2. Advanced Practice Nursing in Portugal – Where do we stand?

Background: Portugal's health workforce has been described as an inefficient mix of resources, impacting productivity and access to health services. To address these issues, stakeholders have advocated for Advanced Practice Nursing (APN) to strengthen the nursing role and improve service delivery. Aim: To analyze the relevance, pertinence, feasibility, and main barriers and facilitators of APN implementation in Portugal. Methods: A scoping review was conducted in the CINAHL, MEDLINE Complete, PubMed, and ScienceDirect databases for peer-reviewed articles in English and Portuguese, published after 2000, on human resource configurations in health and nursing role expansion in Portugal. A review of grey literature further complemented the research. Results: A total of 59 studies (43 international, 16 national), 31 technical reports (17 international, 14 national), seven press articles, and two theses were screened. Findings were grouped into themes of APN conceptualization, regulation, barriers and facilitators, and funding. APN implementation depends on a consensus regarding APN conceptualization, regulation, and scope of practice. Key barriers include medical opposition, limited political commitment, and lack of comprehensive data on APNs' social and financial impact in Portugal. Conclusions: The implementation of Advanced Practice Nursing in Portugal is relevant and necessary, though its feasibility depends on strong leadership, political commitment, and consensus among stakeholders. Further research is essential to assess APNs' social and economic impacts within the Portuguese healthcare system.

A equação escalar de Helmholtz descreve os harmônicos-temporais de ondas acústicas. É bem conhecido que métodos de diferenças finitas e elementos finitos apresentam o efeito de poluição do erro para números de onda médio e alto. Neste trabalho são analisados três novos esquemas de diferenças finitas centradas de segunda ordem de precisão em uma e duas dimensões. Esses novos esquemas são consistentes e foram obtidos realizando novas aproximações apenas no segundo termo da equação de Helmholtz. A análise de dispersão, o comportamento do erro e os resultados numéricos mostram o bom desempenho dos Novos Esquemas 2 e 3. O Novo Esquema 3 é capaz de eliminar o efeito de poluição do erro em uma dimensão e minimizar a dispersão da onda plana em duas dimensões.

4. Seaweed extract-based fertilizer and water stress on potato crops

ABSTRACT Seaweed extract-based fertilizers are applied to mitigate the effects of stress on plants. In this study, we evaluated the effects of different concentrations of seaweed extract-based fertilizer and different irrigation depths on potato crops. The Ágata cultivar and the Markies cultivar were used to conduct two experiments simultaneously. A 4 × 4 experimental design was used, with four doses of seaweed extract-based fertilizer (0, 0.5, 1.0, and 4.0 L ha-1) and four irrigation depths (50%, 75%, 100%, and 125% of crop evapotranspiration); each treatment had three replicates. Leaves were collected 67 days after emergence to analyze macronutrients and micronutrients; additionally, the number of stems per plant and physiological variables were assessed. After harvesting, the number and weight of tubers in different classes were recorded. The differences in parameters among treatment groups were determined by analysis of variance using the F-test, and regression analysis was performed when a significant difference was recorded. The results showed that the contents of potassium, phosphorus, magnesium, and calcium in the leaves of the Ágata cultivar were below the optimal levels for the crop, irrespective of the irrigation level and the dose of seaweed extract-based fertilizer applied. The highest irrigation depths at 100% and 125% of crop evapotranspiration were associated with an increase in the number and weight of the large tubers of the Ágata and Markies cultivars.

5. Assessment of the components of fluid balance in patients with septic shock: a prospective observational study

Background: The optimal amount for initial fluid resuscitation is still controversial in sepsis and the contribution of non-resuscitation fluids in fluid balance is unclear. We aimed to investigate the main components of fluid intake and fluid balance in both survivors and non-survivor patients with septic shock within the first 72 hours. Methods: In this prospective observational study in two intensive care units, we recorded all fluids administered intravenously, orally, or enterally, and losses during specific time intervals from vasopressor initiation: T1 (up to 24 hours), T2 (24 to 48 hours) and T3 (48 to 72 hours). Logistic regression and a mathematical model assessed the association with mortality and the influence of severity of illness. Results: We included 139 patients. The main components of fluid intake varied across different time intervals, with resuscitation and non-resuscitation fluids such as antimicrobials and maintenance fluids being significant contributors in T1 and nutritional therapy in T2/T3. A positive fluid balance both in T1 and T2 was associated with mortality (p = 0.049; p = 0.003), while nutritional support in T2 was associated with lower mortality (p = 0.040). The association with mortality was not explained by severity of illness scores. Conclusions: Non-resuscitation fluids are major contributors to a positive fluid balance within the first 48 hours of resuscitation. A positive fluid balance in the first 24 and 48 hours seems to independently increase the risk of death, while higher amount of nutrition seems protective. This data might inform fluid stewardship strategies aiming to improve outcomes and minimize complications in sepsis.

6. Drug binding and drug-drug interaction considerations in individuals with obesity before and after bariatric surgery: A retrospective cross-sectional study

Obesity impacts pharmacokinetic and pharmacodynamic parameters, changing the drug-binding state via plasma proteins, which is critical for drug effectiveness and toxicity. Albumin and α1-acid glycoprotein (AAG) are the two main proteins accountable for drug binding in humans. Previous studies have shown higher values of AAG in individuals with obesity, but the interference of AAG in the pharmacokinetics of drugs remains unclear. We, therefore, aimed to analyze the pharmacotherapeutic profile and interfering factors in patients undergoing bariatric surgery in the pre-operative period and after 12 months. A retrospective cross-sectional study design was conducted and serum AAG was determined, and potential drug-drug interactions were monitored in patients with severe obesity who underwent bariatric surgery. The drug classes most used were antidepressants, cardiovascular agents, and lipid-modifying agents, with a decrease in use after 12 months of surgery. Before surgery, it was found that more than half of the patients had at least one drug interaction in their therapeutic regimen. Of these, most were classified as moderate. After surgery, a decrease in drug interaction was observed. Serum AAG showed a significant reduction after 12 months, which can influence the free fraction of drugs with a high-affinity rate and how they impact therapeutic efficacy.

