To verify whether it is possible, in people with diabetes mellitus (DM) considered at very high cardiovascular (CV) risk, stratify this risk better and identify significant modifiable risk factor (including lifestyle habits) to help patients and clinicians improve CV prevention. People with DM and microvascular diseases or one or more CV risk factors (hypertension, hyperlipidemia, smoking, poor dietary habits, overweight, physical inactivity) included in the Risk and Prevention study were selected. We considered the combined endpoint of non-fatal acute myocardial infarction and stroke and CV death. A multivariate Cox proportional analysis was carried out to identify relevant predictors. We also used the RECPAM method to identify subgroups of patients at higher risk. In our study, the rate of major CV events was lower than expected (5 % in 5 years). Predictors of CV events were age, male, sex, heart failure, previous atherosclerotic disease, atrial fibrillation, insulin treatment, high HbA1c, heart rate and other CV diseases while being physically active was protective. RECPAM analysis indicated that history of atherosclerotic diseases and a low BMI defined worse prognosis (HR 4.51 95 % CI 3.04–6.69). Among subjects with no previous atherosclerotic disease, men with HbA1c more than 8 % were at higher CV risk (HR 2.77; 95 % CI 1.86–4.14) with respect to women. In this population, the rate of major CV events was lower than expected. This prediction model could help clinicians identify people with DM at higher CV risk and support them in achieving goals of physical activity and HbA1c.

Background Trials have shown a beneficial effect of n-3 polyunsaturated fatty acids in patients with a previous myocardial infarction or heart failure. We evaluated the potential benefit of such therapy in patients with multiple cardiovascular risk factors or atherosclerotic vascular disease who had not had a myocardial infarction. Methods In this double-blind, placebo-controlled clinical trial, we enrolled a cohort of patients who were followed by a network of 860 general practitioners in Italy. Eligible patients were men and women with multiple cardiovascular risk factors or atherosclerotic vascular disease but not myocardial infarction. Patients were randomly assigned to n-3 fatty acids (1 g daily) or placebo (olive oil). The initially specified primary end point was the cumulative rate of death, nonfatal myocardial infarction, and nonfatal stroke. At 1 year, after the event rate was found to be lower than anticipated, the primary end point was revised as time to death from cardiovascular causes or admission to the hospital for cardiovascular causes. Results Of the 12,513 patients enrolled, 6244 were randomly assigned to n-3 fatty acids and 6269 to placebo. With a median of 5 years of follow-up, the primary end point occurred in 1478 of 12,505 patients included in the analysis (11.8%), of whom 733 of 6239 (11.7%) had received n-3 fatty acids and 745 of 6266 (11.9%) had received placebo (adjusted hazard ratio with n-3 fatty acids, 0.97; 95% confidence interval, 0.88 to 1.08; P=0.58). The same null results were observed for all the secondary end points. Conclusions In a large general-practice cohort of patients with multiple cardiovascular risk factors, daily treatment with n-3 fatty acids did not reduce cardiovascular mortality and morbidity. Quantitative insight into the impact of land use on groundwater composition is vital for the sustainable management of aquifers, especially those situated in urban areas. The objective of the present study was to determine the effects of land use on groundwater composition throughout the Netherlands, relative to the influence of groundwater age, groundwater origin, and geochemical processes. Using a nationwide dataset on shallow groundwater from the Dutch National Groundwater-Monitoring Network, land-use effects were quantified statistically for all major ionic substances. Analysis of variance was applied to the entire dataset, to a priori groups based on groundwater age, and to non-a priori statistical clusters determined by fuzzy c-means clustering. The results indicated that the effect of land use on groundwater composition in the Netherlands strongly depends on geochemical processes and groundwater age. Land-use effects were most pronounced in younger, geochemically less-evolved groundwater: for example, urban areas showed an increase in Ca, Na, Cl, and HCO3 by a factor 2-4, an increase in K and B by a factor 4-10, and a decrease in Al by a factor 10 compared to natural areas. Similarly, agricultural areas showed an increase in Ca, NO3, and Fe by a factor 2-4, an increase in K by a factor 4-10, and a decrease in Al by a factor 2. Compared to urban areas, agricultural areas contained less Na, Cl, and Al and more NO3. Our results demonstrate that the importance of the processes controlling groundwater composition in the Netherlands is scale-dependent, with geochemical processes being more dominant on a national scale and land use on regional and local scales. © 2007 Elsevier B.V. Dit resulteert, met name bij toetsing van grond op meerdere stoffen, tot het significant toenemen van de kans op het afkeuren van grond die feitelijk niet belast is. Dit onterecht afkeuren van grond leidt zowel tot een negatief imago voor de Nederlandse landbouwgrond als tot afzetproblemen van tarragrond. Het is daarom noodzakelijk om bij de toetsing aan de streefwaarde een compensa-tiemechanisme te definiëren, waardoor rekening wordt gehouden met het feit dat de streefwaarden binnen de verdeling van achtergrondgehalten vallen.

