Informality is a reality for three-quarters of domestic workers in Brazil who, without a formal employment relationship, are not protected by the legal guarantees established since 2013. This lack of legal protection is exacerbated by the emphasis on entrepreneurship promoted by a growing number of digital platforms offering domestic work intermediation. In this article, we examine the structural standards of digital intermediation companies for domestic work in Brazil. Four representative platforms (GetNinjas, Parafuzo, Helpty, and Crafty) were analyzed. Our research corpus consists of their websites and applications, their terms of use, as well as journalistic articles and complaints from workers and clients on the website Reclame Aqui. The results are presented in three parts: (i) the history and general characteristics of each platform; (ii) their content organized into four categories of analysis (marketing, hiring, remuneration, and reputation systems); and (iii) the accounts of clients and workers on the Reclame Aqui website. The study found that digital intermediaries for domestic work present complex challenges for workers, as they amplify existing risks in the dominant labor model, such as intense subordination, economic dependence, lack of transparency about the value of their work, and exclusion from legal labor protection, whose recent expansion has not resulted in effective change. The study also suggests that this intensification of risks increases directly with the degree of intervention by the digital intermediary in the client-worker relationship

Several studies investigated the role of socio-demographic, clinical, and psychosocial factors in predicting relevant outcomes in breast cancer care, thus developing predictive models. Our aim is to summarize predictive models for specific psychological and behavioral outcomes: psychological distress, quality of life, and medication adherence. Specifically, we aim to map the determinants of the outcomes of interest, offering a thorough overview of these models.Methods: Databases (PubMed, Scopus, Embase) have been searched to identify studies meeting the inclusion criteria: a breast cancer patients’ sample, development/validation of a predictive model for selected psychological/behavioral outcomes (ie, psychological distress, quality of life, and medication adherence), and availability of English full-text.Results: Twenty-one papers describing predictive models for psychological distress, quality of life, and adherence to medication in breast cancer were included. The models were developed using different statistical approaches. It has been shown that treatment-related factors (eg, side-effects, type of surgery or treatment received), socio-demographic (eg, younger age, lower income, and inactive occupational status), clinical (eg, advanced stage of disease, comorbidities, physical symptoms such as fatigue, insomnia, and pain) and psychological variables (eg, anxiety, depression, body image dissatisfaction) might predict poorer outcomes.Conclusion: Predictive models of distress, quality of life, and adherence, although heterogeneous, showed good predictive values, as indicated by the reported performance measures and metrics. Many of the predictors are easily available in patients’ health records, whereas others (eg, coping strategies, perceived social support, illness perceptions) might be introduced in routine assessment practices. The possibility to assess such factors is a relevant resource for clinicians and researchers involved in developing and implementing psychological interventions for breast cancer patients.Keywords: breast cancer, predictive model, psychological distress, quality of life, adherence, predictors

BackgroundChronic pain is one of the most common and critical long-term effects of breast cancer. Digital health technologies enhance the management of chronic pain by monitoring physical and psychological health status and supporting pain self-management and patient treatment decisions throughout the clinical pathway. ObjectiveThis pilot study aims to evaluate patients’ experiences, including usability, with a novel digital integrated health ecosystem for chronic pain named PainRELife. The sample included patients with breast cancer during survivorship. The PainRELife ecosystem comprises a cloud technology platform interconnected with electronic health records and patients' devices to gather integrated health care data. MethodsWe enrolled 25 patients with breast cancer (mean age 47.12 years) experiencing pain. They were instructed to use the PainRELife mobile app for 3 months consecutively. The Mobile Application Rating Scale (MARS) was used to evaluate usability. Furthermore, pain self-efficacy and participation in treatment decisions were evaluated. The study received ethical approval (R1597/21-IEO 1701) from the Ethical Committee of the European Institute of Oncology. ResultsThe MARS subscale scores were medium to high (range: 3.31-4.18), and the total app quality score was 3.90. Patients with breast cancer reported reduced pain intensity at 3 months, from a mean of 5 at T0 to a mean of 3.72 at T2 (P=.04). The total number of times the app was accessed was positively correlated with pain intensity at 3 