2. Escore VExUS na Alta Hospitalar como Preditor de Readmissão em Pacientes com Insuficiência Cardíaca Agudamente Descompensada: Estudo de Coorte

Resumo Fundamento A congestão venosa residual é um dos principais contribuintes para a readmissão de pacientes com insuficiência cardíaca, e o escore de ultrassonografia de excesso venoso (VExUS) é uma ferramenta potencialmente útil para avaliar a congestão sistêmica. Objetivos O objetivo do presente estudo foi investigar a associação entre o escore VExUS antes da alta hospitalar em pacientes com insuficiência cardíaca e o risco de readmissão por insuficiência cardíaca agudamente descompensada (ICAD) em até 90 dias após a alta. Métodos O presente estudo de coorte prospectivo envolveu adultos com sinais e sintomas de ICAD, fração de ejeção do ventrículo esquerdo de 40% ou menos (insuficiência cardíaca com fração de ejeção reduzida), sintomas de classe funcional II a IV da New York Heart Association e evidência clínica de congestão venosa necessitando de diuréticos intravenosos. Momentos antes da alta, realizamos avaliação do escore VExUS. O desfecho primário foi um desfecho composto de readmissão ou visitas de emergência devido à ICAD dentro de 90 dias após a alta hospitalar. A significância estatística foi estabelecida em p < 0,05. Resultados A coorte foi composta por 49 indivíduos, dos quais 11 (22,4%) apresentaram o desfecho primário. Na alta, 34,7% dos participantes tiveram escore VExUS de 2 ou 3. Os pacientes com VExUS de 2 e 3 tiveram maior proporção do desfecho primário quando comparados aos pacientes com VExUS de 0 (35,3% versus 9%, p = 0,044). Conclusões Uma proporção significativa de pacientes com insuficiência cardíaca com fração de ejeção reduzida admitidos por ICAD apresentou sinais clínicos e ultrassonográficos de congestão residual na alta. Pacientes com escore VExUS de 2 ou 3 no momento da alta hospitalar apresentaram maior risco de readmissões ou visitas de emergência por ICAD após 90 dias.

ABSTRACT: Treatment with prostaglandin F2α (PGF) induces ovulation and increases conception rates in cows, while improving embryo production in buffalos. However, its effect on superovulated cows is unknown. This study verified whether single PGF administration concurrent with artificial insemination (AI) improves fertilization and embryo production rates in superovulated cows. In each replicate, embryo donor cows were equally allocated to two groups: the untreated control and PGF groups. The latter of which received 482 µg of cloprostenol concurrent with the first AI. Each cow (n = 35) was subjected to two superovulations (SOV) in a crossover design (total = 70 embryo collections). In the control and PGF groups, respectively, the observed responses were [median (95% CI)]: 12 (10-18) and 15 (12-18) total structures, 9 (7-11) and 7 (6-10) viable embryos, 1 (0-1) and 1 (1-3) degenerated embryos, and 1 (0-3) and 2 (0-5) oocytes (P > 0.05). In conclusion, single PGF treatment concurrent with the first AI did not affect embryo production in superovulated cows.

Objetivo: Verificar a prevalência de sintomas para transtornos alimentares (TA), sobrepeso e obesidade em uma população de escolares do município de Bom Jesus-RS. Métodos: Trata-se de um estudo epidemiológico transversal. Foi utilizado um questionário autoaplicável onde se avaliou transtornos alimentares, insatisfação com a imagem corporal, bullying (Kidscape), sexo, idade e número de refeições diárias. Neste estudo também foram coletadas a massa corporal total e a estatura. Resultados: O estudo encontrou 29,9% dos indivíduos com sintomas para transtornos alimentares, 23,2% de indivíduos com sobrepeso e 6,7% com obesidade. Excesso de gordura corporal mostrou-se associado a sintomas de transtornos alimentares. Vítimas de bullying apresentaram quase 3 vezes mais chances de estarem com excesso de peso e indivíduos que realizavam 4 ou mais refeições apresentaram 50% menos chances de estarem com excesso de peso. Conclusão: Os resultados se assemelham a outras cidades do país e do mundo, mostrando a necessidade de medidas preventivas. ABSTRACT Prevalence of symptoms for eating disorders, overweight and obesity in schoolchildren from the city of Bom Jesus-RS Objective: To determine the prevalence of symptoms for eating disorders, overweight and obesity in a population of students in the city of Bom Jesus-RS. Methods: This is an epidemiological cross-sectional study. It was used a self-administered questionnaire for assessment of eating disorders, body image dissatisfaction, bullying (Kidscape), gender, age and number of daily meals. The total body mass and height were also collected in this study. Results: The study found 29.9% of individuals with symptoms for eating disorders, 23.2% of overweight and 6.7% obese. Excess body fat was associated with symptoms for eating disorders. Victims of bullying had almost 3 times more likely to be overweight and individuals who performed 4 or more meals showed 50% less likely to be overweight. Conclusions: The results are similar to other cities in the country and the world, showing the need for preventive measures.

