Abstract

BACKGROUND: The SAfety and FEasibility of standard EVAR outside the instruction for use (SAFE-EVAR) Study was designed to define the attitude of Italian vascular surgeons towards the use of standard endovascular repair (EVAR) for infrarenal abdominal aortic aneurysm (AAA) outside the instruction for use (IFU) through a Delphi consensus endorsed by the Italian Society of Vascular and Endovascular Surgery (Società Italiana di Chirurgia Vascolare ed Endovascolare - SI CVE). METHODS : A questionnaire consisting of 26 statements was developed, validated by an 18-member Advisory Board, and then sent to 600 Italian vascular surgeons. The Delphi process was structured in three subsequent rounds which took place between April and June 2023. In the first two rounds, respondents could indicate one of the following five degrees of agreement: 1) strongly agree; 2) partially agree; 3) neither agree nor disagree; 4) partially disagree; 5) strongly disagree; while in the third round only three different choices were proposed: 1) agree; 2) neither agree nor disagree; 3) disagree. We considered the consensus reached when ≥70% of respondents agreed on one of the options. After the conclusion of each round, a report describing the percentage distribution of the answers was sent to all the participants. RESULTS: Two-hundred-forty-four (40.6%) Italian Vascular Surgeons agreed to participate the first round of the Delphi Consensus; the second and the third rounds of the Delphi collected 230 responders (94.3% of the first-round responders). Four statements (15.4%) reached a consensus in the first rounds. Among the 22 remaining statements, one more consensus (3.8%) was achieved in the second round. Finally, seven more statements (26.9%) reached a consensus in the simplified last round. Globally, a consensus was reached for almost half of the proposed statements (46.1%). Abstract

Peer review can be used as a teaching methodology to improve students’ learning and critical thinking. However teachers have many concerns about the reliability and validity of students’ grading. The paper describes the application of peer review as a teaching strategy to the large course of Biomedical Informatics in the School of Medicine at the University of Florence. The aim of the study was twofold: (I) assessing the validity of students’ reviews, calculating the correlation between students’ assigned score and instructor’s assigned score; (II) assessing the validity of student’s self-evaluation, calculating the correlation between student’s assigned score and teacher’s assigned score. To this aim a statistical analysis was performed. The results showed a moderate concordance between the marks assigned by peers and those assigned by the instructor. Neverthless the comparison between the teacher median and the peer-review median shows a minimal difference that has almost no effect on changing the final grade. Instead, there was poor concordance between the marks attributed by the instructor and those relating to the student’s self-evaluation.. Even if further studies are needed, the promising results can begin to dispel teachers’ concerns about students’ grading skills that prevent the application of peer review. On such basis, the use of peer review systems can streamline the application of peer.review in classes with a high number of students reducing the workload on the teacher.

The article describes the application of the teaching strategy based on the peer-review process in the course of Computer Science with a high number of students in the Degree Course of Medicine at the University of Florence. Learners were asked to produce a paper by choosing from a list of topics about Biomedical Informatics. Subsequently, each learner reviewed and analyzed the papers produced by three peers and, applying the same criteria, assessed their own. At the end, an anonymous satisfaction questionnaire about peer-review was administered. In addition to the learning results related to the course, the responses to the questionnaire were analyzed. The peer-review was appreciated by the students so much that they hoped for its replication in other courses of the same Degree Course. The most valuable papers were selected by the teachers to be collected in a volume which was used as reference book in the following academic years. L’articolo descrive l’applicazione della metodologia didattica basata sul processo di peer-review nell’insegnamento di Informatica ad elevata numerosità di studenti del Corso di Laurea in Medicina e Chirurgia presso l’Università di Firenze. Ai discenti è stato richiesto di produrre un elaborato scegliendo tra un elenco di argomenti di Informatica Biomedica. Successivamente ciascun discente ha revisionato e analizzato gli elaborati prodotti da tre pari e, applicando gli stessi criteri, valutato il proprio. Al termine delle attività è stato somministrato un questionario anonimo di gradimento relativo all’attività di peer-review. Oltre ai risultati di apprendimento relativi al corso sono state analizzate le risposte ai questionari di gradimento, da cui è emerso come la metodologia didattica peer-review sia stata apprezzata dagli studenti tanto da auspicarne la replicazione in altri insegnamenti del medesimo Corso di Laurea. Gli elaborati di maggiore pregio sono stati selezionati dagli Autori per essere raccolti in un volume che è stato utilizzato come testo di riferimento negli anni accademici successivi.

