The ABA pain guidelines were developed 14 years ago and have not been revised despite evolution in the practice of burn care. A sub-committee of the American Burn Association's Committee on the Organization and Delivery of Burn Care was created to revise the adult pain guidelines. A MEDLINE search of English-language publications from 1968 to 2018 was conducted using the keywords "burn pain," "treatment," and "assessment." Selected references were also used from the greater pain literature. Studies were graded by two members of the committee using Oxford Centre for Evidence-based Medicine-Levels of Evidence. We then met as a group to determine expert consensus on a variety of topics related to treating pain in burn patients. Finally, we assessed gaps in the current knowledge and determined research questions that would aid in providing better recommendations for optimal pain management of the burn patient. The literature search produced 189 papers, 95 were found to be relevant to the assessment and treatment of burn pain. From the greater pain literature 151 references were included, totaling 246 papers being analyzed. Following this literature review, a meeting to establish expert consensus was held and 20 guidelines established in the areas of pain assessment, opioid medications, nonopioid medications, regional anesthesia, and nonpharmacologic treatments. There is increasing research on pain management modalities, but available studies are inadequate to create a true standard of care. We call for more burn specific research into modalities for burn pain control as well as research on multimodal pain control.

ObjectivesStudies suggest that a restrictive transfusion strategy is safe in burns, yet the efficacy of a restrictive transfusion policy in massive burn injury is uncertain. Our objective: compare outcomes between massive burn (≥60% total body surface area (TBSA) burn) and major (20-59% TBSA) burn using a restrictive or a liberal blood transfusion strategy.MethodsPatients with burns ≥20% were block randomized by age and TBSA to a restrictive (transfuse hemoglobin 0.05).ConclusionsA restrictive transfusion strategy may be beneficial in massive burns in reducing ventilator days, ICU days and blood utilization, but does not decrease infection, mortality, hospital LOS or wound healing.

Emotion involves oneself in relation to a subject of attention; e.g., sadness is to be sad about something/someone. This study examined emotional responses to perceiving a loss of connection from oneself. Evidence suggests that Europeans tend to perceive salient objects in the foreground, while East Asians are more likely to perceive holistically, considering the interrelationships between the context and the object. We studied how this distinction affected European Americans' (EA) and Chinese Americans' (CA) sensitivity to perceiving the loss of connection. Both groups were exposed to loss by playing Cyberball, a balltossing video game, and then watched a film clip on grief. We hypothesized that EA would respond with increasing heart rate (HR) variance around the mean when perceiving loss. CA were predicted to show no difference from controls. We also hypothesized that EA would feel sadder, in terms of decreased HR and increased respiratory sinus arrhythmia (RSA), earlier during the film clip. In total, 53 subjects were recruited, of which 40 were EA (47.5% women, age 21.08 ± 1.94 years) and 13 were CA (61.5% women, age 21.05 ± 1.74 years); 25 subjects (19 EA, 6 CA) received 2 out of 48 balls tossed in Cyberball and the controls received 10. ECG, respiration, and facial electromyography (fEMG) data were acquired. The results during Cyberball showed that EA's HR variance relative to baseline (HR SDc/b) had an upward trend on perceiving loss. Contrary to prediction, CA also showed higher levels of HR variance relative to baseline. The ANOVA of HR SDc/b revealed that the interaction effect of two factors, time and condition, was statistically significant (p = 0.009). However, as predicted, EA in the experimental condition had decreased HR and increased RSA, a sign of withdrawal in sadness, 30 to 60 s into the sad clip. fEMG data at the corrugator muscle revealed that EA activated higher peak intensity 5.5 s earlier than CA (increased 1.571 vs. 0.844). This difference, however, was not statistically significant. The evidence suggests that increased exposure to loss automatically led to increased HR variance in both groups even when subjects were informed that players were computer-generated. ObjectiveOur objective was to compare outcomes of a restrictive to a liberal red cell transfusion strategy in 20% or more total body surface area (TBSA) burn patients. We hypothesized that the restrictive group would have less blood stream infection (BSI), organ dysfunction, and mortality.BackgroundPatients with major burns have major (>1 blood volume) transfusion requirements. Studies suggest that a restrictive blood transfusion strategy is equivalent to a liberal strategy. However, major burn injury is precluded from these studies. The optimal transfusion strategy in major burn injury is thus needed but remains unknown.MethodsThis prospective randomized multicenter trial block randomized patients to a restrictive (hemoglobin 7-8 g/dL) or liberal (hemoglobin 10-11 g/dL) transfusion strategy throughout hospitalization. Data collected included demographics, infections, transfusions, and outcomes.ResultsEighteen burn centers enrolled 345 patients with 20% or more TBSA burn similar in age, TBSA burn, and inhalation injury. A total of 7054 units blood were transfused. The restrictive group received fewer blood transfusions: mean 20.3 ± 32.7 units, median = 8 (interquartile range: 3, 24) versus mean 31.8 ± 44.3 units, median = 16 (interquartile range: 7, 40) in the liberal group (P < 0.0001, Wilcoxon rank sum). BSI incidence, organ dysfunction, ventilator days, and time to wound healing (P > 0.05) were similar. In addition, there was no 30-day mortality difference: 9.5% restrictive versus 8.5% liberal (P = 0.892, χ test).ConclusionsA restrictive transfusion strategy halved blood product utilization. Although the restrictive strategy did not decrease BSI, mortality, or organ dysfunction in major burn injury, these outcomes were no worse than the liberal strategy (Clinicaltrials.gov identifier NCT01079247).

