In laymen's terms, it can be difficult to communicate how much force is needed to create a stab wound into a person's chest. Previous work has determined the force to puncture the skin, rib cartilage, or bone but without putting the results in terms that are easy to understand.The purpose of this study was to determine the force needed to puncture 3 types of fruit using 3 different types of knives, namely, a steak knife, a butcher's knife, and a lock-blade knife, to help put these previous results in perspective.There was wide variation in the force required to insert a knife into different fruits, but no force for any knife at any location for all fruits exceeded 93.1 N. Results show that force needed to penetrate the skin and allow for an 8-cm blade penetration into the chest is similar to the force required to insert a steak knife for a 6-cm distance into a cantaloupe. In addition, the force needed to penetrate the cartilage is most similar to stabbing a watermelon to 6 cm with a butcher knife. However, the forces required to penetrate the bone are greater than those required to penetrate any fruit with any type of blade.

Background Dart throw motions are frequently used during rehabilitation but the ideal orientation of a dart throw motion is unknown. Questions/Purposes The purpose of this study was to measure the axial force on the distal radius during different dart throw motions with the wrist and forearm in various positions. Our hypothesis was that there would be a significant difference on the axial force between various forearm positions and different dart throw orientations. Methods Eight fresh frozen cadaver wrists were moved through 10 different orientations of a dart throw motion with the forearm in neutral, in pronation, and during a dynamic forearm rotation motion while the axial force was measured. Results Significantly smaller axial force occurred with the forearm in pronation than during the dynamic forearm motion. The shorter dart throw motions which were oriented equally toward the flexion/extension and radioulnar deviation axes had significantly smaller distal radius forces than nearly all of the other dart throw motions. Conclusion/Clinical Relevance Rehabilitation protocols incorporating a dart throw motion may be of benefit after injury or surgery. To minimize the axial force transmitted through a healing distal radius fracture, short dart throw motions, oriented at 45 degrees from the sagittal and coronal planes, with the forearm in pronation, might be preferable for range of motion activities during rehabilitation.

The purpose of this cadaveric biomechanical experiment was to evaluate the effects of suture button suspensionplasty of the first carpometacarpal joint on thumb biomechanics and thumb position compared with an intact, arthritic specimen.Six tendons in 8 cadaver hands were loaded to simulate 6 activities of daily living and passively moved through a circumduction motion. Proximal migration of the base of the first metacarpal was measured using optical motion sensors in the intact hand, after trapeziectomy, and following insertion of a suture button suspensionplasty with nominal tightening (approximately 4.5 N) and with firm tightening (approximately 44.5 N).Removal of the trapezium caused a significant increase in the proximal migration of the first metacarpal during a simulated jar grasp, opposition, flexion, extension, and abduction (average, 9.5 mm) compared with its location with the thumb in the intact, neutral position (average, 3.8 mm). Firm tightening of the tightrope caused a near elimination of the proximal migration of the first metacarpal (average, 0.7 mm). In all 6 static loading cases with the trapezium removed, firm tightening caused a significantly smaller migration than in the absence of tightening.This biomechanical cadaver study supports the hypothesis that trapeziectomy results in proximal migration of the first metacarpal. Suture suspensionplasty mitigates against this migration while maintaining normal motion of the first metacarpal compared with the intact state. Firm tightening of the suture does not adversely affect the first metacarpal's mobility and further decreases proximal migration. However, firm tightening may cause impingement between the first and second metacarpals.Suture button suspensionplasty can be used in addition to trapeziectomy in the treatment of basal joint arthritis, and may diminish the need for ligament reconstruction or temporary K-wire insertion.

Background Multiple partial wrist fusions exist for the management of arthritic disease. Limited information is available on their effect on wrist range of motion in the dart-throwing direction of wrist motion, even though it is used in most activities of daily living. Purpose The purpose of this study was to measure the retained motion for different orientations of dart-throwing motion for seven different partial wrist fusions and proximal row carpectomy (PRC). Methods Eight fresh frozen right cadavers were tested with the wrist intact and followed simulated fusions. Fusions were performed using an external fixation technique and included scaphocapitate, scapholunate (SL), capitolunate, radiolunate, radioscapholunate, scaphotrapeziotrapezoid, 4 corner fusion, and PRC. Results In the intact wrist, the average arc of wrist motion with the wrist oriented at 20 degrees away from the flexion-extension axis was significantly larger than at any other orientation of motion. All partial wrist fusions and the PRC had significantly smaller average dart-throw arc of motion compared with intact at an orientation 20 and 25 degrees away from flexion-extension. The SL fusion provided a significantly larger arc of motion than most of the other fusions at most orientations. Conclusion/Clinical Relevance This study provides a comprehensive compilation of the range of motion in a functional plane, “the dart-throw motion,” for limited wrist fusions and PRC. These data provide the clinician with important information that can be used to educate patients regarding expectations after surgery.

When investigating deaths due to stab wounds, a common question asked to the forensic pathologist concerns the force required to inflict a given wound.In this study, tests were performed on 6 human cadavers. A material testing machine was used to produce the stab wounds and to record the force required to penetrate skin, muscle, cartilage, and rib bone of the chest. Three different blades were used: a steak knife, a butcher knife, and a lock-blade knife. On each cadaver, chest injuries were produced at the following locations: (a) skin, intercostal soft tissues; (b) skin, muscle, and cartilage; and (c) skin, muscle, and bone. After the experiment, a chest dissection was performed to confirm the correct locations of the produced stab wounds.The force required to insert a knife into cartilage or bone was significantly greater than the force to insert it into a region only covered by skin. There was wide variation in the force required to insert a knife into different bodies, but no force for any knife at any location for all bodies was greater than 261 N.This study allowed us to obtain quantitative measures of the force required to penetrate human chest tissues, removing subjective factors.

Purpose This study evaluated the biomechanics of Geissler IV (G4) wrists in cadavers and compared them with intact specimens after multiple ligament sectioning to create scapholunate instability. It also evaluated carpal motion changes after sectioning of the lunotriquetral interosseous ligament (LTIL). Methods Eight cadaver wrists determined to be G4 arthroscopically were tested using a wrist joint motion simulator. The LTIL was then sectioned, and carpal motion was recorded again. Carpal motions were compared with 37 normal wrists after sectioning of the scapholunate interosseous ligament and other ligaments to create a G4 wrist. Results Carpal motion of the 37 normal wrists after ligamentous sectioning was similar to motion of the 8 specimens noted to be G4. These wrists did not demonstrate subluxation of the scaphoid that may occur after ligament sectioning. After sectioning of the LTIL, there were significant changes in lunate and triquetral motion. Conclusions These findings support the hypothesis that sectioning multiple ligaments in normal wrists to create scapholunate instability causes average motion comparable to that seen in G4 wrists. Ligamentous sectioning can cause a range of scaphoid instability. Lunotriquetral interosseous ligament sectioning in native G4 wrists caused greater changes in triquetral than scaphoid range of motion. Clinical relevance Patients with arthroscopically determined G4 lesions have an incompetent SLIL and scapholunate instability but do not necessarily have scapholunate dissociation and subluxation. Cadaver studies that evaluate instability by sectioning specific intact wrist ligaments are similar to the G4 specimens and thus are a good approximation of naturally occurring wrist instability. The functionality of secondary stabilizers not seen arthroscopically may explain the differences in motion. Geissler IV wrists and ligament-sectioned wrists are points on the spectrum of carpal instability, which is determined by the extent of damage to multiple ligamentous structures.

