No. of bitstreams: 0 Previous issue date: 2019-01-01 Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (CAPES) Purpose: This study evaluated the effect of the use of a powder-type adhesive on masticatory performance and oral health related quality of life (OHRQoL) in patients with normal and resorbed mandibular ridges during adaptation period with complete dentures. Methods: Forty-two edentulous subjects (12 men, 30 women) were distributed in four groups: normal mandibular ridges with adhesive, n = 10; normal mandibular ridges without adhesive, n = 10; resorbed mandibular ridges with adhesive, n = 11; resorbed mandibular ridges without adhesive, n = 11. Masticatory performance was evaluated by sieving method and OHRQoL by Oral Health Impact Profile in Edentulous Adults (OHIP-EDENT) inventory at 30, 60 and 90 days after the insertion of new complete dentures. Data were analyzed using 2-way ANOVA test and Generalized Estimating Equations (GEEs), α = 0.05. Results: The use of adhesive resulted in better masticatory performance at 30 (35.76 ± 12.63%) and 60 days (30.06 ± 10.54%) after the insertion of the dentures and did not influence on OHRQoL in participants with normal ridges during the adaptation period. The use of adhesive did not interfere on masticatory performance during adaptation period and had a negative effect on masticatory discomfort/disability subscale OHRQoL at 30-day period [5.2(3.6, 6.8)] for resorbed mandibular ridges with new complete dentures. Conclusions: The use of adhesive can improve masticatory performance of new complete dentures users with normal mandibular ridges in the initial adaptation period up to 60 days after insertion. For resorbed ridges subjects, the use of adhesive had a negative impact on masticatory discomfort/disability OHRQoL at 30 days after insertion. No. of bitstreams: 0 Previous issue date: 2021-01-01 Purpose:The purpose of this study was to evaluate in vitro the accuracy of different splinting techniques using transfers combined with different tray types. Materials and Methods: The research group fabricated a maxillary master cast with four implants and a passive metallic bar on this master cast. For the impression techniques, 48 casts were used with six different impression techniques: (1) metal tray with resin splinted transfers, (2) metal tray with metal and resin splinted transfers, (3) plastic tray with resin splinted transfers, (4) plastic tray with metal and resin splinted transfers, (5) multifunctional guide with resin splinted transfers, and (6) multifunctional guide with metal and resin splinted transfers (n = 8) using polyvinyl siloxane impression material. This study used a passive metallic bar to measure the malalignment between the framework and the analogs (A, B, C, and D) in 2D and 3D. The master and experimental casts were scanned with a contact scanner to compare the accuracy in 3D impression techniques. Discrepancies between the analogs were measured in three x-, y-, and z-axes. Results: There was no statistically significant difference [P > .05) between the groups in vertical malalignments (2D). In the 3D evaluation, for the z-axis and combination of xz-axis, plastic tray with metal, and resin splinted transfers (z = 487 µm; xz = 888 µm), there was a statistically significant difference compared with the multifunctional guide and resin splinted transfers (z = 772 µm; xz = 1,380 µm). When analyzing by analog, in C, the multifunctional guide with metal and resin splinted transfers (302 µm) presented a statistically significant difference compared with the multifunctional guide and resin splinted transfers (492 µm). Conclusion: The evaluation methods for the accuracy impression technique presented different results between them. There was no difference in vertical malalignments (2D), but in 3D, the bonding with metal and acrylic resin presented better results than the bond with only acrylic resin when using the plastic tray and multifunctional guide, respectively, in the z-axis and the combination between the xz-axes. The bonding technique of the transfers with metal and acrylic resin presents better results in the 3D analysis for the multifunctional guide impressions. No. of bitstreams: 0 Previous issue date: 2021-01-01 Rehabilitation of atrophic maxilla with dental implants is still a challenge in clinical practice especially in cases of alveolar bone resorption due to peri-implantitis and pneumatization of the maxillary sinuses. Several surgical approaches have been employed to reconstruct the lost tissues allowing the proper tridimensional position of the implants. In this context, the aim of this case report is to describe a surgical and prosthetic approach to fully rehabilitate the atrophic maxilla with dental implants. The patient presented with unsatisfactory functional and esthetical implant-supported prosthesis with some of the implants already lost by peri-implantitis. The remaining three implants were also affected by peri-implantitis. Reversal prosthetic planning was performed, and a provisional prosthesis was fabricated and anchored in two short implants. Sinus floor augmentation procedure and onlay bone graft were then accomplished. After a healing period of 8 months, digital-guided surgery approach was performed to place the implants. Finally, a definitive prosthesis was installed. One-year follow-up has revealed stabilization of the bone tissue level, successful osseointegration, and a pleasant esthetic and functional result. A proper diagnosis and careful planning play an important role to enhance precision and to achieve patient esthetic and functional outcomes. No. of bitstreams: 0 Previous issue date: 2020-01-01 Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (CAPES) Statement of problem: Changes in physicochemical properties because of implant material aging and natural deterioration in the oral environment can facilitate microbial colonization and disturb the soft-tissue seal between the implant surfaces. Purpose: The purpose of this in vitro study was to investigate the effect of aging time on the physicochemical profile of titanium (Ti) and zirconia (ZrO2) implant materials. Further microbiology and cell analyses were used to provide insights into the physicochemical implications of biological behavior. Material and methods: Disk-shaped specimens of Ti and ZrO2 were submitted to roughness, morphology, and surface free energy (SFE) analyses before nonaging (NA) and after the aging process (A). To simulate natural aging, disks were subjected to low-temperature degradation (LTD) by using an autoclave at 134 ºC and 0.2 MPa pressure for 20 hours. The biological activities of the Ti and ZrO2 surfaces were determined by analyzing Candida albicans (C. albicans) biofilms and human gingival fibroblast (HGF) cell proliferation. For the microbiology assays, a variance analysis method (ANOVA) was used with the Tukey post hoc test. For the evaluation of cellular proliferation, the Kruskal-Wallis test followed by Dunn multiple comparisons were used. Results: Ti nonaging (TNA) and ZrO2 nonaging (ZNA) disks displayed hydrophilic and lipophilic properties, and this effect was sustained after the aging process. Low-temperature degradation resulted in a modest change in intermolecular interaction, with 1.06-fold for TA and 1.10-fold for ZA. No difference in biofilm formation was observed between NA and A disks of the same material. After 48 hours, the viability of the attached HGF cells was very similar to that in the NA and A groups, regardless of the tested material. Conclusion: The changes in the physicochemical properties of Ti and ZrO2 induced by the aging process do not interfere with C. albicans biofilm formation and HGF cell attachment, even after long-term exposure. How the physical-chemical properties intrinsic to each material over time can affect adhesion, given constant cell turnover and biofilm development, remains unclear. Purpose: The purpose of this in vitro study was to evaluate the influence of aging on the viability, adhesion, and proliferation of normal oral keratinocytes (Nok-si) and on the multispecies biofilm formation of Fusobacterium nucleatum (F. nucleatum), Porphyromonas gingivalis (P. gingivalis), and Streptococcus sanguinis (S. sanguinis). Material and methods: Zirconia (ZrO2) and titanium (Ti) disks were analyzed by surface roughness, water contact angle, and X-ray diffraction before and after aging in an autoclave. The Nok-si cell viability was evaluated by using a 3-(4.5-dimethylthiazole-2-yl)2.5-diphenyl tetrazolium bromide assay (MTT), morphology by scanning electron microscopy (SEM), and proliferation and adhesion by using a confocal microscope. Multispecies biofilms were analyzed quantitatively by colony-forming units per milliliter (CFU/mL) and qualitatively by SEM. Results: For Ti, the aging process affected the roughness and wettability. O tratamento com implantes dentários tem evoluído significativamente desde a década de 70 até os dias atuais, tanto na questão de planejamento quanto na bioengenharia e tratamento da superfície desses implantes, com o objetivo de tornar o tratamento mais eficaz. Atualmente o uso de implantes imediatos tem sido cada vez mais estudado pelos pesquisadores e utilizado para a reabilitação oral dos pacientes. A literatura nos mostra que a inserção imediata de implantes dentários pós-extração é um tipo de tratamento com altas taxas de sucesso, possibilitando ao paciente ser submetido a apenas uma cirurgia, retorno à função mastigatória e estética mais rápidas. Para isso são necessários planejamento correto e seleção adequada do paciente para o sucesso do tratamento, levando em consideração a condição óssea e periodontal. O objetivo deste estudo é discorrer sobre o planejamento da reabilitação oral de uma paciente que foi submetida da extração dentária total dos dentes inferiores, instalação de 4 implantes dentários imediatos e confecção de uma prótese total com carga imediata do tipo protocolo.

