5. Proposed ratios and cutoffs for the assessment of lipodystrophy in HIV-seropositive individuals

Abstract

OBJECTIVES: To propose objective ratios using anthropometry and dual-energy X-ray absorptiometry (DXA) and to suggest cutoff points for them in order to classify lipodystrophy in male patients. METHODS: It is a cross-sectional study. DXA was applied and anthropometric measurements were performed in 100 men on highly active antiretroviral therapy. Receiver operating characteristic curves were used to propose cutoffs. Individuals were divided in without (lipo-) or with (lipo+) lipodystrophy and their metabolic parameters were compared. RESULTS: The following ratios were proposed: fat mass ratio by DXA (FMR), waist thigh ratio (WTR), waist calf ratio (WCR), and arm to trunk ratio (ATR). The best cutoffs observed for FMR, WTR and ATR were 1.26, 1.74 and 2.08, respectively. Using the proposed cutoff for FMR, we observed worse metabolic profile, with increased tryglicerides, fasting serum glucose and more hypercholesterolemia in the lipo+ group. WTR and ATR showed a significant correlation with FMR. CONCLUSIONS: Anthropometric ratios (WTR/ATR) and FMR can be used to aid the diagnosis of lipodystrophy in order to contribute to a more accurate and earlier diagnosis permitting intervention and even preventing metabolic disturbances. Abstract

In the study of palaeotsunamis it is crucial to decipher the sedimentological record, to derive intensity of pastevents and to infer different inundation phases. To achieve this goal, it is important to apply high-resolutiontechniques that allow magnifying intra-deposit details (at a sub-centimetric scale) that otherwise would not beperceived; consequently, valuable information could be overlooked.In this work, we applied successfully high-resolution geochemical and grain-size analyses – XRF core-scanningand image analysis, respectively – to the AD 1755 tsunami deposit. This quartz sand enriched in bioclastdeposit (exhibiting high Si/Al and Ca/Ti) was recognized in the coastal stratigraphic sequence of Salgadoslagoon due to its contrasting composition when compared with the under and overlying mud layers with scarcebioclasts (exhibiting low Si/Al and Ca/Ti). In the absence of textural evidence, the identification of peakingconcentrations of Cl, S and Br (all major constituents of sea salt) in a continuous muddy sequence allowedslightly extending farther inland the limit of inundation. In addition, grain-size analysis data attested the fininginland of the deposit. Furthermore, despite the macroscopic massive structure of the tsunami deposit, throughoutthe lagoon, grain-size results revealed more complexity and allowed inferring up to four depositional sequencesdirectly associated with the AD 1755 tsunami inundation.

Abstract

This work constitutes the first contribution for the systematisation of the geological heritage knowledge in Portugal, following the international recomendations for the characterisation of geological heritage (IUGS, ProGEO). The application of the ProGEO methodology has resulted on the creation of fourteen frameworks with international relevance, established by consensus among the Portuguese geological community. The description of each category in this paper is not exhaustive and just the most relevant scientific settings are presented. The following are the three key outcomes of this work: i) At a national level, the most important geosites are identified, indicating where geoconservation efforts should be prioritised through scientific justification; ii) At a regional level, conditions have been developed to foster dialogue with Spanish colleagues in order to create Iberian frameworks; iii) At an international level, is now possible to integrate Portuguese geosites in the global inventories promoted by IUGS, UNESCO, and ProGEO.
