Abstract

Abstract Background Anxiety, depression, and Type D personality are strongly correlated with the prognosis of IHD and the effectiveness of therapy. The main purpose of this study was to assess the proportions and associations of anxiety, depression, and Type D personality among clinically stable IHD patients (aged 18–60) treated at an outpatient clinic operated by a government hospital in Sri Lanka, who were diagnosed with IHD within the preceding three months. Methods A cross-sectional study design was analysed using SPSS® version 23.0. The validated Sinhalese version of the Hospital Anxiety and Depression Scale (HADS) was used to measure anxiety and depression, while the DS-14 was used to determine Type D personality traits. Results Among the 399 patients, 29.8% (n = 119) had anxiety, 24.8% (n = 99) had depression, and 24.6% (n = 24.6) had Type D personality. The level of anxiety had a significant association with depression (p = 0.002) and Type D personality (p = 0.003). Furthermore, depression was significantly associated with ethnicity (p = 0.014), occupation (p = 0.010), and type D personality (p = 0.009). Type D personality was the strongest predictor of anxiety, with patients being 1.902 times more likely to experience anxiety (95% CI 1.149–3.148; p = 0.012). Anxiety was a significant predictor of depression, with patients being 1.997 times more likely to experience depression (95% CI 1.210–3.296; p = 0.007). Non-Sinhalese ethnic background was also a significant predictor of depression (OR: 0.240; 95% CI 0.073–0.785; p = 0.018). Anxiety increased the likelihood of having Type D personality traits by 1.899 times (95% CI 1.148–3.143; p = 0.013). Conclusion The current study recommends the importance of screening and treating the psychological risk factors of IHD patients parallel to their IHD treatment to improve their prognosis. These insights highlight the need for targeted interventions that address depression, anxiety and the impact of Type D personality traits in enhancing the overall management and prognosis of IHD.

Abstract

Let $n$ and $a$ be positive integers such that $2\leq a\leq \frac{n}{2}$. In this short note, we compute for the exact value of the distance spectral radius, vertex-forwarding index, and some distance-based topological indices of the complement of circulant networks $C_n(1,a)$ and $C_{m^h}(1,m,m^2,\ldots,m^{h-1})$. Abstract

Tuberculosis (TB) is one of the most contagious diseases that has a greater mortality rate than HIV/AIDS and the cases of TB are feared to rise as a repercussion of the COVID-19 pandemic. The pharmaceutical industry is constantly looking for ways to improve drug design processes in order to combat the growth of infections and cure newly identified syndromes or genetically based dysfunctions with the help of QSPR models. QSPR models are mathematical tools that establish relationships between a molecular structure and its physicochemical attributes using structural properties. Topological indices are such properties that are generated from the molecular graph without any empirically derived measurements. This work focuses on developing a QSPR model using distance-based topological indices for anti-tuberculosis medications and their diverse physicochemical features.

Abstract

In this paper, we use Breadth-first search algorithm to determine the distance matrix of multiplicative circulant graph of order power of two and three. As a consequence, the diameter of the graphs were determined. We also give their distance spectral radii, average distances, as well as the exact values of vertex-forwarding indices. Finally, using some known relationships between the distance spectral radii and forwarding indices of a graph, we give some bounds for their edge-forwarding indices., Comment: 22 pages, 12 figures

Abstract

The development of the National Institutes of Health (NIH) COVID-19 Treatment Guidelines began in March 2020 in response to a request from the White House Coronavirus Task Force. Within 4 days of the request, the NIH COVID-19 Treatment Guidelines Panel was established and the first meeting took place (virtually-as did subsequent meetings). The Panel comprises 57 individuals representing 6 governmental agencies, 11 professional societies, and 33 medical centers, plus 2 community members, who have worked together to create and frequently update the guidelines on the basis of evidence from the most recent clinical studies available. The initial version of the guidelines was completed within 2 weeks and posted online on 21 April 2020. Initially, sparse evidence was available to guide COVID-19 treatment recommendations. However, treatment data rapidly accrued based on results from clinical studies that used various study designs and evaluated different therapeutic agents and approaches. Data have continued to evolve at a rapid pace, leading to 24 revisions and updates of the guidelines in the first year. This process has provided important lessons for responding to an unprecedented public health emergency: Providers and stakeholders are eager to access credible, current treatment guidelines; governmental agencies, professional societies, and health care leaders can work together effectively and expeditiously; panelists from various disciplines, including biostatistics, are important for quickly developing well-informed recommendations; well-powered randomized clinical trials continue to provide the most compelling evidence to guide treatment recommendations; treatment recommendations need to be developed in a confidential setting free from external pressures; development of a user-friendly, web-based format for communicating with health care providers requires substantial administrative support; and frequent updates are necessary as clinical evidence rapidly emerges.