The species Rosmarinus officinalis L. (rosemary) is an herb from the Lamiaceae family, widely used in cooking as a food preservative, seasoning or condiment. It also stands out for its therapeutic properties, mainly presenting antioxidant, antibacterial and antitumor activity. The aim of this study was to evaluate the cytotoxic and mutagenic activity of R. officinalis L. essential oil through the Allium cepa bioassay. This test constitutes an excellent plant model routinely used due to its sensitivity, low cost and good correlation with test systems in mammals. The defined concentrations for carrying out the test were 750, 243, 81 and 27 µg mL-1. Five bulbs were used, 4 roots of each, measuring approximately 2 cm, and they were analyzed on two slides. All assays were performed at least in triplicate and compared to the negative control. The statistical test of analysis of variance (ANOVA with a fixed factor) was used, followed by Tukey's multiple comparisons test, for p < 0.05. For this purpose, the GraphPad Prism program (version 6.0) was used. The results showed a cytotoxic and mutagenic effect for all concentrations used of the essential oil of R. officinalis L. However, it is important to conduct further research using other genotoxicological tests with different endpoints and at different concentrations, in order to clarify the interaction of the essential oil of the species R. officinalis L. with the genetic material of the cell and its possible mechanism of action.

Dermatofibrosarcoma protuberans (DFSP) is an extremely rare and potentially aggressive soft tissue tumor. Around 85 to 90% of DFSP cases are low-grade tumors, while the fibrosarcomatous variant (DFSP-FS) is associated with medium and high-grade neoplasms. DFSP affects more young adults, in their 30s; however, it can be present in all ages. It has a high potential for locoregional dissemination and recurrence, directly related to tumor size. In the present study, a case of dermatofibrosarcoma protuberans with local recurrence after surgical treatment will be reported, subsequently treated with imatinib and a new surgical approach, with no new recurrences so far. The effectiveness of imatinib is noted in a deep, infiltrative and relapsed DFSP, in which upfront surgery would cause significant functional and cosmetic damage. Imatinib allowed an important clinical improvement for the patient, with tumor reduction in the initial weeks of treatment.

Although it appears biologically plausible for iron to be associated with gastric carcinogenesis, the evidence is insufficient to lead to any conclusions. To further investigate the relationship between body iron status and gastric cancer risk, we conducted a nested case-control study in the multicentric European Prospective Investigation into Cancer and Nutrition (EPIC) study. The study included 456 primary incident gastric adenocarcinoma cases and 900 matched controls that occurred during an average of 11 years of follow-up. We measured prediagnostic serum iron, ferritin, transferrin and C-reactive protein, and further estimated total iron-binding capacity (TIBC) and transferrin saturation (TS). Odds ratios (ORs) and 95% confidence intervals (CIs) for the risk of gastric cancer by iron metrics were estimated from multivariable conditional logistic regression models. After adjusting for relevant confounders, we observed a statistically significant inverse association between gastric cancer and ferritin and TS indices (ORlog2 = 0.80, 95% CI = 0.72-0.88; OR10%increment = 0.87, 95% CI = 0.78-0.97, respectively). These associations appear to be restricted to noncardia gastric cancer (ferritin showed a p for heterogeneity = 0.04 and TS had a p for heterogeneity = 0.02), and no differences were found by histological type. TIBC increased risk of overall gastric cancer (OR50 µg/dl = 1.13, 95% CI = 1.02-1.2) and also with noncardia gastric cancer (p for heterogeneity = 0.04). Additional analysis suggests that time between blood draw and gastric cancer diagnosis could modify these findings. In conclusion, our results showed a decreased risk of gastric cancer related to higher body iron stores as measured by serum iron and ferritin. Further investigation is needed to clarify the role of iron in gastric carcinogenesis.

13. Bartter Syndrome-Related Variants Distribution: Brazilian Data and Its Comparison with Worldwide Cohorts

Background: Genetic testing is recommended for accurate diagnosis of Bartter syndrome (BS) and serves as a basis for implementing specific target therapies. However, populations other than Europeans and North Americans are underrepresented in most databases and there are uncertainties in the genotype-phenotype correlation. We studied Brazilian BS patients, an admixed population with diverse ancestry. Methods: We evaluated the clinical and mutational profile of this cohort and performed a systematic review of BS mutations from worldwide cohorts. Results: Twenty-two patients were included; Gitelman syndrome was diagnosed in 2 siblings with antenatal BS and congenital chloride diarrhea in 1 girl. BS was confirmed in 19 patients: BS type 1 in 1 boy (antenatal BS); BS type 4a in 1 girl and BS type 4b in 1 girl, both of them with antenatal BS and neurosensorial deafness; BS type 3 (CLCNKB mutations): 16 cases. The deletion of the entire CLCNKB (1–20 del) was the most frequent variant. Patients carrying the 1–20 del presented earlier manifestations than those with other CLCNKB-mutations and the presence of homozygous 1–20 del was correlated with progressive chronic kidney disease. The prevalence of the 1–20 del in this BS Brazilian cohort was similar to that of Chinese cohorts and individuals of African and Middle Eastern descent from other cohorts. Conclusion: This study expands the genetic spectrum of BS patients with different ethnics, reveals some genotype/phenotype correlations, compares the findings with other cohorts, and provides a systematic review of the literature on the distribution of BS-related variants worldwide.

