6. The Prognostic Importance of Patient Pre-Operative Expectations of Surgery for Lumbar Spinal Stenosis.

Abstract

Examines patients (N=257) with lumbar spinal stenosis preoperatively and at six months to relate patient expectation to baseline function and pain and to determine how patient expectations and preoperative function interact to predict postoperative outcomes. Results show that patients with many preoperative expectations, particularly those with low baseline function, reported more improvement in postoperative function than patients with few expectations. Abstract

An educational program that teaches workers ways to avoid back injury does not appear to be beneficial. Physical therapists taught 2,534 postal workers and 134 supervisors the principles of back safety, including proper ways of lifting and handling, posture, exercise and pain management. Although the program increased workers' knowledge of back safety, it did not reduce the number of back injuries, time lost from work, or costs associated with back injuries when their outcome was compared to a group that did not receive the training.

Abstract

Worker's compensation does not appear to influence reporting of symptoms or functional status among carpal tunnel patients participating in a research study. Objectively measurable parameters have been shown to differ from patient self-report of function and symptoms. Studies, therefore, need to measure both. However, it is believed that patients receiving worker's compensation may be inclined to overreport disability. Of 268 study participants with carpal tunnel syndrome, 155 were receiving worker's compensation. Self-report of function, symptoms, and satisfaction did not differ between the two groups. The correlation between self-report of weakness and measurement of grip strength was poor but did not differ between groups. The perceived improvement six months later was greater in the group receiving worker's compensation. At least in the research setting, where there are no financial incentives, self-report is a reliable, useful measure.

Abstract

Background and Purpose. When factors that influence exercise behavior are known, health care professionals can more likely design and modify patient education materials targeted to promote exercise behavior. This study aimed to identify predictors of exercise behavior in patients with rheumatoid arthritis 6 months after a visit with their rheumatologist. Subjects and Methods. Twenty-five rheumatologists and 132 patients with rheumatoid arthritis participated. One hundred thirteen patients (85.6%) completed the 6-month follow-up. Rheumatologists and patients completed baseline questionnaires and were audiotaped during a subsequent visit. Physical function and exercise behavior were ascertained via questionnaire 6 months following the visit. Multivariate logistic regression identified predictors of exercise behavior at 6 months. Eighty-nine patients (79%) were female. The average age was 54.8 years (SD=14.4, range=20-94). 33. A controlled evaluation of continuous passive motion in patients undergoing total knee arthroplasty

Abstract

Continuous passive motion (CPM) may be more effective and less expensive than traditional physical therapy for rehabilitation of patients who have undergone total knee arthroplasty, or knee replacement surgery. This method involves the use of a CPM machine set for a certain rate and arc of motion to exercise the patient's knee. Among 93 patients with arthritis who underwent total knee arthroplasty, 48 were treated with CPM plus conventional physical therapy and 45 were treated with conventional physical therapy alone. Patients in the CPM group had increased active range of motion, less swelling and less need for manipulations, compared with those in the conventional physical therapy group. Patients treated with CPM did not experience significantly less pain or fewer complications than those who were not treated with CPM. Costs associated with treatment were less for patients in the CPM group than for those in the conventional physical therapy group., Objective.--To evaluate the efficacy of continuous passive motion (CPM) in the postoperative management of patients undergoing total knee arthroplasty. Design.--A randomized controlled single-blind trial of CPM plus standardized rehabilitation vs standard rehabilitation alone. Setting.--A referral hospital for arthritis and musculoskeletal care. Patients.--Consecutive patients with end-stage osteoarthritis or rheumatoid arthritis undergoing primary total knee arthroplasty who had at least 90[degree] of passive knee f exion. One hundred fifty-four patients were eligible and 102 patients agreed to participate and were randomized. Ninety-three patients completed the study protocol. Intervention.--Continuous passive motion machines programmed for rate and specified arc of motion within 24 hours of surgery with range increased daily as tolerated with standardized rehabilitation program compared with standardized rehabilitation program alone. Main Outcome Measures.--Primary outcomes were pain, active and passive knee range of motion, swelling (or circumference), quadriceps strength at postoperative day 7, as well as complications, length of stay, and active and passive range of mot on and function at 6 weeks. Results.--Use of CPM increased active flexion and decreased swelling and the need for manipulations but did not significantly affect pain, active and passive extens on, quadriceps strength or length of hospital stay. At 6 weeks there were no differences between the two groups in either range of motion or function. In this series, use of CPM resulted in a net savings of $6764 over conventional rehabilitation in achieving these results. Conclusion.--For the average patient undergoing total knee arthroplasty, CPM is more effective in improving range of motion, decreasing swelling, and reducing the need for manipulation than is conventional therapy and lowers cost. Abstract

Abstract Background Studies on critical pathway use have demonstrated decreased length of stay and cost without compromise in quality of care. However, pathway effectiveness is difficult to determine given methodological flaws, such as small or single center cohorts. We studied the effect of critical pathways on total knee replacement outcomes in a large population-based study. Methods We identified hospitals in four US states that performed total knee replacements. We sent a questionnaire to surgical administrators in these hospitals including items about critical pathway use and hospital characteristics potentially related to outcomes. Patient data were obtained from Medicare claims, including demographics, comorbidities, 90-day postoperative complications and length of hospital stay. The principal outcome measure was the risk of having one or more postoperative complications. Results Two hundred ninety five hospitals (73%) responded to the questionnaire, with 201 reporting the use of critical pathways. 9,157 Medicare beneficiaries underwent TKR in these hospitals with a mean age of 74 years (± 5.8). After adjusting for both patient and hospital related variables, patients in hospitals with pathways were 32% less likely to have a postoperative complication compared to patients in hospitals without pathways (OR 0.68, 95% CI 0.50-0.92). Patients managed on a critical pathway had an average length of stay 0.5 days (95% CI 0.3-0.6) shorter than patients not managed on a pathway. Conclusion Medicare patients undergoing total knee replacement surgery in hospitals that used critical pathways had fewer postoperative complications than patients in hospitals without pathways, even after adjusting for patient and hospital related factors. This study has helped to establish that critical pathway use is associated with lower rates of postoperative mortality and complications following total knee replacement after adjusting for measured variables.
