Abstract

The management of unruptured intracranial aneurysms requires knowledge of the natural history of these lesions and the risks of repairing them.A total of 2621 patients at 53 participating centers in the United States, Canada, and Europe were enrolled in the study, which had retrospective and prospective components. In the retrospective component, we assessed the natural history of unruptured intracranial aneurysms in 1449 patients with 1937 unruptured intracranial aneurysms; 727 of the patients had no history of subarachnoid hemorrhage from a different aneurysm (group 1), and 722 had a history of subarachnoid hemorrhage from a different aneurysm that had been repaired successfully (group 2). In the prospective component, we assessed treatment-related morbidity and mortality in 1172 patients with newly diagnosed unruptured intracranial aneurysms.In group 1, the cumulative rate of rupture of aneurysms that were less than 10 mm in diameter at diagnosis was less than 0.05 percent per year, and in group 2, the rate was approximately 11 times as high (0.5 percent per year). The rupture rate of aneurysms that were 10 mm or more in diameter was less than 1 percent per year in both groups, but in group 1, the rate was 6 percent the first year for giant aneurysms (or =25 mm in diameter). The size and location of the aneurysm were independent predictors of rupture. The overall rate of surgery-related morbidity and mortality was 17.5 percent in group 1 and 13.6 percent in group 2 at 30 days and was 15.7 percent and 13.1 percent, respectively, at 1 year. Age independently predicted surgical outcome.The likelihood of rupture of unruptured intracranial aneurysms that were less than 10 mm in diameter was exceedingly low among patients in group 1 and was substantially higher among those in group 2. The risk of morbidity and mortality related to surgery greatly exceeded the 7.5-year risk of rupture among patients in group 1 with unruptured intracranial aneurysms smaller than 10 mm in diameter.

Abstract

Three small watersheds located in Westmoreland County, Virginia, were monitored to evaluate the impact of forest clearcutting on surface water quality and to evaluate the effectiveness of forestry best management practices (BMPs) for minimizing hydrologic and water quality impacts associated with timber harvesting. One watershed (7.9 ha) was clearcut without implementation of BMPs, one watershed (8.5 ha) was clearcut with the implementation of BMPs and a third watershed (9.8 ha) was left undisturbed as a control Forest clearcutting without BMP implementation reduced storm runoff volume and did not significantly change peak flow rates. Following site preparation, both storm flow volumes and peak flow rates decreased significantly. For the watershed with BMP implementation, storm flow volume decreased significantly following harvest, while peakflow increased. Site preparation did not change storm flow volumes over post-harvest conditions, bur did significantly reduce storm peak flow rates. Disruptions in subsurface flow pathways during harvest or rapid growth of understory vegetation following harvest could have caused these hydrologic changes. Harvest and site preparation activities significantly increased the loss of sediment and nutrients during storm events, Storm event concentrations and loadings of sediment, nitrogen, and phosphorus increased significantly following forest clearcutting and site preparation of the No-BMP watershed. Both the BMP watershed and the Control watershed showed few changes in pollutant storm concentrations or loadings throughout the study. Results of this study indicate forest clearcutting and site preparation without BMPs can cause significant increases in sediment and nutrient concentrations and loadings in the Virginia Coastal Plain. Abstract

A 51-item Death Anxiety Scale-Extended was constructed. This scale consists of the 15 Death Anxiety Scale items plus 36 new items which were generated on a rational basis that both survived content validity ratings and correlated at the .001 level with three out of four groups (one Kuwaiti, one Sudanese, and two American) participants. The Death Anxiety Scale-Extended correlated .81 with the Death Anxiety Scale. The 36 new items (Death Anxiety Scale-New) correlated .64 with the Death Anxiety Scale and .97 with the Death Anxiety Scale-Extended. The Kuder-Richardson formula 20 coefficients of internal consistency were .73 for the Death Anxiety Scale, .91 for the Death Anxiety Scale-New, and .92 for the Death Anxiety Scale-Extended. Abstract

Pretreatment measures including demographic variables, adjustment index variables and psychological testing variables were studied in relationship to treatment dropout and therapist ratings of overall response to treatment among PTSD veterans in an inpatient PTSD program. Analysis comparing a group of fourteen veterans who dropped out of treatment early and a random sample of fourteen who successfully completed treatment showed no significant differences. Analysis comparing a group of 35 veterans who received the highest therapist ratings on response to treatment with a group of 35 veterans receiving the lowest ratings on response to treatment also showed no significant differences. Analysis of subgroups of patients who had completed the Millon Clinical Multiaxial Inventory (MCMI) and received high versus low therapist ratings showed one significant difference on the hypomania scale. Overall findings on the MMPI and MCMI appeared similar to other investigations of PTSD. [ABSTRACT FROM AUTHOR]