16. Effects of the positive end-expiratory pressure increase on sublingual microcirculation in patients with acute respiratory distress syndrome

Objective: The aim of this study was to evaluate the impact of increased positive end-expiratory pressure on the sublingual microcirculation. Methods: Adult patients who were sedated, under mechanical ventilation, and had a diagnosis of circulatory shock and acute respiratory distress syndrome were included. The positive end-expiratory pressure level was settled to obtain a plateau pressure of 30 cm H2O and then maintained at this level for 20 minutes. Microcirculatory (obtained by videomicroscopy) and hemodynamic variables were collected at baseline and compared with those at the end of 20 min. Results: Twelve patients were enrolled. Overall, the microcirculation parameters did not significantly change after increasing the positive end-expiratory pressure. However, there was considerable interindividual variability. There was a negative, moderate correlation between the changes in the De Backer score (r = −0.58, p = 0.048), total vessel density (r = −0.60, p = 0.039) and baseline values. The changes in total vessel density (r = 0.54, p = 0.07) and perfused vessel density (r = 0.52, p = 0.08) trended toward correlating with the changes in the mean arterial pressure. Conclusion: Overall, the microcirculation parameters did not significantly change after increasing the positive end-expiratory pressure. However, at individual level, such response was heterogeneous. The changes in the microcirculation parameters could be correlated with the baseline values and changes in the mean arterial pressure. Resumo: Objetivo: O objetivo deste estudo foi avaliar o impacto do aumento de pressão positiva expiratória final (PEEP) sobre a microcirculação sublingual. Métodos: Os pacientes adultos que foram sedados, sob ventilação mecânica, com diagnóstico de choque circulatório e síndrome do desconforto respiratório agudo foram incluídos. O nível da PEEP estabelecido para obter uma pressão de platô de 30 cm H2O e depois mantida nesse nível por 20 minutos. As variáveis microcirculatória (obtida por microscopia de vídeo) e hemodinâmica foram registradas na fase basal e comparadas com aquelas ao final de 20min. Resultados: Doze pacientes foram incluídos. Em geral, os parâmetros da microcirculação não apresentaram alterações significativas após o aumento da PEEP. Porém, houve considerável variabilidade interindividual. Houve uma correlação negativa, moderada, entre as alterações no escore de De Backer (r = -0,58, p = 0,048), na densidade total do vaso (r = -0,60, p = 0,039) e nos valores basais. As alterações na densidade total do vaso (r = 0,54, p = 0,07) e na densidade do vaso perfundido (r = 0,52, p = 0,08) apresentaram tendência de correlação com as alterações na pressão arterial média. Conclusão: Em geral, os parâmetros da microcirculação não apresentaram alterações significativas após o aumento da PEEP. No entanto, individualmente, essa resposta foi heterogênea. As alterações nos parâmetros da microcirculação puderam ser correlacionadas com os valores basais e alterações na pressão arterial média. Resumo Objetivos Avaliar o perfil dos trabalhadores rurais do município de Picos, no Estado do Piauí e suas práticas e atitudes quanto à utilização de agrotóxicos. Métodos A coleta dos dados (n=159) foi realizada em Picos com levantamento de dados socioeconômicos, utilização de equipamentos de proteção individual (EPIs), armazenamento e destino das embalagens de agrotóxicos, toxicidade e risco ambiental. Resultados A maioria dos agricultores era do sexo feminino (63%), não concluiu o ensino fundamental (55,6%), tinha entre 31 e 50 anos (55,3%), com renda de até um salário mínimo (66%) e possuía água encanada (84,3%). Quanto ao uso, 64,1% (102) utilizavam agrotóxicos, principalmente na agricultura (86,3%). Eles obtiveram informações sobre essas substâncias com vizinhos (44,2%), liam o rótulo para utilizar o produto (64,8%), usavam agroquímicos devido à ação rápida (81,4%), compravam em casas agropecuárias (87,4%), sem receituário agronômico (92%) e armazenavam embalagens dentro da própria casa (33,6%). Mais da metade não usava EPIs (56,8%), embora considerassem os agrotóxicos prejudiciais à saúde humana (94,1%) e ao meio ambiente (80,4%), além de 15% (24 pessoas) terem sentido algum sintoma de envenenamento. Conclusões Os entrevistados, em sua maioria, usavam agrotóxicos inadequadamente, tinham baixa escolaridade e conheciam sobre os riscos individuais e coletivos aos quais estavam expostos, mas não usavam EPIs.

Abstract Objective: To analyze the applicability of musical dynamics for nursing students in respect to palliative care in pediatric oncology. Methods: A qualitative research using the creative and sensitive method was developed in 2016 in the School of Nursing of a federal university in Rio de Janeiro. 'Musical Body' dynamics was used with ten students in the last year of the graduation course with data being submitted to thematic analysis. Results: The dynamics favored reflection on coping strategies and improving the care provided to children besides reinvigorating the healthcare team. It prompted reflection on empathy, unpreparedness and lack of opportunity to experience pediatric oncology and mourning and was thus an approximation to practical experience. Participants emphasized the possibility of using musical dynamics in the professional setting. Conclusion: Musical dynamics proved to be a useful strategy to sensitize nursing students about palliative care in pediatric oncology.

In termites, substrate-borne vibrations play an important role in communication among nestmates. The adaptive significance of such an ability has led to an ever-increasing number of studies aimed at improving knowledge on vibroacoustic communication in these insects. Such studies are commonly carried out in laboratory arenas consisting of Petri dishes made of plastic or glass. However, the rigidness of such materials may limit the transmission of vibrational waves impairing accurate records of the feeble vibrations produced by termites. This is one of the reasons why such experiments must be carried out under strictly controlled conditions, using extremely sensitive equipment, usually connected to amplifiers. If, instead, arenas bear a flexible floor (hence simulating a tympanum), vibrations might not be dampened or even easily amplified, thereby overcoming the need for such a specialized setup. Here we test such a hypothesis, using an accelerometer to measure and record vibrations whose intensity was tailored to mimic the feeble vibrations of a small termite species, Constrictotermes cyphergaster. Results support the notion that tympanic arenas portray such vibrations far more accurately than arenas made of plastic or glass. We hence recommend this type of arena as a cheap, albeit accurate, alternative in studies of vibroacoustic behaviors of termites and other insects of comparable size, especially in situations where noise is minimally controlled. These arenas, then, can be useful in conducting such studies just after termite collection in remote regions where well-equipped labs are not available. In doing so, we minimize the stress involved in transporting termites over long distances.