BACKGROUND Scapholunate interosseous ligament (SLIL) injury following a fall on an outstretched hand may lead to carpal instability and in some cases require long-term rehabilitation following repair. Rehabilitation, especially in athletes, may include pushups, but little is known as to what type of pushup may be safer. OBJECTIVE To determine biomechanical differences between two pushup positions (neutral or extended). METHODS Six fresh cadaver wrists with pre-existing SLIL damage were uni-axially loaded in neutral and extension in order to simulate two different pushup styles. The motions of the scaphoid and lunate in relation to the radius were measured. The dorsal, proximal, and volar insertion sites of the SLIL were identified and, using the collected kinematic data, gap distances were calculated for each site. RESULTS Gap distance between the proximal SLIL insertion points was significantly greater in neutral than in extension. There was a trend that the dorsal and volar SLIL insertion points were also greater in neutral than in extension. After the wrist was extended 90o, the scaphoid extended 70.1o and the lunate extended 28.6o compared to their positions with the wrist in neutral. CONCLUSION The larger gap distances between the scaphoid and lunate in neutral suggest that a neutral style pushup could put higher forces on a wrist with pre-existing SLIL damage and may thus hinder recovery for a person with a repaired SLIL. A pushup in extension, in these injured wrists, may be less detrimental.

Background Wrist structure is complicated by distinct anatomical patterns. Previous studies defined radiographic wrist types based on lunate and capitate shape within the midcarpal joint. We hypothesized that these disparate structural patterns will transfer forces differently through the wrist. Objective This study aims to correlate force transferred to the distal radius and ulna with morphological measurements in cadaver arms. Methods Radiographs from 46 wrists, previously tested for force transfer between the radius and ulna, were examined. The percentage of compressive force through the distal ulna was determined by mounting load cells to the radius and ulna, while 22.2 Newton (N) tensile forces were individually applied to multiple tendons. Each wrist was tested in a neutral flexion–extension and radial-ulnar deviation position. Results Wrist type and lunate type were associated with percentage of force transfer through the ulna (p = 0.002, p = 0.0003, respectively). Percentage of force transfer was correlated with capitate circumference (p = 0.02, r = 0.34). Conclusions This study supports distinct force transfer between morphological wrist types. Clinical Relevance Understanding the mechanical significance of different structural variations in the wrist bones will improve our ability to understand wrist function and the distinctive development of wrist pathology. Background As numerous repairs, reconstructions, and replacements have been used following scapholunate interosseous ligament (SLIL) injury, there is a need to define the structural requirements for any reconstruction or replacement. Methods Research has been conducted on the force needed to keep the scaphoid and lunate reduced following simulated injury, the failure force of the native SLIL and various replacements, the stiffness of the SLIL and replacements, and the torsional resistance of the scaphoid relative to the lunate. Results Forces on the order of 50 N are needed to keep the scaphoid and lunate reduced during simple wrist motions in the chronically injured wrist. Even greater forces (up to 110 N) are needed to keep the bones reduced during strenuous activities, such as pushups. The failure force of the entire SLIL has been reported to be as high as 350 N and the failure force of just the dorsal component of the SLIL to be 270 N. Conclusions The design requirements for a reconstruction or repair may vary depending upon the demands of the patient. In a high demand patient, a reconstruction needs to support the above-mentioned forces during cyclic loading (50 N), when performing strenuous activities (110 N), or during a fall (at least 350 N). Any artificial replacement must undergo careful biocompatibility testing.

Purpose To determine the 6 degrees of freedom forces and moments in the distal radius that occur during a pushup or other active wrist motions. Methods Eight fresh-frozen cadaveric wrists were moved through 6 physiological motions and held at 1 static pushup position while the force through the distal radius was measured with a 6 degrees of freedom load cell. Three levels of compressive force were applied at the pushup position. Results Active wrist motions caused axial forces up to 283 N and moments up to 0.7 N-m. Those motions with a smaller range had significantly smaller axial forces than the larger flexion-extension or dart-thrower’s motions. With an 89-N pushup force applied, the average maximum axial force was 69 N, the radially directed force was 12 N, and the moment about the radioulnar axis was 2.3 N-m. Linear extrapolation of the forces to 100% body weight indicate that the axial force going through the distal radius would be 663 N, the radial force would be 147 N, and the moment about the radioulnar axis would be 18.6 N-m. Conclusions Large distal radius forces and moments can occur during pushup and active wrist motions. There are not only large axial compressive forces but also nontrivial radially directed forces. Clinical relevance This study may help surgeons and therapists better treat complicated distal radius fractures as well as provide for better comparisons of existing or new distal radius plates and constructs that are designed to treat these complicated loading patterns.

Background In joints, structure dictates function and consequently pathology. Interpreting wrist structure is complicated by the existence of multiple joints and variability in bone shapes and anatomical patterns in the wrist. Previous studies evaluated lunate and capitate shape in the midcarpal joint, and two distinct patterns have been identified. Purpose Our purpose was to further characterize the two wrist patterns in normal wrist radiographs using measurements of joint contact and position. Our hypothesis was that we will find significant differences between the two distinct anatomical patterns. Patients and Methods A database of 172 normal adult wrist posteroanterior (PA) radiographs was evaluated for radial inclination, height, length, ulnar variance, volar tilt, radial-styloid-scaphoid distance, and lunate and capitate types. We measured and calculated percent of capitate facet that articulates with the lunate, scapholunate ligament, scaphoid, and trapezoid. These values were compared between the wrist types and whole population. Results Type-1 wrists (lunate type-1 and spherical proximal capitate) were positively associated with a longer facet between capitate and distal lunate (p = 0.01), capitate and base of middle metacarpal (p = 0.004), and shorter facet between the capitate and hamate (p = 0.004). The odds ratio of having a type-1 wrist when the interface between the capitate and lunate measures >8.5 mm is 2.71 (confidence interval [CI] 1.07, 6.87) and when the line between the capitate and the base of middle metacarpal >9.5 mm is 3.5 (CI 1.38, 9.03). Conclusion We characterized the two-wrist patterns using intracarpal measurements. Translating these differences into three-dimensional contact areas may help in the understanding of biomechanical transfer of forces through the wrist. Purpose To determine the contact pressures between the scaphoid and lunate and the distal radius during 2 wrist push-up positions before and following scapholunate interosseous ligament (SLIL) sectioning. Methods Eight fresh cadaveric wrists were tested in a neutral flexion-extension (knuckle) push-up position and in an extended push-up position. Pressure measurements were acquired as each wrist was loaded with the wrist in extension and with the wrist in a neutral position. Data were acquired with the SLIL intact and following sectioning of its dorsal, volar, and proximal components. The wrist was disarticulated and a map drawn on each sensor to identify each joint fossa. Results A push-up performed with the wrist in extension caused a significantly greater peak pressure in the radioscaphoid fossa but not in the radiolunate fossa. Moving the wrist into extension caused a significant dorsal movement of the pressure centroid an average of 2.9 mm in the radiolunate fossa and an average of 5.7 mm in the radioscaphoid fossa. Sectioning the SLIL caused the centroid of pressure in the radioscaphoid fossa to significantly move an average 1.4 mm radially. Conclusions A push-up with the wrist in extension causes a significant increase in the pressure in the radioscaphoid fossa but not in the radiolunate. This finding may help explain why degenerative arthritis first develops in the radioscaphoid fossa before involving the radiolunate fossa. As expected, gapping that occurs with SLIL injury was observed here as a radial translation of the scaphoid and not as a movement of the lunate. The new location of scaphoid contact may be an additional reason for the development of radioscaphoid arthritis occurring before radiolunate arthritis. Clinical relevance This study provides a possible explanation as to why degenerative arthritis may first occur in the radioscaphoid fossa.