Data bases and environmental research Concern for the environment has grown rapidly in the last decade(s). Many potential threats to our surroundings have been recognized. The precise effects of these threats are often not known, and much research is needed. Considerable effort is put into the acquisition of data in time and space to evaluate the influence of anthropogenical processes in the environment. One of the tools used is monitoring by networks. Large numbers of samples are collected and their analysis by modem analytical methods produces very sizeable data bases. Fortunately, with the appearance of personal computers for the data storage and interpretation no costly investments have to be made. Compared to the considerable effort and capital put into the data acquisition, the investment made for (sophisticated) interpretation of this enormous amount of data is often limited. In many cases the evaluation focuses on answering questions put forward by governments. However, environmental data sets should be studied extensively by scientists, in order to extract more knowledge from the data, which may set new perspectives for environmental policies. The Dutch National Ground Water Quality Monitoring Network Quality monitoring of ground water in the Netherlands basically started in the nineteenth century, with the regular examination of extracted drinking water. A systematic approach on a national scale was implemented in 1979, when the Dutch National Institute of Public Health and Environmental Protection (RIVM) initiated the Dutch Groundwater Quality Monitoring Network (LMG). The national network has been designed to keep track of the shallow ground water quality down to 25 m below surface, with an average monitoring site density of 1 per 100 km2• Site selection criteria were mainly based on soil type, land use and on the hydrogeological situation (van Duijvenbooden, 1987). Samples are collected every year and are analyzed for all macrochemical parameters and a selection of trace elements. The available data set contains over 7500 chemical analyses. A first official evaluation was made in 1989 to support formulation of the Dutch environmental policy (van Duijvenbooden, 1989). Aims of this thesis In 1989 ten years of data from the LMG became available for scientific study, which resulted in this thesis. A combined statistical and geochemical approach was taken, focused on an understanding of the fundamental controls of ground water quality. The aims of this thesis are: (1) to derme an efficacious geochemical framework for the interpretation of shallow ground water compositions of the Netherlands. (2) to determine the short- and long term geochemical processes, that change and/or threat the ground water quality. (3) to discuss the merits and limitations of national monitoring networks and to recommend additional tools for the study of ground water quality threatening processes. In pursuing these aims, this thesis serves to illustrate how a large data set can be surveyed and interpreted in a detailed and sophisticated way.

- Abstract
In Parkinson's disease (PD) misfolded alpha-synuclein (aSyn) accumulates in the substantia nigra, where dopaminergic neurons are progressively lost. The mechanisms underlying aSyn pathology are still unclear, but they are hypothesized to involve the autophagy-lysosome pathway (ALP). LRRK2 mutations are a major cause of familial and sporadic PD, and LRRK2 kinase activity has been shown to be involved in pS129-aSyn inclusion modulation. We observed selective downregulation of the novel PD risk factor RIT2 in vitro and in vivo. Rit2 overexpression in G2019S-LRRK2 cells rescued ALP abnormalities and diminished aSyn inclusions. In vivo, viral mediated overexpression of Rit2 operated neuroprotection against AAV-A53T-aSyn. Furthermore, Rit2 overexpression prevented the A53T-aSyn-dependent increase of LRRK2 kinase activity in vivo. On the other hand, reduction of Rit2 levels leads to defects in the ALP, similar to those induced by the G2019S-LRRK2 mutation. Our data indicate that Rit2 is required for correct lysosome function, inhibits overactive LRRK2 to ameliorate ALP impairment, and counteracts aSyn aggregation and related deficits. 33. Squalene-Based Nano-Assemblies Improve the Pro-Autophagic Activity of Trehalose.