This study described the use of household insecticides in Picos (Piauí, Brazil), identify which are the most used types of insecticides and describes the incidence of poisoning and environmental awareness of the population. After home visits (n = 700), it was seen that the majority of respondents was represented by women (75%), with 31-55 years-old (49%), incomplete primary education (38.1%) and income between 1-2 earnings (64%). Most homes have between 1-3 residents (48%), 85% of the persons use insecticides mainly chosen in TV and radio and only 54% of them read the label before employing the product. The most used form of presentation is the aerosol (70.7%). Majority (79%) recognizes that insecticides are harmful to health, but 74% do not use any Personal Protective Equipment (PPE). Symptoms of toxicity were reported by 27% of people interviewed. Two women reported irritation, dizziness and respiratory problems and need for medical intervention and hospitalization. All interviewed discard the package as regular trash, since Picos does not has selective collection. In conclusion, most people use insecticides, know about the individual and collective risks to which they are exposed but do not use PPE, though they believe insecticides are toxic. It was noted that acquisition of knowledge does not necessarily result in behavioral changes, since learning does not translate into appropriate preventive attitudes and practices, emphasizing the requirement for awareness campaigns about toxicity and environmental risks, preparation of professionals and surveillance policy against indiscriminate sale.

BACKGROUND/OBJECTIVES: High intakes of unprocessed red or processed meat may increase the risk of stroke. We aimed to examine the association between unprocessed red meat, processed meat and total red meat consumption and risk of total stroke and ischaemic stroke. SUBJECTS/METHODS: Cox proportional hazards regression analyses were conducted based on the data for 41 020 men and women aged 29-69 years at baseline. RESULTS: During a mean follow-up of 13.8 years, 674 incident cases of stroke (531 ischaemic strokes, 79 haemorrhagic strokes, 42 subarachnoid haemorrhages and 22 mixed or unspecified events) were identified. After multiple adjustment, unprocessed red meat, processed meat and total red meat consumption were not correlated with incidence of total stroke or ischaemic stroke in either men or women. The hazard ratios (HRs) for unprocessed red meat and processed meat and risk of total stroke comparing the highest with the lowest quintiles were, respectively, 0.81 (95% confidence interval (CI) 0.54-1.21; P-trend =0.15) and 0.92 (95% CI 0.64-1.32; P-trend = 0.82) in men and 1.21 (95% CI 0.79-1.85; P-trend =0.10) and 0.81 (95% CI 0.51-1.27; P-trend =0.17) in women. The HRs for unprocessed red meat and processed meat and risk of ischaemic stroke were, respectively, 0.80 (95% CI 0.51-1.25; Ptrend = 0.51) and 0.86 (95% CI 0.57-1.29; P-trend =0.77) in men and 1.24 (95% CI 0.74-2.05; P-trend = 0.13) and 0.82 (95% CI 0.471.42; P-trend = 0.31) in women. The somatic cell count (SCC) of refrigerated raw milk reflects the occurrence of mastitis in the herd and the management of rural property. Therefore, this study aimed to evaluate the SCC of the refrigerated raw milk and its determinant factors in dairy properties of Viçosa, State of Minas Gerais. Monthly, milk samples were taken from 44 cooling tanks during 2012. A structured questionnaire was applied to evaluate the production and management characteristics of the herds. There was a significant variation (p < 0.05) in the mean SCC of the milk, which was lower from June to August. Higher mean SCC and percentages of samples above the limit set by the legislation were observed in months of higher rainfall and ambient temperature (p < 0.05). Mechanical milking, lower milk yield, productivity, inadequate milking procedures, equipment hygiene, and water quality were all factors that influenced (p < 0.05) the SCC of raw refrigerated milk. Milker training is required for production of low SCC milk.

Here we present data on distinct stimuli as elicitors of substrate-borne vibrations performed by groups of termites belonging to the species Constrictotermes cyphergaster (Blattodea: Isoptera: Termitidae: Nasutitermitinae). The study consisted of assays where termite workers and soldiers were exposed to different airborne stimuli and the vibrations thereby elicited were captured by an accelerometer attached under the floor of the arena in which the termites were confined. A video camera was also used as a visual complement. The data provided here contribute to fill a gap currently existing in published datasets on termite communication.

Objective: To identify the epidemiological profile of older patients with proximal femoral fractures treated at the Hospital Regional de Cotia, SP - Brazil, and describe the nutritional status of these patients. Methods: Data were obtained from the electronic patient medical records from August 2020 to April 2021. The variables studied were age, gender, ethnicity, weight and height (for BMI calculation), and presence of comorbidities. Fracture circumstances were also assessed, including trauma mechanism, anatomical location, and treatment. Moreover, the nutritional profile of patients was assessed using the Mini Nutritional Assessment (MAN). Results: Most patients were white women, with a mean age of 80 years and an average BMI of 23.55 kg/m2. Almost all patients suffered the fracture at home and the most common comorbidities were systemic arterial hypertension, diabetes mellitus, and Alzheimer’s disease. Most patients were considered to be malnourished or at risk of malnutrition. Conclusion: The nutritional status of older adults seems to be directly related to the risk of proximal fractures of the femur. Level of Evidence II, Retrospective Study. RESUMO Objetivo: Traçar o perfil epidemiológico dos pacientes idosos com fratura de fêmur proximal atendidos no Hospital Regional de Cotia - SP, além de descrever o estado nutricional desses pacientes. Método: As informações para confecção deste trabalho foram obtidas a partir dos prontuários eletrônicos dos pacientes atendidos no período agosto de 2020 a abril de 2021. As variáveis estudadas foram: idade, sexo, etnia, peso e altura (para cálculo do índice de massa corporal - IMC) e presença de comorbidades. Também foram avaliadas as circunstâncias da fratura, tais como mecanismo do trauma, localização anatômica e tratamento instituído. Ainda, avaliou-se o perfil nutricional dos pacientes por meio da Mini Avaliação Nutricional (Mini-MAN). Resultados: Predominaram pacientes do sexo feminino, brancas, com média de 80 anos de idade e IMC médio de 23,55 kg/m 2 . Quase todos os pacientes se acidentaram em casa, e as comorbidades mais observadas foram hipertensão arterial sistêmica, diabetes mellitus e doença de Alzheimer. A grande maioria dos pacientes foi considerada desnutrida ou em risco de desnutrição. Conclusão: O estado nutricional dos idosos parece estar diretamente relacionado ao risco de fraturas proximais do fêmur. Nível de Evidência II, Estudo Retrospectivo.

