a positive impact on job stress.

Abstract

Objective To examine the pain intensity and pain self-efficacy interaction in workers' work functioning impairment. Methods A cross-sectional study using a self-reporting survey. Of 6657 Japanese construction company employees, 5129 (77.0%) valid responses were analyzed for work functioning impairment. Further analysis using current pain intensity and pain self-efficacy was performed on 1622 employees who experienced physical pain in the past 12 months. Results Pain intensity showed a dose-response relationship with work functioning impairment. Further analysis showed an interaction between pain intensity and pain self-efficacy for work functioning impairment. Stratification by pain self-efficacy showed that high pain self-efficacy was not associated with an increase in work functioning impairment, even in those with severe pain (odds ratio = 1.79; 95% confidence interval = 0.69 to 4.68). Conclusions Pain self-efficacy may be a key determinant for work functioning impairment in workers with pain.

Abstract

ObjectivesThis study examined the association between perceived organizational support (POS) and COVID-19 vaccine intention and the influence of the implementation of workplace infection prevention measures.MethodsWe analyzed 23,846 workers using data from an Internet survey of workers aged 20–65 years conducted in December 2020, during a period of widespread COVID-19 infection in Japan.ResultsA higher POS was associated with a higher intention to vaccinate. The relationship between POS and vaccine intention was attenuated when adjusted for infection prevention measures in the workplace.ConclusionsIn workplaces where POS is present, a sense of responsibility to the group and altruistic behavior may arise. This means employees act to acquire herd immunity to protect others, which may result in increased vaccine intention. The association between POS and vaccination intention was attenuated by adjusting for workplace infection prevention measures, which suggested that infection prevention measures may be a confounding factor or that POS created a health climate that promoted infection prevention measures. The results suggest that working to improve employee well-being and implementing appropriate workplace infection prevention measures during infectious disease outbreaks may promote vaccination behavior and contribute to the acquisition of herd immunity in the community.

Abstract

BackgroundVaccine hesitancy is an issue for vaccines required for herd immunity. Although various factors such as sociodemographics can affect vaccine hesitancy, the research results differ and it is unclear whether these differences depend on the subjects or the situation, such as the type of infection or vaccine. Therefore, we investigated the relationship between seasonal influenza vaccination behavior and sociodemographic factors under the COVID-19 pandemic. In addition, we analyzed the relationship between individual factors of risk preference and seasonal influenza vaccination on the premise that there is a difference in the association between efficacy and the risk of side effects of the two vaccines. MethodsA cross-sectional study was conducted on workers aged 20-65 years on December 22-25, 2020, using data from an Internet survey. We set the presence or absence of 2020/2021 seasonal influenza vaccination as the dependent variable, and each aspect of sociodemographic factors and risk preference as independent variables. We performed a multilevel logistic regression analysis nested by residence. ResultsIn total, 26,637 respondents (13,600 men, 13,037 women) participated. Significantly more women than men were vaccinated. Both men and women had higher vaccination rates if married, highly educated, with high income, and with underlying disease. Men aged 60-65 had a significantly higher vaccination rate than those aged 20-29, while women aged 40-49 and 50-59 had a significantly lower rate than those aged 20-29. These results differed from those of a previous study on the vaccination intentions of the same subjects for COVID-19. Those with low risk preference had higher vaccination rates than those with high risk preference. ConclusionsOur results suggested that the effect of sociodemographic factors on vaccine hesitancy depends on the situation affecting efficacy of the vaccine and the perception of vaccination risk.
