This article examines the prognostic significance of Crooke cell adenomas (CCA), a rare subtype of corticotroph pituitary neuroendocrine tumor (PitNET)/adenoma. The study compares CCAs to non-CCA corticotroph PitNET/adenomas in terms of time-to-recurrence (TTR). The results show that while CCAs are associated with larger tumors and cavernous sinus invasion, they do not demonstrate a higher likelihood of recurrence when controlling for other known prognostic factors. The study suggests further investigation into whether CCAs have higher recurrence rates due to their aggressive phenotype or their propensity to invade the cavernous sinus. Reversible cerebral vasoconstriction syndrome (RCVS) can present with hemorrhage, ischemia, or both. We aim to compare the differences in presentation and outcomes between patients with RCVS.The hemorrhagic presentation group had 58 patients, and the non-hemorrhagic presentations had 30 patients. Subgroup analysis compared patients with evidence of one or more types of hemorrhage (n = 53), no evidence of hemorrhage or infarction (n = 23), evidence of infarction only (n = 7), and combination of hemorrhage and infarction (n = 5). Clinical and radiographic data were analyzed.Migraine (p = 0.030) and intracranial tumors (p = 0.004) were more frequent in non-hemorrhagic presentation. Seizures on admission (p = 0.047) and higher than average C-reactive protein (CRP) (p = 0.037) were seen at a higher rate in patients with hemorrhagic presentation. RCVS2 scores were not unexpectedly higher in patients with hemorrhage than non-hemorrhagic presentations (p = 0.010). Outcomes between the hemorrhagic and non-hemorrhagic groups were comparable. Subgroup analysis found a higher subset of patients with opiate use (p = 0.046) in the hemorrhage-only group. Patients with hemorrhage presented with a thunderclap headache (p < 0.001) more often when compared to the other three groups. RCVS2 score was not unexpectedly higher in the hemorrhage-only group compared with the other groups (p = 0.004).A history of migraines was associated with ischemia, while intracranial tumor was significantly associated with evidence of either an infarct, or no changes on imaging. Exposure to opiates, and seizures or thunderclap headache on presentation were associated with hemorrhage. If our data are reproducible, the RCVS2 score may benefit from inclusion of other, small hemorrhages as criterion for diagnosis for RCVS. [ABSTRACT FROM AUTHOR]

Recent scholarly work has focused on borders, both geopolitical and cultural, giving attention to border lives and identities. This article, which focuses on sociocultural practices, addresses what can be calledborder languaculture. With attention on the Texas-Mexico border in North America, the authors discuss three key features of hybrid practices on the US ‘side’: (1) code-mixing that has a strong rhetorical component; (2) cross-border continuation but transformation of ‘traditional’ forms and conventions associated historically with Spain and Mexico; and (3) creation of ‘new’ hybrid forms and cultural products that establish social affiliations and distinctions. These languacultural features support the central point of the article: that self-positioning in this borderland is characterized by the multiplicity, contradictions and syncretism now being labeledmestizaje. Although cultural change has often been explained throughacculturation, the syncretic nature of these border practices is better explained throughtransculturation, which emphasizes mutual influences and shifting power relations. Background: Higher risk of secondary brain tumor, carotid stenosis and stroke has been reported after conventional sella irradiation for pituitary neuroendocrine tumors (PitNET). Stereotactic radiosurgery (SRS), which is a more focused approach, is now increasingly used instead. The aim was to assess the risk of secondary brain tumor, carotid stenosis/occlusion and stroke after SRS., Methods: In this multicentric retrospective study, 2,254 patients with PitNET were studied, 1,377 in the exposed group and 877 in the control group., Results: There were 9,840.1 patient-years at risk for the SRS and 5,266.5 for the control group. The 15-year cumulative probability of secondary intracranial tumor was 2.3% (95%CI:0.5%, 4.1%) for SRS and 3.7% (95%CI:0%, 8.7%) for the control group (p=0.6), with an incidence rate of 1.32 per 1,000 and 0.95 per 1,000, respectively. SRS was not associated with increased risk of tumorigenesis when stratified by age (HR: 1.59 [95%CI: 0.57, 4.47], p=0.38). The 15-year probability of new carotid stenosis/occlusion was 0.9% (95%CI: 0.2, 1.6) in the SRS and 2% (95%CI: 0, 4.4) in the control group (p=0.8). The 15-year probability of stroke was 2.6% (95%CI: 0.6%, 4.6%) in the SRS and 11.1% (95%CI: 6%, 15.9%) in the control group (p<0.001). In cox multivariate analysis stratified by age, SRS (HR 1.85[95%CI:0.64, 5.35], p=0.26) was not associated with risk of new stroke., Conclusion: No increased risk of long-term secondary brain tumor, new stenosis or occlusion and stroke was demonstrated in SRS group compared to control in this study with imaging surveillance., (© The Author(s) 2024.