BACKGROUND/OBJECTIVES: Diets high in saturated and trans fat and low in unsaturated fat may increase type 2 diabetes (T2D) risk, but studies on foods high in fat per unit weight are sparse. We assessed whether the intake of vegetable oil, butter, margarine, nuts and seeds and cakes and cookies is related to incident T2D. SUBJECTS/METHODS: A case-cohort study was conducted, nested within eight countries of the European Prospective Investigation into Cancer (EPIC), with 12 403 incident T2D cases and a subcohort of 16 835 people, identified from a cohort of 340 234 people. Diet was assessed at baseline (1991-1999) by country-specific questionnaires. Country-specific hazard ratios (HRs) across four categories of fatty foods (nonconsumers and tertiles among consumers) were combined with random-effects meta-analysis. RESULTS: After adjustment not including body mass index (BMI), nonconsumers of butter, nuts and seeds and cakes and cookies were at higher T2D risk compared with the middle tertile of consumption. Among consumers, cakes and cookies were inversely related to T2D (HRs across increasing tertiles 1.14, 1.00 and 0.92, respectively; P-trend < 0.0001). All these associations attenuated upon adjustment for BMI, except the higher risk of nonconsumers of cakes and cookies (HR 1.57). Higher consumption of margarine became positively associated after BMI adjustment (HRs across increasing consumption tertiles: 0.93, 1.00 and 1.12; P-trend 0.03). Within consumers, vegetable oil, butter and nuts and seeds were unrelated to T2D. CONCLUSIONS: Fatty foods were generally not associated with T2D, apart from weak positive association for margarine. The higher risk among nonconsumers of cakes and cookies needs further explanation. Social bees make use of natural or animal-built structures to protect their colonies from environmental stressors. Here, particularly attractive shelters are active termite nests because they provide a stable climatic environment for inquilines. Several social bee species form obligatory associations with termites, among these the stingless bee Partamona seridoensis (Apidae, Meliponini), whose distribution is limited to the Tropical Dry Forest in the Brazilian Northeast. So far, colonies of this meliponine species have been found mainly in arboreal nests of the termite Constrictotermes cyphergaster, which suggests a tight relationship between these two social insect species. The present study was conducted in an area of the Tropical Dry Forest in the Brazilian State of Rio Grande do Norte, where P. seridoensis naturally occurs albeit the absence of C. cyphergaster. We registered 14 colonies of P. seridoensis, all occupying active arboreal nests of termites of the genus Microcerotermes. The only other termites with arboreal nests present in the study area, Nasutitermes corniger, never housed P. seridoensis. This selective preference of the bees for Microcerotermes nests might be due to differences between termites concerning defense mechanisms or concerning thermal stability within the nests.

Nomuraea rileyi represents an important natural control agent of Anticarsia gemmatalis preventing populations from reaching economic threshold levels in soybean. During the processes of host infection, entomopathogenic fungi produce extracellular proteases, which degrade the host cuticle and are suggested to be virulence determinants. It was examined the production of subtilisin-like (Pr1) and trypsin-like (Pr2) proteases in two strains (NR458 and CG434) of N. rileyi and its possible role in the process of pathogenicity to this caterpillar. Fungal growth was performed in a mineral medium containing nitrate, and supplemented with the cuticle or exuviae from A. gemmatalis, or with the non-cuticular substrate casein. In medium containing nitrate as sole nitrogen source, no detectable Pr1-like activity occurred in the culture supernatants of the two fungal strains. However, both strains of N. rileyi produced Pr1-like protease in all medium amended with exogenous nitrogen source, and it was highly expressed in the presence of insect cuticle. Pr2-like activity was significantly inferior to Pr1-like activity and it was detected only in some of the media culture and incubation periods tested. In the NR458 culture supernatant the highest activity was observed in medium containing nitrate as nitrogen source. Correlation analysis between the percentage of A. gemmatalis mortality in bioassays and Pr1-like protease activity of strain NR458 suggests a positive correlation for these variables.O objetivo deste estudo foi avaliar a produção de proteases dos tipos subtilisina [Pr1] e tripsina [Pr2] por duas linhagens do fungo entomopatogênico Nomuraea rileyi e a correlação entre a atividade de Pr1 e a patogenicidade contra Anticarsia gemmatalis. O crescimento do fungo foi realizado em meio mínimo contendo nitrato e suplementado com a cutícula ou exúvia de A. gemmatalis, ou com substrato não cuticular caseína. Em meio contendo nitrato, nenhuma atividade de Pr1 foi detectada nos sobrenadantes das culturas. Entretanto, as duas linhagens de N. rileyi produziram Pr1 em meio suplementado com fonte exógena de nitrogênio, e alta atividade foi verificada na presença da cutícula do inseto. A atividade de Pr2 foi inferior à atividade de Pr1. A análise de correlação entre a atividade de Pr1 da linhagem NR458 e mortalidade de A. gemmatalis sugere uma correlação positiva para essas variáveis. A avaliação da atividade de enzimas em diferentes condições pode ajudar na compreensão do processo infeccioso de N. rileyi em A. gemmatalis.