- Abstract
The disaccharide trehalose is a well-established autophagy inducer, but its therapeutic application is severely hampered by its low potency and poor pharmacokinetic profile. Thus, we targeted the rational design and synthesis of trehalose-based small molecules and nano objects to overcome such issues. Among several rationally designed trehalose-centered putative autophagy inducers, we coupled trehalose via suitable spacers with known self-assembly inducer squalene to yield two nanolipid-trehalose conjugates. Squalene is known for its propensity, once linked to a bioactive compound, to assemble in aqueous media in controlled conditions, internalizing its payload and forming nanoassemblies with better pharmacokinetics. We assembled squalene conjugates to produce the corresponding nanoassemblies, characterized by a hydrodynamic diameter of 188 and 184 nm and a high stability in aqueous media as demonstrated by the measured Z-potential. Moreover, the nanoassemblies were characterized for their toxicity and capability to induce autophagy in vitro.

- Abstract
A small set of trehalose-centered putative autophagy inducers was rationally designed and synthesized, with the aim to identify more potent and bioavailable autophagy inducers than free trehalose, and to acquire information about their molecular mechanism of action. Several robust, high yield routes to key trehalose intermediates and small molecule prodrugs (2-5), putative probes (6-10) and inorganic nanovectors (12a - thiol-PEG-triazole-trehalose constructs 11) were successfully executed, and compounds were tested for their autophagy-inducing properties. While small molecules 2-11 showed no pro-autophagic behavior at sub-millimolar concentrations, trehalose-bearing PEG-AuNPs 12a caused measurable autophagy induction at an estimated 40 μM trehalose concentration without any significant toxicity at the same concentration., (Copyright © 2021 Elsevier Ltd. The Parkinson's disease (PD)-associated kinase Leucine-Rich Repeat Kinase 2 (LRRK2) is a crucial modulator of the autophagy-lysosome pathway, but unclarity exists on the precise mechanics of its role and the direction of this modulation. In particular, LRRK2 is involved in the degradation of pathological alpha-synuclein, with pathogenic mutations precipitating neuropathology in cellular and animal models of PD, and a significant proportion of LRRK2 patients presenting Lewy neuropathology. Defects in autophagic processing and lysosomal degradation of alpha-synuclein have been postulated to underlie its accumulation and onset of neuropathology. Thus, it is critical to obtain a comprehensive knowledge on LRRK2-associated pathology. Here, we investigated a G2019S-LRRK2 recombinant cell line exhibiting accumulation of endogenous, phosphorylated alpha-synuclein. We found that G2019S-LRRK2 leads to accumulation of LC3 and abnormalities in lysosome morphology and proteolytic activity in a kinase-dependent fashion, but independent from constitutively active Rab10. The disaccharide trehalose is an autophagy inducer, but its pharmacological application is severely limited by its poor pharmacokinetics properties. Thus, trehalose was coupled via suitable spacers with squalene (in 1:2 and 1:1 stoichiometry) and with betulinic acid (1:2 stoichiometry), in order to yield the corresponding nanolipid-trehalose conjugates 1-Sq-mono , 2-Sq-bis and 3-Be-mono . The conjugates were assembled to produce the corresponding nano-assemblies (NAs) Sq-NA1 , Sq-NA2 and Be-NA3 . The synthetic and assembly protocols are described in detail. The resulting NAs were characterized in terms of loading and structure, and tested in vitro for their capability to induce autophagy. Our results are presented and thoroughly commented upon.