Malignant otitis externa is an invasive infection of the external auditory canal and temporal bone with potentially life-threatening complications. Elderly patients with type 2 diabetes mellitus are the population most commonly affected by malignant otitis externa, but any type of immunosuppression predisposes to the disease. Prader–Willi syndrome is a genetic cause of obesity, often associated with insulin resistance and type 2 diabetes mellitus. This report describes a case of a 21-year-old male patient with Prader–Willi syndrome who had malignant otitis externa that progressed to sepsis during hospitalization. To the best of the authors’ knowledge, this is the first description of malignant otitis externa in a young patient with Prader–Willi syndrome.

Vivemos atualmente a transição nutricional onde a obesidade transformou-se na nova epidemia do século XXI. O presente estudo teve por objetivo verificar a tendência do excesso de gordura corporal em escolares de 11 e 12 anos, mensurados em 2 distintos momentos, 2007 e 2011 na cidade de Caxias do Sul. O estudo em questão conta com dados de 2 estudos já realizados. No estudado de 2007 foram avaliados 1.440 escolares de 7 a 12 anos enquanto no estudo de 2011 foram avaliados 1.191 escolares entre 11 e 14 anos. Os indicadores antropométricos de obesidade utilizados foram as dobras cutâneas do tríceps e da panturrilha (em milímetros) nos dois estudos. No ano de 2007 a prevalência de gordura acima do desejado era 21,8% e em 2011 passou para 39,5% nos meninos. No sexo feminino, no ano de 2007 a gordura acima do desejado era 16%, e no ano de 2011 passou para 40,8%. Conclui-se que o excesso de gordura corporal teve crescimento significativo nos escolares no decorrer do período avaliado. Sendo assim o presente estudo propicia resultados que poderão auxiliar na implementação de programas que visam à prevenção e a promoção da saúde relacionados à obesidade e sobrepeso em escolares. ABSTRACT Excess body fat trends in schoolchildren from Caxias do Sul city Introduction: We live today the nutrition transition where obesity has become the new epidemic of XXI century. Objective: This study aimed to verify the tendency of body fat excess in 11 and 12 students, measured at two different times, 2007 and 2011 in the city of Caxias do Sul. Methods: This study has data from two previous studies. In the 2007 study, were evaluated 1.440 students aged 7 to 12 years old, while in the 2011 study, were evaluated 1.191 students between 11 and 14 years old. In both studies, the skinfold from triceps and calf (in millimeters) were used as anthropometric indicators of obesity. Results: In 2007 the prevalence of fat above the desired level was 21.8% and in 2011 became 39.5% in boys. In the female gender, in 2007 the fat above the desired was 16%, and in 2011 came to 40.8%. Conclusion: It is concluded that excess body fat had significant growth in students during the study period. Thus, the present study provides results that can assist in the implementation of programs aimed at prevention and health promotion related to obesity and overweight in schoolchildren.

8. Occurrence of Aspergillus sp., Fusarium sp., and aflatoxins in corn hybrids with different systems of storage

The aim of this study was to assess alternatives for viable corn storage for small rural properties in two annual storage experiments. A 4×5 factorial design was used with four types of storage (conventional bags, hermetic bags, metal silos and corncobs) and five periods of storage (0, 3, 6, 9 and 12 months). We used corn hybrids 2B688RR and 30K73Hx cultivated in winter 2012 and summer 2012/2013 in the city of Dois Vizinhos, Paraná, Brazil. The moisture contents, counts of Aspergillus sp. and Fusarium sp., and the occurrence of aflatoxins (B1, B2, G1 and G2) were assessed. The kernels stored in hermetic bags had lower moisture contents. Aspergillus sp. and Fusarium sp. were observed in 20.37 and 86.11% of winter storage samples, respectively, and in 83.3 and 91.6% of summer storage samples, respectively. The storage system and time of storage had no influence on the occurrence of Aspergillus sp. and aflatoxins in the winter crop samples. The corncobs from the summer crop samples had the lowest counts of Aspergillus sp. and did not have aflatoxins. We detected aflatoxins at concentrations of 2.8-14.5 and 3-197.5 µg kg-1 in the winter and summer crop samples, respectively.

