Background and Objectives: This study investigates the efficacy of 4% articaine buccal infiltration in patients with mandibular molar irreversible pulpitis. The goal is to understand the anatomical factors contributing to articaine’s success in mandibular infiltrations. Materials and Methods: A randomized controlled trial was conducted with 50 patients diagnosed with symptomatic irreversible pulpitis in mandibular molars. Patients were divided into two groups: 4% articaine buccal infiltration (n = 30) and 2% lidocaine nerve block (n = 20). Pain was assessed using visual analog scales (VASs) before diagnosis, after thermal pulp sensitivity testing, and 5 and 10 min post-anesthetic application. Cone beam computed tomography (CBCT) was used to measure mandibular bone parameters. Results: The success rate for articaine buccal infiltration was 36.55%, with no significant difference between the efficacy in the first and second molars. CBCT measurements indicated no significant influence of buccal cortical bone thickness or distance from the mental foramen on anesthetic efficacy. However, articaine administration achieved anesthesia in some cases where lidocaine did not. Conclusions: Anatomical structures such as cortical bone thickness and distance from the mental foramen do not significantly influence the anesthetic efficacy of articaine in mandibular posterior infiltration for irreversible pulpitis treatment. These findings suggest that factors other than anatomical variations contribute to articaine’s superior performance in some clinical scenarios.

Abstract Background L-arginine (Arg) is an amino acid that contributes to several aspects of human biochemistry. Individuals with malnutrition and certain physical conditions could benefit from arginine intake. However, as Arg is required by certain viruses, it is advised to avoid it in one's diet and supplementation during viral illnesses. New studies have emerged during the COVID-19 pandemic, and pioneering research has been reviewed. Main body of the abstract The purpose of this review is to determine when and why Arg depletion, supplementation, or avoidance is advisable, considering the divergent results. A narrative review was conducted by surveying scientific publications indexed in electronic databases. Studies published from 1960 up to 2024, with no language restrictions, were included. Arg comes from proteins in the human diet. The kidney is the main site of endogenous Arg synthesis and also responsible for the overall metabolism of this amino acid, participating in synthesis, degradation and reabsorption. The liver can synthesize Arg, but since this is completely recycled in the urea cycle, it contributes little or no to the Arg plasma flux. Arg present in diet is passively absorbed in the small intestine and also transformed into urea and ornithine via urea cycle in hepatocytes. It is associated with macrophage metabolism, vasomotor control, intracellular signaling, memory formation, immune response, and an important messenger of the bronchopulmonary, cardiovascular and neural systems. Thus, excessive or decreased Arg concentration could impair health condition. High Arg concentrations stimulated rapid reactivation and resumption of protein synthesis in some viruses. Conclusion According to research, caution should be exercised when supplementing or depleting the amino acid arginine. Individuals who are carriers of latent viruses, such as herpesviruses, and/or who have been exposed to other viruses studied, should avoid arginine supplements and the consumption of foods rich in arginine. However, as prophylaxis or antiviral therapy, control of arginine intake as well as the use of lysine supplements, its antagonist, is recommended for short periods starting after a possible viral exposure, or in face of stimuli that can remove viruses from their latent state and/or at the very beginning of the viral manifestation, in order to avoid a large viral multiplication and consequently control the infection. Long-term arginine depletion can significantly affect cellular metabolism and its use as supplemental therapy needs case-by-case evaluation. Graphical abstract

Abstract Accurate mandibular canal (MC) detection is crucial to avoid nerve injury during surgical procedures. Moreover, the anatomic complexity of the interforaminal region requires a precise delineation of anatomical variations such as the anterior loop (AL). Therefore, CBCT-based presurgical planning is recommended, even though anatomical variations and lack of MC cortication make canal delineation challenging. To overcome these limitations, artificial intelligence (AI) may aid presurgical MC delineation. In the present study, we aim to train and validate an AI-driven tool capable of performing accurate segmentation of the MC even in the presence of anatomical variation such as AL. Results achieved high accuracy metrics, with 0.997 of global accuracy for both MC with and without AL. The anterior and middle sections of the MC, where most surgical interventions are performed, presented the most accurate segmentation compared to the posterior section. The AI-driven tool provided accurate segmentation of the mandibular canal, even in the presence of anatomical variation such as an anterior loop. Thus, the presently validated dedicated AI tool may aid clinicians in automating the segmentation of neurovascular canals and their anatomical variations. It may significantly contribute to presurgical planning for dental implant placement, especially in the interforaminal region.

Abstract This crossover study aimed to compare the anesthetic effects of buffered 2% articaine with 1:200,000 epinephrine with that of non-buffered 4% articaine with 1:200,000 epinephrine. Forty-seven volunteers were administered two doses of anesthesia in the buccal region of the second mandibular molars in two sessions using 1.8 mL of different local anesthetic solutions. The onset time and duration of pulp anesthesia, soft tissue pressure pain threshold, and the score of pain on puncture and burning during injection were evaluated. The operator, volunteers, and statistician were blinded. There were no significant differences in the parameters: onset of soft tissue anesthesia (p = 0.80), duration of soft tissue anesthesia (p = 0.10), onset of pulpal anesthesia in the second (p = 0.28) and first molars (p = 0.45), duration of pulp anesthesia of the second (p = 0.60) and first molars (p = 0.30), pain during puncture (p = 0.82) and injection (p = 0.80). No significant adverse events were observed. Buffered 2% articaine with 1:200,000 epinephrine did not differ from non-buffered 4% articaine with 1:200,000 epinephrine considering anesthetic success, safety, onset, duration of anesthesia, and pain on injection.

Lidocaine is the most commonly used local anesthetic worldwide, known for its rapid onset and moderate duration of anesthesia. However, it is short-lived and does not effectively promote effective topical anesthesia in the oral cavity when used alone. Our aim was to investigate whether an approximate 50% encapsulation of lidocaine in poly(ε-caprolactone) nanocapsules (LDC-Nano) would be able to increase its permeation and analgesic efficacy and reduce cytotoxicity. In this study, we characterized LDC-Nano and conducted MTT tests with HaCaT cells to assess their in vitro cytotoxicity. Additionally, in vitro permeation assays across the pig esophageal epithelium and the anesthetic efficacy of the hind paw incision model in rats were performed. Plain lidocaine (LDC) was compared with LDC-Nano and lidocaine hydrochloride plus epinephrine (LDC-Epi). The physicochemical characteristics of LDC-Nano were satisfactory (pH: 8.1 ± 0.21; polydispersity index: 0.08 ± 0.01; mean diameter (nm): 557.8 ± 22.7; and encapsulation efficiency (%): 51.8 ± 1.87) and remained stable for up to 4 months. LDC-Nano presented similar in vitro cytotoxicity to LDC but was higher than LDC-Epi (LD50: LDC = 0.48%; LDC-Nano = 0.47%; and LDC-Epi = 0.58%; p < 0.0001). Encapsulation increased the permeability coefficient about 6.6 times and about 7.5 the steady-state flux of lidocaine across the mucosal epithelium. Both encapsulation and epinephrine improved anesthesia duration, with epinephrine demonstrating superior efficacy (100% of animals were anesthetized up to 100, 30, and 20 min when LDC-Epi, LDC-nano, and LDC were used, respectively). Although LDC-Epi demonstrated superior in vivo anesthetic efficacy, the in vitro permeation and cytotoxicity of LDC-Nano indicate promising avenues for future research, particularly in exploring its potential application as a topical anesthetic in the oral cavity.