months (P=.03). The engagement (P=.03), information (P=.04), and subjective quality (P=.007) subscales were positively correlated with shared decision-making. Furthermore, participants with a lower pain self-efficacy at T2 (mean 40.83) used the mobile app more than participants with a higher pain self-efficacy (mean 48.46; P=.057). ConclusionsThe data collected in this study highlight that digital health technologies, when developed using a patient-driven approach, might be valuable tools for increasing participation in clinical care by patients with breast cancer, permitting them to achieve a series of key clinical outcomes and improving quality of life. Digital integrated health ecosystems might be important tools for improving ongoing monitoring of physical status, psychological burden, and socioeconomic issues during the cancer survivorship trajectory. BackgroundAdherence to oral anticancer treatments is critical in the disease trajectory of patients with breast cancer. Given the impact of nonadherence on clinical outcomes and the associated economic burden for the health care system, finding ways to increase treatment adherence is particularly relevant. ObjectiveThe primary end point is to evaluate the effectiveness of a decision support system (DSS) and a machine learning web application in promoting adherence to oral anticancer treatments among patients with metastatic breast cancer. The secondary end point is to collect a set of new physical, psychological, social, behavioral, and quality of life predictive variables that could be used to refine the preliminary version of the machine learning model to predict patients’ adherence behavior. MethodsThis prospective, randomized controlled study is nested in a large-scale international project named “Enhancing therapy adherence among metastatic breast cancer patients” (Pfizer 65080791), aimed to develop a predictive model of nonadherence and associated DSS and guidelines to foster patients’ engagement and therapy adherence. A web-based DSS named TREAT (treatment adherence support) was developed using a patient-driven approach, with 4 sections, that is, Section A: Metastatic Breast Cancer; Section B: Adherence to Cancer Therapies; Section C: Promoting Adherence; and Section D: My Adherence Diary. Moreover, a machine learning–based web application was developed to predict patients' risk factors of adherence to anticancer treatment, specifically pertaining to physical status and comorbid conditions, as well as short and long-term side effects. Overall, 100 patients consecutively admitted at the European Institute of Oncology (IEO) at the Division of Medical Senology will be enrolled; 50 patients with metastatic breast cancer will be exposed to the DSS and machine learning web application for 3 months (experimental group), and 50 patients will not be exposed to the intervention (control group). Each participant will fill a weekly medication diary and a set of standardized self-reports evaluating psychological and quality of life variables (Adherence Attitude Inventory, Beck Depression Inventory-II, Brief Pain Inventory, 13-item Sense of Coherence scale, Brief Italian version of Cancer Behavior Inventory, European Organization for Research and Treatment of Cancer Quality of Life 23-item Breast Cancer-specific Questionnaire, European Organization for Research and Treatment of Cancer Quality of Life Questionnaire, 8-item Morisky Medication Adherence Scale, State-Trait Anxiety Inventory forms I and II, Big Five Inventory, and visual analogue scales evaluating risk perception). The 3 assessment time points are T0 (baseline), T1 (1 month), T2 (2 months), and T3 (3 months). This study was approved by the IEO ethics committee (R1786/22-IEO 1907). ResultsThe recruitment process started in May 2023 and is expected to conclude on December 2023. ConclusionsThe contribution of machine learning techniques through risk-predictive models integrated into DSS will enable medication adherence by patients with cancer. BackgroundChronic pain (CP) and its management are critical issues in the care pathway of patients with breast cancer. Considering the complexity of CP experience in cancer, the international scientific community has advocated identifying cutting-edge approaches for CP management. Recent advances in the field of health technology enable the adoption of a novel approach to care management by developing integrated ecosystems and mobile health apps. ObjectiveThe primary end point of this pilot study is to evaluate patients’ usability experience at 3 months of a new digital and integrated technological ecosystem, PainRELife, for CP in a sample of patients with breast cancer. The PainRELife ecosystem is composed of 3 main technological assets integrated into a single digital ecosystem: Fast Healthcare Interoperability Resources–based cloud platform (Nu platform) that enables care pathway definition and data collection; a big data infrastructure connected to the Fast Healthcare Interoperability Resources server that analyzes data and implements dynamic dashboards for aggregate data visualization; and an ecosystem of personalized applications for patient-reported outcomes collection, digital delivery of interventions and tailored information, and decision