A 10-year-old female presented with a palpable mass occupying the entire abdomen. Computerized tomography scan showed a large expansive lesion measuring 22 cm × 20 cm × 13 cm. The mass was resected and the diagnosis of leiomyoma was made from immunehistochemical findings. Mesenteric leiomyoma is an uncommon tumor among gastrointestinal stromal tumors.

11. Otoacoustic Emissions in Children with Long-Term Middle Ear Disease

Introduction: Otoacoustic emissions (OAEs) evaluate the functional status of the cochlea. Repeated otitis media (OM) can cause changes in the peripheral structures of the auditory system, and, in this way, middle ear infection may irreversibly damage the middle ear, or even the cochlea. Objectives: To analyze the results of transiently evoked otoacoustic emissions (TEOAEs) and distortion product otoacoustic emissions (DPOAEs) in individuals with a history of OM. Method: Participants with 8 to 16 years of schooling were split into two groups: a control group (CG) of 50 subjects who had no history of otological disease and an experimental group (EG) of 50 subjects who had a history of recurrent otitis in childhood and had consequently undergone myringotomy to insert bilateral ventilation tubes. All children underwent basic audiological assessment (tonal audiometry, speech audiometry, and immittance testing) and otoacoustic emission testing (TEOAEs and DPOAEs). Results: There were no significant differences between the groups when audiometrically tested via air and bone conduction. OAEs were found in all CG subjects. For the EG, there were no TEOAE responses in 17 ears and no DPOAEs in nine ears; response amplitudes were lower at all frequencies. The emission level and the signal-to-noise ratio were statistically different between the two groups, and OAEs in the EG were statistically smaller compared to the GC. Conclusion: In the EG, responses were more likely to be absent and were of statistically smaller amplitude compared to the CG. A history of repeated OM apparently interferes with the generation and transmission of TEOAEs and DPOAEs.

- Abstract
Background: To identify indicators of guideline-concordant care in lung cancer, to implement such indicators with cancer registry data linked to health databases, and to pilot them in a cohort of patients from the cancer registry of the Milan Province. Methods: Thirty-four indicators were selected by revision of main guidelines by cancer epidemiologists, and then evaluated by a multidisciplinary panel of clinicians involved in lung cancer care and working on the pathway of lung cancer diagnosis and treatment in the Lombardy region, Italy. With a modified Delphi method, they assessed for each indicator the content validity as a quality measure of the care pathway, the degree of modifiability from the health professional, and the relevance to the health professional. Feasibility was assessed using the cancer registry and the routine health records of the Lombardy region. Feasible indicators were then calculated in the cohort of lung cancer patients diagnosed in 2007–2012 derived from the cancer registry of the Milan Province. Criterion validity was assessed reviewing clinical records of a random sample of 114 patients (threshold for acceptable discordance ≤20%). Finally, reliability was evaluated at the provider level. Results: Initially, 34 indicators were proposed for evaluation in the first Delphi round. Of the finally 22 selected indicators, 3 were not feasible because the required information was actually not available. The remaining 19 were calculated on the pilot cohort. After assessment of criterion validity (3 eliminated), 16 indicators were retained in the final set and evaluated for reliability. Conclusion: The developed and piloted set of indicators is now available to implement and monitor, over time, quality initiatives for lung cancer care in the studied health system.

13. VExUSScore in the Management of Patients With Acute Kidney Injury in the Intensive Care Unit: AKIVEXStudy

- Abstract
Venous congestion is a potential cause of acute kidney injury (AKI) and venous excess ultrasound (VExUS) score is a potentially useful tool in this scenario. The aim of this study is to verify whether the VExUS score can serve as a guide to decongestion in patients with severe AKI and whether the modification of the score can be associated with an increase in the number of renal replacement therapy (RRT)‐free days in 28 days. This quasi‐experimental study was conducted in patients admitted to the intensive care unit who developed severe AKI. The intervention was to suggest to the attending physician the use of diuretic in patients with VExUS >1. After 48 hours, a new VExUS assessment was performed. Primary outcome was RRT‐free days at Day 28. Ninety patients were included. Patients with a VExUS score >1 (n = 36) at enrollment had a greater use of diuretics in the following 48 hours (75.0%, n = 27) than patients with a VExUS ≤1 (n = 54) at enrollment (38.9%, n = 21), P= .001. Patients who reduced the VExUS score had a significantly greater number of RRT‐free days at Day 28 (28.0; 8.0–28.0) when compared with those who did not reduce (15.0; 3.0–27.5), P= .012. We found a higher diuretic use in patients with a higher VExUS score, and patients who reduced the VExUS in 48 hours had significantly more RRT‐free days in 28 days.