Abstract

This article introduces the course “Digital competences at school”, held at the University of Florence in the academic year 2016-2017 as a case of post-graduate training. The course was mainly delivered online and was based on a gradual collaboration process including project-based work. Participants were about thirty. They were mostly teachers showing very different levels of engagement, which led rethinking the initial methodological model. In particular, the collaborative aspect, which was rather demanding in terms of time and commitment, was redesigned in more sustainable forms, enhancing the idea of a shared teaching presence. L’esperienza del perfezionamento “Le competenze digitali nella scuola” Il contributo presenta come caso di formazione post-lauream il corso di perfezionamento “Le competenze digitali nella scuola”, realizzato presso l’Università degli Studi di Firenze nell’anno accademico 2016-2017. Il corso era erogato prevalentemente online ed era improntato a una progressiva collaborazione in ottica di project-based work. I corsisti sono stati circa una trentina, per lo più provenienti dal mondo della scuola, ed hanno avuto modalità di partecipazione molto differenziate, che hanno portato a rivisitare il modello metodologico iniziale. In particolare, l’aspetto collaborativo, risultato piuttosto oneroso in termini di tempo ed impegno, è stato ripensato in forme più sostenibili, valorizzando l’idea di una teaching presence condivisa.

Abstract

The residential sector is characterized by new digital challenges. The Internet of Things (IoT) is a key-driver of innovation and operations management. This study aims to measure and assess IoT devices at the level of individuals, which are households, in European countries. For this scope, through the multi-criteria decision analysis (MCDA), we analyse data from Eurostat providing a mix of indicators allowing information to be aggregated at the level of individual Europeans and disaggregated by age group. The results highlight that only four countries (Netherlands, Denmark, Sweden and Malta) are classified as a high cluster in the examined scenarios. The 16–24 age group is the most involved in the uses of IoT devices, but the previous three northern European countries also show very high values for the 35–44 age group. IoT devices serve as a springboard for achieving a powerful propulsion toward technological innovation in the new business models, identifying opportunities and being a way to make many routine tasks more agile. Training programs and awareness campaigns are policy suggestions for the development of IoT devices favouring a cultural change on their use. However, there is an emerging need for studies that monitor environmental health impacts to prevent possible threats.

Un esempio paradigmatico di cooperazione tra sanità civile e militare è stato realizzato tra Guardia di Finanza e ARES 118, realtà di riferimento nazionale nella organizzazione dei servizi sanitari di emergenza territoriale, per il trasporto aereo internazionale di pazienti critici provenienti dall’Ucraina destinati verso ospedali italiani. L’apporto sanitario militare si è rivelato fondamentale nel conciliare gli aspetti di pertinenza clinica e medico legale con le esigenze relative al trasporto aereo, in termini di risorse umane impiegate, assetto ed equipaggiamento dei mezzi utilizzati. La strategia di supporto sanitario implementata ha consentito di definire un protocollo valido quale best practice per la standardizzazione di procedure sinergiche civili­militari nel campo del soccorso e del trasporto sanitario internazionale, anche nel particolare contesto emergenziale bellico. Abstract