Introdução: O Plasmodium é o agente etiológico da malária, e a infecção no hospedeiro vertebrado ocorre envolvendo diferentes células e depende de uma complexidade de eventos celulares e bioquímicos no hospedeiro para realizar o ciclo de desenvolvimento.Os casos de malária reportado pela Organização Mundial da Saúde (OMS), ocorrem em países distribuídos ao longo da faixa intertropical do globo terrestre, são áreas com condições favoráveis à ocorrência e desenvolvimento do vetor Anopheles spp. No ano de 2019 ocorreram cerca de 229 milhões de novos casos de malária e 409 mil mortes, crianças menores de 5 anos são mais vulneráveis, representando cerca de 67% de todas as mortes no ano de 2019. Objetivo: Em vista disso, O presente estudo tem finalidade descritiva e através da análise de artigos científicos, avaliar alterações da morfologia hepática e detecção de fibrose nos hospedeiros infectados pelo Plasmodium submetidos ou não aos tratamentos com antimalaricos, observar o ciclo de vida do Plasmodium, como ocorre a infecção no humano da doença malárica, as possíveis alterações morfológicas e fisiopatológicas causadas pela hemozoína no tecido hepático e presença de fibrose. Metodologia: A metodologia utilizada é baseada em artigos científicos de revisão exploratória. Foram selecionados artigos referentes à malária, nos idiomas português e inglês. São artigos levantados por meio de consultas através dos sites acadêmicos PudMed, SciElo, Science Direct, Malaria Journal e WHO, com período de abrangência de 2015 a 2021. Resultados: As alterações fisiopatológicas no fígado do hospedeiro infectado estão relacionadas com o processo inflamatório pela hemozoína e necessitam de estudos para a compreensão da relação parasita x hospedeiro e do potencial comprometimento tecidual hepático. É observado presença de fibrose hepática, células mortas, neoplasias e quadro inflamatório tecidual hepático, desencadeado pela hemozoína, quando não há tratamento para regressão da fibrose hepática e a lesão celular continua sendo persistente, ocorre o desenvolvimento de cirrose, que pode evoluir para carcinoma hepatocelular. Conclusão: A observação das alterações hepática durante e após a infecção da malária é essencial para entender os potenciais danos teciduais hepáticos, avaliar os tratamentos disponíveis e ações preconizadas pela OMS para redução da infecção e óbitos causados pela malária.

Devido ao isolamento causado pela pandemia do novo coronavírus, as instituições de ensino superior tiveram que moldar suas atividades, adaptando-as para o formato remoto. O presente trabalho tem como objetivo relatar a percepção de discentes da graduação em Fisioterapia e Nutrição sobre as avaliações virtuais em mulheres na fase do climatério. Trata-se de um estudo descritivo, com recorte pertencente a um projeto de extensão e pesquisa. Participaram das ações uma docente efetiva da área de Fisioterapia em Saúde da mulher, dois discentes do curso de Fisioterapia, dois discentes do curso de Nutrição e 40 mulheres climatéricas. Avaliações virtuais foram realizadas por meio das plataformas de comunicação Google Meet e WhatsApp. A estratégia de continuidade do projeto foi extremamente importante para a experiência acadêmica por simplificar a linguagem científica e ampliar os conceitos de educação em saúde, devido à troca de informações estabelecida entre o avaliador e a participante.

Durante a pandemia do novo coronavírus (SARS-CoV-2), as mulheres apresentaram dificuldade de acesso à saúde mediante ao risco de transmissão do vírus. Devido a essa necessidade, foi criado o projeto de extensão “Mulher em Evidência – Conhecimento além dos muros do Trairi" que usou o Instagram para disponibilizar conteúdos relacionados à Saúde da Mulher, com embasamento cientifico e linguagem acessível. Foram publicados 72 conteúdos, alcançando 27.325 pessoas e o tema mais curtido foi “O tabu da estética íntima” com 236 curtidas. Inicialmente os extensionistas relataram dificuldade para traduzir o conhecimento científico, entretanto este projeto de extensão alcançou o objetivo de disseminar ciência de qualidade e promover saúde para mulheres. To assess whether an increase in mean arterial pressure in patients with septic shock and previous systemic arterial hypertension changes microcirculatory and systemic hemodynamic variables compared with patients without arterial hypertension (control).Prospective, nonblinded, interventional study.Three ICUs in two teaching hospitals.After informed consent, we included patients older than 18 years with septic shock for at least 6 hours, sedated, and under mechanical ventilation. We paired patients with and without arterial hypertension by age.After obtaining systemic and microcirculation baseline hemodynamic variables (time 0), we increased noradrenaline dose to elevate mean arterial pressure up to 85-90 mm Hg before collecting a new set of measurements (time 1).We included 40 patients (20 in each group). There was no significant difference in age between the groups. After the rise in mean arterial pressure, there was a significant increase in cardiac index and a slight but significant reduction in lactate in both groups. We observed a significant improvement in the proportion of perfused vessels (control: 57.2 ± 14% to 66 ± 14.8%; arterial hypertension: 61.4 ± 12.3% to 70.8 ± 7.1%; groups: p = 0.29; T0 and T1: p0.001; group and time interaction: p = 0.85); perfused vessels density (control: 15.6 ± 4 mm/mm to 18.6 ± 4.5 mm/mm; arterial hypertension: 16.4 ± 3.5 mm/mm to 19.1 ± 3 mm/mm; groups: p = 0.51; T0 and T1: p0.001; group and time interaction: p = 0.70), and microcirculatory flow index (control: 2.1 ± 0.6 to 2.4 ± 0.6; arterial hypertension: 2.1 ± 0.5 to 2.6 ± 0.2; groups: p = 0.71; T0 and T1: p = 0.002; group and time interaction: p = 0.45) in both groups.Increasing mean arterial pressure with noradrenaline in septic shock patients improves density and flow in small vessels of sublingual microcirculation. However, this improvement occurs both in patients with previous arterial hypertension and in those without arterial hypertension.

