Moreover, a problem of increasing importance is the emergence of drug-resistant CMV infection. Here, we describe the first case of off-label use of letermovir (LMV) as preemptive antiviral therapy, in a pediatric allogeneic peripheral blood stem cell transplant recipient with ganciclovir-resistant CMV infection who was intolerant to foscarnet and unable to achieve viral clearance after seven doses of cidofovir. After the administration of LMV, a gradual reduction in viral load was observed and within 6 weeks of LMV treatment, after more than 6 months of positive CMV-DNAemia, the patient cleared the infection. No adverse effects associated with LMV were observed during treatment. In this pediatric study case, the off-label use of LMV for the treatment of CMV infection has been well tolerated and proved to be effective in leading to the suppression of viral replication.Keywords: pediatric allogeneic peripheral blood stem cell transplant, GCV-resistant CMV, intolerance to FOS, off-label letermovir use

Abstract

Hospital-acquired pneumonia (HAP) and ventilation-associated pneumonia (VAP) are challenging clinical conditions due to the challenging tissue penetrability of the lung. This study aims to evaluate the potential role of fosfomycin (FOS) associated with ceftazidime/avibactam (CZA) in improving the outcome in this setting. We performed a retrospective study including people with HAP or VAP treated with CZA or CZA+FOS for at least 72 h. Clinical data were collected from the SUSANA study, a multicentric cohort to monitor the efficacy and safety of the newer antimicrobial agents. A total of 75 nosocomial pneumonia episodes were included in the analysis. Of these, 34 received CZA alone and 41 in combination with FOS (CZA+FOS). People treated with CZA alone were older, more frequently male, received a prolonged infusion more frequently, and were less frequently affected by carbapenem-resistant infections (p = 0.01, p = 0.06, p < 0.001, p = 0.03, respectively). No difference was found in terms of survival at 28 days from treatment start between CZA and CZA+FOS at the multivariate analysis (HR = 0.32; 95% CI = 0.07–1.39; p = 0.128), while prolonged infusion showed a lower mortality rate at 28 days (HR = 0.34; 95% CI = 0.14–0.96; p = 0.04). Regarding safety, three adverse events (one acute kidney failure, one multiorgan failure, and one urticaria) were reported. Our study found no significant association between combination therapy and mortality. Further investigations, with larger and more homogeneous samples, are needed to evaluate the role of combination therapy in this setting.

Abstract

The thermal behavior of a poly(methylmethacrylate) (PMMA) pigmented with titanium dioxide (TiO2) is studied in both Steady state and transient regimes in the present work. The numerical results of thermal conductivity, based on the finite element method, are compared to theoretical models and experimental measurements, which varies depending on the quenching temperature and pigment content. Time evolution of temperatures during the quenching of the composite is taken into account for different quenching temperatures and different pigment contents. It is noted that the heat exchange becomes slower for a pigment fraction of 0.5%, and the steady state is reached more rapidly for higher pigment content. The AFM image of the PMMA/TiO2 composite with content equal to 3% of titanium dioxide. This demonstrates a good distribution of the particles throughout the matrix, with the individual particles being uniformly dispersed and securely embedded in the polymer matrix, thereby avoiding any clustering. An improvement in heat exchange is observed in the composite with a high content of titanium dioxide. This improvement is attributed to the increase in the thermal conductivity of the PMMA/TiO2 composite. [ABSTRACT FROM AUTHOR]

Abstract

The potential distribution of one the most venomous snake species of Iran, the endemic Macrovipera razii, has been studied. A total of 54 occurrence records with four informative environmental variables, including temperature seasonality (bio4), annual precipitation (bio12), precipitation seasonality (bio15), and precipitation of driest quarter (bio17) for present and future periods were used for identifying suitable habitat. Annual precipitation (bio12) was the most important driving factor with 48 % of permutation importance, followed by precipitation seasonality (bio15) with 42.7 % of contribution to the modeling. According to our results and literature, the most suitable habitats of the species are in highland and mountainous regions. Western and southwestern regions of Iran are highly suitable areas for M. razii according to current conditions, which are highly compatible with its known distribution, with the exception of the predicted suitability in a small patchy region of northeastern Iran where another related taxon, Macrovipera lebetina subsp. cernovi, is present. Future predictions reveal a similar potential distribution. Further research on M. razii is crucial and should encompass various aspects such as the analysis of venom components, field studies, and comprehensive molecular investigations. These efforts are necessary to determine the taxonomic status of M. razii, understand its distribution patterns, and develop effective anti-venom treatments.