Purpose Therapeutic strategies for prostate cancer (PCa) have been evolving dramatically worldwide. The current article reports on the evolution of surgical management strategies for PCa in Italy. Methods The data from two independent Italian multicenter projects, the MIRROR-SIU/LUNA (started in 2007, holding data of 890 patients) and the Pros-IT-CNR project (started in 2014, with data of 692 patients), were compared. Differences in patients' characteristics were evaluated. Multivariable logistic regression models were used to identify characteristics associated with robot-assisted (RA) procedure, nerve sparing (NS) approach, and lymph node dissection (LND). Results The two cohorts did not differ in terms of age and prostate-specific antigen (PSA) levels at biopsy. Patients enrolled in the Pros-IT-CNR project more frequently were submitted to RA (58.8% vs 27.6%,p < 0.001) and NS prostatectomy (58.4% vs. 52.9%,p = 0.04), but received LND less frequently (47.7% vs. 76.7%,p < 0.001), as compared to the MIRROR-SIU/LUNA patients. At multivariate logistic models, Lower Gleason Scores (GS) and PSA levels were significantly associated with RA prostatectomy in both cohorts. As for the MIRROR-SIU/LUNA data, clinical T-stage was a predictor for NS (OR = 0.07 for T3, T4) and LND (OR = 2.41 for T2) procedures. As for Pros-IT CNR data, GS >= (4 + 3) and positive cancer cores >= 50% were decisive factors both for NS (OR 0.29 and 0.30) and LND (OR 7.53 and 2.31) strategies. Conclusions PCa management has changed over the last decade in Italian centers: RA and NS procedures without LND have become the methods of choice to treat newly medium-high risk diagnosed PCa.

- Abstract
BACKGROUND: We assessed patients and tumor characteristics, as well as health-related quality of life (HRQoL) items, associated with curative intent treatment decision-making in clinically localized prostate cancer (PCa) patients. METHODS: Clinically localized PCa treated with either radical prostatectomy (RP) or radiation therapy (RT) within 12 months from diagnosis were abstracted from The PROState cancer monitoring in ITaly, from the National Research Council (Pros-IT CNR) database. Multivariable logistic regression (MLR) models predicting RT vs. RP were fitted, after adjustment for HRQoL items, patients and tumor characteristics. RESULTS: Of 1041 patients, 631 (60.2%) were treated with RP and 410 (39.8%) with RT. Relative to RT, RP patients were younger age (mean age 64.5±6.6 vs. 71.4±4.9, P<0.001) and had higher rates of D’Amico low-intermediate risk groups (31.8 vs. 21.9% low, 46.3% vs. 43.5% intermediate and 21.9% vs. 34.6% high risk, P<0.001). Overall, 93.2% of RP patients were enrolled by urologists and 82.7% of RT patients by radiation oncologists. RP patients had generally higher means values of HRQoL items. In MLR models, higher RT rates were independently associated with more advanced age (odds ratio [OR] 6.14, P<0.001) and BMI≥30 kg/m2 (OR 1.78, P<0.001). Conversely, lower rates of RT were independently associated with married (OR 0.55, P=0.01) and worker status (OR 0.52, P=0.004), enrollment in academic centers (OR 0.59, P=0.005) and higher physical composite score (OR 0.88, P=0.03) and baseline sexual function items (OR 0.92, P<0.001). CONCLUSIONS: Most patients with clinically localized prostate cancer undergoing definitive treatment at Italian institutions receive RP instead of RT. Moreover, those who are younger, married, working, as well as those with better physical and sexual function are more likely to undergo surgery.

The cyanobacteria Spirulina platensis has been cultivated in a fed batch process with urea as the nitrogen source, in order to obtain dehydrated biomass for incorporation into food, aiming at nutritional enrichment and the production of a functional character, due to the amount of proteins, vitamins and several bioactive compounds found in this cyanobacterium. In this study, response surface methodology was used to analyze the substitution of wheat flour by cassava in the development of doughnuts with added Spirulina platensis biomass and inverted sugar, in order to increase the rate of the Maillard's reaction and mask the green colour of the biomass. The formulations were evaluated in relation to their proximate, sensory and technological compositions, which, when compared to the standard formulation, without the addition of S. platensis biomass and inverted sugar, showed the feasibility of adding the biomass to bestow nutritional enrichment without significantly affecting the sensory acceptance of the product or its typical characteristics.Cultivos da cianobactéria Spirulina platensis vêm sendo conduzidos utilizando-se ureia como fonte de nitrogênio, em processo descontínuo alimentado, para obtenção de biomassa desidratada para ser incorporada em alimentos visando enriquecimento nutricional e conferir caráter funcional em virtude da composição rica em proteínas, vitaminas e diversos componentes bioativos. Neste trabalho, foi estudada a substituição da farinha de trigo pela mandioca, além da adição de biomassa de Spirulina platensis e açúcar invertido, para desenvolver um 'sonho' com elevada taxa da Reação de Maillard para mascarar a coloração verde da biomassa, por meio da metodologia de superfície de resposta. As formulações foram avaliadas em relação aos aspectos de composição centesimal, sensoriais e tecnológicos que, quando comparados com o padrão, sem adição de biomassa de S. platensis e açúcar invertido, demonstraram a viabilidade da adição da biomassa para conferir enriquecimento nutricional sem afetar, de forma significativa, a aceitação sensorial e as características típicas do produto obtido.