Background: Sulforaphane (SF, 1-isothiocyanato-4-(methyl-sulfinyl)-butane) is found in broccoli, cabbage and cauliflower. Methods: we performed a critical review on the antioxidative, chemopreventive and antitumor effects of SF from cruciferous vegetables against prostate cancers and molecular pathways. For a complete and reliable review, primary and secondary resources were used, including original and review articles, books and government documents published until March 2018. Articles that are in duplicity and disconnected are not considered for review. SF is derived from glucoraphanin (4-methyl-sulfinyl-butyl-glucosinate), being one of the most commonly found isothiocyanates in vegetables from Brassica spp., especially in broccoli samples. In vitro studies indicate that SF induces apoptosis in a dependent or non-dependent method of androgens by transcription of tumor suppressor genes, oxidation response and higher expression of phase II enzymes in prostate cancer cells. Sulforaphane also decreases transcription of the nuclear factor kB and antiapoptotic proteins, expression of cyclin D2 and survivin and DNA synthesis, increases Nrf2 gene activity, interferes with genome compacting by inhibition of histone deacetylases and disrupts Hsp90 complexes, which cause cell cycle arrest, mitosis interruption, activation of caspases and mitochondria depolarization. Conclusion: SF and cruciferous vegetables play antioxidative and chemopreventive role, delaying or blocking in vivo carcinogenesis, causing biochemical and epigenetic changes, preventing, delaying, or reversing preneoplastic or advanced prostate lesions, and frequently activating tumor cell death by intrinsic methods of apoptosis. These outcomes encourage the consumption of Brassica specimens, which could be easily achieved by the incorporation of food and vegetables rich in cruciferous isothiocyanates in the diet.

INTRODUCTION: Urethral prolapse can be characterized by a circular protrusion of the urethra distally to the external urethral meatus, and its diagnosis is entirely clinical. The etiology is still not completely defined, but there are some hypotheses of clinical causes. Urethral prolapse is also one of the findings in child sexual abuse, however, as a rare condition, health professionals have little experience in diagnosing and distinguish it from other events that occur in childhood. OBJECTIVE: To demonstrate the importance of the differential diagnosis of clinical conditions that can also characterize suspected child sexual abuse. MATERIALS AND METHODS: This is a descriptive study, which presents the report of three cases of urethral prolapse, in which suspicions of sexual abuse were raised in a medium-sized city in Minas Gerais. DISCUSSION: Urethral prolapse is an uncommon condition, but it should be considered in the differential diagnosis in cases of urogenital hemorrhage in prepubertal children. CONCLUSION: It is essential to correctly characterize the urethral prolapse, its probable etiology and, in the case of suspected sexual abuse, to be investigated by the expert medical authority.

42. Study of genes\' expression involved in the NF-kB pathway in late-onset neonatal sepsis: potential value in diagnosis and prognosis

- Abstract
Os avanços na terapia intensiva neonatal proporcionaram uma melhora na sobrevida dos pacientes, entretanto a sepse ainda constitui uma das principais causas de mortalidade em recém-nascidos (RN). Até o momento, nenhum marcador biológico único apresentou vantagem significativa sobre outros para o diagnóstico da sepse neonatal. Deste modo, a disponibilização de um painel de biomarcadores gênicos, que possa diferenciar mais precocemente RN infectados dos não infectados e, ainda, identificar diferentes padrões de resposta causados por agentes etiológicos distintos, pode ter impacto significativo na morbimortalidade da sepse neonatal, contribuindo para a redução dos custos hospitalares e, ainda, diferenciar um bom ou mau prognóstico. O objetivo desse estudo é descrever a expressão de genes relacionados com a via de sinalização do fator nuclear Kappa B (NF-kB) em RN com sepse tardia clínica e comprovadamente causada por bactérias Gram-positivas (G+) e Gram-negativas (G-), em três momentos diferentes: no dia do diagnóstico (dia 0), e após 3 e 7 dias de antibioticoterapia. Foram coletadas 32 amostras de sangue de RN distribuídas em grupos de acordo com o agente etiológico: com sepse causada por bactérias G+ (n = 12); causada por bactérias G- (n = 9); RN com sepse clínica (n = 11). O grupo controle foi composto por 21 amostras de RN sem sinais de infecção. A expressão gênica foi avaliada utilizando-se a metodologia RNA-Seq por meio de sequenciamento paralelo em larga escala de um painel de genes relacionados à via de sinalização do NF-kB, utilizando o kit TruSeq Targeted RNA Custom (Illumina Inc., San Diego, CA, USA), na plataforma Illumina MiSeq. Na análise, os fragmentos foram alinhados à referência hg38 do genoma humano utilizando o software STAR (PMID: 23104886). Para a normalização e análise da expressão diferencial, foi utilizado o software DESeq2 (PMID: 25516281). A avaliação do perfil de expressão gênica revelou 11 genes diferencialmente expressos (DEG) no grupo de sepse por G+, 22 genes no grupo G- e 12 genes no grupo de sepse clínica em relação aos controles. Os termos de ontologia gênica (GO) significantemente enriquecidos para esses genes estão associados à atividade de citocinas e ligação aos seus receptores, regulação da via de sinalização do NF-B e lúmen do grânulo terciário. Em todos os grupos de sepse observamos a hiperexpressão do gene MMP9 e a hipoexpressão do CXCL2 e somente nos grupos de sepse comprovada por bactérias G+ e G-, a hiperexpressão do gene MBL2. No grupo G+, o gene MMP9 apresentou hipoexpressão no dia 0 comparado aos dias 3 e 7 e IL12B apresentou hipoexpressão no dia 0 em relação ao dia 7. No grupo G-, foi obervada a hiperexpressão do gene TREM1 no dia 0 comparado aos dias 3 e 7 e também a hipoexpressão do IL12B no dia 0 em relação ao dia 7. No grupo de sepse clinica, foi observada hiperexpressão do gene IRF1 no dia 0 comparado ao dia 3 e hipoexpressão do FOXP3 no dia 3 comparado ao dia 7 pós-diagnóstico. Os DEG MMP9, MBL2, IL-12B, ESM1 e TREM1 foram selecionados e validados funcionalmente por meio da quantificação da proteína por ensaio imunoenzimático. Os resultados revelaram que os genes MMP9 e MBL2 são importantes para identificação dos casos de sepse comprovada e no grupo de sepse clínica, foi observado um perfil de expressão diferente, sem alteração na expressão do gene MBL2, porém com hiperexpressão do IRF1, sugerindo uma possível infecção viral ou bacteriana não identificada. O gene TREM1 constitui um possível preditor de mau prognóstico para a sepse neonatal tardia. Os genes diferencialmente expressos MMP9, MBL2 e TREM1 codificam proteínas importantes, comprovadamente envolvidas na patogênese da sepse neonatal e apresentam potencial para serem utilizados como biomarcadores gênicos The advances in neonatal intensive care provided an improvement in patient survival, however sepsis still constitutes one of the main causes of mortality in the newborn (NB). So far, no single biological marker showed significant advantage over others for the diagnosis of neonatal sepsis. Thus, the availability of a panel of gene biomarkers, which could differentiate infected from non-infected NBs earlier and, which could also identify different response patterns caused by different etiologic agents, can have a significant impact on the morbidity and mortality of neonatal sepsis, contributing to the reduction of hospital costs and, also, differentiate a good or poor prognosis. The aim of this study is to describe gene expression related to the nuclear factor-Kappa B (NF-kB) signaling pathway in NBs with clinical and culture-proven late-onset sepsis caused by Gram-positive (G+) and Gram-negative (G-) bacteria, at three different moments: on the day of diagnosis (day 0), and after 3 and 7 days of antibiotic therapy. Thirty-two blood samples were collected from NBs distributed in groups according to the etiologic agent: sepsis caused by G+ bacteria (n = 12); caused by G- bacteria (n = 9); NBs with clinical sepsis (n = 11). The control group was composed of 21 NB samples without signs of infection. Gene expression was evaluated using the RNA-seq methodology through large-scale parallel sequencing of a gene panel related to the NF-B signaling pathway, using the TruSeq Targeted RNA Custom Kit (Illumina Inc., San Diego, CA, USA), on the Illumina MiSeq platform. In the analysis, the fragments were aligned to the hg38 reference of human genome using the STAR software (PMID: 23104886). For normalization and analysis of differential expression, the DESeq2 software (PMID: 25516281) was used. The evaluation of the gene expression profile revealed 11 differentially expressed genes (DEG) in the G+ sepsis group, 22 genes in the G- group and 12 DEG genes in the clinical sepsis group compared to controls. The terms of gene ontology (GO) significantly enriched for these genes are associated to cytokine activity and binding to its receptors, regulation of the NF-kB signaling pathway and tertiary granule lumen. In all sepsis groups, we observed MMP9 gene up-regulation and CXCL2 down-regulation and solely in the culture-proven G+ and G- groups, the up-regulation of MBL2, as compared to controls. In the G+ group, MMP9 gene was down-regulated on day 0 as compared to days 3 and 7 and IL12B was down-regulated on day 0 relative to day 7. In G- group, TREM1 gene was up-regulated on day 0 compared to days 3 and 7 and IL12B was also down-regulated on day 0 relative to day 7. In the clinical sepsis group, up-regulation of IRF1 gene was observed on day 0 compared to day 3 and down-regulation of FOXP3 on day 3 compared to day 7 post-diagnosis. The DEG MMP9, MBL2, IL-12B, ESM1 and TREM1 were selected and functionally validated by protein quantification by immunoenzymatic assay. The results revealed that MMP9 and MBL2 genes are important for the identification of culture-proven sepsis cases and in the clinical sepsis group, a different expression profile was observed, without alteration in the MBL2 gene expression, however with IRF1 over expression, suggesting a possible unidentified viral or bacterial infection. TREM1 gene is a possible predictor of poor outcome for late neonatal sepsis. The differentially expressed genes MMP9, MBL2 and TREM1 encode important proteins, proven to be involved in the pathogenesis of neonatal sepsis and have the potential to be used as gene biomarkers

