Abstract

Autistic children and young people (CYP) experience mental health difficulties but face many barriers to accessing and benefiting from mental health care. There is a need to explore strategies in mental health care for autistic CYP to guide clinical practice and future research and support their mental health needs. Our aim was to identify strategies used to improve mental health care for autistic CYP and examine evidence on their acceptability, feasibility, and effectiveness. A systematic review and meta-analysis were carried out. All study designs reporting acceptability/feasibility outcomes and empirical quantitative studies reporting effectiveness outcomes for strategies tested within mental health care were eligible. We conducted a narrative synthesis and separate meta-analyses by informant (self, parent, and clinician). Fifty-seven papers were included, with most investigating cognitive behavioral therapy (CBT)-based interventions for anxiety and several exploring service-level strategies, such as autism screening tools, clinician training, and adaptations regarding organization of services. Most papers described caregiver involvement in therapy and reported adaptations to communication and intervention content; a few reported environmental adjustments. In the meta-analyses, parent- and clinician-reported outcomes, but not self-reported outcomes, showed with moderate certainty that CBT for anxiety was an effective treatment compared to any comparison condition in reducing anxiety symptoms in autistic individuals. The certainty of evidence for effectiveness, synthesized narratively, ranged from low to moderate. Evidence for feasibility and acceptability tended to be positive. Many identified strategies are simple, reasonable adjustments that can be implemented in services to enhance mental health care for autistic individuals. Notable research gaps persist, however. Abstract

Objective: To systematically review the literature on clinicians' and patients' experiences of supported mealtimes in the treatment of anorexia nervosa. Method: This systematic review was conducted in accordance with PRISMA guidelines and registered on PROSPERO (CRD42022372565). PsycINFO, MEDLINE and Embase were searched up to the 20th of November 2023 for qualitative articles investigating the perspectives of healthcare professionals and patients on clinician‐supported meals across all clinical settings. Data were analysed using thematic synthesis. The Critical Appraisal Skills Programme was used to evaluate the quality of selected studies. Results: This review comprised of 26 studies; eight concerned with the perspectives of clinicians only, 16 addressing patients' views, and two studies exploring the views of both groups. Experiences of both groups were generally negative, and three overlapping themes were identified: lack of consistency in care provided, high levels of negative emotions and an uncomfortable power dynamic. Conclusions: This review suggests that supported mealtimes are experienced more positively by patients when rules are clear and consistently enforced, and when clinicians make informal conversation and supportive comments. Our findings highlight the need for best practice guidelines and clinician training to improve the delivery of supported mealtimes. Such guidelines and training should be coproduced in collaboration with patients and carers. Highlights: Supported mealtimes are a core aspect of anorexia nervosa treatment, however, there are currently no guidelines or evidence‐based protocols for how this intervention should be conducted.This review revealed that both patients and clinicians consider supported mealtimes an invaluable part of treatment, identifying several positive aspects of the intervention, such as valuable emotional support, strict rules and increasing responsibility‐taking.This review also highlights negative experiences of supported mealtimes for both clinicians and patients due to high levels of adverse emotions, inconsistency in the delivery of care and a power dynamic akin to "policing". Abstract

Safewards reduces conflict and containment on adult inpatient wards but there is limited research exploring the model in Children and Young People (CYP) mental health services. We investigated whether Safewards can be successfully implemented on twenty CYP wards across England. A process and outcomes evaluation was employed, utilizing the Integrated Promoting Action on Research Implementation in Health Sciences (i-PARiHS) framework. Existing knowledge and use of Safewards was recorded via a self-report benchmarking exercise, verified during visits. Implementation of the 10 Safewards components on each ward was recorded using the Safewards Organizational Fidelity measure. Data from 11 surveys and 17 interviews with ward staff and four interviews with project workers were subject to thematic analysis and mapped against the four i-PARiHS constructs. Twelve of the 20 wards implemented at least half of the Safewards interventions in 12 months, with two wards delivering all 10 interventions. Facilitators and barriers are described. Results demonstrated Safewards is acceptable to a range of CYP services. Whilst implementation was hindered by difficulties outlined, wards with capacity were able and willing to implement the interventions. Results support the commissioning of a study to evaluate the implementation and outcomes of Safewards in CYP units. [ABSTRACT FROM AUTHOR]

21. Understanding eating disorders: Exploring the impact of self-esteem, emotional intelligence and health literacy on disordered eating attitudes and behaviours

