This is an open access article distributed under the terms of the Creative Commons Attribution License, which permits unrestricted use, distribution, and reproduction in any medium, provided the original author and source are credited.Introduction: The strength of evidence underpinning care and treatment recommendations in traumatic brain injury (TBI) is low. Comparative effectiveness research (CER) has been proposed as a framework to provide evidence for optimal care for TBI patients. The first step in CER is to map the existing variation. The aim of current study is to quantify variation in general structural and process characteristics among centers participating in the Collaborative European NeuroTrauma Effectiveness Research in Traumatic Brain Injury (CENTER-TBI) study. Methods: We designed a set of 11 provider profiling questionnaires with 321 questions about various aspects of TBI care, chosen based on literature and expert opinion. After pilot testing, questionnaires were disseminated to 71 centers from 20 countries participating in the CENTER-TBI study. Reliability of questionnaires was estimated by calculating a concordance rate among 5% duplicate questions. Results: All 71 centers completed the questionnaires. Median concordance rate among duplicate questions was 0.85. The majority of centers were academic hospitals (n = 65, 92%), designated as a level I trauma center (n = 48, 68%) and situated in an urban location (n = 70, 99%). The availability of facilities for neuro-trauma care varied across centers; e.g. 40 (57%) had a dedicated neuro-intensive care unit (ICU), 36 (51%) had an in-hospital rehabilitation unit and the organization of the ICU was closed in 64% (n = 45) of the centers. In addition, we found wide variation in processes of care, such as the ICU admission policy and intracranial pressure monitoring policy among centers. Conclusion: Even among high-volume, specialized neurotrauma centers there is substantial variation in structures and processes of TBI care. This variation provides an opportunity to study effectiveness of specific aspects of TBI care and to identify best practices with CER approaches.

Abstract

The data collected is based on the answers of 60 respondents to an online questionnaire. The respondents were among a controlled population of signatories, who endorsed the Pisa Declaration on Policy Development for Grey Literature Resources published in English on May 16, 2014. Translations in 22 other languages have since appeared published and the Declaration remains online, open for endorsement. The dataset consists of responses to ten questions of which one allowed a single response, six allowed for multiple responses, and three were open-ended. All 10 questions invited additional comments. After two years from its initial publication, the survey sought to understand how important the organizational, educational, legal, financial, and technical points in the Pisa Declaration are to the respondent's organization, what additions and/or revisions deserve consideration, and did the respondent have an opportunity to promote public awareness to the Pisa Declaration. The data was collected over a 12-week period in 2016 via SurveyMonkey, where it remains stored along with a copy housed in the DANS Easy Archive. It's potential for reuse resides in its full open access compliance and lends itself to comparison with other Declarations published in the field of information. The reuse of the data may also be considered of value in leveraging information resources.

Abstract

GreyNet's web-access portal and repository is the GreyGuide - an internet resource that is fully open access compliant. Having benefited from technical developments, the migration of hundreds of metadata full-text records, and the addition of enriched fields and functionality since its launch, the GreyGuide now offers GreyNet1 a testbed from which to map and measure its capacity in open access publishing. The population of this study is drawn from digital resources accessible via the GreyGuide Portal and Repository . The selection is based on the criteria that GreyNet is the content provider, that they are open access compliant, that they are sustained information resources, and that there are available use statistics from which to draw upon. This study focusses on the open access to GreyNet's range of publications, where attention is drawn to the specific document types that meet the sampling criteria.

