Background Coronavirus disease 2019 (COVID-19) has spread worldwide, causing a high burden of morbidity and mortality, and has affected the various health service systems in the world, demanding disease monitoring and control strategies. The objective of this study was to identify risk areas using spatiotemporal models and determine the COVID-19 time trend in a federative unit of northeastern Brazil. Methods An ecological study using spatial analysis techniques and time series was carried out in the state of Maranhão, Brazil. All new cases of COVID-19 registered in the state from March 2020 to August 2021 were included. Incidence rates were calculated and spatially distributed by area, while the spatiotemporal risk territories were identified using scan statistics. The COVID-19 time trend was determined using Prais–Winsten regressions. Results Four spatiotemporal clusters with high relative risks for the disease were identified in seven health regions located in the southwest/northwest, north and east of Maranhão. The COVID-19 time trend was stable during the analysed period, with higher rates in the regions of Santa Inês in the first and second waves and Balsas in the second wave. Conclusions The heterogeneously distributed spatiotemporal risk areas and the stable COVID-19 time trend can assist in the management of health systems and services, facilitating the planning and implementation of actions toward the mitigation, surveillance and control of the disease. Background Geographical overlap of human visceral leishmaniasis (HVL) and HIV infection favors occurrences of HVL/HIV coinfection. The increasing numbers of cases of HVL/HIV coinfection are a worldwide concern and knowledge of the factors involved can help in developing preventive measures. Methods We aimed to identify spatial patterns and describe the epidemiological profile of HVL/HIV coinfection in Brazil from 2007 to 2015. This was an ecological study, in which thematic maps were created through geoprocessing tools, based on secondary data obtained from open-access platforms, to identify priority areas for interventions for controlling HLV/HIV coinfection, using the TerraView 4.2.2 software. Results We found spatial autocorrelations between the HVL/HIV rates of neighboring municipalities according to the global Moran index (0.10; p<0.01). The highest HVL/HIV rate was in the central-western region. Among the epidemiological characteristics according to the regions, an increasing trend in the annual variation rate was observed, with positive values over the years and statistical significance (p<0.05) in the North with 1.62 (95% CI 0.57 to 2.69; p=0.02) and Northeast with 6.41 (95% CI 2.60 to 10.37; p=0.006). Similarly, increasing trends were observed in the states of Maranhão with 21.34 (95% CI 13.99 to 9.16; p<0.001) and Sergipe with 5.44 (95% CI 0.61 to 10.50; p=0.04). Conclusions Our results showed spatial overlap between occurrences of HVL and HIV with spatial patterns of HVL/HIV coinfection, thus revealing that the numbers of cases reported are increasing. Identifying areas with higher coinfection indices contributes to applying interventions and control measures among targeted populations, to prevent new cases. [ABSTRACT FROM AUTHOR]

Humanized Childbirth consists of a set of strategies that are adopted by the nursing team with other professionals that aims to provide the most comfortable and less painful parturition process for women. To identify factors associated with the humanization of care during labor, delivery and birth. This is a cross-sectional study of descriptive - exploratory and quantitative approach carried out at the Regional Maternal And Child Hospital of Imperatriz (HRMI) from October to December 2019. The research had the participation of 120 puerperal women of vaginal delivery aged between 18 and 41 years. The participants were mostly brown and with an income of up to 1 minimum