Background. The incidence of gastric ulcer (GU) remains high worldwide with limited prevention. While promising animal experiments have suggested a potential preventive role of folic acid (FA) in the development of gastric ulcers, the lack of robust clinical evidence has hindered its widespread implementation as a preventative measure. Therefore, this research aims to determine the relationship between FA supplementation and GU genetically by Mendelian randomization (MR) approach, in order to establish a foundation for developing more effective preventative strategies for this condition. Methods. Genome-wide association studies (GWAS) investigating the association between folic acid or folate supplementation and gastric ulcers were sourced from the UK Biobank. The primary methods for Mendelian randomization analysis were the inverse variance-weighted (IVW) methods, including fixed-effect and random-effect IVW models. Other methods used to test the robustness of the results included simple model and median, weighted model and median, as well as penalized weighted median. Results. MR analysis was performed to investigate the causal effect of FA adjuvant therapy on GU. Seven single nucleotide polymorphisms (SNPs) of genetic loci associated with FA adjuvant therapy were identified. The random-effect and fixed-effect IVW models revealed that genetically predicted FA complementary therapy was significantly related to the reduction of GU risk (OR, 0.870; 95% CI, 0.826–0.917, p

Robotics have important applications in the field of disaster medical rescue. The deployment of urban rescue robots at the earthquake site can help shorten response time, improve rescue efficiency and keep rescue personnel away from danger. This discussion introduces the performance of some robots in actual rescue scenarios, focuses on the current research status of robots that can provide medical assistance, and analyzes the merits and shortcomings of each system. Based on existing studies, the limitations and development directions of urban rescue robots are also discussed.

The data-driven time-frequency analysis (DDTFA)-method-based initial phase selection directly influences the convergence and calculation results of the algorithm. The central frequency accuracy of each component by means of variational mode decomposition (VMD) is high, the calculation speed of VMD is fast, and the method shows satisfactory noise resistance, but the decomposed components are sensitive to noise. A VMD method that is complementary to the DDTFA method is introduced in this paper to estimate the initial phase of DDTFA, and the VMD-DDTFA method is proposed for time-varying non-stationary signals. This method first analyzes the time-varying non-stationary signal through VMD and estimates the initial phase of each signal component, then uses DDTFA to sparsely decompose the signal after phase smoothing. To examine the analytical ability of VMD-DDTFA on time-varying non-stationary signals, the five aspects of the VMD-DDTFA method, accuracy, noise resistance, efficiency, applicability and anti-mode-mixing ability, are analyzed. The VMD-DDTFA method compares with the current commonly used signal analysis methods of VMD and ensemble empirical mode decomposition (EEMD) and the comparison results confirm that VMD-DDTFA has a superior decomposition accuracy, satisfactory noise resistance and efficiency. In addition, VMD-DDTFA features strong anti-mode mixing and applicability even under strong noise. The VMD-DDTFA method is applied to the fault diagnosis of measured gear crack and broken tooth in variable working conditions. In addition, the results of the VMD-DDTFA method are compared with those obtained by VMD and EEMD; the results verify the effectiveness and superiority of the method.

The development of artificial intelligence technology has promoted the rapid improvement of human-computer interaction. This system uses the Kinect visual image sensor to identify human bone data and complete the recognition of the operator’s movements. Through the filtering process of real-time data by the host computer platform with computer software as the core, the algorithm is programmed to realize the conversion from data to control signals. The system transmits the signal to the lower computer platform with Arduino as the core through the transmission mode of the serial communication, thereby completing the control of the steering gear. In order to verify the feasibility of the theory, the team built a 4-DOF robotic arm control system and completed software development. It can display other functions such as the current bone angle and motion status in real time on the computer operation interface. The experimental data shows that the Kinect-based motion recognition method can effectively complete the tracking of the expected motion and complete the grasping and transfer of the specified objects, which has extremely high operability.

