Background The diagnostic value of screening the contralateral breast with MRI in patients with newly diagnosed breast cancer is poorly understood. Purpose To assess the impact of MRI for screening the contralateral breast on long-term outcomes in patients with newly diagnosed breast cancer and to determine whether subgroups with unfavorable prognoses would benefit from MRI in terms of survival. Materials and Methods Data on consecutive patients with newly diagnosed breast cancer seen from January 2008 to December 2010 were reviewed retrospectively. Patients with neoadjuvant chemotherapy, previous breast cancer, distant metastasis, absence of contralateral mammography at diagnosis, and no planned surgical treatment were excluded. Groups that did and did not undergo preoperative MRI were compared. Survival analysis was performed using the Kaplan-Meier method for propensity score-matched groups to estimate cause-specific survival (CSS) and overall survival (OS). A marginal Cox proportional hazards model was used to evaluate association of MRI and clinicopathologic variables with OS. Results Of 1846 patients, 1199 fulfilled the inclusion criteria. Median follow-up time was 10 years (range, 0-14 years). The 2:1 matched sample comprised 705 patients (470 in the MRI group and 235 in the no-MRI group); median ages at surgery were 59 years (range, 31-87 years) and 64 years (range, 37-92 years), respectively. MRI depicted contralateral synchronous disease more frequently (27 of 470 patients [5.7%] vs five of 235 patients [2.1%]

Purpose: Assess the upgrade rate of biopsy-proven stromal fibrosis without associated atypiato inform the most appropriate management approach in patients with biopsy-proven stromal fibrosis without associated atypia.Materials and Methods: REB-approved consecutive retrospective review of patient’s data of imaging-guided biopsy-proven stromal fibrosis carried out between 2014 and 2019. The upgrade rate of malignancy and the imaging features of the upgraded cases on surgical excision were recorded. The results were correlated with surgical histopathology as the ground truth or an uneventful clinical and radiological follow-up of at least 2 years. Predictors for malignancy were examined, and appropriate statistical tests were applied. Results: Out of 9814 consecutive imaging-guided breast biopsies performed during the study period, 334 patients (4.8%) median age 52 years, had biopsy-proven stromal fibrosis without atypia and fulfilled the inclusion criteria, representing the study cohort. Out of 334 cases, 4 cases (1.2%) were upgraded to malignancy in the two years follow-up. Older patients (> 50 years) are associated with an upgrade to malignancy (p ), and those with a personal history of breast cancer have a trend to upgrade to malignancy (p=0.066). No other clinical and imaging features are related to the malignancy upgrade.Conclusion: For benign radiology-pathology concordant stromal fibrosis on imaging-guided breast biopsy, the upgrade for biopsy results demonstrating stromal fibrosis without atypia is low at 1.2% but not 0; therefore diagnostic follow-up for 2 years as BI-RADS 3 (defined as > 50 years) and those with a personal history of breast cancer are subgroups of patients that may justify multidisciplinary discussion and possible re-sampling.

Purpose To identify features of the incidental MRI-detected enhancing lesions associated with false-positive outcomes, impacting patient care. Methods Institutional review board-approved retrospective imaging studies and patient's chart review of consecutive asymptomatic women with incidental MRI enhancing lesions detected between January-December 2018, who underwent MRI-guided biopsy. Lesions' frequency and imaging features were recorded, differentiating benign and high-risk lesions. The results were correlated with histopathology as the ground truth or uneventful follow-up of at least one year. Univariate analysis explored correlation between baseline variables and false-positive results. Results Two-hundred-nineteen women (median age, 49 years; range, 26–85 years) with 219 incidental MRI enhancing lesions that underwent MRI-guided vacuum-assisted biopsy during the study period formed the study cohort. Out of 219, 180 lesions (82.2%) yielded benign pathology results, 137 benign (76%) and 43 high-risk (24%) outcomes. Variables associated with true-positive results were age OR 1.05 (CI 95%, 1.02–1.08), p = 0.0015, irregular compared with oval/round mass shape lesion OR 11.2 (CI 95%, 1.6–78.4), p = 0.015, and clumped/clustered ring compared with homogeneous non-mass enhancement OR 3.17 (CI 95%, 1.38–7.28), p . T2-hyperintense mass-lesion correlated to the false-positive result OR 0.13 (95% CI, 0.02–0.76), p = 0.024. Conclusion Young patients, oval/round mass-lesion shape, T2-hyperintense mass-lesion, and homogeneous pattern of non-mass enhancement showed the strongest association with false-positive results of incidental MRI enhancing lesions. It may impact the patient's management, suggesting follow-up rather than interventional procedure when these demographic/imaging parameters are present, consequently decreasing patient's anxiety and health care costs.

This review focuses on the description of the operational measures implemented in a breast imaging department in accordance with existing recommendations for the treatment of breast cancer during the COVID-19 pandemic to make optimal use of finite resources without interruption of essential imaging services for breast cancer patients. It will also apply during a second-wave of the pandemic, which, according to experts, is inevitable and requires us to be better prepared.

The ability to quickly recognize postoperative anatomy is critical to accurately and efficiently interpret routine imaging studies and to diagnose postoperative complications.

This paper defines the property of kinematic fault tolerance and develops a general constructive proof of the existence of fault tolerant manipulators. After formulating the necessary and sufficient condition for fault tolerance, a planar manipulator with a minimal kinematic structure is designed. A generalization to spatial manipulators, using a numerical approach is formulated. Finally, it is shown how this approach enables us to solve the spatial design problem.