wage. The vast majority considered that there was humanization in their delivery through the conducts adopted by the team. Regarding the application of non-pharmacological pain relief methods at the time of delivery, more than 60% of the participants reported having received this type of care, which indicates the adoption of humanized practices. Good practices of care in childbirth were found in this research according to the perception of postpartum women. The professionals promoted actions with the objective of making labor and delivery a more effective and less painful process. The conducts performed by the nursing team and shown in this study are principles for the adoption of humanized delivery, which provides women with a safer and more humane delivery contributing to maternal and child well-being. El parto humanizado consiste en un conjunto de estrategias que son adoptadas por el equipo de enfermería con otros profesionales que tiene como objetivo proporcionar el proceso de parturición más cómodo y menos doloroso para las mujeres. Identificar los factores asociados con la humanización de la atención durante el parto, el parto y el nacimiento. Se trata de un estudio transversal de enfoque descriptivo - exploratorio y cuantitativo llevado a cabo en el Hospital Regional Materno-Infantil de Imperatriz (HRMI) de octubre a diciembre de 2019. La investigación contó con la participación de 120 mujeres puerperales de parto vaginal de entre 18 y 41 años. Los participantes eran en su mayoría marrones y con un ingreso de hasta 1 salario mínimo. La gran mayoría consideró que había humanización en su entrega a través de las conductas adoptadas por el equipo. En cuanto a la aplicación de métodos no farmacológicos de alivio del dolor en el momento del parto, más del 60% de los participantes informaron haber recibido este tipo de atención, lo que indica la adopción de prácticas humanizadas. En esta investigación se encontraron buenas prácticas de atención en el parto de acuerdo con la percepción de las mujeres posparto. Los profesionales promovieron acciones con el objetivo de hacer del trabajo y la entrega un proceso más eficaz y menos doloroso. Las conductas realizadas por el equipo de enfermería y mostradas en este estudio son principios para la adopción del parto humanizado, que proporciona a las mujeres un parto más seguro y humano que contribuye al bienestar materno infantil. O Parto Humanizado consiste em um conjunto de estratégias que são adotadas pela equipe de enfermagem junto aos demais profissionais que visa proporcionar o processo parturitivo mais confortável e menos doloroso a mulher. Identificar fatores associados à humanização da assistência durante o trabalho de parto, parto e nascimento. Trata-se de um estudo transversal de caráter descritivo – exploratório e de abordagem quantitativa realizada no Hospital Regional Materno Infantil de Imperatriz (HRMI) no período de outubro a dezembro de 2019. A pesquisa contou com a participação de 120 puérperas de parto vaginal com idades entre 18 e 41 anos. As participantes eram em sua maioria de cor parda e com renda de até 1 salário-mínimo. A grande maioria considerou que houve humanização no seu parto por meio das condutas adotadas pela equipe. Com relação a aplicação dos métodos não farmacológicos de alívio da dor na hora do parto, mais de 60% das participantes informaram terem recebido esse tipo de assistência, o que indica a adoção de práticas humanizadas. Constatou-se nessa pesquisa boas práticas de atenção assistencial no parto de acordo com a percepção das puérperas. Os profissionais promoviam ações com objetivo de tornar o trabalho de parto e parto um processo mais eficaz e menos doloroso. As condutas realizadas pela equipe de enfermagem e mostradas nesse estudo são princípios para a adoção do parto humanizado, o que proporciona a mulher um parto mais seguro e humano contribuindo para o bem-estar materno-infantil.