The attributes of five packages of orange juice and orange nectar were evaluated by focus group and conjoint analysis techniques. The attributes and levels were identified such as brand name (well known and less known), price (high and low), information (“natural, “no food preservative”, “vitamin C-rich” and without information) and type of juice (juice and nectar), as being relevant to consumers purchase attitude. Eight packages images containing different combinations of the chosen attributes were elaborated, according to a fractional factorial design (24-1). The images were evaluated, in terms of purchase intention, by 101 consumers. There was a segmentation of the consumers in three groups according to the similarity in the purchase behavior. The previous knowledge of the brand name, low price and the declarations of “natural”, “no food preservatives” and “vitamin C-rich” in the frontal part of the packing reveal to be important to improve the commercialization of the product.Atributos de cinco embalagens comerciais de suco e néctar de laranja foram avaliados por meio das técnicas grupos de foco e análise conjunta. Foram levantados os atributos e níveis, marca (muito conhecida e menos conhecida), preço (alto e baixo), declarações (“natural, sem conservantes, rico em vitamina C” e sem informação) e tipo de bebida (suco e néctar), como relevantes na atitude de compra de consumidores. Elaboraram-se oito imagens de embalagens contendo diferentes combinações dos atributos levantados, obedecendo ao delineamento fatorial fracionado (24-1). As imagens foram avaliadas, quanto à intenção de compra, por 101 consumidores. Houve a segmentação dos consumidores em três grupos de acordo com a similaridade no comportamento de compra. O conhecimento prévio da marca, preço baixo e as declarações “natural”, “sem conservantes” e “rico em vitamina C” na face frontal da embalagem mostram-se importantes para a maior comercialização do produto.

O objetivo desta revisão sistemática foi verificar a relação das concentrações de IL-6, TNF-α e MCP-1 em crianças com excesso de massa corporal. As bases de dados investigadas PUBMED, SciELO, LILACS e Periódico Capes foram consultadas retrospectivamente para os últimos seis anos (2009 a 2014) utilizando combinações de palavras chaves como inflamação, IL-6, TNF-α, MCP-1 combinadas com crianças e escolares. Foram analisados 21 artigos. Foi encontrado associação entre sobrepeso/obesidade com IL-6, TNF-α e MCP-1 em 73,3% (11/15), 80% (12/15) e 60% (3/5) dos estudos que analisaram tais marcadores, respectivamente. Crianças com excesso de massa corporal possuem concentrações elevadas de IL-6, TNF-α e MCP-1 resultando em inflamação sistêmica crônica e aumentando o risco de desenvolvimento de outras doenças cardiovasculares.

- Abstract
Background There are several treatments available to newly diagnosed prostate cancer (PCA) patients. Although surgery and radiotherapy (RT) with or without androgen deprivation therapy (ADT) are widely adopted treatment options for localized PCA together with active surveillance (AS), there is no consensus nor randomised trials on treatment selection, prospective quality of life (QOL), along with toxicity outcomes and according to treatment modality in the Italian population. The current study aimed to describe clinical-therapeutic features and QOL at PCA diagnosis, according to different treatment patterns in a large prospective, Italian population, enrolled in the Pros-IT CNR study. Methods The Pros-IT CNR is an on-going national, multicenter, observational, prospective study on patients affected by PCA who have been referred by 97 Italian Urology, Radiation Oncology and Medical Oncology facilities participating in the project. The possible relationships between the treatment patterns reported in the 6 month follow-up case report form and patients’ features at diagnosis were evaluated using exploratory multiple correspondence analysis (MCA) and other data analysis method. Results At diagnosis, surgery and AS patients were significantly younger, had fewer comorbidities, lower PSA levels and Gleason Score (GS) values; they were also diagnosed at an earlier stage of disease with respect to the RT or ADT patients who showed significantly worse QoL scores at the time of diagnosis. Conclusions An analysis of the data collected at baseline and 6 months later uncovered substantial differences in ages, comorbidities, clinical and QOL features in the various treatment groups. These findings do not fully reflect the current PCA treatment guidelines and suggest the need for a multidisciplinary consensus guideline to ameliorate both the counselling and treatments of PCA patients.