ABSTRACT The aim of this study was to emphasize the importance of monitoring vital signs, especially blood pressure, during dental care. Through a narrative review, the changes in blood pressure that may occur during outpatient procedures and the measures to be taken were discussed. The available literature was consulted in databases (PubMed, Scielo, Web of Science and Google Scholar), complemented by the analysis of the bibliographic references included and updated until April 2021. The keywords were used: “Anamnesis”, “Arterial Pressure”, “Arterial Hypertension”, “Hypertensive Crisis”, “Risk factors”, “Stroke”, “Subclavian Theft Syndrome” and “Atherosclerosis”. It was concluded that care should be applied to all patients, but mainly to those over 50, hypertensive or not, with hypercholesterolemia, diabetes, anxious and fearful. All vital signs must be monitored, including the pain score. The situations of anxiety, pain and fear are triggers for changes in blood pressure even in normotensive patients as in controlled hypertensive patients. The blood pressure measurement, in the first consultation, must be performed in both arms and repeated annually or whenever there is a change in the patient’s health status. In all other consultations, blood pressure control, as well as the assessment of all vital signs, must be performed before, during, after the procedure and before discharge. In the face of any major change, it is necessary to assess the risk/benefit ratio of the continuity of the procedure, so that risks to patients are avoided or minimized, as well as for the necessary referrals to be made.

ABSTRACT Background: Obesity and bariatric surgery may be related with mental and oral disorders. Aim: To evaluate the impact of bariatric surgery on anxiety, initial dental caries lesion and gingival bleeding in obese patients. Methods: Eighty-nine patients were divided in two groups: Control Group (CG) - obese patients and Experimental Group (EG) - patients submitted to bariatric surgery. EG was analyzed before and 12 months after bariatric surgery; for the CG, was respected an interval of 12 months between the evaluations. International Caries Detection and Assessment System, Gingival Bleeding Index and Trace-State Anxiety Inventory were used. Medical profile, anthropometrics data, sociodemographic and behavioral variables were considered. Results: There were no statistically significant differences between groups in evaluation times regarding to initial dental caries lesion and anxiety. Abstract: Evaluating factors that may influence the nose dimensions and, therefore, contribute to the singularity of a person, is important for obtaining a precise facial reconstruction (FR) in forensic anthropology. Thus, the aim in this study was to evaluate the influence of sex, skeletal class, and facial type on nose dimensions of Brazilian individuals using cone-beam computed tomography (CBCT) scans. CBCT images of 54 males and 67 females were classified according to skeletal class (class I, II, or III) and facial type (brachycephalic, dolichocephalic and mesocephalic). Linear and angular measurements of the nose were performed using the CS 3D Imaging software. The measurements obtained for males and females were compared by one-way ANOVA. Two-way ANOVA was used to compare the measurements in the different skeletal classes and facial types. Significance level was set at 5%. All linear measurements were greater in males (p < 0.05); the angular measurements, however, did not differ between sexes (p > 0.05). Linear and angular measurements of class III males were significantly different from the other classes; the same was observed for the angular measurements in females (p < 0.05). Regarding facial types, some linear measurements were significantly greater in dolichocephalics males (p < 0.05). For females, there was no influence of facial type (p > 0.05), except for the nasal convexity angle. Nose dimensions differed in specific points among different skeletal classes and facial types, and there was sexual dimorphism in all linear nose dimensions. This information may be applicable for nose reconstructions, allowing more reliable FR.

To determine whether the permeation capacity and analgesic efficacy of articaine (ATC) could be increased and cytotoxicity decreased by encapsulation in poly(ɛ-caprolactone) nanocapsules (ATCnano), aiming at local or topical anesthesia in dentistry. Cellular viability was evaluated (using the MTT test and fluorescence microscopy) after 1 h and 24 h exposure of HaCaT cells to ATC, ATCnano, ATC with epinephrine (ATCepi), and ATC in nanocapsules with epinephrine (ATCnanoepi). The profiles of permeation of 2% ATC and 2% ATCnano across swine esophageal epithelium were determined using Franz-type vertical diffusion cells. Analgesic efficacy was evaluated with a von Frey anesthesiometer in a postoperative pain model in rats, comparing the 2% ATC, 2% ATCnano, 2% ATCepi, and 2% ATCnanoepi formulations to 4% ATCepi (a commercially available formulation). Abstract Purpose: To evaluate the influence of subcutaneous injection nicotine in osseointegration process on different implant surfaces. Methods: Twenty-two male rabbits were distributed into two groups according to the subcutaneous injections: (1) nicotine 3 mg/day/kg and (2) 0.9 % NaCI 3 mL/day/kg, three times a day; subgroups were then designated-machined and anodized implants were placed in the right and left tibia bones, respectively. The animals were submitted euthanasia after periods of eight weeks to determine nicotine and cotinine levels, alkaline phosphatase and biomechanical analysis. Results: The plasmatic levels of nicotine and cotinine were 0.5 ± 0.28 ng/mL and 9.5 ± 6.51 ng/mL, respectively. The alkaline phosphatase analyses in blood levels in control group were observed 40.8 ± 11.88 UI/L and 40.75 ± 12.46 UI/L, for the surfaces machined and anodized, respectively. In the test group was observed levels 37.9 ± 4.84 UI/L, for both implant surfaces. No significant differences were observed between control and test groups and between the implant surfaces regarding alkaline phosphatase blood levels. For biomechanics, no significant differences were observed in control group between the machined (25±8.46 Ncm) or anodized (31.2 ± 6.76 Ncm) implants. However, the treatment with nicotine induced higher torque than control in both machined (38.3 ± 13.52 Ncm) and anodized (35.5 ± 14.17 Ncm) implants, with p = 0.0024 and p = 0.0121, respectively. Conclusion: Subcutaneous injection of nicotine following implant insertion didn’t have effect on osseointegration, independently from the implant surface.