Alternative methods for pain relief aim to make childbirth as natural as possible, reducing unnecessary and harmful interventions, according to the World Health Organization, and rescuing the parturient's autonomy. The aim of this study was to investigate alternative therapies for the relief of pain from normal delivery in the literature available in different parts of the world. A systematic literature review was carried out in PubMed®, Scientific Electronic Library Online (SciELO), Biblioteca Virtual em Saúde (BVS) and ScienceDirect databases searching for studies published between 2002 and 2019. The research took place between February and April 2020, according to the descriptors “parturition”, “complementary therapies”, “pain” and “normal delivery”. PRISMA recommendations were used and registered in PROSPERO (CRD42020186056). We analyzed 6,758 articles of clinical trial model. A total of 16 articles for qualitative synthesis involving five different countries were included. Most of the studies had primiparous participants, and by inclusion criteria, healthy women, and with usual risk of prenatal care. The 16 studies involved ten different non-pharmacological techniques, with acupuncture and bathing with hot therapy as the most used. Most of the alternative therapies responded satisfactorily in aid of labor pain, besides not being harmful to newborns. The alternative methods in childbirth are comprehensive and collaborative throughout the gestational and parturition process, being statically significant with pain, and providing a range of models that can be planned and employed by maternity hospitals in an easy and economical way. Los métodos alternativos para aliviar el dolor tienen como objetivo hacer que el parto sea lo más natural posible, reduciendo las intervenciones innecesarias y dañinas según la Organización Mundial de la Salud y rescatando la autonomía del parto. El objetivo de este estudio fue investigar terapias alternativas para el alivio del dolor del parto normal en la literatura disponible en diferentes partes del mundo. Entre 2002 y 2019 se llevó a cabo una revisión sistemática de la literatura en las bases de datos PubMed®, Scientific Electronic Library Online (SciELO), Biblioteca Virtual em Saúde (BVS) y ScienceDirect, según el tiempo de entrega, según los descriptores “parto”, “terapias complementarias”, “dolor” y “parto normal”. Las recomendaciones de PRISMA se utilizaron y registraron en PROSPERO (CRD42020186056). Analizamos 6.758 artículos del modelo de ensayo clínico. Se incluyeron un total de 16 artículos para la síntesis cualitativa en los que participaron cinco países diferentes. La mayoría de los estudios tuvieron participantes caros, y por criterios de inclusión, mujeres sanas, y con riesgo habitual de atención prenatal. Los 16 estudios involucraron diez técnicas no farmacológicas diferentes, con acupuntura y baño con terapia caliente como los más utilizados. La mayoría de las terapias respondieron satisfactoriamente en ayuda del dolor de parto, además de no ser perjudiciales para los recién nacidos. Los métodos alternativos en el parto son integrales y colaboración a lo largo del proceso de gestación y parto, fueron estáticamente significativos con dolor, oportunistas una gama de modelos que pueden ser planificados y empleados por los hospitales de maternidad de una manera fácil y económica. Os métodos alternativos para o alívio da dor têm a finalidade de tornar o parto o mais natural possível, reduzindo as intervenções desnecessárias e prejudiciais, segundo a Organização Mundial da Saúde, e resgatando a autonomia da parturiente. O objetivo deste estudo foi investigar as terapias alternativas para o alívio da dor do parto normal na literatura disponível em diferentes partes do mundo. Foi feita uma revisão sistemática de literatura nas bases de dados PubMed®, Scientific Electronic Library Online (SciELO), Biblioteca Virtual em Saúde (BVS) e ScienceDirect de estudos publicados entre os anos de 2002 e 2019. A pesquisa ocorreu entre os meses de fevereiro a abril de 2020, conforme os descritores “parturição”, “terapias complementares”, “dor” e “parto normal”. Utilizaram-se as recomendações PRISMA e registradas no PROSPERO (CRD42020186056). Foram analisados 6.758 artigos. Foram incluídos 16 artigos para síntese qualitativa, envolvendo cinco países diferentes. Os estudos tiveram, em sua maioria, participantes primíparas, e, por critérios das inclusões, mulheres saudáveis e com risco habitual de pré-natal. Os 16 estudos envolveram dez técnicas não farmacológicas diferentes, sendo a acupuntura e o banho de aspersão com termoterapia quente as mais utilizadas. A maioria das terapias alternativas respondeu satisfatoriamente ao auxílio da dor do parto, além de não se apresentarem prejudiciais aos recém-nascidos. Os métodos alternativos no parto são abrangentes e colaborativos durante todo o processo gestacional e parturitivo, sendo estaticamente significativos em relação à dor e oportunizando uma gama de modelos que podem ser planejados e empregados pelas maternidades de modo fácil e econômico.