support of patients and caregivers (PainRELife app). MethodsThis is an observational, prospective pilot study. Twenty patients with early breast cancer and chronic pain will be enrolled at the European Institute of Oncology at the Division of Medical Senology and the Division of Pain Therapy and Palliative Care. Each patient will use the PainRELife mobile app for 3 months, during which data extracted from the questionnaires will be sent to the Nu Platform that health care professionals will manage. This pilot study is nested in a large-scale project named “PainRELife,” which aims to develop a cloud technology platform to interoperate with institutional systems and patients' devices to collect integrated health care data. The study received approval from the Ethical Committee of the European Cancer Institute in December 2021 (number R1597/21-IEO 1701). ResultsThe recruitment process started in May 2022 and ended in October 2022. ConclusionsThe new integrated technological ecosystems might be considered an encouraging affordance to enhance a patient-centered approach to managing patients with cancer. This pilot study will inform about which features the health technological ecosystems should have to be used by cancer patients to manage CP. The early detection of head and neck squamous cell carcinoma (HNSCC) is essential to improve patient prognosis and enable organ and function preservation treatments. The objective of this study is to assess the feasibility of using electrical bioimpedance (EBI) sensing technology to detect HNSCC tissue. A prospective study was carried out analyzing tissue from 46 patients undergoing surgery for HNSCC. The goal was the correct identification of pathologic tissue using a novel needle-based EBI sensing device and AI-based classifiers. Considering the data from the overall patient cohort, the system achieved accuracies between 0.67 and 0.93 when tested on tissues from the mucosa, skin, muscle, lymph node, and cartilage. Furthermore, when considering a patient-specific setting, the accuracy range increased to values between 0.82 and 0.95. This indicates that more reliable results may be achieved when considering a tissue-specific and patient-specific tissue assessment approach. Overall, this study shows that EBI sensing may be a reliable technology to distinguish pathologic from healthy tissue in the head and neck region. This observation supports the continuation of this research on the clinical use of EBI-based devices for early detection and margin assessment of HNSCC.

Introduction:Rehospitalization after renal transplantation predicts morbidity and mortality, is associated with higher costs, and indicates health care quality. Identifying risk factors for rehospitalization makes it possible to implement measures that improve the quality of care and develop interventions in the pre-transplant stage. Objectives: Establish the incidence of early rehospitalization in renal transplantation and evaluate variables associated with rehospitalization and its impact on the recipient and graft. Materials and Methods: Observational, longitudinal, retrospective study. We analyzed a cohort of adult kidney transplant patients from 01/01/2006 to 12/31/2018. Results: 196 adult kidney transplant patients were included in the study. The incidence of early rehospitalization was 17.8% (n=35). Patient and graft survival were similar in both groups. The cause of death was predominantly infectious 52% (n=22), while the cause of graft loss was chronic allograft nephropathy 24% (n=14) and death with a functioning graft 48% (n=28). Conclusions: The incidence of early rehospitalization after renal transplantation in our center is low compared to other publications. Our study did not show that readmission after renal transplantation is associated with worse outcomes, Introducción: La rehospitalización después del trasplante renal es un predictor de morbimortalidad, se asocia a mayores costos y es un indicador de calidad de atención de la salud. La identificación de factores de riesgo de rehospitalización permite implementar medidas que mejoren la calidad de atención y desarrollar intervenciones que puedan ser aplicadas en la etapa pretrasplante. Objetivos: Establecer la incidencia de rehospitalización temprana en trasplante renal, evaluar variables asociadas a rehospitalización y su impacto en el receptor e injerto. Materiales y Métodos: Estudio observacional, longitudinal, retrospectivo. Se analizó una cohorte de pacientes trasplantados renales adultos en el período comprendido desde el 01/01/2006 al 31/12/2018. Resultados: Se incluyeron 196 pacientes adultos trasplantados renales en el período de estudio. La incidencia de rehospitalización temprana fue de 17.8% (n=35). La supervivencia del paciente y del injerto fueron similares en ambos grupos. La causa de muerte fue predominantemente infecciosa 52%(n=22), mientras que la causa de pérdida del injerto fue la nefropatía crónica del injerto 24%(n=14) y la muerte con injerto funcionante del 48%(n=28). Conclusiones: La incidencia de rehospitalización temprana postrasplante renal de nuestro centro es baja comparado a otras publicaciones. Nuestro estudio no mostró que la reinternación postrasplante renal se asocie a peores resultados.