- Abstract
Background: To report health-related quality of life outcomes as assessed by validated patient-reported outcome measures (PROMs) after radical prostatectomy (RP).-Methods: This study analyzed patients treated with RP within The PROState cancer monitoring in Italy, from the National Research Council (Pros-IT CNR). Italian versions of Short-Form Heath Survey and university of California los Angeles-prostate cancer index questionnaires were administered. PROMs were physical composite scores, mental composite scores and urinary, bowel, sexual functions and bothers (UF/B, BF/B, SF/B). Baseline unbalances were controlled with propensity scores and stabilized inverse weights; differences in PROMs between different RP approaches were estimated by mixed models. Results: Of 541 patients treated with RP, 115 (21%) received open RP (ORP), 90 (17%) laparoscopic RP (LRP) and 336 (61%) robot-assisted RP (RARP). At head-to-head-comparisons, RARP showed higher 12-month UF vs. LRP (interaction treatment ∗ time p = 0.03) and 6-month SF vs. ORP (p < 0.001). At 12-month from surgery, 67, 73 and 79% of patients used no pad for urinary loss in ORP, LRP and RARP respectively (no differences for each comparison). Conversely, 16, 27 and 40% of patients declared erections firm enough for sexual intercourse in ORP, LRP and RARP respectively (only significant difference for ORP vs. RARP, p = 0.0004). Conclusions: Different RP approaches lead to significant variations in urinary and sexual PROMs, with a general trend in favour of RARP. However, their clinical significance seems limited.

Background There are several treatments available to newly diagnosed prostate cancer (PCA) patients. Although surgery and radiotherapy (RT) with or without androgen deprivation therapy (ADT) are widely adopted treatment options for localized PCA together with active surveillance (AS), there is no consensus nor randomised trials on treatment selection, prospective quality of life (QOL), along with toxicity outcomes and according to treatment modality in the Italian population. The current study aimed to describe clinical-therapeutic features and QOL at PCA diagnosis, according to different treatment patterns in a large prospective, Italian population, enrolled in the Pros-IT CNR study. Methods The Pros-IT CNR is an on-going national, multicenter, observational, prospective study on patients affected by PCA who have been referred by 97 Italian Urology, Radiation Oncology and Medical Oncology facilities participating in the project. The possible relationships between the treatment patterns reported in the 6 month follow-up case report form and patients' features at diagnosis were evaluated using exploratory multiple correspondence analysis (MCA) and other data analysis method. Results At diagnosis, surgery and AS patients were significantly younger, had fewer comorbidities, lower PSA levels and Gleason Score (GS) values; they were also diagnosed at an earlier stage of disease with respect to the RT or ADT patients who showed significantly worse QoL scores at the time of diagnosis. Conclusions An analysis of the data collected at baseline and 6 months later uncovered substantial differences in ages, comorbidities, clinical and QOL features in the various treatment groups. These findings do not fully reflect the current PCA treatment guidelines and suggest the need for a multidisciplinary consensus guideline to ameliorate both the counselling and treatments of PCA patients.

- Abstract
To systematically review the literature regarding the effectiveness of different positioning methods for procedural pain relief in neonates admitted to the Neonatal Intensive Care Unit (NICU). A systemized search of the literature was carried out by means of two independent evaluators through the systematic search of electronic index databases. A search for relevant studies was performed in four databases (Medline, Web of Science, Scopus, and BVS-BIREME). Manual searches were conducted on suitable references from the included articles, and 1,941 publications were eligible for the analysis. The flowchart for the articles' selection was based on Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) criteria, and in relation to bias risks according to the Physiotherapy Evidence Database (PEDro) scale. Based on the inclusion criteria, only 20 publications remained. According to the PEDro scale, 75% of the studies presented good methodological quality, with scores between 6 and 8, and 5% scored ≤4 points. None of them were blinded in relation to the therapies, but all of them performed intergroup statistical comparisons. According to the results of this review, we recommend facilitated tucking by parents (FTP) in NICU of at least 30 minutes duration, starting 15 minutes before, during the painful procedure, and 15 minutes after to relieve pain and to stabilize the physiological, hormonal, and behavioral responses of the newborns. Positioning should be used as a nonpharmacological strategy for procedural pain relief in newborns. This review showed that facilitated tucking by parents for 30 minutes was the best position for pain relief in premature newborns during procedures in the NICU. Positioning is recommended as a nonpharmacological method for pain relief; FTP of at least 30 minutes duration should be the first positioning choice during procedures in the NICU. - Abstract
Background: The National Research Council (CNR) prostate cancer monitoring project in Italy (Pros-IT CNR) is an observational, prospective, ongoing, multicentre study aiming to monitor a sample of Italian males diagnosed as new cases of prostate cancer. The present study aims to present data on the quality of life at time prostate cancer is diagnosed. Methods: One thousand seven hundred five patients were enrolled. Quality of life is evaluated at the time cancer was diagnosed and at subsequent assessments via the Italian version of the University of California Los Angeles-Prostate Cancer Index (UCLA-PCI) and the Short Form Health Survey (SF-12). Results: At diagnosis, lower scores on the physical component of the SF-12 were associated to older ages, obesity and the presence of 3+ moderate/severe comorbidities. Lower scores on the mental component were associated to younger ages, the presence of 3+ moderate/severe comorbidities and a T-score higher than one. Urinary and bowel functions according to UCLA-PCI were generally good. Almost 5% of the sample reported using at least one safety pad daily to control urinary loss; less than 3% reported moderate/severe problems attributable to bowel functions, and sexual function was a moderate/severe problem for 26.7%. Diabetes, 3+ moderate/severe comorbidities, T2 or T3-T4 categories and a Gleason score of eight or more were significantly associated with lower sexual function scores at diagnosis. Conclusions: Data collected by the Pros-IT CNR study have clarified the baseline status of newly diagnosed prostate cancer patients. A comprehensive assessment of quality of life will allow to objectively evaluate outcomes of different profile of care.