Thermal ablation techniques are procedures of growing interest for management of bone metastases. Among these, cryoablation is probably the most advanced. It allows treatment of large and irregular volumes of pathological tissue, real-time evaluation of the area of ablation and appears less painful than heat-based ablative techniques like radiofrequency and microwaves. Literature shows the effectiveness of cryoablation in the management of bone metastases in terms of pain palliation, but also its employment with curative intent is recommended. We reviewed the outcomes of cryoablation procedures performed in our radiology department over the last seven years, confirming the results in terms of pain palliation and local control of disease. We retrospectively evaluated results of 28 procedures of cryoablation, of which 17 treated with palliative and 11 with curative intent. In a 3-month follow-up study, we recorded an overall reduction of pain (evaluated using a VAS 0-10 scale) between pre- and post-treatment. The mean values dropped from 6.9 (SD: ± 1.3) to 3.5 (SD ± 2.6) (p 0.0001). In the group of patients treated for local tumor control (follow-up: 22.4 months), we recorded a stability and/or reduction in volume of the lesion in 10 out 11 patients. No major complications were recorded.

L'articolo prende le mosse dalla distinzione proposta da Lawrence Lessig fra Internet e ciberspazio, dove quest'ultimo viene descritto come luogo di partecipazione e produzione di significato, ben oltre le tipiche funzionalità di Internet, riconducibili alla ricerca di risorse e alla fruizione di servizi di vario genere. Si riportano esempi che illustrano il valore aggiunto che può emergere dall'aggregazione nel ciberspazio, in contesti attinenti alla formazione scolastica o universitaria: due riguardano la formazione di un network spontaneo, di insegnanti in un caso e di studenti in un altro, e due l'applicazione di strumenti di blogging e microblogging nelle attività di classi universitarie. Infine, si discute il ruolo che le tecnologie web possono avere nella sperimentazione di nuove pratiche didattiche, in relazione all'impatto che possono avere sull'apprendimento.

Si descrivono alcuni aspetti tecnici relativi al metodo di insegnamento della blogoclasse. Con questo metodo si rinuncia al modello di insegnamento convenzionale basato su relazioni insegnante-studenti unidirezionali, caratterizzato da uno scarso grado di personalizzazione, centrato sulla figura dell’insegnante e dove gli studenti giocano un ruolo sostanzialmente passivo. In sintesi, la blogoclasse è una comunità di pratica e come tale essa richiede delle attenzioni particolari. Dal punto di vista tecnico, la blogoclasse si realizza mediante l’impiego di blog, feed syndication e qualche altro strumento web 2.0. Tutti gli studenti e il medesimo insegnante mantengono un blog che devono utilizzare nello svolgimento del corso. Poiché la blogoclasse è una cosa viva, è molto importante concepirla e osservarla come un tutto, in modo da farla crescere adeguatamente, concentrandosi in particolare sulle relazioni fra studenti e sulla sua natura di rete. In questo lavoro si discutono alcuni aspetti tecnici relativi al tracciamento delle attività e ai processi di rete che hanno luogo nella blogoclasse.

Abstract

Congenital pseudoarthrosis of the tibia (CPT) is a rare disease characterised by the onset of bone anomalies or fractures, leading to deformities in paediatric patients. The aetiology of this pathology is unknown. The main theories include the presence of hamartomatous tissue related to Neurofibromatosis type 1, vascularisation deficit of the periosteum and alterations in the numbers and functions of the osteoblasts and osteoclasts in loco. Surgical treatment generally requires multiple operations during the patient's childhood and adolescence. The best outcomes seem to occur when using intramedullary nailing, vascularised fibular transplant and external fixation with the Ilizarov technique. The purpose of this paper is to evaluate the effectiveness of in-situ injections of Bone Marrow Aspirate Concentrate (BMAC) as an adjuvant therapy for congenital pseudoarthrosis of the tibia in patients treated with external fixation and that of radiographic healing over time compared to external fixation treatment alone. We performed a retrospective review of clinical and radiographic records of patients affected by CPT and treated in the Paediatric Orthopaedics and Traumatology Department of the Gaetano Pini Orthopaedic Institute with in-situ injections of bone marrow aspirate concentrate (BMAC) on the pseudoarthrosis site, in addition to pseudoarthrosis site excision and application of circular external fixator frame in compression (Group A). The time needed to reach the radiological consolidation of the resection site was recorded and compared to that needed for patients treated with only pseudoarthrosis site excision and application of circular external fixator frame in compression (Group B). There is a statistically relevant improvement of healing time in patients affected by congenital pseudoarthrosis of the tibia treated with external fixation and bone marrow aspirate concentrate compared to patients affected by the same pathology treated with external fixation only. Injection of MSC in the pseudoarthrosis site after focus removal in combination with circular external fixation achieves faster bone healing compared with external fixation only, and the lower refracture percentage may be associated with the better quality and structure of the new bone. However, it would be desirable to have a longer followup to determine if the results of the BMC as adjuvant therapy will hold up over time.