Persistent identifiers such as a DOI1 for a publication and an ORCiD2 for an author/researcher can be approached from both the demand-side as well as supply-side of information. It appears however that the former attracts more attention. Here emphasis lies in the access to and preservation of research output. Yet, it is on the supply-side regarding the acquisition of research output that persistent identifiers may by the same token have influence in identifying and populating prospective data archives and repositories. This study will look at the influence persistent identifiers have in securing the acquisition of grey literature for public access. The goal of this project is twofold. First, to carry out a survey within the grey literature community as to the opinions, uses, and applications of persistent identifiers. And second, to initiate a project geared to populate a new collection housed in the GreyGuide Repository3 by using the DOI as an incentive. Resources in Grey Literature (RGL) is as a generic, multidisciplinary collection that will serve for this purpose. Using GreyNet's distribution channels and social media, stakeholders in the field of grey literature are invited to enter one or more of their publications in the RGL collection. Each new entry will receive a DOI minted by GreyNet International and further stored and preserved in the DataCite registry . Also, a system generated citation will be added to each new entry in order to facilitate record use. The types of grey literature documents eligible for entry in the RGL collection are numerous . Brief guidelines for record entry require that it be self-archived using the existing online-template and that both the metadata record and accompanying full-text document(s) are in English. An additional descriptive field does allow for entry in another language. And, a translation of the document can also be uploaded in the repository. Finally, it is understood that by submitting the metadata record and file(s), they become open access compliant under Creative Commons license CC-BY-SA. The initial phase of the project commenced in April 2019 and closed in October 2019. Records harvested during this period along with the results of the survey will be analyzed in its second phase. In the final phase, the project's outcome will be published. Results should indicate whether the AccessGrey Project be extended to other collections in the GreyGuide, and if this project would be of value to other communities of practice in the field of grey literature.

Abstract

Introduction: Chronic subdural haematomas (CSDHs) are one of the most common neurosurgical conditions. The goal of surgery is to alleviate symptoms and minimise the risk of symptomatic recurrences. In the past, reoperation rates as high as 20%–30% were described for CSDH recurrences. However, following the introduction of subdural drainage, reoperation rates dropped to approximately 10%. The standard surgical technique includes burr-hole craniostomy, followed by intraoperative irrigation and placement of subdural drainage. Yet, the role of intraoperative irrigation has not been established. If there is no difference in recurrence rates between intraoperative irrigation and no irrigation, CSDH surgery could be carried out faster and more safely by omitting the step of irrigation. The aim of this multicentre randomised controlled trial is to study whether no intraoperative irrigation and subdural drainage results in non-inferior outcome compared with intraoperative irrigation and subdural drainage following burr-hole craniostomy of CSDH. Methods and analysis: This is a prospective, randomised, controlled, parallel group, non-inferiority multicentre trial comparing single burr-hole evacuation of CSDH with intraoperative irrigation and evacuation of CSDH without irrigation. In both groups, a passive subdural drain is used for 48 hours as a standard of treatment. The primary outcome is symptomatic CSDH recurrence requiring reoperation within 6 months. The predefined non-inferiority margin for the primary outcome is 7.5%. To achieve a 2.5% level of significance and 80% power, we will randomise 270 patients per group. Secondary outcomes include modified Rankin Scale, rate of mortality, duration of operation, length of hospital stay, adverse events and change in volume of CSDH. Abstract

Background: The pathophysiological basis of idiopathic normal pressure hydrocephalus (iNPH) is still unclear. Previous studies have shown a familial aggregation and a potential heritability when it comes to iNPH. Our aim was to conduct a novel case-controlled comparison between familial iNPH (fNPH) patients and their elderly relatives, involving multiple different families. Methods: Questionnaires and phone interviews were used for collecting the data and categorising the iNPH patients into the familial (fNPH) and the sporadic groups. Identical questionnaires were sent to the relatives of the potential fNPH patients. Venous blood samples were collected for genetic studies. The disease histories of the probable fNPH patients (n = 60) were compared with their ≥ 60-year-old relatives with no iNPH (n = 49). A modified Charlson Comorbidity Index (CCI) was used to measure the overall disease burden. Fisher’s exact test (two-tailed), the Mann–Whitney U test (two-tailed) and a multivariate binary logistic regression analysis were used to perform the statistical analyses. Results: Diabetes (32% vs. 14%, p = 0.043), arterial hypertension (65.0% vs. 43%, p = 0.033), cardiac insufficiency (16% vs. 2%, p = 0.020) and depressive symptoms (32% vs. 8%, p = 0.004) were overrepresented among the probable fNPH patients compared to their non-iNPH relatives. In the age-adjusted multivariate logistic regression analysis, diabetes remained independently associated with fNPH (OR = 3.8, 95% CI 1.1–12.9, p = 0.030). Conclusions: Diabetes is associated with fNPH and a possible risk factor for fNPH. Diabetes could contribute to the pathogenesis of iNPH/fNPH, which motivates to further prospective and gene-environmental studies to decipher the disease modelling of iNPH/fNPH.

