3. A web-based annotation tool for clinical trial failure reasons

- Abstract
Clinical trials are currently the best tool in determining the safety and therapeutic efficacy of interventions. Many trials fail due to different reasons such as a lack of funding, recruitment issues, treatment futility, and many more. Trial registries like ClinicalTrials.gov and the EU clinical trials register provide digital descriptions of trials and outcomes. Failure reasons for trials are at best available as full text information and not easily analyzed but would benefit policy-making as well as drug-repurposing efforts as negative training data among other things. Here we describe a novel web-based annotation platform for failure reasons of clinical trials. As of September 23rd 2022 the database contains 14,232 clinical trials meeting the failed trial criteria of which 7,329 (51.5%) are already annotated. These annotations already allow basic assumptions, however, more annotation and consensus work is needed., Klinische Studien sind derzeit das beste Instrument, um die Sicherheit und therapeutische Wirksamkeit von Interventionen zu bestimmen. Viele Studien scheitern aus unterschiedlichen Gründen wie fehlender Finanzierung, Problemen bei der Rekrutierung, fehlender Wirksamkeit der Behandlung und weiteren. Studienregister wie ClinicalTrials.gov und das EU-Register für klinische Studien bieten digitale Beschreibungen von Studien und Ergebnissen. Die Gründe für das Scheitern von Studien sind bestenfalls als Volltext-Informationen verfügbar und schwierig zu analysieren, würden aber unter anderem als negative Trainingsdaten dem Drug-Repurposing zugutekommen. Hier beschreiben wir eine neuartige, webbasierte Annotationsplattform für klinische Studien, um die Gründe des Scheiterns zu kategorisieren. Stand 23. September 2022 enthält die Datenbank 14.232 klinische Studien, welche die Kriterien für fehlgeschlagene Studien erfüllen, von denen 7.329 (51,5%) bereits kommentiert sind. Diese Annotationen erlauben bereits grundlegende Annahmen, jedoch ist weitere Annotations- und Konsensarbeit erforderlich.

- Abstract
The CMS detector at the CERN LHC features a silicon pixel detector as its innermost subdetector. The original CMS pixel detector has been replaced with an upgraded pixel system (CMS Phase-1 pixel detector) in the extended year-end technical stop of the LHC in 2016/2017. The upgraded CMS pixel detector is designed to cope with the higher instantaneous luminosities that have been achieved by the LHC after the upgrades to the accelerator during the first long shutdown in 2013-2014. Compared to the original pixel detector, the upgraded detector has a better tracking performance and lower mass with four barrel layers and three endcap disks on each side to provide hit coverage up to an absolute value of pseudorapidity of 2.5. This paper describes the design and construction of the CMS Phase-1 pixel detector as well as its performance from commissioning to early operation in collision data-taking.

CMS collaboration: et al., The CMS detector at the CERN LHC features a silicon pixel detector as its innermost subdetector. The original CMS pixel detector has been replaced with an upgraded pixel system (CMS Phase-1 pixel detector) in the extended year-end technical stop of the LHC in 2016/2017. The upgraded CMS pixel detector is designed to cope with the higher instantaneous luminosities that have been achieved by the LHC after the upgrades to the accelerator during the first long shutdown in 2013–2014. Compared to the original pixel detector, the upgraded detector has a better tracking performance and lower mass with four barrel layers and three endcap disks on each side to provide hit coverage up to an absolute value of pseudorapidity of 2.5. Estuaries and the coastal ocean experience a high degree of variability in the composition and concentration of particulate and dissolved organic matter (DOM) as a consequence of riverine and estuarine fluxes of terrigenous DOM, sediments, detritus and nutrients into coastal waters and associated phytoplankton blooms. Our approach integrates biogeochemical measurements, optical properties and remote sensing to examine the distributions and inventories of organic carbon in the U.S. Middle Atlantic Bight and Gulf of Maine. Algorithms developed to retrieve colored DOM (CDOM), Dissolved (DOC) and Particulate Organic Carbon (POC) from NASA's MODIS-Aqua and SeaWiFS satellite sensors are applied to quantify the distributions and inventories of DOC and POC. Horizontal fluxes of DOC and POC from the continental margin to the open ocean are estimated from SeaWiFS and MODIS-Aqua distributions of DOC and POC and horizontal divergence fluxes obtained from the Northeastern North Atlantic ROMS model. SeaWiFS and MODIS imagery reveal the importance of estuarine outflow to the export of CDOM and DOC to the coastal ocean and a net community production of DOC on the shelf.