Purpose: This study observed the effect of garlic extracts and amoxicillin against an induced staphylococcal infection model. MIC and MBC were also obtained for aqueous extracts of Allium sativum (Asa) and Allium tuberosum (Atu) against Staphylococcus aureus penicillin-sensitive (PSSA - ATCC 25923) and MRSA (ATCC 33592). Methods: Granulation tissues were induced in the back of 205 rats. After 14 days, 0.5 mL of 108 CFU/mL of PSSA or MRSA were injected inside tissues. After 24h, animals were divided: G1 (Control) – 0.5 mL of NaCl 0.9%; G2 – Asa 100 mg/kg or 400mg/kg; G3 – Atu 100 mg/kg or 400 mg/kg; G4 – amoxicillin suspension 50 mg/kg, considering PSSA infection; and G5 (Control) - 0.5 mL of NaCl 0.9%; G6 – Asa 400mg/kg; G7 – amoxicillin 50 mg/kg; and G8 - Asa 400 mg/kg + amoxicillin 50 mg/kg for MRSA. All treatments were administered P.O. every 6h. Animals were killed at 0, 6, 12 and 24h. Samples were spread on salt-mannitol agar. Colonies were counted after 18 h at 37 °C. Atu was not able to inhibit or kill PSSA and MRSA. Considering Asa, MIC and MBC against PSSA were 2 mg/mL and 4 mg/mL, respectively; and 16 mg/mL and 64 mg/mL against MRSA. Results: No effect was observed in vivo for control, Asa 100 mg/kg and Atu 100 mg/kg, while amoxicillin, Atu 400 mg/kg and Asa 400 mg/kg decreased PSSA counts in all-time points. No effect of any group against MRSA was observed at any time. Conclusion: Thus, A. sativum and A. tuberosum were able to reduce PSSA infection, but not MRSA infection.

Objective: The aim of the present study was to encapsulate vancomycin in different liposomal formulations and compare the in vitro antimicrobial activity against Staphylococcus aureus biofilms.Methods: Large unilamellar vesicles of conventional (LUV VAN), fusogenic (LUVfuso VAN), and cationic (LUVcat VAN) liposomes encapsulating VAN were characterized in terms of size, polydispersity index, zeta potential, morphology, encapsulation efficiency (%EE) and in vitro release kinetics. The formulations were tested for their Minimum Inhibitory Concentration (MIC) and inhibitory activity on biofilm formation and viability, using methicillin-susceptible S. aureus ATCC 29213 and methicillin-resistant S. aureus ATCC 43300 strains.Key Findings: LUV VAN showed better %EE (32.5%) and sustained release than LUVfuso VAN, LUVcat VAN, and free VAN. The formulations were stable over 180 days at 4°C, except for LUV VAN, which was stable up to 120 days. The MIC values for liposomal formulations and free VAN ranged from 0.78 to 1.56 µg/ml against both tested strains, with no difference in the inhibition of biofilm formation as compared to free VAN. However, when treating mature biofilm, encapsulated LUVfuso VAN increased the antimicrobial efficacy as compared to the other liposomal formulations and to free VAN, demonstrating a better ability to penetrate the biofilm.Conclusion: Vancomycin encapsulated in fusogenic liposomes demonstrated enhanced antimicrobial activity against mature S. aureus biofilms.

Permeation assays are important for the development of topical formulations applied on buccal mucosa. Swine buccal and esophageal epithelia are usually used as barriers for these assays, while frozen epithelia have been used to optimize the experimental setup. However, there is no consensus on these methods. In transdermal studies, barrier integrity has been evaluated by measuring electrical resistance (ER) across the skin, which has been demonstrated to be a simple, fast, safe, and cost-effective method. Therefore, the aims here were to investigate whether ER might also be an effective method to evaluate buccal and esophageal epithelium mucosa integrity for in vitro permeation studies, and to establish a cut-off ER value for each epithelium mucosa model. We further investigated whether buccal epithelium could be substituted by esophageal epithelium in transbuccal permeation studies, and whether their permeability and integrity were affected by freezing at −20 °C for 3 weeks. Fresh and frozen swine buccal and esophageal epithelia were mounted in Franz diffusion cells and were then submitted to ER measurement. Permeation assays were performed using lidocaine hydrochloride as a hydrophilic drug model. ER was shown to be a reliable method for evaluating esophageal and buccal epithelia. The esophageal epithelium presented higher permeability compared to the buccal epithelium. For both epithelia, freezing and storage led to decreased electrical resistivity and increased permeability. We conclude that ER may be safely used to confirm tissue integrity when it is equal to or above 3 kΩ for fresh esophageal mucosa, but not for buccal epithelium mucosa. However, the use of esophageal epithelium in in vitro transmucosal studies could overestimate the absorption of hydrophilic drugs. In addition, fresh samples are recommended for these experiments, especially when hydrophilic drugs are involved.

ABSTRACT Objective: Obesity is associated with an increased risk for breast cancer. Recent studies have shown that aromatase inhibitors may be less effective in women with a high body mass index (BMI). The aim of this study was to establish the relationship between the BMI and plasma estrone and estradiol levels in postmenopausal women with hormone receptor-positive breast cancer using anastrozole. Methods: In this cohort study, the patients were divided into three groups according to BMI (normal weight, overweight and obese) to compare and correlate plasma hormone levels before starting anastrozole hormone therapy and three months after treatment. Plasma hormone levels were compared for age and use of chemotherapy. Results: A statistically significant reduction in estrone and estradiol levels was observed between baseline and three months after starting the anastrozole treatment (p < 0.05). There was no statistically significant difference in plasma estrone and estradiol levels among the BMI groups (p > 0.05), but a significant reduction in plasma estrone levels was observed after three-months' treatment relative to baseline in all groups, as well as a reduction in estradiol in the obese group (p < 0.05). The use of chemotherapy and age > 65 years had no influence on plasma steroid levels. Conclusion: Changes in estrone and estradiol levels in the studied groups were not associated with BMI, chemotherapy or age.

Purpose: The aim of this study was to evaluate the antibothropic and anti-inflammatory properties of J. elliptica. Methods: Phytochemical screening and thin-layer chromatography (TLC) assays were performed on J. elliptica hydroalcoholic extract (TE) in order to observe its main constituents. The antibothropic activity of TE was evaluated by the in vitro neuromuscular blockade caused by Bothrops jararacussu venom (Bjssu), in a mouse phrenic nerve-diaphragm model (PND). A quantitative histological study was carried out to observe a possible protection of TE against the venom myotoxicity. The anti-inflammatory activity was also evaluated in two models, Bjssu-induced paw edema, and carrageenan-induced neutrophils migration in the peritoneal cavity. Results: TLC analysis revealed several compounds in TE, such as saponins, alkaloids, and phenolic constituents. TE was able to neutralize the blockade and the myotoxicity induced by venom, when it was pre-incubated for 30 min with venom. In addition, it showed anti-inflammatory activity, inducing less neutrophils migration and reducing paw edema. Conclusion: J. elliptica showed both antibothropic and anti-inflammatory properties.

ABSTRACT Background: Systemic bone loss may lead to more severe periodontal destruction, decreasing local bone mineral density. Aim: A cross-sectional designed was performed to study associations among alveolar bone pattern, salivary leptin concentrations, and clinical periodontal status in premenopausal obese and eutrophic women. Methods: Thirty morbid obese (G1) and 30 normal-weight (G2) women were included. Anthropometric and periodontal measurements (probing depth - PD, clinical attachment levels - CAL, presence of calculus, bleeding on probing -BOP, and plaque accumulation) were assessed. OHIP-14 was used for assessment of oral health impact on quality of life. Panoramic radiography was used to obtain the panoramic mandibular index (PMI), mandibular cortical index (MCI), and mental index (MI). Intraoral periapical (PA) radiography was taken to measure the total trabecular bone volume. Leptin was measured in saliva of fasted overnight women. Results: Groups 1 and 2 differed in all anthropometric aspects, but height. Pocket depth, calculus, BOP, and plaque index were worse in G1. No differences between groups were found considering OHIP. Normal-weight subjects showed higher proportion of dense bone trabeculae than obese subjects for pre-molars, but not for molars. Mental and panoramic mandibular indexes did not differ and were in normal level. Leptin concentration was dependent only on BMI. Conclusion: Obesity affected the periodontal conditions, the alveolar bone pattern, and the salivary leptin concentration.