Abstract

Lipophilicity is known to influence a wide range of ADMET (absorption, distribution, metabolism, excretion and toxicity) properties and is widely regarded as one of the most important parameters within drug discovery programs. Unfortunately, in recent years lipophilicity modulation has often been abused to increase the potency of drug molecules. This has caused in an overall increase in lipophilicity for orally available drugs, typically resulting in undesirable effects on the aforementioned ADMET properties. Hence, as late-stage drug attrition is very costly, in order to improve the druggability of a compound there has been an increased awareness of the importance of lipophilicity modulation within drug discovery programs. Fluorination is a tool commonly used within drug development to modulate a wide range of pharmacokinetic properties, in particular lipophilicity. While the effects of aromatic fluorination on lipophilicity have been well studied, due to constraints of commonly utilized analytical techniques used to measure lipophilicity (requirement of a UV chromophore), aliphatic fluorination has not. Fortunately, through the use of a 19F NMR based method, the effects of aliphatic fluorination can now be reliably measured. Therefore, within this thesis the synthesis and lipophilicity measurement of a wide range of fluorinated alkanols, containing both known and novel motifs, will be covered. This allowed for an in-depth discussion into the effects of aliphatic fluorination on lipophilicity. It is also of interest for medicinal chemists whether these lipophilicity modulations persist on more complex drug scaffolds. Hence, the incorporation of a series of interesting aliphatic fluorinated motifs into a drug molecule was performed and their influence on lipophilicity was reproduced.

Abstract

OBJECTIVES:This study sought to determine whether T2 cardiac magnetic resonance (CMR) can stage both hemorrhagic and nonhemorrhagic myocardial infarctions (MIs). BACKGROUND:CMR-based staging of MI with or without contrast agents relies on the resolution of T2 elevations in the chronic phase, but whether this approach can be used to stage both hemorrhagic and nonhemorrhagic MIs is unclear. METHODS:Hemorrhagic (n = 15) and nonhemorrhagic (n = 9) MIs were created in dogs. Multiparametric noncontrast mapping (T1, T2, and T2*) and late gadolinium enhancement (LGE) were performed at 1.5- and 3.0-T at 5 days (acute) and 8 weeks (chronic) post-MI. CMR relaxation values and LGE intensities of hemorrhagic, peri-hemorrhagic, nonhemorrhagic, and remote territories were measured. Histopathology was performed to elucidate CMR findings. RESULTS:T2 of nonhemorrhagic MIs was significantly elevated in the acute phase relative to remote territories (1.5-T: 39.8 ± 12.8%; 3.0-T: 27.9 ± 16.5%; p < 0.0001 for both) but resolved to remote values by week 8 (1.5-T: -0.0 ± 3.2%; p = 0.678; 3.0-T: -0.5 ± 5.9%; p = 0.601). In hemorrhagic MI, T2 of hemorrhage core was significantly elevated in the acute phase (1.5-T: 17.7 ± 10.0%; 3.0-T: 8.6 ± 8.2%; p < 0.0001 for both) but decreased below remote values by week 8 (1.5-T: -8.2 ± 3.9%; 3.0-T: -5.6 ± 6.0%; p < 0.0001 for both). Abstract

Background: Ischemic heart disease (IHD) has been identified as the foremost cause of morbidity and mortality in Sri Lanka. Aims and Objectives: The main aim of this study was to develop a socio-culturally appropriate health counseling intervention protocol to enhance the adherence of prescribed physical activity, diet, and medication among IHD patients attending medical clinics in Sri Lanka. Materials and Methods: This protocol was designed to be used by health professionals and consisted of two sections – an assessment and an intervention. The intervention section integrated communication skills with motivational interviewing, goal setting, and problem-solving. A panel of five experts reviewed each section of the protocol, in a Delphi process. The ratings received at Delphi stage one that was evaluated for degree of consensus. The protocol was modified according to the comments received and resent for a second Delphi round. Again, the re-ratings were reevaluated for the degree of consensus. With regard to assessing consensus, each section was revised/removed if 70% or more of the re-ratings were at 0–3 and kept as the same if 70% or more of the re-ratings were at four or above. Results: All the subsections were rated in categories of four and above in both stages. Therefore, none of the sections was removed from the original protocol. Some modifications were done according to the suggestions received in the Delphi review. Conclusion: The finalized protocol would be initially implemented in selected hospitals in Galle district as a quasi-experimental study in the near future.