Continental shelves are believed to play a major role in carbon cycling due to their high productivity. Particulate organic carbon (POC) burial has been included in models as a carbon sink, but we show here that seasonally produced dissolved organic carbon (DOC) on the shelf can be exported to the open ocean by horizontal transport at similar rates (1-2 mol C/sq m/yr) in the southern U.S. Mid-Atlantic Bight (MAB). The dissolved organic matter (DOM) model imbedded in a coupled circulation-biogeochemical model reveals a double dynamics: the progressive release of dissolved organic nitrogen (DON) in the upper layer during summer increases the regenerated primary production by 30 to 300%, which, in turns ; enhances the DOC production mainly from phytoplankton exudation in the upper layer and solubilization of particulate organic matter (POM) deeper in the water column. This analysis suggests that DOM is a key element for better representing the ecosystem functioning and organic fluxes in models because DOM (1) is a major organic pool directly related to primary production, (2) decouples partially the carbon and nitrogen cycles (through carbon excess uptake, POM solubilization and DOM mineralization) and (3) is intimately linked to the residence time of water masses for its distribution and export.

- Abstract
Understanding and predicting the ecological consequences of different management alternatives is becoming increasingly important to support environmental management decisions. Ecological models could contribute to such predictions, but in the past this was often not the case. Ecological models are often developed within research projects but are rarely used for practical applications. In this synthesis paper, we discuss how to strengthen the role of ecological modeling in supporting environmental management decisions with a focus on methodological aspects. We address mainly ecological modellers but also potential users of modeling results. Various modeling approaches can be used to predict the response of ecosystems to anthropogenic interventions, including mechanistic models, statistical models, and machine learning approaches. Regardless of the chosen approach, we outline how to better align the modeling to the decision making process, and identify six requirements that we believe are important to increase the usefulness of ecological models for management support, especially if management decisions need to be justified to the public. These cover: (i) a mechanistic understanding regarding causality, (ii) alignment of model input and output with the management decision, (iii) appropriate spatial and temporal resolutions, (iv) uncertainty quantification, (v) sufficient predictive performance, and (vi) transparent communication. We discuss challenges and synthesize suggestions for addressing these points. © 2019 The Author(s)

Aim: Systematic conservation planning is vital for allocating protected areas given the spatial distribution of conservation features, such as species. Due to incomplete species inventories, species distribution models (SDMs) are often used for predicting species habitat suitability and species probability of occurrence. Currently, SDMs mostly ignore spatial dependencies in species and predictor data. Here, we provide a comparative evaluation of how accounting for spatial dependencies, that is, autocorrelation, affects the delineation of optimized protected areas. Location: Southeast Australia, Southeast U.S. Continental Shelf, Danube River Basin. Methods: We employ Bayesian spatially explicit and non-spatial SDMs for terrestrial, marine and freshwater species, using realm-specific planning unit shapes (grid, hexagon and subcatchment, respectively). We then apply the software gurobi to optimize conservation plans based on species targets derived from spatial and non-spatial SDMs (10% 50% each to analyse sensitivity), and compare the delineation of the plans. Results: Across realms and irrespective of the planning unit shape, spatially explicit SDMs (a) produce on average more accurate predictions in terms of AUC, TSS, sensitivity and specificity, along with a higher species detection probability. All spatial optimizations meet the species conservation targets. Spatial conservation plans that use predictions from spatially explicit SDMs (b) are spatially substantially different compared to those that use non-spatial SDM predictions, but (c) encompass a similar amount of planning units. The overlap in the selection of planning units is smallest for conservation plans based on the lowest targets and vice versa. Main conclusions: Species distribution models are core tools in conservation planning. Sea turtle stranding events provide an opportunity to study drivers of mortality, but causes of strandings are poorly understood. A general sea turtle carcass oceanographic drift model was developed to estimate likely mortality locations from coastal sea turtle stranding records. Key model advancements include realistic direct wind forcing on carcasses, temperature driven carcass decomposition and the development of mortality location predictions for individual strandings. We applied this model to 2009-2014 stranding events within the Chesapeake Bay, Virginia. Predicted origin of vessel strike strandings were compared to commercial vessel data, and potential hazardous turtle-vessel interactions were identified in the southeastern Bay and James River. Commercial fishing activity of gear types with known sea turtle interactions were compared to predicted mortality locations for stranded turtles with suggested fisheries-induced mortality. Probable mortality locations for these strandings varied seasonally, with two distinct areas in the southwest and southeast portions of the lower Bay. Spatial overlap was noted between potential mortality locations and gillnet, seine, pot, and pound net fisheries, providing important information for focusing future research on mitigating conflict between sea turtles and human activities. Our ability to quantitatively assess spatial and temporal overlap between sea turtle mortality and human uses of the habitat were hindered by the low resolution of human use datasets, especially those for recreational vessel and commercial fishing gear distributions. This study highlights the importance of addressing these data gaps and provides a meaningful conservation tool that can be applied to stranding data of sea turtles and other marine megafauna worldwide.