The physical chemical properties intrinsic to each material can affect the first step to biofilm development and is an important precursor to the adaptive behavior of pathogenic bacteria species. Purpose The purpose of this in vitro study was to evaluate the effect of 2 commercially available implant abutment materials on the adhesion phase and biofilm formation. Material and methods Disks (8 mm in diameter, 2 mm thick) of machined pure titanium (Ti) and yttrium-stabilized zirconia (ZrO2) materials were used to mimic implant abutments. The physical chemical surface properties were investigated using different approaches. Initial adherent bacteria and biofilm formation were evaluated after 16 and 48 hours by incubating the disks in a rich medium containing representative saliva-derived oral microbial community. Unpaired t test, 2 tailed, was used to compare the groups. Results Ti presented lower hydrophobicity and surface free energy values than the ZrO2, and 6.1-fold fewer bacteria adhered to the Ti. After 48 hours, detailed quantitative analysis showed that biofilm biomass and biofilm density were lower on the Ti disks than on ZrO2. The quantity of phylotypes on the Ti and ZrO2 surfaces was relatively similar during the attachment and early biofilm formation periods. Conclusions Although no difference in the bacteria profile was observed between both materials independent of the time point, the highest level of colonization was on ZrO2. For the long-term success of implants, it is necessary to achieve a direct contact between the implant and the subjacent bone. To avoid bacterial penetration that could adversely affect the initial wound healing as well as the long-term behavior of the implants, an early tissue barrier must form that is able to protect the biological peri-implant structures. Given the need of an effective tissue early barrier around dental implants, the present study evaluated, in vitro, the influence of physical and chemical characteristics of two implant abutment surfaces on gingival epithelial cells (OBA-9) adhesion. To this end, titanium (Ti) and zirconia (ZrO2 ) disk-shaped specimens were used mimicking the abutment components surfaces, while bovine enamel (BE) and glass cover slips (GCS) disks served as positive and negative controls, respectively. Roughness and surface free energy (SFE) of all materials were evaluated previously to cellular adhesion step. In sequence, the effect of each material on cells morphology and viability was analyzed after 1 and 24 hr. The results showed that roughness and SFE had no effect on the cell viability data or on their interaction (p = .559), independent of a post-contact analysis of 1 or 24 hr. However, cells attachment and spreading increased after 24 hr on Ti and ZrO2 than BE, corresponding to the highest SFE values. SFE appears to be an important property interfering on the quality of the soft tissue surrounding dental implants. These data can be considered a trigger point for developing new material surfaces.

Orthodontic extrusion with multidisciplinary treatment can provide predictable outcomes in selected situations, reducing the costs and the adaptation times of gingival tissues after implant integration. Forced orthodontic extrusion is strongly related to interactions of teeth with their supportive periodontal tissues. This article reports a case of orthodontic extrusion of the maxillary incisors for later implant rehabilitation in a patient with periodontal disease. Slow forces were applied for 14 months. After this time, the teeth were extracted, and the implants were placed on the same day. Also in the same session, the provisional crown was fabricated for restoration of the anterior maxillary interdental papillae loss and for gingival contouring. Clinical and radiographic examinations at the 6-year follow-up showed successful tooth replacement and an improved esthetic appearance achieved by this multidisciplinary treatment. The decision to perform orthodontic extrusion for implant placement in adult patients should be multidisciplinary.

It is composed of a base paste and a catalyzer containing lead dioxide. The high toxicity of lead to humans is ground for much concern, since it can attack various systems and organs. The present study involved a quantitative analysis of the concentration of lead in the material Permlastic®. The lead was determined by plasma-induced optical emission spectrometry (Varian model Vista). The percentages of lead found in the two analyzed lots were 38.1 and 40.8%. The lead concentrations in the material under study were high, but the product’s packaging contained no information about these concentrations. At 7 years old, the patient fractured the maxillary left central incisor and the dentist did a crown with the fragment. Twenty years later, the patient was referred to a dental clinic for orthodontic treatment, with the chief complaint related to an accentuated deep bite, and a professional started an orthodontic treatment. After sixteen months of orthodontic treatment, tooth 21 fractured. The treatment plan included an orthodontic extrusion of tooth 21 and implant placement. This case has been followed up and the clinical and radiographic examinations show excellence esthetic results and satisfaction of patient. The forced extrusion can be a viable treatment option in the management of crown root fracture of an anterior tooth to gain bone in a vertical direction. This case emphasizes that to achieve the esthetic result a multidisciplinary approach is necessary. Denture repairs should have adequate strength, dimensional stability and color match, and should be easily and quickly performed as well as relatively inexpensive. Objective: The aim of this study was to evaluate the flexural strength of acrylic resin repairs processed by different methods: warm water-bath, microwave energy, and chemical polymerization. Material and methods: Sixty rectangular specimens (31×10×2.5 mm) were made with warm water-bath acrylic resin (Lucitone 550) and grouped (15 specimens per group) according to the resin type used to make repair procedure: 1) specimens of warm water-bath resin (Lucitone 550) without repair (control group); 2) specimens of warm water-bath resin repaired with warm water-bath; 3) specimens of warm water-bath resin repaired with microwave resin (Acron MC); 4) specimens of warm waterbath resin repaired with autopolymerized acrylic resin (Simplex). O objetivo desse estudo foi avaliar a aceitação de modelos de estudos digitais, impressos e de gesso por cirurgiões-dentistas de diferentes especialidades e os fatores relevantes na prática clínica odontológica oferecidos por eles. Um total de 172 dentistas solicitaram modelos de estudos odontológicos (ME) em três diferentes formatos: digital (D), impressos (I) e gesso (G). Depois do estudo do caso utilizando os três tipos oferecidos, os dentistas responderam um questionário eletrônico sobre sua experiência e satisfação. O questionário incluía: especialidade, preferência entre os três modelos, informação adicional oferecida pelo modelo, reprodução da arcada, relevância na prática clínica, do modelo físico (impresso e gesso), do modelo digital, custo e tempo de recebimento do modelo. As respostas dos dentistas foram resumidas considerando frequências absolutas e relativas. O teste do qui-quadrado foi aplicado na relação entre a aceitação do modelo e a especialidade, modelo físico, digital, custo e tempo. Não houve influência significativa da especialidade na preferência. O tipo de modelo e o custo mostraram influência significativa na aceitação. O fator tempo foi importante para todos os dentistas (acima de 80%), sem diferença significativa entre eles. A aceitação pelos diferentes formatos de modelos de estudo não foi influenciada pelaespecialidade. O formato do modelo, físico ou digital, e custo tiveram diferença na aceitação entre os dentistas. O fator tempo de recebimento do modelo foi relevante para todos os grupos. The objective of study was to evaluate the acceptance of digital, printed and traditional plaster dental models among dentists considering different specialties and factors related to clinical practice. A total of 172 dentists requested dental models(DM) for study purpose, they received in three formats:Digital (D), fused deposition modelling (FDM) and traditional plaster (T). After studying cases using the three DM types, they answered an electronic questionnaire about their experience and satisfaction. The questionnaire included: Specialty, DM preference, additional information provided by DM, arcade reproduction, relevance of clinical practice, physical file (printed and traditional plaster), digital file, cost and time, respectively. Dentists\' responses weresummarized considering absolute and relative frequencies. The chi-square test was applied to verify whether there is a relation between the DM\'s Acceptance and the specialty, physical file,digital file, cost relevance, and time. There is no significant influence of specialty to acceptance. The type of the model, and cost relevance showed significant influence on acceptance. Time relevance was an important factor for all dentists, without significant difference. Acceptance was not influenced by the specialty. Physical and digital format, and cost had a difference of acceptance among dentists. The time for receipt was relevant for all groups.