ABSTRACT The objective of this study was to determine the effect of Azospirillum brasilense inoculation and different nitrogen (N) rates applied as topdressing on the productivity of a maize crop and the nutritional value of maize silage. Two experiments were conducted in the 2012/2013 and 2013/2014 harvests. Treatments were distributed in a randomized block design in a factorial arrangement, which consisted of two maize hybrids (AS 1572 and Defender) coupled with nitrogen rates (0, 60, 120, 240, and 480 kg ha-1), inoculated or uninoculated with A. brasilense. Inoculated seeds were treated with the A. brasilense strains Ab-V5 and Ab-V6. Inoculation with A. brasilense showed interaction with the hybrids, agricultural years, and nitrogen rates for the maize plant yield. In the 2012/2013 agricultural year, inoculation increased the AS 1572 hybrid silage yield by 6.16% and, in the 2013/2014 harvest, A. brasilense inoculation produced an increase of 16.15% for the Defender hybrid. Nitrogen fertilization applied at 0, 60, and 120 kg ha-1 N benefited the plants inoculated with A. brasilense. The statistical equations revealed that N rates between 0 and 184 kg ha-1 in A. brasilense inoculated plants raised the plant productivity for silage when compared with the control plants. Regarding the nutritional value of the silage, inoculation with A. brasilense increased the ether extract levels and total digestible nutrients and reduced the amount of acid detergent fiber. For all this, positive results with inoculation for silage yield are dependent on nitrogen fertilization rate. Inoculation with A. brasilense can promote changes in the maize silage quality, but with obtained results it is not possible to definitely conclude upon nutritive value of maize silage.

Os atletas receberam aleatoriamente bebida carboidratada (CHO) ou placebo (PLA) e participaram de duas sessões de treino de rúgbi, separados por sete dias. Coletas de saliva foram feitas antes (Pré-E), imediatamente após (Pós-E) e 1 h após (1 h Pós-E) o jogo-treino. Houve diferença significativa em taxa de secreção de IgA-s para PLA somente no tempo 1 h Pós-E. DATC, dias antes de treino de rúgbi, preserva função imunológica oral independente da suplementação de CHO durante treino.

- Abstract
Introduction: Several efforts are being implemented at the European level to measure provision of up-to-date radiation treatments across the continent.Methods: A snapshot survey involving all radiation oncology centers within Lombardy, Italy, was performed in 2012 and repeated in 2014 and 2016, in cooperation with regional governmental officers. Centers were asked to provide detailed information concerning all individual patients being treated on the index day, and to report data on available local resources.Results: We observed an increase in the number of centers and of megavoltage units (MVU) (from 76 to 87, i.e., 8.7 MVU per million inhabitants in 2016). Mean number of MVU per center was 2.5. Average age of MVU increased from 5.3 to 7.5 years and patients on the waiting list also increased. Conformal 3D radiotherapy (RT) treatments decreased from 56% to 42% and were progressively replaced by intensity-modulated RT treatments (from 39% to 49%). Waiting times were overall satisfactory. Radiation oncologists treated on average 152 and radiation therapists 100 RT courses per year. Average reimbursement per course was €4,879 (range €2,476–€8,014).Conclusions: The methodology of snapshot survey proved feasible and provided valuable information about radiation oncology provision and accessibility in Lombardy.