Applying the workflow allows identification of gaps in habitat protection, followed by the prioritization of potential areas of high protection value using the conservation planning software Marxan. We found that, in Germany, N2000 covers all target habitats. However, common habitats were proportionally underrepresented relative to rare ones, which contrasts with studies focussing on the representation of species. Moreover, the German case study suggests that especially highly protected areas (i.e. covered by more than 90% with N2000 sites) build an excellent basis towards a cost-effective and efficient conservation network. Our workflow provides a generic approach to deal with the common problem of missing habitat distribution data outside of N2000 sites, information which is however crucial for managers to plan conservation actions appropriately across Europe. - Abstract
Background: Dysphagia after stroke is common, especially in severely affected patients who have had a tracheotomy. In a pilot trial, pharyngeal electrical stimulation (PES) improved swallowing function in this group of patients. We aimed to replicate and extend this single-centre experience. Methods: We did a prospective, single-blind, randomised controlled trial across nine sites (seven acute care hospitals, two rehabilitation facilities) in Germany, Austria, and Italy. Patients with recent stroke who required tracheotomy were randomly assigned to receive 3 days of either PES or sham treatment (1:1). All patients had the stimulation catheter inserted; sham treatment was applied by connecting the PES base station to a simulator box instead of the catheter. Randomisation was done via a computerised interactive system (stratified by site) in blocks of four patients per site. Patients and investigators applying PES were not masked. The primary endpoint was assessed by a separate investigator at each site who was masked to treatment assignment. The primary outcome was readiness for decannulation 24–72 h after treatment, assessed using fibreoptic endoscopic evaluation of swallowing and based on a standardised protocol, including absence of massive pooling of saliva, presence of one or more spontaneous swallows, and presence of at least minimum laryngeal sensation. We planned a sequential statistical analysis of superiority for the primary endpoint. Interim analyses were to be done after primary outcome data were available for 50 patients (futility), 70 patients, and every additional ten patients thereafter, up to 140 patients. Analysis was by intention to treat. This trial is registered with the ISRCTN registry, number ISRCTN18137204. Findings: From May 29, 2015, to July 5, 2017, of 81 patients assessed, 69 patients from nine sites were randomly assigned to receive PES (n=35) or sham (n=34) treatment. 33. Challenges of modeling depth-integrated marine primary productivity over multiple decades: A case study at BATS and HOT

- Abstract
The performance of 36 models (22 ocean color models and 14 biogeochemical ocean circulation models (BOGCMs)) that estimate depth-integrated marine net primary productivity (NPP) was assessed by comparing their output to in situ 14C data at the Bermuda Atlantic Time series Study (BATS) and the Hawaii Ocean Time series (HOT) over nearly two decades. Specifically, skill was assessed based on the models' ability to estimate the observed mean, variability, and trends of NPP. At both sites, more than 90% of the models underestimated mean NPP, with the average bias of the BOGCMs being nearly twice that of the ocean color models. However, the difference in overall skill between the best BOGCM and the best ocean color model at each site was not significant. Between 1989 and 2007, in situ NPP at BATS and HOT increased by an average of nearly 2% per year and was positively correlated to the North Pacific Gyre Oscillation index. The majority of ocean color models produced in situ NPP trends that were closer to the observed trends when chlorophyll-a was derived from high-performance liquid chromatography (HPLC), rather than fluorometric or SeaWiFS data. However, this was a function of time such that average trend magnitude was more accurately estimated over longer time periods. Among BOGCMs, only two individual models successfully produced an increasing NPP trend (one model at each site). We caution against the use of models to assess multiannual changes in NPP over short time periods. Ocean color model estimates of NPP trends could improve if more high quality HPLC chlorophyll-a time series were available. © 2010 by the American Geophysical Union.