Abstract

In Fall 2007 David Wiley, professor at Utah State University held a course about Open Education. That time, however, Dr Wiley’s course was followed by a rather unusual group of students. The Fall 2007 edition, in fact, was available to anybody, free of charge, all over the world. The only requisite required was the possession of a blog for the completion of the weekly assignments. The present paper, whose authors attended the course completing it successfully, is an account of the experience they had. It can be considered an innovating experience from many different viewpoints and can be regarded as an example of how the world of the formal education can meet the demands of the informal one, in the broader landscape of professional training and lifelong learning.

Abstract

We present here a direct least-squares estimation (DLSE) method for the determination of renal kinetic parameters from sequences of very fast acquisitions performed with a three-headed single photon emission computed tomography (SPECT) system. A simple linear model for the behavior of the radiopharmaceutical, as well as a spatial model for its spatial distribution are defined. The model enables one to estimate the kinetic parameters directly from the projections, once the plasma concentration function is known. A new technique for the accurate reconstruction of time-radioactivity curves based on the direct reconstruction of the region-of-interest contents from a series of data from three-projections is presented. The technique is used to determine the plasma concentration function with a sub-second time resolution. The spatially-variant geometrical response is also included in the model to compensate for the spatial resolution of the SPECT system. Results obtained from simulations are presented. Basic spatial and time features of the simulations are derived from a patient study. Noise and segmentation errors are also simulated. The DLSE method is compared with the conventional one of deriving kinetic parameters from the time series of reconstructed images. The standard deviation of results given by DLSE is less than 2%, whereas with the conventional method it is between 5% and 6%. Within the limit of statistical fluctuations, DLSE results are unbiased whereas those of the conventional method are overestimated by 24%. Recently, a general theoretical description of multi-hole collimators has been proposed by one of the authors. In this work, the theory was used to study the artifacts, which are caused by the hole-array pattern of high-energy collimators. The relevant outcomes of the study were that the response to a sheet source does not depend on its distance from the collimator, whereas the response depends on the gap between the collimator and the image plane that is located in the scintillation crystal; the dependence of hole-array artifacts on this gap was assessed and the existence of optimal values for which they are minimized was found. A large field of view camera, equipped with a high-energy general purpose collimator for [sup.131]I with hexagonal holes placed in an hexagonal lattice, was used. A series of images of a flood source placed on the external face of the collimator was acquired. Each image was acquired with a different value of the crystal-collimator distance. The analysis of the images clearly showed the existence of optimal crystal-collimator distances to reduce the hole-array artifacts. These distances have been found in agreement with the values predicted by the theory. This work may be useful for the design of new collimators, for the optimal arrangement of camera-collimator system and it may also foster the study of some artifacts compensation methods. Iterative SPECT reconstruction algorithms allow for the inclusion of information related to the geometric system response with accuracy and flexibility. Since the experimental setup of a camera-collimator system may vary considerably, it is necessary to fit the restoration potential of the algorithm to the actual acquisition conditions. In order to avoid any experimental procedure, we developed a method to calculate weighting factors from the theoretical knowledge of the geometric system response of a multihole collimator. The calculation of the weighting factors was based on an appropriate discretization of the Radon transform whose kernel was modified to describe the variable system response of the collimator.