Network traffic classification (NTC) plays an important role in network measurement and management. Recently, Artificial Intelligence (AI) based NTC has been widely considered a good candidate because of its accuracy in processing both clear and encrypted data traffic. However, those AI-based NTC schemes usually apply full-length packets, e.g., through padding. Such lengthy inputs can lead to complex designs of NTC models, which can be challenging to network devices. To tackle this issue, we propose a byte importance distillation scheme to extract the packet payload bytes that contribute the most to traffic classification results. In the proposed scheme, a Bayesian Multilayer Perception (BMLP) is first initialized based on full-size data packets. The byte importance is then defined as the corresponding mean absolute weights in the first layer of a BMLP model with K-integration. By choosing the important bytes, input data packets can be reduced dramatically to a few bytes that contribute the most to the classification output. Two popular AI-based NTC models, i.e., MLP based and CNN based, are implemented to evaluate the proposed byte importance distillation scheme. The results demonstrate that the data packets optimized from the proposed scheme can speed up the MLP and CNN based NTC models by one to two magnitudes, depending on the implementation platforms, while maintaining high classification accuracy. In comparison, packets that are reduced to the same size through traditional dimension reduction approaches such as principal component analysis and convolutional block attention module cannot maintain high classification accuracy of the AI-based NTCs.

Introduction: Sperm DNA integrity has been considered as one of the important determinants of normal fertilization and embryonic development in natural and assisted pregnancy. It is difficult for men with high levels of sperm DNA fragmentation (SDF) in semen to conceive their partners naturally and assist in conception. The studies have found that the level of SDF in the semen of patients with varicocele (VC) was on the high side. In recent years, the effect of VC surgery on DNA fragmentation index has attracted the attention of researchers. In this study, we will evaluate the effectiveness of VC repair as a way to alleviate SDF and improve male fertility. Methods and analysis: Electronic databases including English databases (PubMed, MEDLINE, EMBASE, Web of Science, Cochrane Library) and Chinese databases (China National Knowledge Infrastructure, China Biology Medicine Database, Wanfang Database, VIP Database) will be searched from their inception to December 2020 to recognize related studies. All the randomized controlled trials of microsurgical varicocelectomy for the management of VC patients will be included. The potential outcome will include improvement in SDF, oxidative stress markers (reactive oxygen species, nitric oxide, and lipid peroxidation products), sperm chromatin compaction, other advanced sperm function characteristics, follow-up of fertility results. We will conduct this study strictly according to the Cochrane Handbook for Systematic Reviews of Interventions. Results: The study is a protocol for systematic review and meta-analysis without results, and data analysis will be carried out after the protocol. We will share our findings on April 5th of 2021. Conclusion: This systematic review will provide more evidence to assess whether varicocelectomy is an effective intervention for patients with SDF. The results will be published in a public issue journal and offer the urologists help to make clinical decisions. Ethics and dissemination: Formal ethical approval is not required in this protocol. We will collect and analyze data based on published research. Since this research does not involve patients, personal privacy will not be affected. Varicocele is defined as the dilation of the pampiniform venous plexus draining the testicle. At present, selective serotonin reuptake inhibitors antidepressants, topical anesthetics, tramadol, phosphodiesterase type 5 inhibitors are the common alternative strategy to improve PE. However, these therapeutic measures have several shortcomings and side effects. Recently, the correlation between varicocele and PE has attracted the attention of some researchers. A few studies consider microsurgical varicocelectomy can be a new remedy for PE. But it is still absent enough a great deal of convincing evidence. The study will assess the effectiveness and safety of the microsurgical varicocelectomy treatment in PE patients. Methods and analysis: Electronic databases including English databases (PubMed, MEDLINE, EMBASE, Web of Science, Cochrane Library) and Chinese databases (China National Knowledge Infrastructure, China Biology Medicine Database, Wanfang Database, VIP Database) will be searched from their inception to December 2020 to recognize related studies. All the randomized controlled trials of microsurgical varicocelectomy for the management of PE patients will be included. The potential outcome will include intravaginal ejaculation latency time, Chinese index of sexual function for premature ejaculation-5, visual analogue score, premature ejaculation diagnostic tool, success treatment rate, serum testosterone levels. We will conduct this study strictly according to the Cochrane Handbook for systematic reviews of interventions. Results: The current study is a protocol for systematic review and meta-analysis without results, and data analysis will be carried out after the protocol. Network intrusion detection is the fundamental of the Cybersecurity which plays an important role in preventing the systems away from malicious network traffic. Recent Artificial Intelligence (AI) based intrusion detection systems provide simple and accurate intrusion detection compared with the conventional intrusion detection schemes, however, the detection performance may not be reliable because the models in the AI algorithms must output a prediction result for each incoming instance even when the models are not confident. To tackle the issue, we propose to adopt Bayesian Deep Learning, specifically, Bayesian Convolutional Neural Network, to build intrusion detection models. Moreover, an ensemble-based detection scheme is further proposed to enhance the detection performance. Two open datasets (i.e., NSL-KDD and UNSW-NB15) are used to evaluate the proposed schemes. In comparison, Convolutional Neural Network and Support Vector Machine are implemented as baseline IDS (i.e., CNN-IDS and SVM-IDS). The evaluation results demonstrate that the proposed BCNN-IDS can significantly boost the detection accuracy and reduce the false alarm rate by adopting the proposed T-ensemble detection scheme.

