2. The Quality of Informed Consent Forms--a Systematic Review and Critical Analysis.

Background: The patient's consent to a medical procedure must be preceded by a pre-procedure discussion with the physician that is documented on a standardized form. Evidence suggests that these forms lack information that would be relevant for an informed decision. Methods: We carried out a systematic literature search up to February 2017 for evidence on the quality and efficacy of informed consent forms. The definition of criteria for the evaluation of meta-information, content, and presentation were derived from current guidelines for evidence-based health information. As an example, we analyzed consent forms currently in use in Germany for 10 medical interventions with regard to decisionally relevant content and intelligibility of format. Results: Our literature search yielded 14 content analyses, which revealed that even some of the more important evaluative criteria were not always met, including information on benefits (9/14), risks (14/14), alternatives (11/14), the option of doing nothing (6/14), and numerical frequencies (2/14). All analyses indicated deficiencies in the content of the consent forms. We then analyzed 37 consent forms obtained from publishing companies (across Germany) and physician's practices in Hamburg. These forms were found to contain information on: the intervention (37/37), benefits (30/37), risks (37/37), alternatives (26/37), the option of doing nothing (4/37), numerical frequencies (10/37), the names of the authors (17/37), sources of information (0/37), and date of issue (21/37). Conclusion: Both the evidence from foreign countries and our own analysis of the consent forms now in use in Germany revealed deficiencies, particularly in the communication of risks. New standards are needed to promote well-informed decision-making. Structural changes in the process of patient information and decision-making should be discussed. [ABSTRACT FROM AUTHOR]

Most organizations use business forms to conduct daily operations and to communicate internally and externally with customers, suppliers, government agencies, and other entities. Traditionally, paper forms have been used and stored with obvious disadvantages of high retrieval and storage cost. With advances in computing technology electronic forms have begun to supplant their paper counterparts. A number of form management systems have been developed. Typically, these systems are closely integrated with a database management system because of the need to retain contents of forms over time. In the context of this emphasis on forms, a research has been conducted on how to use forms in the database analysis and design process. This research was sparked by two observations, because of familiarity, end users can effectively communicate many requirements through forms they use and usually the most widely used data are gathered or reported in a form. Thus, forms provide an important input source for database design.

Purpose – The purpose of this paper is to convey how the transition to market-based orientations by state-owned enterprises (SOEs), particularly the military sector, represents a coevolutionary process between business and regulatory institutions that has an impact on both the military and civilian markets. Design/methodology/approach – The authors conducted a longitudinal case study of a military SOE, the Aosheng Group, between 1951 and 2012 to understand the dynamics between institutions and organizations. A comparative analysis between the main stages of evolution was completed, and conclusions about the main patterns of organizational and institutional change were reached. Findings – The study reports evidence on the coevolutionary nature of change in big SOEs in China, demonstrating how institutional changes are bigger drivers in promoting reorientations than are market pressures. Within the framework of punctuated equilibrium theory, the determining role that managers may play in leading and implementing organizational reorientations is emphasized. Research limitations/implications – A triangulated methodology was employed to analyse a long period; however, its application to just a single case might be questioned in terms of generalizing any of the findings. Originality/value – The longitudinal perspective applied in this case study contributes to critical questioning as to how Chinese agencies define forms of control and the goals for SOEs under their jurisdiction and the importance of allowing managerial discretion to the assigned managers. No document standard is sufficient for all purposes because the requirements significantly differ among businesses, industries, and geopolitical regions. On the other hand, the ultimate aim of business document interoperability is to exchange business data among partners without any prior agreements related to the document syntax and semantics. Therefore, an important characteristic of a document standard is its ability to adapt to different contexts, its extensibility, and its customization. The UN/CEFACT Core Component Technical Specification (CCTS) is an important landmark in this direction. In this article, we present a survey and an analysis of some of the prominent UN/CEFACT CCTS-based electronic document standards. We describe their document design principles and discuss how they handle customization and extensibility. We address their industry relevance and the recent efforts for their harmonization and convergence. Abstract: In this study, a simple but important user interface design choice is examined: when marking required-fields in online forms, should GUI designers stick with the often used asterisk that many form design guidelines cite as the de-facto web standard, or should they choose a colored background as a new design solution to visually signal which input fields are required? An experiment with 24 participants was conducted to test the hypotheses that efficiency, effectiveness and satisfaction ratings of colored required-fields exceed those of asterisk-marked required-fields. The notion of forms as a way of organizing and presenting data has been used since the beginning of the World Wide Web. Web-based forms have evolved together with the development of new markup languages, in which it is possible to provide validation scripts as part of the form code to test whether the intended meaning of the form is correct. However, for the form designer, part of this intended meaning frequently involves other features which are not constraints by themselves, but rather attributes emerging from the form, which provide plausible conclusions in the context of incomplete and potentially inconsistent information. As the value of such attributes may change in presence of new knowledge, we call them defeasible attributes. In this paper, we propose extending traditional web-based forms to incorporate defeasible attributes as part of the knowledge that can be encoded by the form designer. The proposed extension allows the specification of scripts for reasoning about form fields using a defeasible knowledge base, expressed in terms of a Defeasible Logic Program. [ABSTRACT FROM AUTHOR]