The combination of genetics and genomics in Parkinson´s disease has recently begun to unveil molecular mechanisms possibly underlying disease onset and progression. In particular, catabolic processes such as autophagy have been increasingly gaining relevance as post-mortem evidence and experimental models suggested a participation in neurodegeneration and alpha-synuclein Lewy body pathology. In addition, familial Parkinson´s disease linked to LRRK2 and alpha-synuclein provided stronger correlation between etiology and alterations in autophagy. More detailed cellular pathways are proposed and genetic risk factors that associate with idiopathic Parkinson´s disease provide further clues in dissecting contributions of single players. Nevertheless, the fine-tuning of these processes remains elusive, as the initial stages of the pathways are not yet clarified.In this review, we collect literature evidence pointing to autophagy as the common, downstream target of Parkinsonian dysfunctions and augment current knowledge on the factors that direct the subsequent steps. Cell and molecular biology evidence indicate that p38 signaling underlies neurodegeneration and autoptic observations suggest a participation in neuropathology. Moreover, alpha-synuclein and LRRK2 also appear involved in the p38 pathway with additional roles in the regulation of GTPase signaling. Small GTPases are critical modulators of p38 activation and thus, their functional interaction with aSyn and LRRK2 could explain much of the detailed mechanics of autophagy in Parkinson´s disease.We propose a novel hypothesis for a more comprehensive working model where autophagy is controlled by upstream pathways, such as GTPase-p38, that have been so far underexplored in this context. In addition, etiological factors (LRRK2, alpha-synuclein) and risk loci might also combine in this common mechanism, providing a powerful experimental setting to dissect the cause of both familial and idiopathic disease.

38. Cryopreservation of Primary Mouse Neurons: The Benefit of Neurostore Cryoprotective Medium.

Primary neuronal culture from rodents is a well-established model to investigate cellular neurobiology in vitro . However, for this purpose cell cultures need to be generated expressly, requiring extensive animal handling. Furthermore, often the preparation of fresh culture generates an excess of cells that are ultimately wasted. Therefore the ability to successfully cryopreserve primary neural cells would represent an important resource for neuroscience research and would allow to significantly reduce the sacrifice of animals. We describe here a novel freezing medium that allows long-term cryopreservation of primary mouse neurons prepared from E15.5 embryos. Combining imaging, biochemical and electrophysiological analyses, we found that cryopreserved cultures are viable and mature regarding morphology and functionality. These findings suggest that cryopreserved neurons are a valuable alternative to acutely dissociated neural cultures.

- Abstract
Dusts are one of the main air pollutants emitted during cement manufacturing. A substantial part of these are breathable particles that are less than 10 μm in diameter (PM10), which represent a potential threat for the health of the exposed population. This study aimed at evaluating the short-term effects of PM10 concentrations on the health of children, aged 6-14 years, who attended the schools in Fumane (Italy), in proximity (1.2 km) to a large cement plant. School absenteeism was used as a proxy indicator of child morbidity. Time series of daily school absences and PM10 concentrations were collected for 3 school-years from 2007 to 2010 (541 school-days, 462 children on average). The associations between PM10 concentrations and school absence rates in the same day (lag0) and in the following 4 days (lag1 to lag4) were evaluated using generalised additive models, smoothed for medium/long term trends and adjusted for day of the week, influenza outbreaks, daily temperature and rain precipitations. The average concentration of PM10 in the period was 34 (range: 4-183) μg/m(3). An average 10 μg/m(3) increase of PM10 concentration in the previous days (lag0-4) was associated with a statistically significant 2.5% (95%CI: 1.1-4.0%) increase in the rate of school absences. The highest increase in the absence rates (2.4%; 95%CI: 1.2-3.5%) was found 2 days after exposure (lag2). These findings provide epidemiological evidence of the acute health effects of PM10 in areas with annual concentrations that are lower than the legal European Union limit of 40 μg/m(3), and support the need to establish more restrictive legislative standards., (Copyright © 2013 Elsevier GmbH. - Abstract
Background: Cardiologists may prescribe antithrombotic therapy to patients with atrial fibrillation (AF), but prescription application mostly depends on general practitioners. The present study aims to assess frequency and appropriateness of antithrombotic therapy in general practice, as a function of thromboembolic risk factors, using the CHADS2 score., Methods: The computer records of 39 general practitioners were evaluated in order to identify patients with non-valvular AF; therapy and thromboembolic risk factors were recorded., Results: AF was documented in 951 patients; 96 (10.1%) had contraindications to oral anticoagulants. Among the remaining 850 patients, 292 (34.4%) did not receive antithrombotic therapy according to guidelines. In particular, 102 (12.0%) did not receive any antithrombotic treatment., Conclusions: Among general practitioners of the Veneto region (Italy), the application of guidelines on antithrombotic therapy in non-valvular AF is comparable, or even slightly better than that reported in published cohort studies, but it deserves further improvement. There is a need for targeted educational interventions and a better coordination between cardiologists and general practitioners.