Com alta previsibilidade de sucesso e sobrevivência, o tratamento com implantes dentários é uma prática relevante da clínica odontológica. O material utilizado para confecção do conector protético parece ter papel importante no resultado clínico dos implantes em longo prazo. A infecção que resulta em um processo inflamatório nos tecidos peri-implantares continua a ser a principal limitação para o sucesso em longo prazo das restaurações suportadas por implantes. Mudanças nas comunidades microbianas são comuns ao longo do tempo, mas podem perturbar a homeostase do microbioma hospedeiro e resultar em inflamação dos tecidos peri-implantares se um biofilme disbiótico for estabelecido. Considerando que diferentes superfícies de substrato oral podem ter um impacto relevante na adesão e colonização microbiana, o objetivo deste estudo foi investigar longitudinalmente as comunidades microbianas do biofilme formado sobre próteses unitárias de implantes com conectores de titânio ou zircônia e sua correlação com as condições clínicas e radiológicas peri-implantares durante 3 anos de função. Vinte indivíduos saudáveis foram selecionados e reabilitados com implantes dentários e restaurações protéticas unitárias cimentadas sobre conectores de zircônia (n=10; região anterior da maxila) ou conectores de titânio (n=10; região posterior da maxila/mandíbula). Amostras de biofilmes supra e subgengival/submucoso e parâmetros clínicos (profundidade de sondagem, recessão gengival, sangramento à sondagem e reabsorção óssea marginal) foram coletados dos sítios associados aos implantes e dentes contralaterais nos períodos T0 = baseline (instalação da prótese), T1 = 12 meses, T2 = 24 meses e T3 = 36 meses de função mastigatória. O sequenciamento do gene rRNA 16S foi usado para caracterizar os biofilmes orais. Análise de bioinformática e estatística multivariada foram usadas para avaliar a diversidade microbiana e os resultados clínicos. Os microbiomas de ambos os materiais apresentaram altos níveis de diversidade alfa durante todo o período experimental, independentemente do tempo de amostragem. As comunidades microbianas de titânio e zircônia foram bastante diferentes ao longo do tempo, diferindo em cerca de 30% após 3 anos de carga funcional. A similaridade do microbioma entre os conectores testados e os dentes contralaterais também foi baixa, variando entre 45% e 50% após 3 anos de investigação. Patógenos periodontais comumente associados à peri-implantite foram encontrados em ambos os grupos. A profundidade de sondagem aumentou nos implantes associados aos conectores de titânio após 12 meses, enquanto nos implantes com zircônia reduziu no mesmo período. A recessão gengival e o sangramento à sondagem aumentaram ao longo do tempo para ambos materiais. Não houve diferença significante nos valores da reabsorção óssea marginal, entre os dois substratos, após 36 meses. Não foi observada nenhuma complicação técnica ou biológica para ambos os conectores, com taxa de sucesso de 100%. Os resultados sugerem a manutenção da saúde peri-implantar ao longo do tempo, em ambos os grupos. Além disso, os conectores de titânio e zircônia mostraram fortes correlações de índices de diversidade e taxa microbiana com os resultados clínicos. De modo geral, pode-se concluir que o tipo de substrato usado como conector protético influenciou de forma significante a diversidade e contagem de comunidades bacterianas durante 3 anos de função mastigatória. O tempo de amostragem não teve efeito sobre as variáveis. Fortes correlações foram encontradas entre os achados microbianos e os resultados clínicos. With high predictability of success and survival, treatment with dental implants is a relevant practice in the dental clinic. The materials employed for manufacturing implant components may influence the microbial adhesion and biofilm formation on dental implant assemblies. Infection resulting in an inflammatory process of the peri-implant tissues remains the major limitation to the long-term success of implantsupported restorations in dentistry. Shifts in microbial communities are common over time, but they may disturb the hostmicrobiome homeostasis and result in inflammation of the periimplant issues if a dysbiotic biofilm is established. Considering that different oral substrate surfaces may have a relevant impact on the microbial adhesion and colonization, the aim of this study was to investigate the microbial communities of the biofilm formed on single-implant restorations using titanium or zirconia abutments and how they correlate with clinical parameters after 3-years of implant loading. Twenty healthy subjects were selected and rehabilitated with single-unit implant restorations cemented on zirconia (n = 10; anterior maxilla) or titanium abutments (n = 10; posterior mandible). Supra and subgingival biofilm samples, and clinical parameters data (probing depth, gingival recession, bleeding on probing, and marginal bone level) were collected from the peri-implant sites and contralateral teeth in periods different time-periods, at the prosthesis installation (T0) and after 12 (T1), 24 (T2) and 36 (T3) months of loading. MiSeq sequencing of 16S rRNA amplicons was used to characterize the oral biofilms of individuals. Bioinformatics analysis and multivariate statistical analysis were used to evaluate the microbial diversity and clinical outcomes. Microbiomes of both abutment materials presented high alphadiversity índices during all the experimental period, irrespective of the time of sampling. Microbial communities of titanium and zirconia were quite different over time, differing about 30% after 3 years of functional loading. Similarity of microbiomes between tested abutments and contralateral teeth was also low, ranging between 45% and 50% after 3 years of investigation. Periodontal pathogens commonly associated with periimplantitis were found in both groups. The probing depth increased in implants associated with titanium connectors after 12 months, while in implants with zirconia it decreased in the same period. Gingival recession and probing bleeding increased over time for both materials. There was no significant difference in marginal bone resorption values between the two substrates after 36 months. No technical or biological complications were observed for both connectors, with a success rate of 100%. The results suggest the maintenance of peri-implant health over time in both groups. Furthermore, both abutment materials presented strong correlations of diversity indices and microbial taxa with clinical outcomes. Overall, it can be concluded that the type of abutment substrate significantly influenced diversity and clustering of communities during 3 years of functional loading. The time of sampling had no effect on the variables. Large correlations were found between microbial findings and clinical outcomes.

O objetivo deste trabalho foi, por meio de metodologia in vitro, formular 07 dentifrícios específicos para higienização de próteses totais à base de óleos essenciais de Bowdichia virgilioides Kunth (BvK - Sucupira), Copaifera officinalis (Co - Copaíba), Curcuma longa (Cl - Cúrcuma), Eucalyptus citriodora (Ec - Eucalipto), Ricinus communis (Rc - Mamona), Melaleuca alternifolia (Ma - Melaleuca), Pinus strobus (Ps - Pinheiro Branco), e avaliá-los quanto às características organolépticas, propriedades físico-químicas e ação antimicrobiana frente a Streptococcus mutans - S. mutans, Staphylococcus aureus - S. aureus e Candida albicans - C. albicans. As características organolépticas (aspecto, cor, odor, sabor) foram avaliadas nos tempos 0, 15, 30, 60 e 90 dias. As propriedades físico-químicas compreenderam densidade (D=M/V), pH (peagâmetro), consistência (escoamento sob carga e tempo determinado), propriedades reológicas (reômetro) e a abrasividade (perda de massa - método gravimétrico). Para a ação antimicrobiana, as formulações foram avaliadas pelos métodos do Poço de Difusão em Ágar (PDA) e contagem de Unidades Formadoras de Colônias (UFC). Para PDA, após semeadura dos inóculos microbianos (106 UFC/mL), 03 poços sobre os meios de cultura foram preenchidos com 20µL dos dentifrícios para posterior incubação e mensuração dos halos de inibição. Para UFC, corpos de prova de resina acrílica foram esterilizados, inoculados (levedura: 106 UFC/mL e bactérias: 107 UFC/mL), incubados, submetidos à escovação mecânica (3 minutos) e processados microbiologicamente para contagem do número de UFC/mL. Para a abrasividade e a ação antimicrobiana as formulações foram comparadas com o dentifrício convencional Trihydral, sendo os dados submetidos ao teste de Kruskal-Walis e pós teste de Dunn (α=0,05). Os resultados mostraram características organolépticas normais, exceto para Cl (odor e sabor levemente modificados). Quanto às propriedades físico-químicas, o pH mostrou-se neutro e os valores de densidade (1,067g/mL a 1,136 g/mL), consistência (81 a 94mm) e características reológicas (baixa viscosidade; área de histerese 0,15cm2 a 1,74cm2 - grau tixotrópico moderado) adequados; todos os dentifrícios experimentais apresentaram perda de massa, com maiores valores para Co (p

O sucesso da reabilitação com implantes está baseado principalmente na adequada distribuição das tensões mecânicas ao osso circundante, a qual pode ser influenciada pelo tipo de conexão, dimensões do pilar protético, dimensões do implante, geometria e profundidade de inserção. O objetivo deste estudo foi avaliar a magnitude e distribuição de tensões ao osso peri-implantar através da utilização de implantes de conexão cônica posicionados em diferentes níveis da crista óssea e conectados a pilares protéticos sólidos de diferentes diâmetros, por meio do método tridimensional de elementos finitos. Para isso, foram confeccionados quatro modelos tridimensionais: A-1.5 (Implante equicristal conectado a um pilar de 3,3 mm de diâmetro); A-3.0 (Implante subcristal conectado a um pilar de 3,3 mm de diâmetro); B-1.5 (Implante equicristal conectado a um pilar de 4,5 mm de diâmetro) e B-3.0 (Implante subcristal conectado a um pilar de 4,5 mm de diâmetro). Todos os modelos foram submetidos a uma carga axial (0°) e oblíqua (30°) de 365 N distribuída em 8 pontos oclusais. Foram analisadas as tensões equivalentes de von Mises (TEvM) para implante e componentes protéticos e as tensões máximas (TMaP) e mínimas principais (TMiP) para o osso peri-implantar. Os resultados mostram que as áreas de maior TEvM ocorreram na interface implante/pilar para todos os modelos, sobretudo sob carga oblíqua. Para o osso peri-implantar os maiores níveis de tensão de tração (TMaP) foram observados no posicionamento subcristal do implante em região de interface entre osso cortical e trabecular, no entanto os maiores níveis de tensão de compressão (TMiP) foram observados para o posicionamento equicristal do implante, em região de crista óssea. Independente da profundidade óssea do implante, foi observado uma diminuição dos valores máximos e da área de concentração de tensões quando foi utilizado um pilar com um maior diâmetro em base de assentamento protético, tanto na interface implante/pilar como para interface implante/osso. Dentro das limitações deste estudo foi possível concluir que a colocação subcristal do implante gerou menores níveis de tensão na crista óssea quando comparado com o posicionamento equicristal do implante e que a utilização de um pilar de maior diâmetro em base de assentamento protético causa uma diminuição de tensões em osso peri-implantar independentemente do tipo de carga e posicionamento do implante. The success of implant rehabilitation is mainly based on the adequate distribution of mechanical stresses to the surrounding bone, which can be influenced by the type of connection, prosthetic abutment dimensions, implant dimensions, geometry and insertion depth. The aim of this study was to evaluate the magnitude and distribution of stresses to the peri-implant bone by using conical connection implants installed at different bone depths and connected to solid prosthetic abutments of different diameters using the three-dimensional finite element method. For this, four threedimensional models were built: A-1.5 (Implant placed at crestal connected to a 3,3 mm diameter abutment); A-3.0 (Implant placed at subcrestal connected to a 3,3 mm diameter abutment); B-1.5 (Implant placed at crestal connected to a 4,5 mm diameter abutment) and B-3.0 (Implant placed at subcrestal connected to a 4,5 mm diameter abutment). All models were submitted to an axial (0°) and oblique (30°) load of 365 N distributed in eight occlusal points. The von Mises equivalent stress (TEvM) for implant and prosthetic components were analyzed, and the maximum (TMaP) and minimum principal stresses (TMiP) for periimplant bone. The results show that the highest TEvM areas occurred at the implant-abutment interface for all models, especially under oblique loading. For the periimplant bone the highest tensile stress levels (TMaP) were observed in the subcrestal implant placement at the interface between cortical and trabecular bone, however the highest compression stress levels (TMiP) were observed for the position of the implant in the bone crest region. Regardless of the bone depth of the implant, a reduction in the maximum values and stress concentration area was observed when a larger diameter abutment was used in the prosthetic base, both at the implant / abutment interface and at the implant / bone interface. Within the limitations of this study it was possible to conclude that the subcrestal placement of the implant generated lower stress levels in the bone crest when compared to the crestal placement of the implant and that the use of a larger diameter abutment in prosthetic base causes a decrease in stress in periimplant bone regardless of the type of load and implant placement.