Purpose The purpose of this study was to determine whether a modified step-cut tibial tubercle osteotomy (Maquet–Fulkerson hybrid) might produce comparable or better results than a standard oblique anteromedialization tibial tubercle osteotomy (Fulkerson type) and thus warrant the surgical need for additional cuts. Methods Six pairs of cadaveric knees were evaluated prior to and after tibial tubercle osteotomies. Simulation was done via a shallow knee bend simulator through 20 to 70° of knee flexion for the intact specimens and following the surgical procedures. The variables tested were trochlear contact forces and pressures and patellar motion. Results Testing showed a decreased force (P=0.027), peak contact pressure (P=0.01) and contact area (P=0.034) on the lateral trochlea of the femur for both types of osteotomies. There was no significant difference in the lateral femoral peak pressure or in the medial femoral peak pressure between the oblique cut and the step-cut. Also, there was no difference in patellar motion after either procedure. Conclusion We conclude that both osteotomies decrease lateral patellofemoral trochlear pressure. The oblique osteotomy may decrease lateral pressure to a greater extent. Regarding biomechanical testing, there was no demonstrable advantage to performing a step-cut osteotomy.

Purpose Distal radioulnar joint (DRUJ) instability may occur after an injury, resulting in pain and reduced strength. When primary repair is not possible or initial fixation has failed, chronic instability may result, requiring a reconstructive procedure such as the Adams procedure. The first purpose of this study was to evaluate the role of the triangular fibrocartilage complex and various components of the interosseous membrane as they were sectioned. The second purpose was to evaluate the Adams procedure in stabilizing the forearm. Methods Eight fresh cadaver forearms were dynamically moved through an average range of 56.8° pronation to 54.8° supination and tested first with the forearm intact and then after sectioning each of the following structures: the dorsal (DRUL) and palmar radioulnar ligaments (PRUL), the distal interosseous membrane, and the central band. Finally, they were tested after reconstruction using the Adams procedure. During each forearm motion and provocative shuck, the motion of the radius and ulna were measured and the locations of the radial attachments of the DRUL, PRUL, and sigmoid notch and ulnar fovea were computed. Results Significant increases in the gap between the ulnar fovea and the attachment sites of the DRUL and PRUL were observed with incremental sectioning, most notably after sectioning of the central band. Reconstruction significantly reduced the gap at the DRUL and PRUL sites during dynamic motion. Conclusions This study reinforces the concept that DRUJ stability depends on more than the radioulnar ligaments, ulnocarpal ligaments, and triangular fibrocartilage complex, but is also significantly affected by the distal and central interosseous membrane. Reconstruction reduces gapping. Clinical relevance These results suggest that the Adams reconstruction is a reasonable option to address DRUJ instability but may be an incomplete solution in the setting of a ruptured interosseous ligament.

Background: Subtalar arthritis is a common consequence following calcaneal fracture, and its development is related to the severity of the fracture. Previous calcaneal fracture models have demonstrated altered contact characteristics when a step-off is created in the posterior facet articular surface. Changes in posterior facet contact characteristics have not been previously characterized for calcaneal fracture gap without step-off. Methods: The contact characteristics (peak pressure, area of contact, and centroid of pressure) of the posterior facet of the subtalar joint were determined in 6 cadaveric specimens. After creating a calcaneal fracture to simulate a Sanders type II fracture, the contact characteristics were determined with the posterior facet anatomically reduced followed by an incremental increase in fracture gap displacement of 2, 3, and 5 mm without a step-off of the articular surface. Results: Peak pressure on the medial fragment was significantly less with a 5-mm gap compared to a 2- or 3-mm gap, or reduced. On the lateral fragment, the peak pressure was significantly increased with a 5-mm gap compared to a 2- or 3-mm gap. Contact area significantly changed with increased gap. Conclusion: In this study, there were no significant differences in contact characteristics between a Clinical Relevance: A small amount of articular incongruity without a step-off can be tolerated by the subtalar joint, in contrast to articular incongruity with a step-off present.

Background Proximal row carpectomy (PRC) is contraindicated in wrists with preexisting arthritis of the proximal capitate or radiolunate fossa. Patients with these conditions frequently pursue wrist arthrodesis with its associated functional limitations. Questions/Purposes The purpose of this study was to evaluate the results of using lateral meniscal allograft interposition (LMAI), in combination with PRC, in patients with symptomatic wrist arthritis. The primary question is whether this allograft will allow wrist function comparable to that in patients having only a PRC. A secondary question was to determine the short-term longevity of the allograft. Patients/Method Between 2006 and 2012, nine wrists underwent PRC with LMAI. Patient demographics and rates of complication or graft failure were determined. During independent clinical exams, functional outcomes were reviewed, patients completed a Disabilities of the Arm, Shoulder, and Hand (DASH) scores, and radiographs were taken. Results Four patients met the inclusion criteria, having clinical follow-up at an average of 4.2 years. DASH scores at the time of follow-up ranged from 9 to 33, with an average of 24. Average radiocapitate joint space in the first postoperative radiograph was 2.8 mm compared with 1.8 mm at the time of final follow-up. No wrists went on to arthrodesis. Conclusion Early outcomes of PRC with LMAI are comparable to those results found in the literature of PRC alone. LMAI with PRC may be a valid short-term option as a motion-preserving procedure in those patients contraindicated to having a PRC alone. Level of Evidence Level IV

Background Damage to the interosseous membrane (IOM) can alter load transmission between the radius and ulna and decrease their axial stability. Less is known about the effect of IOM sectioning on the transverse stability between the radius and ulna. Purpose The purpose of this study was to quantify the radioulnar gapping at the distal radioulnar joint (DRUJ) during forearm rotation when the IOM was experimentally sectioned while maintaining the integrity of the distal radioulnar ligaments. Methods In 12 fresh-frozen cadaver forearms tested in a combined wrist–forearm simulator, the increase in gap between the radius and ulna, at the level of the DRUJ, was determined during cyclic forearm rotation following IOM sectioning. Results IOM sectioning caused a significant increase in dorsal gapping at the DRUJ by 2.1 mm in supination and 0.6 mm in pronation. It also caused an increase in palmar gapping by 1.3 mm in supination and 0.5 mm in pronation. Conclusion This experiment has shown that the IOM has an important role in stabilizing the DRUJ, especially in supination, and that IOM sectioning caused greater loads on the palmar and dorsal radioulnar ligaments. Since DRUJ instability is primarily treated by fixing the laxity at the dorsal radioulnar ligament (DRUL) and palmar radioulnar ligament (PRUL), untreated IOM damage could permit additional injury and instability to the radioulnar ligaments or their reconstruction. Clinical Relevance Reconstruction of a torn IOM should be considered in the presence of persistent DRUJ instability following DRUJ reconstruction.