- Abstract
We investigated 32 net primary productivity (NPP) models by assessing skills to reproduce integrated NPP in the Arctic Ocean. The models were provided with two sources each of surface chlorophyll-a concentration (chlorophyll), photosynthetically available radiation (PAR), sea surface temperature (SST), and mixed-layer depth (MLD). The models were most sensitive to uncertainties in surface chlorophyll, generally performing better with in situ chlorophyll than with satellite-derived values. They were much less sensitive to uncertainties in PAR, SST, and MLD, possibly due to relatively narrow ranges of input data and/or relatively little difference between input data sources. Regardless of type or complexity, most of the models were not able to fully reproduce the variability of in situ NPP, whereas some of them exhibited almost no bias (i.e., reproduced the mean of in situ NPP). The models performed relatively well in low-productivity seasons as well as in sea ice-covered/deep-water regions. Depth-resolved models correlated more with in situ NPP than other model types, but had a greater tendency to overestimate mean NPP whereas absorption-based models exhibited the lowest bias associated with weaker correlation. The models performed better when a subsurface chlorophyll-a maximum (SCM) was absent. As a group, the models overestimated mean NPP, however this was partly offset by some models underestimating NPP when a SCM was present. Our study suggests that NPP models need to be carefully tuned for the Arctic Ocean because most of the models performing relatively well were those that used Arctic-relevant parameters.

- Abstract
Strong and strategic collaborations among experts from academia, federal operational centers, and industry have been forged to create a U.S. IOOS Coastal and Ocean Modeling Testbed (COMT). The COMT mission is to accelerate the transition of scientific and technical advances from the coastal and ocean modeling research community to improved operational ocean products and services. This is achieved via the evaluation of existing technology or the development of new technology depending on the status of technology within the research community. The initial phase of the COMT has addressed three coastal and ocean prediction challenges of great societal importance: estuarine hypoxia, shelf hypoxia, and coastal inundation. A fourth effort concentrated on providing and refining the cyberinfrastructure and cyber tools to support the modeling work and to advance interoperability and community access to the COMT archive. This paper presents an overview of the initiation of the COMT, the findings of each team and a discussion of the role of the COMT in research to operations and its interface with the coastal and ocean modeling community in general. Detailed technical results are presented in the accompanying series of 16 technical papers in this special issue.

New 3-D seismic investigations carried out across the Sevastopol mud volcano in the Sorokin Trough present 3-D seismic data of a mud volcano in the Black Sea for the first time. The studies allow us to image the complex three-dimensional morphology of a collapse structured mud volcano and to propose an evolution model. The Sevastopol mud volcano is located above a buried diapiric structure with two ridges and controlled by fluid migration along a deep fault system, which developed during the growth of the diapirs in a compressional tectonic system. Overpressured fluids initiated an explosive eruption generating the collapse depression of the Sevastopol mud volcano. Several cones were formed within the depression by subsequent quiet mud extrusions. Although gas hydrates have been recovered at various mud volcanoes in the Sorokin Trough, no gas hydrates were sampled at the Sevastopol mud volcano. A BSR (bottom-simulating reflector) is missing in the seismic data; however, high-amplitude reflections (bright spots) observed above the diapiric ridge near the mud volcano at a relatively constant depth correspond to the approximate depth of the base of the gas hydrate stability zone (BGHSZ). Thus we suggest that gas hydrates are present locally where gas/fluid flow occurs related to mud volcanism, i.e., above the diapir and close to the feeder channel of the mud volcano. Depth variations of the bright spots of up to 200 ms TWT might be caused by temperature variations produced by variable fluid flow.