O grau de adaptação na interface implante/componente protético demonstra a precisão na fabricação de um determinado sistema de implante. Quando uma pobre adaptação está presente, esta permite a infiltração de micro-organismos através da interface e a colonização dos espaços internos dos implantes, levando ao surgimento de reações inflamatórias dos tecidos peri-implantares e um possível insucesso clínico do tratamento. O objetivo deste trabalho foi avaliar, in vitro, a microinfiltração através da interface implante/componente protético, do meio externo para o interior dos implantes, pelo método de hibridização DNA-DNA Checkerboard. Foram utilizados 36 conjuntos de implantes Cone Morse/Mini-pilares, divididos em 3 grupos: Grupo A, onde foram avaliados sem a utilização do gel, que foi nosso grupo controle, Grupo B, onde foram avaliados após a utilização do gel inerte, e Grupo C, onde foram avaliados após a utilização do gel contendo metronidazol. Todos esses conjuntos implantes/componentes foram imersos em tubos de ensaio contendo saliva humana, e os resultados foram analisados através de análise estatística. A análise estatística foi realizada pelos testes Wald-Type e ANOVA Type Statistics. Quando comparamos a contagem de micro-organismos entre os 3 grupos avaliados verificamos que os valores se apresentam em ordem crescente. Diferenças estatisticamente significantes foram observadas entre os grupos Controle e Gel Inerte (p0,05). Concluímos que o gel antimicrobiano contendo metronidazol não foi eficaz na prevenção da ocorrência de infiltração de micro-organismos através da interface implante/componente protético, no sistema Cone Morse. The degree of adaptation at the interface implant/abutment demonstrates the precision in the manufacture of a certain implant system. When a poor adaptation is present, it allows the infiltration of microorganisms through the interface and the colonization of the internal spaces of the implants, leading to the appearance of inflammatory reactions of the peri-implant tissues and a possible clinical failure of the treatment. The objective of this study was to evaluate in vitro the microleakage through the implant/ abutment interface, from the external environment to the interior of the implants, by the DNA-DNA Checkerboard hybridization method. Thirty-six sets of Cone Morse/Mini-pillar implants were used, divided into three groups: Group A, where they were evaluated without the use of the gel, which was our control group, Group B, where they were evaluated after inert gel, and Group C, where they were evaluated after the use of the metronidazole gel. All 3 implants/components sets were immersed in test tubes containing human saliva, and the results were analyzed by statistical analysis (Wald-Type and ANOVA Type Statistics). When we compare the count of microorganisms among the 3 evaluated groups we verified that the values are presented in ascending order. Statistically significant differences were observed between the Control and Inert Gel groups (p0.05). We concluded that the antimicrobial gel containing metronidazole was not effective in preventing the occurrence of infiltration of microorganisms through the implant/prosthetic interface, in the Cone Morse system.

Devido à crescente utilização de componentes cerâmicos como estruturas de suporte para restaurações sobre implantes e a ausência de resultados conclusivos, em longo prazo, sobre a influência deste tipo de material no processo de formação e manutenção do biofilme oral e saúde dos tecidos peri-implantares, este estudo teve por objetivo investigar o perfil microbiano do biofilme formado sobre próteses unitárias de implantes com conectores de zircônia (Zr) ou titânio (Ti) e sua implicação clínica nos tecidos de suporte ao longo de 3 anos de função. Vinte indivíduos saudáveis participaram do estudo e foram reabilitados com implantes dentários e coroas protéticas unitárias cimentadas sobre conectores de zircônia (n=10; região anterior de maxila) ou conectores de titânio (n=10; região posterior da mandíbula). Amostras de biofilmes supra e subgengival, e dados de indicadores clínicos (profundidade e sangramento à sondagem, recessão gengival e nível ósseo marginal) foram coletados dos sítios peri-implantares e dentes contralaterais em 4 períodos distintos, na instalação da prótese (T0) e após 12 (T1), 24 (T2) e 36 (T3) meses de função mastigatória. Trinta e sete espécies microbianas foram identificadas e quantificadas pelo método de hibridização DNA-DNA Checkerboard. Os dados foram submetidos à análise estatística não-paramétrica de regressão linear mista (Brunner and Langer nonparametric analysis of longitudinal data in factorial experiments) e múltiplas comparações foram realizadas por meio do teste de Friedman-Conover, corrigido por Benjamini-Hockberg False Discovery Rate (FDR). Para análise da reabsorção óssea marginal foi utilizado o teste Two-Way ANOVA. Todas as análises foram realizadas com um nível de significância de 5%. O perfil microbiano detectado nos implantes restaurados com conectores de zircônia, titânio e seus respectivos dentes contralaterais foi semelhante a partir do período de 12 meses de investigação. Espécies patogênicas e não-patogênicas foram detectadas nos biofilmes supra e subgengival dos dois substratos, com um aumento significante da quantificação das espécies ao longo do tempo. Apesar do perfil semelhante, os substratos investigados influenciaram na quantidade total de micro-organismos identificados (p0,05). De modo geral, pode-se concluir que o perfil microbiano dos biofilmes supra e subgengival dos implantes com Zr ou Ti foi semelhante, com um aumento da quantificação das espécies ao longo do tempo. A microbiota investigada neste estudo parece não ter influenciado os indicadores clínicos ao longo dos 36 meses de acompanhamento. The data were submitted to Brunner and Langer nonparametric analysis of longitudinal data in factorial experiments followed by Friedman-Conover multiple comparisons test, corrected by Benjamin-Hockberg False Discovery Rate (FDR). Two-Way repeated ANOVA was used to analyze marginal bone resorption. Statistical significance was set at 0.05 probability level. The microbial profiles detected in the implants restored with zirconia or titanium abutments and their respective contralateral teeth were similar from the 12-month follow-up. Pathogenic and non-pathogenic species were detected in the supra and subgingival biofilms of the two substrates, with a significant increase in the microbial counts over time. Despite the similar profile, the investigated substrates influenced the total amount of microorganisms identified (p 0.05). Overall, it can be concluded that the microbial profile of supra and subgingival biofilms of the implants with Zr or Ti was quite similar, and the microbial counts increased over time. The microbiota investigated in this study does not seem to have influenced the clinical parameters during the 36 months of follow-up. The clinical results suggest the maintenance of peri-implant tissue health in the different substrates investigated

O objetivo deste estudo foi avaliar clinicamente em humanos sistemas semissólidos (géis) e filmes contendo sal de metronidazol e conjugado de benzoato de metronidazol associados à raspagem e alisamento radicular (RAR), para o tratamento de periodontite. Foram avaliados o pH do fluido crevicular, os níveis clínicos de inserção óssea de suporte aos dentes envolvidos no tratamento periodontal, antes e após os tratamentos, além do perfil de liberação dos fármacos por meio de cromatografia líquida de alta eficiência (CLAE). Participaram do estudo 45 pacientes da Faculdade de Odontologia de Ribeirão Preto USP, de ambos os gêneros, os quais possuíam ao menos 2 dentes com periodontite crônica, compondo um total para o estudo de 96 dentes, divididos aleatoriamente em 3 grupos: grupo 1 raspagem e alisamento radicular (RAR) controle ativo; grupo 2 RAR + filme com metronidazol; grupo 3 RAR + gel de metronidazol. As concentrações de metronidazol e o tempo de liberação do fármaco no fluido crevicular gengival foram monitorados, avaliando-se ainda os efeitos dessas formulações com relação a parâmetros clínicos da periodontite. Os efeitos dos tratamentos foram avaliados pelo acompanhamento longitudinal de parâmetros clínicos (profundidade de sondagem, nível clínico de inserção e sangramento à sondagem). Os resultados da análise das concentrações de metronidazol e benzoato de metronidazol na bolsa periodontal dos pacientes foram considerados em avaliação paralela intragrupos e demonstraram que o filme e o gel foram detectados em concentração efetiva até 48 horas após a aplicação in situ, porém não foi observada diferença significante entre os grupos avaliados. Quanto aos valores de pH após o tratamento, eles variaram entre 6 e 8. No nível clínico de inserção houve diferença estatística significante entre os três grupos avaliados, sendo que o grupo que recebeu apenas tratamento periodontal convencional apresentou os melhores valores. Houve redução progressiva do sangramento à sondagem nos tempos T15 e T30 nos três grupos considerados. No que se refere aos parâmetros clínicos, ambas as terapias (gel e filme) foram eficazes em promover a redução na profundidade à sondagem, ganho no nível clínico de inserção e apresentaram melhora na saúde periodontal considerando-se o menor sangramento após a sondagem que o grupo controle (RAR somente). The objective of this study was to clinically evaluate in humans semisolid (gels) and films systems containing metronidazole and metronidazole benzoate conjugate salts associated with scaling and root planing (SRP) for the treatment of chronic periodontitis. It was evaluated the pH of crevicular fluid and clinical bone attachment levels of the teeth involved in periodontal disease, before and after treatment, in addition to the release profile of drugs by high-performance liquid chromatography (HPLC). The study included 45 patients (both genders), from School of Dentistry of Ribeirão Preto, University of São Paulo, which had at least two teeth with chronic periodontitis, making a total for the study of 96 teeth which were randomly divided into 3 groups: Group 1 - scaling and root planing (SRP) - active control; Group 2 - RAR + film with metronidazole; Group 3 - SRP + gel with metronidazole. The metronidazole concentration and the drug release profile time in gingival crevicular fluid were monitored by evaluating the effects of such formulations, further with respect to clinical parameters of periodontitis. The effects of the treatments were evaluated for longitudinal monitoring of clinical parameters (probing depth, clinical attachment level and bleeding on probing). The results of the analysis of metronidazole and metronidazole benzoate concentrations in periodontal pockets of the patients were considered in a intragroup parallel evaluation and showed that the film and the gel were both detected in effective concentration till 48 hours after in situ application, but there was no difference significant between the groups. As for pH values after treatment, they ranged from 6 to 8. In clinical attachment level it was no statistically significant difference among the three groups, and the group that received only conventional periodontal treatment showed the best values. There was a progressive reduction in bleeding on probing in T15 and T30 times in all the three groups considered. With regard to clinical parameters, both therapies have been effective in promoting reduction in probing depth and gain in clinical attachment level as the control group, and both therapies showed improvement in periodontal health considering minor bleeding after probing that the (SRP alone) control group.

