Abstract

It is important to prepare middle level teacher candidates with the dispositions, content knowledge, and pedagogy needed to implement justice-oriented practices in middle level curriculum. This article explores using "Hateful Things," a traveling exhibit curated by the Jim Crow Museum of Racist Memorabilia, to integrate issues of stereotypes, biases, and racism into middle grades curriculum. Specific lessons, strategies and resources for integrating the history of the Jim Crow era, stereotypes, and American racism in middle level English Language Arts curriculum are provided.

Abstract

BackgroundAdolescents with chronic kidney disease (CKD) are a unique population with a high prevalence of hypertension. Management of hypertension during the transition from adolescence to adulthood can be challenging given differences in normative blood pressure values in adolescents compared with adults.MethodsIn this retrospective analysis of the Chronic Kidney Disease in Children Cohort Study, we compared pediatric versus adult definitions of ambulatory- and clinic-diagnosed hypertension in their ability to discriminate risk for left ventricular hypertrophy (LVH) and kidney failure using logistic and Cox models, respectively.ResultsOverall, among 363 adolescents included for study, the prevalence of systolic hypertension was 27%, 44%, 12%, and 9% based on pediatric ambulatory, adult ambulatory, pediatric clinic, and adult clinic definitions, respectively. All definitions of hypertension were statistically significantly associated with LVH except for the adult ambulatory definition. Presence of ambulatory hypertension was associated with 2.6 times higher odds of LVH using pediatric definitions (95% CI 1.4-5.1) compared to 1.4 times higher odds using adult definitions (95% CI 0.8-3.0). The c-statistics for discrimination of LVH was statistically significantly higher for the pediatric definition of ambulatory hypertension (c=0.61) compared to the adult ambulatory definition (c=0.54), and the Akaike Information Criterion was lower for the pediatric definition. All definitions were associated with progression to kidney failure.ConclusionOverall, there was not a substantial difference in pediatric versus adult definitions of hypertension in predicting kidney outcomes, but there was slightly better risk discrimination of the risk of LVH with the pediatric definition of ambulatory hypertension.

38. Experimental investigation of the impact of goal-oriented mental imagery on reward perception

Abstract

Aims: Recent studies have shown that mood can bias perceived reward value, with this effect being strongest in individuals with more mood instability. Spontaneous use of mental imagery has been highlighted as an important feature in generating and maintaining mood symptoms in bipolar disorder. We examined whether mental imagery influencing motivation biases perceived reward value during learning, and to what extent effects are modulated by mood symptoms. Method: 50 healthy participants completed a brief, online-based manipulation in which they generated mental images related to goal-attainment and goal-failure with a view to increasing and decreasing motivation, respectively. We quantified the efficacy of this manipulation on mood and motivation, as well as on the perception of reward stimuli encountered in two learning blocks. Participants performed each block under one of the two types of imagery, thus using a within-participants design. To test for bias in perceived reward value, participants were subsequently asked to indicate their preference in pairwise choices between all stimuli encountered. Trait mood instability (HPS), propensity towards imagery (SUIS), and depression symptoms (PHQ-9) were included in analyses to test for modulatory effects on biased preference. Results: Goal-oriented mental imagery effectively impacted subjective motivation, with higher ratings in the goal-attainment imagery block, compared to goal-failure. Depression symptoms, but not mood instability, were observed to have a modulating effect on change in motivational state. The degree to which momentary motivation was impacted by imagery was positively associated with bias in perceived reward value, and further modulated by depression symptoms. Conclusions: Our findings indicate that goal-oriented mental imagery is effective in impacting motivational state in healthy individuals reporting more depression symptoms, and that motivational state in turn modulates reward perception. Insights are offered to aid development of interventions using mental imagery as an emotional and motivational "amplifier" to improve depressed mood.

Abstract

BACKGROUND AND OBJECTIVES: Hypertension is highly prevalent in pediatric kidney transplant recipients and contributes to cardiovascular death and graft loss. Improper blood pressure (BP) measurement limits the ability to control hypertension in this population. Here, we report multicenter efforts from the Improving Renal Outcomes Collaborative (IROC) to standardize and improve appropriate BP measurement in transplant patients. METHODS: Seventeen centers participated in structured quality improvement activities facilitated by IROC, including formal training in quality improvement methods. The primary outcome measure was the proportion of transplant clinic visits with appropriate BP measurement according to published guidelines. Prospective data were analyzed over a 12-week pre-intervention period and a 20-week active intervention period for each center and then aggregated as of the program-specific start date. We used control charts to quantify improvements across IROC centers. We applied thematic analysis to identify patterns and common themes of successful interventions. RESULTS: We analyzed data from 5392 clinic visits. At baseline, BP was measured and documented appropriately at 11% of visits. Center-specific interventions for improving BP measurement included educating clinic staff, assigning specific team member roles, and creating BP tracking tools and alerts. Appropriate BP measurement improved throughout the 20-week active intervention period to 78% of visits. CONCLUSIONS: We standardized appropriate BP measurement across 17 pediatric transplant centers using the infrastructure of the IROC learning health system and substantially improved the rate of appropriate measurement over 20 weeks. Accurate BP assessment will allow further interventions to reduce complications of hypertension in pediatric kidney transplant recipients.

Abstract

BACKGROUND: We studied whether increased systolic blood pressure (SBP), as determined by auscultatory SBP, ambulatory SBP, and the number of cardiovascular health risk indicators, are associated with neurocognition in adolescents. METHODS: This cross-sectional study included 365 adolescents (mean age, 15.5 years) from 6 academic medical centers in the United States. The sample was 59.5% male, 52.6% White, with 23.9% of the caregivers having less than or equal to a high school degree. Primary exposures included the following: auscultatory SBP, ambulatory SBP, and the number of cardiovascular risk factors. Neurocognitive outcomes comprised nonverbal IQ, attention, and parent ratings of executive functions. RESULTS: After examining the models for the effects of targeted covariates (eg, maternal education), higher auscultatory SBP was associated with lower nonverbal IQ (β=−1.39; P <0.001) and verbal attention (β=−2.39; P <0.05); higher ambulatory 24 hours. SBP (β=−21.39; P <0.05) and wake SBP (β=−21.62; P <0.05) were related to verbal attention; and all 3 ambulatory blood pressure measures were related to sustained attention accounting for small to medium amounts of variance (adjusted R2=0.08–0.09). Higher ambulatory blood pressure sleep SBP also was significantly associated with parent ratings of behavior regulation (β=12.61; P <0.05). These associations remained stable after a sensitivity analysis removed cases with hypertension. Number of cardiovascular risk factors performed similarly, with more risk factors being associated with lower nonverbal IQ (β=−1.35; P <0.01), verbal attention (β=−1.23; P <0.01), and all parent ratings of executive functions. CONCLUSIONS: Elevated SBP, even below the hypertension range, and general cardiovascular health are associated with neurocognitive outcomes in adolescents.
