1. Quality of life and burden of morbidity in primary care users with multimorbidity

Participants completed the Disease Burden Impact Scale (DBIS) questionnaire, the five dimension-five level Euro-Qol (EQ-5D-5L) and standard demographics questions. The DBIS asks participants to self-report chronic conditions and to rate the impact of each condition. Descriptive statistics and analysis of variance were used to determine quality of life, count of diseases and burden of morbidity. Multiple linear regression analyses determined whether disease count or the DBIS, adjusted for demographics, was more predictive of the EQ-5D-5L scores.Results: Thirty-one percent (n=917) responded, from which 69 were excluded as they reported no or only one condition, leaving 848 (92%) in the analysis. Slightly more women (50.9%) participated; the mean age was 67.0 (SD 13.9) and the mean number of conditions was 6.5 (SD 3.49). The mean scores were: DBIS 15.5 (SD 12.00; score range 0–140, with higher scores indicating higher multimorbidity burden), EQ-5D-5L score 0.69 (SD 0.28; score range −0.28 [a state worse than death] to 1 [best possible health state]) and EQ-5D Visual Analog Scale (EQ-VAS) 65.44 (SD 23.66; score range 0–100 with higher scores meaning better health). The model using the DBIS score was more predictive of the EQ-5D-5L score and EQ-VAS than the model using the disease count (R2adj=0.53 using DBIS and R2adj=0.42 using disease count for EQ-5D-5L score, and R2adj=0.44 using DBIS versus R2adj=0.34 using disease count for EQ-VAS). Participants were recruited through primary care. The interviews were transcribed verbatim and analyzed by thematic analysis. The analysis served to refine the conceptual framework, based on reviews of the literature and stakeholder consultations, for developing candidate items for a new measure for long-term conditions.Results: Three main organizing concepts were identified: impact of long-term conditions, experience of services and support, and self-care. The findings helped to refine a conceptual framework, leading to the development of 23 items that represent issues of importance in long-term conditions. The 23 candidate items formed the first draft of the measure, currently named the Long-Term Conditions Questionnaire.Conclusion: The aim of this study was to refine the conceptual framework and develop items for a patient-reported outcome measure for long-term conditions, including single and multiple morbidities and physical and mental health conditions. Qualitative interviews identified the key themes for assessing outcomes in long-term conditions, and these underpinned the development of the initial draft of the measure. These initial items will undergo cognitive testing to refine the items prior to further validation in a survey. 4. Rediscovering the history of economic thought

A recent issue of Oxford Economic Papers used the fiftieth anniversary of the British Hitory of Economic Thought Conference to reflect on the reawakening of interest in the history of economic ideas that has accompanied the movement of the twentieth century from ‘the present’ to ‘the past’. 5. Could reputation-bias be a bigger problem than inflation-bias?

Abstract

The theory of policy credibility has been influential in both the design of monetary policymaking institutions and in the implementation of policy. In particular, the idea that 'reputation' is important has been widely accepted. However, careful attention to its assumptions and implications of the theory reveals many sources of doubt as to its empirical value. First, the theory is implausible, and even if taken seriously does not point to many of the conclusions frequently supposed to be based on it. Second, evidence suggests the theory is false. Third, even policymakers who profess themselves concerned about the maintenance of credibility do not behave consistently in the way the theory says they should. Although many policy proposals ostensibly based on the theory of credibility therefore seem to lack persuasive support, the idea of credibility still poses a danger to effective policymaking since it creates motives for excessively contractionary policy. Although it is frequently asserted that monetary policy can have no long-term effects on economic performance, the idea that a loss of 'reputation' will have lasting detrimental effects appears to motivate much policy. In the absence of convincing arguments that reputation – in its technical sense – is important, this would seem to be undesirable and probably dangerous.

It is widely accepted that the importance of Friedman's Presidential Address to the American Economic Association lies in its criticism of policy based on the Phillips curve. It is argued that a reading of the text does not support such a view, and this and other considerations suggest that any such aim was far from Friedman's mind in 1968. His objective was the quite different one of making a case for policy 'rules' rather than discretion.

Abstract

It is noted that Friedman (1968) suggested the adjustment to a change in the rate of inflation would take decades and that this is rather a long time. Various suggestions as to why Friedman may have said this are considered. It is argued that he may have had in mind not a more or less rational change in expectations, but something more like a change in the habits of thought. It is noted that if this is correct, his view on the point is not generally accepted.

17. Two lectures by Friedman: One famous, one good

Abstract

Friedman’s Presidential Address to the American Economic Association is compared to a slightly later lecture he gave to the American Philosophical Society. The earlier piece is found to be poorly structured, badly argued, and full of clear mistakes. The later one, although pursuing the same objectives, is in every way much better crafted. It is noted that the earlier one is however much more highly cited, and it is suggested that this means, inter alia, that the large number of citations of the Presidential Address do not reflet much reading of it.

18. Traps in the Measurement of Independence and Accountability of Central Banks

Abstract

Measures of central banks' independence and central banks' accountability which are based on an interpretation of their statutes provide, in themselves, no guidance for the assessment of legislative proposals. In the effectiveness of monetary policy, the crucial considerations relate to how central banks and other policymakers behave. The pattern of incentives is not determined by the statutes, which may be wholly irrelevant, and therefore one cannot discover the effectiveness of policy by inspecting the statutes. On the other hand, an assessment of the democratic acceptability of an arrangement requires a normative argument. Depending on that argument, statutory characteristics may be the crucial ones, but for the assessment to be worthwhile, the argument must be explicit.