Background The detection of spatiotemporal clusters of deaths by coronavirus disease 2019 (COVID-19) is essential for health systems and services, as it contributes to the allocation of resources and helps in effective decision making aimed at disease control and surveillance. Thus we aim to analyse the spatiotemporal distribution and describe sociodemographic and clinical and operational characteristics of COVID-19-related deaths in a Brazilian state. Methods A descriptive and ecological study was carried out in the state of Maranhão. The study population consisted of deaths by COVID-19 in the period from 29 March to 31 July 2020. The detection of spatiotemporal clusters was performed by spatiotemporal scan analysis. Results A total of 3001 deaths were analysed with an average age of 69 y, predominantly in males, of brown ethnicity, with arterial hypertension and diabetes, diagnosed mainly by reverse transcription polymerase chain reaction in public laboratories. The crude mortality rates the municipalities ranged from 0.00 to 102.24 deaths per 100 000 inhabitants and three spatiotemporal clusters of high relative risk were detected, with a mortality rate ranging from 20.25 to 91.49 deaths per 100 000 inhabitants per month. The headquarters was the metropolitan region of São Luís and municipalities with better socio-economic and health development. Conclusions The heterogeneous spatiotemporal distribution and the sociodemographic and clinical and operational characteristics of deaths by COVID-19 point to the need for interventions. Background Congenital syphilis is an infectious disease that affects 1 million children a year worldwide. Methods The objective of this study was to describe a spatial analysis of the epidemiological characteristics of congenital syphilis in the state of Maranhão, Brazil from 2007 to 2018. This was an ecological study using data obtained in May 2020 from three Brazilian information systems. The spatial correlation was analyzed according to local and global Moran indexes. Results During the 12-y period, 1 426 177 children were born. Of these children, 3684 acquired congenital syphilis (0.26%; mean annual rate of 2.62/1000 live births) and 70 died (1.90%; mean annual rate 0.05). The clusters were statistically significant according to the global Moran index of 0.40 (p=0.01). Spearman's correlation coefficient between the rate of detection of syphilis in pregnant women and family health strategy teams was strong, positive and significant (ρ=0.73; p<0.00). Most of the variables studied showed a trend of annual increase (p<0.05). Conclusions The use of spatial analysis tools have made it possible to detect areas with both a greater and a lesser need for intervention, and to more effectively improve and monitor those areas to change the epidemiological profile of the disease. This is a descriptive, cross-sectional, quantitative study developed between July 2014 and July 2016 with parents or caregivers of 10 children and 40 adolescents with type 1 diabetes mellitus. The participants were interviewed in their homes with prior scheduling. The average age of the subjects was 13.1 years old, 64.0% were females, early weaning was evidenced for 34 individuals and there was the insertion of whole cow´s milk in the diet of 26 children besides juices, fruits, and other foods. It is concluded that exclusive breastfeeding up to 6 months is a safe, economical and emotionally satisfying way to feed babies.

Introduction: The analysis of factors associated with multibacillary leprosy is important for the development of strategies to mitigate the disease, which persists as a public health problem in Brazil and the world. The objective of this study was to verify the associations between sociodemographic and clinical-epidemiological variables and multibacillary leprosy in the state of northeastern Brazil., Methodology: This is a cross-sectional, analytical, and retrospective study, with a quantitative approach, carried out in 16 municipalities in the southwest of Maranhão State, northeastern Brazil. All cases of leprosy reported between January 2008 and December 2017 were considered. Sociodemographic and clinical-epidemiological variables were analyzed using descriptive statistics. The identification of the risk factors associated with multibacillary leprosy was conducted using Poisson regression models. The prevalence ratios and respective 95% confidence intervals were estimated using regression coefficients at a 5% significance level., Results: A total of 3,903 leprosy cases were analyzed. Individuals older than 15 years, males, with less than 8 years of education, with level I, II, or "not evaluated" disability, and with type 1 or 2 or both reactional states were more likely to have multibacillary leprosy. Therefore, these characteristics may be considered risk factors. No protective factors were identified., Conclusions: The investigation revealed important associations between risk factors and multibacillary leprosy.