Network traffic classification has been widely studied to fundamentally advance network measurement and management. Machine Learning is one of the effective approaches for network traffic classification. Specifically, Deep Learning (DL) has attracted much attention from the researchers due to its effectiveness even in encrypted network traffic without compromising neither user privacy nor network security. However, most of the existing models are created from closed-world datasets, thus they can only classify those existing classes previously sampled and labeled. In this case, unknown classes cannot be correctly classified. To tackle this issue, an autonomous learning framework is proposed to effectively update DL-based traffic classification models during active operations. The core of the proposed framework consists of a DL-based classifier, a self-learned discriminator, and an autonomous self-labeling model. The discriminator and self-labeling process can generate new dataset during active operations to support classifier update. Evaluation of the proposed framework is performed on an open dataset, i.e., ISCX VPN-nonVPN, and independently collected data packets. The results demonstrate that the proposed autonomous learning framework can filter packets from unknown classes and provide accurate labels. Thus, corresponding DL-based classification models can be updated successfully with the autonomously generated dataset.

Background Immunotherapy is important for the treatment of esophagogastric cancer. The purpose of this study is to compare the efficacy and safety of PD-(L)1 antibody, chemotherapy, and supportive treatment in the management of pretreated advanced esophagogastric cancer. Methods The randomized controlled trials were identified by searching electronic databases including PubMed, Cochrane Library and Embase database. The network meta-analysis (NMA) was carried out using software R 3.3.2. Main outcomes including overall survival (OS), progression-free survival (PFS), all grades and serious treatment-related adverse events (TRAEs) were extracted and analyzed. The ranking results for all outcomes were performed to identify the best treatments. Results Seven high-quality RCTs involving 1,891 patients were taken into analysis. Compared with supportive treatment, PD-(L)1 antibody and chemotherapy both had a significantly longer OS time. Chemotherapy could obvious improve PFS than supportive treatment, but it had more all grades and serious TRAEs than PD-(L)1 antibody and supportive treatment. No significant difference was found in other comparisons. The probabilities of rank plot showed that PD-(L)1 antibody was the best in the outcome of OS. Chemotherapy ranked first in PFS and ranked last in all grades and serious TRAEs. Conclusions According to our results, PD-(L)1 antibody had excellent survival benefits and tolerable TRAEs for pretreated advanced esophagogastric cancer. It might be a suitable potential choice, especially for patients with high PDL1 CPS or with gastroesophageal junction cancer.