A escolha de uma carreira é sempre um desafio para os jovens universitários, uma decisão que tem vindo a ser dificultada devido às constantes alterações que caracterizam o atual mercado de trabalho e ao aumento da oferta de cursos superiores. Deste modo, é importante identificar os fatores que promovem uma melhor adaptação ao contexto universitário e/ou profissional. A presente investigação insere-se neste contexto e visa analisar em que medida a personalidade e as competências têm impacto no desenvolvimento de carreira dos estudantes do Ensino Superior. A amostra é constituída por 551 estudantes que frequentam o 1.º e o 2.º ciclo do Ensino Superior, e cujas idades variam entre os 18 e os 45 anos. O estudo foi operacionalizado através de uma metodologia quantitativa de carácter hipotético-dedutivo que assenta em três modelos teóricos: o Big Five (personalidade), o RIASEC (competências) e o modelo das crenças de autoeficácia no desenvolvimento de carreira. Os dados foram recolhidos através do Big Five Inventory -10, do Self-Directed Search, do Career Development Self-Efficacy Inventory e de um conjunto de questões de natureza sociodemográfica. Os resultados obtidos permitem-nos constatar que a personalidade e as competências se correlacionam positivamente e que ambas têm um impacto significativo no desenvolvimento de carreira. Constatou-se, ainda, que a Conscienciosidade, do modelo Big Five, e o perfil Social de competências, do modelo RIASEC, são as dimensões que mais influenciam o desenvolvimento de carreira dos alunos do Ensino Superior.

As enteroparasitoses constituem um grave problema de saúde pública no Brasil e no mundo, sendo causadas por parasitas como os helmintos que provocam danos nutricionais e podem levar o hospedeiro ao óbito. Os fármacos utilizados no tratamento de helmintoses podem agir de diversas maneiras, contudo, as opções desses medicamentos no mercado ainda encontram-se limitadas. Sabe-se que o Ascaridol tem demonstrado grande potencial farmacológico em terapias anti-bacterianas, anti-fúngicas, anti-viral e anti-neoplásica. Assim, este trabalho tem como objetivo avaliar a atividade anti-helmítica do Ascaridol através de um estudo in silico. Como metodologia utilizou-se o software gratuito Pass Online® para análise da probabilidade de atividade da molécula. O Ascaridol demonstrou um bom resultado para probabilidade de ativação (Pa) em relação aos valores da probabilidade de inativação (Pi) nas atividades anti-helmíntica nematoide e anti-helmíntica fasciola. Conclui-se que esse monoterpeno apresentou um elevado potencial como anti-helmítico, entretanto, ainda são necessários estudos in vitro e in vivo em maior diversidade de espécies helmínticas e em testes toxicológicos com intuito de elucidar sua segurança e índice terapêutico.