Purpose To determine the relative roles of the dorsal and volar portions of the scapholunate interosseous ligament (SLIL) in the stability of the scaphoid and lunate. Methods Sixteen fresh cadaver wrists were moved through physiological motions using a wrist joint simulator. Electromagnetic sensors measured the motion of the scaphoid and lunate. Data were collected with the wrist intact, after randomly sectioning the dorsal SLIL first (8 wrists) or the volar SLIL first (8 wrists), and after full ligamentous sectioning. Differences in the percent increase in scaphoid flexion or lunate extension were compared using a t test with significance set at P Results Sectioning the dorsal SLIL accounted for 37%, 72%, and 68% of the increase in scaphoid flexion in wrist flexion-extension, radioulnar deviation, and dart throw motion as compared with complete SLIL sectioning. Sectioning the volar SLIL accounted for only 7%, 6%, and 14%, respectively. In the same 3 motions, sectioning the dorsal SLIL accounted for 55%, 57%, and 58% of the increase in lunate extension, whereas volar SLIL sectioning accounted for 27%, 28%, and 22%. Conclusions The dorsal SLIL provides more stability to the scaphoid and lunate in biomechanical testing. The volar SLIL does provide some, although less, stability. Clinical relevance Although this study supports the critical importance of dorsal SLIL repairs or reconstructions, it also shows that there may be some value in implementing a volar SLIL repair or reconstruction.

Background: Morphology may provide the basis for the understanding of wrist mechanics. Methods: We used classification systems based on cadaver dissection of lunate and capitate types to evaluate a normal database of 70 wrist radiographs in 35 subjects looking for associations between bone shapes. Kappa statistics and a log-linear mixed -effects model with a random intercept were used. Results: There were 39 type-1, 31 type- 2 lunates, 50 spherical, 10 flat and 10 V-shaped capitates. There was a significant difference in lunate and capitate shape between the hands of the same individual p Conclusion: Further study to better understand the development of radiographic parameters of the midcarpal joint may aid in our understanding of the morphology and mechanics of the wrist.

Background Little is known about changes in scaphoid and lunate supination and pronation following scapholunate interosseous ligament (SLIL) injury. Information on these changes may help explain why some SLIL reconstructions have failed and help in the development of new techniques. Purpose To determine if following simulated SLIL injury there was an increase in scaphoid pronation and lunate supination and to determine if concurrently there was an increase in the extensor carpi ulnaris (ECU) force. Materials and Methods Scaphoid and lunate motion were measured before and after sectioning of the SLIL and two volar ligaments in 22 cadaver wrists, and before and after sectioning of the SLIL and two dorsal ligaments in 15 additional wrists. Each wrist was dynamically moved through wrist flexion/extension, radioulnar deviation, and a dart-throwing motion. Changes in the ECU force were recorded during each wrist motion. Results Scaphoid pronation and lunate supination significantly increased following ligamentous sectioning during each motion. There were significant differences in the amount of change in lunate motion, but not in scaphoid motion, between the two groups of sectioned ligaments. Greater percentage ECU force was required following ligamentous sectioning to achieve the same wrist motions. Conclusion Carpal supination/pronation changed with simulated damage to the scapholunate stabilizers. This may be associated with the required increases in the ECU force. Clinical Relevance In reconstructing the SLIL, one should be aware of the possible need to correct scaphoid pronation and lunate supination that occur following injury. This may be more of a concern when the dorsal stabilizers are injured.

Purpose To determine the force required to maintain reduction of Geissler grade 4 scapholunate dissociations during physiological wrist motions. Methods Fresh-frozen cadaveric wrists with Geissler grade 4 scapholunate dissociations were identified by arthroscopy. Following reduction, a load cell was attached to measure the force across the scapholunate joint at a neutral position and during 4 different wrist motions re-created using a servohydraulic wrist simulator, including a large flexion-extension motion (FEM), small and large dart-thrower’s motions (DTMs), and a large DTM with ulnar offset. Results Five wrists with isolated preexisting scapholunate interosseous ligament (SLIL) tears were analyzed. The force required to maintain reduction was significantly greater in extension than in flexion during the large FEM and large DTM with ulnar offset. The forces were significantly larger in this study of preexisting SLIL dissociations compared with results from a prior study of acutely sectioned SLILs. In addition, forces to maintain reduction during DTMs were significantly less than forces during FEMs. Finally, a set of 3 wrists with preexisting scapholunate and lunotriquetral interosseous ligament (LTIL) tears were identified and had significantly decreased forces to maintain reduction at maximum extension and had decreased maximal forces during a cycle of motion compared with the wrists with isolated SLIL tears. Conclusions The forces required to maintain reduction of a Geissler grade 4 wrist are higher than forces after acutely sectioned SLIL. The forces are greater in extension than in flexion and less during the DTM compared with the FEM. Wrists with both SLIL and LTIL tears required less force to maintain reduction than those with isolated SLIL tears. Clinical relevance This study helps determine the strength of reconstruction required to maintain reduction of a Geissler grade 4 scapholunate dissociation.

Introduction Osteoarthritis (OA) in the wrist usually develops in a pattern described as scapholunate advanced collapse (SLAC). We observed an alternative pattern of OA that involves the lunocapitate joint in isolation with minimal involvement of the radioscaphoid articulation. Case Series The series was observed from a series of 100 wrist radiographs that were retrospectively reviewed. In order to characterize the alternative pattern of OA, we compared demographic data, presentation, and physical and radiographic examination characteristics between the patients with lunocapitate OA and SLAC wrists. Fifteen radiographs showed OA, nine had a SLAC pattern, and six had lunocapitate OA. The demographics were similar, but the clinical presentation was different. Concomitant ligamentous injury in distal radius fractures (DRF) may explain continued pain following surgery. The purpose of this study was to compare radiographic measurements assessing scaphoid translation in DRF after reduction, to measurements performed on normal radiographs. This may allow noninvasive evaluation of radiocarpal ligamentous integrity.Fifty postoperative radiographs were evaluated. The distance between the ulnar border of the radial styloid and the radial border of the scaphoid was measured midway between the styloid tip and scaphoid base, and then divided by scaphoid width at the same level. The measured ratios were compared to previously established normal data, established radiographic measurements of fracture reduction, fracture characteristics and fixation methods.Radiographic scaphoid position measurements differed significantly from normals (p = 0.0001). Fracture characteristics, surgical difficulty, and technique were not associated with scaphoid position.Despite accurate surgical reduction, abnormal positioning of the scaphoid may persist. This may reflect ligamentous injury, which generates suboptimal clinical results. Identifying and addressing ligamentous injury during surgery may prevent the development of instability and improve outcome after DRF.