- Abstract
Glioblastoma, isocitrate dehydrogenase-wildtype (GB), is the most common and aggressive primary brain malignancy with poor outcome. Immune checkpoint inhibitors (ICIs) have been tested in GB and, despite disappointing results, the identification of a small subgroup of responders underlies the need to improve our understanding of the tumour microenvironment (TME) immunity. This study aimed to determine whether the expression of selected immune checkpoints on tissue-resident memory T cells (Trm) may predict patient outcome. We conducted a single cohort observational study. Tumour samples were collected from 45 patients with histologically confirmed GB (WHO grade 4) and processed to obtain single-cell suspensions. Patients were assessed for the correlation of Trm phenotype with overall survival (OS) or progression-free survival (PFS) using multiparametric flow cytometry and uni/multivariate analyses. Levels of Trm expressing programmed cell death protein 1 (PD1) and T cell immunoglobulin and mucin domain-containing protein 3 (TIM3) were found to be linked to clinical outcome. Low frequency of Trm expressing PD1 or TIM3 or both markers defined subgroups as independent positive prognostic factors for patient survival. On multivariate analysis, low CD8+CD103+PD1+TIM3+ Trm and Karnofsky performance status (KPS) >= 70 were confirmed to be the most predictive independent factors associated with longer OS (hazard ratios-HR [95%CI]: 0.14 [0.04-0.52] p < 0.001, 0.39 [0.16-0.96] p = 0.04, respectively). The CD8+CD103+ Trm subgroups were also age-related predictors for survival in GB.

Decoration of nanoparticles with specific molecules such as antibodies, peptides, and proteins that preserve their biological properties is essential for the recognition and internalization of their specific target cells. Inefficient preparation of such decorated nanoparticles leads to nonspecific interactions diverting them from their desired target. We report a simple two-step procedure for the preparation of biohybrid nanoparticles containing a core of hydrophobic quantum dots coated with a multilayer of human serum albumin. These nanoparticles were prepared by ultra-sonication, crosslinked using glutaraldehyde, and decorated with proteins such as human serum albumin or human transferrin in their native conformations. These nanoparticles were homogeneous in size (20–30 nm), retained the fluorescent properties of quantum dots, and did not show a “corona effect” in the presence of serum. The uptake of transferrin-decorated quantum dot nanoparticles was observed in A549 lung cancer and SH-SY5Y neuroblastoma cells but not in non-cancerous 16HB14o- or retinoic acid dopaminergic neurons differentiated SH-SY5Y cells. Furthermore, digitoxin-loaded transferrin-decorated nanoparticles decreased the number of A549 cells without effect on 16HB14o-. Finally, we analyzed the in vivo uptake of these biohybrids by murine retinal cells, demonstrating their capacity to selectively target and deliver into specific cell types with excellent traceability.

Third-generation tyrosine kinase inhibitors are the first-line gold standard in treating advanced non-small-cell lung cancer bearing common EGFR mutations, but data documenting clinical efficacy in uncommon mutations are currently limited. In this paper, we describe the case of a patient bearing uncommon compound EGFR mutations in exon 20, who experienced a near-complete response to third-line Osimertinib, with metabolic complete response of pulmonary, nodal and ostheolytic lesions. This radiological assessment corresponded to an ECOG PS improvement (from three to one) and a substantial clinical benefit for the patients. Out of two mutations, S768I was associated with poor response to third-generation TKI and V774M had unknown clinical significance, highlighting the complexity of the correct management of these kinds of mutations. We reviewed the literature to document the up-to-date preclinical and clinical data concerning third-generation tyrosine kinase inhibitors for the treatment of patients bearing uncommon EGFR mutations. Informality is a reality for three-quarters of domestic workers in Brazil who, without a formal employment relationship, are not protected by the legal guarantees established since 2013. This lack of legal protection is exacerbated by the emphasis on entrepreneurship promoted by a growing number of digital platforms offering domestic work intermediation. In this article, we examine the structural standards of digital intermediation companies for domestic work in Brazil. Four representative platforms (GetNinjas, Parafuzo, Helpty, and Crafty) were analyzed. Our research corpus consists of their websites and applications, their terms of use, as well as journalistic articles and complaints from workers and clients on the website Reclame Aqui. The results are presented in three parts: (i) the history and general characteristics of each platform; (ii) their content organized into four categories of analysis (marketing, hiring, remuneration, and reputation systems); and (iii) the accounts of clients and workers on the Reclame Aqui website. The study found that digital intermediaries for domestic work present complex challenges for workers, as they amplify existing risks in the dominant labor model, such as intense subordination, economic dependence, lack of transparency about the value of their work, and exclusion from legal labor protection, whose recent expansion has not resulted in effective change. The study also suggests that this intensification of risks increases directly with the degree of intervention by the digital intermediary in the client-worker relationship. [ABSTRACT FROM AUTHOR]