O comportamento mecânico das próteses parciais fixas implanto-suportadas são foco de vários estudos, onde o entendimento do processo de transmissão de forças mastigatórias e suas reações na estrutura óssea peri-implantar ainda não são totalmente compreendidos. Por isso, o objetivo deste trabalho foi verificar a deformação gerada na região peri-implantar de prótese parcial fixa mandibular. Para isto foi utilizado um modelo de poliuretano em forma de U simulando o osso mandibular com dois implantes hexágono externo de 3,75mm de diâmetro por 13mm de comprimento, nos quais foram fixados intermediários multi-unit. O trabalho apresentou dois grupos divididos de acordo com o tipo de liga das infraestruturas (CoCr ou PdAg) os quais foram submetidos a quatro situações de aplicação de carga. Foram aplicadas forças de 300N, nos pontos de referência pré-determinados na barra entre os implantes e na extremidade livre (5mm, 10mm e 15mm de cantilever). Foram realizadas leituras das deformações geradas na distal, lingual, mesial e vestibular de cada implante, com o uso de extensômetros lineares elétricos, as quais foram correlacionadas com a teoria de remodelação óssea proposta por Frost. Os resultados do estudo demonstraram que a deformação é influenciada pelo ponto de aplicação de carga e pelo módulo de elasticidade da liga da barra. A deformação é diretamente proporcional ao comprimento do cantilever, onde comprimentos maiores do que 10mm provocam tensões que superam a tensão máxima fisiológica proposta pela teoria de Frost. The mechanical behavior of implant-supported prosthesis is focus of some studies, with the process of bite forces transmissions and its peri-implant bone reactions is not completely understood yet. Therefore, the aim of this study was to verify the deformation in peri-implant region of mandibular fixed partial prosthesis. A U shaped polyurethane model was used to simulated the mandibular bone with two external hexagon implants with 3.75mm of diameter an 13mm of length, provided with multi-unit abutments. Two groups were formed according to the type of alloy of the framework (CoCr or PdAg), which were submitted to four load conditions. Loads of 300N were applied, in the predetermined points on the framework between the implants and the cantilever (5mm, 10mm and 15mm distal to the terminal implant). Readings of the deformations generated on the distal, lingual, mesial and bucal aspects of each implant were obtained with the use of strain gages, which were correlated to the theory proposed by Frost on bone remodeling. The results of the study demonstrated that deformation is influenced by the point of load application and by the elastic modulus of the alloy of the framework. The deformation is directly proportional to the cantilever length. Lengths bigger than 10mm generate tensions that exceed the maximum effective strain proposed by Frost.

Este estudo foi desenvolvido com o objetivo de avaliar os níveis de desajustes em interfaces pilar / cilindros protéticos obtidos a partir de cilindros calcináveis e pré-usinados fundidos em duas diferentes ligas metálicas, e a perda de torque em parafusos protéticos de titânio utilizados para fixar estas interfaces, analisando a relação entre estas variáveis. Os parafusos protéticos foram submetidos a análises de microscopia eletrônica de varredura antes e após o aperto. Os valores de torque foram aferidos por um torquímetro digital, os valores de desajustes registrados por um microscópio óptico e foram registradas imagens dos corpos-de-prova por uma câmera digital acoplada a outro microscópio óptico. Todos os valores de desajustes e perda de torque foram submetidos a testes de freqüência e analisados pelos testes T e Mann-Whitney para comparação entre os grupos, e pelo teste Spearman para a verificação da correlação entre as variáveis. Os resultados evidenciaram diferenças estatisticamente significantes (p ≤ 0,05) para os diferentes tipos de cilindros utilizados quanto ao desajuste (p = 0,000), e não foram evidenciadas diferenças estatísticas significantes (p ≤ 0,05) entre os cilindros quanto à perda de torque (p = 0,052) nem entre as diferentes ligas quanto ao desajuste (p = 0,615) e perda de torque (p = 0,811). A análise estatística confirmou a correlação (p ≤ 0,05) entre as variáveis desajuste e perda do torque (p = 0,017). Foram verificadas alterações nas superfícies das roscas dos parafusos após o torque e defeitos de fundição em alguns cilindros protéticos, especialmente nos cilindros fundidos a partir de cilindros calcináveis. De acordo com os resultados, a metodologia empregada e as limitações deste estudo, concluiu-se que os cilindros pré-usinados proporcionam níveis de desajustes mais favoráveis do que os cilindros calcináveis, existe correlação entre os desajustes nas interfaces pilar / cilindro protético e perda de torque nos parafusos de titânio, e defeitos de fundição podem influenciar nos desajustes. This study was developed with the objective to evaluate the levels of misfit in abutment / coping joint obtained from casting and pre-machined copings in two different alloys and the torque loosening in prosthetic titanium screws used to connect this joint, analyzing the relation between the two variable. The prosthetic screws were submitted to analyses on scanning electron microscopy before and after the torque. The values of torque were checked by one digital torque meter, the values of misfits were registered by an optic microscope and images of the samples were registered by an digital camera connected to another optic microscope. All the values of misfits and torque loosening was submitted to frequency tests and analyzed by the T and Mann-Whitney tests for the comparison between the groups, and by the Spearman test for the verification of the correlation between the two variable. The results had evidenced significant statistical differences (p ≤ 0,05) for the different types of copings used about misfit (p = 0,000), and had not been evidenced significant statistical differences (p ≤ 0,05) between the copings about the torque loosening (p = 0,052), neither for the different alloys about misfit (p = 0,615) and torque loosening (p = 0,811). The statistical Spearman test analysis confirmed the correlation between the misfit and torque loosening (p = 0,017). It was verified alterations in the surfaces of the threads of the screws after the torque and casting defects in some prosthetic cylinders, especially in the casting cylinders. In accordance with the results, the employed methodology and the limitations of this study, it has been concluded that the pre-machined copings provide more favorable levels of misfits than the casting copings, there is correlation between misfits in the abutment / coping joints and torque loosening in the titanium screws, and casting defects should influence in the misfits.

As restaurações fixas são rotineiramente fabricadas sobre um modelo de gesso, obtidos a partir do molde de um dente preparado. A precisão na adaptação destas é influênciada diretamente pelas propriedades dos materiais de moldagem empregado. Qualquer que seja o elastomêro eleito, o método mais confiável para se avaliar o seu desempenho em conduzir a modelos de gesso fiéis, parece ser aquele no qual é empregado um coroa-padrão que se adapta com precisão a um troquel apropriado. Este trabalho tem por objetivo avaliar se dentre as técnicas da dupla moldagem (com alívio de 1 e 2 mm) e a do casquete, existe uma mais adequada ou se ambas apresentam adequações suficiente e semelhantes. Foram empregados cinco elastômeros (Express, Impregum F, Imprint, Permlastic e President), os moldes foram obtidos empregando-se um dispositivo de moldagem idealizado por ARAÚJO e JÖRGENSEN6 e vazados com gesso especial tipo IV (Vel-Mix). A coroa-padrão foi posicionada nos troquéis e avaliada, quanto à sua precisão de adaptação, em um microscópio de medição de profundidade. Os resultados estatístico mostraram que a técnica do casquete apresentou melhores resultados para os materiais Impregum F e Imprint, vindos a seguir o Express e o President (ambos com alívio de 2 mm) e o Express (alívio de 1 mm); o Permlastic e o Express, na técnica do casquete, apresentaram piores resultados que os anteriores, mas semelhantes entre si, sendo o President (casquete) semelhante à este último; o President (alívio de 1 mm), foi o que apresentou pior resultado. Fixed restorations are usually produced on gypsum dies obtained from a prepared tooth mold and its precise adaptation is directly influenced by the properties of the impression materials. The objective of this study was to compare the performance of five impression materials and different methods of impression. Materials were tested using the putty-wash method being the first impression realeased in 1 or 2 mm (for Express and President) and the individual resin tray shell (Express, Impregum F, Imprint, Permlastic e Predident). The deviced used by ARAÚJO; JÖRGENSEN6 and the Vel-Mix (type IV) gypsum were used. The adaptation of one standard-crown to the dies was analysed with a depth measuring microscope. The results showed that Impregum F and Imprint (with a shell) gave the best results, followed by Express and President (putty/wash – 2 mm) and Express (putty/wash – 1 mm). The results with Permlastic and Express (shell) were less satisfactory and the difference between them wasn’t statistically significant. Express (shell) was not statistically different from the President (shell), and the worse result was given by the President (putty/wash - 1 mm) when compared to all the others.