Purpose To determine whether flexor carpi ulnaris (FCU) forces and tendon displacements change after pisotriquetral arthrodesis or after pisiform excision. Methods Nine cadaver wrists were moved through 4 variations of a dart throw motion, each having an oblique plane of motion, but with different ranges of motion and different antagonistic forces. The FCU tendon force and movement were measured in the intact wrist, following pisotriquetral arthrodesis, and following pisiform excision. Changes in force and tendon movement were compared using a repeated measures analysis of variance. Results After excision of the pisiform, a significantly greater FCU force was required during the 2 variations of the dart throw motion having a larger range of motion and during the smaller motion having a larger antagonistic force. Pisotriquetral arthrodesis did not cause a significant increase in the peak FCU force. Excision of the pisiform caused the FCU tendon to significantly retract during all wrist motions as compared to the intact wrist or after pisotriquetral arthrodesis. Conclusions Greater FCU forces are required to move the wrist when the pisiform with its moment arm function has been removed. This occurs during large oblique plane wrist motions and also in a smaller motion when greater antagonistic forces are applied. Excision of the pisiform also allows the FCU to move proximally, again because its moment arm function has been eliminated. Clinical relevance Excision of the pisiform requires greater FCU forces during large wrist motions and during motions that include large gripping forces such that excision may be a concern in high-demand patients with pisotriquetral arthritis. Although pisotriquetral arthrodesis does not alter the mechanical advantage of the FCU, its use in high-demand patients with pisotriquetral osteoarthritis cannot yet be recommended until the effects of that arthrodesis on midcarpal kinematics are further clarified.

Background To allow osseous integration to occur and thus provide long-term stability, initial glenoid baseplate fixation must be sufficiently rigid. A major contributing factor to initial rigid fixation is baseplate screw fixation. Current baseplate designs use a 4-screw fixation construct. However, recent literature suggests adequate fixation can be achieved with fewer than 4 screws. The purpose of the present study was to determine whether a 4-screw construct provides more baseplate stability than a 2-screw construct. Methods A flat-backed glenoid baseplate with 4 screw hole options was implanted into 6 matched pairs of cadaver scapulas using standard surgical technique. Within each pair, 2 screws or 4 screws were implanted in a randomized fashion. A glenosphere was attached allowing cyclic loading in an inferior-to-superior direction and in an anterior-to-posterior direction. Baseplate motion was measured using 4 linear voltage displacement transducers evenly spaced around the glenosphere. Results There was no statistical difference in the average peak central displacements between fixation with 2 or 4 screws ( P = .338). Statistical increases in average peak central displacement with increasing load ( P P Conclusion This study demonstrates no statistical difference in baseplate motion between 2-screw and 4-screw constructs. Therefore, using fewer screws could potentially lead to a reduction in operative time, cost, and risk, with no significant negative effect on overall implant baseplate motion.

Purpose Controversy exists as to whether a proximal row carpectomy (PRC) is a better procedure than scaphoid excision with 4-corner arthrodesis for preserving motion in the painful posttraumatic arthritic wrist. The purpose of this study was to determine how the kinematics and tendon forces of the wrist are altered after PRC and 4-corner arthrodesis. Methods We tested 6 fresh cadaver forearms for the extremes of wrist motion and then used a wrist simulator to move them through 4 cyclic dynamic wrist motions, during which time we continuously recorded the tendon forces. We repeated the extremes of wrist motion measurements and the dynamic motions after scaphoid excision with 4-corner arthrodesis, and then again after PRC. We analyzed extremes of wrist motion and the peak tendon forces required for each dynamic motion using a repeated measures analysis of variance. Results Wrist extremes of motion significantly decreased after both the PRC and 4-corner arthrodesis compared with the intact wrist. Wrist flexion decreased on average 13° after 4-corner arthrodesis and 12° after PRC. Extension decreased 20° after 4-corner arthrodesis and 12° after PRC. Four-corner arthrodesis significantly decreased wrist ulnar deviation from the intact wrist. Four-corner arthrodesis allowed more radial deviation but less ulnar deviation than the PRC. The average peak tendon force was significantly greater after 4-corner arthrodesis than after PRC for the extensor carpi ulnaris during wrist flexion-extension, circumduction, and dart throw motions. The peak forces were significantly greater after 4-corner arthrodesis than in the intact wrist for the extensor carpi ulnaris during the dart throw motion and for the flexor carpi ulnaris during the circumduction motion. The peak extensor carpi radialis brevis force after PRC was significantly less than in the intact wrist. Conclusions The measured wrist extremes of motion decreased after both 4-corner arthrodesis and PRC. Larger peak tendon forces were required to achieve identical wrist motions with the 4-corner arthrodesis compared with the intact wrist. We observed smaller forces for the PRC. Clinical relevance These results may help explain why PRC shows early clinical improvement, yet may lead to degenerative arthritis.

Purpose Early motion therapy protocols are desirable to reduce wrist stiffness after carpal injuries and distal radius fractures. Based on our assumption that a reduction in wrist tendon forces will be associated with a reduction in radioulnar carpal joint reaction force, the goal of this study was to determine the forearm position at which the wrist tendon forces required to initiate wrist motion were the least. Methods We tested 8 fresh-frozen cadaver forearms in a wrist and forearm motion simulator. In each specimen, we generated a wrist flexion-extension motion and a radioulnar deviation motion by pulling on the wrist flexor and extensor tendons with the forearm in supination, pronation, or neutral. We also performed a wrist flexion-extension motion during active forearm rotation. During each motion, we continuously recorded the forces in the 5 wrist tendons. We used repeated-measure analyses of variance to compare tendon forces. Results During the wrist flexion-extension motion, the summed peak wrist extensor forces were significantly less with the wrist in forearm supination than with the forearm in neutral or during active forearm rotation. For the summed peak wrist flexor forces, flexor tendon forces were significantly less with the forearm in supination compared with active forearm rotation. The extensor carpi radialis longus and flexor carpi ulnaris forces were significantly less in supination than during active forearm motion. During wrist radioulnar deviation motion, peak extensor carpi ulnaris force was significantly less with the forearm in supination than in neutral, and peak flexor carpi radialis force was significantly less with the forearm in supination than in pronation. Conclusions Peak wrist tendon forces were least in the static forearm supinated position. Clinical relevance To reduce deforming forces on a fresh carpal injury or distal radial fracture, active rehabilitation appears to be best initiated with the forearm held in supination.

The purpose of this study was to establish a normal measure of scaphoid position in the radioulnar plane in standard neutral, radial and ulnar deviation posteroanterior radiographs. This measurement may allow indirect evaluation of the radiocarpal ligaments and comparison between normal and pathologic states (following radius fractures, perilunate dislocations). Measurements were trialed on 74 normal wrist radiographs and 25 cadaver wrists. We evaluated the distance between the radial styloid and the scaphoid and corresponding scaphoid width. The ratio of distance/width at the mid styloid level (0.35, imprecision SD = 0.1) had the lowest random error and is therefore the most precise measurement of true scaphoid translation. This measurement is independent of scapholunate ligament integrity and may provide a better assessment of the radiocarpal component of ulnar translational instability. Abnormal movement of the scaphoid in the radioscaphoid joint likely reflects ligamentous injury. Identifying and addressing these injuries may prevent the development of arthritis.