21. Economists on Samuelson and Solow on the Phillips curve

Abstract

Samuelson and Solow published a widely read paper in the May issue of the American Economic Review of 1960. It discussed the causes of inflation, the Phillips curve, and related matters. Discussion of their paper frequently says that it presented the Phillips curve as a stable, exploitable relation, and hence played an important role in the development of inflationary policy. This is hardly so. Sometimes authors notice this, but they nevertheless say it was misread as advocating inflationary policy and hence played the same role in policy development. Close attention to what was said about it in the relevant period – the 1960s – reveals that it was not then seen as advocating inflationary policy at all. This raises a strange puzzle as to why it was that, rather suddenly, it came to be incorrectly said that Samuelson and Solow had been interpreted as being inflationist when they neither were that, nor had been interpreted in that way.

The idea that the 'credibility' of monetary policy is important receives almost universal assent. But the word can mean very different things in different contexts. In particular, the meaning that it has been given by the work of Robert Barro and David Gordon is a very special one. It should not be presumed that in ordinary discussion of 'credibility' theirs is always the meaning people have in mind. Consequently, Alan Blinder's survey on attitudes to 'credibility', which failed to specify what he meant by the term, might be most misleading, and if unwarranted inferences are not to be drawn a most careful interpretation of it is required.

Abstract

This paper presents empirical findings from the comparison between two principal preference elicitation techniques: discrete choice experiments and profile-based best-worst scaling. Best-worst scaling involves less cognitive burden for respondents and provides more information than traditional " pick-one" tasks asked in discrete choice experiments. However, there is lack of empirical evidence on how best-worst scaling compares to discrete choice experiments. This empirical comparison between discrete choice experiments and best-worst scaling was undertaken as part of the Outcomes of Social Care for Adults project, England, which aims to develop a weighted measure of social care outcomes. The findings show that preference weights from best-worst scaling and discrete choice experiments do reveal similar patterns in preferences and in the majority of cases preference weights - when normalised/rescaled - are not significantly different. © 2011.

Abstract

The role of coronary-artery bypass grafting (CABG) in the treatment of patients with coronary artery disease and heart failure has not been clearly established.Between July 2002 and May 2007, a total of 1212 patients with an ejection fraction of 35% or less and coronary artery disease amenable to CABG were randomly assigned to medical therapy alone (602 patients) or medical therapy plus CABG (610 patients). The primary outcome was the rate of death from any cause. Major secondary outcomes included the rates of death from cardiovascular causes and of death from any cause or hospitalization for cardiovascular causes.The primary outcome occurred in 244 patients (41%) in the medical-therapy group and 218 (36%) in the CABG group (hazard ratio with CABG, 0.86; 95% confidence interval [CI], 0.72 to 1.04; P=0.12). A total of 201 patients (33%) in the medical-therapy group and 168 (28%) in the CABG group died from an adjudicated cardiovascular cause (hazard ratio with CABG, 0.81; 95% CI, 0.66 to 1.00; P=0.05). Abstract

The idea of the ‘L-shaped aggregate supply curve’, supposedly a feature of primitive macroeconomic models, is in fact a reasonable reconstruction of a well developed way of thinking that specifically denied a relation between wage change and aggregate employment. Neither that approach nor the idea of cost-push inflation to which it is related need be crude or superficial. Although the ideas in question were swept away by the Phillips curve, they have much merit and their reintroduction to mainstream macroeconomics might pay large dividends.

Abstract

Objective: The aim of this study was to develop a measure of social care outcome, an equivalent to the quality-adjusted life year (QALY) in health, which could be used in a range of circumstances. Design: The project drew on previous and parallel work developing the Adult Social Care Outcome Toolkit and the national Adult Social Care Survey. We developed and tested an instrument designed to reflect service users' social care-related quality of life (SCRQoL) and tested it with 30 service users from a variety of user groups and 300 older home care service users. In parallel, we explored discrete choice experiment (DCE) and best-worst scaling (BWS) approaches to preference elicitation with 300 members of the general population, and cognitively tested these with service users. We also cognitively tested a computer-aided time trade-off (TTO) exercise using SCRQoL attributes with members of the general population. In the second phase, using the finalised instruments, BWS interviews were conducted with 500 members of the general population, TTO interviews with a follow-up sample of 126 of these respondents, and BWS interviews with 458 people using equipment services. Main outcome measures: The final measure had eight domains: personal cleanliness and comfort, accommodation cleanliness and comfort, food and drink, safety, social participation and involvement, occupation, control over daily life and dignity. In addition to measuring current SCRQoL, the instrument includes questions used to establish service users' views of their 'expected' SCRQoL in the absence of services. The difference between a person's current and 'expected' SCRQoL provides an indicator of service impact. Results: There was good evidence for the validity of the descriptive system and the validity of the current, expected and SCRQoL gain scales. Abstract

This paper presents empirical findings from the comparison between two principal preference elicitation techniques: discrete choice experiments and profile-based best-worst scaling. Best-worst scaling involves less cognitive burden for respondents and provides more information than traditional " pick-one" tasks asked in discrete choice experiments. However, there is lack of empirical evidence on how best-worst scaling compares to discrete choice experiments. This empirical comparison between discrete choice experiments and best-worst scaling was undertaken as part of the Outcomes of Social Care for Adults project, England, which aims to develop a weighted measure of social care outcomes. The findings show that preference weights from best-worst scaling and discrete choice experiments do reveal similar patterns in preferences and in the majority of cases preference weights - when normalised/rescaled - are not significantly different. © 2011.