Abstract: Aims: To determine the incidence of cancer treatment-induced diarrhoea in patients submitted to irradiation. Methods: Forty-five Italian radiation oncology departments took part in this prospective observational study and a total of 1020 patients were enrolled. The accrual lasted three consecutive weeks; evaluation was based on diary cards filled in daily by patients during radiotherapy and one week after cessation. Diary cards recorded both the onset and intensity of diarrhoea. Results: A total of 1004 patients were eligible for this analysis. 147/1004 (14.6%) patients had diarrhoea. The median minimum number of daily events was 1 (range 1–7) with a median maximum events of 3 (range 1–23). 82/147 patients (56.2%) had a drug prescription for diarrhoea. In the evaluation of the onset of diarrhoea, in multivariate analysis, we found the following factors to be statistically significant predictors of an increased likelihood of diarrhoea: primitive tumour site, therapeutic purpose and field size. Conclusions: Patients with abdominal-pelvic cancer, treated with curative purpose and using large field sizes are at high risk of cancer treatment-induced diarrhoea. Diarrhoea was also observed in patients treated at other sites. In this population group there is the need for more stringent monitoring during the delivery of radiation therapy. 49. Radiotherapy in Italy for Non-Small Cell Lung Cancer: Patterns of Care Survey

- Abstract
Aims and Background Surveys in clinical practice are useful to find how current clinical approaches follow recommendations from evidence-based medicine, to stimulate discussion in a multidisciplinary team, and to hypothesize collaborative multi-centric trials. To assess management strategies for the use of radiotherapy in the treatment of lung cancer in Italy, in 2009, the Italian Society of Radiation Oncology Lung Cancer Study Group proposed the survey to all Italian radiation oncology institutions. Results were compared with literature data and international reports.Study Design Questionnaires on patterns of care of non-small cell lung cancer were sent to radiation oncology centers active at June 2009 and evaluated data recorded in 2008.Results A total of 65 of 143 Italian centers responded to the questionnaire. The responding centers reflect the distribution of radiotherapy centers throughout the country. Of the treated patients, 55.2% were stage III, and most cases had a good performance status. FDG-PET was routinely used by 51% of centers for diagnostic and contouring phases. Postoperative radiotherapy was prescribed to pN1 and pN2 patients in 42.2% and 98.5%, respectively. The possible use of neo-adjuvant concomitant chemoradiation was declared by 70% of responders. A sequential chemoradiation approach was actually used in 43.6% of cases, induction chemotherapy followed by concomitant radiochemotherapy in 42.4%, and upfront concomitant radiochemotherapy in only 14%. In 53% of the institutions, patients have a clinical examination by a radiation oncologist only after the beginning of chemotherapy and in 82.4% of cases they have already received 2–4 cycles of chemotherapy. Most of the institutions exclude elective nodal irradiation from routine application. Total dose and fractionation in adjuvant, neoadjuvant, curative and palliative settings confirm literature data. There were significant differences in treatment planning constraints applied for lung, esophageal and cardiac tissues. Of the responding centers, 41% had stereotactic therapy for primary inoperable lung cancer and for metastatic lesions.Conclusions In Italy, daily practice differs in some ways from the evidence supported by the results of meta-analyses/clinical trials as regards concurrent chemoradiation approaches. It could be postulated that there is an urgent need for groups that collaborate with the other societies involved in the treatment of non-small cell lung cancer in order to offer the best therapy to our patients.

Aims and background In 2006 a survey was performed to define the resources available in Italy for the provision of radiotherapy services to head and neck cancer patients. This was the first initiative of the newly founded Head and Neck Group of the Italian Association for Radiation Oncology.Methods A questionnaire was sent to all 138 radiotherapy centers active in the country. Items investigated included total numbers of head and neck cancer patients treated per year, waiting time before the start of treatment, general technical issues, and integration with surgery and chemotherapy.Results Sixty-nine questionnaires were returned (50% response rate). The total number of patients treated was 4,670, averaging 68 cases per center. The larynx was the primary site most frequently involved. Average waiting time was 30 days and 47 days for nonresected and postoperative cases, respectively. The combination of chemotherapy and radiotherapy was delivered to nonresected and resected patients in 96% and 54% of centers, respectively. Survey response rates, waiting time, and the use of organ preservation protocols were the issues showing more variations across the country.Conclusions This survey provides important data on radiotherapy resources available for head and neck cancer patients in Italy. The evidence of significant differences across the country concerning several relevant issues and the potential for cooperative clinical efforts in this relatively rare group of diseases urge the Group to plan further initiatives.