ObjectivesHospice enrollment is known to reduce risk of hospitalizations for nursing home residents who use it. We examined whether residing in facilities with a higher hospice penetration: (1) reduces hospitalization risk for nonhospice residents; and (2) decreases hospice-enrolled residents' hospitalization risk relative to hospice-enrolled residents in facilities with a lower hospice penetration.MethodsMedicare Beneficiary File, Inpatient and Hospice Claims, Minimum Data Set Version 2.0, Provider of Services File, and Area Resource File. Retrospective analysis of long-stay nursing home residents who died during 2005-2007. Overall, 505,851 nonhospice (67.66%) and 241,790 hospice-enrolled (32.34%) residents in 14,030 facilities nationwide were included. We fit models predicting the probability of hospitalization conditional on hospice penetration and resident and facility characteristics. We used instrumental variable method to address the potential endogeneity between hospice penetration and hospitalization. Distance between each nursing home and the closest hospice was the instrumental variable.ResultsIn the last 30 days of life, 37.63% of nonhospice and 23.18% of hospice residents were hospitalized. Every 10% increase in hospice penetration leads to a reduction in hospitalization risk of 5.1% for nonhospice residents and 4.8% for hospice-enrolled residents.ConclusionsHigher facility-level hospice penetration reduces hospitalization risk for both nonhospice and hospice-enrolled residents. The findings shed light on nursing home end-of-life care delivery, collaboration among providers, and cost benefit analysis of hospice care.

In addition to social communication deficits, restricted repetitive behaviors (RRBs) are a key diagnostic feature of autism spectrum disorder (ASD). Dysfunction of the autonomic nervous system (ANS) in ASD has been posited as a mechanism of RRBs; however, most studies investigating ANS activity in ASD have focused on its relation to social functioning. This study used respiratory sinus arrhythmia (RSA) patterns to measure ANS functioning and analyze its relation to RRBs in children with and without an ASD diagnosis. Baseline RSA and RSA reactivity predicted RRB severity and exploratory analyses revealed these measures may be associated with RRB subgroups. These results are discussed in regards to the behavioral literature on RRBs and the benefits of finding biomarkers for these behaviors.

The Society for Psychophysiological Research and accompanying journal, Psychophysiology, have increasingly incorporated diversity and inclusion into their posted values, conference programming, and science. Much of this focus on equity, diversity, and inclusion work has occurred since 2010. The current review content analyzed the articles published in Psychophysiology from 2010 through 2020 to determine if SPR and Psychophysiology's commitment to diversity and inclusion has resulted in changes to reporting and analysis of participant demographics. Demographic reporting practices were compared to APA reporting standards and the use of demographic variables assessed according to the guidance proffered in the introduction to Psychophysiology's 2016 Special Issue on Diversity and Representation. Results of the content analysis indicated near perfect reporting of biological sex and frequent reporting of average age. Age range and educational attainment were reported in over half of studies, while race or ethnicity were reported in only 17%. Socioeconomic status, income, gender identity, and sexual orientation were almost never reported. In over 60% of studies at least one major demographic variable was reported, but was not used in preliminary, main, or supplementary analyses as a covariate, moderator, or otherwise. SPR and Psychophysiology should continue advocating for increased reporting of major demographic variables and ethical analysis of demographic modulation of various psychophysiological mechanisms. We provide a preliminary template of reporting standards and call for the inclusion of more open science practices by psychophysiologists. Content analyses can help a field understand the impact of its policies or stated values. This paper outlines the reporting and use of demographic variables in articles published in Psychophysiology to trace the impact of SPR's commitment to DEI.