Abstract

Introduction: Although a great majority of patients with cervical radiculopathy syndrome can successfully be treated non-operatively, a considerable proportion experience persistent symptoms, severe enough to require neurosurgical intervention. During the past decade, cervical spine procedures have increasingly been performed on an outpatient basis and retrospective database analyses have shown this to be feasible and safe. However, there are no randomised controlled studies comparing outpatient care with inpatient care, particularly with emphasis on the patients’ perception of symptom relief and their ability to return to normal daily activities and work. Methods and analysis: This is a prospective, randomised, controlled, parallel group non-inferiority trial comparing the traditional hospital surveillance (inpatient, patients staying in the hospital for 1–3 nights after surgery) with outpatient care (discharge on the day of the surgery, usually within 6–8 hours after procedure) in patients who have undergone anterior cervical decompression and fusion procedure. To determine whether early discharge (outpatient care) is non-inferior to inpatient care, we will randomise 104 patients to these two groups and follow them for 6 months using the Neck Disability Index (NDI) as the primary outcome. We expect that early discharge is not significantly worse than the current care in terms of change in NDI. Non-inferiority will be declared if the mean improvement for outpatient care is no worse than the mean improvement for inpatient care, by a margin of 17.3%. We hypothesise that a shorter hospital stay results in more rapid return to normal daily activities, shorter duration of sick leave and decreased secondary costs to healthcare system. Secondary outcomes in our study are arm pain and neck pain using the Numeric Rating Scale, operative success (Odom’s criteria), patient’s satisfaction to treatment, general quality of life (EQ-5D-5L), Work Ability Score, sickness absence days, return to previous leisure activities and complications. Ethics and dissemination: The study was approved by the institutional review board of the Helsinki and Uusimaa Hospital District on 6 June 2019 (1540/2019) and duly registered at ClinicalTrials.gov. We will disseminate the findings of this study through peer-reviewed publications and conference presentations.

Abstract

Introduction: The strength of evidence underpinning care and treatment recommendations in traumatic brain injury (TBI) is low. Comparative effectiveness research (CER) has been proposed as a framework to provide evidence for optimal care for TBI patients. The first step in CER is to map the existing variation. The aim of current study is to quantify variation in general structural and process characteristics among centers participating in the Collaborative European NeuroTrauma Effectiveness Research in Traumatic Brain Injury (CENTER-TBI) study. Methods: We designed a set of 11 provider profiling questionnaires with 321 questions about various aspects of TBI care, chosen based on literature and expert opinion. After pilot testing, questionnaires were disseminated to 71 centers from 20 countries participating in the CENTER-TBI study. Reliability of questionnaires was estimated by calculating a concordance rate among 5% duplicate questions. Results: All 71 centers completed the questionnaires. Median concordance rate among duplicate questions was 0.85. The majority of centers were academic hospitals (n = 65, 92%), designated as a level I trauma center (n = 48, 68%) and situated in an urban location (n = 70, 99%). The availability of facilities for neuro-trauma care varied across centers; e.g. 40 (57%) had a dedicated neuro-intensive care unit (ICU), 36 (51%) had an in-hospital rehabilitation unit and the organization of the ICU was closed in 64% (n = 45) of the centers. In addition, we found wide variation in processes of care, such as the ICU admission policy and intracranial pressure monitoring policy among centers. Conclusion: Even among high-volume, specialized neurotrauma centers there is substantial variation in structures and processes of TBI care. This variation provides an opportunity to study effectiveness of specific aspects of TBI care and to identify best practices with CER approaches.