In the European and national policies for the realization of the objectives and strategies of “Lisbon 2010” and “Horizon 2020”, lifelong guidance is recognized as a permanent right for every person. The main objective of the research is to develop a self-orientation online path that helps to increase and improve the self-reflection of people with disabilities on their own knowledge, soft skills, interests, strengths and weaknesses. The final result is supposed to be the recognition of the career project closer to their personal and professional profile. The platform value is related to the active role played by the person with disabilities in the orientation process.

Abstract

The aim of the present study was to evaluate the impact of the aortic bifurcation (AB) morphological characteristics, analyzed on computed tomography angiography (CTA), on outcomes of patients with abdominal aortic aneurysms (AAAs), treated by endovascular aneurysm repair (EVAR) in a single-center experience. A retrospective analysis was conducted using a prospectively collected database. Morphological features considered as potentially impacting outcomes were maximum AB diameter (ABmax), minimum diameter (ABmin), mean diameter (ABaverage), AB area (ABarea), and AB calcification (ABcalcification) and thrombosis (ABthrombosis). Outcome measures were perioperative, 30-day, and midterm AAA-related reinterventions and all-cause mortalities. Investigators reviewed 306 preoperative CTA scans. Maximum aortic diameter was 51.4 ± 12.4 mm (range 40–110), and mean ABmax was 24.2 ± 8.8 mm (range 10–60), ABmin 17.0 ± 5.4 mm (range 4–40), ABaverage 20.6 ± 6.5 mm (range 9–47.5), and ABarea 35.2 ± 24.2 mm2 (range 6–176). ABcalcification ≥ 50% was present in 63 patients (20.6%), and ABthrombosis ≥ 50% in 102 patients (33.3%). Technical success was obtained in all cases, without perioperative reintervention or death. At 30-day follow-up, the reintervention rate was 3.3%, and mortality rate was 1.3%. At a mean follow-up period of 35 ± 28.6 (range, 1–72) months, reintervention and mortality rates were 6.5 and 4.9%, respectively. None of the analyzed thresholds were predictive of adverse outcomes. At multivariate analysis, association of a narrowed AB with severe calcification of the distal aorta showed a significant differences in terms of reinterventions (p = 0.009). Our limited experience seems to reveal that a cutoff of ≤ 20 mm for AB diameter, as in current guidelines, is ineffective in predicting outcomes after EVAR.

48. Analisi del contenuto delle autocaratterizzazioni degli allievi di una scuola di specializzazione in psicoterapia

Abstract

Il presente studio ha avuto come obiettivo la valutazione dei cambiamenti, durante l’arco dei quattro anni di formazione, di un gruppo di 20 allievi di una scuola di specializzazione in psicoterapia di orientamento cognitivo. I dati del gruppo sperimentale sono stati confrontati con quelli di un gruppo di controllo. Entrambi i gruppi hanno redatto, all’inizio e al termine della formazione, un’autocaratterizzazione, strumento che permette di comprendere la prospettiva con la quale un individuo costruisce la propria realta con modalita di cambiamento in continuo divenire. Alle autocaratterizzazioni e stata applicata l’analisi del contenuto, definita da Berelson come capace di descrivere in modo obiettivo, sistematico e quantitativo il contenuto manifesto della comunicazione. E stato adottato un protocollo che ha definito le procedure seguite, il manuale dei codici interpretativi e la griglia di codifica. Ogni autocaratterizzazione e stata analizzata da un gruppo di analisti con meccanismi di controllo delle decodifiche soggettive. Per verificare se ai soggetti siano accaduti, nell’arco temporale preso in considerazione, eventi che possano aver inciso sull’assetto cognitivo e stato somministrato il Questionario sugli Avvenimenti della Vita (QAV).I risultati della ricerca attestano variazioni significative tra gruppo sperimentale e gruppo di controllo. Gli allievi, dopo quattro anni di formazione, ritengono che gli altri riconoscano in loro una maggiore capacita di spiegare comportamenti e atteggiamenti degli altri, una maggiore capacita autoriflessiva, un senso di soddisfazione per il completamento del proprio percorso formativo e quindi l’assunzione di un ruolo professionale.