Background:Posterior horn detachment (PHD) lesions of the lateral meniscus are commonly associated with acute anterior cruciate ligament (ACL) tears. Multiple surgeons have advocated for repair of this lesion at the time of ACL reconstruction. However, the biomechanical consequences of this lesion and its subsequent repair have not been evaluated.Hypothesis:The PHD lesion of the lateral meniscus will lead to increased tibiofemoral contact pressures, and repair of this lesion to bone via a tibial tunnel can restore normal contact pressures during simulated gait.Study Design:Controlled laboratory study.Methods:Lateral compartment contact pressures were measured via a sensor on the tibial plateau in 8 cadaver knees with the knee intact, after sectioning the posterior horn of the lateral meniscus to simulate PHD, and after repairing the injury. The repair was performed using an ACL tunnel guide to drill a tunnel from the anteromedial tibia to the posterior horn attachment site. Dynamic pressure data were continuously collected using a conductive ink pressure sensing system while each knee was moved through a physiological gait flexion cycle.Results:Posterior horn detachment caused a significant increase in tibiofemoral peak contact pressure from 2.8 MPa to 4.2 MPa ( P = .03). After repair of the lesion to bone was performed through a transtibial tunnel, the peak contact pressure was 2.9 MPa. Posterior horn detachment also significantly decreased the maximum contact area over which tibiofemoral pressure is distributed from 451 mm2in the intact state to 304 mm2in the detached state. Repair of the PHD lesion increased the maximum contact area to 386 mm2, however, this area was also significantly less than in the intact state ( P = .05).Conclusion:Posterior horn detachment of the lateral meniscus causes increased peak tibiofemoral contact pressure. The peak pressure can be reduced to a normal level with repair of the lesion to bone via a transtibial tunnel.Clinical Relevance:Posterior horn detachment of the lateral meniscus is a lesion often associated with an acute ACL tear. Debate exists concerning the importance of repairing PHD lesions at the time of ACL reconstruction. The data provided in this study may influence surgeons’ management of the lesion.

Purpose To evaluate the biomechanical alterations that occur after traditional scaphoid excision and midcarpal arthrodesis with and without excision of the triquetrum. The hypothesis of this study was that removal of the triquetrum increases the radiolunate contact pressure. Methods We cyclically moved 10 fresh cadaver wrists using a wrist joint motion simulator while measuring the contact pressures between the proximal carpal row and the distal radius and ulna using a dynamic pressure sensor. We acquired data in the intact wrist, after a midcarpal arthrodesis with the scaphoid excised, and then again with the triquetrum removed, which is also known as a capitolunate arthrodesis. Results The peak pressures in the radiolunate fossa significantly increased with either of the midcarpal arthrodeses compared with the intact wrist during each of the 3 dynamic wrist motions. In comparing the 2 midcarpal arthrodeses, the peak pressure in the ulnocarpal fossa significantly decreased after the triquetrum was removed during wrist radioulnar deviation and in the static ulnarly deviated position. After arthrodesis, we could identify no differences during any motion or static wrist position in the peak radiolunate pressures with or without the triquetrum. Conclusions We found that scaphoid excision and 4-corner arthrodesis shifts loads to the radiolunate joint. Isolated capitolunate arthrodesis with excision of the scaphoid and triquetrum further alters carpal kinematics and loading patterns. Clinical relevance These findings raise concern about routine excision of the triquetrum when performing a midcarpal arthrodesis.

Purpose The purpose of this study is to provide a thorough understanding of the anatomy of the cubital tunnel and to outline specific anatomical parameters of the cubital tunnel retinaculum (CuTR) that might aid in the management of ulnar nerve problems. The hypotheses of this study are (1) that the nerve elongates with elbow flexion and (2) that the cross-sectional area of the cubital tunnel is inversely proportional to the degree of elbow flexion. Methods Eleven fresh-frozen cadaver arms were dissected at the medial elbow. The CuTR was identified, and its thickness was measured. After excising the CuTR, we measured the elongation of the anterior and posterior aspects of the ulnar nerve, as well as the length of the CuTR origin/insertion, at increasing intervals of elbow flexion (15°, 30°, 45°, 90°, 120°, and 135°). Using 3-dimensional digitization technology, the surface of the cubital tunnel was recorded at 4 positions of elbow flexion (15°, 45°, 90°, and 135°) and analyzed to define the tunnel geometry. Results The CuTR origin-to-insertion length and the ulnar nerve length both increased significantly with increasing flexion angle. Both lengths at 90°, 120°, and 135° of elbow flexion were greater than at 15° or 30°. The cubital tunnel area was significantly less at 135° compared to either 45° or 90° of flexion. There was a linear relationship between the cubital tunnel area of the different arms with the corresponding nerve cross-sectional area when measured at the level of the epicondyle and when the arm was at 90° of elbow flexion. Conclusions The CuTR begins to stretch at 60° of flexion and continues to stretch with increasing flexion. Similarly, the ulnar nerve is more taut in flexion. The area within the cubital tunnel decreases beyond 90° of elbow flexion. Clinical relevance Understanding the dynamic anatomical relationships of the cubital tunnel might help in the safe treatment of cubital tunnel syndrome when using minimally invasive techniques and instrumentation.

Purpose To determine the amount of scaphoid and lunate translation that occurs in normal cadaver wrists during wrist motion, and to quantify the change in ulnar translation when specific dorsal and volar wrist ligaments were sectioned. Methods We measured the scaphoid and lunate motion of 37 cadaver wrists during wrist radioulnar deviation and flexion-extension motions using a wrist joint motion simulator. We quantified the location of the centroids of the bones during each motion in the intact wrists and after sectioning either 2 dorsal ligaments along with the scapholunate interosseous ligament or 2 volar ligaments and the scapholunate interosseous ligament. Results In the intact wrist, the scaphoid and lunate statistically translated radially with wrist ulnar deviation. With wrist flexion, the scaphoid moved volarly and the lunate dorsally. After sectioning either the dorsal or volar ligaments, the scaphoid moved radially. After sectioning the dorsal or volar ligaments, the lunate statistically moved ulnarly and volarly. Conclusions Measurable changes in the scaphoid and lunate translation occur with wrist motion and change with ligament sectioning. However, for the ligaments that were sectioned, these changes are small and an attempt to clinically measure these translations of the scaphoid and lunate radiographically may be limited. The results support the conclusion that ulnar translocation does not occur unless multiple ligaments are sectioned. Injury of more than the scapholunate interosseous ligament along with either the dorsal intercarpal and dorsal radiocarpal or the radioscaphocapitate and scaphotrapezial ligaments is needed to have large amounts of volar and ulnar translation.

Purpose To create a biomechanical model of palmar midcarpal instability by selective ligament sectioning and to analyze treatment by simulated partial wrist arthrodesis. Methods Nine fresh-frozen cadaver arms were moved through 3 servohydraulic actuated motions and 2 passive wrist mobilizations. The dorsal radiocarpal, triquetrohamate, scaphocapitate, and scaphotrapeziotrapezoid ligaments were sectioned to replicate palmar midcarpal instability. Kinematic data for the scaphoid, lunate, and triquetrum were recorded before and after ligament sectioning and again after simulated triquetrohamate arthrodesis (TqHA) and radiolunate arthrodesis (RLA). Results Following ligament sectioning, the model we created for palmar midcarpal instability was characterized by significant increases in (1) lunate angular velocity, (2) lunate flexion-extension, and (3) dorsal/volar motion of the capitate during dorsal/volar mobilizations. Simulated TqHA caused significantly more scaphoid flexion and less extension during the wrist radioulnar deviation motion. It also increased the amount of lunate and triquetral extension during wrist flexion-extension. Simulated RLA significantly reduced scaphoid flexion during both wrist radioulnar deviation and flexion-extension. Conclusions Both simulated arthrodeses eliminate wrist clunking and may be of value in treating palmar midcarpal instability. However, simulated RLA reduces proximal row motion whereas simulated TqHA alters how the proximal row moves. Long-term clinical studies are needed to determine if these changes are detrimental. Clinical relevance Palmar midcarpal instability is poorly understood, with most treatments based on pathomechanical assumptions. This study provides information that clinicians can use to design better treatment strategies for this unsolved condition.