Abstract

SUMMARY: Medicinal plants (MPs) are vital in treating and controlling diseases and parasites in indigenous chickens, enhancing their health management. However, the impact of climate change on these MPs, particularly those used for indigenous chicken health, needs further exploration. This paper reviews the impact of climate change on the cultivation and conservation of MPs used in treating diseases and controlling parasites in indigenous chickens. A database search was conducted in Google Scholar, ScienceDirect, EBSCOhost, and Connected Papers using keywords such as "medicinal plants," "climate change," "indigenous chicken," "health management," "Aloe ferox," "Helichrysum," "Agave sisalana," "Centella asiatica," "Gunnera perpensa," "Lippia javanica," and "Tagetes minuta." A total of 318 articles were retrieved, of which 84% were discarded for not focusing on medicinal plants for animal diseases or the impact of climate change on MP cultivation and conservation. Ultimately, 16% of the articles were relevant to the study's scope. The effect of climate change on the cultivation and conservation of Aloe ferox, Helichrysum, and Tagetes minuta is negligible. Lippia javanica, however, is threatened by climate change, with its interactions with pollinators affecting its reproduction. Conversely, Aloe ferox thrives under arid and semi-arid conditions associated with climate change, and Helichrysum petiolare shows resilience to climate change-induced water stress. Overall, MPs used for treating indigenous chicken diseases and controlling parasites may adapt well to changing climatic environments. Additionally, most MPs are naturally found in their habitats and do not require sophisticated production methods for conservation. Using MPs to treat indigenous chicken diseases and control parasites is crucial for promoting Indigenous Chicken Production and food security. 35. Addressing Mental Health in Rural Settings: A Narrative Review of Blueberry Supplementation as a Natural Intervention.

Abstract

Depression and anxiety are major public health issues; however, there is an unmet need for novel, effective, and accessible treatments, particularly in rural communities. Blueberries are an unexplored nutraceutical for these conditions due to their excellent nutritional profile, with particularly high levels of polyphenols and anthocyanins and benefits on mood, cognition, and health. Here, we present a narrative review of the literature concerning the etiology and treatments of major depressive disorder (MDD) and generalized anxiety disorder (GAD). In both animal and human studies, blueberry supplementation can ameliorate behavioral symptoms of both anxiety and depression. The mechanistic underpinnings of these behavioral improvements are not fully defined, but likely involve biochemical alterations in the gut–brain axis, including to inflammatory cytokines, reactive oxygen species, and growth factors. We also review the limitations of traditional therapies in rural settings. Finally, we assess the potential benefit of nutraceutical interventions, particularly blueberries, as novel therapeutics for these distinct, yet related mental health issues. [ABSTRACT FROM AUTHOR]

Abstract

Based on topological descriptors, QSPR analysis is an incredibly helpful statistical method for examining many physical and chemical properties of compounds without demanding costly and time-consuming laboratory tests. Firstly, we discuss and provide research on kidney cancer drugs using topological indices and done partition of the edges of kidney cancer drugs which are based on the degree. Secondly, we examine the attributes of nineteen drugs casodex, eligard, mitoxanrone, rubraca, and zoladex, etc and among others, using linear QSPR model. The study in the article not only demonstrates a good correlation between TIs and physical characteristics with the QSPR model being the most suitable for predicting complexity, enthalpy, molar refractivity, and other factors and a best-fit model is attained in this study. This theoretical approach might benefit chemists and professionals in the pharmaceutical industry to forecast the characteristics of kidney cancer therapies. This leads towards new opportunities to paved the way for drug discovery and the formation of efficient and suitable treatment options in therapeutic targeting. Gold is widely recognized as a noble metal due to its inherent inertness in its bulk form. Nevertheless, gold exhibits reactivity in its ionic form. The inert qualities of bulk gold have led to its extensive recognition as a fundamental raw ingredient in several biomedical processes. These applications encompass drug delivery microchips, dental prostheses, reconstructive surgery, food additives, and endovascular stents. Gold in large amounts can be thought of as safe. Gold can also exist as molecules or ions, specifically gold ions, making it easier to make gold nanomaterials. The distinctive characteristics of gold set it apart from its molecular or bulk states, making its execution a very efficient instrument in the field of nanomedicine. Some of these traits are ease of synthesis, a higher ratio of surface area to volume, more reactive particles, the ability to withstand changes to the surface, and strong optical properties. Abstract

Poor understanding of market demand is a key factor in the recurrent failure of many production-orientated aquaculture interventions. In the Dry-Zone of Sri Lanka stocking-based fisheries enhancements in small-scale reservoirs have been promoted by the State since the 1980's with no evidence of sustainable adoption. Through an evaluation of positive (actual) and normative (aspirational) consumer preferences for inland fish and its substitutes, we aimed to assess underlying reasons for this recurrent failure and future prospects for the policy and rural food-security. Work commenced with a detailed scoping-analysis to establish socio-economic context and a sample frame for systematic surveys. Consumption recall and preference ranking/scoring techniques were applied in two iterative survey phases across 6 villages of Northwest Province. Totals of 39 and 165 respondents were surveyed across the two phases with selection stratified on age, gender, caste and wealth criteria. Results underscored the importance of inland fish to rural food security. Fresh inland fish was consumed by 60% of all respondents at least once per fortnight and by 82% at least once per month with tilapias accounting for 75–85% of meals. Preference for inland fish compared over similarly priced substitutes is attributable to their freshness at point of sale above any other intrinsic product quality.