Background: Chart abstraction is a common method for measuring the quality of surgical care. In this study we examine how the use of standardized operative dictation and history forms improves documentation rates of bariatric quality measures. Materials and methods: Two independent reviewers evaluated 201 patient charts from two multi-surgeon bariatric surgery practices for documentation of five intraoperative and seven preoperative bariatric quality measures. Group 1 used fully standardized templates to dictate or collect both, while Group 2 did not. Documentation rates were compared between the groups. Results: Operative reports more consistently documented quality assessment information for cases where a dictation template was used versus where it was not (89% versus 58%, respectively, P < 0.001). The greatest discrepancies between the two groups were found in “exploration of the abdomen” (95% in Group 1 versus 43% in Group 2, P < 0.001) and in “evaluation of the gallbladder” (76% versus 28%, P < 0.001). In comparison, overall documentation rates for preoperative comorbidities were greater in both groups but remained higher for Group 1, who used fully standardized forms (98% versus 74%, P < 0.001). Group 1 had statistically significant higher rates of documentation for all seven comorbidities. Conclusions: The use of standardized dictation templates and history forms is associated with significantly higher documentation rates of quality measures in bariatric surgery. The adoption of these methods into routine use will be needed to allow for wide scale quality assessment and improvement for surgical practices. [Copyright &y& Elsevier]

form model and an expert database design system that analyzes instances of the form model to derive a conceptual schema are proposed. The form model describes the properties of form fields such as their origin, hierarchical structure, and cardinality. The expert database design system creates a conceptual schema by incrementally integrating related collections of forms. The rules of the expert system are divided into six phases: 1) form selection, 2) entity identification, 3) attribute attachment, 4) relationship identification, 5) cardinality identification, and 6) integrity constraints. The rules of the first phase use knowledge about the form flow to determine, the order in which forms are analyzed. The rules in other phases are used in conjunction with a designer dialog to identify the entities, relationships, and attributes of a schema that represents the collection of forms. The article describes an instructional exercise that uses a university's form titled, "Request for Complimentary Parking Permits," as a tool to help students explore the concept of effective control mechanisms. Each student in an introductory management course regarding the form for parking permits is asked what is wrong with the request form. Students found little fault with the form initially. When pressed to be more critical, they began to uncover faults including the disapproval signatures, justification for the request of waived fees, and the number of persons for event and event portions of the form.

Too often, systematic recording procedures are not developed which describe and assess the delivery of social work — and, more specifically, case management — services. The authors report on the Service Tracking and Referral Form developed by California's Adolescent Family Life Evaluation Project to document the receipt of services by pregnant adolescent clients as a result of case management intervention. The rationale and process underlying the development and use of the project's Service Tracking and Referral Form are discussed. Data are presented to illustrate the kinds of information which can be generated from the form.