Abstract Pain due to administration of local anesthetics is the primary reason for patients' fear and anxiety, and various methods are used to minimize it. This study aimed to measure the degree of pain during administration of anesthesia and determine the latency time and duration of pulpal anesthesia using two anesthetic methods in the maxilla. Materials and Methods: A randomized, single-blind, split-mouth clinical trial was conducted with 41 volunteers who required class I restorations in the maxillary first molars. Local anesthesia was administered with a needleless jet injection system (experimental group) or with a carpule syringe (control) using a 30-gauge short needle. The method of anesthesia and laterality of the maxilla were randomized. A pulp electric tester measured the latency time and duration of anesthesia in the second molar. Visual analogue scale (VAS) was used to measure the degree of pain during the anesthetic method. Data were tabulated and then analyzed by a statistician. The t-test was used to analyze the differences between the groups for basal electrical stimulation. Duration of anesthesia and degree of pain were compared using the Mann-Whitney test. A 5% significance level was considered. Results: There was no statistical difference in the basal electrical stimulation threshold (mA) and degree of pain between the two methods of anesthesia (p>0.05). Temporomandibular disorders (TMD) affecting the articular disc and/or the facial muscles are common among the population, recording a higher incidence in women age 20-40 years. The aim of this study was to investigate the correlation between facial types and muscle TMD in women. This study comprised 56 women age 18 to 49 years, seeking treatment for TMD at the School of Medicine, Federal University of São Paulo. All of the study individuals were diagnosed with muscle TMD, based on the Research Diagnostic Criteria (RDC). Facial type was determined using the Facial Brugsch Index and classified as euryprosopic (short and/or broad), mesoprosopic (average width) and leptoprosopic (long and/or narrow). The data were submitted to the Chi-square test and ANOVA-Tukey’s test to conduct the statistical analysis. The faces of 27 individuals were classified as euryprosopic (48%), 18 as mesoprosopic (32%), and 11 as leptoprosopic (20%). A statistically significant difference (Chi-square, p = 0.032) was found among the facial types, in that leptoprosopic facial types showed the lowest values for muscle TMD. A greater number (p = 0.0007) of cases of muscle TMD were observed in the 20 to 39 year-old subjects than in the subjects of other age segments. In conclusion, women with euryprosopic facial types could be more susceptible to muscle TMD. Further studies are needed to investigate this hypothesis.

Aim: This study carried out cranial measurements (in mm) [Zygion-Zygion (Zy-Zy); Zygion-Glabella-right side (Zy-Ga-right); Zygion-Glabella-left side (Zy-Ga-left); Zygion-Glabella-mean (Zy-Ga-mean); Rhinion-Anterior Nasal Spine (Rhi-ANS); Nasal Width (Na Wid); Nasion-Anterior Nasal Spine (Na-ANS); Glabella-Anterior Nasal Spine (Ga-ANS); Glabella-Prosthion (Ga-Pr)], to verify whether they are dimorphic. Methods: We used skulls from the Eduardo Daruge Laboratory of Forensic Physical Anthropometry, which did not present growth abnormalities and belonged to the age range of 18 to 100 years. Linear measurements were performed by digital caliper, properly calibrated. Inter and intra-calibrator calibration was performed by obtaining as result the value of 0.98 (considered excellent). Results: We found that all measures carried out are dimorphic, and we were able to create a logistic regression model (logit: Sex = −33.6 + (0.15 × Zy-Zy) + (0.21 × Rhi-ANS) + (0.16 × Na-ANS)) to estimation the sex. Conclusions: We concluded that the developed quantitative method results in 85.2% sensitivity, 76.2% specificity, and 81.1% accuracy, being, therefore, more effective in the prediction of sex than the mere random hit.

Little is known about the effects of stress hormones on the etiologic agents of halitosis. Thus, the aim of this study was to evaluate in vitro the effects of adrenaline (ADR), noradrenaline (NA) and cortisol (CORT) on bacteria that produce volatile sulfur compounds (VSC), the major gases responsible for bad breath. Cultures of Fusobacterium nucleatum (Fn), Porphyromonas endodontalis (Pe), Prevotella intermedia (Pi) and Porphyromonas gingivalis (Pg) were exposed to 50 µM ADR, NA and CORT or equivalent volumes of sterile water as controls for 12 and 24 h. Growth was evaluated based on absorbance at 660 nm. Portable gas chromatography was used to measure VSC concentrations. Kruskal-Wallis and the Dunn post-hoc test were used to compare the groups. For Fn, ADR, NA and CORT significantly reduced bacterial growth after 12 h and 24 h (p < 0.05). All the substances tested increased hydrogen sulfide (H2S) production (p < 0.05). For Pe, all the substances tested reduced bacterial development after 24 h (p < 0.05), and NA significantly increased the H2S concentration after 12 h (p < 0.05). In the Pg and Pi cultures, no effects on bacterial growth were observed (p > 0.05). In the Pi cultures, ADR, NA and CORT increased H2S (p < 0.05). Catecholamines and cortisol can interfere with growth and H2S production of sub-gingival species in vitro. This process appears to be complex and supports the association between stress and the production of VSC.

OBJECTIVE:To investigate the associations among salivary bacteria, oral emanations of volatile sulfur compounds, and academic-related chronic stress in healthy male subjects. MATERIALS AND METHODS:Seventy-eight healthy male undergraduate dental students were classified as stressed or not by evaluation of burnout, a syndrome attributed to academic-related chronic stress. This evaluation was carried out using the Maslach Burnout Inventory-Student Survey questionnaire. Oral emanations of hydrogen sulfide, methyl mercaptan, and dimethyl sulfide were measured using an Oral Chroma™ portable gas chromatograph. The amounts in saliva of total bacteria and seven bacteria associated with halitosis were quantified by qPCR. The in vitro production of H2S by S. moorei and/or F. nucleatum was also measured with the Oral Chroma™ instrument. RESULTS:The stressed students group showed increased oral emanations of hydrogen sulfide and dimethyl sulfide, together with higher salivary Solobacterium moorei levels (p < 0.05, Mann Whitney test). There were moderate positive correlations between the following pairs of variables: Fusobacterium nucleatum and S. moorei; F. nucleatum and hydrogen sulfide; Tannerella forsythia and F. nucleatum; T. forsythia and S. moorei. These correlations only occurred for the stressed group (p < 0.05, Spearman correlation). The in vitro experiment demonstrated that S. moorei increased H2S production by F. nucleatum (p < 0.05, ANOVA and Tukey's test). CONCLUSION:The increased amount of S. moorei in saliva, and its coexistence with F. nucleatum and T. forsythia, seemed to be responsible for increased oral hydrogen sulfide in the healthy male stressed subjects.