Abstract

In December 2013, the GreyGuide was formerly launched as an online forum and repository of good practice in grey literature. The project partners then turned to the acquisition of both proposed and published good practices. During this same timeframe, GreyNet- one of the project partners - welcomed far reaching developments in its infrastructure. Three new committees were established in line with its fourfold mission dedicated to research, publication, open access, and education in the field of grey literature. In the process of coordinating and facilitating the work of these new committees, it became clear that a multitude of web-based content, which is currently maintained on GreyNet's website and the GL conference site, should be made accessible via a unique web portal. This would allow for browse, search, and retrieval across collections. The GreyGuide was tested for this purpose and it was then decided to select and migrate content to the GreyGuide. While the web origins of four such collections have been identified a) GreySource Index b) Who's Who in Grey Literature c) International Directory of Organizations in Grey Literature d) GL Conference Proposals the work of defining the metadata for these collections, their subsequent data entry, and additional cross-linking indicate the work that will be undertaken in the coming months. It is anticipated that just as GL15 provided the occasion for the launch of the GreyGuide Repository, GL16 will demonstrate its enhanced function as a web access portal

Abstract

The impact of a biocontrol agent spreading from a point source on crop–weed interactions was modeled. The model encompassed: (i) severity of crop–weed competition as affected by a rust pathogen, (ii) velocity of spread of the rust pathogen, and (iii) density of infected plants introduced in the weed population as starting points (inoculum sources) for an epidemic. The model was parameterized for a study system encompassing the crop Daucus carota (carrot), the weed Senecio vulgaris (common groundsel), and its antagonist Puccinia lagenophorae. The parameters of (i) were estimated in a greenhouse study using a response-surface design. Estimates of the parameter of (ii) were obtained from the literature. The density of infected plants (iii) was varied to simulate crop loss as function of density. Simulations were run for various densities of the weed and various velocities of rust pathogen spread. The results of the simulations indicated a crop-loss ranging from 5 to 10% at levels of relatively weak D. carota–S. vulgaris competition. Density of inoculum sources and velocity of P. lagenophorae spread had only minor effects on crop loss. In contrast, density of inoculum sources and velocity of spread had major effects on crop loss at levels of intermediate (range of 10–35% loss) and severe competition (range of 30–70% loss). The results are discussed both with respect to biological control of S. vulgaris using P. lagenophorae as biocontrol agent and as a general model describing the impact of the spatial dynamics of a pathogen (natural enemy) on plant competition.

Abstract

Perhaps the most cost effective research carried out in information science today is in the field of grey literature. The published proceedings in the GL-Conference Series provide a record of impressive research results in this field of in formation. These results are based on various and diverse approaches and methodologies, whereby citation data, survey data, systems data, bibliographic and metadata, as well as other evidence based variables and indicators are compiled, processed, and analysed. Notwithstanding the fact that knowledge generation is an important measure of wealth in science and technology, the costs in human and material resources appropriated from research budgets must also weigh-in to determine real effectiveness. Only in this way can our initial rhetoric be transformed into a working hypothesis. This research project, which lies within the structure of the GL-Conference Series, seeks to analyse not only the benefits of research on grey literature but also the costs related to carrying-out and publishing research results. In order to gather evidence-based data, a Project In formation Document (PID) Form similar to others that are in place and use elsewhere has been designed to accommodate GL research. The PID-Form will be distributed to those authors/researchers, who respond to the GL8 Call for Papers, as well as to authors/researchers from last year's conference in this series. Results gathered from these research resumes are expected to provide answers to relevant questions such as the percentage of research on the topic of grey literature that is formally funded, the ratio of ad hoc research, the ratio of individual to team research, average research costs and expenses, the duration of research projects, etc. This evidence-based data will then enable us to grasp the cost effectiveness of research on grey literature and compare other types of data compiled within a conference structure. And, in so doing, our results will help to demonstrate the power of grey literature to other information professionals as well as policy and decision makers, funding bodies and new investors.