Purpose A variety of soft tissue surgical procedures have been developed for treatment of scapholunate dissociation. One reconstruction method, using the dorsal intercarpal ligament, has been used clinically with some success. The purpose of this study was to evaluate biomechanically use of the dorsal intercarpal ligament for static scapholunate dissociation. Methods Eight cadaver wrists were tested in a wrist joint motion simulator. Each wrist was moved in continuous cycles of flexion–extension and radial–ulnar deviation. Kinematic data for the scaphoid and lunate were recorded for each wrist in the intact state, after the scapholunate interosseous, dorsal radiocarpal, and dorsal intercarpal ligaments were sectioned, and after reconstruction using the dorsal intercarpal ligament. Results Ligamentous sectioning resulted in static scapholunate dissociation. Visually, the repair initially reduced the gap between the scaphoid and lunate, but within a few cycles of wrist motion, there were statistically significant increases in scaphoid flexion, scaphoid ulnar deviation, and lunate extension. In 6 arms, gapping between the scaphoid and lunate was observed. In 2 arms, a gap occurred and the repair also pulled out of the bone junction. Conclusions This study does not support the hypothesis that the dorsal intercarpal ligament repair alone will stabilize the scaphoid and lunate after scapholunate instability in the immediate postoperative period.

Background The anterior intermeniscal ligament of the knee is at risk during knee arthroscopy, anterior cruciate ligament reconstruction, and tibial nail insertion. Hypothesis Release of the anterior intermeniscal ligament, in knees with type I ligaments, will result in altered contact pressures in the medial compartment. Study Design Controlled laboratory study. Methods Five fresh-frozen human cadaveric knees with intact type I anterior intermeniscal ligaments were chosen for testing in a modified MTS machine from 0° to 60° of flexion under 2 conditions: (1) intact and (2) after sharp sectioning of the anterior intermeniscal ligament. Measurements were made using inframeniscal contact pressure sensors covering the medial compartment. Poststudy analysis was done in 10° increments between 0° and 60° of flexion, looking at peak contact pressure and the amount of contact area seeing pressure. Results Sectioning of the anterior intermeniscal ligament caused a statistically significant increase in the peak pressure at 20°, 30°, 40°, and 50° of knee flexion. The largest change occurred at 40° of knee flexion, when the peak pressure increased by 27.5% (3.68 MPa to 4.69 MPa). Contact area decreased, although this difference was not statistically significant. Conclusion Release of the anterior intermeniscal ligament results in increased peak contact pressures in the medial compartment of the knee. Clinical Relevance Care should be taken to avoid sacrifice of this ligament during surgery.

Purpose The purpose of this study was to determine whether use of a lateral meniscal interposition allograft combined with proximal row carpectomy would reduce the peak joint contact pressures and increase the contact area when compared with proximal row carpectomy alone. Methods Six cadaver wrists were cyclically moved through flexion–extension and radioulnar deviation ranges of motion. Joint contact pressure was measured with the carpus intact, after proximal row carpectomy, and after insertion of a lateral meniscal allograft. Contact pressure data were also collected with the wrist in 5 static positions. Results Proximal row carpectomy caused statistically greater peak pressures and smaller contact areas when compared with the intact wrist. Insertion of the allograft statistically restored the pressures and areas to that observed in the intact wrist. Conclusions These results support the clinical trial of a lateral meniscal interposition allograft in patients with contraindications for proximal row carpectomy, such as pre-existing arthritis in the capitate head or lunate facet of the radius.

The first purpose of this study was to develop a noninvasive clinical tool that could predict whether the scapholunate interosseous ligament and other secondary stabilizing ligaments are injured in the presence of suspected scapholunate instability. The second purpose of this study was to determine which of those ligaments or ligament groups have been injured. Kinematic and three-dimensional (3D) meaurements from 62 cadaver wrists moved in a wrist joint motion simulator were used to develop various neural network predictive models. One group of models was based on angular changes in scaphoid and lunate motion before and after ligament sectioning (representing scapholunate instability). A second group of models was based on changes in the minimum distance between the scaphoid and lunate as well as other 3D gap measurements. The models, based on the scaphoid and lunate angular data, could predict with a 93% accuracy rate whether the wrist ligaments were intact. These models could also predict whether it was the dorsal ligaments or the volar ligaments that were sectioned 84% of the time. The models worked best using data with the wrist in 10 to 30 degrees of wrist flexion. The viability of a CT-based predictive model has been demonstrated by obtaining high prediction rates, sensitivity, specificity, and kappa statistic values.

Purpose Untreated scapholunate dissociation can lead to pain and eventually arthritis. There has been minimal limited success of soft tissue surgical procedures to repair or functionally replicate the torn structures. In this study, we evaluated a new surgical repair for scapholunate dissociation using a bioresorbable polymer approved for human applications. Methods Eight cadaver wrists were tested in a wrist joint motion simulator. Each wrist was moved in continuous cycles of flexion-extension and radioulnar deviation. Kinematic data for the scaphoid and lunate were recorded for each wrist in the intact state; after the scapholunate interosseous, dorsal radiocarpal, and dorsal intercarpal ligaments were sectioned; after repair using a 4-hole bioresorbable plate; and after 1,000 cycles of wrist motion to mimic continued use after surgery. Results Sectioning of these 3 wrist ligaments resulted in static scapholunate dissociation. Application of the polymer plate statistically restored the scaphoid and lunate kinematics to that of the intact specimen. Scapholunate instability and any gap between the bones was eliminated. After 1,000 cycles of motion, the plate maintained intact kinematics in 5 of 8 arms. During cyclic motion, either the plate failed or the screws pulled out in the remaining 3 arms. This occurred in the smallest and most osteoporotic cadavers in which positioning a sensor post and the 2 screws for the plate in the small lunate compromised the pullout strength of the screws. Conclusions Use of a resorbable polymer plate in restoring normal kinematics in patients with scapholunate dissociation is supported by this study.

The dart-throwing motion (DTM) plane can be defined as a plane in which wrist functional oblique motion occurs, specifically from radial extension to ulnar flexion. Most activities of daily living are performed using a DTM. The DTM utilizes the midcarpal joint to a great extent. Scaphotrapezio-trapezoidal anatomy and kinematics may be important factors that cause a DTM to be a more stable and controlled motion. During a DTM, there is less scaphoid and lunate motion than during pure flexion-extension or radioulnar deviation. Clinically, a DTM at the plane approximately 30 degrees to 45 degrees from the sagittal plane allows continued functional wrist motion while minimizing radiocarpal motion when needed for rehabilitation.