OBJECTIVE: To detect the presence and concentration of methylparaben in cartridges of commercial Brazilian local anesthetics. MATERIAL AND METHODS: Twelve commercial brands (4 in glass and 8 in plastic cartridges) of local anesthetic solutions for use in dentistry were purchased from the Brazilian market and analyzed. Different lots of the commercial brands were obtained in different Brazilian cities (Piracicaba, Campinas and São Paulo). Separation was performed using high performance liquid chromatography (HPLC) with UV-Vis detector. The mobile phase used was acetonitrile:water (75:25 - v/v), pH 4.5, adjusted with acetic acid at a flow rate of 1.0 ml.min-1. RESULTS: When detected in the solutions, the methylparaben concentration ranged from 0.01% (m/v) to 0.16% (m/v). One glass and all plastic cartridges presented methylparaben. CONCLUSION: 1. Methylparaben concentration varied among solutions from different manufacturers, and it was not indicated in the drug package inserts; 2. Since the presence of methylparaben in dental anesthetics is not regulated by the Brazilian National Health Surveillance Agency (ANVISA) and this substance could cause allergic reactions, it is important to alert dentists about its possible presence.

PURPOSE:To develop an anesthetic mucoadhesive film containing Acmella oleracea (jambu) extract for topical use on oral mucosa. METHODS:Ethanolic extracts from aerial parts of jambu were prepared by maceration. Pigment removal was obtained by adsorption with activated carbon. Three mucoadhesive films were developed using a film casting method: 10 or 20% of crude jambu extract (10% JB and 20% JB), and 10% of crude jambu extract treated with activated carbon (10% JBC). The mucoadhesive films were characterized regarding their uniformity, thickness, pH, and spilanthol content, and their stability was evaluated during 120 days. Gas chromatography was used to quantify the amount of spilanthol. In vitro tests determined the permeation of spilanthol across pig esophageal epithelium mucosa in Franz diffusion cells. Topical anesthetic efficacy was assessed in vivo using a tail flick test in mice. RESULTS:The three mucoadhesive films showed physical stability and visual appearances suitable for use on oral mucosa. The permeation study revealed that the spilanthol from 10% JBC presented higher flux and permeability coefficient values, compared to 10% or 20% JB (p < 0.001). Moreover, 10% JBC showed better topical anesthetic efficacy than the other films (p < 0.01). CONCLUSION:Mucoadhesive film containing crude extract of jambu treated with activated carbon is a potential alternative for oral, topical use, encouraging future clinical studies.

OBJECTIVE: Antimicrobial agents in combination with anti-inflammatory drugs have been usually prescribed in both Medicine and Dentistry. However, few scientific reports support this clinical practice. The aim of this study was to evaluate the effect of betamethasone and diclofenac sodium on serum and tissue concentration of amoxicillin in rats. METHODS: Four polyurethane sponges were implanted in the back skin of 48 rats. After seven days the animals were divided into 6 groups (n=8). Group 1: amoxicillin (25 mg/kg); G2: diclofenac sodium (2.5 mg/kg); G3: betamethasone (0.1 mg/kg); G4: diclofenac sodium and amoxicillin; G5: betamethasone and amoxicillin; and G6: 0.9% sodium chloride solution (1.0 mL - control group). All drugs were administered in a single dose. After 90 minutes, the granulomatous tissues of each animal were surgically removed and weighed. Blood was collected from cervical plexus, centrifuged and 10µL of serum was placed on paper discs. In order to estimate amoxicillin concentration, serum and granulomatous tissues were separately submitted to microbiological assay, which used 10(8)cfu/mL of Staphylococcus aureus ATCC 25923 (penicillin-susceptible strain). After incubation (18 hours, 37ºC) the inhibition zones were measured and compared to a regression curve. RESULTS: No inhibition zones were observed for groups 2, 3 and 6. Tissue and serum concentrations of both G1 (4.14µg/g and 2.06µg/mL, respectively) and G5 (3.87µg/g and 1.70µg/mL, respectively) showed statistically significant differences (Kruskal-Wallis, p0.05). CONCLUSION: Considering single doses, betamethasone did not interfere with amoxicillin levels but diclofenac sodium reduced both tissue and serum levels of amoxicillin in rats.OBJETIVO: A prescrição de antimicrobianos associados a antiinflamatórios é uma prática comum em odontologia, embora na maioria das vezes não haja justificativa para tal conduta. O objetivo deste trabalho foi avaliar, em um estudo in vivo em ratos, os efeitos da betametasona e do diclofenaco sódico nas concentrações sérica e tecidual da amoxicilina. MÉTODOS: Foram utilizados 48 ratos Wistar machos (6 grupos, n=8), com idade de 60 dias. Esponjas de PVC (policlorovinil) foram implantadas em quatro pontos no dorso de cada animal. Após sete dias, foram administrados por via intragástrica ou intramuscular: Grupo 1: amoxicilina (25 mg/kg); G2: diclofenaco sódico (2,5 mg/kg/i.m.); G3: betametasona (0,1 mg/kg/i.m.); G4: diclofenaco sódico e amoxicilina; G5: betametasona e amoxicilina; e G6: solução de cloreto de sódio a 0,9% (1,0 mL - grupo controle). Após 90 minutos, foram colhidos 2 tecidos granulomatosos e amostras séricas de cada animal e colocados em meios de cultura inoculados com 10(8) ufc/mL de Staphylococcus aureus ATCC 25923. Os halos de inibição foram medidos após 18 horas de incubação (37ºC), e através do teste microbiológico foram obtidas as concentrações séricas e teciduais da amoxicilina. RESULTADOS: Não foram observados halos de inibição para os grupos 2, 3 e 6. As concentrações séricas e teciduais de G1 (4,14µg/g e 2,06µg/mL, respectivamente) e G5 (3,87µg/g e 1,70µg/mL, respectivamente) demonstraram diferenças estatisticamente significantes (Kruskal-Wallis, p0,05). CONCLUSÃO: Considerando uma dose única, a betametasona não interferiu nas concentrações sérica e tecidual de amoxicilina, enquanto o diclofenaco sódico reduziu as concentrações sérica e tecidual de amoxicilina em ratos.