Abstract

The Montarbu (CE Sardinia) is the second highest carbonate massif of Sardinia. This area mainly consists of a group of limestone mountain outcrops named “tonneri” or “tacchi”, with a great variety of micro-environmental and topographic conditions that host a high floristic richness. The main aim of this work is to present an updated checklist of the vascular flora of Montarbu to assess its conservation priorities. Based on several field surveys carried out from 2012 to 2023 and integrated by bibliographic and herbaria analyses, the updated checklist of the vascular flora is presented here. The flora amounts to 874 taxa, belonging to 94 families and 425 genera. Regarding the endemic component, we found 126 taxa, of which 44 are exclusive to Sardinia and 40 are shared with Corsica. The alien taxa are 33, but it appears worrying that 20 are recorded for the first time during the last two decades. The analysis of biologic and chorologic data highlighted the peculiarities of this territory and its biogeographic autonomy. Due to the relatively high number of endemics exclusive to Montarbu and its geological and geomorphological peculiarities, despite the already implemented initiatives, we suggest further activities supporting the conservation of this area. Abstract

This work presents the chemical modification of Spanish broom flour (SBF), and the study of SBF loading and surface treatment on the performances of polypropylene (PP) biocomposites. In order to enhance the interfacial interactions between the PP matrix and the SBF, two types of chemical treatments were used: 2 wt% of sodium hydroxide (NaOH) for different times (8, 24 and 48 h) and 5 wt% of vinyltrimethoxysilane (VTMS), respectively. Different techniques for characterization such as the melting flow index (MFI), X-ray diffraction, transient plane source (TPS) and water absorption were used. The experiment results showed a decrease of the MFI with increasing of modified SBF content, independently of the type of the chemical treatment. Moreover, this decrease became significant in the biocomposites containing SBF-VTMS. The X-ray patterns showed that surface treatment of SBF could improve their crystallinity and crystallite sizes. The TPS measurements illustrates that the thermal conductivity of the biocomposites decreases with 10 wt% of modified SBF loading. Higher content than 20 wt% of SBF, improved the thermal conductivity of the biocomposites. Meanwhile, the lowest values were found when the VTMS is used. Besides, it was accompanied by a decrease in absorptivity due to the better interfacial adhesion SBF-PP.

Abstract

The storage of thermal energy in phase change materials (PCMs) has found wide applications that enable energy conservation and management. Paraffin is a major PCM with its low cost, wide availability, and relatively high latent heat, yet its low thermal conductivity may become a drawback in high-power applications. In this study, composites of paraffin were prepared with multiwalled carbon nanotubes and activated carbon by a dispersion technique to overcome these drawbacks. Thermal, chemical, and physical influences of incorporating carbon additives with varying structures in paraffin composites on thermal storage capacity were determined. Results indicated that the thermal conductivities of paraffin-activated carbon composites (PACC) and paraffin multiwalled carbon nanotube composites (PCNC) were improved by a factor of 39.1 and 34.1%, respectively, compared with the conductivity of pure paraffin. As a bonus, the thermal energy storage capacities of PCNCs were enhanced by 9.6%, whereas this remained unchanged for PACCs. © 2016 John Wiley & Sons, Ltd.

Abstract

This work aims to point out the influence of the polymer surface layer on the adhesion of polymer matrix composites. To this end, correlations between surface parameters (chemistry, roughness, wettability, morphology, etc.) resulting from several surface treatments or preparations and single lap shear tests are performed. The results presented here show the usefulness of surface cleaning and the influence of the manufacturing process before the adhesion stage. Moreover, the use of peel ply surface treatment is also evidenced in terms of surface roughening and cleaning. Furthermore, most of the available surface treatments are limited by the cohesive failure occurring under the polymer surface layer at the fiber—polymer interface of the composite material. Hence, laser ablation is proposed as an alternative way to increase adhesion performance when compared to common surface treatments.

Abstract

Significant results on steady state control of the plasma parameters have been obtained with RF systems on Tore Supra. A sustained effort is under way to improve power transmission reliability. Developments of sources and antennas are in progress for future operation on Tore Supra with pulse length in the range of several hundreds of seconds.

Abstract

The human genome has been subjected to selective pressures to resist to infectious agents in spite of a heavy segregational load. With this regard, thalassaemia and glucose-6-phosphate dehydrogenase deficiency have been considered an efficient genetic protection against P. falciparum malaria in Sardinia, insular Italy. In this island, some multiple sclerosis (MS)-associated HLA haplotypes have the highest odds ratios in the same highestrate malarious areas of the island. Moreover, tumor necrosis factor (TNF) polymorphisms epidemiologically associated with both MS and malaria are ten-fold more frequent amongst Sardinians compared to other populations worldwide4. A possible association between MS and malaria in this island was never analysed experimentally. We studied the immunological response of mononuclear cells to P. falciparum and the killing effect of macrophages on parasites in Sardinian MS patients and in matched healthy controls (HC).