Purpose The purpose of this study was to determine whether the excursion of the scaphoid tuberosity and therefore scaphoid motion is minimized during a dart-throwing motion. Methods Scaphoid tuberosity excursion was studied as an indicator of scaphoid motion in 29 cadaver wrists as they were moved through wrist flexion-extension, radioulnar deviation, and a dart-throwing motion. Results Study results demonstrate that excursion was significantly less during the dart-throwing motion than during either wrist flexion-extension or radioulnar deviation. Conclusion If the goal of early wrist motion after carpal ligament or distal radius injury and reconstruction is to minimize loading of the healing structures, a wrist motion in which scaphoid motion is minimal should reduce length changes in associated ligamentous structures. Therefore, during rehabilitation, if a patient uses a dart-throwing motion that minimizes his or her scaphoid tuberosity excursion, there should be minimal changes in ligament loading while still allowing wrist motion. Purpose To determine the in vitro force between the scaphoid and the lunate supported by the scapholunate interosseous ligament (SLIL) during 2 wrist pushup positions. Methods Six fresh-frozen cadaveric wrists were tested in a neutral flexion-extension (knuckle) pushup position and in an extended pushup position. Tensile forces were measured across the scapholunate joint as half body weight axial forces were applied through the radius and ulna. Forces were measured after sectioning the SLIL, after also sectioning the dorsal radiocarpal and dorsal intercarpal ligaments, and then after also sectioning the radioscaphocapitate ligament. Results In the neutral position with the SLIL sectioned, the tensile force across the scapholunate joint was significantly larger in the extended position (45 N) than in the neutral position (25 N). Sectioning additional ligaments did not significantly increase the measured force. Extrapolation of the measured force in wrist extension with application of 1 time body weight suggests a force of 110 N would occur in the SLIL. Conclusions This study demonstrated that, with the SLIL sectioned, with or without the dorsal radiocarpal, dorsal intercarpal, and radioscaphocapitate ligaments sectioned, the tensile force across the scapholunate joint is greater in extension than in the neutral wrist position. Clinical relevance This study helps characterize the forces experienced across the scapholunate articulation when the SLIL is disrupted.

Introduction To determine if trabecular, total and cortical bone densities of the capitate, navicular, cuboid, and first cuneiform were equivalent to those of the scaphoid, such that these bones could be used in place of the scaphoid in evaluating new headless scaphoid compression screws. Methods Fifty scaphoids, capitates, naviculars, cuboids, and first cuneiforms were harvested from fresh frozen cadavers. The trabecular, total and cortical bone densities were measured using pQCT technology and statistically compared. Results A paired t comparison between paired scaphoids and capitates showed no difference between the trabecular bone densities. However, their total bone and cortical densities were found to be different. An independent measures ANOVA comparison of the five bones, showed no significant difference in mean trabecular density between the capitates, naviculars and first cuneiforms when compared to the scaphoids. However, the mean total and cortical densities of the first cuneiforms were less than the scaphoids and the mean trabecular, total and cortical bone densities of the cuboids were all less than the scaphoids. Discussion Compression fracture fixation studies of headless compression screws could be conducted using the capitate, navicular, and first cuneiform as models of the scaphoid when the supply of scaphoids is limited.

This study is a continuation of our previous investigation of the ligaments stabilizing the scaphoid and lunate. We evaluated the effects of sectioning the scapholunate interosseous ligament, radioscaphocapitate ligament, and scaphotrapezial ligament in 3 sequences.Three sets of 8 cadaver forearms were placed in a wrist simulator and moved in continuous cycles of flexion-extension and radial-ulnar deviation. Kinematic data for the scaphoid and lunate were recorded for each wrist in the intact state, after the 3 ligaments were sectioned in various sequences and after the wrist was moved through 1,000 cycles of motion.Sectioning only the scaphotrapezium ligament (ST) or the radioscaphocapitate ligament (RSC) resulted in minimal angular changes to the motion of the scaphoid and lunate. Sectioning of the scapholunate interosseous ligament (SLIL) or 1,000 cycles of repetitive wrist motion after ligament sectioning altered scaphoid and lunate kinematics.Based on these findings it was concluded that the SLIL is the primary stabilizer and the RSC and ST are secondary stabilizers of the scapholunate articulation. Repetitive motion after ligament injury probably results in further carpal instability.

Purpose The use of early active motion protocols after repair of the thumb ulnar collateral ligament (UCL) theoretically could avoid the complications of postoperative immobilization and improve ligament healing. The goals of this study were as follows: (1) to develop an accurate model of acute UCL rupture, (2) to determine the strain pattern in the UCL during constrained active thumb motion in intact and repaired thumbs, and (3) to determine the load to failure and strain of the UCL during rupture in forced abduction. Methods Sixteen fresh-frozen adult cadaver thumbs were mounted in a testing apparatus designed for testing the strain in the UCL during constrained active motion and abduction load to failure. Strain data for the UCL during motion were measured. Specimens were tested to failure using an MTS machine. Dynamic strain data were acquired throughout the loading cycle. Repair of the torn ligament was performed with a suture anchor technique. Strain and load-to-failure measurements then were repeated in the repaired specimens. Differences in the strain values and failure forces between the intact and repaired specimens then were compared. Results A reliable model of a UCL rupture was created. Strains in the UCL were similar during active motion in both intact and repaired specimens. A significant decrease in maximum load to failure was noted in repaired specimens but failure reliably occurred at strains 3 times greater than expected with active motion. Conclusions A controlled active motion therapy protocol after suture anchor repair of a ruptured UCL of the thumb is safe from a biomechanical point of view.

Defects from curettage for giant cell tumors of bone frequently have been reconstructed with bone cement with or without reinforcement pins. The biomechanical basis for the addition of reinforcement pins was examined using a model of a contained defect in the proximal tibia. Fifty-four cadaveric proximal tibia in matched pairs were divided into five test groups: intact tibia, medial metaphyseal contained defect, defect reconstructed with cement alone, defect reconstructed with cement and pins inserted within the medullary canal, and defect reconstructed with cement and pins inserted through the cortex. Specimens were tested to failure during one cycle of compressive loading. Defect specimens were significantly weaker and less stiff than intact specimens, establishing the validity of the model-contained defects. For the reconstructions, there was no statistically significant difference in load to failure, stiffness, energy to failure, or displacement for the polymethylmethacrylate treatment alone when compared with matched specimen receiving polymethylmethacrylate and pins treatment. Similarly, there was no statistical difference in biomechanical properties in comparing matched specimens treated with polymethylmethacrylate alone or polymethylmethacrylate/pins (cortex). For contained defects of the proximal tibia that are typical after curettage for giant cell tumor, there appears to be no biomechanical advantage to use of reinforcement pins in the cement.

The role of the dorsal radiocarpal wrist ligament has been the subject of several investigations. Several biomechanical studies have used sensors inserted dorsally into the wrist joint to evaluate its pressure distribution. The purpose of this study was to evaluate whether a dorsal capsulotomy that sections the dorsal radiocarpal ligament or insertion of a flexible pressure sensor alters scaphoid or lunate kinematics. Eight cadaver upper extremities were instrumented with motion sensors and placed in a wrist joint simulator. Each arm was moved through continual cycles of wrist flexion/extension and radial/ulnar deviation. Motion data were obtained in the intact state, after a capsulotomy, and after insertion of the sensor. We found that either a dorsal capsulotomy sectioning the dorsal radiocarpal ligament or insertion of the pressure sensor alters scaphoid and lunate kinematics during dynamic wrist motion. This study supports the clinical belief that this dorsal wrist ligament should be spared during surgical approaches to the carpus.