Background: The incidence of male infertility is increasing worldwide, and has become an important problem that plagues many married couples. Half of the infertility cases have induced by male infertility. Wuzi Yanzong Pill is a traditional Chinese herbal formula used in treating spermatorrhea, premature ejaculation, erectile dysfunction, lumbago and male sterility widely. Therefore, in this systematic review, we design to evaluate the effectiveness and safety of Wuzi-Yanzong Pill for the treatment of male infertility. Methods: The English and Chinese literature published before June 30, 2020 will be searched in PubMed, EMBASE, Cochrane library, and Chinese literature in China National Knowledge Infrastructure, Chinese biomedical document service system, VIP Chinese Science and Technology Journal Database, WANFANG data. All related randomized controlled trials that meet the eligibility criteria will be included and other studies will be excluded. We will search literature with text keywords “male infertility” or “sperm” or “semen” and “Wuzi Yanzong Pill” or “Wuziyangzong” or “WZYZ”. Progressive motility, sperm concentration, sperm morphology, sperm viability, sperm DNA fragmentation, sperm number per ejaculate, pregnancy rates will be evaluated. RevMan 5.3 and Stata 14.0 will be used to conduct this systematic review. The preferred reporting items for systematic reviews and meta-analysis protocols statement is followed in this protocol and the PRISMA statement will be followed in the completed systematic review. Conclusion: The efficacy and safety of Wuzi Yanzong Pill in the treatment of male infertility will be e evaluated. The results of this review may provide some help for the clinician's decision.

Background Erectile dysfunction is a common disease. It affects the quality of life of both husband and wife and its prevalence is higher in patients with overt cardiovascular disease or cardiovascular risk factors. In recent years, multiple studies confirm that nebivolol exerts protective effects on erectile function against the disruptive effects of cardiopulmonary bypass in patients undergoing coronary artery bypass grafting, but its quality and efficacy have not been systematically evaluated. Therefore, it is necessary to carry out a systematic review and meta-analysis to fully evaluate the efficacy and safety of nebivolol on erectile function in the cases with coronary artery bypass grafting. Methods and analysis Chinese and English literature of nebivolol on erectile function in the cases with coronary artery bypass surgery published before August 31, 2020 will be comprehensive searched in PubMed, Cochrane Library, EMBASE, WANFANG, China National Knowledge Infrastructure, VIP Chinese Science and Technology Journal Database, Chinese biomedical document service system, and Clinicaltrials.gov. Only randomized controlled trials that meet the eligibility criteria will be included. Two researchers will independently complete literature screening, data extraction and assess the risk of bias, and the third investigator will handle disagreements. Our main evaluation includes 2 outcome indicators including the international index of erectile function 5 score and adverse events. RevMan 5.3 and Stata 14.0 will be used to conduct this systematic review. The preferred reporting items for systematic reviews and meta-analysis protocols (PRISMA-P) statement is followed in this protocol and the PRISMA statement will be followed in the completed systematic review. Conclusion and dissemination The efficacy and safety of nebivolol on erectile function in the cases with coronary artery bypass grafting will be evaluated. We will publish the results of this systematic review in peer-reviewed journals to provide new evidence to clinicians. Introduction: Diabetic erectile dysfunction (DED) has gradually become a worldwide problem. Due to the mechanism of DED is not clear, it is impossible to treat it pertinently. Recently, some studies have shown that vitamin D is associated with DED, type 2 diabetes mellitus (T2DM) and erectile dysfunction (ED), but there is no systematic review and meta-analysis on the relationship between vitamin D and DED. Methods and analysis: The databases of English databases (PubMed, MEDLINE, EMBASE, Web of Science, Cochrane Library) and Chinese databases (China National Knowledge Infrastructure, China Biology Medicine Database, Wanfang Database, VIP Database) will be retrieved. The search strategy that will be run in the PubMed and tailored to the other database when necessary is presented in Table 1Table 1PubMed search strategy. . RevMan 5.3 and Stata 11.0 will be used for Systematic Review and Meta-analysis. This protocol reported under the Preferred Reporting ltems for Systematic Reviews and Meta-Analyses Protocols (PRISMA-P) statement, and we will report the systematic review by following the PRISMA statement. Results: Through a systematic review, and meta-analysis when necessary, we can obtain the relationship between vitamin D and DED. We will share our findings in the third quarter of 2021. Conclusion: The association between serum vitamin D levels and type 2 diabetic erectile dysfunction will be assessed. Besides, the results of this review may provide some help for clinicians to make decisions. Besides, the choice of wedge points is concerned with the horizonal resistance ratio of soil (m values) and the rigidity of the floor. Through some simpli-fied models, this paper illustrates that different choices of stiffness and m values how to impact on wage points.