The aim of this study was to identify the impact of oral disease on the quality of life of morbid obese and normal weight individuals. Cohort was composed of 100 morbid-obese and 50 normal-weight subjects. Dental caries, community periodontal index, gingival bleeding on probing (BOP), calculus, probing pocket depth, clinical attachment level, dental wear, stimulated salivary flow, and salivary pH were used to evaluate oral diseases. Statins are important lipid-lowering agents with other pleiotropic effects. Several studies have explored a possible protective effect of statins to reduce the morbidity and mortality of many infectious diseases. Staphylococcus aureus is one of the main pathogens implicated in nosocomial infections; its ability to form biofilms makes treatment difficult. The present study observed the MIC of atorvastatin, pravastatin and simvastatin against S. aureus, Pseudomonas aeruginosa, Escherichia coli and Enterococcus faecalis. Simvastatin was the only agent with activity against clinical isolates and reference strains of methicilin-sensitive S. aureus (MSSA) and methicillin-resistant S. aureus (MRSA). Thus, the effects of simvastatin on the growth, viability and biofilm formation of S. aureus were tested. In addition, a possible synergistic effect between simvastatin and vancomycin was evaluated. Simvastatin's MIC was 15.65 µg/mL for S. aureus 29213 and 31.25 µg/mL for the other strains of S. aureus. The effect of simvastatin was bactericidal at 4xMIC and bacteriostatic at the MIC concentration. No synergistic effect was found between simvastatin and vancomycin. However, the results obtained against S. aureus biofilms showed that, in addition to inhibiting adhesion and biofilm formation at concentrations from 1/16xMIC to 4xMIC, simvastatin was also able to act against mature biofilms, reducing cell viability and extra-polysaccharide production. In conclusion, simvastatin showed pronounced antimicrobial activity against S. aureus biofilms, reducing their formation and viability.

Reabsorções radiculares (RR) são intercorrências preocupantes no tratamento ortodôntico. De aparecimento incontrolável e gravidade progressiva, podem comprometer a permanência do dente na arcada, em condições extremas. O ultrassom pulsado de baixa intensidade com frequência de 3 MHz atinge tecidos superficiais, como as raízes dos dentes, produzindo estimulação da cicatrização, por efeito de cavitação ou micromassagem. Pesquisas prévias mostraram a ação destas ondas sobre a consolidação de fraturas ósseas, estimulação da formação do tecido dentário e reparo de reabsorções radiculares. Neste trabalho foram medidos, em cortes trans-axiais vestíbulo-palatinos de tomografia computadorizada de feixe cônico tomados a 0,5 mm aquém do vértice, os ápices dos primeiros pré-molares superiores de 15 indivíduos com necessidade de exodontia destes dentes no plano de tratamento ortodôntico. O movimento de intrusão foi realizado com a instalação de arco utilidade, e colocação de elástico em cadeia unindo os braquetes, colados por vestibular, ao arco. A distensão do elástico foi feita até que se pudesse aferir força de 50 g. Foram aplicadas sessões de ultrassom pulsado de baixa intensidade de 10 minutos de duração no lado direito, imediatamente após a primeira ativação, e aos três, seis e nove dias após o início da movimentação. Nestas ocasiões também foi reavaliada a força, mantida em 50 g por meio de reativação dos elásticos, nos dois lados, tratado e controle. Aos 30 dias após a primeira ativação foram feitas novas tomografias e novas medidas efetuadas. A análise estatística mostrou que não houve diferença significativa (teste t pareado, p = 0,1316) entre as medidas iniciais e finais das raízes vestibulares dos dentes submetidos ao tratamento. As medidas finais das raízes vestibulares dos dentes controle foram significativamente menores (teste t pareado, p = 0,0455) que as iniciais, sugerindo que o ultrassom pulsado de baixa intensidade foi eficaz em preservar o tamanho da raiz.

INTRODUÇÃO: O bloqueio do nervo alveolar inferior (BNAI) apresenta alta porcentagem de falha na Odontologia. A fim de melhorar esse índice, vêm-se estudando diferentes alternativas, como diferentes técnicas e soluções anestésicas. OBJETIVO: Avaliar duas diferentes técnicas - técnica convencional e de Vazirani-Akinosi - para o bloqueio do nervo alveolar inferior, bem como compará-las quanto à sua efetividade e quantificar o percentual de aspirações positivas nas duas diferentes técnicas. MATERIAL E MÉTODO: Foram avaliados 160 pacientes de ambos os sexos, sendo 80 submetidos ao bloqueio do nervo alveolar inferior de Vazirani-Akinosi e bloqueio do nervo bucal (G1), e 80 submetidos ao bloqueio do nervo alveolar inferior convencional e ao bloqueio do nervo bucal (G2). Em ambos os grupos, utilizou-se a combinação de articaína 4% com epinefrina 1:100.000 para bloqueio do nervo bucal, e lidocaína 2% com epinefrina 1:100.000 para bloqueio do nervo alveolar inferior. Foram avaliados: a quantidade de aspirações positivas, a eficácia da anestesia e o momento em que ocorreu a falha anestésica durante o procedimento cirúrgico. RESULTADO: Não houve diferenças estatisticamente significantes (p = 0,2453) entre os grupos G1 e G2 observando-se a eficácia e o índice de aspirações positivas, e o momento em que ocorreu a falha anestésica, observando-se uma maior eficácia de ambas as técnicas, quando comparadas com a literatura (90%) CONCLUSÃO: Não houve diferença significativa entre o BNAI pela técnica convencional e o BNAI pela técnica de Vazirani-Akinosi quanto a quantidade de aspirações positivas e eficácia, sendo que o uso da articaína 4% com epinefrina 1:100.000 no bloqueio do nervo bucal possivelmente aumentou a eficácia anestésica de ambas as técnicas.

ABSTRACT Objective Halitosis can be caused by microorganisms that produce volatile sulphur compounds (VSCs), which colonize the surface of the tongue and subgingival sites. Studies have reported that the use of natural products can reduce the bacterial load and, consequently, the development of halitosis. The aim of this study was to evaluate the antimicrobial activity of the essential oil of Melaleuca alternifolia on the growth and volatile sulphur compound (VSC) production of oral bacteria compared with chlorhexidine. Material and Methods The effects of these substances were evaluated by the Minimum Inhibitory Concentration (MIC) and Minimum Bactericidal Concentration (MBC) in planktonic cultures of Porphyromonas gingivalis and Porphyromonas endodontalis. In addition, gas chromatography analyses were performed to measure the concentration of VSCs from bacterial cultures and to characterize M. alternifolia oil components. Results The MIC and MBC values were as follows: M. alternifolia - P. gingivalis (MIC and MBC=0.007%), P. endodontalis (MIC and MBC=0.007%=0.5%); chlorhexidine - P. gingivalis and P. endodontalis (MIC and MBC=1.5 mg/mL). INTRODUÇÃO: Uma infecção crônica nos tecidos periapicais pode desencadear uma lesão periapical em função da estimulação e da proliferação dos restos epiteliais de Malassez. Ao estudo microscópico, é possível detectar a presença de fendas de cristais de colesterol no interior de cistos e granulomas. De acordo com alguns estudos, a origem do colesterol no interior de lesões no periápice pode ser, parcialmente, por causa da condensação e da cristalização de colesterol dentro da parede do cisto. Outros estudos afirmam que a presença de colesterol pode ser derivada de produtos metabólicos locais ou da circulação vascular. OBJETIVO: O presente estudo teve por objetivo analisar a relação entre as fendas de cristais de colesterol encontradas nas lesões periapicais e os níveis de colesterol sérico. MATERIAL E MÉTODO: Estudo observacional de natureza descritiva, que incluiu 70 pacientes portadores de lesões periapicais, de acordo com exame radiográfico, atendidos no Departamento de Odontologia da Universidade Federal de Sergipe. Esses pacientes foram submetidos à exodontia da unidade dentária acometida e à remoção da lesão periapical, sendo esta peça encaminhada para exame anatomopatológico. Na mesma consulta, era solicitado um lipidograma para avaliação das taxas de colesterol sérico dos pacientes. RESULTADO: Fendas de cristais de colesterol estavam presentes apenas em cistos grandes (maiores que 1,5 cm de diâmetro, aproximadamente) e, portanto, mais antigos. CONCLUSÃO: A presença de cristais de colesterol é rara nas lesões periapicais. O estudo sugere uma relação entre a presença dos cristais de colesterol e o índice de colesterol sérico elevado, em lesões grandes e antigas.

