Abstract

BackgroundIron from haemolysed blood is implicated in secondary injury after intracerebral haemorrhage. We aimed to assess the safety of the iron chelator deferoxamine mesylate in patients with intracerebral haemorrhage and to establish whether the drug merits investigation in a phase 3 trial.MethodsWe did a multicentre, futility-design, randomised, placebo-controlled, double-blind, phase 2 trial at 40 hospitals in Canada and the USA. Adults aged 18-80 years with primary, spontaneous, supratentorial intracerebral haemorrhage were randomly assigned (1:1) to receive deferoxamine mesylate (32 mg/kg per day) or placebo (saline) infusions for 3 consecutive days within 24 h of haemorrhage onset. Randomisation was done via a web-based trial-management system centrally in real time, and treatment allocation was concealed from both participants and investigators. The primary outcome was good clinical outcome, which was defined as a modified Rankin Scale score of 0-2 at day 90. We did a futility analysis: if the 90% upper confidence bound of the absolute risk difference between the two groups in the proportion of participants with a good clinical outcome was less than 12% in favour of deferoxamine mesylate, then to move to a phase 3 efficacy trial would be futile. Primary outcome and safety data were analysed in the modified intention-to-treat population, comprising only participants in whom the study infusions were initiated. This trial is registered with ClinicalTrials.gov, number NCT02175225, and is completed.FindingsWe recruited 294 participants between Nov 23, 2014, and Nov 10, 2017. The modified intention-to-treat population consisted of 144 patients assigned to the deferoxamine mesylate group and 147 assigned to the placebo group. At day 90, among patients with available data for the primary outcome, 48 (34%) of 140 participants in the deferoxamine mesylate group, and 47 (33%) of 143 patients in the placebo group, had modified Rankin Scale scores of 0-2 (adjusted absolute risk difference 0·6% [90% upper confidence bound 6·8%]). By day 90, 70 serious adverse events were reported in 39 (27%) of 144 patients in the deferoxamine mesylate group, and 78 serious adverse events were reported in 49 (33%) of 147 patients in the placebo group. Ten (7%) participants in the deferoxamine mesylate and 11 (7%) in the placebo group died. None of the deaths were judged to be treatment related.InterpretationDeferoxamine mesylate was safe. However, the primary result showed that further study of the efficacy of deferoxamine mesylate with anticipation that the drug would significantly improve the chance of good clinical outcome (ie, mRS score of 0-2) at day 90 would be futile.FundingUS National Institutes of Health and US National Institute of Neurological Disorders and Stroke.

6. No State Left Behind: The Challenges and Opportunities of ESEA 2001.

Abstract

On January 8, 2002, President George W. Bush signed into law the revised Elementary and Secondary Education Act (ESEA). It sets deadlines for states to expand the scope and frequency of student testing, revamp their accountability systems, and guarantee that every classroom is staffed by a teacher qualified to teach in his or her subject area. It will also provide nearly $1 billion over the next 5 years to help states and districts strengthen K-3 reading programs, plus increased federal support for before- and after-school programs, school libraries, and many other programs. This report provides a summary of the major provisions and requirements of the new law, information about timelines and funding levels, an updated look at states' readiness to implement various provisions of the new law, and a set of "self-assessment" questions for policymakers to consider as they make decisions about how to move forward. Nine appendices describe the alignment of Title I and general state accountability systems for 1999-2000, examples of state approaches to comprehensive data systems, resources for adequate yearly progress, a description of corrective actions for low-performing schools, teacher preparation/certification, appropriations to states and territories, and a list of publications by the Education Commission of the States. 22. Report of a Study to Determine the Feasibility of Establishing a Central Foreign Credential Evaluation Service Under Non-Governmental Auspices.

Abstract

The study was conducted to determine whether a non-governmental Foreign Credential Evaluation Service (FCES) should be established and if so, what services it would provide, what the demand for these services would be, and what financial resources it would require. A survey of admissions officers at 223 educational institutions and representatives of 38 agencies (federal government, private, and state teacher certification and nursing licensing boards) revealed that there is a continuing need for a central agency to evaluate credentials of foreign students and professionals. The evaluative reports provided by the FCES of the US Office of Education are authoritative and up-to-date, but are considered as somewhat general in nature. If this Service were to be discontinued, it was felt that a new independent agency should replace it. The new FCES would speed up the delivery of more detailed reports which would include: number of a student's years of formal education, quality of academic work in terms of his country's rating system and of the US grading scale, number of credit hours in major field and in all subjects, US equivalence for foreign certificates and degrees, and other items. No consensus was reached on the organizational structure of a new FCES but outside funds would be needed for its first 5 years, after which it would be entirely self-supporting. The report contains recommendations on staffing, budget, fees and billing arrangements, and 56 illustrative tables. (WM)

The federal government has a relatively long history of regulating gambling, but gambling has not been a very intense or consistent concern. Abstract

The Salmonella typhimurium has a minimum growth pH of 4.0, which is lower than that of either Shigella flexneri (pH 4.8) or Escherichia coli (pH 4.4), but in a complex medium E. coli and S. flexneri persist when exposed to lower pH levels (2-2.5) compared to S. typhimurium (3.0). S. typhimurium and E. coli, but not S. flexneri, exhibit log-phase and stationary-phase acid tolerance response systems that give protection to pH 3.0. Some additional acid survival systems that require supplementation are used by E. coli and S. flexneri but not S. typhimurium.