The aim of this research is to explore the factors that are involved in the development and maintenance of disordered eating attitudes and behaviours. While major strides in research have provided an understanding into the complexity of these disorders, considerable gaps remain with little evidence existing for the effective prevention and treatment of these disorders. The theories of self-esteem, emotional intelligence and health literacy are considered as central themes to understanding the onset and maintenance of these disorders. This study aims to explore these onset and maintenance variables using a mixed methods sequential design. Phase one used a qualitative design conducting a series of semi-structured interviews, focus groups and personal narrative to explore the experiences of individuals with personal or professional experience of eating disorders (n=32). Phase two used a quantitative design to statistically test and model the findings within a large general sample (n=435). The results from this study provide an in-depth qualitative and quantitative exploration of the variables which motivate and maintain disordered eating. These results provide insights into the unique barriers and risk factors which are associated with disordered eating. The results indicate these three variables have considerable impact on the onset and maintenance of disordered eating, with interactions occurring between these variables, suggesting unique pathways towards clinical risk of eating disorder development. This indicates the need to develop holistic interventions that help target programs towards the barriers and factors which maintain an eating disorder. This study provides a unique contribution to the field of eating disorders as it offers new insights into the impact of low self-esteem, emotional intelligence and health literacy as key variables within disordered eating onset and maintenance. It is envisaged that the findings of this study can be used to inform and further develop intervention areas to promote positive, and timelier, interventions.

Abstract

This study explores the perceptions and experiences of providers of eating disorder services towards men to understand the challenges and barriers experienced by both professionals and patients in addressing the gender bias within this field. A total of 21 professionals took part in the study. Using interviews and focus groups, the study explored service providers' experience, understanding and knowledge in working with men with eating disorders using thematic analysis. Findings suggest that the growing issue of eating disorders affecting men, while a serious concern for practitioners, remains a 'touchy subject', a difficult and sometimes even uncomfortable topic for service providers to tackle. These findings provide insight into the challenges and barriers faced by professionals and men seeking help or treatment for an eating disorder and highlights the need for targeted training and resources to improve the eating disorders health literacy of service providers in relation to men's experiences. Abstract

Given the scope for improvement in guidance and the lack of appropriate incentives, we have every reason to suspect there will be issues in MAs investigating treatments for ADHD. Alternatively, given the risk of harm of misleading or unreliable MA findings, one might assume that MAs are properly reported and are appreciably reproducible. To our knowledge, neither conclusion has been systematically tested. Thus, in this project, we aim to conduct a feasibility study assessing adherence to PRISMA reporting guidelines and the reproducibility of results of MAs that investigate treatments for ADHD.

Abstract

Purpose The COVID-19 pandemic has many potential impacts on people with mental health conditions and on mental health care, including direct consequences of infection, effects of infection control measures and subsequent societal changes. We aimed to map early impacts of the pandemic on people with pre-existing mental health conditions and services they use, and to identify individual and service-level strategies adopted to manage these. Methods We searched for relevant material in the public domain published before 30 April 2020, including papers in scientific and professional journals, published first person accounts, media articles, and publications by governments, charities and professional associations. Search languages were English, French, German, Italian, Spanish, and Mandarin Chinese. Relevant content was retrieved and summarised via a rapid qualitative framework synthesis approach. Results We found 872 eligible sources from 28 countries. Most documented observations and experiences rather than reporting research data. We found many reports of deteriorations in symptoms, and of impacts of loneliness and social isolation and of lack of access to services and resources, but sometimes also of resilience, effective self-management and peer support. Immediate service challenges related to controlling infection, especially in inpatient and residential settings, and establishing remote working, especially in the community. We summarise reports of swiftly implemented adaptations and innovations, but also of pressing ethical challenges and concerns for the future. Conclusion Our analysis captures the range of stakeholder perspectives and experiences publicly reported in the early stages of the COVID-19 pandemic in several countries. We identify potential foci for service planning and research.

Abstract

Background All NHS providers collect data on patient experience, although there is limited evidence about what to measure or how to collect and use data to improve services. We studied inpatient mental health services, as these are important, costly and often unpopular services within which serious incidents occur. Aims To identify which approaches to collecting and using patient experience data are most useful for supporting improvements in inpatient mental health care. Design The study comprised five work packages: a systematic review to identify evidence-based patient experience themes relevant to inpatient mental health care (work package 1); a survey of patient experience leads in NHS mental health trusts in England to describe current approaches to collecting and using patient experience data in inpatient mental health services, and to populate the sampling frame for work package 3 (work package 2); in-depth case studies at sites selected using the work package 2 findings, analysed using a realist approach (work package 3); a consensus conference to agree on recommendations about best practice (work package 4); and health economic modelling to estimate resource requirements and potential benefits arising from the adoption of best practice (work package 5). Using a realist methodology, we analysed and presented our findings using a framework based on four stages of the patient experience data pathway, for which we coined the term CRAICh (collecting and giving, receiving and listening, analysing, and quality improvement and change). The project was supported by a patient and public involvement team that contributed to work package 1 and the development of programme theories (work package 3). Two employed survivor researchers worked on work packages 2, 3 and 4. Setting The study was conducted in 57 NHS providers of inpatient mental health care in England.