INTRODUÇÃO: Medo e ansiedade são comuns a pacientes que requerem tratamento odontológico e, em se tratando de procedimentos cirúrgicos, a ansiedade pode tornar-se um fator complicador, a partir do momento em que a alteração dos sinais vitais do paciente pode gerar situações de emergência. OBJETIVO: Avaliar o grau de ansiedade pré-operatória dos pacientes submetidos a cirurgias orais, identificando qual procedimento cirúrgico causa mais ansiedade nos pacientes e qual o momento cirúrgico causador de maior ansiedade. MATERIAL E MÉTODO: Foram selecionados 200 pacientes que iriam se submeter a cirurgias bucais nos ambulatórios do Departamento de Odontologia da Universidade Federal de Sergipe. No dia da cirurgia, os pacientes respondiam à escala EAD, tendo sido avaliadas a frequência cardíaca (FC) e a pressão arterial sanguínea (PA), considerados dados basais dos parâmetros físicos empregados para a avaliação da ansiedade pré-operatória. RESULTADO: Não houve diferenças estatisticamente significantes (Kruskal-Wallis, p=0,6933) entre os tratamentos em relação à ansiedade. BACKGROUND AND OBJECTIVES:Submucosal dexamethasone injection directly in the surgical area has been used in different dental procedures, but there are still few studies evaluating its efficacy as compared to oral route for impacted third molar surgeries. So, this study aimed to evaluate postoperative pain, edema and trismus after impacted third molar surgeries using oral or submucosal local injection of dexamethasone.METHODS:This was a prospective, controlled, crossover and randomized study involving 36 patients with indication of lower third molar surgeries, who were randomly distributed in two groups: group A - submucosal local injection of dexamethasone (4mg/1mL) after local anesthesia, and group B - oral dexamethasone tablet (4mg) one hour before procedure. Edema and trismus were clinically evaluated in the postoperative period and in the 1st, 2nd, 3rd and 7th postoperative days. Patients were oriented to record pain intensity in the visual analog scale in periods zero (preoperative), 1h, 2h, 4h, 12h, 1 day, 2 days and 3 days and one week after surgery. Data were submitted to statistical analysis with significance level of 5%.RESULTS:There were no significant differences in surgical time with regard to operated sides (p=0.4). Edema and trismus values were not statistically different between observed groups (p>0.05). Mean pain values recorded in the visual analog scale were not statistically different between groups and patients have not reported major postoperative discomfort and had no need to prolong analgesic medication (p>0.05).CONCLUSION:Both dexamethasone administration routes were effective to control pain, edema and trismus after lower third molar surgeries, presenting similar results.

INTRODUÇÃO: No âmbito da clínica odontológica, o bloqueio do nervo alveolar inferior é o mais utilizado; todavia, diversos estudos têm mostrado as mais altas taxas de fracasso, as quais, normalmente, têm como motivo a falha na observação da posição, da forma e da anatomia dos nervos. Essas falhas decorrentes da variação anatômica do nervo alveolar inferior já vêm sendo estudadas na literatura, através de estudos com a análise de radiografias panorâmicas, como proposto neste estudo. OBJETIVO: O objetivo deste estudo foi verificar a ocorrência e a prevalência das variações anatômicas, bem como a correlação das variações do canal da mandíbula com lado e sexo, que podem ocorrer no canal mandibular, por meio de radiografias panorâmicas de pacientes atendidos no Departamento de Odontologia da Universidade Federal de Sergipe (UFS). MATERIAL E MÉTODO: Foram analisadas 1.500 radiografias panorâmicas de pacientes atendidos pelo Departamento de Odontologia da Universidade Federal de Sergipe (UFS). Imagens radiográficas foram observadas sobre um negatoscópio, utilizando-se uma máscara preta ao redor das radiografias, em ambiente com luminosidade apropriada. RESULTADO: Neste estudo, foram observados 5,3% de bifurcações do canal mandibular; 47,5% de canais altos; 16,8% de canais intermediários; 27,1% de canais baixos, e 8,6% de canais com outras variações. CONCLUSÃO: Baseando-se na altura do canal mandibular, houve maior prevalência dos canais altos do que os demais nas mulheres, sendo que não houve diferenças entre os gêneros com relação aos demais tipos e lados afetados. Na classificação dos canais bífidos, não houve diferença estatisticamente significante entre homens e mulheres. Observou-se que a maior prevalência foi para canais sem bifurcação.

JUSTIFICATIVA E OBJETIVOS: O adequado controle da dor pós-operatória constitui um desafio entre as especialidades cirúrgicas, a despeito dos recentes avanços das técnicas de analgesia e dos analgésicos. A cafeína tem sido utilizada como adjuvante terapêutico para potencializar a eficácia analgésica de alguns fármacos, porém ainda não existe relatos de investigação científica da associação com a dipirona sódica em dor pós-operatória em procedimentos odontológicos. Dessa forma, o objetivo deste estudo foi observar e comparar a eficácia da dipirona sódica isolada e da dipirona sódica associada à cafeína no controle da dor pós-operatória em cirurgia dental, considerando os parâmetros hemodinâmicos dos pacientes como indicador de ansiedade e de dor durante a cirurgia. MÉTODOS: Estudo cruzado e duplo encoberto incluiu 50 pacientes jovens e saudáveis (25 homens e 25 mulheres) com indicação de exodontia bilateral de terceiros molares mandibulares impactados. O lado operado, o gênero do paciente e o fármaco analgésico usado foram randomizados. Os escores obtidos pela escala analógica visual foram submetidos ao teste de Friedman (α = 0,05) para comparação das intensidades dolorosas em intervalos definidos (pré-operatório, pós-operatório imediato, 1, 2, 4 12 horas, 1, 2, 3 e 7 dias após as cirurgias) e as alterações nos parâmetros de pressão arterial e frequência cardíaca foram mensurados no pré-operatório, após a injeção anestésica e no pós-operatório (teste de Friedman, α = 0,05). RESULTADOS: Os pacientes experimentaram dor leve nos dois primeiros dias de pós-operatório e não houve diferença estatisticamente significante entre a eficácia analgésica da dipirona sódica isolada e da dipirona sódica associada à cafeína nos diferentes intervalos medidos. A maioria das alterações cardiovasculares observadas estava dentro da normalidade, considerando a ansiedade e estresse induzido pela cirurgia. CONCLUSÃO: O protocolo terapêutico proposto neste estudo não demonstrou diferença estatística significante na eficiência analgésica da dipirona sódica associada ou não a cafeína no controle da dor pós-exodontia.