O presente estudo teve como objetivos testar a hipótese nula de que não há diferença entre o grupo com Disfunção Temporomandibular (DTM) e o grupo Controle, para as frequências genotípicas e de alelos dos SNPs candidatos COMT Val(158)Met (rs4680); TNFA-308 (rs1800629); IL6-174 (rs1800795); IL-1β-3954 (rs1143634); IL10-592 (rs1800872); MMP1-1607 (rs1799750) e TLR4-896 (rs4986790); e determinar, nos casos com DTM, a influência dos SNPs candidatos na sensibilidade dolorosa mecânica experimental, na intensidade e no tempo de dor. As frequências genotípicas e de alelos dos sete SNPs candidatos foram comparadas entre 152 indivíduos com DTM e 91 controles, por meio da técnica de PCR em tempo real. A sensibilidade dolorosa mecânica foi avaliada pelo limiar de dor à palpação (LDP) e a intensidade de dor, utilizando-se uma escala analógica visual. As diferenças entre as frequências dos polimorfismos estudados foram avaliadas por meio do teste qui-quadrado (x2) e os valores de LDP entre os grupos foram comparados pelo teste t. A análise de possíveis diferenças entre os subgrupos de genótipos, com relação aos valores de LDP, EAV e tempo de dor, foi feita pelo teste estatístico one-way ANOVA, seguido pelo teste de Tukey. A odontologia reabilitadora moderna preconiza cada vez mais o uso de implantes dentais para a substituição de dentes ausentes. É sabido que micro-organismos presentes na cavidade oral, em especial os relacionados à doença periodontal, são responsáveis pelos maiores índices de insucesso dos implantes. Este trabalho estudou a ocorrência de infiltração bacteriana através da interface implante-conector protético de implantes Cone Morse (CM) e Hexágono Interno (HI) após a associação com uma pasta antimicrobiana ou a adição de sais de prata nos pilares. Foram utilizados 72 implantes odontológicos de titânio (PROSS® - Sistema de Implantes, Dabi-Atlante, Ribeirão Preto, Brasil), 36 com conexão do tipo hexágono interno e 36 com conexão do tipo cone-morse, divididos em grupos, da seguinte forma: Grupo Pasta Antimicrobiana- 12 conjuntos implantes HI/conectores protéticos e 12 conjuntos implantes CM/conectores protéticos, Grupo Íons de Prata- 12 conjuntos implantes HI/conectores protéticos e 12 conjuntos implantes CM/conectores protéticos e Grupo Controle- 12 conjuntos implantes HI/conectores protéticos e 12 conjuntos implantes CM/conectores protéticos. Os implantes e os conectores protéticos foram retirados de suas embalagens para aplicação do torque final (20N/cm). Antes disso, em um dos grupos experimentais, uma camada da pasta antimicrobiana foi aplicada sobre a superfície interna dos implantes e conectores protéticos e seus respectivos parafusos. Os conjuntos implantes-conectores foram parcialmente imersos na solução de saliva humana em tubos de ensaio e incubados em estufa bacteriológica a 37°C durante 7 dias. Após esse período, amostras do conteúdo interno dos conjuntos implantes-conectores protéticos dos 3 grupos foram colhidas com o objetivo de detectar e quantificar os micro-organismos presentes, utilizando para isto, a técnica de hibridização com sondas de DNA genômico DNA Checkerboard. O objetivo deste estudo foi avaliar clínica e radiograficamente implantes curtos com 5 e 6 mm de comprimento (Titamax WS Neodent/ Curitiba-Brasil) unitários instalados em região posterior de mandíbula e maxila com pouca altura óssea. Foram instalados 10 implantes em 8 pacientes devidamente selecionados a partir de critérios de inclusão pré-determinados. Os mesmos receberam carregamento protético somente após o tempo convencional de espera para a osseointegração. Seis implantes foram instalados em mandíbula e 4 implantes em maxila. Tomadas radiográficas foram realizadas para avaliação das perdas ósseas verticais e horizontais nos tempos T0 (Instalação cirúrgica), T1 (Reabertura), T2 (Instalação protética) e T3 (Acompanhamento de 6 meses). Foram avaliados parâmetros biológicos como, sangramento gengival, índice de mucosa ceratinizada, índice de placa e índice de inflamação gengival. Também foram avaliados parâmetros protéticos como proporção coroa-implante e distância mesiodistal. Apenas um implante (Titamax WS Cortical 5.0x6.0) foi perdido ainda no período de osseointegração portanto o índice de sucesso foi de 90% no período avaliado. A alteração do nível ósseo foi analisada em três períodos diferentes, da instalação cirúrgica à cirurgia de reabertura (período 1), da reabertura à instalação protética (Período 2) e da instalação protética ao controle de seis meses (Período 3). O comportamento biomecânico de próteses implantossuportadas foi analisado por fotoelasticidade e por correlação de imagens digitais (CID). Este trabalho estudou o comportamento biomecânico de próteses parciais fixas parafusadas sobre implantes, com coroas isoladas ou unidas, simulando a reabilitação de área posterior mandibular, com e sem a presença de elemento dental distal aos implantes. Devido às dificuldades em produzir estruturas múltiplas com assentamento passivo, alguns autores sugerem que implantes adjacentes sejam restaurados por próteses unitárias, minimizando os efeitos das tensões transferidas aos implantes. A análise fotoelástica é uma técnica experimental para análise de tensões, bastante empregada devido à sua relativa simplicidade e confiabilidade quanto à correspondência clínica dos achados observados. A análise por correlação de imagens digitais produz o mapeamento completo das tensões geradas na superfície do modelo analisado, possibilitando análises precisas. Não existe consenso na literatura quanto ao melhor planejamento protético para reabilitações parciais com múltiplos implantes adjacentes, a fim de minimizar as tensões geradas na interface osso-implante. O material de recobrimento estético empregado na restauração protética também foi analisado (cerâmica e resina). O modelo de estudo foi composto por um pré-molar em resina, dois implantes (Titamax GT - 4,0 x 11,0 mm - Neodent) substituindo o 2º pré-molar e o 1º molar, e um 2º molar em resina. Outro modelo foi produzido sem a presença do 2º molar. Os grupos analisados foram: UC - coroas unidas e recobertas por cerâmica; IC - coroas isoladas e recobertas por cerâmica; UR - coroas unidas e recobertas por resina; IR - coroas isoladas e recobertas por resina. O efeito do torque na fixação das estruturas foi avaliado por meio de análise fotoelástica. Análise fotoelástica qualitativa e quantitativa foi realizada após a aplicação de carga vertical puntiforme (50 N) na superfície oclusal da coroa substituindo o 1º molar. A análise por correlação de imagens também foi feita qualitativa e quantitativamente, com a carga aplicada na mesma posição que para a análise fotoelástica, mas com maior intensidade (250 N). Tensões na direção horizontal (εxx) e de cisalhamento (εxy) foram analisadas e comparadas pelo método CID. Os dois métodos para análise de tensões aplicados neste estudo foram satisfatórios e adequados para as comparações necessárias ao trabalho proposto. De acordo com os resultados obtidos, pode-se concluir que a esplintagem de coroas sobre múltiplos implantes adjacentes promoveu melhor distribuição de tensões em torno dos implantes e pelas estruturas de suporte, principalmente quando o contato proximal distal esteve ausente. As tensões totais no modelo não foram influenciadas pela presença do 2º molar, mas este dente participou da distribuição de tensões entre as estruturas. O material de recobrimento das coroas não foi significativo para as tensões transferidas ao modelo fotoelástico e para as tensões compressivas (εxx). As tensões de cisalhamento (εxy) foram influenciadas diretamente pela presença do 2º molar inferior e, com sua ausência, um material de recobrimento com baixo módulo de elasticidade (resina) foi significativo para minimizar estas tensões. A photoelastic stress analysis and a digital image correlation (DIC) method were used to analyze the biomechanical behavior of implant supported prosthesis. This study compared the biomechanical behavior of screwed, splinted and non-splinted, implant supported fixed partial dentures, simulating the rehabilitation of a mandibular posterior region, with and without the presence of a second molar proximal contact. Since complete passivity is difficult to achieve when splinted restorations supported by multiple implants are used, some authors suggest individually restoring adjacent implants to allow for a passive fit in the resulting restorations, thus reducing the strains transferred to the implants. Photoelasticity is an experimental technique for stress analysis and has been constantly used due its simplicity, reliable results and correlation with clinical findings. The digital image correlation method is capable of providing a full-field surface strain measurement, allowing for an accurate analysis. The ideal restoration of a partially edentulous space remains controversial as to the best prosthetic design for a partial rehabilitation with multiple implants in order to minimize the strains transferred to the bone-implant interface. The influence of the veneering materials was also compared (porcelain and resin). An epoxy resin model was made with acrylic resin replicas of a mandibular first premolar and second molar and with threaded implants (Titamax GT - 4.0 x 11.0 mm - Neodent) replacing the second premolar and first molar. Another model was made without the presence of the second molar. Groups were: UC - porcelain-veneered splinted crowns; IC - porcelain-veneered non-splinted crowns; UR - resin-veneered splinted crowns; IR - resin-veneered non-splinted crowns. Strains transferred to the photoelastic model were analyzed after tightening the prosthetic screws. Qualitative and quantitative photoelastic stress analysis was performed after the application of a vertical load (50 N) on the occlusal surface of the crown replacing the first molar. The image correlation analysis was also performed qualitatively and quantitatively, with the load applied in the same position as for the photoelastic analysis, although with greater intensity (250 N). Strains in the horizontal direction (εxx) and shear strains (εxy) were analyzed and compared with the DIC method. Both methods for strain analysis used in this study performed satisfactory and are adequate for the comparisons proposed in this work. According to the results found, it can be concluded that splinting multiple adjacent implant supported crowns provided a better strain distribution around the implants and the supporting structures, especially when the distal proximal contact was absent. The total strains in the model were not influenced by the presence of a distal proximal contact, but the presence of the second molar optimized the strain distribution between the structures. The crowns veneering material was not significant for the strains transferred to the photoelastic model and for compressive strains (εxx). Shear strains (εxy) were influenced by the presence of the second molar, and when it was absent, the use of a veneering material with low elasticity modulus (resin) significantly reduced these tensions.