INTRODUÇÃO: Um conhecimento profundo dos anestésicos odontológicos, como o tempo de latência da droga, pode assegurar o êxito do controle da dor no trans e no pós-operatório. OBJETIVO: Comparar a latência entre quatro soluções anestésicas, ou seja, o tempo entre o início da deposição do anestésico local e o momento em que seus efeitos tornam-se perceptíveis. Entretanto, isso não está relacionado com o êxito do controle da dor no trans e no pós-operatório (profundidade da anestesia). MATERIAL E MÉTODO: Foi realizado um estudo duplo cego, cruzado e randomizado, com 30 pacientes voluntários submetidos a quatro procedimentos em intervalos de uma semana, a partir de bloqueio do alveolar superior posterior. No segundo molar a ser tratado, foi utilizado o 'pulp tester' em intervalos de 2 minutos, considerando a insensibilidade da polpa quando da ausência de resposta após dois testes consecutivos de 80muV, chegando ao máximo de 10 minutos e determinando, assim, o período de latência do anestésico. Resumo Introdução O medo de sentir dor é um dos principais motivos pelos quais muitos pacientes evitam submeter-se a tratamento dentário. Em vários procedimentos odontológicos, o uso de anestésicos locais se faz necessário. O aparelho Morpheus® propõe a introdução da agulha sem dor e anestesia satisfatória, com uso de menor volume de anestésico local. Quando há necessidade de intervenção em molares e pré-molares inferiores, é preconizado o uso deste aparelho para técnica intrasseptal CaZOE, substituindo o bloqueio do nervo alveolar inferior tradicional (BNAI). Objetivo O presente trabalho de pesquisa configurou-se como um estudo randomizado, duplo cego e cruzado, cujo objetivo foi avaliar as propriedades anestésicas através da técnica intrasseptal CaZOE e da técnica convencional do bloqueio do nervo alveolar inferior (BNAI). Material e método Os procedimentos anestésicos foram realizados em duas sessões, por um único operador previamente treinado. Assim, foram avaliados parâmetros físicos, saturação periférica de O2, ansiedade, sensibilidade dolorosa, eficácia, tempo de latência e duração da anestesia em cada uma das técnicas, sendo então os respectivos dados comparados. Resultado Não houve diferenças significativas quanto aos parâmetros físicos e à saturação periférica de O2. A técnica intrasseptal CaZOE apresentou maior taxa de sucesso anestésico, menor tempo de latência, menor tempo de duração e menor desconforto na execução, quando comparada ao BNAI. Conclusão Inserir a injeção eletrônica na rotina da clínica odontológica pode proporcionar maior conforto aos pacientes submetidos à anestesia local e diminuir o abandono dos tratamentos odontológicos por medo e ansiedade associados à anestesia local.

INTRODUÇÃO: A ropivacaína (ROPI) é um anestésico local de longa duração de ação, introduzido mais recentemente em Medicina, mas ainda não disponível para uso odontológico em tubetes. Estudos clínicos e com animais confirmam que a ROPI é um anestésico local eficaz e que possui um efeito vasoconstritor intrínseco.OBJETIVO: Avaliar os efeitos da ROPI sobre a reatividade vascular em artéria mesentérica isolada de rato, além de comparar esse efeito ao da lidocaína (LIDO) e avaliar o possível envolvimento do endotélio vascular na resposta induzida pela ROPI em anel de artéria mesentérica isolada de rato.MATERIAL E MÉTODO: Foram utilizados 14 ratos Wistar machos (250-300g). Os animais foram eutanasiados e, através de uma incisão no abdome do animal, foi retirada a artéria mesentérica. Desta artéria, foram obtidos anéis (1-2 mm), que foram mantidos em cubas contendo 10 mL de solução nutritiva de Tyrode mantida a 37 °C e gaseificada com carbogênio. Para o registro das contrações isométricas, cada anel foi suspenso, por linhas de algodão, a um transdutor de força conectado a um sistema de aquisição.RESULTADO: Tanto a LIDO como a ROPI não apresentaram efeito vasoconstritor sobre o tônus basal de anéis com endotélio funcional. Porém, quando os anéis foram pré-contraídos com fenilefrina, ambas as drogas foram capazes de induzir vasorrelaxamentos dependentes da concentração (Emáx = 31,7 ± 3,3%; n = 6, para a LIDO; Emáx = 69 ± 8%; n = 6, para a ROPI), que não foram alterados após a remoção do endotélio (Emáx = 28,7 ± 1,3%; n = 7, para a LIDO; Emáx = 58,8 ± 5,9%; n = 6, para a ROPI). Em anéis sem endotélio funcional, porém, pré-contraídos com solução despolarizante de Tyrode (KCl a 80 mM), o efeito vasorrelaxante induzido pela LIDO não apresentou alterações significativas (Emáx = 29 ± 3%; n = 7). Porém, o efeito da ROPI foi reduzido significativamente neste protocolo, bem como na presença de 1 mM de tetraetilamônio (TEA) (Emáx = 21,2 ± 5,1%; n = 7 e Emáx = 17,4 ± 3,7; n = 4, respectivamente).CONCLUSÃO: A ROPI produz efeito vasorrelaxante em artéria mesentérica superior isolada de rato pré-contraída com fenilefrina, sendo este independente da participação do endotélio. Este efeito parece envolver os canais para K+ na célula muscular lisa vascular.

Abstract Introduction Blockade of the inferior alveolar nerve is among the most used and important anesthesia in dentistry, but there are no studies that compare the electronic anesthesia (Morpheus) and traditional manual anesthesia, using a syringe Carpule. Objective To evaluate and compare two anesthesia systems with respect to pain sensitivity during and after anesthesia with lidocaine 2% associated with epinephrine 1: 100,000 in anesthetic techniques for inferior alveolar nerve. Material and method This clinical trial was performed in a randomized, crossover, double-blind, involving 30 volunteers, who required dental treatment and who underwent blockade of the lower alveolar nerve, using the syringe type Carpule in the first session and the gun controlled speed Morpheus, where it was held in two sessions, with an interval of at least two weeks between each session. At the end of each session was applied to Visual Analogue Scale (VAS) to assess pain sensitivity to injection. Result A comparison between the conventional methods revealed that induce showed higher values than the EVA Morpheus. The chi -square for expected equal proportions showed that the technique with Morpheus had a higher (p = 0.0062) preference than conventional. Conclusion Taken together, the data show that the technique employing the Morpheus was superior to conventional in 3 times. The implementation of anesthesia performed with Morpheus proved to be more comfortable, have greater acceptance and preference for volunteers.