A precisão na fabricação de um implante pode ser verificada pelo grau de adaptação na interface implante/componente protético. Uma pobre adaptação permite a infiltração de microrganismos através dessa interface e a colonização dos espaços vazios dos implantes, levando ao surgimento de reações inflamatórias dos tecidos periimplantares e ao insucesso clínico do tratamento. O objetivo deste trabalho foi avaliar, in vitro, a infiltração de uma bactéria, o Staphylococcus aureus, através da interface implante/componente protético, pelo método de cultura bacteriana. Foram utilizados vinte conjuntos de implantes Cone Morse, com os Pilares CM (NEODENT®, Curitiba, Paraná, Brasil), divididos em dois grupos: Grupo A, onde foram avaliados quanto à infiltração bacteriana de fora para dentro dos implantes, e Grupo B, onde foram avaliados quanto à infiltração bacteriana de dentro para fora dos implantes. Foram utilizados também vinte conjuntos de implantes Titamax II Plus, com os Mini Pilares Cônicos II Plus (NEODENT®, Curitiba, Paraná, Brasil), divididos em dois grupos: Grupo C, onde foram avaliados quanto à infiltração bacteriana de fora para dentro dos implantes, e Grupo D, onde foram avaliados quanto à infiltração bacteriana de dentro para fora dos implantes. Na avaliação da infiltração bacteriana de dentro para fora dos implantes, foi feita a inoculação da bactéria na parte interna dos implantes, e estes mergulhados em tubos de ensaio contendo um meio de enriquecimento estéril. Para avaliação da infiltração bacteriana de fora para dentro dos implantes, os conjuntos implantes/componentes foram mergulhados em tubos de ensaio contendo um meio de enriquecimento com cepas da bactéria. Ocorreu infiltração bacteriana em todos os grupos, e não houve diferença estatisticamente significante entre os grupos A e C, nem entre os grupos B e D, em relação à percentagem da passagem do microrganismo na interface implante/componente protético. The precision in the manufacture of an implant can be determined by the degree of adaptation at the abutment-implant interface. A poor adaptation allows the infiltration of microorganisms through this interface and colonization of empty spaces of the implants, leading to inflammatory reactions and clinical failure of treatment. The aim of the present in-vitro study was to investigate leakage of Staphylococcus aureus through the abutment-implant interface, by the method of bacterial culture. It was used twenty sets of Morse Taper implants with Pillars CM (NEODENTTM, Curitiba, Paraná, Brazil), divided into two groups: Group A, which were evaluated for bacterial infiltration into the inner part of the implants, and Group B, which were evaluated for bacterial infiltration from the inner part of the implants. Were also used twenty sets of Titamax II Plus implants, with the conical Mini Pillars II Plus (NEODENTTM, Curitiba, Paraná, Brazil), divided into two groups: Group C, which were evaluated for bacterial infiltration into the inner part of the implants and Group D, which were evaluated for bacterial infiltration from the inner part of the implants. In the evaluation of bacterial infiltration from the implants, the assemblies had the inner parts inoculated with S. aureus, and each assembly was incubated in BHI broth. For assessment of bacterial leakage into the implants, each assembly was submerged in S. aureus culture in tubes. Bacterial infiltration occurred in all groups, and there was no statistically significant difference between groups A and C, or between groups B and D.

O presente trabalho se propôs a avaliar, por meio de análise fotoelástica, o comportamento biomecânico de próteses parciais fixas cimentadas sobre implantes cone Morse (Titamax CM 4.0x11mm, Neodent) com coroas isoladas ou unidas, simulando a reabilitação de áreas posteriores com e sem a presença de elemento dental distal aos implantes, e ainda variando o tipo de material de recobrimento estético empregado na restauração protética (cerâmica ou resina), quando sobre estas próteses foram aplicadas diferentes tipos de cargas estáticas. Foi confeccionado um modelo fotoelástico simulando um espaço protético intercalado, com ausência do segundo pré-molar e primeiro molar, reabilitado por meio de quatro tipos de coroas cimentadas sobre dois implantes adjacentes: UC - coroas unidas com recobrimento estético em cerâmica; IC - coroas individuais com recobrimento estético em cerâmica; UR - coroas unidas com recobrimento estético em resina; IR - coroas individuais com recobrimento estético em resina. Análise fotoelástica qualitativa foi realizada sob diferentes condições de aplicação de carga na superfície oclusal das coroas: oclusal distribuída (10kgf), puntiforme simultânea (10kgf), puntiforme alternada no molar e no pré-molar (5kgf). A primeira análise foi feita na presença de contato proximal efetivo entre as próteses e o dente a distal dos implantes. A segunda análise foi realizada após a eliminação da coroa do dente distal no modelo fotoelástico. Registros fotográficos foram obtidos de cada situação de análise, a fim de facilitar a observação e comparação do padrão de distribuição das franjas isocromáticas em torno dos implantes. De acordo com os resultados obtidos, pode-se concluir que as restaurações cimentadas provocaram tensões no modelo fotoelástico mesmo quando da ausência de cargas oclusais. A ferulização das restaurações em cerâmica promoveu melhores distribuições das tensões isocromáticas em torno dos implantes. A presença do pilar distal com ponto de contato efetivo e ajustado nas restaurações modificou a distribuição das tensões isocromáticas, sendo que os estresses gerados pelas cargas oclusais foram também dissipados pelas réplicas dos dentes e transmitidos ao sistema fotoelástico. Ressalta-se a importância da obtenção de oclusão bem ajustada e equilibrada, para que haja adequada distribuição de forças aos implantes e osso de suporte, garantindo maior longevidade e previsibilidade de sucesso. The purpose of this study was to evaluate the biomechanical behavior of fixed prostheses cemented over morse taper implants simulating a posterior rehabilitation with or without distal teeth to the implants, by use of a photoelastic stress analysis technique. Non-segmented or segmented ceramic or resin crowns were constructed over the implants and were evaluated when different static loads were applied. A photoelastic model simulating a prosthetic space without the second premolar and the first molar was done. The edentulous space was rehabilitated with crowns cemented over morse taper implants: UC = non-segmented ceramic crowns; IC = segmented ceramic crowns; UR = non-segmented resin crowns, and IR = segmented resin crowns. Qualitative photoelastic analysis was done with different loads conditions at the occlusal surface of the crowns: distributed occlusal (10kgf), punctiform over the first molar and second premolar simultaneously (5kgf), and punctiform over the first molar or second premolar alternatively (5kgf). The first analysis was done when the proximal contact was present between the crowns and the distal teeth to implants. The second was done after the remotion of the distal teeth at the photoelastic model. The resulting stress in all areas of the model was monitored and recorded photographically in the field of a circular polariscope. The fringe pattern findings were collected for the loadings subjected to each condition and loading position. According to the results obtained it would be concluded that the cemented restorations promoted stress even without occlusal loads. The ferulization of the ceramic restorations promoted a better stress distribution around the implants. The presence of the distal teeth with an effective and adjusted contact point modified the stress distribution. Stresses generated by occlusal loads was distributed by the replicas of the teeth and transmitted to the photoelastic system. The adjusted and balanced occlusion is important to the correct load distribution to the implants and support bone, which assure a better prognostic to the rehabilitation.

A palavra DOR é definida como uma percepção consciente do indivíduo de impulsos nociceptivos modulados que originam uma experiência emocional e sensitiva desagradável, associada à lesão tecidual real ou potencial, ou descrita em termos de tal lesão. Considerando-se que a dor é um dos motivos mais comuns que levam um paciente a procurar por atendimento em consultório odontológico, este estudo teve como objetivo quantificar e qualificar a analgesia da musculatura mastigatória e da articulação temporomandibular proporcionada por medicamentos antiinflamatórios não esteroidais, associados ou não a outros agentes terapêuticos. O estudo clínico foi desenvolvido em pacientes que sofriam de algias crônicas na musculatura mastigatória, decorrentes de disfunções temporomandibulares. Foram selecionados, após anamnese e avaliação com a ferramenta RDC/TMD traduzido para a língua Portuguesa (PEREIRA JUNIOR, 2007), 18 voluntários para avaliar o efeito terapêutico (entendendo-se como efeito terapêutico o alívio da sintomatologia dolorosa e do restabelecimento da amplitude dos movimentos bordejantes mandibulares), dos três tratamentos coadjuvantes abaixo-relacionados, sendo dois medicamentos e um placebo para eliminar o efeito psicológico. Os tratamentos avaliados foram: um antiinflamatório não-esteroidal (AINES) Flanaren® (diclofenaco sódico), uma panacéia Sedilax® composta por AINES, miorrelaxante e analgésicos (diclofenaco sódico + carisoprodol + paracetamol + cafeína), ambos produzidos pelo laboratório Teuto® ; e um placebo, que consistia de pílulas preenchidas com 110 g de amido de milho, produzidas pela Faculdade de Ciências Farmacêuticas de Ribeirão Preto - USP. A administração de cada medicamento consistia de 1 unidade a cada 12 horas, durante um período de 10 dias, precedido e sucedido por avaliações de dor dos pacientes. Foi estabelecido um período de washout de 11 dias entre cada troca de tratamento. A avaliação dos tratamentos medicamentosos foi desenvolvida com diferentes ferramentas, como o McGill Pain Questionnaire (VAROLI; PEDRAZZI, 2006), para qualificar e quantificar dor não provocada, a escala visual analógica para dor à palpação, escala numérica para a quantificação da dor durante o tratamento, além de mensurações de amplitude de movimentos excursivos mandibulares. Foram colhidas também informações sobre possíveis efeitos colaterais indesejáveis relacionados aos tratamentos. O projeto foi submetido e aprovado pelo Comitê de Ética em Pesquisa envolvendo Seres Humanos da Faculdade de Odontologia de Ribeirão Preto da USP, Processo n.2006.1.558.58.0, Caae n.0022.0.138.000-06. Os resultados mostraram que a analgesia para a dor em repouso foi melhor com a utilização do Flanaren® e para a dor à palpação, igual para ambos os tratamentos. Os medicamentos Sedilax® e Flanaren® reduziram significantemente a dor após três dias de tratamento, enquanto o placebo, após oito dias. Não foram observadas melhoras na amplitude dos movimentos limítrofes da mandíbula. Também não foram observados efeitos colaterais significantes estatisticamente. Concluiu-se que o tratamento utilizando o diclofenaco sódico como adjuvante reduziu a dor em repouso; todos os tratamentos promoveram analgesia à dor à palpação, mas tanto o diclofenaco isolado como associado agiram no terceiro dia e o placebo, apenas no oitavo. Nenhum efeito colateral observado foi estatisticamente significante. The word PAIN is defined as a conscious perception of modulated nociceptive input from an unpleasant emotional and sensitive experience, associated to a real or potential, or described in terms of such lesion. Considering that pain is one of main reasons which motivate patients to search for dental treatment, the aim of this study was quantify and qualify analgesia in masticatory muscles and temporomandibular joints by administration of non steroidal anti-inflammatory drugs, isolated or associated to other therapeutic agents. This clinical trial has been developed treating patients who had been suffering with chronic pain in masticatory muscles due to temporomandibular disorders. Eighteen volunteers were selected after anamnesis exam and assessment using RDC/TMD translated to Portuguese (PEREIRA JUNIOR, 2007), to evaluate the therapeutics effect (pain relief and maximum eccentric jaw movement recovery) of three adjuvant treatment: two medicines and one placebo, to eliminate psychological effects. Assessed treatments were: a non steroidal anti-inflammatory Flanaren® (sodium diclofenac), a panacea composed by an anti-inflammatory, muscle relaxant and analgesics (sodium diclofenac + carisoprodol + acetaminophen + caffeine), both produced by pharmaceutical laboratory Teuto® ; and a placebo, that were pills filled by 110 g of corn starch, produced by Faculty of Pharmaceutical Sciences of Ribeirão Preto USP. The dosage of all medicines was one pill every 12 hours, during 10 days, preceded and succeeded by patients` pain assessment. An 11 days washout period among each therapy has been established. The assessment of drug therapies were done using distinct instruments, as McGill Pain Questionnaire (VAROLI; PEDRAZZI, 2006), to qualify and quantify unprovoked pain; visual analogue scale for pain on palpation, numerical scale to quantify pain during treatment, and measurement of range of motion during maximum eccentric jaw movements. It has been obtained information about side effects related to treatments. The research project was submitted and approved by Ethics in Research Committee of Faculty of Dentistry of Ribeirão Preto USP, Lawsuit n.2006.1.558.0, CAAE n. 0022.0.138.000-06. Data analysis has shown that relief of unprovoked pain was better using Flanaren® , and reduction of pain on palpation was equal in all treatments. Both, diclofenac alone, also diclofenac associate to other drugs, reduced significantly pain after three days of treatment, while placebo, after eight days. It has not been observed increase of range of motion during maximum jaw excursive movements, neither statistically significant side-effect. It has been concluded that treatment using diclofenac as an adjuvant reduced unprovoked pain; all therapies relief pain on palpation, but it was observed on third day for diclofenac and diclofenac associated and on eighth day for placebo. There was not any statistically significant side effect.

Objetivos: Avaliar o impacto da Enxaqueca e de outros subgrupos de DTM na severidade da Dor Miofascial da musculatura mastigatória e cervical. Adicionalmente, comparar índices de depressão e ansiedade, além da intensidade de dor subjetiva e outras características associadas entre pacientes com Dor Miofascial com e sem o diagnóstico adicional de Enxaqueca. Material e Métodos: Foram selecionados 203 pacientes, com idade média de 40,3 anos (89,2% do gênero feminino), que se apresentaram à Clínica de Dor Orofacial da UCLA-USA, todos com diagnóstico primário de Dor Miofascial. Pacientes com diagnóstico secundário de Enxaqueca, Cefaléia Tipo-Tensional, Osteoartrite e Capsulite também foram incluídos. Para a análise do impacto, foi utilizado o teste de Regressão Linear Múltipla. O teste de Mann-Whitney foi utilizado para comparar o grupo 1 (Dor Miofascial) com o 2 (Dor Miofascial +Enxaqueca) quanto à intensidade de dor objetiva (palpação) e subjetiva (EAV), ansiedade (BAI) e depressão (BDI), estado de humor, problemas com a função e qualidade do sono (EAV), e incapacidade (EAV e MIDAS). O objetivo desta pesquisa foi avaliar a influência do gênero e do bruxismo na força máxima de mordida. Além disso, a concordância inter-examinadores para o exame físico de bruxismo, foi estudada. Para isso, foram selecionados 120 indivíduos, de ambos os gêneros, portadores ou não de bruxismo, com idade média de 24,06 anos. Para o estabelecimento dos grupos, todos os indivíduos foram submetidos a critérios de exclusão e inclusão, e a exame físico específico para bruxismo (realizado por três examinadores). Após isso, a força máxima de mordida de cada indivíduo foi mensurada com o auxílio de um gnatodinamômetro na região de primeiro molar, três vezes de cada lado, em duas sessões separadas por dez dias, em média. O maior valor de força, dentre os seis encontrados para cada lado, foi utilizado como resultado. Os dados, então, foram analisados estatisticamente através de análise de variância a dois critérios, com nível de significância de 5%. Para a análise da concordância inter-examinadores utilizou-se o teste estatístico de Kappa. Os valores de força máxima de mordida, independentemente do grupo, foram significantemente maiores para o gênero masculino (p=0,00). Por outro lado, não foi observada, independentemente do gênero, diferença significante entre os valores de força máxima de mordida para os indivíduos portadores ou não de bruxismo (p=0,25). Ainda, a concordância inter-examinadores para o exame físico de bruxismo foi considerada ótima entre os 3 examinadores dessa pesquisa. Baseado nos resultados obtidos, conclui-se que a força de mordida não deve ser usada como parâmetro para detecção de bruxômanos. The objective of this research was to evaluate the influence of gender and bruxism in the maximum bite force. The concordance for the physical examination of bruxism between the examiners was evaluated as well. One hundred and twenty individuals, from both genders, bruxists and non bruxists, with mean age of 20.46 years old, were selected for the research. For the group establishment, every individual was submitted to inclusion and exclusion criteria, and to a specific physical examination for bruxism (performed by three different examiners). After that, the maximum bite force of every individual was measured using a gnatodinamometer at the first molar site, three times on each side, at two different times, separated by an average period of ten days. The highest measurement found among the six readings on each side was recorded. The data were then statistically analyzed using a two-way ANOVA, with a level of significance of 5%. For the concordance between examiners the Kappa statistical test was applied. The maximum bite force values were statistically higher for males (p=0.00), independently from the group analyzed. No statistically difference on the maximum bite force was observed from the gender (p=0.25). Finally, the concordance between examiners for physical examination of bruxism was considered optimal between the examiners. Based on these results, the authors can conclude that the bite force shall not be used as parameter for diagnosis of bruxism.
